- Abstract
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou. A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica. Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas. A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica. A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros. Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.

Essential oils (EO) from aromatic and medicinal plants generally perform a diverse range of biological activities because they have several active constituents that work in different mechanisms of action. EO from Citrus peel have an impressive range of food and medicinal uses, besides other applications. EO from Citrus reticulata, C. sinensis and C. deliciosa were extracted from fruit peel and analyzed by GC-MS. The major constituent of EO under evaluation was limonene, whose concentrations were 98.54%, 91.65% and 91.27% for C. sinensis, C. reticulata and C. deliciosa, respectively. The highest potential of inhibition of mycelial growth was observed when the oil dose was 300 μL. Citrus oils inhibited fungus growth in 82.91% (C. deliciosa), 65.82% (C. sinensis) and 63.46% (C. reticulata). Anti-Sclerotinia sclerotiorum activity of 90% pure limonene and at different doses (20, 50, 100, 200 and 300 μL) was also investigated. This monoterpene showed to be highly active by inhibiting 100% fungus growth even at 200 and 300 μL doses. This is the first report of the in vitro inhibitory effect of natural products from these three Citrus species and its results show that there is good prospect of using them experimentally to control S. sclerotiorum, in both greenhouse and field conditions.

The study evaluated the efficacy and soybean spectral responses to fifteen foliar fungicide mixtures labeled to control Asian soybean rust. Canopy level reflectance was measured using a multispectral camera onboard a multirotor drone before and two hours after each spray. The third application of fungicides improved control of soybean rust and increased yield. Nevertheless, up to three consecutive foliar fungicides applications did not affect the reflectance of soybean plants at visible and infrared wavelengths. Thus, drones can be a viable strategy for data acquisition regardless of the application of the fungicides. RESUMO: Esse estudo avaliou a eficácia e as respostas espectrais de plantas de soja a quinze misturas de fungicidas utilizados no controle da ferrugem asiática da soja (FAS). A refletância do nível do dossel foi medida usando uma câmera multiespectral a bordo de um drone multirotor antes e duas horas após cada pulverização. A terceira aplicação de fungicidas melhorou o controle de FAS e aumentou a produtividade. Porém, três aplicações foliares consecutivas de fungicidas não afetaram a refletância de plantas de soja nos comprimentos de onda visível e infravermelho. Assim, drones podem ser uma estratégia viável para aquisição de dados independentemente da aplicação de fungicidas.

Introduction: Breast cancer is the most common type of cancer among women. Treatments can lead to complications modifying upper limbs movement patterns and generating pain and functionality loss. Kinesiotherapy and myofascial reorganization (MR) have shown positive effects reducing chronic pain and improving upper limbs function. We hypothesize that these techniques can maximize results and reduce treatment time in clinical practice. Objective: To develop a study protocol to verify whether MR associated with kinesiotherapy is more effective than isolated kinesiotherapy to treat chronic pain and upper limb dysfunction in breast cancer survivors. Methods: Participants will be divided into two groups: intervention group (myofascial reorganization + kinesiotherapy) and sham group (traditional massage + kinesiotherapy). Six treatment sessions (once a week) and three-time assessment will occur. Instruments for assessing pain and functionality will be Visual Analogue Scale, Body Pain Diagram, Disabilities of the Arm, Shoulder and Hand Questionnaire, and goniometry. Statistical analysis will be conducted based on intention-to-treat analysis. To analyze the difference of means between groups, we will use T-Student or U Mann-Whitney test. Repeated measures ANOVA will be used to check treatments effects. Significance level for all tests will be 5%. Conclusion: We believe that the developed study protocol will show that MR associated with kinesiotherapy improve chronic pain and upper limbs functionality of breast cancer survivors. Resumo Introdução: O câncer de mama é o tipo de câncer mais comum entre as mulheres. Os tratamentos podem levar a complicações, modificando os padrões de movimento dos membros superiores e gerando dor e perda de funcionalidade. A cinesioterapia e a reorganização miofascial (RM) têm demonstrado efeitos positivos na redução da dor crônica e na melhora da função dos membros superiores. A hipótese do presente estudo é que essas técnicas podem maximizar os resultados e reduzir o tempo de tratamento na prática clínica. Objetivo: Desenvolver um protocolo para verificar se a RM associada à cinesioterapia é mais eficaz do que a cinesioterapia isolada no tratamento da dor crônica e disfunção do membro superior em sobreviventes de câncer de mama. Métodos: As participantes serão divididas em dois grupos: grupo intervenção (reorganização miofascial + cinesioterapia) e grupo sham (massagem tradicional + cinesioterapia). Serão realizadas seis sessões de tratamento (uma vez por semana) e três avaliações. Os instrumentos de avaliação da dor e da funcionalidade serão a Escala Visual Analógica, o Diagrama de Dor Corporal, o Questionário de Deficiências do Braço, Ombro e Mão e a goniometria. A análise estatística será realizada com base na análise de intenção de tratar. Para analisar a diferença de médias entre os grupos, serão utilizados o teste T-Student ou U Mann-Whitney. ANOVA de medidas repetidas será utilizada para verificar os efeitos dos tratamentos. O nível de significância para todos os testes será de 5%. Conclusão: Espera-se que a RM associada à cinesioterapia melhore a dor crônica e a funcionalidade dos membros superiores de sobreviventes de câncer de mama.

