Objective. To describe the characteristics of patients with type 2 diabetes diagnosed before age 40 (early-onset type 2 diabetes) identified in a nation-wide, population-based study. Materials and Methods. The survey was done in Mexico during 2006. Medical history, anthropometric and biochemical measurements were obtained in every subject. Results. Cases diagnosed before (n=181) and after age 40 (n=659) were included. Early-onset type 2 diabetes was present in 13.1% of the previously diagnosed, 30.3% of the cases identified during the survey and 21.5% of the whole population with diabetes. These individuals had a greater prevalence of obesity and hypertriglyceridemia compared to the cases diagnosed after age 40. Conclusions. Early-onset type 2 diabetes was present in 21.5 % of patients with type 2 diabetes in Mexico. Close to 70% of them were obese or overweight and had the clinical profile of the metabolic syndrome. Objective. To estimate the percentage of Mexican adults that may require lipid-lowering treatment according to National Cholesterol Education Program-III guidelines, using data from the National Health and Nutrition Survey 2006 (ENSANUT2006). Material and Methods. Information was obtained from 4 040 subjects aged 20 to 69 years, studied after a 9 to 12 hours fast. Results. A cardiovascular risk equivalent was found in 13.8% and ≥2 risk factors were present in 31.5% of the population. LDL-C concentrations were above the treatment goal in 70% of the high-risk group and in 38.6% of subjects with ≥2 risk factors. Nearly 12 million Mexicans should be taught how to change their lifestyles and close to 8 million individuals require drug therapy to decrease their cardiovascular risk. Conclusions. Thirty percent of Mexican adults require some form of lipid-lowering treatment (lifestyle modifications in 36.25%, drug therapy in 24.19%). [ABSTRACT FROM AUTHOR]

Objective. To examine the prevalence of metabolic syndrome (MS) and its associated risk factors in Mexican adults aged 20 years or older, using data derived from the National Health and Nutrition Survey 2006 (ENSANUT 2006). Material and Methods. The ENSANUT 2006 was conducted between October 2005 and May 2006. Questionnaires were administered to 45 446 adult subjects aged 20 years or older who were residents from urban and rural areas. Anthropometric and blood pressure measurements were obtained from all subjects and fasting blood specimens were provided by 30% of participants. We randomly selected a sub-sample of 6 613 from which laboratory measurements were carried out for glucose, insulin, triglycerides, total cholesterol and HDL-cholesterol. For this analysis, we included only results from eight or more hours of fasting samples (n=6 021). We used individual weighted factors in the statistical analysis and considered the survey's complex sampling design to obtain variances and confidence intervals. All analyses were done using SPSS 15.0. Results. In accordance with definitions by the National Cholesterol Education Program Adult Treatment Panel III (ATP III), the American Heart Association/National Heart, Lung and Blood Institute (AHA/NHLBI), and the International Diabetes Federation (IDF), the prevalence of MS in Mexican adults aged 20 years or older was 36.8, 41.6 and 49.8%, respectively. Women were more affected than men due to the higher prevalence of central obesity among females. Prevalence of MS increased with age and was higher among populations living in metropolitan areas, in the west-central region, and those with lower education. Discussion. Regardless of the MS definition, a large proportion of Mexican adults has the condition, so preventive measures are needed to decrease the prevalence of the MS components in this population. MS can predict type 2 diabetes and cardiovascular disease, two of the main causes of death in the adult population in Mexico. The intentional search of MS components allows stratifying the population according to risk levels. Treatment for each component should be implemented properly to prevent or delay onset of type 2 diabetes and cardiovascular disease. [ABSTRACT FROM AUTHOR]

