Objective The aim of this study was to evaluate the relationship between cardiovascular risk scores and the visceral adiposity index (VAI) in patients with psoriatic disease (PsD). Methods A total of 101 PsD patients meeting psoriatic arthritis (CASPAR) criteria and 98 healthy individuals were included in this study. Systematic coronary risk evaluation index (SCORE), Framingham risk score (FS), and VAI values in patients and VAI values in healthy individuals were calculated. Results The mean body mass index (BMI) (kg/m²) of the patients was 29.63±5.66. According to the SCORE measurements, the patients were classified as low risk, moderate risk, and high risk-53 (52.5%), 45 (44.6%), and 3 (3%), respectively. No patient was found to be at very high risk. According to the FS, the low-risk, moderate-risk, and high-risk patient rates were 72 (71.3%), 22 (21.8%), and 7 (6.9%), respectively. Statistically significantly higher rates of metabolic syndrome, obesity (BMI >30), and VAI levels were found in the PsA group compared to the healthy control group (p<0.05). Statistically significantly higher VAI levels were observed in PsA patients with metabolic syndrome, BMI >30 (obesity), diabetes mellitus, and hypertension compared to patients without these comorbidities (p<0.05). A statistically significant correlation was found between low- and moderate-risk FS and VAI levels of PsA patients. Conclusion Since VAI can be easily calculated using routine tests, it can provide information about cardiometabolic risks and comorbidities in newly diagnosed PsD patients. Tofacitinib, the first member of targeted synthetic disease-modifying antirheumatic drugs, is an oral inhibitor of Janus kinases (JAK), preferentially JAK 1 and 3. It is an analog of adenosine triphospahte and inhibits several proinflammatory cytokines and pathways. Tofacitinib is rapidly absorbed and eliminated mainly via the liver. The efficacy of tofacitinib has been studied extensively in rheumatoid arthritis (RA) patients with different clinical scenarios. Tofacitinib is now approved in several countries for the treatment of RA, psoriatic arthritis, ankylosing spondylitis, polyarticular and systemic juvenile idiopathic arthritis, and ulcerative colitis. In addition, studies to assess the efficacy of tofacitinib in patients with several different indications are under consideration. Neutropenia, anemia, elevation of transaminase levels, hyperlipidemia, and increased risk of infections with several causes are well-known side effects. However, recent data from the ORAL Surveillance study shed light on the risk of cardiovascular events and malignancy. In that study, RA patients over 50 years with at least 1 cardiovascular risk factor were randomized to anti-tumor necrosis factor or tofacitinib, revealing increased cardiovascular event risk and malignancy (especially lung cancer and lymphoma) in the tofacitinib arm. Although post-hoc analysis of the dataset suggested a possible link between a history of cardiovascular disease and both cardiovascular and malignancy endpoints, the Food And Drug Administration and European Medical Agency announced black-box warnings for all JAK inhibitors covering all indications. To determine the accuracy of magnetic resonance imaging-proton density fat fraction (MRI-PDFF) measurements for detecting liver fat content in potential living liver donors and to compare these results using liver biopsy findings. METHODS A total of 139 living liver donors (men/women: 83/56) who underwent MRI between January 2017 and September 2021 were included in this analysis retrospectively. The PDFFs were measured using both MR spectroscopy (MRS) and chemical shift-based MRI (CS-MRI) for each donor in a blinded manner. RESULTS Significant positive correlations were found between liver biopsy and MRS-PDFF and CS-MRI PDFF in terms of hepatic steatosis detection [r = 0.701, 95% confidence interval (CI): 0.604-0.798, r = 0.654, 95% CI: 0.544-0.765, P < 0.001, respectively). A weak level correlation was observed between liver biopsy, MRI methods, and vibration-controlled transient elastography attenuation parameters in 42 available donors. Based on receiver operating characteristic (ROC) analysis, MRS-PDFF and CSMRI PDFF significantly distinguished >5% of histopathologically detected hepatic steatosis with an area under the ROC curve (AUC) of 0.837 ± 0.036 (P < 0.001, 95% CI: 0.766-0.907) and 0.810 ± 0.036 (P < 0.001, 95% CI: 0.739-0.881), respectively. The negative predictive values (NPVs) of MRS-PDFF and CS-MRI PDFF were 88.3% and 81.3%, respectively. In terms of distinguishing between clinically significant hepatic steatosis (≥10% on histopathology), the AUC of MRS-PDFF and CS-MRI were 0.871 ± 0.034 (P < 0.001 95% CI: 0.804-0.937) and 