Unlabelled: Despite recent advances in treatment of severe injured patients, e.g. due to damage control orthopaedics, multi organ dysfunction syndrome (MODS) and sepsis are major complications in daily practice. During one year 94 patients were prospectively collected.Inclusion Criteria: ISS 16, age 18-60 y, primary admission to our level-1 trauma center, survival > 48 hours after trauma. The development of MODS and sepsis were observed and different groups were formed (+/-). Demographic data revealed no significant differences between the subgroups. Comparing groups +MODS and -MODS significant differences on admission day were observed, when PCT showed first on day 2 after trauma differences. Regarding the development of sepsis PCT was advantageous to IL-6 showing significant higher plasma levels in group +sepsis from the first day after trauma. Serum levels of IL-6 and PCT could be useful in early identification of high risk patients to develop posttraumatic MODS. For sepsis PCT is the better prognostic factor. [ABSTRACT FROM AUTHOR]

Background: Isocentric three-dimensional C-arms allow for more effective intraoperative fracture reduction control compared to two-dimensional imaging techniques. However, this design is not appropriate for shoulder scanning., Objective: To assess the feasibility of using a newer generation variable isocentric flat detector 3D C-arm for intraoperative glenohumeral and acromioclavicular joint assessment and to compare the accuracy of its intraoperative 3D imaging technology to a standard two-dimensional (2D) flat detector fluoroscope., Methods: Five whole-body human cadavers were used (ten shoulders). Native shoulder scans were obtained. A glenohumeral arthrotomy was performed and several injuries and procedures were simulated. Five independent orthopaedic surgeons reviewed each scan and filled out a questionnaire assessing the quality of the images using a visual analog scale (VAS) and a points scoring system., Results: The examiners rated the 3D images as very-good-to-excellent according to the established parameters: image quality; visualization of the corticalis and the spongiosa; delineation of the joint surface; presence of artifacts; and clinical assessment capability. This high quality of the images led to a higher interobserver reliability for 3D images compared to 2D images., Conclusions: Variable isocentric 3D C-arm technology is feasible for intraoperative assessment of shoulder procedures. Assessment of 3D images in shoulder procedures showed better interexaminer reliability in this experiment compared to 2D images. With the aid of intraoperative 3D shoulder imaging, intraoperative 3D C-arm navigation could help improve accuracy in the clinical setting.

Controversy exists in the literature regarding the most appropriate treatment for anterior cruciate ligament (ACL) ruptures in the skeletally immature patient. To study the histological and biomechanical stages following ACL reconstruction during growth, it is necessary to establish an equivalent model to replicate the situation of an immature skeleton. In the present study we evaluated a large animal model of ACL reconstruction during growth using skeletally immature sheep. In contrast to existing models we did not intend to provoke growth disturbances. Instead, we identified those criteria known to be essential to prevent growth disturbances in the human situation and transformed them into an animal model. Applying those criteria to skeletally immature sheep at an age of four months, we performed a fully transphyseal ACL reconstruction of their right knee. The contralateral knee served as a control. The sheep were then sacrificed 3, 6, 12 and 24 weeks after surgery. This animal model is described in detail in the present manuscript. Basic science data are presented in order to 1) make it easier for other research groups to study ACL reconstruction during growth using our model and 2) to improve this model by modifying the one or other detail of the proposed technique. Using this model, remaining open questions concerning ACL reconstruction during growth can be addressed in future studies.
