Total knee arthroplasty is a well established treatment for degenerative joint disease, which is also performed as a treatment in younger and middle-aged patients who have a significant physical activity and high life expectancy. However, complications may occur due to biological responses to wear particles, as well as local and systemic hypersensitivity reactions triggered by metal ions and particles such as cobalt, chromium and molybdenum. The purpose of the study was to perform a highly demanding activities (HDA) knee wear simulation in order to compare the wear characteristics and metal ion release barrier function of a zirconium nitride (ZrN) coated knee implant, designed for patients with suspected metal ion hypersensitivity, against an uncoated knee implant made out of CoCrMo. The load profiles were applied for 5 million HDA cycles, which represent 15-30 years of in vivo service depending on the activity level of the patient. Results showed a significant wear rate reduction for the coated group (1.01 ± 0.29 mg/million cycles) in comparison with the uncoated group (2.89 ± 1.04 mg/million cycles). The zirconium nitride coating showed no sign of scratches nor delamination during the wear simulation, whereas the uncoated femurs showed characteristic wear scratches in the articulation areas. Furthermore, the metal ion release from the coated implants was reduced up to three orders of magnitude in comparison with the uncoated implants. These results demonstrate the efficiency of zirconium nitride coated knee implants to reduce wear as well as to substantially reduce metal ion release in the knee joint., (Copyright © 2018 Elsevier Ltd. The objective of our study was to evaluate the impact of the tibial keel & stem length in surface cementation, of a full cemented keel and of an additional tibial stem on the primary stability of a posterior stabilised tibial plateau (VEGA® System Aesculap Tuttlingen, Germany) under dynamic compression-shear loading conditions in human tibiae. We performed the cemented tibial plateau implantations on 24 fresh-frozen human tibiae of a mean donor age of 70.7years (range 47-97). The tibiae were divided into four groups of matched pairs based on comparable trabecular bone mineral density. To assess the primary stability under dynamic compression shear conditions, a 3D migration analysis of the tibial component relative to the bone based on displacements and deformations and an evaluation of the cement layer including penetration was performed by CT-based 3D segmentation. Within the tested implant fixation principles the mean load to failure of a 28mm keel and a 12mm stem (40mm) was 4700±1149N and of a 28mm keel length was 4560±1429N (p=0.996), whereas the mean load to failure was 4920±691N in full cementation (p=0.986) and 5580±502N with additional stem (p=0.537), with no significant differences regarding the dynamic primary stability under dynamic compression-shear test conditions. From our observations, we conclude that there is no significant difference between a 40mm and a 28mm tibial keel & stem length and also between a surface and a full cementation in the effect on the primary stability of a posterior stabilised tibial plateau, in terms of failure load, migration characteristics and cement layer thickness including the penetration into the trabecular bone., (Copyright © 2017 Elsevier Ltd. Background: The objective of our study was to evaluate the impact of a trabecular stem fixation versus a cortical teeth fixation technique on the primary stability of cemented unicompartmental femoral components under dynamic flexion movement loading conditions in human femora., Methods: Ten fresh-frozen human knees of a mean donor age of 73.9years were used to perform medial unicompartmental knee arthroplasty under a less invasive parapatellar surgical approach. The femora were divided into two groups of matched pairs based on comparable trabecular bone mineral density. To assess the primary stability, a new method based on a combination of dynamic flexion movement, double-peak loading simulating stair climbing, kinematic analysis of the femoral component migration relative to the bone and an evaluation of the cement layer by fragments cut through the implant-cement-bone interface in the sagittal plane of the medial condyle was introduced., Findings: For the "trabecular stem fixation" technique the mean load to failure was 2340 (SD 650) N and for "cortical teeth fixation" it was 1080 (SD 455) N, with a substantially enhanced dynamic fixation strength for the "trabecular stem fixation" (p=0.008). In the distal area the cement layer of the "trabecular stem fixation" showed a significant decreased thickness compared to the "cortical teeth fixation" (p=0.029), while a substantially deeper cement penetration (p=0.044) has been achieved for the "trabecular stem fixation"., Interpretation: From our observations, we conclude that there is a significantly enhanced primary stability with a "trabecular stem fixation" compared to a "cortical teeth fixation" technique of cemented unicompartmental femoral components, in terms of dynamic failure load and migration characteristics., (Copyright © 2016 Elsevier Ltd. The objective of our study was to evaluate the impact of a biphaseal anterior-posterior (AP) and internal-external (IE) motion restraint system on the wear behaviour, tibio-femoral kinematics and particle release of a mobile bearing posterior stabilised knee design in comparison to the widely used linear restraint. in vitro wear simulation was performed using a posterior stabilised total knee replacement with a mobile rotating platform gliding surface design to compare the standard ISO 14243-1:2002 (E) protocol with a linear AP and IE motion restraint and the new ISO 14243-1:2009 (E) protocol with a biphaseal AP and IE motion restraint. For the mobile gliding surfaces, an increase in wear rate by more than a magnitude was measured applying the biphaseal protocol (8.5±1.6 mg/million cycles) in a direct comparison to the linear protocol (0.33±0.07 mg/million cycles), with statistically significant difference. The amplitudes of AP displacement were 3.22±0.47 mm for the biphaseal test, compared to 1.97±0.22 mm in the linear test and the amplitudes of the IE rotation angle had mean values of 7.32°±0.91° under the biphaseal setup, compared to 1.97°±0.14° under linear motion restraint test conditions. From our observations, we conclude that the changes in AP translation and IE rotation motion restraints from ISO linear to ISO biphaseal test conditions highly impact the knee joint kinematics and wear behaviour of a mobile bearing posterior stabilised knee design., (Copyright © 2014 Elsevier Ltd. Background: The objective of our study was to evaluate the impact of a single- ("implant only") versus a double-layer ("implant & bone") cementing technique on the primary stability of unicompartmental tibial plateaus under dynamic compression-shear loading conditions in human tibiae., Methods: Twelve fresh-frozen human knees of a mean donor age of 72.3 years were used to perform medial UKA under a less invasive parapatellar surgical approach. The tibiae were divided into two groups of matched pairs based on comparable trabecular bone mineral density. To assess the primary stability, a new method based on a combination of dynamic compression-shear testing, kinematic analysis of the tibial plateau migration relative to the bone and evaluation of the cement layer by CT-scans and fragments cut through the implant-cement-bone interface in the frontal plane was introduced., Findings: For the "implant only" cementation technique the mean load to failure was 2600 (SD 675) N and for "implant & bone" it was 2820 (SD 915) N. Between the final load level at failure and the bone mineral density a significant correlation was found for the groups "implant only" (r(s) = 0.875) and "implant & bone" (r(s) = 0.907)., Interpretation: From our observations, we conclude that there is no significant difference between a single- ("implant only") and double-layer ("implant & bone") cementing technique in the effect on the primary stability of unicompartmental tibia plateaus, in terms of failure load, correlation between final load at failure and bone mineral density, migration characteristics, cement layer thickness and penetration depth., (© 2013.)

Background: A novel tibial baseplate design (Transversal Support Tibial Plateau) as a new treatment concept for bi-cruciate retaining total knee arthroplasty is evaluated for mechanical stability and compared to other tibial baseplate designs. This concept should provide better primary stability and thus, less subsidence, than implantation of two separate unicondylar tibial baseplates., Methods: Different baseplates were implanted into synthetic bone specimens (Sawbones® Pacific Research Laboratories, Inc., Washington, USA), all uncemented. Using a standardized experimental setup, subsidence was achieved, enabling comparison of the models regarding primary stability., Findings: Overall implant subsidence was significantly increased for the two separate unicondylar tibial baseplates versus the new Transversal Support Tibial Plateau concept, which showed comparable levels to a conventional tibial baseplate. Reduced subsidence results in better primary stability., Interpretation: Linking of two separate baseplates appears to provide increased primary stability in terms of bony fixation, comparable to that of a conventional single tibial baseplate., (© 2013. The objective of our study was to examine the effect of biphaseal AP translation and IE rotation restraint, using a system defined specifically for posterior stabilised knee designs, on wear, kinematics and particle release in comparison to linear motion restraint as required by the established ISO 14243-1:2002(E) protocol. In the ISOlinear groups, an AP motion restraint of 30 N/mm and an IE rotation restraint of 0.6 Nm/° were applied in the knee wear simulation. In the ISOgap biphaseal groups with PCL sacrificing implants, the restraining AP force was zero in a ±2.5mm range with, externally, a constant of 9.3N/mm applied proportionally to the AP translation of the tibia plateau, whereas the restraining IE torque was zero in a ±6° range with, externally, a constant of 0.13 Nm/° applied proportionally to the IE rotation of the tibia plateau. Using the ISOgap biphaseal protocol on a posterior stabilised knee design, we found an increase of 41% in AP translation and of 131% in IE rotation, resulting in a 3.2-fold higher wear rate compared to the results obtained using the ISOlinear protocol. Hip prostheses with a modular neck exhibit, compared to monobloc prostheses, an additional interface which bears the risk of fretting as well as corrosion. Failures at the neck adapter of modular prostheses have been observed for a number of different designs. It has been speculated that micromotions at the stem-neck interface were responsible for these implant failures. The purpose of this study was to investigate the influence of material combinations and assembly conditions on the magnitude of micromotions at the stem-neck interface during cyclic loading. Modular (n = 24) and monobloc (n = 3) hip prostheses of a similar design (Metha, Aesculap AG, Tuttlingen, Germany) were subjected to mechanical testing according to ISO 7206-4 (F(min) = 230N, F(max) = 2300N, f = 1Hz, n = 10,000 cycles). The neck adapters (Ti-6Al-4V or Co-Cr29-Mo alloy) were assembled with a clean or contaminated interface. The micromotion between stem and