Percutaneous and endovascular techniques take an important role in the therapeutic management of patients with hepatocarcinoma. Different techniques of percutaneous ablation, especially indicated in tumors up to 2cm diameter offer, at least, similar results to surgical resection. Taking advantage of double hepatic vascularization and exclusive tumor nutrition by the artery, several endovascular techniques of treating the tumor have been developed. Intra-arterial administration of embolizing particles, alone or charged with drug (chemoembolization), will produce ischemia and consequent necrosis with excellent results in selected patients. Certain types of particles may exclusively be carriers of a therapeutic agent when they incorporate a radioisotope that facilitates the direct intratumoral selective irradiation (radioembolization). This technique has demonstrated its efficacy in lesions not susceptible to be treated with other methods and should be considered, together with ablation and chemoembolization, in the therapeutic algorithms of hepatocarcinoma., (Copyright © 2017 SERAM. Objectives: To analyze the safety and efficacy of percutaneous placement of coated self-expanding metallic stents (SEMS) in patients with biliary leaks., Material and Methods: This ethics committee at our center approved this study. We retrospectively reviewed all coated SEMS placed between October 2008 and September 2015. We analyzed patient-related factors such as the primary underlying disease, prior hepatic procedures, and clinical outcome. We evaluated the location, the number and type of leak (anastomotic or non-anastomotic), and the characteristics of the interventional procedure (number of stents deployed, location of the stents, technical success, and primary functionality). We recorded the complications registered., Results: We studied 14 patients (11 men and 3 women). The mean follow-up period was 375.5 days (range 15-1920 days). Leaks were postsurgical in 12 patients. One patient developed an arteriobilioportal fistula. In another, the biliary leak occurred secondary to the rupture of the common bile duct after ERCP. A total of 23 coated SEMS were placed, including 21 Fluency ® stents (Bard, Tempe, AZ, USA) and 2 Wallflex ® stents (Boston Scientific, Galway, Republic of Ireland). The technical success of the procedure was considered total in 11 (78.6%) patients, partial in 2 (14.3%) patients, and null in 1 (7.2%) patient. The clinical outcome was good in 13 of the 14 patients. The mean period of primary functionality of the coated SEMS was 331 days (range 15-1920 days). No major complications were observed in 11 (78.6%) patients., Conclusions: Percutaneous placement of coated SEMS for the treatment of benign biliary leaks is safe and efficacious, with a high rate of technical and clinical success and a moderate rate of complications., (Copyright © 2016 SERAM. Objective: To review the principal findings on computed tomography angiography for segmental arterial mediolysis, and to emphasize the points that help to differentiate it from other vasculopathies such as vasculitis. We also review the protocols for follow-up and the various treatment options., Conclusion: Segmental arterial mediolysis is a rare disease that is defined as a non-atherosclerotic, non-hereditary, and non-inflammatory vasculopathy characterized by lysis of the medial layer of the arterial wall. It should be suspected in middle-aged patients with aneurysms, dissections, or spontaneous ruptures of visceral arteries of unknown etiology who do not fulfill the clinical and laboratory criteria for vasculitis. The arteries of the abdominal organs are the most commonly affected, including the arteries of the celiac trunk and the superior and inferior mesenteric arteries. Radiologically, segmental arterial mediolysis can present as arterial dilation; single or multiple, saccular or fusiform aneurysms; stenoses; or dissections., (Copyright © 2016 SERAM. Objectives: To evaluate the safety and patency of self-expanding stents to treat hepatic venous outflow obstruction after orthotopic liver transplantation. To evaluate differences in the response between patients with early obstruction and patients with late obstruction., Material and Methods: This is a retrospective analysis of 16 patients with hepatic venous outflow obstruction after liver transplantation treated with stents (1996-2011). Follow-up included venography/manometry, ultrasonography, CT, and laboratory tests. We did a descriptive statistical analysis of the survival of patients and stents, technical and clinical success of the procedure, recurrence of obstruction, and complications of the procedure. We also did an inferential statistical analysis of the differences between patients with early and late obstruction., Results: The mean follow-up period was 3.34 years (21-5,331 days). The technical success rate was 93.7%, and the clinical success rate was 81.2%. The rate of complications was 25%. The survival rates were 87.5% for patients and 92.5% for stents. The rate of recurrence was 12.5%. The rate of primary patency was 0.96 (95% CI 0.91-1) at 3 months, 0.96 (95% CI 0.91-1) at 6 months, 0.87 (95% CI 0.73-1) at 12 months, and 0.87 (95% CI 0.73-1) at 60 months. There were no significant differences between patients with early and late obstruction, although there was a trend toward higher rates of primary patency in patients with early obstruction (P=.091)., Conclusions: Treating hepatic venous outflow obstruction after orthotopic transplantation with self-expanding stents is effective, durable, and effective. Patients with malignant liver tumors, whether primary tumors or metastases, that are not candidates for surgical treatment can benefit from different endovascular treatments with proven efficacy in local control of the disease. Correct treatment requires a careful angiographic technique and precise knowledge about the vascular anatomy afferent to the lesion. Occasionally, lesions considered relapse are actually areas that were untreated because the afferent pedicle was not adequately detected. On the other hand, some of the complications of endovascular treatments are related with material passing into non-hepatic vessels. Knowing the hepatic vascular anatomy and correctly identifying all the extrahepatic vessels will make it possible to perform safer, more efficacious treatments. Intra-arterial treatment has become a first-line alternative in the therapeutic arsenal against primary and metastatic hepatic tumors. Despite its proven safety and efficacy, intra-arterial treatment with lipiodol, chemotherapy, and particles carries the risk of complications, some of which can be lethal. We present the case of a 32-year-old woman with multifocal hepatocellular carcinoma treated with microsphere embolization. After the second session, the patient died of a pulmonary embolism composed of particles and tumor cells.