0.855 ± 0.036 (P < 0.001, 95% CI: 0.784-0.925), respectively. The NPVs of MRS-PDFF and CS-MRI were 99% and 92%, respectively. CONCLUSION The methods of MRS-PDFF and CS-MRI PDFF provide a non-invasive and accurate approach for assessing hepatic steatosis in potential living liver donor candidates. These MRI PDFF techniques present a promising clinical advantage in the preoperative evaluation of living liver donors by eliminating the requirement for invasive procedures like liver biopsy. Introduction: In this study, we aimed to determine the functional dominance of the right and left hemispheres of the operated patients and the effectiveness of postoperative pain therapy when the patients underwent surgery on the same or different sides with the dominant hemisphere. Methods: The patients were given a Miles test for the dominant eye in the preoperative period and were instructed while the key was in front of the patient to take the key and open the door to determine the dominant hand. For the dominant foot, the patient was given the command of "move your foot" in a calm environment and accepted as the foot that he carried the dominant foot. After routine spinal anesthesia and elective knee arthroplasty surgery, patients underwent Patient Controlled Analgesia device, 24th hour Numeric Rating Scale (NRS) scores, and the amount of total applied analgesic and additional analgesic were recorded. Results: When the clinical characteristics of the patients were examined, there were 38 patients on the right side of the dominant eye, 34 on the left side, 67 on the right hand dominant side, 5 on the left side, 49 on the right foot dominant side, and 23 on the left side. The NRS score on the surgical side was compared with the dominant foot, dominant eye, and dominant hand. There was no statistically significant difference between the NRS pain scale and other variables except those opposed to the dominant eye (p>0.05). The NRS pain scale median (median: 3.0) was found to be significantly lower in patients who were operated on the same side as the dominant eye (p=0.016). Conclusion: According to the results of our study, the NRS score was not changed in patients operated on the same side of the dominant foot and hand, whereas the NRS score was lower in patients operated on the same side with the dominant eye. Objectives: Endometrial carcinoma (EC) is a typical gynecological malignant tumor that occurs more frequently every year. Obesity is a significant contributor to the development of EC and its prognosis. Lipid metabolism and malignant tumors have a long history of association. Elevated cholesterol levels are made possible by adenosine triphosphate-binding cassette protein A1 (ABCA1) deficiency, which eventually promotes cancer cell survival. The aim of this study was to examine at the ABCA1 gene expression levels in EC patients. The relationship between ABCA1 and the occurrence, progression, and prognosis of EC is discussed in this article as a potential mechanism. Materials and Methods: The samples of 45 endometrial adenocarcinoma patients were retrospectively included in this study and they were further divided into Grade 1 (15), Grade 2 (15), Grade 3 (15) tumors, control group. Twenty-nine endometrial tissues without a confirmed diagnosis of endometrial cancer made up the control group. ABCA1 gene expression was examined using real-time polymerase chain reaction. Results: According to the results, the gene expressions of the patient group were higher than the control group When each Grade was compared with the control group, statistically significant results were obtained. After analyzing the data, it was found that the patient group was generally higher than the control group (p < 0.05) and there were differences in the grades of the patient group (p < 0.05). When the ABCA1 expressions of the grade groups and control groups were compared separately, a difference was found between Grade 1, Grade 2 and Grade 3 and the control group (p= 0.0001). Conclusion: According to the findings of our study, a key component in the growth of EC tumors is the increase in cholesterol production caused by a reduction in ABCA1. [ABSTRACT FROM AUTHOR]

