Consumptive thirst is widely discussed in Huangdi Neijing and it is classified as a " strange disease" in Suwen Strange Diseases Treatise, which reflects the intractable nature of consumptive thirst. This paper explores and analyzes consumptive thirst based on the core concept in Huangdi Neijing. First, this paper approaches the subject through the use of image thinking method from Huangdi Neijing, recognizing that consumptive thirst can lead to changes in the internal climate of the human body. Then, guided by the theories of essence and qi, yin and yang, and the five elements in Huangdi Neijing, it deconstructs and analyzes the causes of these changes and the laws of qi transformation. It points out that the changing climate of consumptive thirst is characterized by "heat symptoms". The main cause of "heat symptoms" is spleen deficiency and excessive dampness, and its qi transformation law is the heat transformation of Shaoyin. The intractable nature of consumptive thirst is mainly reflected in the uncontrolled "heat symptoms" caused by the imbalance of the five elements. Based on the understanding of the heat symptoms of consumptive thirst, this paper proposes a treatment strategy for preventing the disease by resolving dampness and regulating the spleen, harmonizing kidney qi to prevent progression, and balancing yin and yang to treat chronic and recalcitrant conditions. The basic diagnostic and treatment principles of traditional Chinese medicine are the pattern-differentiation-deduced pathogenesis and treatment based on examination of the pathogenesis. Shanghan Lun established a system of pattern differentiation of six meridians, systematically summarized the evolution laws of exogenous febrile disease, and discussed miscellaneous diseases as exogenous diseases. It contains 112 classic formulas and advocates that "those whose disease corresponds to the formulas should take them accordingly", combining disease differentiation and pattern differentiation, containing not only the knowledge of treatment based on pattern identification, but also the profundity of pattern-differentiation-deduced pathogenesis. Based on the original meaning of "pathogenesis", from the three angles of "subtle meaning", "core meaning", and "appropriate meaning", this paper exemplifies the concept of pattern differentiation for six meridians in Shanghan Lun, analyzes the relationship and connotation of formula patterns of classical formulas in Shanghan Lun by "taking pattern with formulas", and explores the concept of pattern-differentiation-deduced pathogenesis in Shanghan Lun. This paper emphasizes that during the clinical practice of traditional Chinese medicine, it is important (i) to deduce the pathogenesis based on pattern differentiation, understand symptoms upon first onset, and deduce the pathogenesis before symptoms appear, thereby treating the disease at an early stage; (ii) to identify the most important concepts, solve complications with simple method, discern the core of the disease, and treat the entire body; and (iii) to optimize timing, act quickly, take advantage of opportunities, accept outcomes, and provide judicious guidance to patients. This paper serves to interpret Shanghan Lun based on pattern-differentiation-deduced pathological mechanism and provides ideas and inspiration for applying the theory of Shanghan Lun to guide clinical practice. Cancer therapy-related thrombopenia, which is called "medicinal poison purpura" in traditional Chinese medicine, is a common hematologic adverse reaction during oncology treatment that is difficult to treat due to the differences in oncology treatments and the complexity of the pathogenesis, resulting in various degrees of thrombocytopenia. Based on the theory that " spleen controlling blood", this paper believes that "medicinal poison purpura" is mainly caused by direct damage to the blood and qi by medicinal poison, leading to qi and blood deficiency; it also attacks the spleen and stomach, resulting in the deficiency of spleen qi and no source of qi and blood production. Due to the spleen deficiency, there is no essence to nourish kidney and bone marrow; their function of generating blood decreases, eventually it becomes " medicinal poison purpura". The theory of "regulating balance and flat regulation" is an important academic idea of our team in the treatment of malignant hematological tumors. In this paper, we have systematically elaborated on the etiology, pathogenesis, and therapeutic principles of the treatment of cancer therapy-related thrombopenia with spleen deficiency pattern through the collation of relevant literature. We believe that the prescription formulated according to the method of invigorating spleen and benefiting qi and controlling blood for the treatment of cancer therapy-associated thrombocytopenia with spleen deficiency pattern is in line with the principle of correspondence between prescription and syndrome, and correspondence between drugs and syndrome in traditional Chinese medicine, which is theoretically feasible and has a high clinical application value. Diabetic kidney disease (DKD) is a severe complication of diabetes. Its incidence increases annually, posing a significant burden on public health. The strategy of symptomatic treatment based on pathogenesis differentiation, focusing on identifying pathogenesis, is particularly meaningful for managing complex and variable chronic diseases like DKD. Within this framework, the state of latent heat persists throughout DKD, with "latent heat causing accumulation" identified as the core pathogenesis affecting and promoting the development and progression of DKD. This paper is centered on the concept of "latent heat causing accumulation" and adopts symptomatic treatment based on pathogenesis differentiation as its guiding principle to explore the role of latent heat in