Abstract

Resume Introduction La Prise En Charge (PEC) des Fractures Pertrochanteriennes (FPT) en filiere Fast Track (FT) est amorcee en traumatologie. Notre hypothese etait que la chirurgie Fast Track des FPT ne compromet pas l’autonomie du patient. Materiel et methode Notre etude etait prospective, comparative, observationnelle, monocentrique, de 2014 a 2016 au CHU d’Angers. Les patients inclus presentaient une FPT A1 ou A2 (classification AO), isolee, osteosynthesee par enclouage centromedullaire, necessitant une PEC en Soins de Suite et Readaptation (SSR). Le groupe expose FT beneficiait d’un transfert des J1 post operatoire en SSR. Le groupe Non Expose (NE) beneficiait d’une PEC post operatoire dans le service de chirurgie avant transfert en SSR. Le critere de jugement principal etait la difference Delta Parker pour chaque patient, entre son score d’arrivee et de sortie. Les criteres secondaires de jugement etaient la nature des aides a la sortie et le lieu de sortie du SSR. Resultats 109 patients etaient inclus initialement, 54 patients (27 paires) apres appariement. Une difference significative de −1,27 (IC95 : −2,27 ; −0,32) (p = 0,012) en faveur du Delta Parker FT etait mise en evidence. Il n’y avait pas de difference significative sur la nature et la quantite des aides a la sortie du SSR, ni sur le lieu de sortie, bien qu’une tendance mette en evidence que le lieu de vie initial restait privilegie chez le groupe FT. Discussion Le virage FT est amorce en orthopedie. Les etudes precedentes mettent en evidence une duree de sejour amoindrit, des taux de survie et de complications superposables, une economie significative. Cette etude repond a la question du devenir, les patient FT perdent significativement moins d’autonomie de marche. Conclusion La PEC FT des FPT diminue significativement la perte d’autonomie de marche du patient, sans que le lieu de vie ou les aides ne soient impactees significativement. Niveau de preuve IIB, Cohorte Exposee/Non exposee.

Abstract

Summary Background & aims Nutrition support is recommended in cachexic patients with cancer. However, there is no clear evidence about its impact on tumour growth. Glycolysis, which is usually higher in cancer than normal cells, can be monitored by 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) imaging that is widely used for cancer staging and therapy efficacy assessment. Here, we used 18F-FDG PET/CT imaging to investigate whether artificial nutrition has an impact on tumour glucose metabolism in patients with cancer and cachexia. Methods This prospective study included ten patients with histologically proven head and neck or oesophageal cancer. All patients underwent 18F-FDG PET/CT imaging at baseline and after (parenteral and/or enteral) nutrition support on average for 7 days. Tumour glucose metabolism changes were evaluated using static (SUVmax, SUVmean and SULpeak) and dynamic (glucose metabolic rate and transport constant rates, k) parameters computed from the 18F-FDG PET/CT data. Results Artificial nutrition (median energy intake of 21.83 kcal/kg/day [13.16–45.90], protein intake of 0.84 g/kg/day [0.56–1.64]) was administered. Eight patients (80%) received enteral nutrition and two patients (20%) parenteral support. Comparison of 18F-FDG PET/CT parameters did not highlight any significant difference in tumour glucose metabolism before and after the period of nutrition support. Conclusions In cachexic patients with head and neck or oesophageal cancer, nutrition support administered according to the current guidelines shows no impact on tumour glucose metabolism, assessed by 18F-FDG PET/CT.

