Abstract

This paper focuses on the influence of the RAP-extracted binder and rejuvenator contents on the steady shear viscosity obtained as a linear viscoelastic property of different binder blends. One pure fresh 50/70 binder, a RAP-extracted binder and a rejuvenator of vegetal origin were mixed in different dosages. Steady shear viscosity ( η 0 ) values at different temperatures (from 25 °C to 85 °C) were determined from complex shear modulus results for all tested binders. In addition, η 0 ( T ) values of all binder blends were estimated from η 0 ( T ) values of base constituents by using two different approaches. Good correspondences with experimental results were found.

Abstract

BackgroundDuring the COVID-19 pandemic, decreased volumes of stroke admissions and mechanical thrombectomy were reported. The study’s objective was to examine whether subarachnoid haemorrhage (SAH) hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.MethodsWe conducted a cross-sectional, retrospective, observational study across 6 continents, 37 countries and 140 comprehensive stroke centres. Patients with the diagnosis of SAH, aneurysmal SAH, ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases, 10th Revision, codes. The 3-month cumulative volume, monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before (1 year and immediately before) and during the pandemic, defined as 1 March–31 May 2020. The prior 1-year control period (1 March–31 May 2019) was obtained to account for seasonal variation.FindingsThere was a significant decline in SAH hospitalisations, with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic, representing a relative decline of 22.5% (95% CI −24.3% to −20.7%, pInterpretationThere was a relative decrease in the volume of SAH hospitalisations, aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the COVID-19 pandemic. These findings in SAH are consistent with a decrease in other emergencies, such as stroke and myocardial infarction.

Abstract

The properties of various blends of a fresh binder (50/70 penetration grade) and a RAP-extracted binder, with and without a rejuvenator, were studied. A mixture of vegetal oils was used as rejuvenator in different dosages. A total of 17 binders were tested, including pure fresh and RAP binders. Conventional tests were performed in order to determine penetration at 25 °C, softening point ( T R & B ), Fraass breaking point ( T F r a a s s ), ductility (elongation at 25 °C) and density at 25 °C. The steady-shear viscosity ( η 0 ) of all binders was calculated from complex modulus test results. Two different approaches were proposed in order to estimate values of penetration, T R & B , T F r a a s s and η 0 of blends from properties of base constituents. Excellent correspondences with experimental results were observed.

Abstract

Background: We aim to evaluate the speed and rates of reperfusion in tandem large vessel occlusion acute stroke patients undergoing upfront cervical lesion treatment (Neck-First: angioplasty and/or stent before thrombectomy) as compared to direct intracranial occlusion therapy (Head-First) in a large international multicenter cohort. Methods: The Thrombectomy In TANdem Lesions (TITAN) collaboration pooled individual data of prospectively collected thrombectomy international databases for all consecutive anterior circulation tandem patients who underwent emergent thrombectomy. The co-primary outcome measures were rates of successful reperfusion (modified Thrombolysis in Cerebral Infarction 2b/3) and time from groin puncture to successful reperfusion. Results: In total, 289 patients with tandem atherosclerotic etiology were included in the analysis (182 Neck-First and 107 Head-First patients). Except for differences in the Alberta Stroke Program Early CT Score (ASPECTS; median 8 [range 7–10] Neck-First vs. 7 [range 6–8] Head-First; p < 0.001) and cervical internal carotid artery (ICA) lesion severity (complete occlusion in 35% of the Neck-First vs. 57% of the Head-First patients; p < 0.001), patient characteristics were well balanced. After adjustments, there was no difference in successful reperfusion rates between the study groups (odds ratio associated with Neck-First: 1.18 [95% confidence interval, 0.60–2.17]). The time to successful reperfusion from groin puncture was significantly shorter in the Head-First group after adjustments (median 56 min [range 39–90] vs. 70 [range 50–102]; p = 0.001). No significant differences in the rates of full reperfusion, symptomatic hemorrhage, 90-day independence, or mortality were observed. Sensitivity analysis excluding patients with complete cervical ICA occlusion yielded similar results. Conclusions: The upfront approach of the intracranial lesion in patients with tandem large vessel occlusion strokes leads to similar reperfusion rates but faster reperfusion as compared to initial cervical revascularization followed by mechanical thrombectomy. Controlled studies are warranted.

