Abstract

AIM In an experimental study the accuracy of ultrasound in the detection of lesions of the achilles tendon was examined. METHODS 60 Achilles tendons were examined post mortem macroscopically, histologically and by means of ultrascan. RESULTS All macroscopical changes were found in the sonograms. Especially changes of the form proved to be good diagnostic patterns for degenerative lesions of the tendon. At a distance of 2 cm proximal to the calcaneus histological changes could be detected with an accuracy of 73.3%. At 4 and 6 cm to the calcaneus we found similar results. CONCLUSION A differentiation of the histological diagnosis by means of the sonographic behaviour was not possible. As a reproducible, effective and inexpensive method ultrasound examination has proved very useful in the detection of degenerative lesions of the Achilles tendon.

Abstract

Objective: To quantify the influence of patient positioning on the determination of collodiaphyseal (CCD) angle and femoral neck anteversion based on computed tomography. Description of a method for positional correction. Methods: We evaluated the CT scans of 84 femora from 77 patients, who received individually adapted prostheses. Projected as well as real anteversion and CCD angles were determined and corrected for positioning errors using a newly proposed mathematical method, and compared to other evaluation methods. Results and Conclusions: We determined an average tilt of 4.2 in the sagittal and of 9° in the coronary plane, which correlated definitely with the deviations found in the graphical determination of the projected anteversion angle. This produced measurement errors of the projected anteversion angle of up to 10 . Mathematically independent angles, such as projected anteversion and real CCD angle, showed only a weak correlation. This indicates also an anatomical independence of these angles.

Abstract

The diagnostic value of MRI and ultrasound in the examination of 37 patients with chronic shoulder pain is compared. All the results were controlled or by arthroscopy/arthrotomy or by arthrography. Ruptures and degenerative changes of the rotator cuff and degenerative lesions could be detected with a high sensibility. The ultrasound examination as a fast, reproducible diagnostic method has now become part of the routine diagnosis. The MRI produces exacter information about the extent of the lesions; but it will be reserved for special questions.

Abstract

The possibilities of MRI and ultrasound in visualizing the structures of the shoulder joint are compared. Both methods have a good accuracy in detecting changes of the soft tissue of the shoulder. Changes of the cartilage and the bony structures are better seen in the MRI. The MRI gives a static and clear view of the shoulder joint, meanwhile the ultrasound depends on the dynamic examination. Ultrasound and MRI are both reproducible and non-invasive methods, who seem to complete one another at the examination of the shoulder.
