Abstract: International audience; In France, the horsemeat market has been declining for about 50 years and has become a specialized market. Our study aims to understand this decline with regard to the drivers and practices of consumers and non-consumers of this meat, in order to estimate the potential for this market development. To study horsemeat consumption, we analyze two sets of data: two large-scale surveys carried out on the French general population, and one ad-hoc survey focusing more specifically on the representations of horses and horsemeat. Our results underline the potential for increasing horsemeat consumption from a sustainability perspective. The question of moral acceptance remains a determining factor in the consumption of this meat. Once this factor is taken into account, horsemeat appears relevant in the diversification of animal protein consumption because of its particular nutritional and environmental properties and similar culinary use to that of other red meats. Horsemeat can thus lay claim to becoming a sustainable alternative to beef consumption.

Abstract: International audience; Background: Fatigue is commonly reported by patients with inflammatory bowel disease (IBD), but the determinants of IBD-related fatigue have yet to be determined.Aims: To identify the factors associated with fatigue in a large population of patients with IBD.Patients and methods: Fatigue and nine other IBD-related disability dimensions were assessed in a cohort of 1704 consecutive patients with IBD using the IBD-disk questionnaire in a cross-sectional survey of 42 French and Belgian centers. Fatigue and severe fatigue were defined as energy subscores >5 and >7, respectively. Determinants of fatigue were assessed using univariate and multivariate analyses (odds ratios (ORs) are provided with 95% confidence intervals).Results: The prevalence rates of fatigue and severe fatigue were 54.1% and 37.1%, respectively. Both fatigue and severe fatigue were significantly higher in patients with active disease than in patients with inactive disease (64.9% vs. 44.7% and 47.4% vs. 28.6%, respectively; p40 years (OR=0.71 [0.54-0.93]), female sex (OR=1.48 [1.13-1.93]), IBD-related sick leave (OR=1.61 (1.19-2.16]), and joint pain (OR=1.60 [1.17-2.18]), abdominal pain (OR=1.78 [1.29-2.45]), regulating defecation (OR=1.67 [1.20-2.32]), education and work (OR=1.96 [1.40-2.75]), body image (OR=1.38 [1.02-1.86]), sleep (OR=3.60 [2.66-4.88]) and emotions (OR=3.60 [2.66-4.88]) subscores > 5.Conclusion: Determinants of fatigue are not restricted to IBD-related factors but also include social factors, sleep, and emotional disturbances, thus supporting a holistic approach to IBD patient care.

Abstract: Crohn’s disease (CD) is associated with an increased risk of small bowel neoplasia (SBN). We aimed to assess preoperative predictors of SBN in CD patients. We conducted a retrospective case-control study including CD patients who underwent surgery: cases were diagnosed with SBN on histopathological analysis and controls had no neoplasia. Preoperative cross-sectional imaging was reviewed by a panel of blinded expert radiologists. Fifty cases were matched to one hundred and fifty consecutive controls. In multivariable analysis, predictors of SBN were age ≥ 50 years (OR = 28, 95% CI = 5.05–206), median CD duration ≥ 17.5 years (OR = 4.25, 95% CI = 1.33–14.3), and surgery for stricture (OR = 5.84, 95% CI = 1.27–35.4). The predictors of small bowel adenocarcinoma were age ≥ 50 years (OR = 5.14, 95% CI = 2.12–12.7), CD duration ≥ 15 years (OR = 5.65, 95% CI = 2.33–14.3), and digestive wall thickening > 8 mm (OR = 3.79, 95% CI = 1.45–11.3). A predictive score based on the aforementioned factors was constructed. Almost 73.7% of patients with a high score had SBA. Old age, long small bowel CD duration, and stricture predicted the presence of SBN, particularly adenocarcinoma when patients have digestive wall thickening > 8 mm on preoperative imaging.

Abstract: International audience; Graft rejection is a critical risk in solid-organ transplantation. To decrease such risk, an understanding of the factors involved in low immunogenicity of liver allografts could potentially make it possible to transfer this tolerogenic property to other transplanted organs. HLA-G, a natural physiological molecule belonging to the Human Leukocyte Antigen class (HLA) Ib family that induces tolerance, is associated with fewer rejections in solid-organ transplantation. In contrast to HLA-G, HLA antigen incompatibilities between donor and recipient can lead to rejection, except in liver transplantation. We compared HLA-G plasma levels and the presence of anti-HLA antibodies before and after LT to understand the low immunogenicity of the liver. We conducted a large prospective study that included 118 patients on HLA-G plasma levels during a 12-month follow-up and compared them to the status of anti-HLA antibodies. HLA-G plasma levels were evaluated by ELISA at seven defined pre- and post-LT time points. HLA-G plasma levels were stable over time pre-LT and were not associated with patient characteristics. The level increased until the third month post-LT, before decreasing to a level comparable to that of the pre-LT period at one year of follow-up. Such evolution was independent of biological markers and immunosuppressive treatment, except with glucocorticoids. An HLA-G plasma level ≤ 50 ng/ml on day 8 after LT was significantly associated with a higher rejection risk. We also observed a higher percentage of rejection in the presence of donor specific anti-HLA antibodies (DSA) and an association between the increase in HLA-G plasma levels at three months and the absence of DSA. The low immunogenicity of liver allografts could be related to early elevated levels of HLA-G, which lead, in turn, to a decrease in anti-HLA antibodies, opening potential new therapeutic strategies using synthetic HLA-G proteins.

Abstract: International audience; BACKGROUND: Whether healthcare workers with inflammatory bowel disease (IBD) are at increased risk of Novel coronavirus disease (COVID-19) due to occupational exposure is unknown. AIM: To assess the risk of COVID-19 in healthcare workers with IBD. METHODS: A case control study enrolled 326 healthcare workers with IBD from 17 GETAID centres and matched non-healthcare workers with IBD controls (1:1) for gender, age, disease subtype and year of diagnosis. The study period was year 2020 during the COVID-19 outbreak. RESULTS: In total, 59 COVID-19 were recorded among cases (n = 32) and controls (n = 27), including 2 severe COVID-19 (requiring hospitalization, mechanic ventilation) but no death. No difference was observed between healthcare workers and controls regarding the overall incidence rates of COVID-19 4.9 ± 2.2 vs. 3.8 ± 1.9 per 100 patient-semesters, P = 0.34) and the overall incidence rates of severe COVID-19 (0.6 ± 7.8 vs. 0.3 ± 5.5 per 100 patient-semesters, P = 0.42). In multivariate analysis in the entire study population, COVID-19 was associated with patients with body mass index > 30 kg/m(2) (HR = 2.48, 95%CI [1.13-5.44], P = 0.02). CONCLUSION: Healthcare workers with IBD do not have an increased risk of COVID-19 compared with other patients with IBD.

