First intravascular coagula were detected as early as in the pre-acidotic phase of artificially hypoxic rabbit foetuses used by the authors in experimental studies. Growing acidosis in circulating blood has been accompanied by excessive increase of fibrin precipitation in the terminal blood track. Further increase of intravascular coagulation was found to be discontinued by predominance of secondary fibrinolysis at blood pH around 6.9. Rising hydrogen ion concentration, as a reflection of a consumptional reaction, was found to be accompanied by decline in both the fibrinogen level and thrombocyte count in the foetal blood, fully in parallel with the above DIC.

The application of the Neurolept-Analgesia to great gynaecological operations was tested in 50 Wertheim-radical-operations with removal of the glands, in comparison to a group of 50 patients whose operations were executed under the usual narcosis. There were controlled: blood-pressure, pulse, respiration, common dosing of the narcotica, dosing of the analgetica, and the patients' post-operative condition. Under NLA, the course of the operation was characterized by stability of circulation and a considerably reduced post-operative recovery, the comparative group, however, did not show so favourable results that can be proved mathematically. The application of the Neurolept-Analgesia as described thoroughly in this paper, has to be considered as especially suitable for executing gynaecological operations.

The effects of subacute intra-uterine hypoxia on the haemostatic system of 205 foetuses from 49 pregnant rabbits were studied by a standardised experimental arrangement, with particular reference being made to intravascular precipitation of fibrin. The asphyxial shock thus induced was accompanied by almost all the rabbit foetuses by disseminated intravascular coagulation by which the central organs were more strongly affected, as compared to peripheral organs also restricted by the shock. The pathophysiological processes which were found to take place in the lungs and placentas of the experimental animals proved different from those in man, which was attributable to species-related differences.

Healthy, spontaneously born infants, delivered at the calculated time shows a postnatal tachycardia combined with harrowing of oscillation. But then there is a normalisation in heart action. This phenomenon is nearly the same both in spontaneous delivery and intubation anesthesia for forceps delivery starting with propanidid. Therefore propanidid is of less risk for the neonate. But after hexobarbital anesthesia our results reveal a lingering cardial adaption of the neonate, which is statistical significant. This points to possible cardiac depressive side effects of this drug. Therefore the usage of hexobarbital anesthesia should be well considered in obstetrics.

Report on 35 patients treated by obstetric neuroleptanalgesia in the course of labour. This method doesn't appear to be qualified for general application. Nevertheless this kind of analgesia offers an additional possibility to influence the progress of labour favourably.

A retrospective evaluation was made of the blood pressures of 131 patients with hypertensive late gestosis who had undergone caesarean section for maternal or foetal indications, between 1971 and 1977. Rauwolfia preparations should be avoided in both long-term and acute hypotensive treatment of late gestosis patients not only for their unfavourable impact upon circulation at large, but also for their cumulative effects in terms of drastic depression of blood pressure in case of anaesthesia necessary for whatever reason. Acute hypotensive-sedative treatment should be discontinued and resumed, only if necessary, not until anaesthesia is over following surgical termination of pregnancy. Imminent eclamptic fit would be the only exception acceptable. Anaesthesia should be induced, using the base anesthetic propanidid which worked extremely well in terms of circulatory stabilisation. It is obviously superior to barbiturates in that particular respect.

Reporting 6 cases as examples the problems of symptoms and diagnosis of peritonitis in puerperium are discussed. Main source of the peritonitis is the ascending of pathogenic germs. The unknown bacteriologic vaginal milieu and the behaviour of the physician associated with caesarean section and premature rupture of the membranes play an important role for developing a peritonitis. Laparotomy with removing of uterus in time is the singlest method to rescue life threatening maternal situations.

Studies were conducted into 55 rabbits and their 327 foetuses to measure the two intravenously applied anaesthetics, Sombrevin and Hexobarbital, for their passage through the placenta and for the concentrations which eventually settle in various organ systems of the foetuses. Fast placental passage, but in different concentrations, was established for either anaesthetic. Sombrevin was found to reach the foetal organism only in low quantity and for short-time retention, whereas Hexobarbital passed the placental barrier in much higher concentration to stay detectably for a longer period of time in the foetal blood. While Sombrevin was not recordable from foetal organs, its intraplacental buildup reached higher concentrations. Relatively high concentrations of Hexobarbital were recordable from both the placenta and vital foetal organs. The conclusions so far derived from those animal experiments seem to suggest a superiority of Sombrevin over Hexobarbital and regard to applicability to obstetric anaesthesia.

In artificially hypoxic rabbit foetuses, rising hydrogen ion concentration with excessive precipitation of fibrin in the microcirculation, with pH values between 7.0 and 6.9, was followed by discontinuation of DIC on account of predominant regular occurrence of secondary fibrinolysis. Such processes of secondary fibrinolysis, which could be inhibited by para-amino methylbenzoic acid (PAMBA), were found to be capable of re-opening completely the circulatory tracks in surviving foetuses. In other cases, they might persist beyond death and cover up pathologico-anatomic findings, if neglected.

Mature newborns reveal the same pulmonal adaption to extrauterine conditions after forceps delivery in general anesthesia for the mother beginning with propanidid or hexobarbital as newborns after spontaneous delivery. Intervalls of apnoe, being physiological during neonatal adaption up to the fourth life-hour, occurred in all three tested groups in the same frequency. Genuine apnoe in combination with following bradycardia and cyanosis couldn't be observed. Moreover during the whole examination time all mean values of respiration frequency were physiological. Therefore we believe that there is no fundamental deterioration of newborns respiration, i.e. a depression in nerve-centre, caused by propanidid or by hexobarbital, despite of repeatedly contrary description.

Application of uninvolved propanidid-infusion anaesthesia to a rabbit in advanced pregnancy is reported in this paper. Uteroplacental circulation was found to be but slightly affected. The condition of the foetuses, consequently, may be considered sufficiently safe for intra-uterine experiments.
