Background: The patellofemoral joint is an important source of pain in knee osteoarthritis. Most biomechanical research in knee osteoarthritis has focused on the tibiofemoral joint during level walking. It is unknown what happens during stair negotiation in patients with patellofemoral joint osteoarthritis, a task commonly increasing pain. Conservative therapy for patellofemoral joint osteoarthritis includes the use of a sleeve knee brace. We aimed to examine the effect of a sleeve knee brace on knee biomechanics during stair negotiation in patellofemoral joint osteoarthritis patients., Methods: 30 patellofemoral joint osteoarthritis patients (40-70 years) ascended and descended an instrumented staircase with force plates under two conditions - wearing a Lycra flexible knee support (Bioskin Patellar Tracking Q Brace) and no brace (control condition). Knee joint kinematics (VICON) and kinetics were recorded., Findings: During stair ascent, at the knee, the brace significantly reduced the maximal flexion angle (2.7 0 , P = 0.002), maximal adduction angle (2.0 0 , P = 0.044), total sagittal range of motion (2.0 0 , P = 0.008), total frontal range of motion (1.7 0 , P = 0.023) and sagittal peak extension moment (0.05 Nm/kg, P = 0.043) compared to control. Introduction: Tripping on stairs results from insufficient foot to step edge clearance and can often lead to a fall in older adults. A stair horizontal-vertical illusion is suggested to increase the perceived riser height of a step and increase foot clearance when stepping up. However, this perception-action link has not been empirically determined in older adults. Previous findings suggesting a perception-action effect have also been limited to a single step or a three-step staircase. On larger staircases, somatosensory learning of step heights may be greater which could override the illusory effect on the top step. Furthermore, the striped nature of the existing stair horizontal-vertical illusion is associated with visual stress and may not be aesthetically suitable for use on public stairs. These issues need resolving before potential future implementation on public stairs., Methods: Experiment 1. A series of four computer-based perception tests were conducted in older (N = 14: 70 ± 6 years) and young adults (N = 42: 24 ± 3 years) to test the influence of different illusion designs on stair riser height estimation. Participants compared images of stairs, with horizontal-vertical illusions or arbitrary designs on the bottom step, to a plain stair with different bottom step riser heights and selected the stair they perceived to have the tallest bottom riser. Horizontal-vertical illusions included a previously developed design and versions with modified spatial frequencies and mark space ratios. Perceived riser height differences were assessed between designs and between age groups. Experiment 2. To assess the perception-action link, sixteen older (70 ± 7 years) and fifteen young (24 ± 3 years) adults ascended a seven-step staircase with and without horizontal-vertical illusions tested in experiment 1 placed onto steps one and seven. Foot clearances were measured over each step. To determine whether changes in perception were linked to changes in foot clearance, perceived riser heights for each horizontal-vertical illusion were assessed using the perception test from experiment 1 before and after stair ascent. Additional measures to characterise stair safety included vertical foot clearance, margins of stability, foot overhang, stair speed, and gaze duration, which were assessed over all seven steps., Results: Experiment 1. All horizontal-vertical illusion designs led to significant increases in the perceived riser height in both young and older adults (12-19% increase) with no differences between age groups. Experiment 2. On step 7, each horizontal-vertical illusion led to an increase in vertical foot clearance for young (up to 0.8 cm) and older adults (up to 2.1 cm). On step 1 significant increases in vertical foot clearance were found for a single horizontal-vertical illusion when compared to plain (1.19 cm increase). The horizontal-vertical illusions caused significant increases in the perceived riser height (young; 13% increase, older; 11% increase) with no differences between illusion design, group or before and after stair ascent. No further differences were found for the remaining variables and steps., Conclusion: Results indicate a perception-action link between perceived riser height and vertical foot clearance in response to modified versions of the horizontal-vertical illusion in both young and older adults. This was shown with no detriment to additional stair safety measures. Introduction: Poor lighting has been associated with stair falls in young and older adults. However, current guidelines for illuminating stairs seem arbitrary, differ widely between sources, and are often difficult to interpret., Aims: Here we examined the influence of real-world bulb illumination properties on stair descent safety in young and older adults, with a view to generating preliminary evidence for appropriate lightbulb use/stair illumination., Methods: Stair tread illumination (lx) was measured in a standard UK home (2.23 m ceiling) from a low (50 W; 630 lm) and a high (103 W, 1450 lm) power compact fluorescent lamp (CFL) bulb from the time they were turned on until they reached full brightness. This enabled modelling of their illumination characteristics during warm up. Illumination was also measured from a low (40 W, 470 lm) and a high (100 W, 1521 lm) power LED bulb at first turn-on. Computer-controlled custom lighting then replicated these profiles, in addition to a Bright control (350 lx), on an instrumented staircase descended (3 × trials per light condition) by 12 young (25.3 ± 4.4 years; 5 males), 12 higher ability older (HAOA: 69.6 ± 4.7 years; 5 males) and 13 lower ability older (LAOA: 72.4 ± 4.2; 3 males) healthy adults. Older adults were allocated to ability groups based on physiological and cognitive function. Stair-specific confidence was assessed prior to the first descent in each new lighting condition, and whole-body 3D kinematics (Vicon) quantified margins of stability and foot clearances with respect to the step edges. Mixed ANOVAs examined these measures for within-subject effects of lighting (×5), between-subject effects of age (×3) and interactions between lighting and age., Results: Use of CFL bulbs led to lower self-reported confidence in older adults (20.37%, p = .01), and increased margins of stability (12.47%, p = .015) and foot clearances with respect to the step edges (10.36%, p = .003). Importantly, using CFL bulbs increased foot clearance variability with respect to the bottom step (32.74%, p = .046), which is where a high proportion of falls occur., Conclusion: Stair-tread illumination from CFL bulbs at first turn on leads to less safe stair negotiation. Stair falls are a major health problem for older people. Most studies on identification of stair fall risk factors are limited to staircases set in given step dimensions. However, it remains unknown whether the conclusions drawn would still apply if the dimensions had been changed to represent more challenging or easier step dimensions encountered in domestic and public buildings. The purpose was to investigate whether the self-selected biomechanical stepping behaviours are maintained when the dimensions of a staircase are altered. Sixty-eight older adults (>65 years) negotiated a seven-step staircase set in two step dimensions (shallow staircase: rise 15 cm, going 28 cm; steep staircase: rise 20 cm, going 25 cm). Six biomechanical outcome measures indicative of stair fall risk were measured. K-means clustering profiled the overall stair-negotiating behaviour and cluster profiles were calculated. A Cramer's V measured the degree of association in membership between clusters. The cluster profiles revealed that the biomechanically risky and conservative factors that characterized the overall behaviour in the clusters did not differ for the majority of older adults between staircases for ascent and descent. A strong association of membership between the clusters on the shallow staircase and the steep staircase was found for stair ascent (Cramer's V: 0.412, p < 0.001) and descent (Cramer's V: 0.380, p = 0.003). The findings indicate that manipulating the demand of the task would not affect the underpinning mechanism of a potential stair fall. Stair falls, especially during stair descent, are a major problem for older people. Stair fall risk has typically been assessed by quantifying mean differences between subject groups (e.g. older vs. younger