Regulatory T cells (Tregs) form a subpopulation of approximately 5–10% of CD4+ T cells. The best marker for Tregs is the forkhead transcription factor P3 (FOXP3), which is essential for their development and function. In humans, functionally mature FOXP3+ Tregs are already found in the foetus at the beginning of the second trimester of pregnancy, but the kinetics of their maturation are not fully understood. In this thesis, we have studied the thymic production of Tregs in relation to their frequency in peripheral blood using flow cytometry and quantitative polymerase chain reaction (PCR). Our data indicate that the thymic output of human Tregs is very stable, but in the periphery, the increased proliferation and homeostasis of FOXP3+ Tregs, as well as the transient expression of FOXP3 by activated conventional cells, affect the level of FOXP3 expression. Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) is a rare, recessively inherited autoimmune disease caused by mutations in the autoimmune regulator gene (AIRE), and provides a unique setup to study the pathogenesis of human autoimmunity. The loss of AIRE function impairs the deletion of autoreactive T cells in the thymus as well as the function of Tregs in the periphery. We showed that in APECED patients, the peripheral naïve recent thymic emigrant (RTE) Tregs are rapidly dividing and decreased in numbers compared with healthy individuals. Furthermore, the patients’ activated Tregs express FOXP3 in substantially lower levels than those in healthy controls. To assess the origin of this defect in the activated Tregs in APECED patients, we used T cell receptor sequencing to analyse the clonal relationship between four T cell subsets in healthy controls and APECED patients. Activated and naïve subsets in Tregs and T helper cells were analysed, and the clonal sharing patterns were strikingly similar in patients and healthy controls. The naïve Tregs had only a minor overlap with the helper T cell subsets, supporting the distinction between these lineages in the thymus. The activated/memory Treg subset shared some clones with helper T cells, but mainly with the naïve Treg population. Our data give a more detailed view of the defect of Tregs in APECED patients, emphasising the role of impaired thymic selection. Puumala hantavirus (PUUV) infection is a rodent-borne zoonosis causing a haemorrhagic fever with renal syndrome (HFRS), also known as nephropatia epidemica (NE). The clinical course of the infection is generally mild but has a high interindividual variation. Indeed, immunopathology is thought to be important in the outcome of the hantaviral disease, but the balance between beneficial and potentially harmful responses has been unknown. We studied the acute T cell response in 24 patients with acute PUUV infection and compared the results with the clinical data. As expected, the CD4 and CD8 T cells showed a significantly increased expression of the cell cycle marker Ki-67 during the acute phase of the infection, whereas the magnitude of the response did not correlate with the disease severity measured with clinical parameters. However, the level of FOXP3 expression, indicating the suppressive phenotype, had a strong positive correlation with the disease severity. Interestingly, the correlation was also observed in samples taken 6–12 months after the clinical disease and