Abstract

Introduction Completeness of cytoreduction is the most important prognostic factor in patients with advanced ovarian cancer (OC). Extensive upper abdominal surgery has allowed to increase the rate complete cytoreduction and the feasibility of resection of celiac lymph nodes (CLN) and porta hepatis disease in these patients has been demonstrated. The aim of our study was to assess the prognostic impact of CLN involvement in patients with primary advanced OC undergoing a complete cytoreductive surgery (CRS). Material and methods We designed a retrospective unicentric study. We reviewed data from patients who underwent CLN resection with or without porta hepatis disease resection, within upfront or interval complete CRS in the frontline treatment of advanced epithelial OC between January 2008 and December 2015. Patients were classified in two groups according to CLN status. Univariate and multivariate analyses were conducted. Survival rates were estimated using Kaplan-Meier method. Results Forty-three patients were included and positive CLN were found in 39.5% of them. The median disease-free survival in the group of patients with positive and negative CLN were 11.3 months and 25.8 months, respectively. In multivariable analysis, both CLN involvement and high peritoneal cancer index were independently associated with decreased disease-free survival. Computed tomography re-reading by an expert radiologist has good sensitivity for detection of positive CLN. Conclusion CLN involvement and high preoperative tumor burden are independently associated with decreased survival after complete cytoreduction for OC. CLN involvement is a marker of diffuse disease and an independent risk factor for early recurrent disease.

Abstract

International audience; Background & aims: Before antiviral therapy, kidney transplant recipients infected with hepatitis B virus (HBV) or hepatitis C virus (HCV) had poor outcomes. Since the 90s, nucleos(t)ide analogues have been widely used in HBV-infected patients, while interferon-based therapy was rarely used in HCV-infected patients. The aim of this study was to assess the impact of HBV and HCV on patient and graft survival, according to viral replication status.Methods: Data from January 1993 to December 2010 were extracted from the French national database CRISTAL. Abstract

IF 6.448 (2017); International audience; The Pemphigus Disease Area Index (PDAI) and Autoimmune Bullous Skin Disorder Intensity-Score (ABSIS) scores have been proposed to provide an objective measure of pemphigus activity. These scores have been evaluated only on already treated patients mainly with mild to moderate activity. The objective was to assess the interrater reliability of ABSIS and PDAI scores and their correlation with other severity markers in a large international study. Consecutive patients with newly diagnosed pemphigus were enrolled in 31 centers. Severity scores were recorded during a 24-month period by the same two blinded investigators. Serum was collected at each visit for ELISA measurement of anti-desmoglein antibodies. The intraclass correlation coefficient (ICC) and Spearman rank correlation coefficient were calculated. A total of 116 patients with pemphigus vulgaris (n = 84) or pemphigus foliaceus (n = 32) were included. At baseline, the ABSIS and PDAI ICCs were 0.90 (95% confidence interval [CI] = 0.85-0.93), and 0.91(95% CI = 0.87-0.94), respectively. The ICCs for PDAI were higher in moderate and extensive pemphigus (ICC = 0.82, 95% CI = 0.63-0.92 and ICC = 0.80, 95% CI = 0.62-0.90, respectively) than in patients with intermediate (significant) extent (ICC = 0.50, 95% CI = 0.27-0.68). Conversely, the ICCs for ABSIS were lower in patients with moderate extent (ICC = 0.44, 95% CI = 0.004-0.74) than in those with intermediate or extensive forms, (ICC = 0.69, 95% CI = 0.51-0.81 and ICC = 0.75, 95% CI = 0.51-0.88, respectively). During patients' follow-up, the ICCs of both ABSIS and PDAI scores remained higher than 0.70. ABSIS and PDAI skin (r = 0.71 and r = 0.75) but not mucosal (r = 0.32 and r = 0.37) subscores were correlated with the evolution of anti-DSG1 and anti-DSG3 ELISA values, respectively. ABSIS and PDAI scores are robust tools to accurately assess pemphigus activity.