DKD. It highlights that the onset of DKD involves environmental and constitutional pathogenesis associated with "the concealment of latent heat" and "stagnant-heat invading collaterals" as the initial pathogenesis," latent heat causing accumulation" as the core mechanism, and" secondary turbid heat" as the derivative pathogenesis. These pathogenesis factors collectively influence the symptoms, sequelae, and prognosis of DKD. Moreover, this paper provides commonly used prescriptions for different stages, syndrome types, and complications of the disease, aiming to offer a reference for clinical practice in flexibly addressing changes in disease conditions based on varied pathogenesis. The correspondence between prescription and syndrome is the advantage and characteristic of traditional Chinese medicine (TCM) treatment. However, the pathogenesis of clinical diseases is complex and the condition is changeable, and the clinical application is difficult to achieve the maximum effect under the existing cognition of the correspondence between prescription and syndrome. In this paper, the five categories of physiological characteristics, pathological characteristics, constitution, syndrome, and disease of the longitudinal classification of "TCM state" are introduced into the correspondence of prescription and syndrome. Under the vertical perspective of "TCM state", the theoretical connotation of the correspondence between prescription and syndrome is interpreted as "correspondence between prescription and state", namely correspondence of "prescription-physiological characteristics", correspondence of "prescription-pathological characteristics", correspondence of "prescription-constitution", correspondence of "prescription-syndrome", and correspondence of "prescription-disease". Based on the theory of "there is a reason but no death" in traditional Chinese medicine, this paper believes that the extension of "there is a reason but no death" is "state" under the perspective of "state of traditional Chinese medicine". In order to reduce the adverse reactions of traditional Chinese medicine in clinic, the macroscopic, mesoscopic and microscopic perspectives should be used together, and the macroscopic, mesoscopic and microscopic parameters should be integrated. The "state" of human physiological characteristics, pathological characteristics, constitution, syndrome and disease should be carefully identified by qualitative and quantitative methods, and the connection between prescription and "state" should be highlighted. If the prescription corresponds to "state", the harm of adverse reactions of traditional Chinese medicine will be reduced. This paper hopes to focus on the guiding role of the theory of "there is a reason but no death" in the research of traditional Chinese medicine, give play to the unique advantages of traditional Chinese medicine, and look at the role of traditional Chinese medicine dialectically. Based on the "You Gu Wu Yun" theory in traditional Chinese medicine (TCM), this paper believes that "Gu" in "You Gu Wu Yun" is extended to "state" from the perspective of "TCM state". In order to avoid the adverse reactions of TCM, the macro, meso, and micro three views should be used together, and macro, meso, and micro parameters should be integrated. We should also carefully identify the physiological characteristics, pathological characteristics, constitution, syndrome, and disease of human body by combining qualitative and quantitative method, highlighting the relationship between the prescription and the "state". The correspondence between prescription and the "state" will reduce the risk of adverse reactions of TCM. Primary hypotension has the characteristics of insidious onset, lingering condition, complex etiology, and common symptoms. Its location is in the blood vessels, and the pathogenesis is mainly deficiency, and the lesions involve the internal organs of the whole body. The theory of "Yang transforms into Qi, Yin forms" comes from the Yellow Emperor's Classic of Internal Medicine, which summarizes the basic laws of the movement and change of Yin and Yang. Based on this, this paper proposes that the basic pathogenesis of primary hypotension lies in insufficient Yang transforming into Qi and abnormal Yin formation. Its essence lies in the overall Yang deficiency that loses its warming and Qi deficiency that has no power to control, causing blood circulation weakness and insufficient protection, which in turn leads to abnormal Yin formation, including internal organ deficiency, obstruction of the formation of fine substances, and accumulation and retention of tangible pathological products such as phlegm and blood stasis. The clinical treatment should be based on the method of replenishing Qi and raising Yang: replenishing Qi to provide power for blood circulation; raising Yang to provide the source of Qi; paying attention to protecting the internal organs and improving the internal environment of Yang deficiency and Yin excess; removing Yin evils such as phlegm and dampness stasis, and unblocking the channels for the circulation of Qi and blood. Applying the theory of "Yang transforms into Qi, Yin transforms into shape" to guide the treatment of primary hypotension will help to gain a deeper understanding of the pathogenesis of the disease and broaden the ideas of clinical diagnosis and treatment. Primary hypotension is a common and frequently occurring blood vessel disease, characterized by insidious onset, lingering condition, complex etiology, and diverse symptoms. Its pathogenesis is primarily characterized by deficiency, and its onset involves multiple zang-fu organs. The "yang transforming qi and yin shaping up body" theory is derived from the Huangdi Neijing . This theory summarizes the fundamental laws of the movement and changes in yin and yang. From this