Background: : Tree stands have remained popular among hunters because of the increased vantage point for the hunter. Although stand styles vary, the typical stand is very minimalistic in design, comprising a seat and an area for the hunter to place his feet. Although there have been studies using state trauma registry data, to date, there has not been a study on the epidemiology of tree stand-related injuries in the national population., Methods: : The 2000 to 2007 National Electronic Surveillance System provided information among individuals aged 16 years and older regarding tree stand-related injuries that required emergency room attention in the United States. Hunter population estimates from the U.S. Fish and Wildlife Services were used to estimates injury rates by sex, age, race, month, and year., Results: : The rate of tree stand-related injuries remained relatively stable from 2000 to 2007. Rates were higher for men compared with women (48.0 vs. 24.7 per 100,000 hunters, respectively) and highest among those 15 to 24 (55.7 per 100,000) and 25 to 34 (61.0 per 100,000)., Conclusions: : Although a majority of hunters are older, the highest rates occurred among the younger ones. Hunters using and manufacturers of tree stands must be aware of the safety precautions during use of the stands. Improvements in the safety design of the stands as well as knowledge to the proper use of stands can help to minimize the burden of injury in the hunter population related to tree stands.

Study Design: Controlled laboratory study in a cadaveric model., Objective: To determine if removing shoulder pads using the elevated torso technique generated less spinal segment motion than using the flat torso method., Summary of Background Data: Guidelines for care of the injured football player with a suspected spinal injury recommend initial immobilization with shoulder pads and helmet in place. There is a need to develop a safe protocol, for shoulder pad removal that maintains optimum cervical stability., Methods: Five lightly embalmed cadavers were studied before and after a globally unstable segment was created at C5-C6. A trained group of medical staff conducted repeated measures trials for 2 pad removal protocols. The elevated torso technique, outlined by the NATA Inter-Association Task Force, is the same as the flat torso except an additional assistant is employed to lift the patient's shoulders 30 degrees to 40 degrees off the ground while the head holder maintains spinal alignment as the pads are removed. An electromagnetic tracking device captured angular and linear motions in 3 planes between the C5-C6 segments., Results: The elevated torso technique generated significantly less C5-C6 motion in flexion/extension (P = 0.015) and lateral bending (P = 0.001), with a trend toward decreased cervical motion in axial rotation (P = 0.052). When moving the spine-injured cadavers, linear translation was also slightly, but not significantly less when the elevated torso technique was used. In the intact spine, significantly less motion was seen in 5 of 6 measures when the elevated torso technique was used. However, the differences were not large enough to be clinically significant in an intact spine., Conclusion: These findings support use of the elevated torso method to minimize cervical spine motion during shoulder pad removal when neither thoracic nor lumbar spinal injury is a concern.

Introduction: The recreational use of trampolines has increased dramatically during the last 10 years. There has been a striking increase in the number of children presenting to fracture clinics with injuries associated with trampoline use. This increase in trampoline injuries has been reported in North America, but there has been a paucity of research in this area in Europe., Methods: We prospectively recorded details of patients presenting to our institution, Our Lady's Children's Hospital, Crumlin (Dublin, Ireland), during the busy summer months of June, July, and August 2005. Details recorded included type and mechanism of injury, the mode of referral, treatment, inpatient days, outpatient visits, specific details relating to trampoline safety, and an analysis of the cost of medical care., Results: There were 101 patients treated for trampoline-related injuries in 3 months from June to August 2005. This represented 1.5% of the total attendances to the emergency department. The average age was 8.5 years (range, 1.4-17.4 years). There were 55 fractures, 38 soft tissue injuries, 5 head injuries, and 5 neck injuries, with an average Pediatric Trauma Score of 11.4. Fifty seven percent (58/101) of patients were on the trampoline with at least 1 other person. Twenty patients (19.8%) were admitted to hospital requiring 71 inpatient days. Twelve patients were treated in theatre. There were 163 fracture clinic visits, 212 x-rays, and 2 magnetic resonance imaging scans., Conclusions: Trampolines are a high-risk activity with the potential for significant orthopaedic injury. In Ireland, we have recently seen a dramatic increase in pediatric trampoline-related injuries mirroring the trend in the United States during the last 10 to 15 years. We found that more than 1 individual on a trampoline is a major risk factor for injury, where the lightest person is 14 times more likely to be injured than the heavier. The lighter person also has a greater chance of being injured with smaller numbers on the trampoline. We reiterate the American Academy of Pediatrics policy statement advice that trampolines be used only in supervised training programs--never at home, in outdoor playgrounds, or in schools. The public should be made aware of the potential dangers of trampolines through public health campaigns, radio, and television.