individuals) for a number of biomechanical parameters individually indicative of risk, e.g., a reduced foot clearance with respect to the stair edge, which increases the chances of a trip. This approach neglects that individuals within a particular group may also exhibit other concurrent conservative strategies that could reduce the overall risk for a fall, e.g. a decreased variance in foot clearance. The purpose of the present study was to establish a multivariate approach that characterises the overall stepping behaviour of an individual. Twenty-five younger adults (age: 24.5 ± 3.3 y) and 70 older adults (age: 71.1 ± 4.1 y) descended a custom-built instrumented seven-step staircase at their self-selected pace in a step-over-step manner without using the handrails. Measured biomechanical parameters included: 1) Maximal centre of mass angular acceleration, 2) Foot clearance, 3) Proportion of foot length in contact with stair, 4) Required coefficient of friction, 5) Cadence, 6) Variance of these parameters. As a conventional analysis, a one-way ANOVA followed by Bonferroni post-hoc testing was used to identify differences between younger adults, older fallers and non-fallers. To examine differences in overall biomechanical stair descent behaviours between individuals, k-means clustering was used. The conventional grouping approach showed an effect of age and fall history on several single risk factors. The multivariate approach identified four clusters. Three clusters differed from the overall mean by showing both risky and conservative strategies on the biomechanical outcome measures, whereas the fourth cluster did not display any particularly risky or conservative strategies. In contrast to the conventional approach, the multivariate approach showed the stepping behaviours identified did not contain only older adults or previous fallers. This highlights the limited predictive power for stair fall risk of approaches based on single-parameter comparisons between predetermined groups. Establishing the predictive power of the current approach for future stair falls in older people is imperative for its implementation as a falls prevention tool., (Crown Copyright © 2019. People with diabetes display biomechanical gait alterations compared to controls and have a higher metabolic cost of walking (CoW), but it remains unknown whether differences in the vertical displacement of the body centre of mass (CoM) may play a role in this higher CoW. The aim of this study was to investigate vertical CoM displacement (and step length as a potential underpinning factor) as an explanatory factor in the previously observed increased CoW with diabetes. Thirty-one non-diabetic controls (Ctrl); 22 diabetic patients without peripheral neuropathy (DM) and 14 patients with moderate/severe Diabetic Peripheral Neuropathy (DPN), underwent gait analysis using a motion analysis system and force plates while walking at a range of matched speeds between 0.6 and 1.6 m/s. Vertical displacement of the CoM was measured over the gait cycle, and was not different in either diabetes patients with or without diabetic peripheral neuropathy compared to controls across the range of matched walking speeds examined (at 1 m/s: Ctrl: 5.59 (SD: 1.6), DM: 5.41 (1.63), DPN: 4.91 (1.66) cm; p > 0.05). The DPN group displayed significantly shorter steps (at 1 m/s: Ctrl: 69, DM: 67, DPN: 64 cm; p > 0.05) and higher cadence (at 1 m/s: Ctrl: 117 (SD1.12), DM: 119 (1.08), DPN: 122 (1.25) steps per minute; p > 0.05) across all walking speeds compared to controls. The vertical CoM displacement is therefore unlikely to be a factor in itself that contributes towards the higher CoW observed recently in people with diabetic neuropathy. The higher CoW in patients with diabetes may not be explained by the CoM displacement, but rather may be more related to shorter step lengths, increased cadence and the associated increased internal work and higher muscle forces developed by walking with more flexed joints., (Copyright © 2018 The Author(s). Descending stairs requires elevated joint moment-generating capability in the lower limbs, making it a challenging daily activity, particularly for older individuals. The aim of the study was to investigate the influence of three different strategies for descending standard and increased height stairs: step-over-step (SoS), step-by-step (SbS) and side-step (SS) on lower limb kinetics in older people. Eleven participants (mean ± SD age: 74.8 ± 3.1 years, height: 1.63 ± 0.07 m, mass: 67.7 ± 9.5 kg) descended a four-step custom built instrumented staircase at a self-selected speed, adopting each of the three strategies, at two configurations: a step-rise height of 170 mm (standard; STD) and a step-rise height of 255 mm (increased; INC). 3D motion capture, synchronised with embedded force plates enabled the calculation of joint kinetics of lead and trail limbs. Data were analysed using a Linear Mixed Model with gait speed selected as a covariate during weight acceptance (WA) and controlled lowering (CL) phases. A large increase in hip extensor moment in both WA and CL in the lead limb was evident during both SoS and SbS at INC step height compared to STD (P < .015 for all), with no such increase in hip flexor moment evident in SS strategy (P = .519). Lead limb knee extensor moment decreased and plantarflexor moment increased in INC SoS compared to STD SoS during CL (P < .001 for both). In the trail limb, increased hip extensor and plantarflexor moments were seen in INC SS compared to STD SS (P < .001 for both). The alternate strategies result in the overall task demand being split between the lead limb (weight acceptance) and trail limb (controlled lowering). Differential demand distribution patterns exist between strategies that imply targeted interventions and/or advice could be provided to older individuals in order to promote safe descent of stairs, particularly for those with specific muscle weaknesses or at high risk of falls., (Copyright © 2018 Elsevier Ltd. When studying muscle and whole-body function in children with cerebral palsy (CP), knowledge about both internal and external moment arms is essential since they determine the mechanical advantage of a muscle over an external force. Here we asked if Achilles tendon moment arm (MA AT ) length is different in children with CP and age-matched typically developing (TD) children, and if MA AT can be predicted from anthropometric measurements. Sixteen children with CP (age: 10y 7m±3y, 7 hemiplegia, 12 diplegia, GMFCS level: I (11) and II (8)) and twenty TD children (age: 10y 6m±3y) participated in this case-control study. MA AT was calculated at 20° plantarflexion by differentiating calcaneus displacement with respect to ankle angle. Seven anthropometric variables were measured and related to MA AT . We found normalized MA AT to be 15% (∼7mm) smaller in children with CP compared to TD children (p=0.003). MA AT could be predicted by all anthropometric measurements with tibia length explaining 79% and 72% of variance in children with CP and TD children, respectively. Diabetic patients have an altered gait strategy during walking and are known to be at high risk of falling, especially when diabetic peripheral neuropathy is present. This study investigated alterations to lower limb joint torques during walking and related these torques to maximum strength in an attempt to elucidate why diabetic patients are more likely to fall. 20 diabetic patients with moderate/severe peripheral neuropathy (DPN), 33 diabetic patients without peripheral neuropathy (DM), and 27 non-diabetic controls (Ctrl) underwent gait analysis using a motion analysis system and force plates to measure kinetic parameters. Lower limb peak joint torques and joint work done (energy expenditure) were calculated during walking. The ratio of peak joint torques and individual maximum joint strengths (measured on a dynamometer) was then calculated for 59 of the 80 participants to yield the ‘operating strength’ for those participants. During walking DM and DPN patients showed significantly reduced peak torques at the ankle and knee. Maximum joint strengths at the knee were significantly less in both DM and DPN groups than Ctrls, and for the DPN group at the ankle. Operating strengths were significantly higher at the ankle in the DPN group compared to the Ctrls. These findings show that diabetic patients walk with reduced lower limb joint torques; however due to a decrement in their maximum ability at the ankle and knee, their operating strengths are higher. This allows less reserve strength if responding to a perturbation in balance, potentially increasing their risk of falling.