Resumo Introdução: O câncer de mama é um problema para a saúde pública devido ao alto número de incidência. Seu tratamento muitas vezes mutilador pode trazer sérios problemas na imagem corporal e na função sexual dessas mulheres. A dança do ventre vem como uma opção passível para melhorar a feminilidade dessas mulheres. Objetivo: Analisar o efeito da prática da dança do ventre na função sexual e na imagem corporal de pacientes em tratamento adjuvante de hormonioterapia para o câncer de mama em Santa Catarina. Métodos: O ensaio clínico randomizado de dois braços foi realizado com 24 mulheres (60 ± 10,4 anos) diagnosticadas com câncer de mama, sendo 11 no grupo de intervenção (GI) e 13 no grupo controle (GC). Utilizou-se um questionário avaliando características pessoais e clínicas, imagem corporal BIBCQ e função sexual FSFI. Resultados: Na escala de imagem corporal, nos domínios estigma corporal (p = 0,002) e preocupações com o corpo (p = 0,017), foram encontradas diferenças significativas intergrupo, com melhor imagem corporal para o GI no período pós-intervenção quando comparado ao GC. Na função sexual não foram encontradas diferenças significativas. Conclusão: Percebe-se a importância do exercício físico como a dança do ventre a fim de auxiliar na melhora da imagem corporal de pacientes com câncer de mama em tratamento adjuvante com hormonioterapia.

Introduction: Patients with Adolescent Idiopathic Scoliosis (AIS) show reduced exercise capacity during the Incremental Shuttle Walk Test (ISWT). However, we not know how patients behave in the late postoperative (LPO) period. Objective: The aim of this study was to evaluate the ISWT distance (ISWTD) and physiological responses during ISWT in AIS patients during the LPO period. Methods: We included 22 patients with AIS in the LPO period (SG) and 21 adolescents in the Control Group (CG). We assessed pulmonary function (FVC and FEV1). During ISWT, a gas analyzer was used to assess peak oxygen (VO2) and submaximal relations: Oxygen Uptake Efficiency Slope (OUES) and the breathing pattern (ΔVT/ΔlnVE). Results: Significantly lower values were observed in SG: VO2 (22 ± 5 vs. 27 ± 4), ISWTD (567 ± 94 vs.604 ± 86), FVC (2.70 ± 0.47 vs. 3.33 ± 0.52) and FEV1 (2.41 ± 0.46 vs. 2.84 ± 0.52). There were significant correlations between ISWTD and VO2/Kg (r = 0.80); between OUES and ΔVT/ΔlnVE (r = 0.65); and between the main thoracic curve with VO2/Kg (r= −0.61). Conclusion: AIS patients in the LPO period have significantly reduced exercise capacity associated with reduced lung function, residual spinal curve and cardiovascular deconditioning. Objetivo: O objetivo deste estudo foi avaliar a distância do ISWT (ISWTD) e as respostas fisiológicas durante o teste em pacientes com EIA no período POT. Métodos: Foram incluídos 22 pacientes com EIA no período POT (GCi) e 21 adolescentes no Grupo Controle (GCo). Avaliamos a função pulmonar (CVF e VEF1). Durante o ISWT, foi utilizado um analisador de gases para avaliar o pico de oxigênio (VO2) e as relações submáximas: eficiência da captação de oxigênio (OUES) e o padrão de respiração (ΔVC/ΔlnVE). Métodos: Se incluyeron 22 pacientes con EIA en el período POT (GQ) y 21 adolescentes en el grupo control (GC). Se evaluó la función pulmonar (CVF y VEF1). Durante el ISWT, se utilizó un analizador de gases para evaluar el pico de oxígeno (VO2) y las relaciones submáximas: eficiencia de la captación de oxígeno (OUES) y el patrón respiratorio (ΔVC/ΔlnVE). Resultados: Se observaron valores significativos más bajos en el GQ: VO2 (22 ± 5 vs. 27 ± 4), ISWTD (567 ± 94 vs. 604 ± 86), CVF (2,70 ± 0,47 vs. 3,33 ± 0,52) y VEF1 (2,41 ± 0,46 vs. 2,84 ± 0,52). Hubo correlaciones significativas entre ISWTD y VO2/Kg (r = 0,80) entre OUES y ΔVC/ΔlnVE (r = 0,65) y entre la curva torácica principal con VO2/Kg (r = −0,61). Conclusión: los pacientes con EIA en el período POT tuvieron reducción significativa de la capacidad de ejercicio asociada a la función pulmonar reducida, la curva residual de la columna vertebral y la falta de condicionamiento cardiovascular. Introduction: Understanding the normal dynamic physiological responses to the incremental shuttle walk test might enhance the interpretation of walking performance in clinical settings. Objective: To assess dynamic physiological responses to the incremental shuttle walk test and its predictors in healthy adults. Methods: We assessed the simultaneous rates of changes of Δoxygen uptake/Δwalking velocity (ΔVO 2 /ΔWV), Δheart rate/Δoxygen uptake (ΔHR/ΔVO 2 ), Δventilation/Δcarbon dioxide production (ΔVE/ΔVCO 2 ), and Δtidal volume/Δlinearized ventilation (ΔVT/ΔlnVE) during the incremental shuttle walk test in 100 men and women older than 40 years. Fat and lean body masses (bioimpedance) were also evaluated. Results: We found that the dynamic relationships were not sex-dependent. Participants aged ≥ 70 presented declines in ΔVO 2 /ΔWV slope compared to those aged 40-49 (215 ± 69 vs. 288 ± 84 mL.min-1.km.h-1). Obese participants presented shallower slopes for ΔVO 2 /ΔWV (2.94 ± 0.90 vs. 3.84 ± 1.21 mL.min-1.kg-1.km.h-1) and ΔVT/ΔlnVE (0.57 ± 0.20 vs. 0.67 ± 0.26). We found negative influence of fat body mass on ΔVT/ΔlnVE (R2 = 0.20) and positive influence of lean body mass on ΔVO 2 /ΔWV (R2 = 0.31), ΔHR/ΔVO2 (R2 = 0.25), and ΔVT/ΔlnVE (R2 = 0.44). Conclusion: Dynamic relationships during walking were slightly influenced by age, but not sex-dependent. Body composition played an important role in these indices. Our results may provide better interpretation of walking performance in patients with chronic diseases. Objetivo: Avaliar as respostas fisiológicas dinâmicas em detrimento do incremental shuttle walk test e seus preditores em adultos saudáveis. Métodos: Foram avaliadas as taxas simultâneas de alterações de Δcaptação de oxigênio/Δvelocidade da caminhada (ΔVO2 /ΔVC), taxa de Δfrequência cardíaca/Δcaptação de oxigênio (ΔFC/ΔVO2), Δventilação/Δliberação de gás carbônico (ΔVE/ΔVCO2 ), e o Δvolume corrente/Δventilação linearizada (ΔVC/ΔlnVE) durante o incremental shuttle walk test em 100 homens e mulheres com mais de 40 anos. Massa gorda e a massa magra corporal (bioimpedância) também foram avaliadas. Resultados: Descobrimos que as relações dinâmicas não eram dependentes de sexo. Os participantes com idade ≥ 70 apresentaram declínios na inclinação da relação ΔVO2 /ΔVC em comparação com aqueles com idade entre 40-49 (215 ± 69 vs 288 ± 84 mL/min/km/h). Os participantes obesos apresentaram achatamento da inclinação ΔVO2 /ΔVC (2,94 ± 0,90 vs 3,84 ± 1,21 mL/min/kg/km/h) e ΔVC/ΔlnVE (0,57 ± 0,20 vs 0,67 ± 0,26). Encontramos influência negativa da massa gorda corporal em ΔVC/ΔlnVE (R 2 = 0,20) e a influência positiva da massa magra corporal sobre ΔVO2 /ΔVC (R 2 = 0,31), ΔFC/ΔVO2 (R 2 = 0,25), e ΔVC/ΔlnVE (R 2 = 0,44). Nossos resultados podem proporcionar uma melhor interpretação do desempenho da caminhada em pacientes com doenças crônicas.