perspective, the paper hypothesizes that the basic pathogenesis of primary hypotension stems from an insufficiency of yang in transforming qi and a malfunction of yin in shaping up body. The essence of the condition lies in an overall yang deficiency that fails to provide warmth and transformation, coupled with qi deficiency, which cannot control and regulate bodily functions. This leads to poor blood circulation and inadequate protection, causing yin to malfunction in forming the physical components of the body. As a result, normal physiological functions of the zang-fu organs are disrupted, the production of beneficial physiological substances is obstructed, and metabolic waste products accumu late. The primary therapeutic approach should be focused on tonifying qi and ascending yang; tonifying qi will provide the impetus for blood circulation, whereas ascending yang will facilitate yang transformation qi. The zang-fu organs should also be protected and nourished to ameliorate the internal environment of yang deficiency and yin excess, as well as to eliminate pathological product accumulation to dredge the channels of blood circulation. Using the " yang transforming qi and yin shaping up the body" theory to guide the treatment of primary hypotension can help deepen the understanding of the pathogenesis of this disease and broaden the clinical diagnosis and treatment strategies. Gastroparesis is a gastric disease characterized by delayed gastric emptying. "Vomiting" is the main clinical manifestation of this disease, while "atrophy" indicates dysfunction of the stomach. Based on the theory of "atrophy, dyspnea and vomiting are ascribed to the upper part", originating from the Chapter of "Zhi Zhen Yao Da Lun" from the Suwen(Basic Questions) section this paper explored the pathogenesis and treatment idea of gastroparesis from the function of the lung. It is put forward that failure of lung qi in dispersion, so the lung qi cannot maintain the circulation of harmonizing qi and blood, leading to malnutrition of stomach and loss of stomach receiving food and drink function. Melancholy impairing lung, so lung cannot govern diffusion, leading to stomach governing the disfunction of dredging and descending. Impaired depurative descending of lung qi, so the large intestine and stomach are lost in the alternating operation of deficiency and excess, leading to stomach governing the disfunction of transportation and transformation. Therefore, the treatment approach is proposed to improve the stomach's function to receive food and drink, transport and transform and promote the recovery of gastric motility by replenishing and restoring lung qi, regulating emotions and catharsis and smoothing lung. Analyzing the effect of function of the lung on gastric motility is of great value for expanding the application of traditional Chinese medicine in gastrointestinal motility disorders, and digging new connotations of classic theories in modern clinical practice. Epilepsy is a disease of the central nervous system caused by excessive neuronal discharges in the brain, characterized by sudden, recurrent and self-limited onset. The brain's qi collateral and the brain neural network are highly correlated and internally consistent in terms of structure and function. The theory of " brain's qi collateral-abnormal collateral", which is centered on the structural disorder and dysfunction of brain's qi collateral leading to the poor circulation of brain's qi collateral, can comprehensively explain the related pathogenesis of epilepsy and the law of disease evolution, so it has important clinical value. Taking the pathogenic characteristics as an entry point and based on the theory of " brain's qi collateral-abnormal collateral", this paper argues that phlegm and qi stagnation, wind in the brain's qi collateral, and phlegm and blood stagnation damaging the brain's collaterals, as well as the structural and functional characteristics of brain's qi collateral that circulate bi-directionally are the key factors for epilepsy to present sudden, recurrent, and self-limited characteristics. According to the therapeutic principle of " Collaterals need to be unobstructed to function normally", it is proposed that the method of regulating the qi and collaterals should be used as the basic treatment principle throughout the treatment. In addition, the method of resolving phlegm and eliminating blood stasis is supplemented for different pathological changes, while combining the syndrome differentiation of zang-fu viscera and attaching importance to the accompanying symptoms of epileptic seizures, to regulate the brain's qi collateral to achieve the effects of wind quenching and epileptic arrest. Migraine is a chronic episodic neurovascular disease with a distinct "episode-remission" feature. According to the theory of traditional Chinese medicine, migraine belongs to the class of diseases including "Toutong" and "Toufeng". Disorder of qi and blood, especially blood stasis due to qi deficiency, lie at the basis of migraine pathogenesis. The physiological function of the neurovascular unit is strongly related to qi and blood. Focusing on the neurovascular unit, this paper expounds the physiological basis of the theory of qi and blood from the perspective of microenvironment homeostatic disorder and the interaction of various cell components, and discusses the scientific connotation of the pathogenesis of qi and blood in migraine. Disordered homeostasis of the neurovascular unit is involved in the pathology of migraine and tallies with the pathogenesis of qi and blood in migraine, which is mainly reflected in dysfunction of astringing, defending, and transforming due to qi deficiency, as well as pathogenic toxin accumulation and blood stasis. The explanation based on homeostasis of the neurovascular unit due to qi and blood pathogenesis in