Background: Stair descent is a frequent daily activity that poses great risks for injury due to falling. Very little is understood about the attentional demands of stair descent and their changes with aging. The present study compared combined locomotor and cognitive functioning during different phases of stair descent between healthy young and older individuals., Methods: Sixteen young and sixteen healthy older subjects walked down a 5-step staircase, performing a simultaneous visual Stroop task (i.e., a dual task) during the approach, transition or steady-state descent phases in some trials. Three dimensional kinematics of trunk and foot motion were recorded along with the accuracy and dual task costs (DTCs) for responses to the Stroop stimuli., Results: Dual tasking influenced both gait and cognitive performance for all subjects, and older adults generally walked slower with higher foot clearances and had greater DTCs. Specific age differences were found at stair transition where older adults showed more attentional effects., Conclusions: Healthy, active older adults showed changes to attention and planning due to normal aging specifically associated with a crucial point of fall risk during stair descent., (Copyright © 2013 Elsevier Inc. Foot clearance is an important variable for understanding safe stair negotiation, but few studies have provided detailed measures of it. This paper presents a new method to calculate minimal shoe clearance during stair descent and compares it to previous literature. Seventeen healthy young subjects descended a five step staircase with step treads of 300 mm and step heights of 188 mm. Kinematic data were collected with an Optotrak system (model 3020) and three non-colinear infrared markers on the feet. Ninety points were digitized on the foot sole prior to data collection using a 6 marker probe and related to the triad of markers on the foot. The foot sole was reconstructed using the Matlab (version 7.0) "meshgrid" function and minimal distance to each step edge was calculated for the heel, toe and foot sole. Results showed significant differences in minimum clearance between sole, heel and toe, with the shoe sole being the closest and the toe the furthest. While the hind foot sole was closest for 69% of the time, the actual minimum clearance point on the sole did vary across subjects and staircase steps. When descending stairs bodyweight becomes supported on a single limb while the forwards-reaching contralateral limb is lowered in order to make contact with the step below. This is associated with lowering of the centre of mass (CoM), which in order to occur in a controlled manner, requires increased ankle and knee joint torque production relative to that in overground walking. We have previously shown that when descending steps or stairs older people operate at a higher proportion of their maximum eccentric capacity and at, or in excess of the maximum passive reference joint range of motion. This suggests they have reduced and/or altered control over their CoM and we hypothesised that this would be associated with alterations in muscle activity patterns and in the CoM vertical acceleration and velocity profiles during both the lowering and landing phases of stair descent. 15 older (mean age 75 years) and 17 young (mean age 25 years) healthy adults descended a 4-step staircase, leading with the right limb on each stair, during which CoM dynamics and electromyographic activity patterns for key lower-limb muscles were assessed. Maximum voluntary eccentric torque generation ability at the knee and ankle was also assessed. Older participants compared to young participants increased muscle co-contraction relative duration at the knee and ankle of the trailing limb so that the limb was stiffened for longer during descent. As a result older participants contacted the step below with a reduced downwards CoM velocity when compared to young participants. Peak downwards and peak upwards CoM acceleration during the descent and landing phases respectively, were also reduced in older adults compared to those in young participants. In contrast, young participants descended quickly onto the step below but arrested their downward CoM velocity sooner following landing; a strategy that was associated with longer relative duration lead-limb plantar flexor activity, increased peak upwards CoM acceleration, and a reduced landing duration. These results suggest that a reduced ability to generate high eccentric torque at the ankle in the forward reaching limb is a major factor for older participants adopting a cautious movement control strategy when descending stairs. Overall reductions in muscle strength typically accompany the aging process. However, older adults show a relatively preserved capacity of producing eccentric strength. The preservation of eccentric strength in older adults is a well-established phenomenon, occurring indiscriminately across different muscle groups, independent of age-related architectural changes in muscle structure and velocity of movement. The mechanisms for the preservation of eccentric strength appear to be mechanical and cellular in origin and include both passive and active elements regulating muscle stiffness. The age-related accumulation of non-contractile material in the muscle-tendon unit increases passive stiffness, which might offer mechanical advantage during eccentric contractions. In addition, the preserved muscle tension and increased instantaneous stiffness of old muscle fibers during stretch increase active stiffness, which might enhance eccentric strength. The fact that the preservation of eccentric strength is present in people with chronic conditions when compared to age-matched healthy controls indicates that the aging process per se does not exclusively mediate the preservation of eccentric strength. Physical inactivity, which is common in elderly and people with chronic conditions, is a potential factor regulating the preservation of eccentric strength. When compared to concentric strength, the magnitude of preservation of eccentric strength in older adults ranges from 2% to 48% with a mean value of 21.6% from all studies. This functional reserve of eccentric strength might be clinically relevant, especially to initiate resistance training and rehabilitation programs in individuals with low levels of strength., (2010 Elsevier Inc. It is not currently known how the mechanical properties of human tendons change with maturation in the two sexes. To address this, the stiffness and Young's modulus of the patellar tendon were measured in men, women, boys and girls (each group, n=10). Patellar tendon force (F(pt)) was calculated from the measured joint moment during a ramped voluntary isometric knee extension contraction, the antagonist knee extensor muscle co-activation quantified from its electromyographical activity, and the patellar tendon moment arm measured from magnetic resonance images. Tendon elongation was imaged using the sagittal-plane ultrasound scans throughout the contraction. Tendon cross-sectional area was measured at rest from ultrasound scans in the transverse plane. Maximal F(pt) and tendon elongation were (mean+/-SE) 5453+/-307 N and 5+/-0.5 mm for men, 3877+/-307 N and 4.9+/-0.6 mm for women, 2017+/-170 N and 6.2+/-0.5 mm for boys and 2169+/-182 N and 5.9+/-0.7 mm for girls. In all groups, tendon stiffness and Young's modulus were examined at the level that corresponded to the maximal 30% of the weakest participant's F(pt) and stress, respectively; these were 925-1321 N and 11.5-16.5 MPa, respectively. Stiffness was 94% greater in men than boys and 84% greater in women than girls (p<0.01), with no differences between men and women, or boys and girls (men 1076+/-87 N/mm; women 1030+/-139 N/mm; boys 555+/-71 N/mm and girls 561.5+/-57.4 N/mm). Young's modulus was 99% greater in men than boys (p<0.01), and 66% greater in women than girls (p<0.05). There were no differences in modulus between men and women, or boys and girls (men 597+/-49 MPa; women 549+/-70 MPa; boys 255+/-42 MPa and girls 302+/-33 MPa). These findings indicate that the mechanical stiffness of tendon increases with maturation due to an increased Young's modulus and, in females due to a greater increase in tendon cross-sectional area than tendon length., (Copyright 2009 Elsevier Ltd. Minimising post-operative donor site morbidity is an important consideration when selecting a graft for surgical reconstruction of the torn anterior cruciate ligament (ACL). One of the most common procedures, the bone-patellar tendon-bone (BPTB) graft involves removal of the central third from the tendon. However, it is unknown whether the mechanical properties of the donor site (patellar tendon) recover. The present study investigated the mechanical properties of the human patellar tendon in 12 males (mean+/-S.D. age: 37+/-14 years) who had undergone surgical reconstruction of the ACL using a BPTB graft between 1 and 10 years before the study (operated knee; OP). The uninjured contralateral knee served as a control (CTRL). Patellar tendon mechanical properties were assessed in vivo combining dynamometry with ultrasound imaging. Patellar tendon stiffness was calculated from the gradient of the tendon's force-elongation curve. Tendon stiffness was normalised to the tendon's dimensions to obtain the tendon's Young's modulus. Cross-sectional area (CSA) of OP patellar tendons was larger by 21% than CTRL tendons (P<0.01). Patellar tendon stiffness was not significantly different between OP and CTRL tendons, but the Young's modulus was lower by 24% in OP tendons (P<0.01). A compensatory enlargement of the patellar tendon CSA, presumably due to scar tissue formation, enabled a recovery of tendon stiffness in the OP tendons. The newly formed tendon tissue had inferior properties as indicated by the reduced tendon Young's modulus, but it increased to a level that enabled recovery of tendon stiffness.