might be significant in the regulation of other immune responses. Regulatoriset T-solut (Treg) ovat lymfosyyttejä eli valkosoluja, joiden tehtävänä on pitää taudinaiheuttajista tai elimistön omista rakenteista aktivoituneet tavalliset T-lymfosyytit eli T-solut kurissa. Tätä kutsutaan perifeeriseksi toleranssiksi. Tämänhetkisen tietämyksen mukaan Treg-solut erottaa selkeimmin muista auttaja T-soluista korkea FOXP3:n ilmentyminen. FOXP3 on useiden geenien luentaa solun tumassa säätelevä proteiini, transkriptiotekijä, joka on välttämätön Treg-solujen kehitykselle ja toiminnalle. Tässä väitöskirjatutkimuksessa käytettiin virtaussytometriaa, PCR-menetelmää ja sekvenointimenetelmää Treg-solujen tutkimiseen. Osoitimme, että kateenkorva tuottaa FOXP3-positiivisia Treg-soluja tasaisesti. Perifeerisen veren lymfosyyttien FOXP3:n ilmentäminen oli myös tasaista, mutta emme havainneet korrelaatiota FOXP3:n ilmentymisessä kateenkorvan ja veren lymfosyyttien välillä. Tämä johtuu todennäköisesti säätelymekanismeista ja auttaja T-solujen FOXP3:n ilmentämisestä perifeerisessä veressä. Lisäksi tutkimme perifeeristen T-solualaluokkien T-solureseptorikloonien suhdetta toisiinsa, ja havaitsimme, että perifeerisen veren Treg-solut ovat pääasiassa peräisin kateenkorvasta, mutta muuntumista muista T-solualaluokista tapahtuu. Autoimmuuni polyendokrinopatia – kandidiaasi - ektodermaalinen dystrofia (APECED) on vakava mutta harvinainen autoimmuunitauti, joka kuuluu suomalaiseen tautiperimään. Sairaus periytyy peittyvästi ja johtuu mutaatioista Autoimmune regulator (AIRE)–geenissä. AIRE säätelee geenien luentaa kateenkorvassa ja sen toiminta on tärkeää negatiivisessa selektiossa T-solukehityksen aikana. Tällöin T-soluilta vaaditaan reagoimattomuutta omia kudoksia kohtaan ja normaalitilanteessa epäasianmukaisesti reagoivat T-solut eivät pääse etenemään T-solukehityksessä seuraavaan vaiheeseen. Tämä suojamekanismi (sentraalinen toleranssi) murtuu APECEDissa ja omia kudoksia tunnistavia soluja pääsee verenkiertoon ja kudoksiin. APECED-potilailla on todettu Treg-solujen toiminnan häiriö, jolloin myös perifeerinen toleranssi murtuu ja kehittyy immuunivaste elimistön omia kudoksia kohtaan, mikä taas johtaa solukatoon ja lopulta kliiniseen tautiin. Kahdessa osajulkaisussa tutkimme yksityiskohtaisemmin Treg-solujen vikaa APECED-potilailla ja tulokset painottavat häiriintyneen Treg-solujen kehityksen roolia kateenkorvassa, mutta lisätutkimuksia tarvitaan vielä. Puumalaviruksen aiheuttama myyräkuume (nefropatia epidemica, NE) on munuaisoireinen verenvuotokuume. Suomessa yleinen myyräkuume tarttuu ihmiseen metsämyyrän eritteistä yleensä hengitysteitse. Suomessa arviolta n. 7000:n ihmistä infektoituu Puumalaviruksella vuosittain. Puumalavirusinfektio on yleisinfektio, jonka oireita ovat korkea kuume, keskushermosto-oireet ja joissakin tapauksissa akuutti munuaisten vajaatoiminta. Suurin osa potilaista selviää infektioista lievin oirein, mutta pienellä osalla taudinkuva on vaikeampi ja menehtymiseen tauti johtaa 0,1%:ssa tapauksista. Ihmiset, joilla on tietty kudostyyppi, saavat vakavamman taudinkuvan, ja tämä saattaa johtua ihmisen immunologisesta vasteesta Puumalavirusinfektiossa. Tulostemme mukaan Treg-solujen liiallinen aktiivisuus korreloi vakavaan taudinkuvaan Puumalavirusinfektiossa ja vaikuttaisi täten vaarantavan normaalin immuunivasteen näillä yksilöillä.