Abstract: Background We aimed to compare the effectiveness of tofacitinib and ustekinumab in patients with ulcerative colitis (UC) previously exposed to at least one anti-TNF agent. Methods In this multicenter study, we retrospectively included consecutive UC adult patients previously exposed to at least one anti-TNF agent, with partial Mayo score (pMS) > 2, having who started tofacitinib or ustekinumab between January 2019 and June 2022. The primary endpoint was steroid-free clinical remission (pMS ≤ 2) (CFREM) at week 16 (W16). Secondary endpoints were endoscopic remission (CFREM + Mayo endoscopic score (MES) ≤ 1), and histological remission (CFREM + MES ≤ 1 + Nancy index ≤ 1). Comparisons were performed using propensity score analyses adjusted on potential confounders. Results Overall, 289 patients were included (tofacitinib = 124 patients, ustekinumab = 165 patients). The groups were comparable (tofacitinib vs vedolizumab) for male gender (50.8% vs 43.3%), mean age (40.7 vs 42.9 years), median UC duration (8.6 vs 9.3 years), concomitant use of 5-ASA (13.7% vs 9.7%), steroids (25.0% vs 29.7%), immunosuppressants (7.3% vs 5.5%), and ≥ 2 prior biologics (85.5% vs 82.4%). Tofacitinib group had more pancolitis (55.6% vs 42.4%, p = 0.026) and UC with pMS > 6 (64.5% vs 50.3%, p = 0.016). In our study, 42.1 % of the patients treated with tofacitinib continued using a dose of 10 mgx2/day until W16 while 47.3% of the patients on ustekinumab required dose escalation to 90 mg/4 weeks before W16. After propensity score analysis, the rate of CFREM at W16 was 37.8% and 35.6% in the tofacitinib and ustekinumab arms, respectively arm (p=0.75). CFREM at W16 was achieved in 43.3% vs 57.1% (p = 0.48) after failure of one biologic, 20.7% vs 37.9% (p=0.16) two biologics and 46.7% vs 23.2% (p=0.047) or ≥ 3 biologics, in tofacitinib and vedolizumab arms, respectively. After primary failure to at least one biologic, the rate of CFREM at W16 was 46.3% on tofacitinib vs 25.9% on ustekinumab (p = 0.13). CFREM at W16 was similar with tofacitinib and ustekinumab in case of more severe UC such as pMS ≥ 6 (40.6% vs 41.5%) and CRP > 30 (27.2% vs 33.0%). No predictor of tofacitinib effectiveness has been identified. Factors associated with no CFREM at W16 on ustekinumab were male gender (p=0.035), ≥ 3 prior biologics (p=0.013), prior use of tofacitinib (p=0.03), primary failure to at least one biologic (p=0.013). Endoscopic remission was achieved in 17.0% vs 11.7% (p =0.47) and histological remission in 4.4% vs 7.8% (p=0.32) of the patients treated with tofacitinib and ustekinumab, respectively. Conclusion Tofacitinib and ustekinumab have similar effectiveness in UC after anti-TNF failure. However, the efficacy of ustekinumab could be more impacted by prior therapeutic failures.

Abstract: International audience; Background While the effect of anti-TNFs on postoperative outcomes in patients with Crohn's disease (CD) has been widely studied, the impact of vedolizumab and ustekinumab on the risk of postoperative complications remains poorly known. Methods All consecutive patients who underwent intestinal resection for CD between July 2014 and April 2022 within 22 French centers were included in a retrospective cohort. The risk of early post-operative complications (≤30days) in patients exposed to biologics was compared to patients not exposed by logistic regression and propensity score-matched analysis adjusted for age, previous intestinal resection, corticosteroids or immunosuppressants exposure, disease activity, presence of abscess, urgent surgery and initial stoma (preoperative contra-indication to anastomosis). Results Among the 1201 patients included, respectively 491 (41%), 76 (6.3%) and 57 (4.7%) were exposed to anti-TNFs, ustekinumab, or vedolizumab within six months before surgery. A total of 317 (26.4%) patients had at least one complication of which 123 (38%) were considered as severe (DINDO III/IV). New surgery was necessary in 69 (5.7%) patients and secondary stoma in 23 (1.9%). Three deaths were observed (0.25%). The rates of overall complications in patients not exposed to biologics, exposed to anti-TNFs, ustekinumab or vedolizumab were respectively 26.1%, 25.1%, 34.7% and 29.8%. The risks of intra-abdominal infectious complications in these four groups were respectively 13.5%, 11.1%, 13.3% and 8.8%. In multivariate analysis, age [OR, 1.02 (1.01-1.04); p=0.004], disease activity [OR, 8.36 (1.79 – 149); p=0.037], the presence of an abscess [OR, 2.01 (1.25-3.20); p=0.004] and initial stoma [OR, 1.70 (1.10 –2.61); p=0.016] were significantly associated with intra-abdominal infectious complications. Conversely, preoperative enteral nutrition [OR, 0.12 (0.01 -0.59); p=0.040] was associated with a reduction in this risk. Exposure to anti-TNFs [OR, 0.80 (0.51-1.24); p=0.31], ustekinumab [OR, 1.17 (0.39-3.51); p=0.78] and vedolizumab [OR, 1.28 (0.32-5.17); p=0.72] within the 3 months before surgery were not associated with the risk of intra-abdominal infectious complications. Similar results were observed in patients exposed to these treatments in the month before surgery. Conclusion In this large cohort, a quarter of patients operated on for CD presented an early postoperative complication and 10% a severe complication. Preoperative exposure to anti-TNFs, vedolizumab or ustekinumab was not associated with an increased risk of early postoperative complications. Preoperative enteral nutrition was associated with a reduced risk of intra-abdominal infectious complication.