Aim: Loss of muscle strength and balance are main characteristics of physical frailty in old age. Postural sway is associated with muscle contractile capacity and to the ability of rapidly correcting ankle joint changes. Thus, resistance training would be expected to improve not only strength but also postural balance., Methods: In this study, age-matched older individuals (69.9+/-1.3 years) were randomly assigned to flywheel (n=12), or weight-lifting (n=12) groups, training the knee extensors thrice weekly for 12 weeks. The hypotheses were that owing to a larger eccentric loading of the knee extensors, flywheel training would result in (a) greater gains in quadriceps strength; (b) greater improvements in balance performance compared with weight-lifting training. Isokinetic dynamometry, B-mode ultrasonography, electromyography, percutaneous muscle stimulation and magnetic resonance imaging were employed to acquire the parameters of interest., Results: Following training, knee extensors peak isokinetic power increased by 28% (P<0.01) in the flywheel group with no change in the weight-lifting group. Adaptations of the gastrocnemius muscle also occurred in both groups. The gastrocnemius characteristic with the highest response to training was tendon stiffness, with increases of 54% and 136% in the weight-lifting and flywheel groups, respectively (P<0.01). The larger increase in tendon stiffness in the flywheel group was associated with an improvement in postural balance (P<0.01)., Conclusion: Quadriceps flywheel loading not only produces a greater increase in power than weight training but its physiological benefits also transfer/overspill to the plantarflexor muscle-tendon unit resulting in a significantly improved balance. These findings support our initial hypotheses.

