Randomized clinical trials have not shown an additional clinical benefit of renal artery stent placement over optimal medical therapy alone. However, studies of renal artery stent placement have not examined the relationship of albuminuria and treatment group outcomes. The CORAL study (Cardiovascular Outcomes in Renal Atherosclerotic Lesions) is a prospective clinical trial of 947 participants with atherosclerotic renal artery stenosis randomized to optimal medical therapy with or without renal artery stent which showed no treatment differences (3(5.8% and 35.1% event rate at mean 43-month follow-up). In a post hoc analysis, the study population was stratified by the median baseline urine albumin/creatinine ratio (n=826) and analyzed for the 5-year incidence of the primary end point (myocardial infarction, hospitalization for congestive heart failure, stroke, renal replacement therapy, progressive renal insufficiency, or cardiovascular disease- or kidney disease-related death), for each component of the primary end point, and overall survival. When baseline urine albumin/creatinine ratio was ≤ median (22.5 mg/g, n=413), renal artery stenting was associated with significantly better event-free survival from the primary composite end point (73% versus 59% at 5 years; P=0.02), cardiovascular disease-related death (93% versus 85%; P≤ 0.01), progressive renal insufficiency (91% versus 77%; P=0.03), and overall survival (89% versus 76%; P≤0.01), but not when baseline urine albumin/creatinine ratio was greater than median (n=413). These data suggest that low albuminuria may indicate a potentially large subgroup of those with renal artery stenosis that could experience improved event-free and overall-survival after renal artery stent placement plus optimal medical therapy compared with optimal medical therapy alone. Introduction: The need for up-to-date and high-quality continuing medical education (CME) is growing while the financial investment in CME is shrinking. Despite online technology's potential to efficiently deliver electronic CME (eCME) to large numbers of users, it has not yet displaced traditional CME. The purpose of this study was to explore what health care providers want in eCME and how they want to use it. Methods: This was a qualitative study. Two 3-hour focus groups were held with physicians in both academic and community practices as well as trainees knowledgeable in the hypertension clinical practice guidelines with a willingness to discuss eCME. Content/thematic analysis was used to examine the data. Results: Three main themes emerged: credibility, content/context, and control. Credibility was the most consistent and dominant theme. Affiliations with medical organizations and accreditation were suggested as methods by which eCME can gain credibility. The content and need for discussion of the content emerged as a key pivot point between eCME and traditional CME: a greater need for discussion was linked to a preference for traditional face-to-face CME. Control over the content and how it was accessed was an emergent theme, giving learners the ability to control the depth of learning and the time spent. They valued the ability to quickly find information that was in a format (podcast, video, mobile device) that best suited their learning needs or preferences at the time. The article discusses research on the proper measurement of blood pressure (BP) in routine clinical practice. It references a study by Martin G. Myers and colleagues, published in the 2010 issue of "Hypertension." Background: In the Ongoing Telmisartan Alone and in Combination With Ramipril Global End Point Trial (ONTARGET), dual therapy did not reduce cardiovascular or renal outcomes compared with either angiotensin-converting enzyme inhibitors or angiotensin receptor blockers alone. Previous controlled trials with angiotensin-converting enzyme inhibitors or angiotensin receptor blockers have demonstrated greater cardiovascular and renal benefit in people with renal risk. We hypothesized that dual therapy would be more effective than monotherapy in patients with low glomerular filtration rate and elevated albuminuria., Methods and Results: Post hoc analysis was performed of renal subgroups of dual therapy versus monotherapy for the ONTARGET study and angiotensin receptor blocker versus placebo for the Telmisartan Randomized Assessment Study in ACE Intolerant Subjects With Cardiovascular Disease (TRANSCEND). The studies featured hazard ratios by subgroups and Cox regression models with factors for treatment, subgroup, and interactions. The main cardiovascular outcome was the composite of cardiovascular death, myocardial infarction, stroke, or hospitalization for heart failure, and the main renal outcome was the composite of chronic dialysis or doubling of creatinine. In ONTARGET, there was no cardiovascular or renal benefit from dual over monotherapy in any subgroup, even with low glomerular filtration rate and/or elevated albuminuria. In TRANSCEND, in the comparison of angiotensin receptor blocker with placebo, there was a significant interaction for the main renal outcome (P = 0.01) in the direction of harm for patients with normoalbuminuria (0.37 versus 0.16 events per 100 patient-years; hazard ratio, 2.35; confidence interval, 1.33 to 4.15) but a trend to benefit with microalbuminuria (0.52 versus 0.89 events per 100 patient-years; hazard ratio, 0.60; confidence interval, 0.25 to 1.46) and macroalbuminuria (1.57 versus 2.60 events per 100 patient-years; hazard ratio, 0.71; confidence interval, 0.21 to 2.44)., Conclusions: This post hoc analysis does not support dual therapy over monotherapy in high-vascular risk patients with low glomerular filtration rate or albuminuria. Objective: To compare readings obtained using two automated blood pressure (BP) recording devices, BpTRU and Omron 907, in a clinical setting., Methods: Two series of 50 patients attending a hypertension unit for 24-h ambulatory BP monitoring had BP recorded either every 1 or every 2 min using BpTRU and Omron 907 devices with the order of the measurements being randomized., Results: No significant differences for systolic BP between the mean readings taken using the BpTRU or Omron 907 recorders at either 1 or 2 min were observed. Diastolic BP readings were similar using the 1-min interval setting but were 5.0 mmHg lower for the Omron 907 (P<0.001) when readings were taken at 2-min intervals., Conclusion: BP measurements can be made in the clinical setting using either the BpTRU or Omron 907 automated sphygmomanometers.

Purpose of Review: The treatment of hypertension has been proven to reduce cardiovascular and renal risk. The role of long-acting calcium channel antagonists in the management of hypertension has been confused in the past because of a lack of controlled clinical trials on people with hypertension and in subpopulations including those with diabetes and renal disease. The year 2002 saw the publication of the results of the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial, the largest ever prospective drug-treatment trial, which involved 33 357 people with hypertension and included a calcium-antagonist group of 9048 individuals. Major publications on blood pressure control in people with kidney disease include the African American Study of Kidney Disease and Hypertension, and publications on people with diabetes include the results of the normotensive arm of the Appropriate Blood Pressure Control in Diabetes trial., Recent Findings: The main finding, from the studies reported in the last year, is that blood pressure control can be achieved using one or more of the first-line agents, including diuretics, calcium antagonists and angiotensin-converting enzyme inhibitors. On the basis of the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial, diuretics make clinical and economic sense as initial therapy for those with hypertension. Calcium antagonists are well tolerated and effective and should be considered as the initial drug therapy when diuretics are not tolerated or when multiple drug therapy is indicated. Angiotensin-converting enzyme inhibitors should be used in people with nephropathy, and, in these patients, will nearly always need to be part of multiple drug therapy to achieve blood pressure control. When blood pressure control can be achieved in largely non-nephropathic populations, there is further evidence that the drug class used as initial therapy may not be important. One of the main themes coming from the literature in the last year is that renal function is increasingly being recognized as an important outcome measure and marker of cardiovascular risk., Summary: The focus in blood pressure management must now be on identifying those with hypertension and bringing their blood pressure to target. For the majority of those with hypertension and renal disease, multiple drug therapy will be required, and, to achieve blood pressure targets, calcium antagonists are an appropriate part of this regimen. Particular attention is needed for nephropathic patients because of their higher risk of progression and the need for combination therapy; this group is likely to be the focus of future research and publications.