migraine is helpful to understand the development of migraine integrally and to provide guidance for traditional Chinese medicine treatment and research of migraine from the theory of qi and blood. Yi He Rehabilitation, which is based on traditional Chinese rehabilitation treatments and rehabilitation principles from modern medicine, is effective in staging treatment of post-stroke hemiplegia. This paper systematically discusses the origin and annotation of Yi and He from the perspectives of traditional Chinese medicine and modern medicine. The medicinal connection between Yi and the body and its function is related to unobstructed attunement. Based on the connotation of Yi and He, we believe that the pathogenesis of post-stroke hemiplegia is the comprehensive result of abnormal effects of Yi on the organism at the microscopic level and abnormal effects of He on function at the macroscopic level, featured as tense muscle movement and a pathological process of abnormal motion, including the disturbance of yang qi with body dysfunction, the disorder of spirit with sinews and vessels with diversion, and the variation of brain collateral with physical and mental inconsistency. By inducing relaxation and calmness, Yi He rehabilitation takes effect in staging treatment of post-stroke hemiplegia with characteristic mechanisms. First, by calming ascending yang and relaxing the disordered body in periods of relaxation, it can achieve the maintenance function of the kinematic chain peripherally with passive rehabilitation. Second, by calming the disordered spirit and relaxing the inhibited meridian sinews in spasmodic periods, it can reconstruct the neural plasticity of motor function centrally with assistive rehabilitation. Third, by calming damaged brain collateral and relaxing the impassable zang organs in the recovery period, it can close the central-peripheral-central loop of rehabilitation with active rehabilitation. The theory of "lung governing skin and hair" has always occupied an important position in the clinical practice of traditional Chinese medicine. However, in western medicine, the close correlation between the lungs and the skin in health and disease has not been established. The difference between these two medical views has triggered an urgent need for the scientific interpretation and clinical application of the theory of "lung governing skin and hair" in western medicine. Therefore, the purpose of this paper is to discuss the application of "lung governing skin and hair" in chinese medicine and the correlation between the lung and the skin in western medicine from the connotation and history of " lung governing skin and hair" in Chinese medicine. In addition, we will discuss the common pathologic biomarkers of lung and skin diseases in western medicine and the co-morbidities between lung and skin, with the aim to provide new ideas for the clinical diagnosis and treatment of lung and skin diseases and modern research, as well as providing a direction for the integration of the theory of "lung governing skin and hair" into the clinical practice of western medicine. Atherosclerosis is classified as "vessel bi" in traditional Chinese medicine, and cold evil is one of its important causes. Cold, having the properties of coagulation, stagnation, and contraction, tends to narrow the diameter of blood vessels and slow down blood flow, which can promote the pathophysiological process of atherosclerosis. At present, the etiological theory of vessel bi caused by cold evil has not been fully elaborated. Based on the explanations of " evening" in "pathogenic cold affecting in the evening", such as "ying", "end of year", "end of day", and "time of yang decline and yin exuberance", this paper proposes the viewpoint that the cold evil tends to affect ying-blood and cause vessel bi, and discusses its pathogenesis. From the perspective of the external environment, cold evil affecting the vessel is conditional, often occurring in winter and in the middle of the night. From the perspective of internal environment, the cold evil affects the vessel in a process which can be manifested in three stages: cold injury, cold stagnation, and cold congelation. The strength of yang qi is the factor that determine whether the cold evil can cause disease. Exploring the etiology and pathogenesis of atherosclerosis from the perspective of "pathogenic cold affecting in the evening" provides a new idea for further elucidating the pathogenesis of the disease and expanding its clinical treatment. The relationship between intestinal flora and diseases is currently a hot topic of research, but most studies tend to explain causality and pathogenesis, and there is still a lack of studies of the traditional Chinese medicine (TCM) characteristics of intestinal microecological disorders from the perspective of etiology. The TCM pathogenic characteristics of hybrid flora need to be further clarified. The intestinal flora is an important bridge between the internal and the external environment of body, and plays an important and complex role in the diagnosis, prognosis, and treatment of diseases. Based on the etiology theory of TCM, this paper discusses the scientific connotation of changes of wind, fire, dryness, dampness, and cold caused by intestinal microecological disorder from the aspects of basic concepts, formation causes, and pathogenic nature and characteristics, combined with the nervous system, immune system, gland secretion, and nutritional metabolism, based on clinical manifestations, using the method of examining symptoms and seeking causes. This paper provides a new idea for the study of intestinal flora in TCM, and supplements and enriches the theory of etiology. At the same time, it provides an innovative theoretical basis for the understanding and research that intestinal microecological disorders can