Between September 2016 and November 2020, 17 cases of difficult-to-treat resistant Pseudomonas aeruginosa (DTR-PA) were reported in haematology patients at a tertiary referral hospital in the North of England.A retrospective case-control study was conducted to investigate the association between DTR-PA infection and clinical interventions, patient movement, antimicrobial use and co-morbidities.Cases were patients colonised or infected with the outbreak strain of DTR-PA who had been admitted to hospital prior to their positive specimen. A retrospective matched case-control study was performed. Exposures were extracted from medical records and cases were compared to controls using conditional logistic regression. Environmental and microbiological investigations were also conducted.17 cases and 51 controls were included. The final model included age (65,aOR 6.85,p=0.232), sex (aOR 0.60,p=0.688), admission under the transplant team (aOR 14.27,p=0.43) and ciprofloxacin use (aOR 102.13,p=0.030). Investigations did not indicate case-to-case transmission or a point source, although a common environmental source was highly likely.This study demonstrates fluoroquinolone use as an independent risk factor for DTR-PA in haematology patients. Antimicrobial stewardship and review of fluoroquinolone prophylaxis should be considered as part of PA outbreak investigations in addition to standard infection control interventions. more...

Genetic heart diseases (GHDs) can be clinically heterogeneous and pose an increased risk of sudden cardiac death (SCD). The implantable cardioverter-defibrillator (ICD) is a lifesaving therapy. Impacts on prospective and long-term psychological and health-related quality of life (HR-QoL) after ICD implant in patients with GHDs are unknown.Investigate the psychological functioning and HR-QoL over time in patients with GHDs who receive an ICD, and identify risk factors for poor psychological functioning and HR-QoL.A longitudinal, prospective study design was used. Patients attending a specialized clinic, diagnosed with a GHD for which they received an ICD between May 2012 and January 2015, were eligible. Baseline surveys were completed prior to ICD implantation with 5-year follow-up after ICD implant. We measured psychological functioning (Hospital Anxiety Depression Scale, Florida Shock Anxiety Scale), HR-QoL (Short-Form 36v2), and device acceptance (Florida Patient Acceptance Scale).Forty patients were included (mean age 46.3 ± 14.2 years; 65.0% male). Mean psychological and HR-QoL measures were within normative ranges during follow-up. After 12 months, 33.3% and 19.4% of participants showed clinically elevated levels of anxiety and depression, respectively. Longitudinal mixed-effect analysis showed significant improvements from baseline to first follow-up for the overall cohort, with variability increasing after 36 months. Nontertiary education and female sex predicted worse mental HR-QoL and anxiety over time, while comorbidities predicted depression and worse physical HR-QoL.While the majority of patients with a GHD adjust well to their ICD implant, a subset of patients experience poor psychological and HR-QoL outcomes. more...

