Background: The literature lacks studies that confirm whether the improved radiographic alignment that can be achieved with computer-navigated total knee arthroplasty improves patients’ activities of daily living or the durability of total knee prostheses. The purpose of this study was to determine whether computer-navigated total knee arthroplasty improves the clinical function, alignment, and survivorship of the components.Methods: We prospectively compared the results of 520 patients with osteoarthritis who underwent computer-navigated total knee arthroplasty for one knee and conventional total knee arthroplasty for the other. The assignment of the knee to navigation or not was done randomly. There were 452 women (904 knees) and sixty-eight men (136 knees) with a mean age of sixty-eight years (range, forty-nine to eighty-eight years) at the time of the index arthroplasty. The mean follow-up period was 10.8 years (range, ten to twelve years). The patients were assessed clinically and radiographically with the rating system of the Knee Society and with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score at three months, one year, and annually thereafter.Results: Total knee scores, knee function scores, pain scores, WOMAC scores, knee motion, and activity scores did not show statistically significant differences between the two groups preoperatively or at the time of the final follow-up. Alignment and the survivorship of the components were not significantly different between the two groups. The Kaplan-Meier survivorship with revision as the end point at 10.8 years was 98.8% (95% confidence interval [CI], 0.96 to 1.00) in the computernavigated total knee arthroplasty group and 99.2% (95% CI, 0.96 to 1.00) in the conventional total knee arthroplasty group.Conclusions: Our data demonstrated no difference in clinical function or alignment and survivorship of the components between the knees that underwent computer-navigated total knee arthroplasty and those that underwent conventional total knee arthroplasty. Background: The number of midterm or long-term studies on the current generation of cementless total hip replacements with alumina-on-alumina ceramic bearings in patients younger than thirty years of age is limited. The purpose of this study was to evaluate the midterm results of the cementless metaphyseal fitting anatomic total hip prosthesis in patients younger than thirty years of age, with a particular emphasis on the prevalence of thigh pain, resorption of bone due to stress-shielding of the proximal part of the femur, aseptic loosening, and osteolysis.Methods: We reviewed the cases of ninety-six patients (127 hips) who had a cementless total hip arthroplasty when they were thirty years or younger at the time of surgery. All surgical procedures were performed by a single surgeon. The most common diagnoses were osteonecrosis (54.3%) and developmental dysplasia of the hip (20.5%). Demographic data, the Harris hip score, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and University of California, Los Angeles (UCLA) activity scores were recorded. Radiographic evaluation was used to evaluate implant fixation and osteolysis. The minimum follow-up interval was ten years (mean, 14.6 years; range, ten to sixteen years).Results: The mean preoperative Harris hip score, WOMAC score, and UCLA activity score were 41 points, 66 points, and 3 points, respectively. At the time of final follow-up, the mean Harris hip score, WOMAC score, and UCLA activity score were 95 points, 16 points, and 8 points, respectively. No patient had thigh pain after one year postoperatively. All of the femoral stems and all but one of the acetabular components were well-fixed at the time of final follow-up. No hip exhibited squeaking, ceramic fracture, loosening, or osteolysis at the time of the final follow-up.Conclusions: These results in patients thirty years of age or younger suggest that the cementless metaphyseal fitting anatomic total hip prosthesis provides outstanding midterm fixation and substantial pain relief well into the second decade postoperatively. Background: We are aware of no information about the mid-term performance of the high-flexion total knee arthroplasty, although early results have been reported. The purpose of this study was to evaluate the mid-term results of high-flexion and conventional knee prostheses.Methods: We prospectively compared the results of 100 patients with osteoarthritis who had received a NexGen Legacy Posterior Stabilized (NexGen LPS) prosthesis in one knee and a NexGen Legacy Posterior Stabilized-Flex (NexGen LPS-Flex) prosthesis in the other. Seventy-five patients (150 knees) were women and twenty-five (fifty knees) were men. The mean age was sixty-five years (range, forty-eight to eighty-five years) at the time of the index procedure. The mean duration of follow-up was 10.3 years (range, ten to 10.6 years). The patients were assessed with radiographs, with the rating system of the Knee Society, and with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score at three months, one year, and annually thereafter.Results: Total knee scores, knee function scores, pain scores, WOMAC scores, knee motion, and activity scores did not differ significantly between the two designs of the implants, on the basis of the numbers studied, either preoperatively or at the time of final follow-up. One knee in the NexGen LPS-Flex group was revised because of recurrent infection. No knee in either group had aseptic loosening of the components. The Kaplan-Meier survivorship at ten years postoperatively, with revision defined as the end point, was 100% (95% confidence interval, 94 to 100) for the NexGen LPS prosthesis and 99% (95% confidence interval, 93 to 100) for the NexGen LPS-Flex prosthesis.Conclusions: After a minimum duration of follow-up of ten years, there were no significant differences between the two groups with regard to implant survivorship, functional outcome, knee motion, or prevalence of osteolysis. Background: To our knowledge, no study to date has compared the clinical results of posterior cruciate-sacrificing mobile-bearing total knee replacements with those of posterior-stabilized mobile-bearing total knee replacements in the same patients. The purpose of the present study was to compare the clinical and radiographic results of these two designs. We hypothesized that the results would be better for knees treated with the posterior-stabilized mobile-bearing prosthesis.Methods: The present study consisted of a consecutive series of 107 female patients (mean age, 66.8 years) who underwent bilateral simultaneous total knee arthroplasty at the same surgical setting. All of these patients received a posterior cruciate-sacrificing mobile-bearing prosthesis in one knee and a posterior-stabilized mobile-bearing prosthesis in the contralateral knee. At the time of each follow-up (mean, 7.4 years; range, seven to 7.6 years), the patients were assessed clinically.Results: The mean postoperative Knee Society knee score (96 compared with 97 points) and Hospital for Special Surgery knee score (93 compared with 94 points) were similar between the two groups. At the time of the latest follow-up, the average range of motion was 127.7° (range, 70° to 150°) in the knees with a posterior cruciate-sacrificing mobile-bearing prosthesis and 132.4° (range, 90° to 150°) in the knees with a posterior-stabilized mobile-bearing prosthesis. With a margin of error of the manual measurement of 5°, this difference was not significant. The estimated survival rate was 97.2% (95% confidence interval, 91% to 99%) at seven years in the posterior-cruciate sacrificing mobile-bearing prosthesis group and 98.1% (95% confidence interval, 92% to 99%) at seven years in the posterior-stabilized mobile-bearing prosthesis group.Conclusions: After a minimum duration of follow-up of seven years, we found no significant differences between the two groups with regard to the clinical and radiographic results, including knee range of motion. Background: Recently, much debate has focused on the effect of gender-specific total knee arthroplasty. The purpose of the present study was to compare clinical and radiographic results as well as femoral component fit in patients receiving either a standard posterior cruciate-substituting LPS-Flex or gender-specific posterior cruciate-substituting LPS-Flex total knee prosthesis.Methods: Sequential simultaneous bilateral total knee arthroplasty was performed for eighty-five patients (170 knees). Eighty-five women (mean age, 69.7 years) received a standard LPS-Flex prosthesis in one knee and a gender-specific LPS-Flex prosthesis in the contralateral knee. The mean duration of follow-up was 2.13 years. At each follow-up, the Knee Society score, the Hospital for Special Surgery knee score, the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) score, and radiographs were evaluated. The aspect ratio of the distal part of the femur was compared with those of the standard LPS-Flex prosthesis and the gender-specific LPS-Flex prosthesis.Results: The mean postoperative Knee Society scores (95.5 points in the standard implant group, compared with 96.5 