To better understand the role of the ankle plantar flexor muscles in stair negotiation, we examined the effects of manipulation of kinematic and kinetic constraints on the behavior of the gastrocnemius medialis (GM) muscle during stair ascent. Ten subjects ascended a four-step staircase at four different step-heights (changing the kinematic constraints): standard (17 cm), 50% decreased, 50% increased and 75% increased. At the standard height, subjects also ascended the stairs wearing a weighted jacket, adding 20% of their body mass (changing the kinetic constraints). During stair ascent, kinematics and kinetics of the lower legs were determined using motion capture and ground reaction force measurements. The GM muscle fascicle length was measured during the task with ultrasonography. The amount of GM muscle fascicle shortening increased with step-height, coinciding with an increase in ankle joint moment. The increase in body mass resulted in an increased ankle joint moment, but the amount of GM muscle fascicle shortening during the lift-off phase did not increase, instead, the fascicles were shorter over the whole stride cycle. Increasing demands of stair ascent, by increasing step-height or body mass, requires higher joint moments. The increased ankle joint moment with increasing demands is, at least in part, produced by the increase in GM muscle fascicle shortening.

Background: The aim of this study was to investigate the architectural alterations of skeletal muscle following hemiplegic cerebral palsy. If associated with functional and clinical measures of disability, information on muscle architecture could then be used as an objective tool in the assessment of motor disability in these patients., Methods: Ultrasonography was used to assess in vivo the gastrocnemius muscle architecture in the paretic and non-paretic legs of eight children with cerebral palsy., Findings: Fascicle length and muscle thickness at the resting ankle position were reduced in the paretic compared to the non-paretic legs by up to 18% and 20%, respectively (P<0.05), indicating a loss of both in-series and in-parallel sarcomeres in the affected muscles. However, pennation angle was similar (P>0.05) in the two legs., Interpretation: The present results indicate that paresis in hemiplegic cerebral palsy may affect the geometry of skeletal muscle. Further studies are required to examine the relation between muscle architecture, severity of motor disability, and treatment.

