Available online: Human alveolar echinococcosis (AE) is a severe hepatic disease caused by Echinococcus multilocula-ris. In France, the definitive and intermediate hosts of E. multilocularis (foxes and rodents, respectively) have a broader geographical distribution than that of human AE. In this two-part study, we describe the link between AE incidence in France between 1982 and 2007 and climatic and landscape characteristics. National-level analysis demonstrated a dramatic increase in AE risk in areas with very cold winters and high annual rainfall levels. Notably, 52% (207/401) of cases resided in French communes (smallest French administrative level) with a mountain climate. The mountain climate communes displayed a 133-fold (95% CI: 95–191) increase in AE risk compared with communes in which the majority of the population resides. A case–control study performed in the most affected areas confirmed the link between AE risk and climatic factors. This arm of the study also revealed that populations residing in forest or pasture areas were at high risk of developing AE. We therefore hypothesised that snow-covered ground may facilitate predators to track their prey, thus increasing E. multilocularis biomass in foxes. Such climatic and landscape conditions could lead to an increased risk of developing AE among humans residing in nearby areas.

International audience; Our objective was to evaluate T2-corrected IVIM and perfusion imaging using a MR-DCE technique for the distinction between non-advanced and advanced fibrosis in patients with chronic liver diseases. The link between perfusion-related diffusion given by IVIM and quantitative perfusion parameters given by MR-DCE imaging was investigated. Results indicated that the combination of IVIM and MR-DCE imaging do not bring additional information for fibrosis assessment in a large spectra of etiologies. Indeed, perfusion parameters given by MR-DCE imaging alone are relevant to evaluate fibrosis severity. Strong correlation between portal perfusion and perfusion related diffusion coefficient illustrated that IVIM reflects the hemodynamic changes occurring in fibrous damage. Pure molecular diffusion coefficient was affected by the deposition of extracellular matrix components and by fat vesicle suggesting that fat overload can constitute a confounding factor in fibrosis assessment with IVIM. Nevertheless, if fat overload is addressed, IVIM could be a useful injection-free method to distinguish between non-advanced and advanced fibrosis.

Abstract

International audience; Renal dysfunction is frequent in liver cirrhosis and is a strong prognostic predictor of orthotopic liver transplantation (OLT) outcome. Therefore, an accurate evaluation of the glomerular filtration rate (GFR) is crucial in pre-OLT patients. However, in these patients plasma creatinine (Pcr) is inaccurate and the place of serum cystatine C (CystC) is still debated. New GFR-predicting equations, based on standardized assays of Pcr and/or CystC, have been recently recommended in the general population but their performance in cirrhosis patients has been rarely studied. We evaluated the performance of the recently published Chronic Kidney Disease Epidemiology Collaboration equations (CKD-EPI-Pcr, CKD-EPI-CystC, and CKD-EPI-Pcr-CystC) and the more classical ones (4- and 6-variable MDRD and Hoek formulas) in cirrhosis patients referred for renal evaluation before OLT. Inulin clearance was performed in 202 consecutive patients together with the determination of Pcr and CystC with assays traceable to primary reference materials. The performance of the GFR-predicting equations was evaluated according to ascites severity (no, moderate, or refractory) and to hepatic and renal dysfunctions (MELD score \textless/= or \textgreater15 and KDOQI stages, respectively). In the whole population, CystC-based equations showed a better performance than Pcr-based ones (lower bias and higher 10% and 30% accuracies). CKD-EPI-CystC equation showed the best performance whatever the ascites severity and in presence of a significant renal dysfunction (GFR \textless60 mL/min/1.73 m(2)). CONCLUSION: Pcr-based GFR predicting equations are not reliable in pre-OLT patients even when an IDMS-traceable enzymatic Pcr assay is used. Whenever a CystC-assay traceable to primary reference materials is performed and when a true measurement of GFR is not possible, CystC-based equations, especially CKD-EPI-CystC, may be recommended to evaluate renal function and for KDOQI staging.