points in the gender-specific implant group) and Hospital for Special Surgery knee scores (90.7 points in the standard implant group, compared with 91.2 points in the gender-specific implant group) were similar in both groups. The mean postoperative WOMAC score was 36.6 points. Postoperatively, the mean ranges of knee motion in the supine position (125 degrees in the standard implant group, compared with 126 degrees in the gender-specific implant group), patient satisfaction (8.3 points in the standard implant group, compared with 8.1 points in the gender-specific implant group), and radiographic results were similar in both groups. The femoral component in the standard implant group fit significantly better than that in the gender-specific implant group (p < 0.0001).Conclusions: The present study did not show any clinical benefits of a gender-specific LPS-Flex total knee prosthesis at the time of short-term follow-up. Background: The greater lubricity and resistance to scratching of oxidized zirconium femoral components are expected to result in less polyethylene wear than cobalt-chrome femoral components.Questions/purposes: We examined polyethylene wear particles in synovial fluid and compared the weight, size (equivalent circle diameter), and shape (aspect ratio) of polyethylene wear particles in knees with an oxidized zirconium femoral component with those in knees with a cobalt-chrome femoral component.Patients and Methods: One hundred patients received an oxidized zirconium femoral component in one knee and a cobalt-chrome femoral component in the other. There were 73 women and 27 men with a mean age of 55.6 years (range, 44-60 years). The minimum followup was 5 years (mean, 5.5 years; range, 5-6 years). Polyethylene wear particles were analyzed using thermogravimetric methods and scanning electron microscopy.Results: The weight of polyethylene wear particles produced at the bearing surface was 0.0223 +/- 0.0054 g in 1 g synovial fluid in patients with an oxidized zirconium femoral component and 0.0228 +/- 0.0062 g in patients with a cobalt-chrome femoral component. Size and shape of polyethylene wear particles were 0.59 +/- 0.05 microm and 1.21 +/- 0.24, respectively, in the patients with an oxidized zirconium femoral component and 0.52 +/- 0.03 microm and 1.27 +/- 0.31, respectively, in the patients with a cobalt-chrome femoral component. Knee Society knee and function scores, radiographic results, and complication rate were similar between the knees with an oxidized zirconium and cobalt-chrome femoral component.Conclusions: The weight, size, and shape of polyethylene wear particles were similar in the knees with an oxidized zirconium and a cobalt-chrome femoral component. To ascertain whether THA in patients with high dislocation after childhood sepsis would relieve pain and improve function, we assessed the rate of postoperative infection, improvement in Harris hip and WOMAC scores, and improvement in range of motion after the THA in 62 patients (62 hips) with high dislocation (Crowe Type 4) after childhood sepsis. The revision rate and the incidence of complications also were assessed. The mean age of the patients was 47.5 years. The minimum followup was 13 years (mean, 15.2 years; range, 13-17 years). One patient had persistent infection. The mean preoperative Harris hip score of 55 points improved to 89 points at the final followup, and the mean preoperative WOMAC score of 65 points improved to 42 points. The mean combined preoperative arc of range of motion of 197 degrees improved to 275 degrees. Four cups (6%) and three stems (5%) were revised. Complications developed in nine hips (15%), but the rate of infection was low. Improved surgical technique and design of components provided favorable results for total hip replacement performed for complications of childhood sepsis in these young and active patients. The purpose of this study was to compare the ranges of motion of the knees of patients treated with a standard posterior cruciate-retaining total knee prosthesis in one knee and a high-flexion posterior cruciate-retaining total knee prosthesis in the other.Methods: Fifty-four patients (mean age, 69.7 years) received a standard posterior cruciate-retaining total knee prosthesis in one knee and a high-flexion posterior cruciate-retaining total knee prosthesis in the contralateral knee. Five patients were men, and forty-nine were women. At a mean of three years postoperatively, the patients were assessed clinically and radiographically with the knee-rating systems of the Knee Society and the Hospital for Special