Abstract: Recent research conducted largely in the United States suggests that most people would like to change one or more of their personality traits. Yet almost no research has investigated the degree to which and in what ways volitional personality change (VPC), or individuals' active efforts toward personality change, might be common around the world. Through a custom-built website, 13,278 college student participants from 55 countries and one of a larger country (Hong Kong, S.A.R.) using 42 different languages reported whether they were currently trying to change their personality and, if so, what they were trying to change. Around the world, 60.40% of participants reported that they are currently trying to change their personalities, with the highest percentage in Thailand (81.91%) and the lowest in Kenya (21.41%). Among those who provide open-ended responses to the aspect of personality they are trying to change, the most common goals were to increase emotional stability (29.73%), conscientiousness (19.71%), extraversion (15.94%), and agreeableness (13.53%). In line with previous research, students who are trying to change any personality trait tend to have relatively low levels of emotional stability and happiness. Moreover, those with relatively low levels of socially desirable traits reported attempting to increase what they lacked. These principal findings were generalizable around the world. Abstract: International audience; Background & aims: The management of intra-abdominal abscesses complicating Crohn's disease (CD) is challenging and surgery with delayed intestinal resection is often recommended. The aims of this study were to estimate the success rate of adalimumab (ADA) in CD patients with an intra-abdominal abscess resolved without surgery, and to identify predictive factors for success.Methods: A multicenter, prospective study was conducted in biologic-naïve CD patients with resolved intra-abdominal abscess treated with ADA with a two-year follow-up. The primary endpoint was ADA failure at W24 defined as a need for steroids after W12, intestinal resection, abscess recurrence, and clinical relapse. Secondary post-hoc endpoint was the long-term success defined as the survival without abscess relapse or intestinal resection at W104. The factors associated with ADA failure at W24 and W104 were identified using a logistic and a cox regression, respectively.Results: From April 2013 to December 2017, 190 patients from 27 GETAID centers were screened, and 117 were included in the analysis. Fifty-eight (50%) patients were male and the median age at baseline was 28 years. At W24, 87 (74%, 95%CI: 65.5-82.0, n=117) patients achieved ADA success. Among the 30 patients with ADA failure, 15 underwent surgery. At W104, the survival rate without abscess recurrence or surgery was 72·9% (95%CI: 62.1-79.8, n=109). Abscess drainage was significantly associated with ADA failure at W24 (OR=4.18; 95%CI: 1.06-16.5; p=0.043). Disease duration (HR=1.32, 95%CI: 1.09-1.59, p=0.008), abscess drainage (HR=5.59, 95%CI: 2.21-14.15, p=0.001), and inflammatory changes in mesenteric fat (HR=0.4, 95%CI: 0.17-0.94, p=0.046) were significantly associated with ADA failure at W104.Conclusion: Provided that the abscess was carefully managed before initiating medical treatment, this study showed the high efficacy of ADA in the short and long term in biologic-naïve patients with CD complicated by an intra-abdominal abscess.

Abstract: International audience; We describe the endocranial structures of Hamadasuchus, a peirosaurid crocodylomorph from the late Albian-Cenomanian Kem Kem group of Morocco. The cranial endocast, associated nerves and arteries, endosseous labyrinths, and cranial pneumatization, as well as the bones of the braincase of a new specimen, are reconstructed and compared with extant and fossil crocodylomorphs, which represent different lifestyles. Cranial bones of this specimen are identified as belonging to Hamadasuchus, with close affinities with Rukwasuchus yajabalijekundu, another peirosaurid from the ‘middle’ Cretaceous of Tanzania. The endocranial structures are comparable to those of R. yajabalijekundu but also to baurusuchids and sebecids (sebecosuchians). Paleobiological traits of Hamadasuchus, such as alert head posture, ecology, and behavior are explored for the first time, using quantitative metrics. The expanded but narrow semi-circular canals and enlarged pneumatization of the skull of Hamadasuchus are linked to a terrestrial lifestyle. Continuing work on the neuroanatomy of supposedly terrestrial crocodylomorphs needs to be broadened to other groups and will allow to characterize whether some internal structures are affected by the lifestyle of these organisms.

In the last twenty years, faced with the constant rise of festival events in Europe and internationally, an increasing number of researchers have focused on the « film festival » as an object of study, by considering it as a conceptual construct of a specific way of looking at cinema (and through it, at reality), and at the same time, as a site of intense professional exchanges and a space of mediation between audiences and film criticism.The aim of this edited collection is to contribute to the identification of a new film festival period which has characterised the 2000s from the French perspective. The approach of the contributing authors consists in thinking about the emergence of « festival societies » in the twenty-first century from an anthropological perspective, focusing on their modes of existence, on the one hand, and questioning the way in which, through their programming acts, festivals think, create and reflect on the representation of contemporary worlds, on the other hand. The fundamental question that underpins these fifteen contributions concerns the way in which otherness is created and circulated within these festival cultures whose complex nature lies at the intersection of sociology, economics, geo-politics and aesthetics., Si les pratiques festivalières mobilisent depuis longtemps la recherche scientifique, il semble qu’un tournant ait été opéré à la fin du XXe siècle avec l’émergence des Film Festival Studies. Depuis une vingtaine d’années, face à l’essor constant des manifestations festivalières en Europe et à l’international, de plus en plus de chercheur.e.s se sont en effet focalisé.e.s sur l’objet « festival de cinéma » en l’envisageant à la fois comme la construction d’un regard spécifique sur le cinéma (et à travers lui sur le réel), un lieu de transactions professionnelles intenses et un espace de médiation avec les publics et la critique de cinéma. L’ambition de cette publication collective est de contribuer à l’identification d’une nouvelle période festivalière caractéristique des années 2000 d’un point de vue cette fois français. La démarche des auteur.e.s ici réuni.e.s a consisté à penser l’émergence des sociétés festivalières du XXIe siècle dans une perspective anthropologique, en s’attachant à leurs modes d'existence d'une part et en interrogeant la manière dont, par leurs actes de programmation, les festivals pensent et fondent la représentation des mondes contemporains d'autre part. La question de fond qui anime les quinze présentes contributions porte sur la façon dont se crée et circule l’altérité dans ces cultures festivalières dont la nature complexe se situe à l’intersection de la sociologie, de l’économie, de la géo-politique et de l’esthétique.

Abstract: International audience; Sebecids constitute a family of notosuchian crocodylomorphs of probable terrestrial habits. They are notable for having survived the Cretaceous-Paleogene mass extinction and are known until the Miocene in South America. However, ecological traits that favoured their resilience in continental ecosystems remain unknown. Here, using a multi-isotopic approach, we infer several palaeoecological traits of a sebecid from the locality of Tiupampa (Bolivia), which contains a diverse vertebrate fauna of the Early Paleocene. After having constrained the effects of diagenesis, the study of the oxygen isotope compositions of apatite phosphate allows the identification of water resources for the various studied taxa and to infer an ectothermic thermoregulation strategy for the sebecids. A terrestrial lifestyle is also supported by different oxygen isotope compositions observed between sebecids and aquatic dyrosaurid crocodylomorphs. The radiogenic strontium isotope compositions confirm the continental affinities of this fauna, with no marine input in the palaeoenvironment frequented by the different faunal elements. The calcium and carbon isotope compositions show that the sebecids from Tiupampa were at the top of a C3-based food web. Finally, a local dry environment with tropical temperatures for those specimens is inferred both from oxygen and carbon isotope compositions of their hard tissues.

