The study of the relationship between Daylight Saving Time (DST) and road safety has yielded contrasting results, most likely in relation to the inability of crash-database approaches to unravel positive (ambient lighting-related) and negative (circadian/sleep-related) effects, and to significant geographical differences in lighting-related effects. The aim of this study was to investigate the effects of DST on driving fatigue, as measured by driving-based, physiological and subjective indicators obtained from a driving simulator experiment. Thirty-seven participants (73 % males, 23 ± 2 years) completed a series of 50-min trials in a monotonous highway environment: Trial 1 was in the week prior to the Spring DST transition, Trial 2 in the following week, and Trial 3 in the fourth week after the transition. Thirteen participants returned for Trial 4, in the week prior to the Autumn switch to civil time, and Trial 5 in the following week. Significant adverse effects of DST on vehicle lateral control and eyelid closure were documented in Trial 2 and Trial 3 compared to Trial 1, with no statistical differences between Trials 2 and 3. Further worsening in vehicle lateral control was documented in Trials 4 and 5. Eyelid closure worsened up to Trial 4, and improved in Trial 5. Participants were unaware of their worsening performance based on subjective indicators. In conclusion, DST has a detrimental impact on driving fatigue during the whole time during which it is in place. Urban design is currently promoting the inclusion of plants in buildings. However, plants emit biogenic volatile organic compounds (BVOCs), which alone or in combination with other airborne molecules such as CO 2 , may result in a general increase in tropospheric pollution. Many studies have documented the effects of biotic and abiotic factors on plant BVOC responses, but few have assessed the contribution of typical CO 2 levels found in indoor work and meeting spaces. To answer this question, we monitored CO 2 and constitutive (MT-limonene) and induced (LOX-cis-3-hexenal) BVOC emissions of a fully developed tomato crop grown hydroponically inside an integrated rooftop greenhouse (i-RTG) in a Mediterranean climate. Two distinctive CO 2 assays were performed at the level of the i-RTG by supplying or not CO 2 . The impact of CO 2 on plant physiological emittance was then assessed, and the resulting BVOC rates were compared with reference to EU-LCI values. MT-limonene was ubiquitous among the assays and the most abundant, while LOX-cis-3-hexenal was detected only under controlled CO 2 management. The highest levels detected were below the indicated LCIs and were approximately tenfold lower than the corresponding LCI for MT-limonene (50.88 vs. 5000 μg m -3 ) and eightfold (6.63 μg m -3 ) higher than the constitutive emission level for LOX-cis-3-hexenal. Over extended sampling (10 min) findings revealed a general emission decrease and significantly different CO 2 concentration between the assays. Despite similar decreasing rates of predicted net photosynthesis (Pn) and stomatal conductance (g s ) their correlation with decreasing CO 2 under uncontrolled condition indirectly suggested a negative CO 2 impact on plant emission activity. Conversely, increasing CO 2 under the controlled assay showed a positive correlation with induced emissions but not with constitutive ones. Because of significantly higher levels of relative humidity registered under the uncontrolled condition, this factor was considered to affect more than CO 2 the emission response and even its collection. This hypothesis was supported by literature findings and attributed to a common issue related with the sampling in static enclosure. Hence, we suggested a careful monitoring of the sampling conditions or further improvements to avoid bias and underestimation of actual emissions. Based on the main outcomes, we observed no evidence of a hazardous effect of registered CO 2 rates on the BVOC emissions of tomato plant. Background: Oral immunotherapy is effective at inducing desensitisation to allergens and induces sustained unresponsiveness (ie, clinical remission) in a subset of patients, but causes frequent reactions. We aimed to investigate whether addition of a probiotic adjuvant improved the efficacy or safety of peanut oral immunotherapy., Methods: PPOIT-003, a multicentre, randomised, phase 2b trial, was conducted in three tertiary hospitals in Australia (Adelaide [SA], Melbourne [VIC], and Perth [WA]) in children aged 1-10 years, weighing more than 7 kg, with peanut allergy confirmed by a double-blind placebo-controlled food challenge (cumulative 4950 mg dose of peanut protein) and positive peanut skin prick test (≥3 mm) or peanut-specific IgE (≥0·35 kU/L). Children were randomly assigned (2:2:1) to receive probiotic and peanut oral immunotherapy (PPOIT), placebo probiotic and peanut oral immunotherapy (OIT), or placebo probiotic and placebo OIT (placebo) for 18 months, and were followed up until 12 months after completion of treatment. Oral immunotherapy consisted of increasing doses of peanut protein (commercially available food-grade 12% defatted peanut flour [50% peanut protein]) until a 2000 mg daily maintenance dose was reached. The probiotic adjuvant was a daily dose of 2 × 10 10 colony-forming units of the probiotic Lactobacillus rhamnosus ATCC 53103. Placebo immunotherapy comprised maltodextrin, brown food colouring, and peanut essence, and placebo probiotic was maltodextrin. Dual primary outcomes were 8-week sustained unresponsiveness, defined as no reaction to a cumulative dose of 4950 mg peanut protein at treatment completion and 8 weeks after treatment completion, in the PPOIT versus placebo groups and the PPOIT versus OIT groups, analysed by intention to treat. Safety endpoints were adverse events during the treatment phase, and peanut ingestion and reactions in the 12-month post-treatment period. This study is registered with the Australian New Zealand Clinical Trials Registry, 12616000322437., Findings: Between July 4, 2016, and Sept 21, 2020, 201 participants were enrolled and included in the intention-to-treat analysis. 