neck adapter was calculated at five reference points based on the measurements of the three eddy current sensors. The largest micromotions were observed at the lateral edge of the stem-neck taper connection, which is in accordance with the crack location of clinically failed prostheses. Titanium neck adapters showed significantly larger micromotions than cobalt-chromium neck adapters (p = 0.005). Contaminated interfaces also exhibited significantly larger micromotions (p < 0.001). Since excessive micromotions at the stem-neck interface might be involved in the process of implant failure, special care should be taken to clean the interface prior to assembly and titanium neck adapters with titanium stems should generally be used with caution., (Copyright © 2011 Elsevier Ltd. Background: Modern unicompartmental knee prostheses represent a valid alternative to total knee replacement. It is known that variations in clinical alignment lead to altered biomechanics and abnormal wear. The aim of this study was to assess the influence, on wear behaviour, of two different cementing interfaces of the femoral components tested on a knee joint wear simulator., Methods: The wear tests were run in a knee wear simulator at a frequency of 1.1 Hz for 3 million cycles in accordance with ISO 14243-3. Twelve commercial mobile GUR 1020 UHMWPE meniscus specimens articulated in between 12 cobalt-chromium-molybdenum alloy femoral and tibial components covered by a multi-layer of chromium nitride and a final layer of zirconium nitride ceramic coating to prevent ion release from the substrate. Two wear tests were performed: in the first test, each femoral component was cemented into a custom made metallic-block shaped to perfectly host it. In the second test, synthetic composite femurs received the femoral components on the basis of guidelines used in current surgery., Findings: The two cementing interfaces showed a significantly different wear behaviour, quantified as mean weight loss (P<0.001). Scanning electron microscope examinations of new and tested metallic components showed macro- and micro-pores of few microns on both configurations., Interpretation: The wear pattern observed at 3 million cycles showed differences between the two methods of fixation for the meniscus femoral components., (Copyright © 2010 Elsevier Ltd. The objective of our study was to test alternative polymer-on-polymer articulations for cervical total disc arthroplasty with favourable biotribological properties and the benefit of radiolucency in comparison to the clinically well established metal-on-polyethylene coupling. In vitro wear simulation was performed according to ISO 18192-1:2008 (E) with the clinically introduced activ C cervical artificial disc (Aesculap AG Tuttlingen, Germany) made of UHMWPE/CoCr29Mo6 in a direct comparison to experimental disc articulations made of PEEK, CFR-PEEK and PEK. Each material combination was tested for 10 million cycles with a customised 6 station spinal wear simulator (EndoLab Thansau, Germany). Gravimetric and geometric wear assessment, optical surface characterisation and an estimation of particle size and morphology were performed. The gravimetric wear rate of the clinical reference polyethylene-on-cobalt-chromium was 1.0+/-0.1 mg/million cycles, compared to 1.4+/-0.4 mg/million cycles for PEEK, to 0.02+/-0.02 mg/million cycles for CFR-PEEK and 0.8+/-0.1 mg/million cycles for PEK. In conclusion, a number of different candidate materials for total cervical disc arthroplasty were compared using the same disc design. Whereas the polymer-on-polymer articulation of PEK showed no substantial benefit in comparison to polyethylene-on-cobalt-chromium and whereas natural PEEK tends towards pitting and delamination, the carbon fibre reinforced PEEK demonstrated an excellent wear behaviour with a reduction in order of a magnitude. Therefore, the CFR-PEEK based polymer-on-polymer articulations may be an alternative to polyethylene-on-metal and have a high potential for next generation disc replacements.

Background: Several studies in literature are dealing with a direct comparison between fixed and mobile bearing knee replacements, but to our knowledge there is no published data comparing the wear behaviour of the two design principles based on the same femur and superior gliding surface geometry. The objective of our study was to investigate a fixed and mobile bearing knee design with identical femoral articulation in regard to wear, tibio-femoral kinematics and particle size distribution., Methods: In vitro wear simulation according to ISO 14243-1 has been performed with the Columbus knee system (Aesculap, Tuttlingen) in the configurations fixed and mobile bearing for five million cycles on a customized four station knee wear simulator. The tests were running under force control and the tibio-femoral kinematics were assessed. A particle analysis has been undergone after each inspection interval when the lubricant was replaced., Findings: Due to the additional wear in the tibial articulation of the mobile bearing design the mean gravimetric wear rates are not statistically different between the two groups. Apart of that there is a substantial reduction in the amount of wear per area unit for the mobile versus the fixed bearing gliding surfaces. Both groups show comparable tibio-femoral kinematics and a similar wear debris morphology., Interpretation: Our investigation of a fixed and mobile bearing knee design with identical femoral articulation demonstrates that there are no significant differences in wear rate, resulting kinematics and polyethylene particle release. Therefore it can be recommended that surgeons decision for one or the other design principle should be based on the individual patient profile.