promote the occurrence and development of various complex diseases. Revealing the rules of traditional Chinese medicine (TCM) syndrome differentiation thinking is an important part of researching the original thinking in TCM, which involves brain cognition and neural activity principles. However, traditional research on syndrome differentiation lacks exploration of the cognitive mechanisms of TCM practitioners during syndrome differentiation. In this context of cognitive science, this paper proposes a new paradigm for studying the cognitive neural mechanisms of TCM syndrome differentiation thinking. Firstly, the paper discusses the cognitive shift in syndrome differentiation research and then points out that working memory may be an important window reflecting TCM syndrome differentiation thinking. Based on this, we propose a working hypothesis for the cognitive neural mechanisms of TCM's yin-yang syndrome differentiation and design relevant cognitive neuroscience experimental protocols to explore how the brain engages in yin-yang syndrome differentiation thinking using techniques such as event-related potential and functional magnetic resonance imaging. The implementation of this research pathway can clarify the intrinsic cognitive neural mechanisms of syndrome differentiation thinking, provide evaluation indicators for TCM syndrome differentiation thinking from cognitive neuroscience, and provide cognitive principles to support the cultivation of syndrome differentiation ability of TCM talents. Acquired cerebral amyloid angiopathy, as a cerebrovascular disease discovered in recent years, mainly spreads through iatrogenic factors. This paper focuses on acquired cerebral amyloid angiopathy and delves into the new applications and profound implications of the "toxins damaging brain collaterals "theory proposed by academician WANG Yongyan in stroke research. Starting from the perspective of "toxins damaging brain collaterals", it comprehensively analyzes the causes and pathological mechanisms of acquired cerebral amyloid angiopathy, emphasizing the importance of external toxins, latent toxins, and turbid toxins in understanding the pathogenesis of the disease. Dampness is one of the important external pathogens in Chinese medicine, and with the changes in peoples living habits, dampness syndrome is becoming more and more common. Due to the different characteristics and functions of different parts of the body, the pathogenesis of dampness is not the same when it attacks, so it is helpful to examine it before clinical treatment. Dampness forms through inner unbalance or invades through external environment. This paper discusses the clinical manifestations of dampness in various parts of the body and highlights the similarities and differences in the pathogenic mechanisms of dampness, taking into account the pathogenic characteristics of dampness as a clue for the external dampness invasion pathway from the surface to the inside. In addition, this paper also explains the disease mechanism of dampness syndrome in the context of western medical research, so as to provide research evidence for the integration of Chinese and western medicine and to improve medical treatment in clinical practice. The "pre-disease" theory of traditional Chinese medicine focuses on the dynamic and continuous evolution from health to disease, and emphasizes early identification and intervention in the complex and gradual process of evolution from health to disease. The "pre-disease" theory and complexity science share the same perspective on health and disease from the standpoint of features of the dynamic evolution and holism, i. e., life is considered as a complex system with ongoing dynamic changes, which exhibit the nonlinear features of "homeostasis-destabilization-phase transition-another homeostasis". In this paper, from the perspective of nonlinear dynamics in complexity science, we explain the scientific connotation of the evolution law of "pre-disease" -disease based on the theory of critical slowing down in traditional Chinese medicine. Based on the theory of critical slowing down and the dynamic network biomarker method generated by its development, combined with the macro signs of comprehensive analysis of data gained by four diagnostic method and the micro features including transcriptomics and the microbiomics, this paper proposes to integrate macro and micro multi-hierarchy information to construct a "pre-disease" critical slowing down identification model with the characteristics of traditional Chinese medicine diagnosis and treatment, which provides a new perspective and method for the early warning of complex diseases. Our team created the pathogenesis theory of cancer toxin in traditional Chinese medicine on the basis of inheriting the academic thought of "cancer toxins" of ZHOU Zhongying, a Chinese medical master.The pathogenesis theory of cancer toxin suggests that cancer toxins is the key factor leading to the occurrence and development of malignant tumors, the basic pathogenesis of malignant tumor is accumulation of evil and toxins, deficiency of vital qi. This paper proposes that the main pathological factors of carcinoma of prostate are deficiency, dampness, heat, stasis and toxins. The core pathogenesis was spleen and kidney deficiency, dampness-heat stasis toxin accumulation in essence chamber. The disease is located in the essence chamber, closely related to kidney and bladder, and involves liver and spleen. Clinical treatment is based on anti-cancer and detoxification, strengthening vital qi to eliminate pathogenic factor as the basic treatment principles, treatment with anti-cancer detoxification as the core, tonifying the spleen and kidney as the fundamental, clearing heat and removing dampness, removing blood stasis and dispersing is key, accompanied by dispersing liver and regulating