Summary Background Internationally, smoking prevalence among people in prison custody (ie, people on remand awaiting trial, awaiting sentencing, or serving a custodial sentence) is high. In Scotland, all prisons implemented a comprehensive smoke-free policy in 2018 after a 16-month anticipatory period. In this study, we aimed to use data on medication dispensing to assess the impact of this policy on cessation support, health outcomes, and potential unintended consequences among people in prison custody. Methods We did an interrupted time-series analysis using dispensing data for 44 660 individuals incarcerated in 14 closed prisons in Scotland between March 30, 2014, and Nov 30, 2019. We estimated changes in dispensing rates associated with the policy announcement (July 17, 2017) and full implementation (Nov 30, 2018) using seasonal autoregressive integrated moving average models. Medication categories of primary interest were treatments for nicotine dependence (as an indicator of smoking cessation or abstinence attempts), acute smoking-associated illnesses, and mental health (antidepressants). We included antiepileptic medications as a negative control. Findings A 44% step increase in dispensing of treatments for nicotine dependence was observed at implementation (2250 items per 1000 people in custody per fortnight, 95% CI 1875 to 2624) due primarily to a 42% increase in dispensing of nicotine replacement therapy (2109 items per 1000 people in custody per fortnight, 1701 to 2516). A 9% step decrease in dispensing for smoking-related illnesses was observed at implementation, largely accounted for by respiratory medications (−646 items per 1000 people in custody per fortnight, −1111 to −181). No changes associated with announcement or implementation were observed for mental health dispensing or antiepileptic medications (control). Interpretation Smoke-free prison policies might improve respiratory health among people in custody and encourage smoking abstinence or cessation without apparent short-term adverse effects on mental health dispensing. Objectives To investigate the safety and cost-effectiveness of lengthening the time between surveillance ultrasound scans in the UK Abdominal Aortic Aneurysm (AAA) Screening Programme. Methods A discrete event simulation model was used to evaluate the cost-effectiveness of AAA screening for men aged 65, comparing current surveillance intervals to 6 alternative surveillance interval strategies that lengthened the time between surveillance scans for 1 or more AAA size categories. The model considered clinical events and costs incurred over a 30-year time horizon and the cost per quality-adjusted life year (QALY). The model adopted the National Health Service perspective and discounted future costs and benefits at 3.5%. Results Compared with current practice, alternative surveillance strategies resulted in up to a 4% reduction in the number of elective AAA repairs but with an increase of up to 1.6% in the number of AAA ruptures and AAA-related deaths. Alternative strategies resulted in a small reduction in QALYs compared to current practice but with reduced costs. Two strategies that lengthened surveillance intervals in only very small AAAs (3.0-3.9 cm) provided, at a cost-effectiveness threshold of £20 000 per QALY, the highest positive incremental net benefit. There was negligible chance that current practice is the most cost-effective strategy at any threshold below £40 000 per QALY. Conclusions Lengthening surveillance intervals in the UK Abdominal Aortic Aneurysm Screening Programme, especially for small AAA, can marginally reduce the incremental cost per QALY of the program. Strong evidence demonstrates that race is associated with health outcomes. Previous neurosurgical research has focused predominantly on subjective data, such as patient satisfaction. Our objective was to assess whether racial disparities are present in primary objective outcomes for treatment of intracranial, unruptured aneurysms in the United States.Data from the 2012-2015 National Inpatient Sample (NIS) database was analyzed. Patients who underwent either open or endovascular treatment of unruptured intracranial aneurysms were included (n = 11663). Patients were stratified by race, and those of unknown race or whose race sample size was too underpowered for analysis were excluded (n = 1202), along with those who experienced head trauma (n = 110) or concurrent AVM (n = 71). Poor outcome was defined as in-hospital mortality, discharge to a nursing facility or hospice, placement of a tracheostomy tube, or placement of a gastrostomy tube. The associations between race and adverse outcomes were determined through multivariate logistic regression, corrected for potentially confounding variables such as age, sex, procedural type, elective procedure, obesity, diabetes, tobacco, severity of illness, and hospital type.7478 White, 1460 Black, 1086 Hispanic, and 279 Asian patients were included in the final analysis. Complication rates were not significantly different between races, however Black patients experienced the highest proportion of complications (24 %). After adjusting for confounders, the odds of poor outcomes were significantly higher for Black patients (OR = 1.32 95 % CI: 1.07-1.62; p = 0.008) when compared to White patients. Black and Hispanic patients demonstrated a longer length of stay (Black, B: 0.04; 95 % CI: 0.03, 0.06; p 0.001; Hispanic, B: 0.04; 95 % CI: 0.02, 0.05; p 0.001) when compared to White patients.Our nationwide analysis using the NIS suggests that Black patients treated for unruptured intracranial aneurysms experience worse outcomes and longer lengths of stay when compared to White patients. Recognizing the differences in objective outcomes and the presence of neurosurgical healthcare disparities is an important first step in providing equitable care to all patients. Affiliative interspecies interactions are important to community structure within an ecosystem yet understudied relative to antagonistic relationships. Mixed-species associations provide important benefits via foraging, predator avoidance and social interactions. Tactile behaviours help with social bond formation and group establishment among conspecifics during social interactions, but little is known about the role of contact between heterospecific animals in mixed-species groups. Our objective was to quantify characteristics of pectoral fin contact between individuals of two sympatric species of wild dolphins, spotted dolphins, Stenella frontalis, and bottlenose dolphins, Tursiops truncatus, off Bimini, The Bahamas, in order to assess possible benefits of heterospecific interactions. Using underwater video recordings from mixed-species dolphin encounters from 2011 to 2017, we observed 74 pectoral fin contacts, of which 27 were between heterospecific animals. Encounters were either affiliative (61.5%) or sociosexual (38.5%). Conspecific and heterospecific dolphins had similar rates of initiating and receiving contacts. This suggests that during mixed-species groups, the cost-benefit trade-off in choosing a social partner is similar for conspecific and heterospecific individuals. Heterospecific contacts initiated by S. frontalis were primarily affiliative whereas heterospecific contacts initiated by T. truncatus were sociosexual. Both sociosexual behaviour and pectoral fin contact in dolphins have been suggested as mechanisms of bond formation. We conclude that mixed-species interactions with pectoral fin contact may be a mechanism for social bond formation between heterospecifics and that these bonds may help to re-establish groups, as needed, for predator avoidance. more...

Objectives To evaluate the relationships between the athlete distribution of team performance indicators and quarter outcome in elite women’s Australian Rules football matches. Design Retrospective longitudinal cohort analysis. Methods Thirteen performance indicators were obtained from 56 matches across the 2017 and 2018 Australian Football League Women’s (AFLW) seasons. Absolute and relative values of 13 performance indicators were obtained for each athlete, in each quarter of all matches. Eleven features were further extracted for each performance indicator, resulting in a total of 169 features. Generalised estimating equations (GEE) and regression decision trees were run across the different feature sets and dependent variables, resulting in 22 separate models. Results The GEE algorithm produced slightly lower mean absolute errors across all dependent variables and feature sets comparative to the regression decision tree models. Quarter outcome was more accurately explained when considered as total points scored comparative to quarter score margin. Team differential and the 75th percentile of individual athlete Inside 50s were the strongest features included in the models. Conclusions Modelling performance statistics by quarter outcomes provides specific practical information for in-game tactics and coaching in relation to athlete performances each quarter. Seminal works by Birch and Ihara gave formulas for the mth power moments of the traces of Frobenius endomorphisms of elliptic curves over F p for primes p ≥ 5 . Recent works by Kaplan and Petrow generalized these results to the setting of elliptic curves that contain a subgroup isomorphic to a finite abelian group A. We revisit these formulas and determine a simple expression for the zeta function Z p ( A ; t ) , the generating function for these mth power moments.