Abstract: International audience; Osteoderms of eight extant and extinct species of crocodylomorphs are studied histologically and morphologically. Most osteoderms display the typical "crocodilian" structure with a woven-fibered matrix surrounded by an upper and a lower parallel fibered matrix. The dorsal ornamentation of those specimens consists of a pit-and-ridge structure, with corresponding remodeling mechanisms. However, an osteoderm of Iberosuchus, studied here for the first time, differs in being nearly devoid of ornamentation; moreover, it shows strong bundles of straight Sharpey's fibers perpendicular to the surface in its lateral and dorsal walls, along with a rough plywood-like structure in its basal plate. This suggests that this osteoderm was more deeply anchored within the dermis than the other osteoderms studied hitherto. This peculiar structure might have been linked to a terrestrial ecology and a specific thermoregulation strategy. Some other notosuchians in our sample do not exhibit ornamentation on their osteoderms, as opposed to neosuchians. Considering current interpretations of osteoderm function(s) in crocodilians, our observations are discussed in reference to possible ecophysiological peculiarities of Notosuchia in general, and Iberosuchus in particular.

Abstract: International audience; Nowadays, malnutrition (i.e. over and undernutrition) is a major public health issue. This reflects energy and macro and micronutrient imbalances. Among strategies to fight malnutrition, governmental food programs, fortification and nutritional optimization of staple food can be alternatively or concomitantly adopted. Pasta is one of the most popular staple foods. Traditionally made from wheat, its formulation has been improved by including legumes beside wheat. The next effort for improving its formulation would be to use raw materials from climate-smart crops. Our study takes place in the European H2020 Innofood Africa project federating European and African countries on the development of climate-smart crops and their use in the context of a balanced diet. It aims to develop new nutritionally optimized pasta made from gluten free African raw materials adapted to the climate transition e.g. drought resistance, low input requirement. Raw materials are cowpea, faba bean and bambara legume flours; teff, finger millet and amaranth cereal grain flours and additional nutritious raw materials such as orange fleshed sweet potato and amaranth leave flours. Nutritional and antinutritional composition of all flours were determined (protein, amino acids content, starch, sugar, fibres, vitamins, minerals, phytic acids, trypsin inhibitor). This constituted a data base for nutritional optimization of pasta by mathematical programming. In order to cover adults needs, the following criteria were selected: an amino acid chemical score above 110 taking into account process and cooking losses, an omega 6/omega 3 ratio closest to 5, a fiber quantity for 100g of pasta less than 25g, quantities of iron, zinc, B9 vitamin and beta carotene at less equal to recommendations and minimization of anti-nutritional factors. The best formulations covering the nutritional requirements, mainly based on cowpea, teff, amaranth leaves flours, have been selected. They were processed by a classical low temperature extrusion followed by a low temperature drying preserving their nutritional qualities. They have been characterized and compared with classical 100% durum wheat semolina pasta for their textural and culinary properties and their nutritional composition. The in vitro protein and starch digestibility was carried out to determine the impact of formulations on the nutritional potential of pasta.

Abstract: International audience; The Last Interglacial (LIG or Marine Isotopic Substage 5.5, MIS5.5), which is comprised between 129 and 116 thousand years Before Present (hereafter ky BP), is poorly documented, especially in continental areas, owing to the scarcity of deposits attributable to this period, which corresponds to the Eemian in the chronostratigraphy of northwestern Europe. The scarcity of docu- mentation is even more pronounced for the deglaciation that precedes the LIG, the MIS 6/5 Transition or Termination 2, which lasted about ten thousand years. In this context, the site of Waziers, in Western Europe (France), has an exceptional sequence of successive fine silty-limestone deposits covering the penultimate deglaciation (MIS 6/5) and peat rich in plant remains contemporary with the last interglacial (MIS 5.5). Oxygen isotope analysis of phosphate of tooth enamel from large herbivorous mammals (horse, Equus sp. ; megaloceros, Megaloceros giganteus ; roe deer, Capreolus capreolus) present in these deposits allows the reconstruction of air paleotemperatures during the lifetime of the animals. The results yield annual mean temperatures of 4.7 ± 0.6°C for an early phase of the deglaciation (5 teeth of a same individual), of 6.9 ± 0.5°C for a mid-phase of the deglaciation (1 tooth), and of 8.6 ± 0.5°C for a phase preceding the establishment of the last interglacial, the MIS5.5 (1 tooth). In deposits contemporary with the latter phase, human artifacts were discovered, as well as in those of MIS5.5. In agreement with the palynological data, the paleotemperatures illustrate a gradual warming during part of the glacial/interglacial transition, the MIS6/5 Transition. The difference of 4°C between the minimal and maximal temperature reconstructed in this study allows us to minimize the temperature difference between the MIS6.2 glacial period and the MIS5.5 interglacial period at the Waziers site.; La dernière période interglaciaire (Last Interglacial, LIG, ou Stade Isotopique Marin 5.5, SIM 5.5), comprise entre environs 129 et environs 16 milliers d’années avant aujourd’hui (ci-dessous ka), est peu documentée, en particulier en domaine continental en raison de la rareté des dépôts attribuables à cette période qui correspond à l’Eemien dans la chronostratigraphie de l’Europe du nord-ouest. Le manque de documentation est encore plus prononcé pour la déglaciation qui précède le LIG, la Transition SIM 6/5 ou Terminaison 2, qui a duré une dizaine de milliers d’années. Dans ce contexte, le site de Waziers, localisé en Europe occidentale (France), présente une séquence exceptionnelle constituée de dépôts successifs fins limono-calcaires couvrant l’avant-dernière déglaciation (SIM 6/5) et des tourbes riches en restes végétaux contemporaines du dernier interglaciaire (SIM 5.5). L’analyse isotopique de l’oxygène du phosphate de l’émail de dents de grands mammifères herbivores (cheval, Equus sp. ; mégalocéros, Megaloceros giganteus ; chevreuil, Capreolus capreolus) présents dans ces dépôts a permis de reconstituer les paléotempératures de l’air du vivant des animaux. Les résultats donnent des températures moyennes annuelles de 4,7 ± 0,6°C pour une première phase de la déglaciation (5 dents d’un même individu), de 6,9 ± 0,5°C pour une phase médiane de la déglaciation (1 dent), et de 8,6 ± 0,5°C pour une phase précédant l’établissement du dernier interglaciaire, le SIM 5.5 (1 dent). Des artefacts paléolithiques (silex) ont été découverts dans les dépôts contemporains de cette dernière phase - ainsi que dans ceux attribués au SIM 5.5. En accord avec les données palyno- logiques, les paléotempératures mettent en évidence un réchauffement progressif au cours d’une partie de la transition glaciaire/ interglaciaire, la Transition SIM 6/5. L’écart de environ 4°C entre la température minimale et la température maximale reconstituées ici permet de minorer l’écart de température entre la période glaciaire SIM 6.2 et la période interglaciaire SIM 5.5 sur le site de Waziers.

