OBJECTIVE Multiple myeloma is an incurable hematological malignancy. Currently, the use of proteasome inhibitors could be superior to chemotherapy-based regimen in the treatment of this disease. However, resistance to bortezomib combination therapy still occurs in some patients. So, this research work aims to assess CD69 and CD56 expression in these cases and their relation to the response to therapy. Background: Patient safety culture (PSC) is a vital feature to assess the ability of any healthcare setting in addressing and reducing patients harm. This study attempted to assess the PSC in Intensive Care Units (ICUs) at Alexandria Main University Hospital (AMUH) from the point of view of physicians and nurses. Methods: A cross-sectional study was implemented in two ICUs at AMUH over period of six months. Seventy-two participants were interviewed using the Hospital Patient Safety Scale, customized by the Agency for Healthcare Research and Quality (AHRQ). Results: The average positive response to individual items in the patient safety scale ranged from 2.7% to 79.2%. The “Teamwork within Units” dimension had the utmost average percentage positive score (63.5%) amongst all participants, on the other hand, the “Non-Punitive Response to Errors” dimension had the lowest one (12.0%). Less than half (45.8%) of the interviewed participants rated patient’s safety at the hospital as accepted. Conclusions: PSC is friable in targeted ICUs, much of work is needed to raise the responsiveness of health care givers regarding this issue. Executives and supervisors need to encourage the practices of PS through a blame free culture.

The article examines the construction of the 1973 war as a legitimating discourse in Egypt. After an analysis of formal texts (for example, school textbooks), semi-formal texts (for example, the Ahram newspaper) and informal texts (for example, songs scripted to commemorate the event), the article finds a pattern which constructed the war as a ‘massive, consistent and unquestionable’ victory for Egypt under the rule of Anwar Sadat (1971–81). Based on critical discourse analysis of these previously untapped texts over the eight years of Sadat's rule after the war and drawn on primary sources and interviews, the article traces the genealogy and operationalization of discourse through exploring linguistic and extra-linguistic features synchronized towards the efficacy, durability and credibility of this process. Pyritic sulfur separation of El-Maghara coal ore was studied using Falcon concentrator model SB40-VFD as an enhanced centigugal gravity separator. The coal sample was ground to below 100 µm to reach liberation between pyrite and coal. Optimization of the separation process has been studied using central composite rotatable design (CCRD) and response surface method (RSM). The studied working factors were the back-water pressure (psi) and rotation frequency (Hz) and their interactions, where the feeding rate and pulp density were kept constant through all experiments. A model describing the relation between pyritic sulfur separation efficiency percentage and the previously mentioned operating variables and their interactions, have been designed. Results showed that maximum separation efficiency reached 70.61%, with decreasing total sulfur, and pyritic sulfur contents reaching 1.72% and 0.48% from original assays reaching 2.90% and 1.60%, respectively. Thirty-one new theophylline derivatives have been synthesized and evaluated for their hypoglycemic activity. Compounds 24 (56% reduction) and 31 (57% reduction) showed better hypoglycemic activity than the standard drug glibenclamide which showed 52% reduction in serum glucose level. Compound 27 remarkably reduced serum glucose level by 53%. Ten compounds showed varying degrees of hypoglycemic activity ranging from 20 to 37% reduction in serum glucose level compared to the standard drug. The aromatic amide functionality is the common feature of these theophylline hypoglycemic derivatives. However, anthranilamide and or aliphatic amides proved to be the least active compounds in the present series.

Thrombocytopenia is one of the most frequent hematological manifestations of hepatitis C virus (HCV) infection; which typically worsens with progression of the liver disease and can become a major clinical complication. Several mechanisms have been postulated to explain thrombocytopenia in HCV hepatic patients, including immune mechanisms. The aim of the present work is to investigate the role of immune mechanisms as a causative agent of thrombocytopenia in HCV hepatic patients. The study included 50 hepatic patients with HCV infection (30 with thrombocytopenia and 20 with normal platelets counts). Platelets associated glycoprotein specific antibodies were evaluated by flow cytometry and confirmed by quantitative monoclonal immobilization of platelet antibodies (MAIPA). The frequency of platelet associated immunoglobulin (PAIg) in thrombocytopenic HCV positive hepatic patients by FCM was 86.7, 83.3, 46.7 and 33.3% for total PAIg, PAIgG, PAIgM and PAIgA respectively. MAIPA found platelet specific antibodies in 26/30 (86.7%) of patients. The most likely target antigen for platelets antibodies were glycoprotein (GP) IIb/IIIa (30%), followed by GP IIIa (20.5), GP IIb (13.3%), GPIb (13.3%), then GPIa (10%). The platelets count was inversely correlated to the levels of platelets GP specific antibodies (r=-0.42, p=0.024), and significantly parallel to spleen size (p=0.024). Platelet associated glycoprotein specific antibodies represent a common mechanism inducing thrombocytopenia in patients with chronic HCV infections.

Experimental studies are described to show that high-frequency radio observations provide a simple way to monitor the E.D.M. process and to distinguish between the effective spark and arc discharges. The paper also describes how radio frequency emitted during machining could be used to optimize the E.D.M. process. It was found that radiation from the region of the gap between the electrode (tool) and the workpiece are significantly different when the following conditions occur: open voltage, sparking (effective machining), arcing and short circuit. These investigations were carried out to determine possible correlations between the EDM machining parameters (pulse on, current) and the machineability components (stock removal, electrode wear, overcut, surface finish and surface integrity). The surface integrity of the component depends on its conformity to tolerance, its surface finish and the metallurgical subsurface characteristics.