Surgery and with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score.Results: The mean postoperative Knee Society and Hospital for Special Surgery knee scores were 93.7 and 89 points, respectively, for the knees with a standard posterior cruciate-retaining prosthesis, and they were 93.9 and 90 points, respectively, for the knees with a high-flexion posterior cruciate-retaining prosthesis. The mean postoperative WOMAC score was 22 points. Postoperatively, the mean ranges of motion without and with weight-bearing were 131 degrees (range, 90 degrees to 150 degrees) and 115 degrees (range, 75 degrees to 145 degrees), respectively, in the knees with a standard prosthesis and 133 degrees (range, 90 degrees to 150 degrees) and 118 degrees (range, 75 degrees to 145 degrees), respectively, in those with a high-flexion prosthesis. Patient satisfaction and radiographic results were similar in the two groups. No knee had aseptic loosening, revision, or osteolysis.Conclusions: After a minimum duration of follow-up of three years, we found no significant differences between the two groups with regard to the range of knee motion or the clinical or radiographic parameters. Background: Although the design features of the high-flexion posterior cruciate-retaining and high-flexion posterior cruciate-substituting total knee prostheses reportedly improve the range of knee motion, a clinical comparison of both systems with regard to range of motion has not been reported, to our knowledge. The purpose of the present study was to compare the range of motion and functional outcome in knees receiving either a high-flexion posterior cruciate-retaining or a high-flexion posterior cruciate-substituting total knee prosthesis.Methods: Two hundred and fifty patients (mean age, 71.6 years) received a high-flexion posterior cruciate-retaining total knee prosthesis in one knee and a high-flexion posterior cruciate-substituting total knee prosthesis in the contralateral knee. Ten patients were men, and 240 were women. At the time of each follow-up (minimum duration of follow-up, two years; mean, 2.3 years), the patients were assessed clinically and radiographically with use of the knee-rating systems of the Knee Society and the Hospital for Special Surgery. In addition, each patient completed the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) questionnaire. Non-weight-bearing and weight-bearing ranges of knee motion were determined in both groups.Results: The mean postoperative Knee Society knee score was 94 points for the knees treated with a high-flexion cruciate-retaining prosthesis and 95 points for those treated with a high-flexion posterior cruciate-substituting prosthesis. The mean postoperative Hospital for Special Surgery knee score was 90 points for the knees that had been treated with a high-flexion posterior cruciate-retaining prosthesis and 91 points for those that had been treated with the high-flexion posterior cruciate-substituting prosthesis. At the time of the latest follow-up, the knees that had been treated with a high-flexion posterior cruciate-retaining prosthesis had a mean non-weight-bearing range of motion of 133 degrees and a mean weight-bearing range of motion of 118 degrees. The knees that had been treated with a high-flexion posterior cruciate-substituting prosthesis had a mean non-weight-bearing range of motion of 135 degrees and a mean weight-bearing range of motion of 122 degrees. No knee had aseptic loosening, revision, or osteolysis.Conclusions: After a minimum duration of follow-up of two years, there was no difference in range of motion or clinical and radiographic results between knees that had received a high-flexion posterior cruciate-retaining total knee prosthesis and those that had received a high-flexion posterior cruciate-substituting total knee prosthesis. STUDY DESIGN: An animal experimental study on hypothermia after spinal cord injury(SCI). OBJECTIVE.: To investigate the neuroprotective effect of moderate epidural hypothermia (MEH) on SCI in rats and evaluate the antiapoptotic and anti-inflammatory effect of hypothermia. SUMMARY OF BACKGROUND DATA: Experimental systemic or local hypothermia has been tried for neuroprotection after ischemic or traumatic SCI. However, its clinical application is limited due to its technical difficulties. If hypothermia is to be considered in the future for treating SCI, then alternative approaches and further studies on the advantages and disadvantages of hypothermia will be mandatory. METHODS: Author's designed MEH (30 degrees C for 48 hours) was tried as a neuroprotector