This study sought to analyze the repercussions of the economic crisis on the public and private sectors of the Brazilian health system and perform a trend analysis of economic and care indicators, based on secondary data from official public sources related to spending, the economic performance of health plans and insurance, and the supply and use of services. The results showed stagnation of public spending on health, as well as reduction of per capita public spending and of access to public health services. On the contrary, in a context of falling income and employment, health plans retained customers, increased revenues, profits, and their care production. The positive performance of companies, before and after the crisis, can be explained by the trend of maintaining public subsidies for the private sector and by financialized business strategies. We conclude that the actions of the Brazilian government during the crisis deepened the restriction of resources to the public sector and favored the expansion of private services, which thus contributed to increase the discrepancy in access to public and private health services in the country.

The economic crisis that began in the United States in 2008 and its impact in Brazil starting in 2014 preceded the health crisis triggered by the COVID-19 pandemic. Considering these three time intervals, this article analyzes the susceptibility of health plan and insurance companies to economic cycles of recession in Brazil and the United States. There is evidence of a relationship between periods of economic crisis and retraction of actions and services in national health systems. However, both the United States and Brazil showed a growth in the number of health and insurance plans. The vectors driving the expansion of the private market in both countries have similarities and singularities. The "Obamacare" marks the beginning of the inclusion of population segments in the coverage of programs that use private care schemes, funded with government resources. In Brazil, despite negative general economic indices and reduction in the public health budget, health plan companies increased revenues and profits, even in the context of the pandemic and the growing economic crisis in the country. Business strategies to attract foreign investors were combined with actions to obtain credits from public institutions and increase tax deductions and exemptions, preserving financial gains. A comparison between the impacts of the financial crisis on the Brazilian and American private health markets allowed us to identify the weaknesses of our universal public system absorbed by market rationality.

The article describes the history of the response to the theoretical and methodological challenges in research on companies and corporate groups in the Brazilian health sector that reorganized their shareholding structures, diversified their activities, and expanded their financial operations. Such movements in corporate concentration and expansion were analyzed with an approach to the frame of reference for financialization in contemporary capitalism in the analysis of selected companies and corporate groups. Corporate strategies were classified in three dimensions: shareholding, financial, and accounting structure and influence on the public agenda and the respective indicators orienting the organization of information from diverse sources for companies and corporate groups from 2008 and 2017. The study provides a profile of the intense and complex process of corporate reorganization in the health sector. Still, the study's exploratory design and difficulties in access to information and selection of companies and corporate groups mean that the observations are necessarily preliminary.

This study aims to analyze the shareholding dimension of changes in companies and corporate groups in the Brazilian health sector from 2008 to 2017. The idea was to understand the strategies of accumulation in a context of financialization, defined as a systemic pattern of wealth in contemporary capitalism. The shareholding changes were submitted to descriptive and exploratory analysis based on different sources. We studied 58 companies from the subsectors of health plans, pharmacies, hospitals, diagnostics, pharmaceutical industry, and social organizations. Data were collected on the legal structure, ownership, and control, equity operations, and economic and patient care activities. Despite the heterogeneity of the companies and their strategies, the results point to an increase in the presence of domestic and international investors, changes in internal organization, capital, financing, and diversification of activities. The companies that stood out were actively pursuing capitalization via foreign investments, purchase and share of assets and shares, and mergers and acquisitions, accelerating the process of expansion, accumulation, and shareholding and financial appreciation. The result is an increase in capital flows and growing integration of economic and patient care structures in the Brazilian health sector with financial circuits, thereby linking peripheral intermediate companies to an expanded process of accumulation under financial dominance. The findings corroborate striking aspects of corporate dynamics in financialization that increasingly influence health systems in Brazil and the world.

This article describes changes in the public, nonprofit, and private components of the health care networks and health insurance and health plan companies in Brazil, based on the accumulated knowledge concerning the gains and obstacles in the Brazilian Unified National Health System (SUS) and differences between policies for democratization and democratizing processes. This central premise allowed analyzing praise versus criticism for the SUS and the contemporary nature of relations between the public and private sectors, drawing on secondary data from agencies in the Executive, Legislative, and Judiciary branches. The article concludes that the highly financialized private and private-charitable sectors imposed anti-democratic and anti-democratizing standards in the use of public funds. The article further concludes that although these sectors have not raised barriers to certain public policies for the expansion of access, they nevertheless prevent the development of the SUS according to the principles set out in the 1988 Constitution.

The concentration and internationalization of health plan companies in Brazil gave them a clearly financial face. Based on the need to understand the health care industry's capital accumulation patterns, the current study examines health plan companies' expansion strategies through the classification of their supply and demand characteristics by recent historical periods and an analysis of recent shareholding trends in one of the leading corporations in the Brazilian health care industry. The 1960s to 2000s witnessed changes in the scale of demands for health plans and adherence by companies to long-term accumulation strategies. Beginning in the early 21st century, changes in the shareholding structures of the largest Brazilian company, consistent with the financialization of its accumulation regime, resulted in the rapid multiplication of its capital. Deepening segmentation of the health care system in a context marked by the downturn in the national economy challenges the preservation of public subsidies for private health plans. Resumo: A concentração e internacionalização de empresas de planos de saúde no Brasil conferiram-lhes feição financeira. Considerando a necessidade de compreender os padrões de acumulação desse setor, o trabalho examina estratégias de expansão das empresas de planos de saúde por meio da periodização de atributos da oferta e demanda e exame da trajetória patrimonial recente de um dos grandes grupos do setor. Entre os anos 1960 e 2000, ocorreram alterações na escala das demandas por planos de saúde e adesão das empresas a estratégias de acumulação de longo prazo. A partir do século XXI, as alterações nas estruturas societárias da maior empresa brasileira, compatíveis com o regime de acumulação financeirizada, resultaram na multiplicação de seu capital em um curto intervalo de tempo. O aprofundamento da segmentação do sistema de saúde, em um contexto marcado pela desaceleração da economia, questiona a preservação dos subsídios públicos para os planos privados.

Consumer complaints against private health insurance plans and companies in Brazil have become increasingly frequent in the country's 'supplementary' (non-public) health care sector, with numerous cases reaching the courts. The problem raised the need for regulation of this private market, which began in 1998, through Law no. 9.656. One of the challenges faced by the National Agency for Supplementary Health Care (ANS) is resistance to the legislation by health insurance companies, besides the fact that there are still some contracts not covered by this law. The objective of the current study was to analyze health insurance policyholders' appeals against court rulings for or against injunctions concerning coverage, in cases heard by the courts in Rio de Janeiro and São Paulo. The main data investigated were: court issuing the ruling; defendant; basis for the case; ruling by the Circuit Court and Court of Appeals; and the legal arguments. Based on the findings, the Brazilian court system still plays an important role in hearing and ruling on complaints by health insurance policyholders. The ANS has an important role in filling some gaps that have still not been solved in regulating the health insurance industry.
