Significance: Individuals with left hemianopic field loss (HFL), especially with neglect history, may have greater difficulties than individuals with right HFL in judging the direction of another person's gaze., Purpose: Individuals with HFL often show a spatial bias in laboratory-based perceptual tasks. We investigated whether such biases also manifest in a more real-world task, perception of mutual gaze direction, an important, nonverbal communication cue in social interactions., Methods: Participants adjusted the eye position of a life-size virtual head on a monitor at a 1-m distance until (1) the eyes appeared to be looking straight at them, or (2) the eyes were perceived to be no longer looking at them (to the right and left)., Results: Participants with right HFL (n = 8) demonstrated a rightward error in line bisection but made gaze judgments within the range of normally sighted controls (n = 17). Participants with left HFL without neglect history (n = 6) made leftward errors in line bisection and had more variable gaze judgments; three had estimates of gaze direction outside the reference range. Four participants with left HFL and neglect history made estimates of gaze direction that were to the right of the reference range., Conclusions: Our results suggest that individuals with left HFL, especially with neglect history, may have greater difficulties than individuals with right HFL in compensating for low-level spatial biases (as manifested in line bisection) when performing the more complex, higher-level task of judging gaze direction.

Risk stratification in patients with stable ischemic heart disease is essential to guide treatment decisions. In this regard, whether coronary anatomy, physiology, or plaque morphology is the best determinant of prognosis (and driver an effective therapeutic risk reduction) remains one of the greatest ongoing debates in cardiology. In the present report, we review the evidence for each of these characteristics and explore potential algorithms that may enable a practical diagnostic and therapeutic strategy for the management of patients with stable ischemic heart disease., (© 2016 American Heart Association, Inc.)

There is a common misperception in the cardiology community that most acute coronary events arise from ruptures of mildly stenotic plaques. This notion has emanated from multiple studies that had measured the degree of angiographic luminal narrowing in culprit plaques months to years before myocardial infarction. However, angiographic studies within 3 months before myocardial infarction, immediately after myocardial infarction with thrombus aspiration or fibrinolytic therapy, and postmortem pathological observations have all shown that culprit plaques in acute myocardial infarction are severely stenotic. Serial angiographic studies also have demonstrated a sudden rapid lesion progression before most cases of acute coronary syndromes. The possible mechanisms for such rapid plaque progression and consequent luminal obstruction include recurrent plaque rupture and healing and intraplaque neovascularization and hemorrhage with deposition of erythrocyte-derived free cholesterol. Moreover, recent intravascular and noninvasive imaging studies have demonstrated that plaques which result in coronary events have larger plaque volume and necrotic core size with greater positive vessel remodeling compared with plaques, which remain asymptomatic during several years follow-up, although these large atheromatous vulnerable plaques may angiographically seem mild. As such, it is these vulnerable plaques which are more prone to rapid plaque progression or are those in which plaque progression is more likely to become clinically evident. Therefore, in addition to characterizing plaque morphology, inflammatory activity, and severity, detection of the rate of plaque progression might identify vulnerable plaques with an increased potential for adverse outcomes., (© 2015 American Heart Association, Inc.)

Purpose: To evaluate the effects of central vision loss (CVL) on mutual gaze perception (knowing whether somebody else is looking at you), an important nonverbal visual cue in social interactions., Methods: Twenty-three persons with CVL (visual acuity 20/50 to 20/200), 16 with a bilateral central scotoma and 7 without, and 23 age-matched control subjects completed a gaze perception task and a brief questionnaire. They adjusted the eyes of a life-size virtual head on a monitor at a 1-m distance until they either appeared to be looking straight at them or were at the extreme left/right or up/down positions at which the eyes still appeared to be looking toward them (defining the range of mutual gaze in the horizontal and vertical planes)., Results: The nonscotoma group did not differ from the control subjects in any gaze task measure. However, the gaze direction judgments of the scotoma group had significantly greater variability than those of the nonscotoma and control groups (p < 0.001). In addition, their mutual gaze range tended to be wider (p = 0.15), suggesting a more liberal judgment criterion. Contrast sensitivity was the strongest predictor of variability in gaze direction judgments followed by self-reported difficulties., Conclusions: Our results suggest that mutual gaze perception is relatively robust to CVL. However, a follow-up study that simulates less-than-optimal viewing conditions of everyday social interactions is needed. The gaze perception task holds promise as a research tool for investigating the effects of vision impairment on mutual gaze judgments. Self-reported difficulty and contrast sensitivity were both independent predictors of gaze perception performance, suggesting that the task captured higher-order as well as low-level visual abilities.