Gas within the brain parenchyma, known as intracerebral pneumocephalus, has been infrequently reported. Head trauma is the most common cause. A case of intracerebral pneumocephalus resulting from a golf club injury and a review of the literature are presented.

Introduction: Metal baseball bats produce higher ball exit velocity (BEV) than wood bats, increasing the risk of impact injuries to infield players. In this paper, maximum BEV from a wood and a metal bat were determined using the finite element method., Methods: Three-dimensional (3-D) bat kinematics at the instant of impact were determined from high-speed videography (N = 17 high-performance batters). A linear viscoelastic constitutive model was developed for stiffer and softer types of baseballs. The risk of impact injury was determined using available movement time data for adult pitchers; the data indicate that 0.400 s is required to evade a batted ball., Results: The highest BEV (61.5 m.s(-1)) was obtained from the metal bat and the stiffer ball model, equating to 0.282 s of available movement time. For five impacts along the long axis of each bat, the "best case scenario" resulted from the wood bat and the softer ball (46.0 m.s(-1), 0.377 s)., Conclusions: The performance difference between the bats was attributed to the preimpact linear velocity of the bat impact point and to differences in orientation on the horizontal plane. Reducing the swing moment of the baseball bat, and the shear and relaxation modulii of the baseball, increased the available movement time.

Treadmills are a burn risk for children. A child's hand can get trapped in the conveyor belt, causing friction burns to the underlying tissue. The purpose of this retrospective study was to review the characteristics and treatment of treadmill-related burns in children from 1998 to 2002. Ten patients, at a mean age of 3.4 years, sustained injuries associated with treadmill use. Trapping of the hand between the conveyor belt and the base was the most frequent injury mechanism. Burn location was predominantly on fingers and palms. Four patients required operative intervention. All patients required specialized wound care as well as scar management and occupational therapy. Treadmills pose a danger to children. Current safety devices are ineffective for preventing serious hand injuries in children. New design modifications and public awareness are needed to improve child safety.

Objective: To compare various soft-core baseballs for their ability to reduce the risk of fatal chest-impact injury., Design: This study used a three-rib biomechanical surrogate to quantitatively analyze chest impacts from nine soft-core baseballs and one standard baseball, which served as the control. Impacts were achieved with an air cannon system, with the velocity of impact being 40, 50, and 60 mph., Materials and Methods: The deflection of the three-rib structure at the sternum was measured and used to calculate the viscous criterion, which correlates with risk of chest-impact injury., Measurements and Main Results: Analysis showed that baseballs with lighter mass had a significantly lower viscous criterion (p < 0.05). Those with a similar mass had no change in the viscous criterion, and the heaviest soft-core baseball had a significantly higher viscous criterion at an impact velocity of 60 mph., Conclusion: The results of this study indicate that soft-core baseballs may not differ from a standard baseball with regard to the risk of fatal chest-impact injury while playing baseball. Other techniques, such as preventive coaching, need to be implemented when trying to improve baseball safety.

Purpose: This report evaluates the clinical characteristics of surfing-related ocular trauma to learn the nature of such injuries and propose possible preventive measures., Methods: The authors reviewed 11 cases of surfing-related eye injuries caused by direct trauma from the surfboard, studying their mechanism of injury, the associated ocular complications, and the anatomic and visual outcomes of surgical repair., Results: Surfing-related ocular injuries occurred exclusively in young males (mean age, 24.8 years; range, 14-37 years). The mechanism of injury most frequently responsible was impact with the sharp nose of the surfboard following a fall. Serious posterior segment complications were observed in all 11 patients, with nine patients suffering ruptured globes. Despite immediate medical attention, five patients did not recover ambulatory levels of visual acuity (>5/200)., Conclusions: Surfing-related ocular trauma presenting to the retinal specialist typically leaves the patient with a permanent visual disability. Important factors contributing to these high-velocity injuries include the sharply pointed nose of the surfboard and the leash keeping the surfer in close proximity to the board following a fall. A simple modification in surfboard design such as blunting the sharp nose of the surfboard, or appropriate protective guards fitted over the surfboard nose, should lessen the severity of such injuries.