after traumatic SCI. The functional recovery and histopathological changes were assessed. A spinal cord contusion (25 g-cm at T-9) was produced in all 16 rats, and these rats were randomly divided into 2 groups; (1) the control group (spinal cord contusion only), (2) the hypothermia group (spinal cord contusion followed by MEH). RESULTS: MEH significantly reduced the apoptosis of neurons and glial cells when compared with the control group (P < 0.05). Furthermore, MEH significantly inhibited the activation of the microglia and it improved functional recovery after SCI (P < 0.05). CONCLUSION: These results suggest that MEH may be a possible therapeutic candidate when combined with clinically proven surgical and medical methods for SCI. [ABSTRACT FROM AUTHOR]

Background: A ceramic femoral head is an alternative to a metal femoral head for the bearing surface of total hip arthroplasty. The purpose of this study was to compare polyethylene wear in patients who had undergone bilateral total hip arthroplasty with implants that differed only with regard to the material used for the femoral head: a zirconia head was used on one side, and a cobalt-chromium head was inserted on the contralateral side.Methods: A prospective, randomized study was performed to evaluate the outcomes in fifty-two patients who had undergone sequential bilateral primary total hip replacement. A zirconia head was used in one hip, and a cobalt-chromium head was used in the other. There were forty-eight men and four women; the mean age at the time of surgery was 44.2 years. The mean duration of follow-up was 7.1 years. Clinical and radiographic evaluations were performed preoperatively and at six weeks; three, six, and twelve months; and yearly postoperatively. Linear wear of the polyethylene liner was measured radiographically. Two femoral components with a zirconia head had aseptic loosening and were revised. The explanted heads were evaluated with use of interferometry, environmental scanning electron microscopy, and x-ray diffraction studies.Results: The mean polyethylene wear rate was 0.08 mm/yr in association with the zirconia heads and 0.17 mm/yr in association with the cobalt-chromium heads (p = 0.004). The mean amount of volumetric polyethylene wear was 350.8 mm3 in association with the zirconia heads and 744.7 mm3 in association with the cobalt-chromium heads (p = 0.004). With regard to surface roughness, the Ra values of the two explanted zirconia heads were 15.87 and 17.35 nm and the Rpm values were 153.86 and 156.18 nm. Two identical zirconia heads that had not been implanted had Ra values of 5.31 and 5.48 nm and Rpm values of 65.27 and 66.35 nm. Four unimplanted cobalt-chromium heads that were identical to the ones implanted in this study had Ra values ranging between 25 and 50 nm and Rpm values ranging between 262.6 and 525.2 nm. Little phase transformation was noted in the two revised zirconia heads.Conclusions: The mean amount and rate of polyethylene wear were significantly lower in the hips with a zirconia head than they were in the hips with a cobalt-chromium head, presumably because the zirconia heads had a smoother articulating surface. STUDY DESIGN.: An experimental comparative study on moderate epidural hypothermia (MEH) versus moderate systemic hypothermia (MSH) after spinal cord injury (SCI). OBJECTIVE.: To compare neuroprotective effects of hypothermia between MEH and MSH after SCI in rats. SUMMARY OF BACKGROUND DATA.: Experimental MEH or MSH has been attempted for neuroprotection after ischemic or traumatic SCI. However, there is no comparative study on neuroprotective effect of MEH and MSH after SCI. If hypothermia is to be considered as 1 modality for treating SCI, further studies on the advantages and disadvantages of hypothermia will be mandatory. METHODS.: A spinal cord contusion was produced in all 32 rats, and these rats were randomly divided into 4 groups-8 rats in each group: (1) the control group (spinal cord contusion only), (2) the methylprednisolone group, (3) the MEH group (28°C for 48 hr), and (4) the MSH group (32°C for 48 hr). The functional recovery was assessed using Basso, Beattie, Bresnahan scale and antiapoptotic and anti-inflammatory effects were assessed. RESULTS.: The Basso, Beattie, Bresnahan scale scores in both the hypothermia groups were significantly higher than that in the control group at 6 weeks. The numbers of terminal deoxynucleotidyl transferase dUTP nick end labeling-positive cells and OX-42 positive cells were significantly lower in both the MEH and MSH groups than that in the control group. The p38 