International audience; Background: The routine D-dimer quantification to exclude venous thromboembolism has led to the development of many assays, the usefulness of which depends on their reliability and performance.Objective: We evaluated the analytical performances of the immunoturbidimetric Yumizen G DDi 2 assay (HORIBA Medical, Montpellier, France) performed on the Yumizen G800 analyzer and compared it with other available D-dimer assays.Methods: Within-run and between-run imprecision were evaluated using low- and high-level quality-control plasma samples. Interference due to hemolysis, icterus, lipemia, rheumatoid factor (RF), or heterophilic antibodies (human antimouse antibodies [HAMAs]) was evaluated by spiking plasma samples with hemolysate, bilirubin, Intralipid, RF, or HAMAs. The measurements obtained with the different D-dimer assays were compared using Passing-Bablok regression analysis and Bland-Altman plot method, using fresh citrated plasma samples collected from 66 consecutive routine patients with a wide range of D-dimer concentrations.Results: Within- and between-run variation coefficients for the Yumizen G DDi 2 assay ranged from 1.7% to 5.8% and from 2.8% to 5.5%, respectively. Hemolysis and icterus did not have any effect up to 10 g/L hemoglobin and 300 mg/L bilirubin. Lipemia seemed to generate an underestimation of D-dimer concentration when the Intralipid concentration was >5 g/L. RF and HAMAs did not have any effect. The Passing-Bablok and Bland-Altman analyses showed small differences with other available D-dimer assays, which were more pronounced with increasing values.Conclusions: Its analytical performances and main technical features indicate that the new Yumizen G DDi 2 assay is suitable for the rapid quantification of D-dimer in clinical hemostasis laboratories.

Abstract

Summary Objectives To report our experience of inflating or changing pressure balloon to treat recurrent urinary incontinence after AMS800 ® implantation instead of changing all the devices. Patients and methods A retrospective study was conducted in a tertiary reference center between 2005 and 2015. All patients, treated by AMS800 ® implantation for post-prostatectomy urinary incontinence and whom balloon was subsequently changed or inflated, were included. Main clinical end point was the need for another surgery. Secondary end points were urethral erosion, infection, and efficacy on pad test and pad use. Results Thirty-one patients were included. All had had a 61–70 cm H 2 0 balloon implanted, with a single cuff (13 with transcorporeal placement). Twenty-one patients had their balloon changed for a 71–80 cm H 2 0 type, while 10 patients had their balloon refilled (median 3 mL [range 2–7]). Median follow-up was 23 months (range 1–129). Overall rate of another subsequent surgery was 48.3% ( n = 15). Erosion and atrophy occurred more frequently after balloon repressurizing than after balloon replacement (80% vs 33%, P = 0.024). At last follow-up, median pad use was higher in repressurizing group (2 vs 1, P = 0.033). Conclusion Balloon repressurizing is associated with a higher erosion and reoperation rate than changing pressure balloon. Continence results seem better when PRB is changed. It could be an alternative instead of changing all devices in patients with frail urethra. Level of evidence 4.