36 (46%) of 79 children in the PPOIT group and 42 (51%) of 83 children in the OIT group achieved sustained unresponsiveness compared with two (5%) of 39 children in the placebo group (risk difference 40·44% [95% CI 27·46 to 53·42] for PPOIT vs placebo, p<0·0001), with no difference between PPOIT and OIT (-5·03% [-20·40 to 10·34], p=0·52). Treatment-related adverse events were reported in 72 (91%) of 79 children in the PPOIT group, 73 (88%) of 83 children in the OIT group, and 28 (72%) of 39 children in the placebo group. Exposure-adjusted incidence of adverse events was 10·58 in the PPOIT group, 11·36 in the OIT, and 2·09 in the placebo group (ratio 0·92 [95% CI 0·85 to 0·99] for PPOIT vs OIT, p=0·042; 4·98 [4·11-6·03] for PPOIT vs placebo, p<0·0001; 5·42 [4·48-6·56] for OIT vs placebo, p<0·0001), with differences seen primarily in gastrointestinal symptoms and in children aged 1-5 years. During the 12-month post-treatment period, 60 (85%) of 71 participants in the PPOIT group, 60 (86%) of 70 participants in the OIT group, and six (18%) of 34 participants in the placebo group were eating peanut; rescue epinephrine use was infrequent (two [3%] of 71 in the PPOIT group, four [6%] of 70 in the OIT group, and none in the placebo group)., Interpretation: Both PPOIT and OIT were effective at inducing sustained unresponsiveness. Background: In patients with chronic lymphocytic leukaemia, achievement of a complete response with minimal residual disease of less than 0·01% (ie, <1 chronic lymphocytic leukaemia cell per 10 000 leukocytes) in bone marrow has been associated with improved progression-free survival. We aimed to explore the activity of induction therapy for 9 months with obinutuzumab and ibrutinib, followed up with a minimal residual disease-driven therapeutic strategy for 6 additional months, in previously untreated patients., Methods: We did a single-arm, phase 2 trial in 27 university hospitals, general hospitals, and specialist cancer centres in France. Eligible patients were at least 18 years old and previously untreated, and had immunophenotypically confirmed B-cell chronic lymphocytic leukaemia; an Eastern Cooperative Oncology Group (ECOG) performance status score of less than 2; a Binet stage C according to IWCLL 2008 criteria or Binet stage A and B with active disease; no 17p deletion or absence of p53 mutation; and were considered medically fit. In the first part of the study (induction phase), all participants received eight intravenous infusions of obinutuzumab 1000 mg over six 4-weekly cycles and oral ibrutinib 420 mg once per day for 9 months. In part 2, after assessment on day 1 of month 9, patients with a complete response and bone marrow minimal residual disease of less than 0·01% received only oral ibrutinib 420 mg once per day for 6 additional months. Patients with a partial response, or with a complete response and bone marrow minimal residual disease of 0·01% or more, received 6 months of four 4-weekly cycles of intravenous fludarabine, cyclophosphamide, and obinutuzumab 1000 mg, alongside continuing ibrutinib 420 mg once per day. The primary endpoint was the proportion of patients achieving a complete response with bone marrow minimal residual disease less than 0·01% on day 1 of month 16 assessed by intention to treat (ITT). This trial is registered with ClinicalTrials.gov (number NCT02666898) and is still open for follow-up., Findings: Between Oct 27, 2015, and May 16, 2017, 135 patients were enrolled. After induction treatment (day 1 of month 9), 130 patients were evaluable, of which ten (8%) achieved a complete response with bone marrow minimal residual disease of less than 0·01% and were assigned to ibrutinib, and 120 (92%) were assigned to ibrutinib plus fludarabine, cyclophosphamide, and obinutuzumab. After minimal residual disease-guided treatment (day 1 of month 16), 84 (62%, 90% CI 55-69) of 135 patients (ITT population) achieved a complete response with bone marrow minimal residual disease of less than 0·01%. The most common haematological adverse event was thrombocytopenia (in 45 [34%] of 133 patients at grade 1-2 in months 1-9 and in 43 [33%] of 130 patients at grade 1-2 in months 9-15). The most common non-haematological adverse events were infusion-related reactions (in 83 [62%] patients at grade 1-2 in months 1-9) and gastrointestinal disorders (in 62 [48%] patients at grades 1 and 2 in months 9-15). 49 serious adverse events occurred, most frequently infections (ten), cardiac events (eight), and haematological events (eight). No treatment-related deaths occurred., Interpretation: Obinutuzumab and ibrutinib induction therapy followed by a minimal residual disease driven strategy is safe and active in patients with previously untreated chronic lymphocytic leukaemia.