The purpose of this study was to examine the relations between patellar tendon moment arm length and several relevant anthropometric characteristics of 22 healthy men. The patellar tendon moment arm length was measured using magnetic resonance imaging with two different methods: (1) measurement of patellar tendon moment arm length (d(PT)) with respect to the tibiofemoral contact point (d(PTCP)) and (2) measurement of d(PT) with respect to the intersection point of the anterior and posterior cruciate ligament (d(PTIP)). Pearson correlation coefficients and a stepwise linear regression analysis were used to examine the relationships between the d(PT) and anthropometric measurements taken. Furthermore, a Student's t-test was used to determine differences between the d(PTCP) and d(PTIP) values. Only knee circumference was a significant d(PTCP) predictor (P < 0.05) but with a very low R2 (0.139). None of the anthropometric parameters examined was found to be a significant d(PTIP) predictor. The correlation coefficients ranged from -0.04 to 0.42. The d(PTIP) values were significantly higher (by 0.84-1.89 cm) than the d(PTCP) values (P < 0.05). These results are in disagreement with previous in vitro findings that d(PT) variance may be explained by knee joint size differences. Hence, existing imaging-based methodologies remain necessary for accurate quantification of the patellar tendon moment arm.

The purpose of this study was to examine the effect of different muscle contraction modes and intensities on patellar tendon moment arm length (d(PT)). Five men performed isokinetic concentric, eccentric and passive knee extensions at an angular velocity of 60 deg/s and six men performed gradually increasing to maximum effort isometric muscle contractions at 90( composite function) and 20( composite function) of knee flexion. During the tests, lateral X-ray fluoroscopy imaging was used to scan the knee joint. The d(PT) differences between the passive state and the isokinetic concentric and extension were quantified at 15( composite function) intervals of knee joint flexion angle. Furthermore, the changes of the d(PT) as a function of the isometric muscle contraction intensities were determined during the isometric knee extension at 90( composite function) and 20( composite function) of knee joint flexion. Muscle contraction-induced changes in knee joint flexion angle during the isometric muscle contraction were also taken into account for the d(PT) measurements. During the two isometric knee extensions, d(PT) increased from rest to maximum voluntary muscle contraction (MVC) by 14-15%. However, when changes in knee joint flexion angle induced by the muscle contraction were taken into account, d(PT) during MVC increased by 6-26% compared with rest. Moreover, d(PT) increased during concentric and eccentric knee extension by 3-15%, depending on knee flexion angle, compared with passive knee extension. These findings have important implications for estimating musculoskeletal loads using modelling under static and dynamic conditions.

Detailed understanding of the knee joint loading requires the calculation of muscle and joint forces in different conditions. In these applications the patellar tendon moment arm length is essential for the accurate estimation of the tibiofemoral joint loading. In this article, different methods that have been used to determine the patellar tendon moment arm length under in vivo and in vitro conditions are reviewed. The limitations and advantages associated with each of the methods are evaluated together with their applications in the different loading conditions that the musculoskeletal system is subjected to. The three main measurement methods that this review considers are the geometric method, the tendon excursion method and the direct load method. A comparison of relevant quantitative results is presented to asses the impact of the errors of each method on the quantification of the patellar tendon moment arm and the implications for joint loading assessment in clinical applications.

In the present study, we measured the contraction-induced shortening (dL) of individual synergistic human muscles in a repeated motor task to assess their contractile behaviour. Ultrasonography was used to obtain dL measurements in the gastrocnemius (GS) and soleus (SOL) muscles of six men performing 11 consecutive isometric plantarflexions. Contractions 1 and 11 were performed with maximal effort, and contractions 2-4, 5-7 and 8-10 were performed with efforts generating 50, 70 and 90%, respectively, of the plantarflexion moment produced in contraction 1. In contractions 5-10, the SOL muscle dL was similar (p > 0.05) to that produced in contraction 1 (approximately 6 mm), indicating that the SOL muscle became fully activated at 70% of the maximum plantarflexion moment. The GS muscle dL in contractions 10 and 11 exceeded by approximately 0.5 mm (p < 0.05) and 1.3 mm (p < 0.01), respectively, that generated in contraction 1 (approximately 10 mm), despite evidence obtained by superimposed stimulation that contraction 1 was produced with full motor unit activation. The consequent paradox that the GS muscle would produce in contractions 10 and 11 a greater activation and therefore more force than its actual potential is resolved when considering the interaction between the time-dependent tensile response of tendon and the performance of muscle as dictated by the sliding filament mechanism of contraction.