Abstract

International audience; Background and purpose: Data on safety and efficacy of periprocedural use of heparin are limited during treatment of acute ischemic stroke patients with anterior circulation tandem occlusion. This study aimed to investigate the impact of heparin use during endovascular therapy of anterior circulation tandem occlusions on the functional and safety outcomes.Methods: A retrospective analysis of the multicenter observational TITAN registry was performed. Patients with anterior circulation tandem occlusion and treated with endovascular therapy (EVT) were included, with or without extracranial carotid intervention. We divided patients into two groups based on periprocedural heparin use (heparin vs. non-heparin). The dose of intravenous unfractionated heparin ranged from 1500 to 2500 I.U. Primary study endpoint was 90-day Modified Rankin Scale (mRS). Secondary study endpoint included angiographic and safety endpoints such as hemorrhagic complications. A propensity-score-matched analysis was performed.Results: Among 369 patients, heparin was used in 68 patients (18.4%). In the propensity-score-matched cohort, favorable outcome (mRS 0-2) occurred in 51.3% in heparin group and 58.0% in non-heparin group (matched OR, 0.76; 95% CI, 0.32-1.78; P = 0.52). Similar result was found in propensity-score-adjusted cohort (adjusted OR, 0.72; 95%CI, 0.39-1.32; P = 0.28). Likewise, there was no difference in the rate of successful reperfusion (mTICI 2b-3) (propensity-score-adjusted OR, 1.03; 95%CI, 0.50-2.09; P = 0.93) neither in safety endpoints between the two groups.Conclusions: Periprocedural heparin use during EVT of anterior circulation tandem occlusions was not associated with better functional, angiographic or safety outcomes. These findings are applicable for low doses of heparin, and further studies are warranted.

Abstract

Background: Acute tandem occlusion (ATO) management is technically challenging and was not sufficiently evaluated in trials. There is evidence suggesting the efficacy and safety of emergent carotid stenting in conjunction with mechanical thrombectomy (MT) for ATO treatment. However, the safety of carotid stenting (CS) post intravenous tPA (IVT) treatment remains unclear. Objective: to report the safety and functional outcomes of emergent CS post IVT, and compare it to non-IVT patients. Methods: This was a retrospective multicenter international TITAN collaboration including 18 endovascular databases. In the present analysis, patients were included if they presented with acute ischemic stroke (AIS) due to ATO and were treated with CS in conjunction with MT. ATO was defined as an extracranial internal carotid artery (ICA) lesion (complete occlusion or stenosis >=90%) and an intracranial proximal occlusion (distal ICA and/or first or second segment of the middle cerebral artery). Outcome measures included final mTICI score, 90 day-modified Rankin Scale (mRS), mortality, procedural complications, and symptomatic intracerebral hemorrhage. Mixed effects logistic model was used to determine the predictors of each outcome Results: Among 454 patients with ATO, 289 were treated with CS and included in the present analysis. One hundred seventy-five patients (60%) of the included patients received IVT prior to MT. Onset to groin was shorter in the IVT group (259 ±251 vs. 353±238; p=0.013). Otherwise, there was no difference in baseline characteristics between the two groups. Heparin use during procedure was less in IVT group (20% vs. 54%; p Conclusion: Emergent CS in association with MT after IVT was safe and was not associated with an increased risk of sICH or procedural complications.

Abstract

The linear viscoelastic (LVE) behaviour of several combinations of two binders, a straight run 35/50 bitumen and a RAP-extracted bitumen in various proportions (20%, 40% and 60% of RAP binder), was simulated. Two different methods were used to carry out the simulations, The first method consists of a blending rule from the 2S2P1D (2 Springs, 2 Parabolic elements, 1 Dashpot) rheological model, that was calibrated for straight run bitumen and RAP. The second approach is based on the self-consistent model proposed by Herve and Zaoui [1], considering the two binders as a layered composite sphere embedded in a homogenous medium. Both possible juxtapositions, 35/50 binder over RAP binder (“fresh over RAP”) and vice versa (“RAP over fresh”), were taken into account. All the simulations were compared with 2S2P1D simulations of experimental data obtained from DSR and Tension/Compression tests on samples of perfect blends of the binders, in the same proportions. The two methods yielded different results. Simulations obtained with the blending rule are remarkably close to 2S2P1D simulations of experimental data. “Fresh over RAP” simulations of the self consistent approach are closer than “RAP over fresh” simulations to the results obtained with the blending rule. A statistical analysis was also performed to compare all the considered combinations. Norm (|E*|) and phase angle (φ) of complex modulus at different frequencies (from 1 × 10−6 to 1 × 10100 Hz) and steady state viscosity (calculated from norm of complex viscosity, |η*|, at 1 × 10−100 Hz) were considered as responses. First, a block design approach was used to estimate that significant differences were obtained between all various simulations and 2S2P1D fits of experimental data. Then, the Turkey method was applied in order to compare all combinations with each other and to identify which pairs of combinations show significant differences. The results of the statistical analysis generally confirm the conclusions drawn from the visual judgment of the rheological simulations.