qi, the whole syndrome differentiation, to maintain a stable period of time. Strengthening vital qi does not leave evil, eliminating evil does not harm vital qi. Taking "harmony of body and spirit" as the theoretical core in Huangdi Neijing and the functional nature and executive structure of the five emotions system as clues, this paper explores and analyzes the relevant theories of the five emotions system in ancient Chinese medicine literature. It is found that the five emotions system is a highly ordered and integrated collection of five functional states. It often resides in the five zang viscera, it operates between the meridians and the xuanfu through the elevation and movement of the qi, and it is the pivotal point for the operation of the qi and blood of the organism, the interactive conduction of information, and the unity of form, emotion, and qi. Therefore, this paper starts from the point of view of system science. It is proposed that the primordial spirit exercises control over the five emotions and the five emotions provide nutrition to the primordial spirit, and that the two are not "islands of information" but have a dynamic and hierarchical internal connection. The executive structure of the five emotions system, namely, the meridians, the xuanfu, and the five zang viscera, is the pivot, and it is recognized that the operation of the five emotions system is a process with self-organizing characteristics and orderly and eternal motion. We think of the qi, blood, ying, and essence as the basis of occurrence, the meridian as the execution channel, and the xuanfu as the place for the spiritual mechanism to enter and exit; the five zang viscera serve as a dwelling place, and ministerial fire serves as the source of energy. Diabetic lower extremity vascular disease is one of the common chronic complications of diabetes, which has a strong disabling effect, clinically, it is of great significance to actively and effectively control the progression of the disease and improve the quality of life of patients. In Sanxiao Lun, LIU Wansu put forward the theory of "dryness-heat and stagnation" according to the pathogenesis of consumptive thirst, and thought that the dryness-heat and stagnation in the stomach and intestines of sanjiao were too severe, the xuanfu was blocked, and the qi and liquid failed to diffuse, which caused diabetes. Vascular endothelial injury and inflammatory reaction are important factors in the development of vascular disease, the pathogenesis evolution of "dryness-heat and stagnation" conforms to the pathological characteristics of inflammatory reaction in western medicine, and the dryness-heat runs through the diabetic lower extremity vascular disease throughout. In this paper, it is believed that diabetic lower extremity vascular disease belongs to the category of "flaccidity and paralysis" in traditional Chinese medicine, and lung scorched by heat is the cause of flaccidity. "Dryness-heat and stagnation" is the source of invading heat (dryness-heat), lung scorched by heat, pulse failure and vein stasis occurred, which causes diabetic lower extremity vascular disease. Treatment should be based on flaccidity, combined with the viewpoint of LIU Wansu's Sanxiao Lun, taking yangming as the top priority, regulating the triple diabetes, solving the problem of dryness-heat and stagnation, combined with the method of purging the south and supplementing the north, so that the invading heat is cleared, lung heat is diminished, and flaccidity disease cannot occur, in order to provide new ideas for the clinical prevention and treatment of diabetic lower extremity vascular disease. The initial step taken by the author involves an analysis of the merits and demerits associated with the theoretical paradigm of traditional acupuncture and moxibustion, specifically referred to as the macro and humanistic paradigm. By adopting a historical materialism and dialectical perspective, the author elucidates how the theoretical paradigm of traditional acupuncture and moxibustion is constrained by historical and technological circumstances. The proposition suggests that in response to the evolving landscape of science and technology across temporal and spatial dimensions, there is a need to shift and augment the traditional acupuncture and moxibustion paradigm. Specifically, this entails transitioning towards the micro and natural science paradigm. The subsequent analysis focused on the issues present within the current paradigm of acupuncture and moxibustion clinical research. It was posited that while the initial objective is to validate the clinical efficacy of acupuncture and moxibustion, the ultimate aim is to establish the instructive significance of traditional acupuncture and moxibustion theory. Moreover, the absence of novel theories and principles capable of guiding the modern scientific innovation in acupuncture and moxibustion clinical practice was identified. Finally, this paper presents the issues and initial concepts that warrant consideration in future clinical research on acupuncture and moxibustion within the context of a paradigm shift. Specifically, emphasis should be placed on investigating the theory and principles of acupuncture and moxibustion in light of the contemporary scientific understanding of human tissue structure and function. For instance, within the theoretical paradigm of local diagnosis and treatment, it is imperative to direct our attention towards the distinctive attributes and principles governing acupuncture therapy at various tissue levels on the external surface of the body. Similarly, within the remote and holistic diagnosis and treatment paradigm, it is crucial to thoroughly examine the instructive implications and pertinent characteristic principles associated with the neural network, fiber network, and vascular network in the