Abstract

Les anticorps anti-peptides-cycliques-citrullinees (anti-CCP) sont des biomarqueurs fiables pour caracteriser les patients atteints de polyarthrite rhumatoide (PR). Des etudes ont montre que leur positivite pouvait influencer la reponse aux biomedicaments. L’objectif de notre etude etait de determiner si le titre des anti-CCP a l’introduction du traitement est predictif de remission chez les patients traites par abatacept, inhibiteurs du facteur de necrose tumorale alpha (anti-TNF-α) ou tocilizumab. Materiels et methodes : Dans cette etude observationnelle, retrospective, ont ete inclus des patients atteints de PR suivis au C.H.U. de Rouen, traites par abatacept, anti-TNF-α ou par tocilizumab entre octobre 2009 et mai 2021. Les patients ont ete separes en 3 groupes : anti-CCP negatifs, anti-CCP positifs 340 UI/mL, et cela pour chaque biomedicament. Pour savoir s’il existe un lien entre le titre des anti-CCP et l’obtention d’une remission ou d’une faible activite (definies par le DAS28) a 12 mois de traitement, des analyses univariees et multivariees avec ajustement sur les caracteristiques initiales ont ete realisees. Une analyse en sous groupe a egalement ete effectuee en distinguant parmi les patients traites par anti-TNF-α ceux traites par etanercept de ceux traites par un anti-TNF-α monoclonal. Resultats : Parmi les 329 patients inclus, 99 etaient traites par abatacept (28 anti-CCP negatifs, 40 anti-CCP positifs 340 UI/mL), 180 par anti-TNF-α (38 anti-CCP negatifs, 93 anti-CCP positifs 340 UI/mL) et 50 par tocilizumab (11 anti-CCP negatifs, 23 anti-CCP positifs 340 UI/mL). L’âge median des patients a l’introduction du traitement etait de 57 [47 - 67] ans. Le sex-ratio etait de 2,96 femmes pour 1 homme. La duree d’evolution mediane de la PR avant l’initiation du biomedicament etait de 7 [2 - 12] ans ; 73,6% des patients presentaient une atteinte structurale. Pour chacune des molecules, les caracteristiques des patients a l’introduction du biomedicament etaient comparables, quel que soit le titre des anti-CCP, sauf pour les facteurs rhumatoides (FR) dont la positivite etait moins frequente dans les groupes anti-CCP negatifs. Apres analyse univariee puis ajustement sur les caracteristiques initiales, aucune association significative n’a ete montree entre le titre des anti-CCP et la proportion de remission ou de faible activite chez les patients traites par abatacept, anti-TNF-α ou tocilizumab. Dans l’analyse en sous groupe, le titre des anti-CCP semblait influencer la remission avec l’etanercept, mais pas avec les anti-TNF-α monoclonaux. Cette association n’etait pas retrouvee en analyse multivariee. Conclusion : Le titre des anti-CCP a l’introduction du biomedicament n’a pas d’impact sur l’obtention d’une remission selon le DAS28 apres un delai de 12 mois de traitement par abatacept, anti-TNF-α ou tocilizumab.

Abstract

Summary Objectives There is no strong evidence for second line therapy after male sling failure for post-prostatectomy urinary incontinence (PPUI). We report the outcomes after periurethral balloons implantation for persistence or recurrence of mild PPI symptoms after male sling implantation. Methods All patients implanted of a ProACT™ device (Uromedica, Inc., MN, USA) following I-STOP transobturator male sling (TOMS) failure, in a tertiary reference center between 2009 and 2016, were included. Patients were evaluated by 24-hour pad-test before and after implantation, and after each balloon repressurizing procedure. PGI-I and Likert scale patient satisfaction were estimated during a telephone interview conducted in 2016. Objective and subjective cure of urinary incontinence were defined by a 24-hour pad-test Results Fourteen patients were included. Median follow-up was 34 months [4–89]. Objective and subjective cure were 29% ( n = 4) and 57% ( n = 8), respectively. Median pad-test decreased from 95 g [IQR: 130] to 34 g [IQR: 83] ( P = 0.022). ProACT™ significantly decreased median pad-test by a factor 2.73 [1.19–6.29]. Eighty-eight percent patients were feeling a little better, much better or very much better and 77% were satisfied or very satisfied at the end of follow-up. Reoperation rate was 28% ( n = 4): 3 balloons were changed for caudally migration ( n = 2) or deflation ( n = 1) and 1 had a urinary sphincter implanted for severe UI. Conclusions ProACT™ is a safe and efficient treatment that can be used in second line therapy after TOMS failure for PPUI. Level of evidence 4.

Abstract

International audience; A wide variety of medical devices (MDs) used in hospitals are made of flexible plasticized polyvinylchloride (PVC). Different plasticizers are present in variable amounts in the PVC matrix of the devices and can leach out into the infused solutions and may enter into contact with the patients. The ARMED1 project aims to assess the migration of these plasticizers from medical devices and therefore the level of exposure in patients. For the first task of the project, eight methods were developed to directly detect and quantify the plasticizers in the PVC matrix of the MDs. We compared the overall performances of the analytical methods using standardized and validated criteria in order to provide the scientific community with the guidance and the technical specifications of each method for the intended application. We have shown that routine rapid screening could be performed directly on the MDs using the FTIR technique, with cost-effective analyses. LC techniques may also be used, but with limits and only with individual quantification of the main plasticizers expected in the PVC matrix. GC techniques, especially GC–MS, are both more specific and more sensitive than other techniques. NMR is a robust and specific technique to precisely discriminate all plasticizers in a MD but is limited by its cost and its low ability to detect and quantify plasticizer contamination, e.g. by DEHP. All these results have been confirmed by a real test, called the " blind test " carried out on 10 MD samples.

