Malign Mezotelyoma (MM) agresif ve kötü prognozlu bir tümördür. Ortalama trombosit hacmi (MPV) trombosit fonksiyonlarını gösterir. Kırmızı kan hücresi dağılım genişliği (RDW) ise dolaşımda bulunan eritrositlerin bir ölçüsüdür ve kalp yetmezliğinde belirteç olarak kullanılmaktadır. Her iki parametrede çeşitli hastalıklarda prognostik öneme sahip olabilmektedir. Perikard tutulumu olan hastalarda MPV ve RDW değerlerinin klinik önemini araştırmak amacıyla 70 (34 perikard tutulumu olan, 36 olmayan) hasta çalışmaya alındı. Perikard tutulumu olarak Toraks Bilgisayarlı Tomografide ve/veya trans-torasik ekokardiyografide perikard tutulumu olması kabul edildi. MM hastalarının yaş ortalaması 57,5 yıl olarak bulundu. Otuz sekiz (% 54,3) erkek ve 32 (% 45,7) kadın hasta çalışmaya alındı. Toplam 44 hastada (% 62,9) çevresel asbest teması pozitifti ve ortalama süresi ise 27,3 yıldı. Perikard tutulumu olan hastaların yaş ortalaması 51,3 tutulumu olmayanların ise 63,3 yıl idi tutulum olanlar anlamlı olarak daha gençti (p= 0,000). Perikard tutulumu olan hastaların ortalama RDW değeri % 20 iken tutulumu olmayan hastaların ortalama RDW değeri ise ve tutulum olanlarda RDW anlamlı olarak daha yüksekti (p= 0,000). Perikard tutulumu ile asbest temas süresi, trombosit ve MPV arasında ise anlamlı bir fark yoktu. Bu çalışma sonucunda RDW'nin bu hastalığın özellikle perikard tutulumu olan hastaların takibinde faydalı bir parametre olabileceğini düşünmekteyiz. Fakat bu konuda geniş serili ve prospektif çalışmalara ihtiyaç vardır., Malignant mesothelioma (MM) is aggressive and has a poor prognosis tumor. In order to investigate the clinical significance of MPV and RDW in pericardial involvement patients 70 (34 pericardial involvement, not 36) patients included in the study. Pericardial invasion in the thoracic computed tomography and / or transthoracic echocardiography was accepted to have pericardial involvement. Mean age of MM patients 57.5 years. Thirty-eight (54.3%) were male and 32 (45.7%) female patients were included in the study. In a total of 44 patients (62.9%) were determined for environmental asbestos exposure and the mean exposure time 27.3 years. Mean age of patients with pericardial involvement were 51.3 years and non-involvement were 63.3 years and patients with pericardial involvement significantly younger than noninvolvement (p=0.000). The mean RDW was 20% in patients with pericardial involvement while the mean RDW was 16% in non- involvement and RDW was significantly higher in patients with involvement (p=0.000). Duration of exposure to asbestos, there was no significant difference between platelets and MPV values. There was no significant difference between pericardial involvements; platelets, duration of exposure to asbestos and MPV. Results of this study RDW may be useful parameter follow-up of patients with this disease, especially with pericardial involvement. However, large series, and prospective studies are needed about this issue.