Unlabelled: The effect of acupuncture on pain perception is controversial. Because late amplitudes of somatosensory evoked potentials (SEPs) to noxious stimuli are thought to correlate with the subjective experience of pain intensity, we designed this study to detect changes of these SEPs before and after acupuncture in a double-blinded fashion. Sixteen volunteers were anesthetized by propofol and exposed to painful electric stimuli to the right forefinger. Then, blinded to the research team, the acupuncture group (n = 8) was treated with electric needle acupuncture over 15 min at analgesic points of the leg, whereas the sham group (n = 8) received no treatment. Thereafter, nociceptive stimulation was repeated. SEPs were recorded during each noxious stimulation from the vertex Cz, and latencies and amplitudes of the N150 and P260 components were analyzed by analysis of variance. P260 amplitudes decreased from 4.40 +/- 2.76 microV (mean +/- SD) before treatment to 1.67 +/- 1.21 microV after treatment (P < 0.05), whereas amplitudes of the sham group remained unchanged (2.64 +/- 0.94 microV before versus 2.54 +/- 1.54 microV after treatment). In conclusion, this double-blinded study demonstrated that electric needle acupuncture, as compared with sham treatment, significantly decreased the magnitudes of late SEP amplitudes with electrical noxious stimulation in anesthetized subjects, suggesting a specific analgesic effect of acupuncture., Implications: This double-blinded study demonstrates that electric needle acupuncture, as compared with sham treatment, significantly decreases the magnitudes of late somatosensory evoked potential amplitudes with electrical noxious stimulation in anesthetized subjects, suggesting a specific analgesic effect of acupuncture.

Of 2,122 consecutive patients undergoing elective coronary angioplasty from 1982 to 1985, 62% had stable angina pectoris (SAP), and 38% had unstable angina pectoris (UAP). There were differences between the two groups in clinical and morphological factors and in initial and late results of angioplasty. UAP patients were more likely than SAP patients to be smokers and to have had prior myocardial infarctions. Lesions in UAP patients were more severe, longer, more eccentric, more irregular, and more likely to have intracoronary thrombi than were lesions in SAP patients. Coronary angioplasty success was achieved in 84% of UAP and in 88% of SAP patients (p less than 0.05), and complications occurred in 6.7% of UAP and in 4.7% of SAP patients (p less than 0.05). Hospital death rates were low and similar, 0.2% for both groups. Follow-up (mean, 37 months) showed recurrent Canadian Cardiovascular Society (CCVS) class III/IV angina in 30.1% of UAP and in 25.2% of SAP patients (p less than 0.05). There was a return to work in 86% of UAP and in 91% of SAP patients (p less than 0.05). When UAP patients' durations of symptoms were further fractionated, it was found that the earlier angioplasty was performed after onset of angina, the lower was the success rate and the higher the complication rate and incidence of late follow-up untoward events. When coronary angioplasty was performed within 1 week of onset of angina ("early"), success was 79.1%; when angioplasty was performed 2 weeks or more after onset of angina ("later"), success was 86.3%. Major cardiac events occurred in 11.5% in the early group and in 4.8% in the later group (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

The surgical management of a case of severe scrotal elephantiasis is presented. Excellent functional and esthetic results were achieved by excision of the involved scrotal tissue, and a transverse closure.

A 9 1/2-year-old boy developed a progressive dyskinesia 7 years after an angiographically demonstrated internal cerebral vein thrombosis. CT revealed bilateral thalamic lesions, more severe on the side contralateral to the movement disorder, without striatal involvement. This is a rare example of survival after internal cerebral vein thrombosis in a child and demonstrates that movement disorders may be delayed consequences of childhood stroke.