Abstract: International audience; Objectives: Type-I interferons (IFNs-I) have potent antiviral effects. IFNs-I are also overproduced in patients with systemic lupus erythematosus (SLE). Autoantibodies (AAbs) neutralising IFN-α, IFN-β and/or IFN-ω subtypes are strong determinants of hypoxemic COVID-19 pneumonia, but their impact on inflammation remains unknown.Methods: We retrospectively analysed a monocentric longitudinal cohort of 609 patients with SLE. Serum AAbs against IFN-α were quantified by ELISA and functionally assessed by abolishment of Madin-Darby bovine kidney cell protection by IFN-α2 against vesicular stomatitis virus challenge. Serum-neutralising activity against IFN-α2, IFN-β and IFN-ω was also determined with a reporter luciferase activity assay. SARS-CoV-2 antibody responses were measured against wild-type spike antigen, while serum-neutralising activity was assessed against the SARS-CoV-2 historical strain and variants of concerns.Results: Neutralising and non-neutralising anti-IFN-α antibodies are present at a frequency of 3.3% and 8.4%, respectively, in individuals with SLE. AAbs neutralising IFN-α, unlike non-neutralising AAbs, are associated with reduced IFN-α serum levels and a reduced likelihood to develop active disease. However, they predispose patients to an increased risk of herpes zoster and severe COVID-19 pneumonia. Severe COVID-19 pneumonia in patients with SLE is mostly associated with combined neutralisation of different IFNs-I. Finally, anti-IFN-α AAbs do not interfere with COVID-19 vaccine humoral immunogenicity.Conclusion: The production of non-neutralising and neutralising anti-IFN-I antibodies in SLE is likely to be a consequence of SLE-associated high IFN-I serum levels, with a beneficial effect on disease activity, yet a greater viral risk. This finding reinforces the recommendations for vaccination against SARS-CoV-2 in SLE.

Abstract: International audience; This article questions the categorical status of evaluative derived lexemes built by suffixation. It would indeed seem that, whatever the lexical class of the latter – noun (maison ‘house’ / maisonnette ‘little house’), adjective (jaune ‘’yellow / jaunâtre ‘yellowish’) or verb (taper ‘to hit’ / tapoter ‘to tap’) – the relationship between the base and the derived lexeme is not identical to that observed in the case of prototypical lexical hierarchies such as hyperonymy (maison ‘house’ / villa ‘villa’; cf. e. g. Kleiber 1990). Thus, for example, while a noun like villa denotes a type of house (Mihatsch 2007), it seems difficult to claim that maisonnette ‘small house’ also denotes a type of house. It is therefore not by chance that, in lexical semantics, evaluative lexemes are never integrated into lexical hierarchies: morphological evaluation can indeed be considered as one of the mechanisms allowing the expression of the categorical approximation.

Abstract: International audience; Among marine reptiles of the Mesozoic, ichthyosaurs exemplify a typical case of morphologicaladaptation toward a pelagic lifestyle. Beside their extensive stratigraphic range, spanning the earlyTriassic to mid-Cretaceous interval, they are noteworthy for having achieved gigantic (up to 21 m totallength) body sizes during the first third of their evolutionary history. Such sizes were never paralleledin subsequent ichthyosaur lineages. The presence of such giants, belonging to Shastasauridae andCymbospondylidae, is now well attested from the North American fossil record, especially prior to theNorian (e.g. Nicholls and Manabe, 2004; Scheyer et al., 2014; Sander et al., 2021). Although alreadyrecorded from Asia by limited material such as Himalayasaurus (Motani et al., 1999), the Europeanrecord also begins to unveil giant forms of younger stratigraphic age (Fischer et al., 2014; Martin et al.2015; Lomax et al., 2018; Sander et al., 2022). Yet, despite their giant size, the European record islimited to a handful of partial skeletal elements. Here, we report new postcranial material belonging toa giant shastasaurid from the Rhaetian of Var, southern France. Abstract: International audience; Background and aims: The harmful impact of heavy alcohol recurrence has been confirmed in patients with alcohol-related cirrhosis although this is based on old studies. The drivers of long-term outcome still need to be clearly investigated.Method: All patients with biopsy-proven compensated alcohol-related cirrhosis included in the CIRRAL cohort (22 centers) were prospectively studied. Prognostic variables of survival and liver event-free survival were assessed using multivariable Cox models with stepwise selection. The prognostic impact of alcohol recurrence during follow-up (computed in glasses-years in the same way as pack-years for tobacco) was assessed using a time-dependent covariable.Results: From 2010 to 2016, 650 patients were included. The median age at baseline was 58.4 years old, 67.4% were men and the median BMI was 27.8 kg/m2, 63.8% had a history of liver decompensation, and 70.2% had discontinued alcohol. At 5 years, 30.9% of abstinent patients developed recurrence and this risk was higher in patients with a history of drug abuse and in those with shorter alcohol discontinuation times. Median survival was 97 months. Age, alcohol consumption at baseline, platelet count and Child-Pugh score >5 were associated with overall and liver event-free survival on multivariate analysis. Alcohol consumption during follow-up of more than 25 glasses-year was independently associated with lower survival and with a trend toward significant liver event-free survival, but the risk increased from 1 glass-year, though not significantly. The study aims to determine the prevalence of CVD and VTE risk factors in a large population of patients with ulcerative colitis (UC).Methods: We conducted a cross-sectional study in 33 French and Belgium referral centers. A questionnaire was developed to explore self-reported risk factors for VTE and CVD, based on the latest international guidelines, in consecutive patients with UC.Results: A total of 1071 patients with UC were included. There were 539 women (50.3%), and the median age of patients was 44 years [32; 57]. The median disease duration was 10 years [6; 17]. In the cohort, 36.5% of patients reported no cardiovascular risk factor (CVRF) and 72% had ≤ 1 CVRF. Regarding cardiovascular risk markers (CVRM) 36.9% of patients reported no CVRM and 78% had ≤ 1 CVRM. Of the 1071 patients, 91.3% of patients reported no VTE strong risk factor and 96% had ≤ 1 VTE moderate risk factor.Conclusion: This is the first cohort specifically designed to assess both VTE and CVD risks in patients with UC. More than one third of patients with UC had no CVRF and around three quarters had ≤ 1 CVRF. In addition, more than nine out of ten patients had no VTE strong risk factor and ≤ 1 moderate risk factor. Physicians should be aware of these factors in their patients.