Introduction: Sedation of coronavirus disease-2019 (COVID-19) acute respiratory distress syndrome (ARDS) patients on mechanical ventilation (MV) has lately become a concern. The purpose of this study was to report the sedation strategy used in COVID-19 ARDS patients who were mechanically ventilated at a single institution. Methods: In this study, we performed a retrospective review of the sedation strategy in mechanically ventilated COVID-19 ARDS patients in our 37-bed intensive care unit. All mechanically ventilated COVID-19 ARDS patients who were sedated and hospitalized between March 2020 and September 2021 were included in this study. Patients reported using sedatives and analgesics as well as suffering from delirium. Results: This study involved 100 patients with COVID-19 ARDS who were both eligible to participate. In all patients, a triple sedation regimen was required. Ketamine attitudes reduced patients' opioid and benzodiazepine needs (p<0.05). Furthermore, the following ketamine administration, the need for vasopressors was significantly reduced (p<0.05). There were no drug interactions documented. Conclusion: We showed that extremely high sedative doses were required in this group of patients with COVID-19 ARDS who needed MV. However, our findings suggest that when ketamine infusion was introduced, benzodiazepine, opiate, and vasopressor doses were reduced without adverse pharmacological effects. Objective: The aim of the study was to investigate the association between geropsychiatric assessment scales and polypharmacy. Materials and Methods: A cross-sectional study was conducted that included patients =65 years, who applied to geriatric outpatients in a university hospital. Functionality was assessed by instrumental activities of daily living (IADL) and activities of daily living (ADL). The clinical frailty scale (CFS) was used for frailty screening. Multidimensional scale of perceived social support (MSPSS), mini-nutritional assessment-short form (MNA-SF), quality of life scale in older people (CASP-19), mini-mental state examination, loneliness scale for elderly, generalized anxiety disorder-7, geriatric depression scale (GDS), templer death anxiety scale, and cumulative illness rating scale for geriatrics (CIRS-G) were performed. Polypharmacy was defined as taking five or more medications. Results: The study included 136 patients with a median (interlique range) age of 72.2 (68.1-76.3). The prevalence of polypharmacy was 52.2%. Age, IADL, GDS, CFS, MSPSS, CASP-19, and CIRS-G scores significantly differed between the polypharmacy and non-polypharmacy groups (p<0.05). There were inverse correlations between the number of medications and ADL, IADL, MNA-SF, MSPSS, and CASP-19. GDS had a weak and positive correlation (p<0.001, r=0.322) whereas, CFS (p<0.001, r=0.463) and CIRS-G (p<0.001, r=0.530) had moderate and positive correlations. In multivariable analysis, age [odds ratio (OR), 1.087; 95% confidence interval (CI), 1.005-1.176], CFS (OR, 1.602; 95% CI, 1.048-2.448), and CIRS-G (OR, 1.273; 95% CI, 1.090- 1.486) were independent variables. Conclusion: Age, CIRS-G, and frailty score by using CFS are independent variables of polypharmacy. The relationship between polypharmacy and functional dependency, depression, social support, and life quality should be supported by future large and comprehensive studies. [ABSTRACT FROM AUTHOR]

PURPOSE We aimed to evaluate the spectrum of magnetic resonance cholangiopancreatography (MRCP) findings in patients with ectopic opening of the common bile duct (CBD) into the duodenal bulb and to determine the effectiveness of the MRCP technique in diagnosis. METHODS Morphologic and morphometric MRCP/MRI features in 16 patients and 36 controls were retrospectively analyzed by 2 radiologists. The frequency of MRCP findings was determined. The significance of the difference between the MRCP observations in patients and controls was evaluated statistically and the diagnostic effectiveness of MRCP was investigated. RESULTS Hook-shaped ending of CBD and bulbar deformity were the most frequent morphologic findings seen on MRCP in the ectopic bulbar opening. Mean pylorus-papilla distance and mean CBD length were significantly shorter and the median diameter of CBD was significantly larger than the control group (patients: 28.6 ± 15.3 mm, 33.7 ± 12.8 mm, 8.6 (2-16) mm; controls: 66.7 ± 11.7 mm, 50.3 ± 14.4 mm, 3.2 (1.5-10) mm, P < .001, respectively). Receiver operating curve analysis showed sensitivity and specificity of MRCP in the diagnosis to be 87.5% and 100%, respectively, if any 3 of the 4 signs (hook-shaped ending of CBD, bulbar deformity, large, and short CBD) were present in a patient whose pylorus-papilla distance was <50 mm. CONCLUSION At MRCP, the presence of short and large CBD with a hook-shaped ending in the deformed duodenal bulb may support the diagnosis of ectopic biliary drainage. [ABSTRACT FROM AUTHOR]

