Autoimmune diseases are a cluster of disorders characterized by a failure of the immune tolerance and a hyperactivation of the immune system that leads to a chronic inflammation state and the damage of several organs. The medications currently used to treat these diseases usually consist of immunosuppressive drugs that have significant systemic toxic effects and are associated with an increased risk of opportunistic infections. Recently, several studies have demonstrated that mesenchymal stem cells have immunomodulatory properties, a feature that make them candidates to be used in the treatment of autoimmune diseases. In the present study, we reviewed the role of this therapy in the treatment of systemic lupus erythematosus, Sjögren's syndrome, systemic sclerosis, Crohn's disease and multiple sclerosis, as well as the potential risks associated with its use., (Copyright © 2013 Elsevier España, S.L.U. A series of measures in the management of patients with systemic lupus erythematosus (SLE) which usually are not found in the lupus guidelines are discussed. In the lupus patient who has been well-controlled in the long term, the dose of hydroxychloroquine should be progressively reduced, without decreasing more than approximately 600 mg per week. We recommend taking this drug in the morning in patients with insomnia, at night in those with dyspepsia and to separate the intake of the drug from the shower (and the water should be as cool as possible) in those patients with aquagenic pruritus. We do not use prednisone on alternate days and exceptionally divide the dose into ¾ before breakfast and ¼ before dinner. Twenty to 30 min should be used per patient in every scheduled visit to assure a good clinical and human practice. We analyzed the follow-up of 112 consecutive patients from our systemic disease unit and found that 71.4% of them had symptoms that were unexplained by lupus and we only referred 8.9% of them to other specialists, probably because of our general training as internal medicine doctors. We suggest that knowing the views of SLE specialists might be of interest since, well-designed studies that would allow to progress in the understanding of this disease could be performed based on their experience., (Copyright © 2013 Elsevier España, S.L. Background and Objective: Besides reducing blood pressure, it is important to think about the blood pressure variability that hypertensive patients may present. Indeed, studies suggest that target organ damage is greater in hypertensive people with high blood pressure variability. The purpose of the present study was to determine the blood pressure variability in patients who had not been treated with antihypertensive medication, as well as its relationship with the glomerular filtration., Patients and Method: We included 702 patients with clinical indication for 24h ambulatory blood pressure monitoring., Results: 43.6% of patients had high variability of the blood pressure. These patients had greater levels of 24-h systolic blood pressure, diurnal and night blood pressure and a lower glomerular filtration. The main factors associated with high blood pressure variability were age, years from diagnosis of arterial hypertension, systolic blood pressure and a glomerular filtration smaller than 60ml/min., Conclusions: A high variability of blood pressure is frequent among hypertensive patients. Since it is not possible to perform ambulatory blood pressure measurement in all them, we should mainly suspect it in old patients and in those with reduced glomerular filtration.

Background and Objective: Arterial stiffness is one of the early lesions of the arteries in patients with cardiovascular risk factors. Pulse wave velocity (PWV) is one form of measurement of arterial stiffness. The renin-angiotensin system seems to be involved in the inflammatory mechanisms that take place at level of the vascular wall. The aim of this study is to investigate the effect of olmesartan (angiotensin II type 1 receptor antagonist) in the arterial stiffness, measuring the PWV. Another secondary objective is to determine the antihypertensive efficacy with ambulatory blood pressure monitoring (ABPM)., Patients and Method: Seventy-one patients with mild-to-moderate essential hypertension were consecutively included. Clinical blood pressure (CBP), ABPM and PWV, automatized measurement between the carotid and femoral arteries using the Complior device (Colson, Paris, France), were determined in a baseline and after 16 weeks of treatment with 10-40 mg of olmesartan., Results: Sixty-four patients completed the study. The mean (standard deviation) age was 48.31 (9.69) years and 44.9% were men. A significant reduction of the PWV was observed. The basal PWV was 10.50 (1.87) m/s and after treatment was 9.26 (1.84) m/s (p < 0.0001). We found only a positive correlation between the decline of PWV and diastolic blood pressure (BP). We could not find such correlation with systolic BP. The reduction degree was higher in the youngest patients where BP decrease was less evident. The BP decreased in a significant way (p < 0.0001) doing so in CBP and in the periods of 24 h, diurnal and nocturnal., Conclusions: Olmesartan has shown effective to decrease the arterial stiffness, mainly in young patients. This effect seems to be independent of the decrease of the systolic BP. Olmesartan reduces effectively the BP during the 24 h.