Abstract: International audience; Obesity is associated with vitamin D (VD) deficiency and arterial stiffness. This randomized control trial assessed the effects of VD supplementation during a weight-loss program on carotid intima-media thickness (IMT) and carotid compliance in obese adolescents. Participants were randomly assigned to receive either a 12-week lifestyle program with VD supplementation (n = 13), a lifestyle program without VD supplementation (n = 13) or a control group composed of normal-weight adolescents (n = 18). Serum total and free 25-hydroxyvitamin D (25(OH)D), IMT and carotid compliance were measured before and after the trial. Insufficiency in 25(OH)D concentration was found in 73% of obese participants compared to 22% among controls. Obese adolescents had lower free 25(OH)D and displayed higher IMT but lower carotid compliance than controls. Free 25(OH)D and IMT were negatively correlated in adolescents displaying VD insufficiency at baseline. After three months, total and free 25(OH)D increased in both groups. The changes of IMT and carotid compliance were similar between groups. The changes in IMT were correlated with the changes in total 25(OH)D in obese adolescents with VD insufficiency at baseline (r = −0.59, p = 0.03). While the lifestyle program with VD supplementation did not affect carotid compliance, IMT reduction was improved in obese adolescents

Abstract: International audience; BACKGROUND: The exact rate of contraindications to anti-TNF therapy and physician perspectives on treatment choices facing to anti-TNF contraindication, are poorly reported. METHODS: A two-week cross-sectional study was conducted in 31 centres. Physicians completed a questionnaire for a total of 1,314 consecutive outpatients with Crohn’s disease, assessing each patient’s potential contraindications to anti-TNF therapy, the choice of alternative therapy to anti-TNFs, and their preference in an unrestricted reimbursement setting. RESULTS: Among the 1,293 responses to the first item, 148 (11.5%) reported 32 absolute contraindications (2.5%) and 116 relative contraindications (9.0%) to anti-TNF therapy. When asked about their preference of alternative therapies in those cases with contraindications to anti-TNF, physicians chose ustekinumab and vedolizumab, 75.6% and 23.9%, respectively. In multivariable analysis, the choice of vedolizumab was the preferred choice for patients aged > 60 years with the L2 phenotype and the absence of perianal lesions. In a hypothetical setting of unrestricted reimbursement, anti-TNFs remained physicians’ preferred first-line biological therapy choice for 78.2%. CONCLUSION: Anti-TNF contraindications occurred in up to 11.5% of patients with Crohn’s disease. Physicians’ choices for alternative therapy to anti-TNF relied on ustekinumab in 75.6% and vedolizumab in 23.9% of these cases. This choice was driven mainly by phenotypical criteria and age.

Abstract: International audience; It is well established that downsizing mechanical structures make their surface-over-volume ratio much larger than for usual objects, so that their ability to interact with their environment is significantly augmented. This has been particularly used in the development of cantilever sensors, where a surface energy change on one side induces the cantilever’s bending. Even though this phenomenon has been implemented for a wide range of molecules, the modeling the mechanical response of a micromechanical structure to a surface energy change has been scarcely investigated. Secondstrain gradient elasticity [1] has been shown to naturally involve the equivalent of surface tension for solids, and thus seems particularly suited to model the deformation of solids subjected to a surface energy change. The constitutive law is however rather complex, so that the experimental identification of the constitutive parameters is a challenge to overcome. First results from an original approach are reported herein. Beams made of a material driven by second-strain gradient elasticity are expected to display short-wavelength sinusoidal components in their displacement field. Abstract: International audience; Both food safety and dietary behaviors are major contributors to the global burden of disease, especially in rapidly urbanising environments. The impact that food safety concerns have on dietary behaviors in low- and middle-income countries (LMICs) is insufficiently documented. Therefore, we examined whether food safety concerns influence consumer behaviors/diets in LMICs. A systematic review identified 46 relevant studies from 20 LMICs for inclusion. A socio-ecological food environment framework was used to map food safety factors that influence consumer behaviors (food acquisition/purchase, eating out of home, food preparation/storage) and diets (consumption of nutrient rich/poor foods). Several studies (n = 11) reported that despite food safety concerns, consumers could not always ensure that they consumed safe food; barriers were affordability, accessibility and appeal. Key concerns included fear of pesticides, fertilizers, hygiene in/around food outlets, unhygienic vendor practices and household storage/preparation methods. These concerns may reduce consumption of animal sourced food and fresh fruit and vegetables; and increase consumption of starchy staples and processed/packaged foods. Policies such as upgrading urban market infrastructure to enhance food safety, accompanied by nutrition and hygiene education, could lead to increased accessibility, affordability and appeal of safe, nutrient-rich foods. Thus, reducing the appeal of packaged/processed food as a means to mitigate food safety risk; thereby contributing to preventing foodborne disease and multiple forms of malnutrition.

Abstract: International audience; Background and aims - Optimal management of patients with inflammatory bowel disease [IBD] after anti-tumour necrosis factor [TNF] discontinuation due to severe induced skin lesions is unclear. Our study aimed to describe dermatological and IBD evolution after anti-TNF discontinuation for this side effect. Methods - We conducted a multicentre retrospective study including consecutive IBD patients who discontinued anti-TNF due to severe induced skin lesions. Our objectives were to determine factors associated with dermatological remission [complete disappearance of skin lesions] and with IBD relapse in patients with inactive disease at inclusion, notably the impact of an early switch to another biological agent within 3 months of anti-TNF discontinuation. Results - Among the 181 patients [134 women, 160 Crohn's disease] included in the 13 participating centres, dermatological remission occurred in 110 [62%] patients with a median [interquartile range, IQR] interval of 8.0 [6.8-11.0] months. Scalp location was independently associated with less remission of skin lesions (hazard ratio [HR] = 0.64 [95% CI 0.43-0.94], p = 0.02) while early switch was independently associated with a higher probability of remission of skin lesions (HR = 1.64 [95% CI 1.1-2.5], p = 0.02). Among the 148 patients with inactive IBD at inclusion, disease relapse occurred in 75 [51%] patients with a median [IQR] interval of 26.0 [23.0-39.1] months. Results: French clinical guidelines for the diagnostic and management of ulcerative colitis were drafted by 16 GETAID members and reviewed by 6 members of the GETAID, the Association Nationale des Hépato-gastroentérologues des Hôpitaux généraux (ANGH) and the Club de Réflexion des Cabinets et Groupes d'Hépato-Gastroentérologie (CREGG), graded according to the level of Evidence and the level of agreement by 68 experts from the latter three societies. The present guidelines focused on classification, diagnosis, histopathology, extra-intestinal manifestations, dysplasia and colorectal cancer, pouchitis, medical management of active disease and maintenance therapy.. Conclusion: This paper provides updated and useful guidelines for French gastroenterologists to diagnose and treat patients with ulcerative colitis. © John Libbey Eurotext, 2022.