PURPOSE In Budd-Chiari syndrome (BCS), unevenly distributed parenchymal changes and perfusion abnormalities occur due to hepatic venous outflow obstruction. This study aimed to evaluate the changes in the liver parenchyma in BCS using the quantitative magnetic resonance (MR) techniques of MR elastography, T1 and T2 mapping, and diffusion imaging and correlate the quantitative MR parameters through biochemical results and prognostic indices. METHODS Fourteen patients with BCS (seven men and seven women) were examined retrospectively. Liver stiffness (kPa), T1 relaxation times (ms) were achieved using the modified Look-Locker inversion recovery (MOLLI) 3(2)3(2)5 sequence and B1-corrected variable flip angle methods, T2 relaxation times (ms), and apparent diffusion coefficient (ADC) values (mm2/s) were measured using regions of interest placed in the same region in all quantitative methods. Measurements were repeated at the precontrast and postcontrast hepatobiliary phases. The reduction rate (RR; %) and adjusted postcontrast T1 (%) were calculated. The values obtained from different liver parenchyma areas (whole liver, caudate lobe, pathological T2 hyperintense tissue, and relatively preserved normal-appearing tissue) were compared using the Wilcoxon signed-rank test. Spearman's correlation coefficient was used to investigate the correlation between quantitative MR parameters and biochemical parameters/prognostic scores (Child-Pugh score, Clichy score, and Rotterdam index). RESULTS The parenchymal stiffness and precontrast T1 values of the caudate lobe were significantly lower than those of the remainder of the parenchyma, whereas the adjusted postcontrast T1 percentages (MOLLI) were significantly higher (P = 0.027). The parenchymal stiffness value, T1 and T2 values, percentages of RR (MOLLI), and adjusted postcontrast T1 values for the pathological tissue and relatively normal tissue were significantly different (P < 0.028). No significant difference was found in terms of ADC values between any of the distinct regions of the liver. A strong correlation was detected between the Child-Pugh score, Clichy score, and precontrast T1 values obtained through the MOLLI sequence (r = 0.867, P = 0.012, r = 0.821, P = 0.023, respectively). No correlation was found between the whole liver stiffness values and the laboratory parameters, fibrosis markers, prognostic indices, or MR parameters. A significant correlation was identified between creatinine levels and several T1 parameters and the T2 relaxation time (r = 0.661, P = 0.052). CONCLUSION Tissue stiffness and T1 relaxation values are high in the areas identified as fibrosis compared with those in the relatively preserved parenchyma. PURPOSE To evaluate the frequency of abdominal computed tomography (CT) findings in patients with coronavirus disease-2019 (COVID-19) and interrogate the relationship between abdominal CT findings and patient demographic features, clinical findings, and laboratory test results as well as the CT atherosclerosis score in the abdominal aorta. METHODS This study was designed as a multicenter retrospective study. The abdominal CT findings of 1.181 patients with positive abdominal symptoms from 26 tertiary medical centers with a positive polymerase chain-reaction test for severe acute respiratory syndrome coronavirus 2 were reviewed. The frequency of ischemic and non-ischemic CT findings as well as the association between CT findings, clinical features, and abdominal aortic calcific atherosclerosis score (AA-CAS) were recorded. RESULTS Ischemic and non-ischemic abdominal CT findings were detected in 240 (20.3%) and 328 (27.7%) patients, respectively. In 147 patients (12.4%), intra-abdominal malignancy was present. The most frequent ischemic abdominal CT findings were bowel wall thickening (n = 120; 10.2%) and perivascular infiltration (n = 40; 3.4%). As for non-ischemic findings, colitis (n = 91; 7.7%) and small bowel inflammation (n = 73; 6.2%) constituted the most frequent disease processes. The duration of hospital stay was found to be higher in patients with abdominal CT findings than in patients without any positive findings (13.8 ± 13 vs. 10.4 ± 12.8 days, P < 0.001). The frequency of abdominal CT findings was significantly higher in patients who did not survive the infection than in patients who were discharged after recovery (41.7% vs. 27.4%, P < 0.001). Increased AA-CAS was found to be associated with a higher risk of ischemic conditions in abdominal CT examinations. CONCLUSION Abdominal symptoms in patients with COVID-19 are usually associated with positive CT findings. The presence of ischemic findings on CT correlates with poor COVID-19 outcomes. Halitosis is a frequent complaint which is estimated to be around 50%-60% of the general population and that carries serious personal and social repercussions. It is estimated that 10%-13% of halitosis causes are extraoral etiology. It originates from the tongue dorsum in patients with a healthy periodontium and good oral hygiene. Most of the causes of halitosis are intraoral origin although many other extraoral systemic etiologies are known. Overt hypothyroidism as a cause of halitosis has never been described in medical literature before. We strongly suggest that hypothyroidism induced hyposalivation gives rise to bacterial overgrowth and eventually halitosis. Two cases with moderate halitosis and overt hypothyroidism who were treated only with thyroxine until achieving euthyroidism are presented in this article. They were both completely recovered from halitosis after 45-50 days and had no complaints after 8 months. [ABSTRACT FROM AUTHOR]