High flow priapism occurs mainly as a result of a posttraumatic arteriocavernous fistula. Treatment aims to seal the fistula without damaging erectile function. Of all the available treatments, supraselective embolization of the lacerated artery achieves optimal sealing of the fistula with minimal damage to the surrounding tissues. There is no consensus regarding the best materials to be used. Although reabsorbable materials have traditionally been the most commonly used, it is also possible to achieve adequate resolution of the priapism with adequate conservation of erectile function using non-reabsorbable materials, especially microcoils. We describe two cases of patients with high flow priapism caused by bilateral arteriocavernous fistulas treated using microcoil embolization.

Objective: To study the prevalence and characteristics of tobacco use in the last years of the high school students in Gran Canaria (Spain) enrolled in the 1996-1997 academic year., Methods: Cross-sectional study of a sample of 931 students obtained through a two-stage method by conglomerates and stratification based on habit and type of educational institution. The information was gathered from a personal questionnaire that included identification variables, characteristics of tobacco consumption, family and social environment and attitudinal factors about restrictive measures., Results: 66.8% of participants were non-smokers and 4% ex-smokers. Of the remaining, 18.1% were habitual smokers and 11.1% occasional smokers. The use of tobacco is higher in women than in men (p < 0.001) even though the quantity of tobacco consumed is the same. On the average, the consumption of tobacco begins at 15 years (+/- 1.5 years). There is not significant relation between tobacco use in parents and their children; however, there is a relation between tobacco use in those questioned and their siblings and friends: 87.5% of smokers had friends who also smoked and 42% a sibling who smoked. When those questioned started, only 12% of their parents had allowed them to smoke, reaching 50% when they were interviewed. Parents were more permissive with regard to tobacco consumption with males., Conclusions: The prevalence of tobacco use and age at starting are lower in our population than in other studies. Any action taken should concentrate on avoiding the incorporation of tobacco into daily life, creating a social and family environment where the use of tobacco is exceptional.

Background: The aim of this study was to determine the effect of ciprofloxacin at subinhibitory concentrations on the expression of P fimbriae of uropathogenic Escherichia coli. Thirty-nine strains of Escherichia coli isolated from out patients with urinary tract infection were studied. Thirty-nine of these strains had been previously characterized as P-fimbriated and the remaining non fimbriated strain was used as a negative control., Methods: Fimbriation was quantitatively studied by electron microscope observation of the strains before and after treatment. To determine possible qualitative variations in the fimbrial proteins and in the external membrane (OMPs), extraction and electrophoretic separation was performed in polyacrylamide gels., Results: No qualitative differences were observed in the OMPs profile and fimbrial proteins induced by ciprofloxacin in any of the strains studied. However, electron microscopic observation generally showed a decrease in the percentage fimbriated bacterial cells by the antimicrobial effect., Conclusions: The mechanism of action of ciprofloxacin at subinhibitory doses may correspond to a process of fimbrial protein synthesis inhibition secondary to the initiation of general repair mechanism of the cell exposed to the antimicrobial and not to a process of specific mutations which qualitatively affect fimbrial protein composition.

Background: The aim of this study was to determine the virulence factors and 0 serogroups of E. coli strains that cause community acquired urinary tract infections (UTI)., Methods: We examined 103 E. coli strains isolated from the urine of patients with UTI. The following virulence factors were investigated using phenotypic techniques: the alpha-haemolysin (Hly), the cytotoxic necrotizing factor type 1 (CNF-1) and the mannose-resistant haemagglutination (MRHA) types III, IVa and IVb expressed by P fimbriated E. coli. Serotyping of 0 antigen was carried out by means of a microtechnique using 163 antisera., Results: Fifty-five (53%) of the 103 E. coli strains examined showed some of the virulence factors investigated in this study; 41% of the strains were Hly+, 28% were CNF-1+ and 48% expressed MRHA types III, IVa or IVb. The uropathogenic strains characterized belonged to 27 different 0 serogroups. However, 68% were from one of 10 serogroups (01, 02, 04, 06, 09, 018, 027, 073, 075 and 077) and 36% were from one of only 3 serogroups (02, 04 and 06). Furthermore, the virulence factors were concentrated in strains belonging to the 3 serogroups most frequently detected. Thus, 36 (97%) of the 37 strains of these 3 serogroups showed virulence factors, versus only 19 (29%) of 66 belonging to other serogroups (p < 0.001)., Conclusions: Our results support the special pathogenicity theory and suggest that many cases of community acquired UTI may be caused by a limited number of uropathogenic E. coli strains that produce toxins (Hly+ and/or CNF-1+) and possess P fimbriae or P-related adhesins (with MRHA types III, IVa or IVb), and that usually belong to 02, 04 and 06 serogroups.