Abstract: Background and Aims There are concerns regarding the potential impact of the COVID-19 outbreak on patients with inflammatory bowel disease [IBD]. We report on the impact of the COVID-19 outbreak in a European prospective cohort study of patients with IBD Patients and Methods We prospectively collected data from 5457 patients with IBD nested in the ongoing I-CARE project and still followed up in April 2020, with monthly online monitoring of clinical activity, treatment, imaging and endoscopy. Investigators were also contacted to report incidental cases. Results In total, 233 [4.3%] reported COVID-19 and 12 [0.2%] severe COVID-19, with no COVID-19 deaths. The risk of COVID-19 in patients with IBD was not increased compared to the general population (standardized incidence ratio [SIR]: 1.18, 95% confidence interval [CI] [1.03–1.34], p = 0.009), as well as the risk of severe COVID-19 (SIR: 0.69, 95% CI [0.35–1.20], p = 0.93). We did not observe any negative impact of the different IBD-related medication on the risk of either COVID-19 or severe COVID-19. In 2020, the COVID-19 outbreak resulted in a drastic decrease in endoscopic and imaging procedures from March to May 2020 compared to 2018 and 2019. No impacts on clinical IBD disease activity as well as ongoing treatment were noted. Conclusion No increases in either COVID-19 or severe COVID-19 incidences were observed in patients with IBD. There was no impact of COVID-19 on IBD-related medication and clinical activity. Access to endoscopy and imaging was restricted during the first months of the first COVID-19 outbreak.

Abstract: International audience; Background: Data comparing tofacitinib and vedolizumab in ulcerative colitis (UC) are lacking.Aims: To compare the effectiveness of tofacitinib and vedolizumab in patients with UC who had prior exposure to anti-TNF therapy METHODS: In this multicentre study, we included consecutive patients with UC ≥18 years old with partial Mayo score >2 and prior anti-TNF exposure, who started tofacitinib or vedolizumab between January 2019 and June 2021. Comparisons were performed using propensity score analyses (inverse probability of treatment weighting).Results: Overall, 126 and 178 patients received tofacitinib and vedolizumab, respectively. Intensified induction (vedolizumab infusion at week 10 or tofacitinib 10 mg b.d until week 16) was performed in 28.5% and 41.5% of patients, respectively. After propensity-score analysis, corticosteroid-free clinical remission (partial Mayo score ≤2) was achieved at week 16 in 45.1% and 40.2% of patients receiving tofacitinib and vedolizumab, respectively (aOR = 0.82 [0.35-1.91], p = 0.64). Endoscopic improvement (corticosteroid-free clinical remission and endoscopic Mayo score ≤1) (aOR = 0.23[0.08-0.65], p = 0.0032) and histological healing (endoscopic improvement + Nancy histological index ≤1) (13.4% vs 3.2%, aOR = 0.21[0.05-0.91], p = 0.023) were higher at week 16 in patients treated with tofacitinib. No factor was predictive of tofacitinib effectiveness. At least one primary failure to a biologic (OR = 0.46[0.22-0.99], p = 0.049), partial Mayo score >6 (OR = 0.39[0.17-0.90], p = 0.029) and CRP level > 30 mg/L at baseline (OR = 0.08[0.01-0.85], p = 0.036) were associated with vedolizumab failure.Conclusion: Tofacitinib and vedolizumab are effective in UC after failure of anti-TNF agents. However, tofacitinib seems more effective, especially in severe disease and primary failure to biologics.

Abstract: Background Little is known about clinical characteristics and kidney outcomes in patients with biopsy-proven immunoglobulin A nephropathy (IgAN) in a context of inflammatory bowel disease (IBD). Methods We conducted a retrospective multicentre study with a centralized histological review to analyse the presentation, therapeutic management and outcome of 24 patients suffering from IBD-associated IgAN relative to a cohort of 134 patients with primary IgAN without IBD. Results Crohn’s disease and ulcerative colitis accounted for 75 and 25% of IBD-associated IgAN cases, respectively. IBD was diagnosed before IgAN in 23 cases (a mean of 9 years previously) and was considered active at IgAN onset in 23.6% of patients. Hypertension was present in 41.7% of patients. The urinary protein:creatinine ratio exceeded 100 mg/mmol in 70.8% of patients (mean 254 mg/mmol). Estimated glomerular filtration rate (eGFR) was >60 mL/min/1.73 m2 in 13/24 patients and only 1 patient required dialysis. In the Oxford mesangial hypercellularity, endocapillary cellularity, segmental sclerosis and interstitial fibrosis/tubular atrophy with crescents classification of renal biopsies, 57% were M1, 48% E1, 76% S1, 57% T1–2 and 38% C1–2. Steroids were administered in 50% of cases. After a mean follow-up of 7.2 years, 4 patients (16.7%) had a poor kidney outcome: end-stage renal disease (n = 3) or a >50% decrease in eGFR from initial values (n = 1). A similar evolution was observed in patients with primitive IgAN. Conclusions This first case series suggests that IBD-associated IgAN has frequent inflammatory lesions at onset and variable long-term outcomes.

Abstract: International audience; INTRODUCTION: There are currently no comparative data on the efficacy and safety of vedolizumab and ustekinumab in ulcerative colitis (UC) after anti-TNF therapy fails. METHODS: We retrieved the full datasets of two observational, multicentre, retrospective studies of patients with UC for whom anti-TNF therapy failed and the patients were then treated with either vedolizumab or ustekinumab. The outcomes included steroid-free clinical remission, clinical remission, treatment persistence, colectomy, hospitalization, and serious and infectious adverse events. Propensity scores weighted comparison was applied. RESULTS: In total, 121 patients were included in the vedolizumab group and 97 were included in the ustekinumab group. At week 14 and week 52, in the weighted cohort, no difference was found between vedolizumab and ustekinumab for steroid-free clinical remission (OR = 0.55 [0.21-1.41], p\,=\,.21 and 0.94 [0.40-2.22], p\,=\,.89, respectively). There was no difference between vedolizumab and ustekinumab for secondary outcomes such as clinical remission, hospitalization, UC-related surgery, treatment persistence and serious and infectious adverse events. CONCLUSION: In patients with UC for whom anti-TNF therapy failed, no difference was found between vedolizumab and ustekinumab after propensity scores weighted comparison. Further studies are required to determine predictive factors of the efficacy of both biological agents.