Resumo Objetivou-se analisar as diferenças metodológicas dos inquéritos antropométricos e alimentares brasileiros classificados em domiciliares, escolares e telefônicos, e refletir sobre as potencialidades e os entraves envolvidos na utilização dos dados secundários dos mesmos. Os documentos que compuseram o corpus deste artigo foram relatórios oficiais publicados dos estudos. Os resultados retrataram diferenças metodológicas em relação à avaliação do consumo alimentar, à antropometria, à periodicidade e à forma de acesso as bases de dados. As fontes de dados secundários, que antes limitavam-se à divulgação de relatórios impressos, passaram a ser disponibilizadas sob a forma de microdados em bases eletrônicas. No entanto, essas bases são ainda pouco exploradas pelos pesquisadores da saúde, devido à complexidade de extração e de análise dos dados. Isso justifica a importância da inserção nos cursos de pós-graduação em saúde de disciplinas que promovam habilidades para uso dessas bases, que contêm variáveis sobre a situação de saúde e nutrição da população, além das socioeconômicas e demográficas não presentes nos sistemas tradicionais de informação em saúde. Conclui-se que o trabalho com dados secundários exige qualificação e o uso dessa fonte de informação contribui para adequado planejamento em saúde. Abstract The scope of the current study was to analyze the methodological differences between Brazilian anthropometric and dietary surveys, which are classified as home, school and telephone surveys, as well as to reflect on the potential benefits and obstacles involved in the use of secondary data. The documents that are the focus of this study are official published reports of the surveys. The results indicated methodological differences in the assessment of food intake, anthropometry, frequency and form of access to the databases. The secondary data sources, which were previously limited to the dissemination of printed reports, were made available as micro-data in electronic databases. However, these databases are as yet not frequently consulted by health researchers due to the complexity of data extraction and analysis. For this reason, courses on how to develop skills to use these databases in graduate health courses should be included as they contain variables related to the health and nutrition status of the population, in addition to socioeconomic and demographic variables, which are not found in traditional health information systems. The conclusion drawn is that working with secondary data requires training and that the use of this source of information contributes to adequate planning in health.

Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine that generates changes in the biomechanics of the rib cage. Digital photogrammetry enables the recording of subtle changes and the interrelationship between parts of the human body that are difficult to measure by other means. The aim of this study was to create angles and thoracic distances and to evaluate the interobserver and intraobserver reliability of these measurements using the Software de Avaliação Postural (SAPO) in patients with AIS. This cross-sectional study evaluated 30 individuals aged between 11 and 18 years with AIS. We used SAPO with the thoracic markers in the form of angles (A) and distances (D) with involves structures like acromion, manubrium, xiphoid process, lower angle of the scapula, last false rib, anterior iliac spine process. Two experienced observers (A and B) analyzed the photos and all followed the same routine of analysis. Intraobserver and interobserver reproducibility was assessed by the Bland-Altman plot and intraclass correlation coefficient (ICC), while intraobserver and interobserver reliability was assessed by the T-Test and Wilcoxon's Test. A high repeatability index was obtained among the evaluations, with twelve of the sixteen variables considered as reliable in all statistical tests. The interobserver analyzes presented excellent correlation coefficients (ICC), showing good reliability for six of the sixteen variables proposed. The SAPO method presented good reproducibility and reliability for most of the thoracic markers created, showing that photogrammetry may be a complementary tool in the evaluation of thoracic alterations in patients with AIS. Resumo A escoliose idiopática do adolescente (EIA) é uma deformidade tridimensional da coluna que gera alterações na biomecânica da caixa torácica. A fotogrametria digital permite o registro de mudanças sutis e a inter-relação entre partes do corpo humano que são difíceis de medir por outros meios. Objetivou-se criar ângulos e distâncias torácicas e avaliar a confiabilidade interobservador e intraobservador dessas medidas utilizando o Software de Avaliação Postural (SAPO) em pacientes com EIA. Este estudo transversal avaliou 30 indivíduos com idades entre 11 e 18 anos com EIA. Utilizamos o SAPO com os marcadores torácicos na forma de ângulos (A) e distâncias (D), envolvendo estruturas como: acrômio, manúbrio, processo xifóide, ângulo inferior da escápula, última costela falsa, processo da espinha ilíaca anterior. Dois observadores experientes (A e B) analisaram as fotos e todos seguiram a mesma rotina de análise. A reprodutibilidade intraobservador e interobservador foi avaliada pelo gráfico de Bland-Altman e coeficiente de correlação intraclasse (CCI), enquanto a confiabilidade intraobservador e interobservador foi avaliada pelo Teste T e Teste de Wilcoxon. Um alto índice de repetibilidade foi obtido entre as avaliações, com doze das dezesseis variáveis consideradas confiáveis em todos os testes estatísticos. As análises interobservadores apresentaram excelentes coeficientes de correlação (ICC), mostrando boa confiabilidade para seis das dezesseis variáveis propostas. O método SAPO apresentou boa reprodutibilidade e confiabilidade para a maioria dos marcadores torácicos criados, mostrando que a fotogrametria pode ser uma ferramenta complementar na avaliação de alterações torácicas em pacientes com EIA.

Resumo A musculatura do assoalho pélvico (AP) exerce importante papel na estabilização pélvico-lombar durante atividades funcionais. Objetivo: Verificar a associação entre aptidão cardiorrespiratória percebida e a função do AP no controle por idade. Métodos: Foram avaliadas 269 mulheres adultas com a Escala de Capacidade Percebida e exame da função muscular do AP por palpação. Resultados: 70,2% das mulheres conseguiam manter apenas uma caminhada por 30 minutos. Aproximadamente 46,9% tinham contração objetiva fraca do AP. Estiveram associados a uma melhor função do assoalho pélvico: aptidão cardiorrespiratória percebida (conseguir correr por 30 minutos) e não apresentar queixas de perdas de urina no dia a dia e durante a prática de atividade física. Abstract Pelvic floor (PF) muscles play an important role in stabilizing lumbar-pelvic area during functional activities. Objective: To investigate the association between the rating of perceived capacity and pelvic floor function controlling age. Methods: We evaluated 269 adult women with the Rating of Perceived Capacity and perineal muscle function palpation exam. Results: 70.2% of the women could just keep walking for 30 minutes. Approximately 4% failed to perform the contraction of the PF muscles and 46.9% had an objectively weak contraction. A better PF muscle function was associated with perceived cardiorespiratory fitness (being able to run for 30 minutes), not presenting complains of urine loss during daily life and during physical activity. Resumen La musculatura del suelo pélvico (SP) ejerce un importante rol en la estabilización pélvico-lumbar en las actividades funcionales. Objetivo: Verificar la asociación entre percepción de la condición física y función del SP controlada por edad. Métodos: Se evaluó a 269 mujeres adultas con la Escala de Capacidad Percibida y el examen de función de la musculatura del suelo pélvico por palpación. Resultados: El 70,2% de las mujeres consiguieron mantener solamente una caminata de 30 minutos. Aproximadamente el 46,9% tenía una contracción objetiva débil del SP. Una mejor función del SP se asoció con la percepción de la condición cardiorrespiratoria (conseguir correr durante 30 minutos) y no manifestar quejas de pérdida de orina en el día a día y durante la práctica de las actividades físicas.