context of clinical acupuncture and moxibustion. ZHU Danxi and ZHANG Congzheng were representative doctors from the Jin-Yuan dynasties. They belonged to the Hejian School, but they had notable differences in the order of reinforcing healthy qi and eliminating pathogenic factors. In the classic medical case " ZHU Danxi's treatment of dysentery", ZHU Danxi adopted the treatment method of " First treating its deficiency and then treating its excess" for patients with excess pathogen, which was contrary to ZHANG Congzheng's claim that " Treatment should first eliminate the pathogenic factor". By systematically analyzing the similarities and differences between the two methods of eliminating pathogenic factors, it was found that both used the three method of sweat promotion, emetic therapy, and purgation, and both paid attention to avoiding the harmful effects of eliminating pathogenic factors on qi of spleen and stomach. However, in the order of reinforcement and elimination, ZHU Danxi emphasized that " The pathogenic factor should be carefully eliminated, and healthy qi should be protected", while ZHANG Congzheng thought that " The healthy qi should not be tonified before the pathogen is eliminated". In the order of treating the spleen and the stomach, ZHU Danxi often supplemented the stomach qi and then eliminated pathogenic factors in patients with spleen-stomach weakness, while ZHANG Congzheng thought that the pathogenic factor should be eliminated first and then stomach qi should be restored. Taking classic medical records as an example, this paper discusses the reasons for the different method between the two doctors from the viewpoints of their ideological origin, the succession of teaching from a master to his disciples, social background, and geographical environment, so as to provide a basis for further exploring the method of learning about medical records and the perspective of understanding medical records. Acupoint sensitization is a breakthrough new theory in the field of acupuncture in the past 20 years. It has important value in enriching the theory of meridians and acupoints and improving the level of clinical diagnosis and treatment of acupuncture. At present, the number of studies on the exploration of surface sensitization points for different diseases is on the rise, but there is still a lack of systematic sorting and summary of the exploration mode of surface sensitization points. According to the characteristics of the openness of the research purpose and the accuracy and variability of the target range in the process of surface sensitization point exploration, this paper divides the exploration mode of surface sensitization points into extensive exploration mode and precise exploration mode. There are obvious differences between the two in exploration efficiency and result presentation. The former can be subdivided into holistic extensive exploration mode and local extensive exploration mode, while the latter can be subdivided into fixed precise exploration mode and variable precise exploration mode. By deeply analyzing the applicable scenarios, characteristics and advantages, existing problems of each exploration mode and proposing some opinions on mode selection, mode innovation and future development direction, it aims to provide useful thinking and inspiration for the subsequent research on the exploration of surface sensitization points of various diseases. The acupoint sensitization theory, a breakthrough in acupuncture and moxibustion over the past two decades, has greatly enhanced the understanding of meridians and acupoints while increasing the effectiveness of clinical diagnosis and treatment in acupuncture and moxibustion practices. The number of studies on surface sensitization site detection for different diseases is increasing. However, systematic sorting and a summary of the detection mode of surface sensitization points are lacking. Therefore, this study categorizes the method of surface sensitization point detection into two modes: pan-scanning and focused scanning. The classification is based on the characteristics of the openness of the research purpose, the precision of the target range, and the degree of variation during detection. The two modes have considerable differences in the exploration efficiency and the presentation of result. The pan-scanning mode can be further subdivided into holistic and localized pan-scanning modes, whereas the focused-scanning mode can be subdivided into fixed- and variable-focused scanning modes. This study analyzes the application scenarios, characteristics, advantages, and limitations of each detection mode and presents opinions on mode selection, mode innovation, and future development directions. Allergic constitution is a common biased physical state. This article discusses allergic constitution based on the basic principles of TCM constitution - life process theory, endowment inheritance theory, form and spirit composition theory, and environmental constraint theory. From the perspective of life process theory, at different ages, allergic constitution may change with the changes in the rise and fall of qi and blood in the body. The overall trend of qi and blood in the body changes from immature to strong and then weak. From the perspective of endowment inheritance theory, allergic constitution is closely related to heredity and has a tendency to gather in families. From the perspective of form and spirit composition theory, people with allergic constitution often have physical characteristics of symptoms such as nose, airway reaction, intestines, and skin, and have psychological characteristics of introverted personality and prone to anxiety, depression, tension and other emotions. From the perspective of environmental constraint theory, the formation of allergic constitution is related to differences in seasons