Objective. To show the prevalence of diabetes mellitus (DM) and its associated factors in adults, using data derived from the 2000 National Health Survey (NHS). Material and Methods. The 2000 NHS was conducted between November 1999 and June 2000. An adult questionnaire was administered to 45 294 subjects 20 years of age and older. Capillary glucose levels, weight, height and blood pressure were obtained. Individual weighted factors were considered in the statistical analysis, as was the survey's complex sampling design to obtain variances using SUDAAN 7.5.6. Results. The national prevalence of DM in adults ages 20 years and older was 7.5% (95% CI: 7.1-7.9). The prevalence was 7.8% in women and 7.2% in men. It was higher according to age: 2.3% in adults 40 years or younger and 21.2% in those older than 60 years of age. In the urban population, prevalence was 8.1% and in the rural population it was 6.5%. The disease was more frequent in the northern region of the country (8.4%) and in the Mexico City metropolitan area (8.1%). DM was more frequent in the population with the least amount of schooling (9.9%), the lowest income (8.1%), high blood pressure (13.7%), hypercholesterolemia (23.3%) microalbuminuria (15.5%) and renal disease (12.3%). Using multivariate logistic regression, stratified by gender, variables associated with DM were identified: age, little schooling, family history of DM, high blood pressure, renal disease or hypercholesterolemia in both genders. Abdominal obesity was associated with DM in women only and living in an urban area was associated with DM only in men. The highest associations were observed with age, family history of DM, and microalbuminuria. Conclusion. Results from the 2000 NHS show that DM represents a serious public health problem in Mexico. The impact increases when DM affects populations with social or economic factors that limit access to treatment. Data reported in this paper are useful to the implementation of screening and prevention programs. Objective. To identify the prevalence of violence against females among those who are health service beneficiaries in Mexico. Material and Methods. The National Survey on Violence against Women (ENVIM, per its Spanish abbreviation) was applied in 2003 to female users of public primary and secondary health care services. The sampling framework was based on a stratified, probabilistic sample in two stages. First the health care units were selected with probability proportional to the number of physicians' offices in the unit, from a list of possible care units. Second, women 15 years and older who sought care at the health care unit were selected for participation in the study through systematic sampling. Univariate analysis and then bivariate analysis were carried out on the data collected with a questionnaire. Results. The sample included 26 042 women between 15 and 92 years of age, with a mean age of 35.8 years. Physical violence during childhood was reported by 42% of the women. Only 7.8% answered yes to a general question about whether they experienced domestic partner violence, but 21.5% reported experiencing violence of any type during the last 12 months as measured by a scale including specific acts of psychological, economic, physical and sexual violence. The most frequently reported type of violence was psychological (19.6%). Of the women who had been pregnant, 14.1 % reported having experienced violence during pregnancy, and 4.4% reported being hit in the abdomen. The prevalence of sexual violence was 17.3% and close to half reported being victims of this type of violence before age 15. Higher prevalence of violence was found among women with lower levels of formal education, living in a rented home, in areas with higher overcrowding indices, and users of Ministry of Health care services. Conclusions. Identifying and measuring violence is complex, given the diverse types of violence and how they are perceived and therefore reported by women themselves. This is an important public health problem, in view of the high frequency observed in this study and the immediate implications. 19. [Violence scale and severity index: a methodological proposal for measuring violence by the partner in Mexican women].

Objective: To construct and validate a scale to assess violence by the male partner against women. An index of severity of the emotional and physical damage was also designed to assess the intensity of the violent actions against women., Material and Methods: The sample consisted of a total of 26 042 women who participated as respondents in the National Survey on Violence against Women (ENVIM per its abbreviation in Spanish) conducted in Mexico during 2003. Respondents were all users of health services provided by the Mexican government. The questionnaire was organized into 17 sections, one of which was a 27-item scale to assess partner violence. The purpose of this scale was to measure the type (physical, emotional, sexual and financial) and degree of violence based on severity. A severity index was constructed based on two procedures: 1) the validity, reliability, and factor analyses of the scale and 2) the assessment of severity by expert judges who assigned a value to each item of the scale., Results: The validity and reliability results indicated this scale has adequate internal validity (Cronbach's Alpha = 0.99). The factor analysis with Varimax rotation yielded a four-factor solution. The factors were: 1) Psychological violence; 2) Physical violence; 3) Severe physical violence; and 4) Sexual violence. The combination of the four factors accounted for 62.2% of the variance in the scale. Using the ratings from the judges a table of values for each of the violent actions described was obtained. The scores assigned by the judges ranged from 0 to 354. Results revealed a prevalence of 21% partner violence in the last twelve months. The prevalence of psychological violence was 18.5%; of physical violence 10.1%; severe physical violence 6.7% and sexual violence 7.0%., Conclusion: The scale of violence described in this article is a very useful and reliable instrument to assess marital violence against women. It is suggested that this instrument be used in other settings to compare results with different samples.

20. [Characteristics of mixed hyperlipidemia cases in a population-based study: results from the Mexican National Survey of Chronic Diseases].

Objective: To describe the characteristics of mixed hyperlipidemia cases, using data derived from the Encuesta Nacional de Enfermedades Crónicas (Mexican National Survey of Chronic Diseases, ENEC)., Material and Methods: The ENEC was conducted in 1993, in 417 Mexican cities. Blood measurements of lipids, glucose, and insulin were obtained from 2206 cases. Differences between dyslipidemia patients and non cases were obtained using analysis of variance or the chi-squared test., Results: Mixed hyperlipidemia was diagnosed in 282 subjects (12.8%). Cases were 42.7+/-12.6 years old. Fifty six percent were males and 46.4% had HDL cholesterol levels < 0.9 mmol/l. Other cardiovascular risk factors were also present. The prevalence of mixed hyperlipidemia was high even among young adults. A logistic regression model showed that obesity, age, male gender, residence in some regions of Mexico, diabetes, arterial hypertension, and fasting insulin levels >21 mU/ml, were factors associated with mixed hyperlipidemia., Conclusions: Mixed hyperlipidemia is a very common condition in Mexican adults. It is more common in males older than 30 years, with additional cardiovascular risk factors. Study findings suggest that the metabolic syndrome plays a role in the pathogenesis of this disorder.