Objective : To determine whether a restrictive pattern on spirometry is associated with the level of physical activity in daily life (PADL), as well as with cardiovascular disease (CVD) risk factors, in asymptomatic adults. Methods : A total of 374 participants (mean age, 41 ± 14 years) underwent spirometry, which included the determination of FVC and FEV1. A restrictive pattern on spirometry was defined as an FEV1/FVC ratio > 0.7 and an FVC < 80% of the predicted value. After conducting demographic, anthropometric, and CVD risk assessments, we evaluated body composition, muscle function, and postural balance, as well as performing cardiopulmonary exercise testing and administering the six-minute walk test. The PADL was quantified with a triaxial accelerometer. Results : A restrictive pattern on spirometry was found in 10% of the subjects. After multivariate logistic regression, adjusted for confounders (PADL and cardiorespiratory fitness), the following variables retained significance (OR; 95% CI) as predictors of a restrictive pattern: systemic arterial hypertension (17.5; 1.65-184.8), smoking (11.6; 1.56-87.5), physical inactivity (8.1; 1.43-46.4), larger center-of-pressure area while standing on a force platform (1.34; 1.05-1.71); and dyslipidemia (1.89; 1.12-1.98). Conclusions : A restrictive pattern on spirometry appears to be common in asymptomatic adults. We found that CVD risk factors, especially systemic arterial hypertension, smoking, and physical inactivity, were directly associated with a restrictive pattern, even when the analysis was adjusted for PADL and cardiorespiratory fitness. Longitudinal studies are needed in order to improve understanding of the etiology of a restrictive pattern as well as to aid in the design of preventive strategies.

CONTEXT AND OBJECTIVES: Accelerometry provides objective measurement of physical activity levels, but is unfeasible in clinical practice. Thus, we aimed to identify physical fitness tests capable of predicting physical inactivity among adults. DESIGN AND SETTING: Diagnostic test study developed at a university laboratory and a diagnostic clinic. METHODS: 188 asymptomatic subjects underwent assessment of physical activity levels through accelerometry, ergospirometry on treadmill, body composition from bioelectrical impedance, isokinetic muscle function, postural balance on a force platform and six-minute walk test. We conducted descriptive analysis and multiple logistic regression including age, sex, oxygen uptake, body fat, center of pressure, quadriceps peak torque, distance covered in six-minute walk test and steps/day in the model, as predictors of physical inactivity. We also determined sensitivity (S), specificity (Sp) and area under the curve of the main predictors by means of receiver operating characteristic curves. RESULTS: The prevalence of physical inactivity was 14%. The mean number of steps/day (≤ 5357) was the best predictor of physical inactivity (S = 99%; Sp = 82%). The best physical fitness test was a distance in the six-minute walk test and ≤ 96% of predicted values (S = 70%; Sp = 80%). Body fat > 25% was also significant (S = 83%; Sp = 51%). After logistic regression, steps/day and distance in the six-minute walk test remained predictors of physical inactivity. CONCLUSION: The six-minute walk test should be included in epidemiological studies as a simple and cheap tool for screening for physical inactivity. RESUMO: CONTEXTO E OBJETIVOS: A acelerometria fornece medida objetiva do nível de atividade física, porém não é viável na prática clínica. Assim, foram investigados testes de aptidão física capazes de predizer inatividade física em adultos. DESENHO E LOCAL: Estudo de teste diagnóstico, desenvolvido em laboratório universitário e uma clínica de diagnósticos. MÉTODOS: 188 participantes assintomáticos tiveram o nível de atividade física avaliado por acelerometria, ergoespirometria em esteira, composição corporal por bioimpedância, função muscular isocinética, equilíbrio postural em plataforma de força e teste de caminhada de seis minutos. Foram realizadas análise descritiva e regressão logística múltipla, incluindo idade, sexo, consumo de oxigênio, gordura corporal, centro de pressão, pico de torque de quadríceps, distância percorrida no teste de caminhada de seis minutos e passos/dia no modelo como preditores da inatividade física. Adicionalmente, foram determinadas a sensibilidade (S), especificidade (Sp) e área abaixo da curva dos principais preditores por meio de curvas de característica de operação do receptor. RESULTADOS: A prevalência da inatividade física foi 14%. O número médio de passos/dia (≤ 5357) foi o melhor preditor da inatividade física (S = 99%, Sp = 82%). O melhor teste de aptidão física foi a distância no teste de caminhada de seis minutos e ≤ 96% dos valores preditos (S = 70%; Sp = 80%). A gordura corporal > 25% também foi significativa (S = 83%, Sp = 51%). Após regressão logística, passos/dia e a distância no teste de caminhada de seis minutos permaneceram preditores da inatividade física. CONCLUSÃO: O teste de caminhada de seis minutos deve ser incluído em estudos epidemiológicos como ferramenta simples e barata para triagem da inatividade física.