Abstract

This French multicenter prospective cohort study recruited 391 patients to investigate the risk factors for persistent pain after elective cesarean delivery, focusing on psychosocial aspects adjusted for other known medical factors. Perioperative data were collected and specialized questionnaires were completed to assess reports of pain at the site of surgery. Three dependent outcomes were considered: pain at the third month after surgery (M3, n = 268; risk = 28%), pain at the sixth month after surgery (M6, n = 239; risk = 19%), and the cumulative incidence (up to M6) of neuropathic pain, as assessed using the Douleur Neuropathique 4 questionnaire (n = 218; risk = 24.5%). The neuropathic aspect of reported pain changed over time in more than 60% of cases, pain being more intense if associated with neuropathic features. Whatever the dependent outcome, a high mental component of quality of life (SF-36) was protective. Pain at M3 was also predicted by pain reported during current pregnancy and a history of miscarriage. Pain at M6 was also predicted by report of a postoperative complication. Incident neuropathic pain was predicted by pain reported during current pregnancy, a previous history of a peripheral neuropathic event, and preoperative anxiety. Perspective Persistent pain after cesarean delivery has a relatively frequent neuropathic aspect but this is less stable than that after other surgeries. When comparing the risk factor analyses with published data for hysterectomy, the influence of preoperative psychological factors seems less important, possibly because of the different context and environment.

Abstract

International audience; PURPOSE: Concomitant treatment with radiation therapy and cisplatin (CDDP) remains the gold standard for bladder preservation in the treatment of muscle-invasive bladder cancer (MIBC). We present the long-term results of a phase 1 clinical trial to assess the association of twice-weekly gemcitabine with CDDP and radiation therapy in this setting. METHODS AND MATERIALS: Patients with pT2-pT4N0M0 MIBC without hydronephrosis or diffuse carcinoma in situ were enrolled in this study. After maximal transurethral resection of the bladder tumor, patients received concomitant radiation therapy (63 Gy in 1.8 fractions) and chemotherapy (CDDP 20 mg/m²/day over 4 days every 21 days and gemcitabine twice a week). The starting dose of gemcitabine was 15 mg/m² with dose escalation to 20, 25, and 30 mg/m². The primary endpoint was the maximum tolerated dose (MTD). Secondary endpoints included toxicity and tumor control. RESULTS: Fourteen patients were enrolled. Dose-limiting toxicity occurred in 2 patients treated with 30 mg/m² gemcitabine (grade 4 thrombocytopenia and severe impairment of World Health Organization performance status, respectively). Nine patients received the complete chemoradiation therapy protocol. The recommended dose of gemcitabine was 25 mg/m². The median follow-up time was 53 months, and the overall and disease-specific 5-year survival rates were 62% and 77%, respectively. Among the patients who received the complete treatment, bladder-intact survival was 76% at 5 years, and the median overall survival was 69.6 months. CONCLUSIONS: This regimen was well tolerated. The gemcitabine MTD was 25 mg/m². Bladder preservation and disease control were promising. A multicenter phase 2 randomized trial is ongoing.

