The present work deals with results of intensive therapy and temporary organ substitution by using hemoperfusion through a suspension of living donor hepatocytes in 71 patients with acute hepatic insufficiency. Hemoperfusion was performed through a suspension of fresh living hepatocytes or through a suspension of cryo-conserved (during 60 days) living hepatocytes. Lethality was 37-42%.

The authors used a method of graded heparinization with special reference to the individual reaction of the patients to heparin. The initial (starting) dose of heparin was injected intravenously, the sustaining dose of heparin was infused through the tube for blood taking. The method described prevented thrombosis of the HMS apparatus and considerably reduced the quantity of heparin used. No complications of hemosorption resulting from heparinization were noted.

The method of extracorporal hemosorption was used by the authors first in the clinical practice for the preoperative preparation of patients with thyrotoxicosis. Oxygenated SKN carbons were used as sorbents. The hemosorption was used in 16 patients with thyrotoxic goiter by the method developed in the clinic. The authors make a conclusion that hemosorption is an expedient method for the preoperative preparation of patients with a toxic goiter.

The authors discuss the experience of using the spheric carbon hemosorbent for extracorporal hemoperfusion of blood in 12 patients with hepatic insufficiency developed against the background of obturation of the common bile duct by a stone or tumor. Positive clinical effects of hemosorption are shown. The method is recommended for the treatment of patients with hepatic insufficiency.

The authors have analyzed complications in 395 hemosorptions in 189 patients with different diseases, burns being most frequent one associated with hypocalcemia resulting from great doses of heparin. Trental and small doses of heparin are proposed for prevention of the complications.