Aims: Rheumatological disease flares may occur after many infections. However, our knowledge of the post-Coronavirus disease-2019 (COVID-19) axial spondyloarthritis (SpA) flares and related factors is limited. Methods: We retrospectively assessed the axial SpA patients who had COVID-19. Demographic and clinical data were collected from the medical records. The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) was applied via telephone for pre- and post-COVID-19 SpA symptoms. An increase of ≥2 points in the BASDAI score or any new extra-articular manifestations were defined as SpA flares and SpA patients were grouped as flares and noflare. Factors predicting SpA flare were also analyzed. Results: A total of 48 axial SpA patients were included in the study [age, mean±standard deviation (SD): 42.3±8.6 years; male: 65%]. Post-COVID-19 SpA flare was identified in 19 patients (40%), and new extra-articular manifestations were recorded in 6 patients (13%). Although the diagnosis of inflammatory bowel disease was more common in the flare group, the difference was not significant compared with that of the no-flare group. Other features of SpA and COVID-19 disease severity were similar between the flare and no-flare groups. In the flare group, the frequency of back pain (84% vs. 62%, p=0.091) and diarrhea (53% vs. 28%, p=0.080), and headache (84% vs. 52%, p=0.021) were higher than the no-flare group. No risk factor for a post-COVID-19 SpA flare could be identified. Conclusions: Post-COVID-19 flare was common in the axial SpA, and even new extra-articular manifestations could be reported. PURPOSE This study aimed to retrospectively evaluate the apparent diffusion coefficient (ADC) histograms in predicting chemoradiotherapy (CRT) response in patients with locally advanced rectal cancer (LARC). METHODS A total of 51 patients who underwent surgery in our institution for rectal cancer following neoadjuvant CRT between November 2013 and July 2019 were enrolled. Conventional magnetic resonance (MR) and diffusion-weighted images obtained before and after CRT were evaluated retrospectively. All tumor-containing regions of interests were drawn in 3 selected axial images, and special software for histogram analysis was used to evaluate ADC distribution. ADC cutoff values from post-CRT ADC histogram were calculated from receiver operating characteristic (ROC) analysis for evaluating CRT response. RESULTS In histopathological analysis, 5 patients (9.8%) had minimal response (group 1), 31 patients (60.8%) had partial response (group 2), and 15 patients (29.4%) had complete or almost complete response (group 3). In the ADC histogram, minimum, maximum, 10th, 25th, 50th, 75th, and 90th percentile, mean ADC values, and skewness values of groups 2 and 3 showed significant changes before and after CRT, but no difference was found within group 1 values. The mean, 25th, 50th, 75th percent ADC values after CRT and skewness, and kurtosis values were significantly different between group 1 and group 3. Skewness value from the ADC histogram in postCRT magnetic resonance imaging had the best diagnostic performance with an area under the ROC curve of 0.851 (P =.003) for detecting group 3. The skewness cutoff calculated from the ROC analysis was 0.210 for evaluating CRT response. The sensitivity and specificity of the cut-off value were 100% and 61.4%, respectively. CONCLUSION The ADC histogram analysis seems to have potential application in predicting response to neoadjuvant CRT in patients with locally advanced rectal cancer. [ABSTRACT FROM AUTHOR]