The immune system should react effectively towards harmful pathogens but tolerate own tissues. The tolerance comprises central and peripheral tolerance. Negative selection, where cells reacting harmfully towards own tissues are eliminated, is part of central tolerance. One part of peripheral tolerance is the regulatory T cell population. Errors in tolerance cause autoimmune diseases, an example of which is APECED, autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy. The basis of tolerance is the development of T lymphocytes in the thymus. During this process most of the developing thymocytes die. To survive, thymocytes have to recognize peptides presented in the major histocompatibility complex (MHC) with proper affinity with their αβ T cell receptor (TCR). TCRs have a huge variation in sequence in their complementarity determining region 3 (CDR3), due to somatic recombination and additional editing. In this thesis I have studied this development of T cells in the human thymus, and then more specifically the regulatory CD4+ FOXP3+ T cell population. The effect of positive and negative selection on the TCR repertoire of developing thymocytes was analyzed. Very few shared sequences were found between different populations, showing the huge diversity of the TCR repertoire. In physicochemical analysis the basic TCR structure remained the same from already the most immature population before selections. A reduction in the average CDR3 length was seen between the CD4+CD8+ double-positive (DP) and CD4+ populations, probably reflecting the effect of thymic selections. To study thymic selection in APECED patients, the observed reduction in the length of CDR3 was used as an indicator of selection. The average CDR3 length of the peripheral CD4+ and CD8+ T cell populations of APECED patients was compared with healthy controls. No significant differences in CDR3 lengths between the patients and healthy controls were found, not even in the CD8+RA+ population that is abnormal in APECED patients. As most studies have been done in mouse models, the precise steps of regulatory T cell development are poorly known in human. DP thymocytes are thought to be the precursors of CD4 FOXP3+ regulatory T cells. After an overnight culture and flow cytometry analysis more apoptotic DP thymocytes than CD4+ thymocyte were found, showing their precursor cell characteristics. Furthermore, the effect of interleukin 7 (IL-7) on developing thymocytes was studied. Mature regulatory T cells express the IL-7 receptor α chain CD127 at low levels. However, stimulation with IL-7 in culture increased the percentage of FOXP3+ DP and CD4 single-positive thymocytes. The mechanism behind this seemed to be an inhibition of apoptosis and possibly also increased FOXP3 expression. These results increase the knowledge on T cell development in human but many details are yet to be revealed. Immuunijärjestelmän täytyy reagoida tehokkaasti haitallisia taudinaiheuttajia vastaan, mutta sietää omia kudoksia. Toleranssiin kuuluu sentraalinen ja perifeerinen toleranssi. Negatiivinen selektio, jossa omia kudoksia kohtaan haitallisesti reagoivat solut poistetaan, on osa sentraalista toleranssia, ja säätelijä-T-solupopulaatio on osa perifeeristä toleranssia. Toleranssin häiriöt voivat aiheuttaa autoimmuunisairauksia, joista yksi esimerkki on APECED, autoimmuunipolyendokrinopatia-kandidiaasi-ektodermidystrofia. Toleranssin perusta on T-solujen kehitys kateenkorvassa. Kehityksen aikana suurin osa kehittyvistä tymosyyteistä kuolee. Selviytyäkseen tymosyyttien täytyy tunnistaa α- ja β-ketjuista muodostuneen T-solureseptorinsa avulla MHC (major histocompatibility complex)-molekyyliin sitoutuneita peptidejä sopivalla affiniteetilla. T-solureseptorisekvensseissä on erittäin suuri variaatio CDR3 (complementarity determining region 3)-alueella somaattisen rekombinaation ja nukleotidimuokkauksen takia. Väitöskirjassani tutkin T-solujen kehitystä ihmisen kateenkorvassa ja näistä erityisesti CD4+ FOXP3+ säätelijä-T-solujen populaatiota. Eri kehitysvaiheissa olevat solut eristettiin ja kateenkorvan positiivisen ja negatiivisen selektion vaikutus T-solureseptorirepertuaarin analysoitiin. Eri solupopulaatioilla oli hyvin vähän yhteisiä sekvenssejä, mikä kuvaa T-solureseptorirepertuaarin suurta monimuotoisuutta. Fysikaalisten ja kemiallisten ominaisuuksien osalta T-solureseptorin rakenne oli hyvin samankaltainen jo epäkypsimmässä, ennen selektioita olevassa solupopulaatiossa. CDR3-alueen keskipituudessa tapahtui lyhenemistä CD4+CD8+ solujen ja CD4+ solujen välillä, todennäköisesti selektioiden