and geographical environment. By exploring the characteristics of allergic constitution and deeply understanding allergic constitution, it provides a reference for better grasping allergic constitution and improving the quality of life of people with allergic constitution. Human beings are born with the Qi of heaven and earth, and the physiological and pathological functions of the human body are affected by the alternation of cold and heat in the four seasons. Epidemiology shows that allergic diseases often occur or worsen when the seasons change, which corresponds to the theory of "the spleen governs the end of the four seasons" in traditional Chinese medicine. Spleen dysfunction causes the body's time self-stabilization system to be destroyed when the seasons change, making it unable to adapt to external climate changes, which is the internal cause of triggering or aggravating certain allergic symptoms. The theory of "the spleen as the guardian" originated from the Yellow Emperor's Classic of Internal Medicine. It is a high-level summary of the spleen's function of defending against external evil and protecting the body. It emphasizes that the spleen transports water and grain internally to replenish the positive Qi and regulates the Wei Qi externally to protect the whole body. It is closely related to the seasonal changes in the human immune function, thus affecting the seasonal onset of allergic diseases. This article explores the time self-stabilization and immune defense functions of the spleen in the seasonal onset of allergic diseases from the perspective of "the spleen as the guardian". It proposes that the dysfunction of "the spleen as the guardian" leads to the imbalance of Ying and Wei, the endogenous generation of phlegm and dampness, and the malnutrition of the five internal organs, which are important reasons for the repeated onset and protracted healing of allergic diseases when the seasons change, providing ideas and inspiration for exploring the mechanism and prevention and treatment of seasonal onset of allergic diseases. Humans are born under the nourishment of heaven and earth qi. The physiological and pathological functions of the human body are influenced by the alternation of cold and heat in the four seasons. Allergic diseases often occur or worsened during seasonal transitions, corresponding to the theory of the spleen dominating the end of the four seasons in traditional Chinese medicine. Splenic dysfunction disrupts the body's internal clock during seasonal transitions, making it unable to adapt to external climate changes, causing the onset or exacerbation of specific allergic symptoms. The theory of "the spleen serves as the guard" originated from Huangdi Neijing, which is a succinct summary of the role of the spleen in defending the body against external pathogens. The theory emphasizes that the spleen transports drinks and foods internally to supplement healthy qi, and regulates defensive qi externally to protect the whole body. This theory is closely linked to the seasonal changes in immune function, thereby influencing the seasonal onset of allergic diseases. This paper explores the roles of the spleen in time-based self-regulation and immune defense against seasonal allergies diseases according to the theory of " spleen serves as the grard." The dysfunction of this protective mechanism, which leads to disharmony in nutritive qi and defensive qi, internal production of phlegm-dampness, and zang-fu organs malnutrition, are the key factors causing recurrent and persistent allergic diseases during seasonal transitions. Shanghan Lun is based on "exogenous cold disease," the change rules of a series of diseases such as superficies tightened by wind-cold and yang deficiency and internal cold, and the rules of treatment based on syndrome differentiation have had a profound impact on traditional Chinese medicine in later generations. Formulae with the name of "Sini" and its variations have the effect of treating deadly cold limbs in Shanghan Lun, for example, Sini Decoction, Tongmai Sini Decoction, Baitong Decoction, Fuling Sini Decoction, Sini Renshen Decoction, Danggui Sini Decoction, and Sini Powder, etc. Their corresponding constitution characteristics, etiology and pathogenesis, changes in disease status, and clinical manifestations have certain commonalities. This paper discusses and analyzes the modifications of the "Sini" formulae, the rules of the formulae, and the principles of application based on the characteristics of yang deficiency constitution, their susceptibility to disease, and the evolutionary pattern of yin excessiveness due to yang deficiency pathogenesis. With the pandemic of COVID-19 being under control, more and more COVID-19 patients have entered the rehabilitation stage. Traditional Chinese medicine (TCM) has always attached importance to the treatment of epidemic diseases in the rehabilitation stage so as to further promote the rehabilitation of patients for a better prognosis, and prevent the recurrence of the disease. It's focal to explore how to carry out accurate rehabilitation treatment for COVID-19 patients in the prevention and treatment of COVID-19 for quite some time to come. This paper systematically sorts out the reported clinical manifestations of the post-COVID-19 syndrome and the current guideline for the practices in COVID-19 rehabilitation. Under the background of the collaborative rehabilitation practices with TCM and western medicine, combined with the unique advantages of TCM in disease rehabilitation, this paper discusses the TCM understanding of epidemic diseases in the rehabilitation stage and its prevention and treatment ideas, and puts forward the collaborative rehabilitation strategy with TCM and western medicine for post-COVID-19 syndrome, so as to promote higher-quality rehabilitation of the patients.