Abstract

Introduction: Previous studies showed that mechanical thrombectomy (MT) is safe and effective in tandem steno-occlusive lesion. The effect on initial tandem extracranial internal carotid artery (ICA) lesion severity on outcome is unknown. In this study, we aimed to compare angiographic, functional and hemorrhagic outcomes according to initial lesion severity Methods: A multicenter international retrospective study that included 18 stroke centers. Patients were included if they presented with ischemic stroke due to tandem lesion and were treated with MT with or without carotid stenting. Tandem lesion was defined as proximal intracranial occlusion (distal ICA, M1/M2) and extracranial internal carotid artery (ICA) occlusion or ≥90% stenosis NASCET. We divided patients into two groups based on extracranial ICA lesion severity (severe stenosis vs. complete occlusion) at the first angiographic run. Outcome measures included 90 day mRS, final mTICI score, procedural complications and symptomatic hemorrhage. Symptomatic hemorrhage (Sich) was defined based on ECASS criteria. Results: A total of 305 patients were included in the present analysis. 134 patients had complete ICA occlusion (group 1) ,and 170 had ≥90% ICA stenosis (group 2). Hyperlipidemia was more frequent in group 1 ((46.5% vs. 33.6%, p=0.032), and diabetes was less frequent (12.4 vs. 23.8%;p=0.011). Otherwise there was no significant difference in baseline characteristics, pretreatment tPA , and ASPECTS between two groups. With respect to outcome, successful recanalization (TICI 2b-3) was lower in group 1 than group 2 (70% vs. 80%; p=0.038). Similarly TICI 3 was achieved in less patients in group 1 (24% vs. 40%;p=0.022). Favorable outcome (mRS 0-2), sICH and procedural complications were similar in both groups. Conclusion: Initial angiographic severity of extracranial ICA lesion in tandem occlusions was associated with lower recanalization rates after MT, without impact on clinical outcomes.

Abstract

Objectives The aim of this study was to identify the optimal endovascular approach in patients with acute stroke with tandem lesions. Background At present, there is no consensus about the ideal technical strategy for the endovascular treatment of patients with acute ischemic stroke with tandem lesions of the extracranial internal carotid artery (ICA) and intracranial cerebral arteries. Methods This was an international, multicenter registry with a total of 482 patients with acute ischemic stroke and tandem lesions. Patients were treated by intracranial thrombectomy as well as 1 of the following 4 strategies: 1) acute carotid artery stenting of the extracranial ICA with antithrombotic agents; 2) acute carotid artery stenting of the extracranial ICA without antithrombotic agents; 3) balloon angioplasty of the extracranial ICA; and 4) intracranial thrombectomy alone. The main outcome endpoints of the study were the degree of recanalization and the 90-day clinical outcome. The safety endpoints were symptomatic intracerebral hemorrhage and all causes of mortality at 90 days. Results Using univariate analysis, the rates of successful reperfusion (modified Thrombolysis in Cerebral Infarction grades 2B and 3) and favorable clinical outcome after 90 days were significantly higher after acute carotid stenting with antithrombotic therapy and thrombectomy compared with the group with thrombectomy alone. After adjusting for confounding variables, acute stenting with antithrombotic therapy was independently associated with successful recanalization (odds ratio: 2.4; 95% confidence interval: 1.25 to 4.59; p = 0.008). The rates of symptomatic intracerebral hemorrhage and 90-day mortality were comparable among all 4 treatment groups. Conclusions Acute stenting of the extracranial ICA with antithrombotic therapy in combination with intracranial thrombectomy is associated with higher recanalization rates in treatment of patients with acute stroke with tandem lesions.

Abstract

This paper presents an experimental characterization of the strain dependency of the complex modulus of bituminous mixtures for strain amplitude levels lower than about $110~\upmu\mbox{m}/\mbox{m}$ . A series of strain amplitude sweep tests are performed at different temperatures (8, 10, 12 and 14°C) and frequencies (0.3, 1, 3 and 10 Hz), during which complex modulus is monitored. For each combination of temperature and frequency, four maximum strain amplitudes are targeted (50, 75, 100 and $110~\upmu\mbox{m}/\mbox{m}$ ). For each of them, two series of 50 loading cycles are applied, respectively at decreasing and increasing strain amplitudes. Before each decreasing strain sweep and after each increasing strain sweep, 5 cycles are performed at constant maximum targeted strain amplitude. Experimental results show that the behavior of the studied material is strain dependent. The norm of the complex modulus decreases and phase angle increases with strain amplitude. Results are presented in Black and Cole–Cole plots, where characteristic directions of nonlinearity can be identified. Both the effects of nonlinearity in terms of the complex modulus variation and of the direction of nonlinearity in Black space seem to validate the time–temperature superposition principle with the same shift factors as for linear viscoelasticity. The comparison between results obtained during increasing and decreasing strain sweeps suggests the existence of another phenomenon occurring during cyclic loading, which appears to systematically induce a decrease of the norm of the complex modulus and an increase of the phase angle, regardless of the type of the strain sweep (increasing or decreasing).