vaikutuksesta. Havaittua CDR3-pituuden lyhenemistä käytettiin selektioiden tutkimiseen APECED-potilaissa. Potilassolujen CDR3-pituuksia perifeerisissä T-solupopulaatioissa verrattiin terveiden verrokkien solujen CDR3-pituuksiin. CDR3-pituuksissa potilaiden ja verrokeiden välillä ei havaittu merkitseviä eroja, ei myöskään CD8+CD45RA+ populaatiossa, joka on epänormaali APECED-potilailla. Ihmisellä säätelijä-T-solujen kehitysaskeleet tunnetaan huonommin kuin hiirimallissa. CD4+CD8+ tymosyyttien ajatellaan olevan CD4+FOXP3+ säätelijä-T-solujen esiasteita. Kun tymosyyttejä viljeltiin soluviljelmässä ja sen jälkeen analysoitiin niitä virtaussytometrian avulla, havaittiin, että solujen kasvatuksen aikana suurempi osa CD4+CD8+ soluista meni apoptoosiin, viitaten niiden olevan esiasteita kypsemmälle solupopulaatiolle. Lisäksi tutkittiin interleukiini 7:n (IL-7) vaikutusta kehittyviin tymosyytteihin. Kypsät säätelijä-T-solut ilmentävät vain vähän interleukiini-7:n α-ketjua. Stimulointi IL-7:llä soluviljelmässä kuitenkin lisäsi FOXP3:a ilmentävien solujen osuutta CD4+CD8+ ja CD4-positiivisissa tymosyyteissä. Tämän ilmiön mekanismeina tulivat esiin apoptoosin esto ja mahdollisesti lisääntynyt FOXP3-ekspressio. Tuloksemme tuovat lisää tietoa ihmisten T-solujen kehityksestä, joskin monet T-solujen erilaistumiseen liittyvät tekijät ovat edelleen tuntemattomia.

To live a healthy life, humans need to co-exist with foreign organisms. These consist of the thousands of different types of microbes that colonize the human body. But also, in the case of a pregnant woman, the fetus can be viewed as a foreign organism. To avoid disease, the barriers of the human body, e.g. the mucosal surfaces, must be maintained. Here the innate immune defense system plays an important role. The salivary scavenger and agglutinin (SALSA), also known as gp340, DMBT1 and SAG, is a molecule found at most mucosal surfaces. SALSA is associated with the epithelium or secreted into the lining fluids, such as tears, saliva and mucus in the respiratory tract. SALSA is known to bind and agglutinate a broad spectrum of bacteria, as well as viruses, and thus play a role in the innate immune defense against invading microbes. The effect of SALSA is mediated in concert with several other defense molecules such as IgA, surfactant proteins A and D, and the complement component C1q. These have all been shown to be ligands of SALSA. Alongside the role of SALSA in innate immunity, evidence for a function in epithelial and stem cell differentiation has emerged. This thesis work has addressed the function of SALSA in innate immunity, especially in early life. SALSA was found in the amniotic fluid and in meconium and feces of newborns. In fact, SALSA was among the most abundant proteins in the intestines of newborn children. By comparing the SALSA protein in the different samples we found size polymorphisms, varying from one individual to another, but also from compartment to compartment within the same individual. These differences were found to alter the ability of SALSA to bind known endogenous and bacterial ligands. SALSA was also found to be expressed in the human placental and decidual tissues. In the 1st trimester of pregnancy, SALSA was detected sporadically in maternal decidual capillaries. Closer to term SALSA was found to be expressed by the syncytiotrophoblast layer of the placental villous trees. In certain sites, e.g. at disrupted and damaged areas of the syncytium, SALSA was found deposited into fibrinoid formations. It partially co-localized with the fibrinoid component fibronectin. Complement activation has been observed at the feto-maternal interface of both healthy and complicated pregnancies. SALSA had previously been found to bind C1q. Thus, it was of interest to investigate the ability of SALSA to interact directly with the complement system. We found that SALSA bound to both mannan-binding lectin and to some extent to all three ficolins (H, L and M). SALSA activated complement, when it was bound to a surface. In contrast, fluid-phase SALSA was able to inhibit the deposition of complement on SALSA non-binding microbial surfaces. It thus acted in dual fashion to target complement attack. In the human placenta we observed C1q-targeting of the SALSA-positive fibrinoid formations. C1q and complement are known to function in the clearance of apoptotic cells and debris. Thereby SALSA and complement probably have a cooperative function in the containment and clearance of the injured structures, thus linking its innate immune activity with the maintenance of tissue homeostasis.