Abstract

International audience; Purpose: To evaluate the combination of liver intra-voxel incoherent motion imaging (IVIM) and a MRI fat quantification method at 3.0 T to make the distinction between NASH and pure steatosis. Methods and Materials: A multi-angle multi-gradient echo acquisition combined with a dedicated algorithm was used to quantify liver fat volume fraction (FVF), corrected for water and fat relaxation times and accounting for the NMR spectrum of fat. IVIM was performed using a single-shot SE-EPI sequence, in free breathing, with 11 b-values (0-20-40-60-80-100-200-300-400-600-800s.mm-2) and a weighted signal averaging procedure. Three orthogonal diffusion gradients were sequentially applied. A 2000 ms TR, 54 ms minimum TE; 21 axial slices of 8 mm thick; 400×300 mm² FOV; 128×96 matrix were used. IVIM parameters (pure molecular diffusion coefficient (DSlow), perfusion-related diffusion coefficient (DFast) and perfusion fraction (f)) were estimated. MR imaging was performed on a healthy volunteers group (n=33), and on patients with histologically confirmed pure steatosis (n=16) or NASH (n=4). Results: Wilcoxon test showed that DSlow was significantly lower when fat is present (FVF > 5.5%) (p < 0.01). DSlow and DFast were found significantly reduced between subjects with pure steatosis (Brunt=0) and NASH (Brunt> 0) (p < 0.05). Conclusion: Fat vesicles in pure steatosis restrict molecular diffusion without modification of micro-perfusion. In NASH, fibrosis still increase the restriction of molecular diffusion compared to patients with pure steatosis and decrease the microperfusion as illustrated by the decrease of perfusion-related diffusion coefficient. These preliminary results suggest that combining MRI fat quantification and IVIM could be a none invasive mean to distinguish between pure steatosis and NASH.

Abstract

article; Autoimmune pancreatitis is a rare disease characterized by inflammation of the pancreatic parenchyma, irregular narrowing of the pancreatic duct, periductal lymphoplasmacytic infiltration and fibrosis at histological examination, the presence of autoantibodies and hypergammaglobulinemia, as well as the possible association of cholangitis and other autoimmune diseases. There is a favorable response to steroid therapy. We report the case of a patient with autoimmune pancreatitis with bile duct involvement and peripheral eosinophilia, requiring long-term immunosuppressant treatment. The diagnosis of a diffuse form of AIP was made without direct histological evidence and based on indirect imaging, clinical and laboratory findings in an autoimmune context. The histological and imaging studies of bile duct involvement and the favourable response to steroids were additional arguments.

14. Anti-beta4 integrin antibodies enhance migratory and invasive abilities of human colon adenocarcinoma cells and their MMP-2 expression

Abstract

Integrin-mediated adhesion of cells to extracellular matrix proteins has been shown to activate various intracellular signaling events. In the present study, we demonstrate that the addition of a monoclonal antibody raised against the beta4 integrin subunit in the culture medium of a clone derived from the colon adenocarcinoma cell line LoVo specifically results in stimulation of cell migration and invasion through reconstituted basement membrane matrices. Moreover, an increase in MMP-2 activity is observed. Conversely, monoclonal anti-alpha6 and anti-beta1 have no effect on MMP-2 expression. The s. c. co-injection of adenocarcinoma cells with antibodies raised against the beta4 integrin subunit to immunosuppressed newborn rats gives rise to tumors displaying altered and disorganized peri-tumoral basement membranes compared with tumors obtained when cells are injected with adenocarcinoma cells alone. Higher metastatic capacity of cells results when they are co-injected with antibodies to the beta4 integrin subunit. Abstract

International audience; The aim of this study was to collect data from patients who underwent liver transplantation (LT) for adenomatosis; to analyze the symptoms, the characteristics of the disease, and the recipient outcomes; and to better define the role of LT in this rare indication. This retrospective multicenter study, based on data from the European Liver Transplant Registry, encompassed patients who underwent LT for adenomatosis between January 1, 1986, and July 15, 2013, in Europe. Patients with glycogen storage disease (GSD) type IA were not excluded. This study included 49 patients. Sixteen patients had GSD, and 7 had liver vascular abnormalities. The main indications for transplantation were either a suspicion of hepatocellular carcinoma (HCC; 15 patients) or a histologically proven HCC (16 patients), but only 17 had actual malignant transformation (MT) of adenomas. GSD status was similar for the 2 groups, except for age and the presence of HCC on explants (P = 0.030). Three patients with HCC on explant developed recurrence after transplantation. We obtained and studied the pathomolecular characteristics for 23 patients. In conclusion, LT should remain an extremely rare treatment for adenomatosis. Indications for transplantation primarily concern the MT of adenomas. The decision should rely on morphological data and histological evidence of MT. Additional indications should be discussed on a case-by-case basis. In this report, we propose a simplified approach to this decision-making process.