The purpose of this study was to quantify in vivo the fibre length (L)-to-moment arm (d) ratio (L/d) in the major ankle plantarflexors and knee extensors of 21 healthy men. Measurements of L were taken in the gastrocnemius medialis, gastrocnemius lateralis, soleus, vastus lateralis and vastus intermedius muscles using ultrasound scanning. Measurements of d were taken in the Achilles tendon (d(AT)) and patellar tendon (d(PT)) using magnetic resonance imaging. The relations between (a) L in each ankle plantarflexor muscle and d(AT), (b) L in each knee extensor muscle and d(PT), and (c) d(AT) and d(PT,) were analyzed with Pearson correlation coefficients. There was no significant relation (P>0.05) between any of the above parameters, with the correlation coefficient values ranging from -0.2 to 0.29. These results contradict previous findings that L scales with d, indicating that skeletal size differences between individuals may not always be associated with predictable differences in muscle architecture. This needs to be accounted for when information about the L/d ratio of a given muscle-joint in a given individual is required, as for example is the case when assessing the suitability of a potential donor muscle in surgical tendon transfer.

Ultrasonography is becoming increasingly popular for studying the tensile behaviour of in vivo human tendons through elongation measurements during isometric contraction. For the plantarflexor tendons specifically, which receive much attention due to their functional role in locomotion, elongations are conventionally measured by fixing the scanning probe on the calf. Elongation corrections are also made to account for artifactual ankle rotations during the isometric test, by assuming that these occur round the tibio-talar joint axis, as is the case for passive ankle plantarflexion-dorsiflexion rotations. The present work was set out to examine the validity of these procedures. The displacement of the calcaneum, a skin marker on the calf, and the gastrocnemius myotendinous junction, were measured from rest to maximum isometric voluntary plantarflexion contraction (MVC) in six men by using ultrasound probes mounted on externally fixed points (active test). A passive ankle plantarflexion rotation equal in magnitude to that recorded during MVC was then performed (passive test). In the active test, the calcaneum and the skin marker shifted in the proximo-distal direction by approximately 13 mm. Moreover, the calcaneal displacements in the active and passive tests took place round different rotational axes, as indicated by a calcaneal displacement difference of approximately 10 mm between the two tests. These effects resulted in underestimating by 35% the actual elongation of the gastrocnemius tendon when following the currently suggested procedures. The present results directly invalidate the procedures conventionally followed for assessing the tensile response of the human plantarflexor tendons with ultrasonography.

In the present experiment we obtained the tensile properties of the human gastrocnemius tendon, a high-stressed tendon suitable for spring-like action during locomotion. Measurements were taken in vivo in six men. The gastrocnemius tendon elongation during tendon loading-unloading induced by muscle contraction-relaxation was measured using real-time ultrasonography. Tendon forces were calculated from the moment generated during isometric plantarflexion contraction, using tendon moment arm length data obtained in vivo with the tendon travel method. Tendon stiffness data were calculated from the slope of the tendon force-elongation curve, and were then normalized to the tendon's original dimensions, obtained from morphometric analysis of sonographs, to estimate the tendon Young's modulus. Mechanical hysteresis values were obtained from area calculations by numerical integration. The elongation of the tendon increased curvilinearly with the force acting upon it, from 1.7+/-1mm (0.8+/-0.3% strain) at 87.5+/-8.5 N to 11.1+/-3.1mm (4.9+/-1% strain) at 875+/-85 N. The tendon Young's modulus and mechanical hysteresis were 1.16+/-0.15 GPa and 18+/-3%, respectively. These values fall within the range of values obtained from in vitro experiments and are very similar to the respective values recently obtained from in vivo measurements in the less highly stressed human tibialis anterior tendon (1.2 GPa and 19%), thus indicating that the material properties of tendon are independent of physiological loading and function. Combining the present tendon force-elongation data with previously reported Achilles tendon force data recorded during walking indicates that the gastrocnemius tendon would provide approximately 6% of the total external work produced by the locomotor system. This estimate illustrates the contribution of passive elastic mechanisms on the economy and efficiency of walking. The contributions would be greater in more active exercise such as running.

The biomechanical properties of tendinous structures have traditionally been studied using excised material. Limitations associated with displacement measurements and clamping, and uncertainties as to whether in vitro testing represents physiological function, necessitate developing a method for assessing the mechanical properties of tendinous tissue in the in vivo state. This paper reviews recent results taken with an in vivo and noninvasive protocol using ultrasound as a means of measuring tendon-aponeurosis elongation during tensile loading applied by contraction of the in-series muscle. The results obtained indicate that: (1) the Young's modulus and mechanical hysteresis of in vivo tendons is independent of physiological function and loading, (2) there is a strain variation along the tendon-aponeurosis, and (3) in vivo tendons may exhibit creep. These findings agree with reports from experiments on isolated material and have important biological implications for both the tendon and the in-series muscle. The method described here allows designing longitudinal studies on tendon adaptability, but it also has direct clinical applications.

Mechanical hysteresis in tendons has traditionally been quantified from tensile testing of isolated specimens. Limitations associated with tendon displacement measurement and clamping, and uncertainties as to whether in vitro material represents intact tendon function necessitate measuring hysteresis under in vivo conditions. In the present study such measurements were taken in the human tibialis anterior (TA) tendon. Having the foot fixed on a dynamometer footplate, the displacement of the TA tendon during stimulation and relaxation of the TA muscle was recorded by means of ultrasonography in six men. Combining moment data corresponding to 0, 20, 40, 60, 80 and 100% of maximum voltage moment and the respective tendon-displacement data, a hysteresis loop was obtained between the load-displacement curves during contraction and relaxation. Measurement of the hysteresis loop area yielded a value of 19%. This value agrees with results from in vitro tensile tests of low-stress tendons, suitable for tensile force transmission and joint displacement control. In fact, the human TA tendon has such functional characteristics. The methodology presented allows design of longitudinal and cross-sectional experimental protocols, and in vivo assessment of tendon function and propensity to overheat.