Objective: Comorbid conditions are known to be associated with poor prognosis in coronavirus disease 2019. This study aimed to investigate the effects of comorbidity burdens of inpatients, identified by the Charlson Comorbidity Index, on their mortalities. Methods: A total of 150 patients who presented to the emergency department of our hospital with various complaints and symptoms were diagnosed with coronavirus disease 2019 as a result of the testing and received inpatient treatment (87 males, mean age 61.6 ± 13.8 years) were included in the study. Charlson Comorbidity Index scores were calculated. Patients were classified into 2 groups based on the state of exitus: group 1, those who did not survive; 33 patients, 19 males; 68.3 ± 11.8 years and group 2, those who survived; 117 patients, 68 males; 59.7 ± 13.8 years. Results: In all patients, the exitus rate was 22%, the rate of intensive care follow-up was 46%, and the intubation rate was 37.3%. The Charlson Comorbidity Index scores were significantly higher in group 1 compared to group 2. Multivariate logistic regression analyses demonstrated that the Charlson Comorbidity Index score was an independent predictor of in-hospital mortality (odds ratio: 1.990, 95% CI: 1.314-3.015, P = .001). The cut-off value for the Charlson Comorbidity Index to predict in-hospital mortality was 5.5, with 81.8% sensitivity and 73.5% specificity. Conclusions: The Charlson Comorbidity Index score, which can be obtained at the time of admission, could be associated with the prognosis of coronavirus disease 2019 patients. Purpose: Nonalcoholic fatty liver disease (NAFLD) can progress to liver cirrhosis and is predicted to become the most frequent indication for liver transplantation in the near future. Noninvasive assessment of NAFLD is important for diagnosis and patient management. This study aims to prospectively determine the liver stiffness and T1 and T2 values in patients with NAFLD and to compare the diagnostic performance of magnetic resonance elastography (MRE) and mapping techniques in relation to the proton density fat fraction (PDFF).Methods: Eighty-three patients with NAFLD and 26 participants with normal livers were imaged with a 1.5 T scanner. PDFF measurements obtained from the multiecho Dixon technique were used to quantify the liver fat. MRE, native T1 mapping (modified Look-Locker inversion recovery [MOLLI] schemes 5(3)3, 3(3)3(3)5, and 3(2)3(2)5 and the B1-corrected variable flip angle [VFA] method), and T2 mapping values were correlated with PDFF. The diagnostic performance of MRE and the mapping techniques were analyzed and compared.Results: T1 values measured with the MOLLI schemes and the B1-corrected VFA (P < 0.001), and the stiffness values from MRE (P = 0.047) were significantly higher in the NAFLD group. No significant difference was found between the groups in terms of T2 values (P = 0.127). In differentiation of the NAFLD and control groups, the B1-corrected VFA technique had slightly higher accuracy and area under the curve (AUC) than the MOLLI schemes. In the NAFLD group, there was a good correlation between the PDFF, MOLLI 3(3)3(3)5 and 3(2)3(2)5, and VFA T1 measurements (r=0.732; r=0.735; r=0.716, P < 0.001, respectively).Conclusion: Liver T1 mapping techniques have the potential to distinguish steatotic from nonsteatotic livers, and T1 values seem to have a strong correlation with the liver fat content. Objective: Simulation-based training is a technique, which evokes or replicates substantial aspects of the real world in a fully interactive fashion and replaces and amplifies real experiences with guided ones, which are generally immersive. Materials and Methods: Twenty-five junior surgeons (20 orthopedic, 5 neurosurgeon) who were not involved in previous posterior cervical spine instrumentation participated in this study. Before the procedures, all surgeons received 2-hour lecture about cervical spine anatomy and screw insertion techniques. Group 1 (10 surgeons) underwent virtual reality simulation with haptic feedback for 20 min, group 2 (15 surgeons) were attended video sessions with experienced surgeons for 20 min for posterior cervical instrumentation training. After, all junior surgeons applied C1-2 screw, C3-4-5 lateral mass screws, and C6-7 cervical pedicle screws to saw bones. All saw bones underwent computerized tomography (CT) imaging and a blinded radiologist reviewed the CT images. Results: Group 1 applied 70 screws and group 2 applied 105 screws. Group 1 showed a 12% misplacement ratio, while group 2 showed a 19% misplacement ratio and 4% rate of a possible vertebral artery trace penetration (p=0.026). Conclusion: According to the results acquired from this study, we observed a lower cervical spine screw misplacement ratio in those who trained with VR based haptic enabled simulation before performing cervical posterior instrumentation. Introduction: Trapezius myalgia (TM) is the chronic pain of trapezius muscle, one of the most common reasons for shoulder pain seen in a common population. The aim of our study is to determine the effects of an existence of preoperative TM to the postoperative pain in the patients, who underwent a laparoscopic cholecystectomy. Methods: After receiving an ethical committee approval, we have included 60 ASA 1-2 patients who underwent a laparoscopic cholecystectomy surgery in the general surgery operating room in University of Health Sciences Turkey, İstanbul Training and Research