Malign Mezotel yoma (MM) etiyolojisinde çevresel ve mesleksel asbest teması olan bir kanser türüdür. Bölgemizde çevresel asbest teması nedeniyle MPM sık görülen bir hastalıktır. Bu çalışmada temel onkolojik belirteçlerinin MM’daki seviyelerini değerlendirmek amaçlanmıştır. Üniversite hastanesinde takip edilen toplam 30 MM (20 plevral ve 10 peritoneal) hastasının dosyaları retrospektif olarak incelendi. Bu amaçla karsinoembriyonik antijen (CEA), karbonhidrat antijeni 15-3 (CA 15-3), karbonhidrat antijeni 19-9 (CA 19-9), karbonhidrat antijeni 72-4 (CA 72-4), karbonhidrat antijeni 125 (CA 125) ve alfa-fetoprotein (AFP) sonuçları dosyalardan alındı. Hastaların on beşi erkekti ve yaş ortalaması 54.6 ± 13.9 yıldı. On iki hasta Ergani üç hasta ise Çermik ilçesinden başvurmuştu. Yirmi iki hastada (%73) asbest teması pozitifti ve ortalama asbest temas süresi 23 yıldı. On yedi hastada epitelyal tip saptandı. Ortalama AFP 2.09±1.53 IU/ml, CA125 139.7±476.5 U/ml, CEA 4.99±13.23 ng/ml, CA19-9 10.25±10.63 U/ml, CA153 36.47 ±26.82 U/ml ve CA 724 1.01±0.25 U/ml olarak bulundu. Ortalama CA125, CEA ve CA153 düzeyleri laboratuar normal değerlerine göre yüksek bulundu diğer belirteçlerin seviyesi ise normaldi. MM oldukça fatal seyreden ve tedavi seçeneklerinin sınırlı olduğu bir kanserdir. Hastalığın tanı ve tedavi takibinde belirteçlere ihtiyaç olabilmektedir. Kanser belirteçlerinin hastalıktaki rolü için geniş serili çalışmalara ihtiyaç vardır., Malignant mesothelioma (MM) in the etiology of cancer is a type of environmental and occupational asbestos exposure. A total of 30 MM (20 pleural and 10 peritoneal) patients were evaluated retrospectively in University Hospital. For this purpose, carcinoembryonic antigen (CEA) and Carbohydrate antigen 15-3 (CA 15-3), Carbohydrate antigen 19-9 (CA 19-9), Carbohydrate antigen 72-4 (CA 72-4), Carbohydrate antigen 125 (CA 125) and alpha-fetoprotein (AFP), the results were included in the files. Fifteen patients were male and mean age was 54.6 ± 13.9 years. Twelve patients admitted to the town of Ergani and three Cermik. Twenty-two patients (73%) were positive for asbestos exposure to asbestos and the mean exposure time 23 years. Seventeen patients had epithelial type. Mean level of AFP 2.09±1.53 IU/ml, CA125 139.7±476.5 U/ml, CEA 4.99±13.23 ng/ml, CA19-9 10.25±10.63 U/ml, CA153 36.47±26.82 U/ml and CA724 1.01±0.25 U/ml were found. Mean CA125, CEA and CA153 levels were significantly higher than the laboratory normal values and the normal level was found other markers. MM is a highly fatal cancer and treatment options are limited. Markers of disease diagnosis and treatment monitoring may be needed. Large series studies are needed for Cancer markers role in disease.

Ülkemizde çevresel asbest ve erionit teması nedeniyle MM yaygın bir kanser türüdür. Çalışmamızda 2005-2011 yılları arasında hastanemizde MM tanısı konan 77 kadın hasta retrospektif olarak değerlendirildi. MM hastalarının dosyaları ayrıntılı olarak incelendi. Çalışmaya alınan hastaların demografik verileri, yaş, şikayet, radyolojik bulgular, tanı yöntemleri, hastalık evreleri daha önceden hazırlanan standart formlara kaydedildi. MM hastalarının yaş ortalaması 57 ± 14.1 yıl idi. Elli yedi hastada (%74) olguda çevresel asbest maruziyeti mevcuttu ve ortalama temas süresi 30.6 yıldı. Hastalardan 21'i (%27.3) asbest kullanımının yaygın olduğu Ergani ilçesinden başvurmuştu. Toplam 63 hastada (%81.8) nefes darlığı, 44 hastada (%57.1) göğüs ağrısı ve 16 hastada (%20.8) kilo kaybı vardı. Ortalama semptom süresi 5.7 ± 4.6 aydı. Ayrıca 47 hastada (%61) histopatolojik tip olarak epitelyal tip olarak saptandı. Hastalarda ortalama sağ kalım 9.54 ± 6.9 ay olarak bulundu. Malign mezotelyoma çevresel asbest maruziyetinin fazla olduğu bölgelerde yaşayan kadınlarda uyumlu semptom ve bulguları olanlarda akla getirilmelidir. Bu kişilerde özellikle plevral tutuluma dikkat edilmelidir. Ülkemizde bu hastalığın önlenmesi için çevresel asbest kullanımının tamamen terk edilmesi için çalışmalar yapmak faydalı olabilecektir., Malignant mesothelioma (MM) in the etiology of environmental and occupational exposure to asbestos often plays a role. The mean age of MM patients was 57 ± 14.1 years. Fifty-seven patients (74%) patients had environmental asbestos exposure and the average exposure time is 30.6 years. 21 patients (27.3%), the use of asbestos was commonly applied to the Ergani district. Total of 63 patients (81.8%) dyspnea, 44 patients (57.1%) chest pain and 16 patients (20.8%) had weight loss. The average duration of symptoms was 5.7 ± 4.6 months. In addition, 47 patients (61%) histological type was the epithelial type. The mean survival was found to be 9.54 ± 6.9 months. Malignant mesothelioma should be considered in patients who women living in areas with more than environmental exposure to asbestos, with compatible symptoms and signs. These people should be especially careful of pleural involvement. In our country, for the prevention of this disease to be completely abandoned the use of environmental asbestos studies will be useful to do.