Abstract

International audience; BACKGROUND: The aims of the present study were to determine both clinical manifestations and outcome of anti-PL7 patients with antisynthetase syndrome (ASS). METHODS: The medical records of 15 consecutive anti-PL7 patients with biopsy proven ASS were retrospectively analyzed without prior selection. RESULTS: Anti-PL7 patients exhibited polymyositis (n=14) and dermatomyositis (n=1); extra-pulmonary manifestations of ASS included: Raynaud's phenomenon (40%), mechanic's hands (33.3%), joint impairment (26.7%), pericardial effusion (20%) and esophageal/gastrointestinal involvement (20%). The outcome of myositis was as follows: remission/improvement (91.7%) and deterioration (8.3%). Fourteen patients (93.3%) experienced interstitial lung disease (ILD). ILD preceded ASS diagnosis (n=5), was identified concomitantly with ASS (n=8) and occurred after ASS diagnosis (n=1). Patients could be divided into 3 groups according to their presenting lung manifestations: acute onset of lung disease (n=1), progressive onset of lung signs (n=11) and asymptomatic patients exhibiting abnormalities consistent with ILD on PFT and HRCT-scan (n=2). No patient had resolution of ILD, whereas 64.3% and 35.7% experienced improvement and deterioration of ILD, respectively. ILD resulted in respiratory insufficiency requiring O2 therapy in 14.3% of cases. Two patients died. Predictive parameters of ILD deterioration were: DLCO

Abstract

DNA microarray technology enables investigators to measure the expression of several 1000 mRNA species simultaneously in a biological specimen. However, the reliability of the microarray technology to detect transcriptional differences representative of the original samples is affected by the quality of the extracted RNA. Thus, it is of critical importance to standardize sample-handling protocols and to perform a quality assessment of RNA preparations. In this report, 59 human tissue samples were used to evaluate the relationships between RNA quality and gene expression. From Affymetrix GeneChip array data analysis of these samples, we compared the performance of the 28S/18S ratio, two computer methods (RIN and degradometer) and our in-house RNA quality scale (RQS) in assessing RNA quality. The optimal RNA reliability threshold was determined for each method using statistical discrimination measures. We showed that RQS, RIN and degradometer have a similar capacity to detect reliable RNA samples whereas the 28S/18S ratio leads to a misleading categorization. Furthermore, we developed a new approach, based on clustering analyses of full chip expression, to control RNA quality after hybridization experiments. The combination of these methods, allowing monitoring of RNA quality prior to and after the hybridization experiments, ensured reliable and reproducible microarray data.

Abstract

National audience; Introduction. Objectif : la zone sous-thalamique offrant les caractéristiques optimales de stimulation chronique (DBS) dans la maladie de Parkinson est encore mal connue ; nous avons souhaité la définir à partir de données électrophysiologiques et anatomiques opératoires, puis la comparer à la zone couverte par le contact électrique (DBS).Matériel-Méthode. Quatorze malades parkinsoniens ont été évalués prospectivement. Les électrodes ont été implantées bilatéralement, en un seul temps, sous anesthésie locale, à partir d’une visée IRM stéréotaxique directe de la portion antérieure du noyau sous-thalamique (NST). Un contact (DBS monopolaire) a été placé, en aveugle de l’anatomie, sur le site de meilleure efficacité pour un faible courant (seuil bénéfique, sb) et où les effets indésirables apparaissaient pour une marge élevée (marge de stimulation, ms). Les 28 trajectoires ont été analysées indépendamment. Les liens entre anatomie (contourage manuel des structures) et électrophysiologie (stimulation et enregistrement) et ceux entre anatomie et contacts (n = 28 ; radiographie stéréotaxique et IRM postopératoire) ont été évalués : ANOVA complétée par une comparaison multiple (Tukey) pour les données quantitatives et Chi2 pour les qualitatives (significativité : p < 0,05).Résultats ou Cas rapporté. La portion latéro-dorsale du NST offrait les meilleurs paramètres de stimulation (mAmp) avec un sb = 0,78 ±0,44 et une ms = 1,78 ±0,88. La portion médio-ventrale du NST possédait la plus forte activité neuronale (burstique et tonique). Le NST latéro-dorsal, le NST intermédiaire (entre les zones latérale et médiale) et les régions de proximité, H2 et zona incerta étaient les principales structures incluses dans le volume du contact DBS (79 % des inclusions).
