Abstract

Currently in Croatia, there are 275 severe asthma patients treated with biological therapy. Most of them (201) are receiving anti-IL5 therapy. To assess the outcomes of the most used biological therapy in severe asthma patients in Croatia, we conducted a real-life study, including 41 patients receiving anti-IL5 therapy at Clinical Center for Lung Diseases Jordanovac. Although our data offers some insight into biological therapeutics outcomes, we need a larger group of patients to make more precise conclusions.

Abstract

Background: Asthma is a heterogeneous disease with poorly defined phenotypes. Aim: To identify metabolic dysregulations associated with asthma severity and evaluate the effects of asthma medication upon observed metabotypes. Methods: Baseline urine was collected from healthy controls (HC, n=108), mild-to-moderate asthmatics (MMA, n=87) and severe asthmatics (SA, n=418) from the U-BIOPRED cohort. 12-18 month longitudinal samples were collected from the SA cohort (n=305). Abstract

Few data are available on clinical and laboratory history after discontinuation of Omalizumab in children with severe asthma. Aim: to evaluate children with severe allergic asthma after Omalizumab withdrawal. Methods: this is a longitudinal cohort study in children with severe asthma treated for at least 2 years with Omalizumab, that was discontinued after a good clinical response. We analyzed at the time of withdrawal (T0), and at 3 (T1), 6 (T2), and 12 months (T3) after discontinuation the followings: asthma control (questionnaires), spirometry (GLI reference values), quality of life (PAQLQ), and markers of Th2-inflammation (FeNO, blood eosinophils, total IgE). Results: we evaluated so far 14 children followed up at least to T2. Ten of 14 were males; median age was 15 years; duration of Omalizumab treatment 2-6 years (median: 3). At T2, all showed good symptoms control (median GINA score remained 0), even if 2 experienced asthmatic exacerbations (AE) (one child 1 AE and the other 2 AE). Only the child with 2 AE required an increase in maintenance therapy. FEV1 did not change significantly (median: T0 92% pred and T2 90,6 %pred; z-score T0: -0,64 and T2: - 0,80; p=0,18). Overall, quality of life was good (median PAQLQ at T0: 6,9; at T3: 7.0; p=0,28). 10/10 patients followed up to T3 maintained good control of the disease and only 1 had an AE. Conclusions: with the limit imposed by the number of patients enrolled so far, these preliminary data show that a number of patients have good asthma control after discontinuation of Omalizumab after 6 months, and some also after 12 months. Recruitment and follow-up are ongoing.

Abstract

Background: Dupilumab, a fully human monoclonal antibody, blocks the shared receptor component for IL‑4/IL-13, key and central drivers of type 2 inflammatory diseases, including allergic asthma. In VENTURE (NCT02528214), add-on dupilumab 300mg q2w significantly reduced OCS dose and severe exacerbations and improved FEV1 in patients (pts) with OCS-dependent, severe asthma. Dupilumab was generally well tolerated. Aim: To assess dupilumab efficacy in VENTURE pts with and without an allergic phenotype (serum total IgE ≥30IU/mL and ≥1 perennial aeroallergen-specific IgE ≥0.35kU/L at baseline [BL]). Methods: Percent reduction in OCS dose at Week 24, annualized severe exacerbation rate (AER), and change from BL at Week 24 in pre-BD FEV1 (L) were assessed post hoc in pts with and without an allergic asthma phenotype. Results: BL characteristics were generally similar in pts with (n=86) and without (n=124) an allergic phenotype. Dupilumab reduced OCS dose by 73% and 69% vs BL and by 32% and 28% vs placebo in pts with and without an allergic phenotype, and reduced AER by 72% and 45% vs placebo, respectively. Dupilumab also improved pre-BD FEV1 in both groups (Table). Conclusions: Dupilumab significantly reduced OCS dose and AER and numerically improved lung function in patients with OCS-dependent, severe asthma regardless of allergic phenotype.

Abstract

Background: Interleukin 5, an archetypal type 2 cytokine, is fundamental to eosinophilic airways disease, considered to signal the bone marrow to promote eosinophil progenitor maturation and increase levels of eosinophils in the circulation. Aim: To investigate the relationship between airway IL-5 concentrations and inflammatory phenotypes of severe asthma. Methods: Healthy volunteers and biologic naive severe asthma patients were clinically characterised and had blood and induced sputum samples collected. Sputum was analysed for differential cell count and supernatant (PBS processed) proteins, measured by single plex ELISA. Eosinophilic asthma was defined as sputum eosinophils >2%. Between group comparisons were assessed by Kruskal Wallis and relationships between variables by spearman rank correlations. Results: 17 healthy controls, 53 non-eosinophilic and 26 eosinophilic patients with severe asthma participated in the study. In severe asthma, there were modest correlations between sputum IL-5 and sputum eosinophils (r = 0.519, p Discussion: Despite high levels of steroid treatment, there is persistent IL-5 generation within the airways of severe asthma.

Abstract

Background: Chronic rhinosinusitis with nasal polyps (CRSwNP) is responsible for an increased overall disease burden among patients with asthma. Biologics are an establish therapy for severe asthma and its efficacy in patients with co-existent CRSwNP has been a subject of interest. Aim: To compare the impact of biologic therapy in severe asthma patients with and without CRSwNP. Materials and methods: Multicenter retrospective study of severe asthma patients under biologic therapy in 2020. We compared 20 patients with CRSwNP with a randomized sample of 34 patients without CRSwNP. Results: 54 severe asthma patients were included and both groups were homogeneous concerning sex, age, BMI, smoking habits and BMI. Discussion: Before treatment patients with CRSwNP had more asthma exacerbations with more short-term use of OCS;after biologics, they experience a greater reduction in the total number of prescribed OCS courses acquiring an exacerbation pattern similar to those under biologics without CRSwNP.