Abstract: International audience; The vertebrate fossil record documents a plethora of transitions between aquatic and terrestrial environments but their causes are still debated. Quantifying the salinity of living environments is therefore crucial for precising the sequence of ecological transitions. Here, we measured lithium stable isotope composition of mineralized tissues (δ 7 Limt) of extant and extinct vertebrates from various aquatic environments: seawater, freshwater/terrestrial, and "transitional environments" (i.e. brackish waters, or seasonal access to freshwater and seawater). We report statistically higher δ 7 Limt values for seawater vertebrates than freshwater ones, taxonomic groups considered separately. Moreover, vertebrates living in transitional environments have intermediate δ 7 Limt values. Therefore, we show that δ 7 Limt values of both extant and extinct vertebrates can discriminate their aquatic habitat.

Abstract: Strategies used by marine vertebrates to regulate their body temperature can result in local variations, and the knowledge of these regional heterothermies is crucial for better understanding the thermophysiologies of extant and extinct organisms. In order to investigate regional heterothermies in vertebrates, we analysed the oxygen isotope composition of phosphatic skeletal elements (δ18Op) of two endothermic fishes (Thunnus thynnus and Xiphias gladius) and three dolphins (two Delphinus delphis delphis and one Cephalorhynchus commersonii kerguelensis). We observed a consistent link between δ18Op variations and temperature heterogeneities recorded by classical methods. Our δ18Op data indicate that: (i) bone hydroxylapatite of the axial skeleton of dolphins mineralise at a warmer temperature than that of the appendicular one, (ii) the skull is the warmest body region in X. gladius, and (iii) T. thynnus possesses high body temperature in the skull and visceral mass region. These results demonstrate the possibility of tracking regional heterothermies in extant marine vertebrates using the δ18Op, paving the way to direct assessment of thermophysiological specificities of both living and extinct vertebrates. From a palaeoenvironmental perspective, the significant observed δ18Op variability questions the use of some taxa or random skeletal elements for the reconstruction of palaeoceanographic parameters such as seawater temperature and δ18O.

Abstract: Background and aims Inflammatory bowel diseases (IBD), including Crohn’s disease (CD) and ulcerative colitis (UC), and human immunodeficiency virus (HIV) both impact innate and adaptive immunity in the intestinal mucosa. As it is a rare situation, the intersection between HIV and IBD remains unclear, especially the impact of HIV infection on the course of IBD, and the drug safety profile is unknown. Methods We conducted a multicenter retrospective cohort study between January 2019 and August 2020. All adult patients with IBD and concomitant HIV infection were included. Each IBD patient with HIV was matched to two HIV-uninfected IBD patients. Results Overall, 195 patients with IBD were included, including 65 HIV-infected patients and 130 without HIV infection. Of the 65 infected patients, 22 (33.8%) required immunosuppressants and 31 (47.7%) biologics. In the HIV-infected group, the need for immunosuppressants (p = 0.034 for CD and p = 0.012 for UC) and biologics (p = 0.004 for CD and p = 0.008 for UC) was significantly lower. The disease course, using a severity composite criterion, was not significantly different between the two groups for CD (hazard ration (HR) = 1.3 [0.7; 2.4], p = 0.45) and UC (HR, 1.1 [0.5; 2.7], p = 0.767). The overall drug safety profile was statistically similar between the two groups. Conclusion Although HIV-infected patients receive less treatments, the course of their IBD did not differ than uninfected, suggesting that HIV infection might attenuate IBD. The drug safety profile is reassuring, allowing physician to treat these patients according to current recommendations.

Abstract: Background Owing to growing number of therapeutic options with similar efficacy and safety, we compared the acceptability of therapeutic maintenance regimens in inflammatory bowel disease (IBD). Methods From a nationwide study (24 public or private centers), IBD patients were consecutively included for 6 weeks. A dedicated questionnaire including acceptability numerical scales (ANS) ranging from 0 to 10 (highest acceptability) was administered to both patients and related physicians. Results Among 1850 included patients (65.9% with Crohn’s disease), the ANS were 8.68 ± 2.52 for oral route (first choice in 65.8%), 7.67 ± 2.94 for subcutaneous injections (first choice in 21.4%), and 6.79 ± 3.31 for intravenous infusions (first choice in 12.8%; P < .001 for each comparison). In biologic-naïve patients (n = 315), the most accepted maintenance regimens were oral intake once (ANS = 8.8 ± 2.2) or twice (ANS = 6.9 ± 3.4) daily and subcutaneous injections every 12 or 8 weeks (ANS = 7.9 ± 3.0 and ANS = 7.2 ± 3.2, respectively). Among 342 patients with prior exposure to subcutaneous biologics, the preferred regimens were subcutaneous injections (≥2 week-intervals; ANS between 9.1 ± 2.3 and 8.1 ± 2.7) and oral intake once daily (ANS = 7.7 ± 3.2); although it was subcutaneous injections every 12 or 8 weeks (ANS = 8.4 ± 3.0 and ANS = 8.1 ± 3.0, respectively) and oral intake once daily (ANS = 7.6 ± 3.1) in case of prior exposure to intravenous biologics (n = 1181). The impact of usual therapeutic escalation or de-escalation was mild (effect size Conclusions Although oral intake is overall preferred, acceptability is highly impacted by the rhythm of administration and prior medication exposures. However, SC treatment with long intervals between 2 injections (≥8 weeks) and oral intake once daily seems to be the most accepted modalities.

Abstract: International audience; BACKGROUND: Patients’ experience with healthcare professionals could influence their clinical outcomes. AIMS: To assess inflammatory bowel disease (IBD) patients’ experience with their disease, their treatment and their relationship with their physician. METHODS: A one-week cross-sectional study was conducted in 42 IBD centres. 2011 consecutive outpatients with IBD completed an anonymous self-report questionnaire assessing their experience with and knowledge of IBD. RESULTS: A quantitative assessment of the doctor-patient relationship revealed that patients’ knowledge of IBD and IBD treatment ranged from 7.4 to 8.3 out of 10. In addition to IBD physicians, other sources of information about IBD and current treatment mainly included the internet (80% and 63%, respectively) and general practitioners (61% and 54%). Knowledge about education programmes (28%) was poor, resulting in a lack of willingness to further use these resources (25%). Concerns about IBD treatment were raised in 76% of patients, mostly related to the fear of adverse events (47%) and a lack of efficacy (33%). The need of alternative healthcare professionals was reported by 89% of the sample. CONCLUSION: In a large cohort of patients, we highlighted gaps in the management of patients with IBD regarding the need for higher-quality information and the implementation of alternative healthcare professionals.