Introduction: Sedentary behavior (SB) has been described as an independent risk factor for health, regardless of the recommended amount of moderate-to-vigorous physical activity (MVPA). However, SB and MVPA as predictors of falls have been poorly investigated. Objective: To compare the associations between SB and MVPA and the occurrence of falls in middle-aged and older adults. Method: The participants wore a triaxial accelerometer over the dominant hip for seven days to measure SB and MVPA. The occurrence of falls and cardiovascular risk factors were assessed by self-report. Isokinetic peak torque (PT) of knee extension, peak oxygen uptake (V’O2) in a ramp treadmill protocol, and lean (LBM) body mass and body fat (BFM) (bioelectrical impedance) were also assessed. The critical roles of SB and MVPA on the occurrence of falls were compared by multiple logistic regression adjusted for age, sex, cardiovascular risk factors, LBM, peak V’O2, and PT of knee extension. Results: 379 participants were evaluated, aged 40-80 years. Forty-eight participants reported at least one fall in the previous 12 months (14.5%). Fallers presented lower SB and higher MVPA. They were predominantly women and older adults with lower physical fitness. After multivariate analysis, MVPA, but not SB, was selected as an independent predictor of falls, increasing the odds ratio of having a fall (1.184, 95% confidence interval, 1.016 - 1.378). Conclusion: Episodes of falls in predominantly middle-aged and women subjects were associated with a higher amount of MVPA, not the opposite, indicating an adverse effect of MVPA in these subjects.

Objective: to build a theoretical model on the experience and meaning of the breast reconstruction process in women with breast cancer. Method: a qualitative research using Grounded Theory and Symbolic Interactionism as theoretical-methodological frameworks. Data collection and analysis occurred simultaneously, from April to October 2015, through theoretical sampling and theoretical saturation of the categories. Theoretical sample with 31 participants, 21 women and 10 health professionals, interviewed at home or at work, comprising four sample groups. Results: the theoretical model “Women face challenges and overcome difficulties: fear of death, breast mutilation and breast reconstruction, changing the way of being a woman” offers an explanation of the phenomenon and emerged from the systematic interrelation of the 11 study categories, with three causal categories, one category representing the context, five categories of intervening conditions, the strategies and the consequences represented by one category each. Conclusion: this study demonstrates the experience of the breast reconstruction process for women with breast cancer, the meaning of overcoming difficulties, value life and oneself, putting themselves first, taking care of themselves and learning to change, to be more tolerant, to value material things less and to face life with courage and faith. It also indicates that research on theoretical models on issues that plague our professional practices can enhance teaching and science and, consequently, improve care in women's health. RESUMEN Objetivo: construir un modelo teórico sobre la experiencia y el significado del proceso de reconstrucción mamaria de mujeres con cáncer de mama. Método: investigación cualitativa, con la teoría fundamentada en los datos y el interaccionismo simbólico como marcos teórico-metodológicos. La recolección y el análisis de datos ocurrieron simultáneamente, de abril a octubre de 2015, a través del muestreo teórico y la saturación teórica de las categorías. Muestra teórica con 31 participantes, 21 mujeres y 10 profesionales de la salud, entrevistados en el hogar o en el trabajo, compuesta por cuatro grupos de muestra. Resultados: el modelo teórico "Las mujeres enfrentan desafíos y superan dificultades: miedo a la muerte, mutilación de la mama y reconstrucción mamaria, cambiando la forma de ser mujer" ofrece una explicación del fenómeno y surgió de la interrelación sistemática provenientes de 11 categorías de estudio, siendo tres categorías causales, una categoría que representa el contexto, cinco categorías de condiciones intermedias, estrategias y consecuencias representadas a través de una categoría cada una. Conclusión: este estudio demuestra la experiencia del proceso de reconstrucción mamaria para mujeres con cáncer de mama, lo que significa superar las