Streptococcus pneumoniae, or the pneumococcus, is a bacterium of the human nasopharyngeal normal flora that also causes non-invasive respiratory tract infections, and serious infections, such as pneumonia, septicaemia, and meningitis. Globally, the rise in pneumococcal antimicrobial resistance over the past few decades is a worrying trend. The bacterium is covered by a polysaccharide capsule and over 90 different kinds of pneumococcal capsules can be recognised by serotyping. The most important serotypes are included in the available polysaccharide and conjugate vaccines. The 10-valent conjugate vaccine has been part of the Finnish national vaccination programme since September 2010. In this study, the serotype and genotype clonality of the invasive pneumococcal population in Finland was studied in relation to antimicrobial resistance. All invasive pneumococci (n=7,765) isolated in Finland during 2002-2011 and a subset of non-invasive multidrug-resistant isolates (n=12) from 2008 were serotyped and studied for antimicrobial susceptibility. The penicillin-resistant isolates were genotyped by multi locus sequence typing (MLST) and pilus-encoding virulence genes were detected by PCR. A sequential multiplex PCR assay for deducting the serotypes of pneumococci was set up tailored to the serotype distribution of invasive isolates recovered in Finland. Serotype 14 was the predominant serotype every year of the study, representing 17.5% of all the invasive isolates. The proportion of invasive isolates non-susceptible to penicillin or erythromycin was high, and it increased over the study period, reaching 22% and 26% for penicillin and erythromycin, respectively, in 2011. The proportion of non-susceptible isolates was particularly high among isolates from children and within serotype 14. Among the genotyped isolates, international resistant clones such as PMEN3 Spain9VST156 and PMEN14 Taiwan19FST236 dominated. However, novel genotypes were also found, which illustrates the continuous recombination of pneumococci. This study showed that in Finland multidrug-resistant serotype 19A pneumococci appeared among the non-invasive isolates prior to large-scale vaccination. In many countries, the clone has emerged following vaccination. Pilus-encoding gene carriage, which may serve as a competitive advantage for the bacterium by facilitating adherence to the epithelium, was frequent among the penicillin- and multidrug-resistant isolates. In the future, a vaccine targeting the pilus proteins might successfully help to control these clones. The serotyping scheme, which was set up, is useful in surveillance, as knowledge of the serotype distribution of the invasive pneumococci is essential for vaccine development and monitoring of the vaccination programme. Streptococcus pneumoniae, pneumokokki, on nenänielun normaaliflooran bakteeri, joka voi aiheuttaa hengitystieinfektioita sekä vakavia tauteja, kuten keuhkokuumetta, verenmyrkytystä ja aivokalvontulehdusta. Pneumokokkien mikrobilääkeresistenssi on maailmanlaajuisesti ollut huolestuttavassa kasvussa. Pneumokokilla tunnetaan yli 90 erilaista polysakkaridikapselia, jotka määritetään serotyypittämällä. Käytössä olevat rokotteet kattavat tärkeimmät vakavia tauteja aiheuttavat serotyypit. 