The paper focuses on the investigation of the effects of a recycling agent on mechanical performances of bituminous mixtures produced with reclaimed asphalt pavement (RAP). Twenty-one different bituminous mixtures were produced and tested, using three distinct virgin binders (15/25, 35/50 and 70/100 penetration grades), with RAP contents varying from 0% to 60%. Seven of the mixtures were produced by adding a recycling agent of vegetable origin (its content was fixed equal to 3.5% by weight of the RAP binder). Abstract

Considerable research efforts are made to develop methods to predict mechanical behavior of bituminous mixtures from properties of basic components. In particular, such methods would be most useful for the design of mixtures containing Reclaimed Asphalt Pavement (RAP) material. The objective of the study is the simulation of Linear ViscoElastic (LVE) behavior of bituminous mixtures containing RAP from LVE properties of base constituents. Five mixtures were produced and tested, a reference mixture produced only with 35/50 binder, a mixture containing 100% RAP and three mixtures produced with 35/50 binder and different RAP contents (20%, 40% and 60%). All mixtures had the same granular size distribution curve. Complete LVE characterization was carried out on all materials. DSR and Tension/Compression (T/C) tests were performed on base and RAP-extracted binders (from -30 C to +70 C and from 0.01Hz to 30Hz). T/C tests were carried out on mixtures (from -25 C to 40 C and from 0.001Hz to 10Hz). Test results on both binders and mixtures were successfully fitted with 2S2P1D (2 Springs, 2 Parabolic elements, 1 Dashpot) model. LVE behavior of mixtures containing RAP was simulated by using as input data only LVE properties of base binders (35/50 and RAP-extracted binders) and of the reference mixture (produced with 35/50 binder, without RAP). In order to do this, two existing procedures, previously developed at the ENTPE, were used conjunctly. The first procedure was applied to predict LVE behavior of binder blends of pure base and RAP-extracted binders over the whole range of frequency and temperature. This procedure allows estimating 2S2P1D parameters of binder blends from those of base bitumens, according to their proportions in the blend. Therefore, LVE properties of blends can be predicted over the whole range of frequency and temperature. The second procedure used is SHStS (Shift, Homothety, Shift in time, Shift) transformation. This analytical tool is useful to determine the relationship between LVE behavior of mixtures and their corresponding binders. Experimental data of pure 35/50 binder and its corresponding mixture (without RAP) were used to calibrate SHStS transformation, depending mainly on aggregate skeleton. LVE behavior of mixtures produced with RAP was then estimated from simulations of LVE properties of blends, having the same proportions of base binders, obtained with the first procedure. Simulations of LVE behavior of mixtures containing RAP were finally compared to T/C test data. Successful correspondence was found between predicted and experimental results. Small discrepancies observed can be reasonably attributed to incomplete blending of base and RAP binders within the mixture. Therefore, as a first approximation, the proposed procedure can be used to predict LVE behavior of mixtures produced with RAP, over the whole range of frequency and temperature, from LVE properties of base binders and of a reference mixture (with the same granular size distribution) are known.

Abstract

Various phenomena other than fatigue (so-called “biasing effects”) occur during laboratory fatigue tests on asphalt mixes because of cyclic loading applications, thus altering experimental results and leading to misleading conclusions. The purpose of the study is to isolate and quantify biasing effects, therefore isolating real fatigue damage. In particular, non-linearity, self-heating and thixotropy (defined as a recoverable viscosity reduction after shear application) were evaluated. Abstract

This study aims at understanding from a statistical point of view the influence of several mix constituents on mechanical properties of asphalt mixes produced with RAP material. The study is developed in two steps. The first section is focused on the qualitative evaluation of the effect of varying certain mix design parameters (aggregate and filler nature, binder content and type) on mechanical properties of mixes containing 20 % of RAP material. Results of the statistical analysis showed that changes in binder nature and binder content have a statistically significant influence on complex modulus of mixes. The second section aims at quantitatively estimating the influence of RAP material content and fresh added binder penetration grade on complex modulus and fatigue life of mixes. Proposed quadratic models satisfactorily fitted experimental data, although some coefficients were not estimated as statistically significant by the software. Observed fatigue and complex modulus performances of mixes containing up to 60 % RAP material are quite encouraging from technical, economical and environmental points of view.

Abstract

The main issue with asphalt pavement recycling is the assessment of the influence of aged bitumen in reclaimed asphalt pavement (RAP) material on viscoelastic properties of a blended binder in the final mixture. Blending charts or empirical laws, such as the “log–log” rule, are currently used to estimate grading parameters for material characterisation.