Background: There are several bacterial determinants that contribute to the onset of urinary tract infection by E. coli. The present study focuses on some of the virulence factors considered to be most important, as P fimbriae, the siderophore aerobactin and bacterial capsule, which were studied among 123 uropathogenic E. coli strains isolated from outpatients from the Basque Community., Methods: Virulence factors were detected using Molecular Biology techniques, namely DNA hybridization to specific probes prepared in our laboratory., Results: When probe pap2, specific for fimbrial adherence was used, 36.5% of the strains showed positive hybridization, and 66 and 73% of the strains hybridized to probes for aerobactin and common capsule region, respectively., Conclusions: We believe that this technology provides a very useful tool for rapid and easy screening of strains harbouring different virulence factors. Nevertheless, the fact that these methods detect genetic determinants that are not always being expressed must be borne in mind.

Objective: To discover the clinical-epidemiological characteristics of pneumonias in our community., Design: Descriptive study., Setting: Andoain Health Centre., Patients and Others Participants: There were 8,862 people in the reference population. All the cases of acquired pneumonia in the community were recorded. This register included those diagnosed in the Health Centre and in Casualty Departments and hospitals where patients were referred, between June 1 1991 and September 30 1992. The inclusion criteria were: acute fever chart and/or acute respiratory infection accompanied by a radiological image of pneumonic condensation., Interventions: The model treatment was 500 mg of Erythromycin at six-hourly intervals for 14 days. 500 mg of Cefuroxim every twelve hours was added in cases considered at risk., Measurements and Main Results: A total of 97 pneumonias were recorded, which represented an incidence of 8.82 pneumonias per thousand inhabitants per year. 63% were male. The average age was 42 (SD 19.25). 83% were diagnosed at the Health Centre. One-third presented a recent viral infection. Half had some risk factor: tobacco use was the most common. Response to treatment was satisfactory. Five percent had some complication. No death was recorded., Conclusions: Pneumonia is a frequent cause of attendance at Primary Care centres. There are several clear clinical symptoms, a good response to treatment with Erythromycin and virtual absence of complications [corrected].

Due to the controversy on the need of performing temporal artery biopsies for the diagnosis of giant cell arteritis (GCA), we have reviewed 204 biopsies of temporal arteries as well as the clinical histories of the 190 patients. Patients without local manifestations were excluded from this study. From the 173 remaining patients, 54 presented a diagnostic biopsy of GCA. The 119 patients with a negative biopsy were divided into different groups according to the final diagnosis. From our data we could not find a clinical pattern which could permit the prediction of biopsy positiveness. We could not find either any clinical entity in the nondiagnostic group which would permit to exclude a diagnosis before performing the biopsy. We consider that the biopsy of the temporal artery can not be avoided nowadays and that it represents the only decisive diagnostic method of GCA.

We have studied the distribution of the coronary reserve, evaluated by serial effort tests, in patients with proved coronaropathy, determining the correlation between clinic (effort and mixed angina) and coronary reserve (fixed and variable), assessing angiographic findings in function to that reserve. We took 120 patients with stable angina to whom 2 effort tests were performed, basal and after vasodilator drugs. It was considered variable reserve if in the second test the S-T descend improved greater than or equal to 1 mm for a similar of greater double product and fixed when it didn't improve. In all patients coronarography was performed. Seventy two patients (60%) showed fixed reserve, 58 with effort angina (80%) and 14 (20%) with mixed. Forty eight showed variable reserve, 40 (80%) with mixed angina and 8 (17%) with effort. The group with fixed reserve had a greater S-T max. descent (2.9 +/- 0.9 vs 2.2 +/- 0.4) (p less than 0.001), a lower double product max. (221 +/- 44 vs 284 +/- 37) (p less than 0.001) and a lower maximal oxygen consumption (MVO2 7 +/- 2 vs 11 +/- 2) (p less than 0.001) than the variable reserve group. Considering the angiography, the fixed reserve group had more number of vessels affected (1.9 +/- 0.7 vs 1.4 +/- 0.5) (p less than 0.01), a higher angiographic score (4.88 +/- 2.4 vs 2.2 +/- 1.2) (p less than 0.001), a lower ejection fraction (59 +/- 8.5 vs 65 +/- 7.5) (p less than 0.001), more multivessel and descending anterior artery lesion than the variable reserve group.(ABSTRACT TRUNCATED AT 250 WORDS)