10-valenttinen pneumokokkikonjugaattirokote on syyskuusta 2010 lähtien ollut osa kansallista rokotusohjelmaa. Tutkimuksessa tarkasteltiin invasiivisten pneumokokkien klonaalisuutta sekä serotyyppi- että genotyyppitasolla suhteessa mikrobilääkeherkkyyteen. Serotyyppi ja mikrobilääkeherkkyys määritettiin kaikille Suomessa vuosina 2002-2011 verestä tai selkäydinnesteestä eristetyille invasiivisille kannoille sekä osalle vuonna 2008 eristetyille ei-invasiivisille moniresistenteille pneumokokeille. Penisilliiniresistenttien kantojen perimää tarkasteltiin edelleen määrittämällä niiden genotyyppi ja pilusgeenien läsnäolo. Lisäksi tutkimuksessa pystytettiin Suomen pneumokokkipopulaatioon räätälöity bakteerin perimää havaitseva serotyypitysmenetelmä. Serotyyppi 14 oli tärkein, se kattoi 17,5 % kaikista tutkimuksen invasiivisista pneumokokeista. Penisilliinille herkkyydeltään alentuneiden kantojen osuus kasvoi tutkimusjaksona ja oli 22 % vuonna 2011. Herkkyydeltään alentuneiden kantojen osuus oli erityisen korkea lapsilta eristettyjen kantojen keskuudessa sekä serotyypillä 14. Myös erytromysiinille herkkyydeltään alentuneiden kantojen osuus oli korkea. Suurin osa genotyypitetyistä kannoista oli sukua maailmanlaajuisille resistenteille pneumokokkiklooneille mutta myös uusia genotyyppejä havaittiin, mikä kuvastaa resistenttien kloonien jatkuvaa kehitystä. Havaittiin, että serotyypin 19A moniresistentti pneumokokkikanta on esiintynyt Suomessa ei-invasiivisissa kannoissa jo ennen laajamittaisia rokotuksia, vaikka se on useassa maassa yleistynyt vasta rokotusten myötä. Valtaosa tutkituista resistenteistä kannoista kantoi limakalvoon tarttumista edistäviä pilusgeenejä. On todennäköistä, että mahdollinen pilusproteiineja sisältävä rokote kykenisi tulevaisuudessa torjumaan tällaisia kantoja. Pystytetyllä serotyypitysmenetelmällä voidaan selvittää invasiivisen pneumokokkipopulaation serotyyppijakauma, jonka tunteminen on ensiarvoisen tärkeää rokoteseurannassa ja rokotekehittämistyössä. Streptococcus pneumoniae, eller pneumokocken, är en bakterie som finns i människans normalflora men också orsakar allt från milda luftvägsinfektioner till svåra invasiva sjukdomar som lunginflammationer, sepsis och hjärnhinne-inflammationer. Globalt sett har antibiotikaresistensen bland pneumokocker ökat oroväckande under de senaste decennierna. Fler än 90 olika polysackaridkapslar har beskrivits; dessa bestäms genom serotypning. De viktigaste serotyperna finns med i de vaccin som utvecklats mot pneumokocksjukdomar. I september 2010 blev det 10-valenta konjugatvaccinet en del av det nationella vaccinationsprogrammet i Finland. I den här undersökningen granskades de invasiva pneumokockernas klonalitet både på sero- och genotypnivå i förhållande till antibiotikaresistensen i Finland. Serotyper och antibiotikakänslighet bestämdes för alla invasiva pneumokocker som isolerades i Finland under åren 2002-2011 och för ett sampel av multiresistenta icke-invasiva pneumokocker från år 2008. De penicillinresistenta stammarna genotypades och deras gener för piluskodande virulensfaktorer utreddes. Ett nytt serotypnings-protokoll baserat på multiplex-PCR sattes också upp. Bland serotyperna var serotyp 14 den viktigaste, den utgjorde 17,5% av alla invasiva stammar. Andelen isolat med nedsatt känslighet mot penicillin eller erytromycin var hög och ökade under forskningsperioden till 22 %, respektive 26 %, av stammarna. Andelen stammar med nedsatt antibiotikakänslighet var speciellt hög bland isolat från barn och inom serotyp 14. Internationella resistenta kloner dominerade bland de genotypade stammarna. Nya genotyper hittades också vilket beskriver de resistenta klonernas fortsatta utveckling. Resultaten visar också att den multiresistenta serotyp 19A-klon som på många håll i världen ökat markant efter vaccinering, hade fått fotfäste i Finland redan före storskalig vaccinering inletts. Majoriteten av de penicillin- och multiresistenta pneumokockerna bar på piluskodande gener, vilket tyder på att ett vaccin som innehåller pilusprotein i framtiden kunde begränsa dessa viktiga kloners framfart. Det nya serotypningsprotokollet möjliggör också i fortsättningen granskningen av pneumokockernas serotypfördelning för vaccin-uppföljning och vaccinutvecklingsbehov.