dificultades, valorar la vida y a sí mismas, ponerse en primer lugar, cuidarse y aprender a cambiar, a ser más tolerante, a dar menos valor a las cosas materiales y a enfrentar la vida con coraje y fe. También indica que la investigación de modelos teóricos sobre temas que afectan nuestras prácticas profesionales puede mejorar la enseñanza y la ciencia y, por consiguiente, mejorar la atención en la salud de las mujeres. RESUMO Objetivo: construir um modelo teórico sobre a experiência e significado do processo de reconstrução mamária da mulher com câncer de mama. Método: pesquisa qualitativa, com Teoria Fundamentada nos Dados e Interacionismo Simbólico como referenciais teórico-metodológicos. Coleta e análise de dados ocorreram simultaneamente, de abril a outubro de 2015, mediante amostragem teórica e saturação teórica das categorias. Amostra teórica com 31 participantes, sendo 21 mulheres e dez profissionais da saúde, entrevistados no domicílio ou local de trabalho, compondo quatro grupos amostrais. Resultados: o modelo teórico “A mulher enfrenta desafios e supera dificuldades: o medo da morte, a mutilação da mama e reconstrução mamária, modificando a maneira de ser mulher” oferece uma explicação sobre o fenômeno e surgiu do interrelacionamento sistemático das 11 categorias do estudo, sendo três categorias causais, uma categoria representando o contexto, cinco categorias de condições intervenientes, as estratégias e as consequências representadas por uma categoria cada. Conclusão: este estudo demonstra a vivência do processo de reconstrução mamária para a mulher com câncer de mama, significando transpassar dificuldades, valorizar a vida e a si mesma, colocando-se em primeiro lugar, cuidando de si e aprendendo a mudar, a ser mais tolerante, a dar menos valor a coisas materiais e a encarar a vida com coragem e fé. Objective: to analyze qualitative studies published in the national and international scientific literature on breast reconstruction after mastectomy for breast cancer, from the perspective of women. Method: systematic literature review using meta-ethnography, where the synthesis of knowledge is based on the induction and interpretation of the data found. Searches in the electronic databases identified 65 studies. 27 studides meet the inclusion and exclusion criteria. 13 studies compressed the analytical corpus of the review, according to the Critical Appraisal Skills Programme checklist. The data were synthesized by means of the meta-ethnographic approach. Results: the analyzed studies showed similarities regarding the women´s decision to perform breast reconstruction, the difficulty of choosing the type of surgery and to deal with the experience of the reconstruction, and that the women had difficulties in establishing relationships with certain health professionals, especially with doctors. They refer to the ambivalence of feelings regarding the expectations and results of breast reconstruction and complex process of "carrying on". Conclusion: this review synthesizes the scientific knowledge regarding women's perspective regarding the breast reconstruction process and highlights that health professionals must understand the perceptions of women undergoing mastectomy and breast reconstruction in order to improve the care provided.

Purpose: Developing and keeping customers’ loyalty in relation to the brand is a strategic requisite for well-succeeded business. Thus, our main purpose was to develop and validate a theoretical model concerning customers brand loyalty. Originality/value: We explored perceived quality, brand awareness, brand personality, and brand love as brand loyalty determinants. In this sense, the focus of this study was to analyze the impact of some brand loyalty determinant constructs. Design/methodology/approach: The study was implemented by means of a survey, applied to 284 customers of a soft drink brand from Northeast Brazil that was acquired by one of the biggest soft drink producers in the world. Data were analyzed through multivariate statistics and applying structural equation modeling technic. Findings: It was possible to evidence that there is a positive relation between the constructs brand awareness and perceived quality and positive influence of brand personality over brand awareness, as well as perceived quality over brand love. Also, evidence showed that brand love influences brand loyalty and that consumption level is a meaningful moderator of this relation.