In the present study, we examined the hypothesis that stretch of tendinous tissue in the human tibialis anterior (TA) muscle-tendon unit upon isometric dorsiflexion maximum voluntary contraction (MVC) varies along the entire tendinous component length. Ultrasound-based measurements of the excursions of the TA tendon origin and proximal end of the TA central aponeurosis were taken in the transition from rest to MVC in six men. Subtracting the TA tendon origin excursion from the excursion of the aponeurosis proximal end, the aponeurosis excursion was estimated. Estimation of the aponeurosis proximal region excursion was obtained subtracting the excursion of the insertion point of a central region fascicle on the aponeurosis from the whole aponeurosis excursion. Subtracting tendon excursion from the excursion of the central fascicle insertion point, the aponeurosis distal region excursion was estimated. Strain values were calculated dividing the excursions obtained by the original resting lengths. All excursions and lengths were measured in the mid-longitudinal axis of the TA muscle-tendon unit at the neutral anatomical ankle position. Tendon excursion and strain were 0.5+/-0. 08 cm (mean+/-SE) and 3.1+/-0.2%, respectively. Aponeurosis excursion and strain were 1.1+/-0.15 cm and 6.5+/-0.6%, respectively. Aponeurosis distal region excursion and strain were 0.3+/-0.05 cm and 3.5+/-0.3%, respectively. Aponeurosis proximal region excursion and strain were 0.8+/-0.12 cm and 9.2+/-1%, respectively. Aponeurosis excursion and strain were larger by 110-120% (P<0.05) compared with tendon. Aponeurosis proximal region excursion and strain were larger by 165-170% (P<0.05) compared with aponeurosis distal region. These findings are in line with results from in vitro animal material testing and have important implications for theoretical models of muscle function.

The aim of this study was to estimate the moment arm of human tibialis anterior (TA) muscle-tendon unit at rest and during isometric dorsiflexion maximum voluntary contraction (MVC) from in vivo sagittal-plane magnetic resonance (MR) and ultrasound scans. Two methods were employed, both of them based on the assumption that the ankle joint complex and TA muscle-tendon unit operate in the sagittal plane. Using method A, moment arms were obtained from MR scans of the foot by measuring the perpendicular distance between a moving centre of rotation in the talo-crural joint and the TA tendon action line. Using method B, moment arms were calculated from the ratio of TA tendon displacement, which was estimated from a planimetric muscle model using pennation angles and muscle thickness measured by ultrasonography, to the tibial rotation around the talus, which was measured from the foot MR scans. Using either of the two methods at rest, the estimated TA moment arm decreased from approximately 4.5 to approximately 2.9 cm in the transition from dorsiflexion to plantarflexion. Using method A, moment arms during MVC were larger by 0.9-1.5 cm (33-44%, P < 0.01) than the respective resting estimations. In contrast, no difference (P > 0.05) was found between the resting and MVC moment arm estimations of method B. Limitations in the oversimplified musculoskeletal model used raise questions for the validity of both method estimations.

Objective: In the present study, we examined the hypothesis that the tibialis anterior tendon moment arm increases during maximum isometric dorsiflexion as compared with rest., Background: In musculoskeletal modelling applications, moment arms from passive muscles at rest are assumed representative of those measured during isometric muscle contraction. The validity of this assumption is questionable in musculotendon actuators enclosed by retinacular systems as in tibialis anterior., Design and Methods: Sagittal-plane magnetic resonance images of the right ankle were taken in six subjects at rest and during maximum isometric dorsiflexion at six ankle angles between dorsiflexion and plantarflexion having the body placed in the supine position and the knee flexed at 90 degrees. Instant centres of rotation in the tibio-talar joint, tibialis anterior tendon action lines and moment arms were identified in the sagittal plane at ankle angles of -15 degrees, 0 degrees,+15 degrees and +30 degrees at rest and during maximum isometric dorsiflexion., Results: At any given ankle angle, the tibialis anterior tendon moment arm during maximum isometric dorsiflexion increased by 0.9-1.5 cm (P<0.01) compared with rest. This was attributed to a displacement of both tibialis anterior tendon action line by 0.8-1.2 cm (P<0.01) and all instant centres of rotation by 0.3-0.4 cm (P<0. 01) distally in relation to their corresponding resting positions., Conclusions and Implications: The assumption that the tibialis anterior tendon moment arm does not change from rest to maximum isometric dorsiflexion is invalid. Erroneous tendon forces, muscle stresses and joint moments by as much as 30% would be calculated using resting tibialis anterior tendon moment arms in the moment equilibrium equation around the ankle joint during maximum isometric dorsiflexion., Relevance: A substantial increase in the tibialis anterior tendon moment arm occurs from rest to maximum isometric dorsiflexion. This needs to be taken into consideration when using planimetric musculoskeletal modelling for analysing maximal static ankle dorsiflexion loads.