Background and Objective: The prevalence of primary aldosteronism (PA) has experienced an important increase, and many authors consider this condition as the main cause of secondary hypertension (HT)., Patients and Method: Retrospective study of a series of 54 patients having PA who were studied in our Unit between 1999 and 2003., Results: The prevalence of PA was 5.1%. Out of 54 PA cases, 13 corresponded to aldosterone-producing adenomas (APA), 30 to bilateral adrenal hyperplasia (BAH), one was one case of nodular bilateral hyperplasia and another case was a nodular unilateral hyperplasia. In 9 cases, an etiologic diagnosis could not be done. APA were more frequent in women and BAH in men; with regard to sex, no significant differences were found. The blood pressure (BP) was significantly higher in patients with APA compared with BAH patients. In patients with APA, kalemia was significantly lower than in BAH patients. Adrenal CT scan identified 90% of APA, while scintigraphy detected 100% of BAH. Spironolactone therapy significantly decreased the BP in APA and BAH patients, although this fall was higher in patients with APA., Conclusions: The prevalence of PA was 5.1%. Although the tests used for the screening and diagnosis of PA are controversial, a PA ought to be investigated in all patients with refractory HT, independently of the existence of hypokalemia. Spironolactone is an effective therapy for BAH and it is an adequate option for APA treatment when an adrenalectomy is not viable.

Basis: A significant proportion of our patients has described to have problems from tolerance to Dolquine, a new presentation of hydroxychloroquine recently marketed in Spain, compared to Plaquenil. The objective was to know the tolerability and the adverse effects of this new presentation., Patients and Method: A cross-sectional multicenter study on 133 patients treated with Dolquine was conducted., Results: Of the 133 patients (87% women; average age [AA]: 32.9 [15.4] years) who received Dolquine during an average period of 6.7 (1.4) months, 32 patients (24%) described to have more problems with this drug in comparison with other antimalarial. The adverse effects experienced were: bitter taste (62.4%), difficulty in swallowing the tablet (13.5%), dyspepsia (9.8%), nausea (7.5%), vomiting (1.5%), pruritus (1.5%), diarrhea (0.7%), and instability feeling (0.7%). The presence of gastrointestinal adverse effects was not related to the consumption of gastroerosive drugs, gastric protectors, or a high number of drugs. The attrition rate was 9.8%. Conclusions. Dolquine induces lower tolerance and more gastrointestinal adverse effects than Plaquenil, pointing out its bitter taste and the difficulty in swallowing it. Despite this higher intolerance there was not an increase in the attition rate from the antimalarial treatment in comparison to other series.

Background and Objective: There are not studies about the utility of modified liberation doxazosin (MLD) in the combined treatment of the true resistant hypertension (TRH). The aim of this study was to determinate the effect of MLD on the blood pressure (BP), insulin resistance, and the catecholamine secretion in these patients., Patients and Method: Twenty seven patients with TRH from our unit were studied above and after 12 weeks of treatment with MLD (8 mg/day). The following parameters, among others, were analysed: office BP, ambulatory BP (ABPM), rennin, aldosterone and insulin plasmatic levels, urinary catecholamines and the insulin sensitivity index., Results: Two patients were excluded because of adverse sides. After the treatment period, 53.8% of patients experimented a decrease of office diastolic BP higher than 10 mmHg, and 44% of them presented a decrease of 24-hour diastolic BP higher than 5 mmHg (answer patients). Likewise, 38.5% and 24% of patients achieved an adequate office BP and ABPM control, respectively. Also, a significant decrease of insulin resistance and urinary catecholamines, and an increase of aldosterone in plasma were observed. The rest of parameters remained invariables., Conclusions: MLD could be useful in the TRH treatment, because this drug decreased significantly BP, insulin resistance and catecholamine secretion in these patients, with a good tolerance.