Hospital between January and June 2015. The patients who were classified as ASA 3-4, had head and neck surgery or trauma, operations which had started as the laparoscopic procedures but then had to be performed as an open surgery, patients who had rheumatological and neurological diseases, tendinitis that affects the shoulder joints, and patients who had to have the emergency surgeries were excluded in this study. Patients were divided into two groups, one with (TM+) and who were not having (TM), when evaluated preoperatively. Each group had analgesia by a patient-controlled device. Using Numeral Rating scale demographical data, shoulder and umbilical pain levels were evaluated, and postoperative analgesic needs and doses were recorded in the case files. Results: Five patients who were excluded; n=26 in TM+ group and n=29 in TM group were taken. There was no significant demographic difference found between two groups. No statistically significant difference was found between TM+ and TM- groups (p>0.05). While no statistically significant difference was established in the tramadol doses between two groups, a total of ten patients out of 26 TM+ had shoulder pain, and additional analgesic need was found statistically increased in TM+ group (p<0.001). Conclusion: We have concluded that the existence of a preoperative TM increased the need for analgesics postoperatively, this could be important for choosing the analgesic regimen, and that the TM should be considered after a laparoscopic cholecystectomy. Abdominal aortic aneurysm (AAA) is a critical disease and is often asymptomatic. It can cause diminished lumbar blood flow. Impaired blood flow in the lumbar arteries is significantly associated with decreased diffusion in lumbar disks and may lead to progressive disk damage. AAA can clinically mimic upper lumbar disk herniation. Disk herniation is rarely seen in the upper lumbar region since the upper part of the lumbar spine is less mobile than the lower region. To the best of our knowledge, no reports have presented concomitant AAA and L2-L3 disk herniation. Objective: The aim of this study is to investigate post-COVID-19 antibody titers in patients who received convalescent plasma (CP) in addition to standard-of-care treatment. Materials and Methods: Hospitalized COVID-19 patients who received CP in addition to standard care were retrospectively investigated. Patients who received CP with a recorded total COVID-19 antibody test result after treatment were included. From among hospitalized COVID-19 patients who received only standard care with a recorded total COVID-19 antibody test result, a control group matched for age, gender, and comorbidities was formed. Total COVID-19 antibody index levels were compared. Results: Thirty-three CP recipients were enrolled in the study. The control group consisted of 34 age-, gender-, and comorbidity-matched standard-care patients. Median total COVID-19 antibody index levels were significantly reduced in the CP group. Conclusion: Although CP therapy may have benefits for disease outcome, its potential ability to hamper long-term immunity may be a problem. [ABSTRACT FROM AUTHOR]

Objective: Myotonic dystrophy type 1 (DM1) is characterized by myotonia and progressive muscle weakness caused by the cytosine-thymineguanine trinucleotide repeat and is the most common muscular dystrophy in adults. Although various ocular abnormalities, particularly cataracts, are well known in patients with DM1, few studies and case reports are available on retinal involvement, such as retinal degeneration, the epiretinal membrane (ERM), and macular degeneration. In this study, we investigated the retinal involvement findings using optical coherence tomography (OCT) in patients who were followed up in the neuromuscular outpatient clinic with a diagnosis of DM1. Methods: In this prospective cross-sectional study, 20 patients with genetically confirmed DM1 aged 18-65 years were included. The control group was formed from healthy individuals of a similar age and gender distribution to the patient group. Patients with diabetes mellitus, diabetic retinopathy, glaucoma, and previous cataract surgery were excluded from the study. Muscular impairment rating (MIR) scales were evaluated according to the distribution findings of myotonia and muscle weakness on a neurological examination. Patients were evaluated with OCT to examine macular and optic nerve morphology after ophthalmologic and fundus examinations were performed by a retinal specialist. Results: The mean central macular thickness (CMT) values were 275.52±40.31 and 262.76±9.06 in the control and study groups, respectively (p=0.09). Subfoveal choroidal thicknesses (SFCT) were significantly higher in the study group (341.8±95.04 