mitogen-activated protein kinases expression of the treated groups was significantly lower than that of the control group. The expression of caspase-8 and caspase-9 significantly decreased in the treated groups compared with that of the control group. However, in terms of caspase-3, only the MSH group has shown to be significantly lower than that of the control group. CONCLUSION.: This study presented that both systemic and epidural hypothermia demonstrated neuroprotective effects after SCI. In this article, a comparison for the hybrid and cementless total hip replacement in patients less than 50 years of age is presented. As stated, randomized trials were conducted and it was found that the patients less than 50 years of age having total hip replacement, hybrid and cementless implants were similar. A chart depicting the comparative outcomes of the hybrid and cementless total hip replacement is also presented.

Purpose: The aim of this study was to investigate the efficacy of RCI001 (RCI) in a mouse model of primary Sjögren syndrome., Methods: Eight 12-week-old NOD.B10-H2b mice were used in this study. All experimental animals were randomly divided into phosphate-buffered saline (PBS) and RCI groups in NOD.B10-H2b mice. The eyes of mice were topically treated with PBS or RCI twice a day for a week. Ocular surface staining (OSS) and tear secretion were compared between before and after treatment. The transcript levels of inflammatory cytokines and nicotinamide adenine dinucleotide phosphate oxidase (NOX) in the conjunctiva and cornea (CC) and lacrimal gland were assayed. In addition, immunofluorescence staining of the conjunctiva was assessed., Results: The RCI group showed significant clinical improvement in OSS and tear secretion after 1 week of treatment compared with the baseline (both P < 0.001) and showed better improvement in OSS and tear secretion than the PBS group after 1 week of treatment (both P < 0.05). The levels of IL-1β and IL-17 in CC and IL-6 in the lacrimal gland were also significantly reduced in the RCI group compared with the PBS group (each P < 0.05). Transcript levels of NOX2 and NOX4 were also significantly reduced in CC of the RCI group compared with those of the PBS group (P < 0.05). The RCI group also resulted in lower conjunctival expression of oxidative stress markers (4-hydroxy-2-nonenal, hexanoyl-lysine, and NOX4) than the PBS group., Conclusions: Topical RCI001 demonstrated excellent therapeutic efficacy in a mouse model of primary Sjögren syndrome by inhibiting inflammation and oxidative stress., Competing Interests: D. Due to limited published data, we investigated 3-year outcomes according to left ventricular ejection fraction (LVEF) in patients older and younger than 75 years with non-ST-segment elevation myocardial infarction (NSTEMI) who underwent successful newer-generation drug-eluting stent (DES) implantation. This research analyzed the data of 4558 patients (1032 older adults [≥75 years] and 3526 younger adults [<75 years]) from the Korea Acute MI Registry-NIH. We further divided the older group based on LVEF: heart failure (HF) with reduced EF (HFrEF, ≤40%, n = 196; group A), HF with mildly reduced EF (HFmrEF, 41-49%, n = 228; group B), and HF with preserved EF (HFpEF, ≥50%, n = 608; group C). Similarly, the younger group was divided into HFrEF (group D, n = 353), HFmrEF (group E, n = 577), and HFpEF (group F, n = 2596). The primary outcome was a composite of major adverse cardiac events (MACE) at 3 years, including all-cause death, recurrent MI, any repeat revascularization, or hospitalization for HF. MACE rates were highest in the HFrEF groups (A and D), followed by the HFmrEF groups (B and E), and lowest in the HFpEF groups (C and F) for both age groups. All-cause death, cardiac death (CD), all-cause death or MI, and hospitalization for HF rates were higher in group A than in groups B and C, and higher in group D than in groups E and F. Across all LVEF categories, MACE, all-cause death, CD, and non-CD, and all-cause death or MI rates were higher in the older group. This multicenter cohort study demonstrates that older patients have higher mortality rates compared to younger patients. Additionally, MACE rates were highest in the HFrEF group, followed by the HFmrEF group, and lowest in the HFpEF group across both age groups. Aims: Considering the limited data regarding clinical outcomes of patients with non-ST-segment on the ECG elevation myocardial infarction (NSTEMI), this study compared the outcomes of patients undergoing percutaneous coronary intervention with newer-generation drug-eluting