Objective: To analyze the clinical manifestations, blood pressure level, and determinants of the degree of hypertension (HT) control in the patients referred by the Primary Care physicians and evaluated for the first time in a Hypertension Clinic., Method: The participants were 1,317 consecutively referred patients for the first time to the Hypertension Clinic by their Primary Care physician, between January 1997 and November 2000. The following initial study protocol was applied: case history and physical exploration, blood and urine biochemistry, chest and abdomen x-rays, fundus oculi and electrocardiogram. According to the initial clinical orientation and the results of the complementary tests described, the study was supplemented or not with other complementary studies. Results were registered in a computer dabatase and subsequently were analyzed with the statistical program SPSS 9.0. RESULTS. 741 (56.3%) of the 1,317 patients were women. The average age was 54.58 14.4 years (range: 14-91 years). Average systolic blood pressure (SBP) and diastolic blood pressure (DBP) were 152.6 22.7 mmHg and 94.2 11.6 mmHg, respectively. 71.6% (943 patients) presented at least two cardiovascular risk factors (CVRF), 50.5% (665 patients) three CVRF and 10.4% (137 patients) four CVRF. 43.6% (575 patients) presented a degree I, 33.2% (438 patients) a degree II and 23.1% (304 patients) a degree III HT. The degree of HT was associated with the age, the time elapsed from the beginning of the HT, the body mass index (BMI) and the levels of blood glucose, total cholesterol and LDL cholesterol. A model of logistic regression to classify the patients as HT degrees I or II-III identified as independent variables the age, the time elapsed from the beginning of HTA, the BMI, the presence of diabetes mellitus (DM) and the plasma LDL cholesterol level greater of 160 mg/dl., Conclusions: Half of the patients sent for the first time by its Primary Care physician to our Hypertension Clinic presented at least three CVRF. Furthermore, 56.4% presented a moderate-severe HTA (degrees II-III). Finally, the degree of HTA of the patients was related to the age, the time elapsed from the beginning of the HTA, the Quetelet's index, and the existence of DM and hypercholesterolemia (HC).

Background: White coat hypertension (WCH) is a prevalent clinical situation which requires a therapeutic management different from persistent hypertension (PH). To distinguish between patients with WCH and uncertain hypertension from patients with PH, an ambulatory blood pressure monitoring (ABPM) is usually indicated, yet it is not available in primary care. Thereby, predictors of WCH on the basis of pre-test (pre-ABPM) clinical characteristics have been suggested. However, little is known about the predictors of PH. The aim of this study was to ascertain predictors of PH in patients referred from the primary care due to suspicion of WCH or uncertain hypertension., Patients and Method: A 24-hours ABPM was performed in 230 consecutive patients referred from primary care because of suspicion of WCH or uncertain hypertension. WCH was defined as an increased office BP with a mean daytime BP, as measured by ABPM, < 135/85 mmHg. Uncertain hypertension was diagnosed when patients had had episodic (2 or more) office BP >140 and/or 90 mmHg together with normal BP determinations. Patients with increased office BP with a mean daytime BP [by ABPM]3 135/85 mmHg were considered as having PH., Results: In 178 patients, ABPM was successful. Eighty-six patients (48.3%) had PH and the remainder (92 patients; 51.7%) were considered as having WCH. In the PH group, there were more males (67.4% vs 43.5%; p < 0.001), patients were older (42.8 [11.8] years vs 35.7 [11.2] years), there were more smokers (39.5 vs 26.1%; p = 0.056), they consumed more alcohol (p = 0.001) and coffee (p < 0.001) and they had higher levels of hemoglobin (p = 0.001) and creatinine (p = 0.003) and lower levels of uric acid (p<0.001) than the WCH group. Also they had an office BP and an ambulatory BP higher than WCH patients. A multivariate logistic regression analysis revealed that PH was significantly associated with a male gender (odds ratio [OR] = 3.26; confidence interval [CI]: 1.54-6.88; p = 0.001), office systolic BP > 145 mmHg (OR = 6.53; CI, 2.67-16.11; p < 0.001), age (> 35 years) (OR = 5.03; CI, 2.35-10.78; p < 0.001) and smoking (OR = 3.07; CI, 1.38-6.84; p = 0.005)., Conclusions: Our findings indicate that in patients referred from primary care due to suspicion of WCH or uncertain hypertension, the prevalence of PH was 48.3%. PH was more frequent among men older than 35 years, smokers and those with an ambulatory systolic BP > 145 mmHg.