vs. 279.59±46.65; p=0.002). The mean retinal nerve fiber layer did not differ between the groups (103.6±14 vs. 101.3±12; p=0.12). No ERM was detected in any of the DM1 patients. No correlation was found between CMT, SFCT, and the MIR stages (p<0.05). Conclusion: Visual disturbances associated with DM1 may be caused by cataracts as well as retinal disorders. Objective: Slipped capital femoral epiphysis (SCFE) or epiphysiolysis is a common pathology that requires urgent surgical intervention. This study aims at evaluating clinical and radiological mid-term results of our patients diagnosed with SCFE and fixed with a single cannulated screw. Methods: We examined 13 hips of 12 patients treated with SCFE and evaluated mid-term results in our study. Results: All of our patients were male with a mean age of 11.8 years (range 7-15). The mean slip angle was calculated as 43.20 degrees (range 26-70), and the average length of follow-up was 19 months (range 14-29 months). The average body mass index was 26.8 (range 24.4-29.7), and 11 patients were overweight (91.7%). Concerning ambulation, 7 patients had stable SCFE (58.3%), and 5 had unstable SCFE (41.6%). According to the Southwick classification, the average sliding angle of the patients was 43.2 degrees (range 26-70). Fixation was performed with a single cannulated screw for all patients. No patients were applied screws to the opposite hip for prophylactic purposes. Loss of reduction was not observed in any of our patients who followed up on average for 19 months (range 14 to 29). In radiological follow-ups, SCFE was not observed in the opposite hip of any patient. The postoperative range of motion was normal in all patients. Shortness and limping were not observed in any case. Conclusion: We suggest that in SCFE cases requiring urgent surgery, the mild reduction can be considered, and subsequent osteosynthesis with a single screw is a sufficient treatment. Objective: BK virus (BKV) infection has been shown to be related to hemorrhagic cystitis (HC) in allogeneic hematopoietic stem cell transplantation (allo-HSCT). There are conflicting data regarding the association between BKV titers in plasma and clinical disease as well as the risk factors for BKV-related HC. Our aim is to study the risk factors and relationship with plasma BK viral load for development of HC in a prospective analysis. Materials and Methods: We prospectively evaluated 59 patients who received allo-HSCT between 2014 and 2016 by quantitative BK virus polymerase chain reaction (PCR) (Altona Diagnostics, Germany) from blood samples at days 0, 30, 60, and 90 after allo-HSCT. The patients were monitored for signs and symptoms of HC. Results: HC was diagnosed in 22 patients (37%) at a mean of 100 days (range: 0-367 days). In multivariate analysis, the usage of cyclophosphamide (sub-distribution hazard ratio [sdHR]: 7.82, confidence interval [CI]: 1.375-39.645, p=0.02), reactivated CMV (sdHR: 6.105, CI: 1.614-23.094, p=0.008), and positive BKV viremia (sdHR: 2.15, CI: 1.456-22.065, p=0.01) significantly increased the risk of developing HC. Patients with higher viral loads at day 30 and day 60 were diagnosed with more severe HC (p<0.001). Median BK viral loads of >101.5 copies/mL at day 0 (sensitivity 0.727, specificity 0.875), >98.5 copies/mL at day 30 (sensitivity 0.909, specificity 0.875), and >90.0 copies/mL at day 60 (sensitivity 0.909, specificity 0.875) were indicative of HC. Conclusion: Our study showed that administration of cyclophosphamide, CMV reactivation, and BK virus positivity were associated with HC. Introduction: Vestibular migraine is a clinical entity characterised by dizziness. Additionally, it has no proven treatment options, and the goal is to control vestibular symptoms by reducing migraine attacks. This study aimed to investigate the effect of onabotulinum toxin type A in reducing vestibular migraine symptoms. Methods: Between 2014 and 2019, the charts of 80 patients with migraine who received onabotulinum toxin type A treatment were collected. Among them, 22 patients who met the criteria of vestibular migraine were included in the study. The treatment outcomes of these patients were investigated. Migraine Disability Assessment scale (MIDAS), Visual Analogue scale (VAS) and Dizziness Handicap index (DHI) were used for evaluation. Pre-treatment and post-treatment values were compared. Results: The mean age of the patients was 41.5 years. When MIDAS, DHI and VAS scores were compared, the post-treatment scores were found to be statistically significantly lower than the pre-treatment scores (p<0.05). Conclusion: Botulinum toxin treatment may be promising in patients with vestibular migraine. The limitation of this study is our relatively small group of patients.