stents stratified by the presence/absence of complex lesions and symptom-to-balloon time (SBT; <48 h or ≥48 h)., Methods: We enrolled 4373 patients with NSTEMI from the Korea Acute Myocardial Infarction Registry-National Institute of Health dataset and stratified them into the complex group (2106 patients; SBT < 48 h, n  = 1365; SBT ≥48 h, n  = 741) and the noncomplex group (2267 patients; SBT < 48 h, n  = 1573; SBT ≥48 h, n  = 694). The primary outcome was the 3-year all-cause mortality rate. The secondary outcomes were any major adverse cardiac events (MACE), including cardiac death (CD), recurrent myocardial infarction, and stroke., Results: The incidence of all-cause mortality (adjusted hazard ratio, 0.656; P  = 0.009), CD ( P  = 0.037), and MACE ( P  = 0.047) in the complex group and of stroke in the noncomplex group ( P  = 0.020) were significantly lower in patients with SBT < 48 h than in those with SBT ≥48 h. Among patients with SBT < 48 h, the stroke incidence ( P  = 0.019) was higher in the complex group than in the noncomplex group, while among patients with SBT ≥48 h, the MACE incidence ( P  = 0.011) was higher in the former than in the latter., Conclusion: SBT reduction effectively decreased the 3-year mortality in patients with NSTEMI in the complex group compared with the noncomplex group., (Copyright © 2024 Italian Federation of Cardiology - I.F.C. Background: Atherosclerosis is a chronic inflammatory disease causing a fatal plaque rupture, and its key aspect is a failure to resolve inflammation. We hypothesize that macrophage-targeted near-infrared fluorescence emitting photoactivation could simultaneously assess macrophage/lipid-rich plaques in vivo and facilitate inflammation resolution., Methods: We fabricated a Dectin-1-targeted photoactivatable theranostic agent through the chemical conjugation of the near-infrared fluorescence-emitting photosensitizer chlorin e6 and the Dectin-1 ligand laminarin (laminarin-chlorin e6 [LAM-Ce6]). Intravascular photoactivation by a customized fiber-based diffuser after administration of LAM-Ce6 effectively reduced inflammation in the targeted plaques of atherosclerotic rabbits in vivo as serially assessed by dual-modal optical coherence tomography-near-infrared fluorescence structural-molecular catheter imaging after 4 weeks., Results: The number of apoptotic macrophages peaked at 1 day after laser irradiation and then resolved until 4 weeks. Autophagy was strongly augmented 1 hour after the light therapy, with the formation of autophagolysosomes. LAM-Ce6 photoactivation increased the terminal deoxynucleotidyl transferase dUTP (deoxyuridine triphosphate) nick end labeling/RAM11 (rabbit monocyte/macrophage antibody)- and MerTK (c-Mer tyrosine kinase)-positive cells in the plaques, suggesting enhanced efferocytosis. In line with inflammation resolution, photoactivation reduced the plaque burden through fibrotic replacement via the TGF (transforming growth factor)-β/CTGF (connective tissue growth factor) pathway., Conclusions: Optical coherence tomography-near-infrared fluorescence imaging-guided macrophage Dectin-1-targetable photoactivation could induce the transition of macrophage/lipid-rich plaques into collagen-rich lesions through autophagy-mediated inflammation resolution and TGF-β-dependent fibrotic replacement. Objective: The purpose of this study was to explore maternity care experiences of African immigrant women during the perinatal period including factors affecting access to and use of care., Methods: We used Sandelowski's (2010) qualitative descriptive approach to examine how African immigrant women from various countries of origin and with diverse ethnic backgrounds experienced and navigated the maternity care system in the United States during pregnancy and childbirth. We conducted semi-structured interviews with 15 African immigrant women living in the Columbus, Ohio area. Participants were recruited using purposive and snowball sampling between February 2021 and May 2021. Interviews were recorded, transcribed, and analyzed using a reflexive thematic analysis approach., Findings: Four major themes defined the experiences of our study participants: access to information, patient-clinician relationships, experiences of discrimination, and costs of maternity care., Clinical Implications: Findings highlight key barriers to providing quality and acceptable maternity care to African immigrant women at multiple levels. This group's unique barriers underlie the importance of incorporating their diverse experiences into maternity care models and clinical practice.