Background: The association of uveitis and systemic disease is well known. Patients suffering from uveitis often undergo a extensive battery of tests in order to detect underlying disease, but the efficiency of such screening is uncertain. The aim of this study was to investigate useful clinical data for recognizing secondary uveitis., Patients and Methods: We conducted a prospective analysis of 115 patients with uveitis of unknown etiology. All of them were included in an extensive protocol study. Four groups were considered: specific ocular disease (SOD), idiopathic uveitis, HLA-B27 associated uveitis without arthritis (HLA-B27-AU) and secondary uveitis. Groups were compared by analysis of variance for continuous variables, and chi 2 test or Student's t-test for discrete variables. A stepwise multiple discriminant analysis was performed for ranking the variables in order of their usefulness for distinguishing idiopathic and secondary uveitis., Results: We diagnosed 11 SOD (9.6%), 54 idiopathic uveitis (47%), 6 HLA-B27-AU (5.2%) and 41 secondary uveitis (35.7%). The discriminant analysis showed that age, an elevated erythrocyte sedimentation rate, presence of cutaneous lesion, joint pain and genital ulcers are the strongest predictors of secondary uveitis. This model classification functions detected 92.5% of idiopathic uveitis and 72% of secondary uveitis. The global percentage of patients with a correct diagnosis was 84.6%., Conclusions: Anamnesis, physical examination and basic laboratory tests are sufficient tools for the diagnostic approach of the majority of patients with uveitis. Subsequent diagnostic procedures must be planned in each patient to confirm a specific disease.

Background: Large granular T lymphocytes (LGL) make up a small portion of cellular population in peripheral blood. An abnormal proliferation of LGL is detected together with cytopenic and other autoimmune disorders and is often associated with rheumatoid arthritis. The association with other autoimmune diseases, such as systemic lupus erythematosus, is poorly known. The clinical and immunological profile in five patients with SLE and LGL proliferation is here reported., Materials and Methods: A clinical follow-up and prospective phenotypic study of mononuclear cells was conducted in patients with SLE for a period of 24 months. LGL were identified on the basis of their shape and analyzed by flow cytometry as cells coexpressing thymic differentiation antigens (CD3 and CD4, or CD8) and NK cells CD16, CD56 or CD57)., Results: Five out of 43 patients with SLE showed recurrent proliferations of LGL (from 2 to 4 per patient) chronologically associated with lupus exacerbations. LGL represented 52 to 78% (mean +/- SD = 56 +/- 8%) from the total of lymphocytes. The phenotype in proliferations was heterogeneous but it was consistent in later relapses in each patient (patient #1 and #2: CD3+CD8-CD4+CD16+CD56+CD57-HLA/D+ patients #3 and #4: CD3+CD8+CD4-CD16+CD56+CD57-HLA/DR+ patient #5: CD3+CD8+CD4- CD16+CD56+CD57-HLA/DR+patient 5:CD3+CD8+CD4-CD16 +/- CD56-CD57+HLA/DR+). These five patients had long term SLE with a greater number of exacerbations and a tendency to develop hemocytopenias, requiring high doses of corticosteroids and even immunosuppressors to control their condition., Conclusions: Some patients with SLE develop LGL proliferations. The activity, clinical severity and hematological involvement seem to be associated with this immunological disorder, but the pathogenic significance and prognosis of these proliferations are still to be elucidated.
