Objectives: The Pediatric Emergency Ruler (PaedER) is a height-based drug dose recommendation tool that was reported to reduce life-threatening medication errors by 90%. The PaedER was introduced into the Cologne Emergency Medical Service (EMS) in 2008 along with educational measures, publications, and lectures for pediatric drug safety. We reviewed the impact of these continuously ongoing measures on medication errors after 10 years., Methods: The PaedER was introduced and distributed to all 14 emergency ambulances and 2 helicopters staffed with emergency physicians in the city of Cologne in November 2008. Electronic records and medical protocols of the Cologne EMS over two 20-month periods from March 2007 to October 2008 and March 2018 to October 2019 data sets were retrieved. The administered doses of either intravenous, intraosseous, intranasal, or buccal fentanyl, midazolam, ketamine, or epinephrine were recorded. Primary outcome measure was the rate of severe drug dosing errors with a deviation from the recommended dose of greater than 300%., Results: A total of 59 and 443 drug administrations were analyzed for 2007/08 and 2018/19, respectively. The overall rate of drug dosing errors decreased from 22.0% to 9.9% (P = 0.014; relative risk reduction, 55%). Four of 5 severe dosing errors for epinephrine were avoided (P < 0.021; relative risk reduction, 78%). Documentation of patient's weight increased from 3.2% in 2007/08 to 30.5% in 2018/19 (P < 0.001)., Conclusions: The distribution of the PaedER combined by educational measures significantly reduced the rates of life-threatening medication errors in a large EMS. Abstract: This study was created to assess whether a 30-mm depth of chest compression (CC) is sufficient and safe for neonatal cardiopulmonary resuscitation.This retrospective analysis was performed with chest computed tomography scans of neonates in 2 hospitals between 2004 and 2018. We measured several chest parameters and calculated heart compression fraction (HCF) using the ejection fraction formula. We evaluated whether one-third of the external anterior-posterior (AP) diameter and HCF with them are the equivalent to 25-, 30-, 35 mm and HCF with them, respectively, and the number of individuals with over-compression (internal chest AP diameter - compressed depth <10 mm) to estimate a safe CC depth. We divided the patients into term and preterm groups and compared their outcomes.In total, 63 of the 75 included individuals were analyzed, and one-third of the external lengths was equivalent to 30 ± 3 mm (P < .001). When the patients were divided into term (n = 53) and preterm (n = 10) groups, the equivalent depth was 30 ± 3 mm in the term group (P < .001) and 25 ± 2.5 mm in the preterm group (P = .004). The HCF with 30 mm was equivalent to that for one-third of the external length (P < .001). When we simulated CCs with a 30-mm depth, over-compression occurred more frequently in the preterm group (20%) compared to the term group (1.9%) (P = .014).A 30-mm depth could be appropriate for sufficient and safe neonatal resuscitation. Objective: To compare the effect of delayed cord clamping on cord blood gas values in vaginally delivered, healthy, term singletons., Data Source: We used MEDLINE, CINAHL, CENTRAL, EMBASE, and ClinicalTrials.gov databases., Methods of Study Selection: Eligible studies included randomized controlled trials (RCTs) comparing cord blood gas values obtained from early compared with delayed cord clamping groups and observational studies using serial cord blood gas from the same umbilical cord. We described the difference in means of cord blood gas parameters and comparative descriptive statistics when a difference in means was not available. We used a domain-based risk bias tool to extract methodologic details and assess potential risk of bias., Tabulation, Integration, and Results: This review included two RCTs and three observational studies. These studies included a total of 234 newborns with early cord clamping and 218 newborns with delayed cord clamping. The observational studies showed that 45-90 seconds delayed cord clamping was associated with mean decreases in umbilical arterial pH (0.02-0.03), HCO3 (0.3-0.8 mmol/L) and increases in base deficit (0.3-1.3 mmol/L) compared with early cord clamping. One observational study showed that delayed cord clamping was associated with decreases in umbilical venous pH (0.01) and HCO3 (0.2 mmol/L) and increase in venous base deficit (0.1-0.3 mmol/L) compared with early cord clamping. These changes were not observed in the two RCTs., Conclusion: Delayed cord clamping up to 120 seconds has either no effect or only a small effect on cord blood acid-base balance; overall, the magnitude of these changes is not clinically significant in vaginally delivered, healthy, term singletons., Systematic Review Registration: PROSPERO, CRD42019135779.