Diyarbakır ilinde çevresel asbest kullanımı yaygındır ve asbeste bağlı hastalıklar sık görülmektedir. Bu çalışmada Diyarbakır'da çevresel asbest maruziyetinin halen devam edip etmediği ve devam ediyorsa hangi amaçlarla kullanıldığı, hangi yollarla temin edildiğinin araştırılması amaçlandı. Çalışma amacıyla Ocak 2011-Nisan 2011 tarihleri arasında Diyarbakır ilinde asbest kullanımının olduğu bilinen Ergani, Çermik, Dicle, Çüngüş ve Hani ilçelerine bağlı toplam 187 köyde anket çalışması yapıldı. Asbest kullanım öyküsü oranı Ergani'de %84, Çermik'te %70, Çüngüş'te %66, Hani'de %66 ve Dicle'de %58 saptandı. Bu köylerden halen asbesti kullanmaya devam edenlerin oranı Hani'de %66, Dicle'de %54, Ergani'de %52, Çüngüş'te %50 ve Çermik'te %44 idi. Halen kullanan köylerde kullanım şeklinin Çermik ve Çüngüş'ün köylerinde sadece pekmez yapımında kullanım iken, diğer köylerde hem duvarların sıvanmasında hem de pekmez yapımında kullanım olduğu anlaşıldı. İlimizde yapılan bu çalışmanın verileri, halen asbest maruziyetinin devam ettiğini ortaya koymuştur. Bu nedenle özellikle kırsal kesimde asbestin zararları hakkında toplumu bilinçlendirmeye yönelik eğitim faaliyetlerinin planlanması ve uygulanması gerekmektedir., Environmental asbestos exposure and asbestosrelated diseases are common in Diyarbakır. We aimed to investigate, whether environmental asbestos exposure ongoing in Diyarbakır or not, used for which purposes if this still in usage and the manner of supplying in this study. A questionnaire was made for this study between January 2011-April 2011 in the 187 village from Ergani, Cermik, Dicle, Cungus and Hani districts that are asbestose usage is known in Diyarbakır city. Rates of asbestos usage history were 84% in Ergani, 70% in Cermik, 66% in Cungus, 66% in Hani and 58% in Dicle. The proportion of these villages that still continue to asbestos usage were 66% in Hani, 54% in Dicle, 52% in Ergani, 50% in Çüngüş and 44% in Çermik. Using pattern was only on molasses making in villages from Cermik and Cungus. But the other villages were using asbestos for plastering walls between molasses making. The d ata of this study has revealed that asbestos exposure stil continues in Diyarbakır city. For this reason planning and implementation of training activities about the harms of asbestos is required for community awareness especially in rural areas.

Bu çalışmada stabil KOA H hastalarında serumda nitrik oksit (NO) ve total antioksidan kapasite (TAOK) düzeyleri çalışılmıştır. KOAH'lı 47 hasta ile aynı sayıdaki sağlıklı erişkin kontrol grubu olarak çalışmaya alındı. Hasta ve kontrol grubu serumlarında oksidatif stres belirteci olarak nitrik oksit (NO) düzeyleri ile antioksidan savunma sisteminin toplam göstergesi olan total antioksidan kapasite (TAOK) ölçüldü. KOAH hasta grubu 47 kişiydi (27 erkek, 20 kadın) ve yaş ortalaması 66.7±8.3 yıldı. Kontrol grubu da 47 kişiydi (28 erkek, 19 kadın) ve yaş ortalaması 63.2±9.4 yıldı ve iki grup yaş ve cinsiyet açısından arasında fark saptanmadı., In this study were studied level of serum nitric oxide (NO) and total antioxidant capacity (TAS) levels in stable COPD patients. A total of 47 patients with stable COPD and 47 control were included the study. In patients and control group, as a marker of oxidative stress in serum of nitric oxide (NO) levels and total anti-oxidant defense system, which is an indicator of total antioxidant capacity (TAS) were measured. 47 was the group of COPD patients (27 men, 20 women) and mean age 66.7±8.3 years. The control group also was the 47 (28 male, 19 female) and mean age 63.2±9.4 years, and there was no difference between the two groups in terms of age and gender.
