21. Variations in common operations in athletes and non-Athletes

Abstract

BACKGROUND: Achieving pre-injury activity level after an injury is the fundamental goal of any orthopedic treatment for an athlete. Unfortunately, pre-injury activity levels differ significantly in different patient categories, especially in athletes and non-athlete. Hence, an outcome suitable to a non-athlete may not be adequate for an athlete. This has led to variations in the surgical approach to the same injury in an athlete and non-athlete. There is plenty of literature published comparing the outcome in athletes and non-athletes after a particular surgery. Scattered discussion about variations in these surgeries based on functional demand was done in many publications. But there was a lack of a comprehensive narrative review summarizing variations in common operations among athletes and non-athletes. AIM: This review attempted to summarize variations in common sports operations between high functional demand patients and low demand patients and discuss the variations from the author's perspective. METHODS: A review of all the relevant papers were conducted focusing on athletes and non-athletes. Most commonly performed sports surgeries were ACL reconstruction, Meniscal repair, PCL reconstruction, and Shoulder instability surgery. A literature search was done for each commonly performed surgery using relevant keywords in PubMed and Google Scholars. Summary of papers pertinent to athletes and non-athletes were compiled to prepare this narrative review. RESULTS: There is a lack of papers directly comparing results in athletes and non-athletes. However, many research papers discussed surgical variations in athletes (high demand) and non-athletes (low demand) patients. There are controversies in all commonly performed surgeries, and none of the papers gives a definitive guideline on the approach to athletes and non-athlete. CONCLUSION: Rather than a common suggestion on surgical variation, an individualized approach would be appropriate to decide on variation in particular surgery in both athletes and non-athletes.

Abstract

A total of 22 patients who had developed an adverse cutaneous reaction to the Moderna or Pfizer vaccine underwent biopsies. Each patient was assessed light microscopically, and, in select biopsies, spike glycoprotein and cytokine assessment were also conducted. The patients developed self-limited cutaneous reactions often described clinically as urticarial or eczematous within 1 day to 4 weeks after receiving the first or second dose of the Pfizer or Moderna vaccine. Classic clinical and morphologic depictions of type IV cutaneous hypersensitivity with features of eczematous dermatitis, interface dermatitis, granulomatous inflammation, and/or lymphocytic vasculitic component were observed. Clinical and/or histologic features of perniosis, pityriasis rosea, pityriasis rubra pilaris, and guttate psoriasis were seen in select cases. In 2 cases the dominant picture was urticarial vasculitis, possibly reflective of an Arthus type III immune complex action. The biopsy specimens of normal skin post vaccine and of skin affected by the post-vaccine eruption showed rare deep microvessels positive for spike glycoprotein with no complement deposition contrasting with greater vascular deposition of spike protein and complement in skin biopsies from patients experiencing severe coronavirus disease 2019 (COVID-19). It is concluded that self-limited hypersensitivity reactions to the vaccine occur possibly owing to a substance found in the vaccine vehicle (eg, polyethylene glycol). An immune response that is directed against human-manufactured spike has to be considered because some of the reactions clinically and or histologically closely resemble mild COVID-19. Finally, vaccine-associated immune enhancement largely attributable to the adjuvant properties of the vaccine may unmask certain inflammatory milieus operational in psoriasis, atopic dermatitis, and subclinical hypersensitivity.

Abstract

The stability of the atmosphere plays a major role in the dispersion of pollutants. The meteorological conditions favoring a high-pressure situation resulting in the formation of inversion layer inhibits pollutant dispersion. The aim of the study was to obtain vertical profiles of meteorological parameters and air pollutants in order to investigate the impact of inversion on the air quality of this city. Vertical profiles help to understand the behaviour of temperature inversion, pollutants and their relationship to each other. The tethered balloon was equipped with in-situ instruments to measure Particulate Matter (PM), Black Carbon (BC), Ultra Fine Particles (UFP) as well as meteorological parameters such as air temperature, relative humidity, wind speed and wind direction of the lower troposphere. The measurements were carried out in “Schlossgarten”, a park area near the city center of Stuttgart. Vertical profiles were obtained by ascending the balloon from ground up to a height of a maximum of 470 m and then descending it back to the ground. One complete sounding (ascent and descent) took around 30–40 min. The measurement campaign took place on 8th and July 9, 2018. 43 soundings were performed during that period. During the measurement campaign, a distinct inversion was observed at nighttime, which trapped the pollutants, hence increasing the pollutant concentration near the ground within the inversion layer. PM, UFP and BC concentrations showed a decreasing trend with increasing altitude but during noon, when the atmosphere was well mixed, the concentrations were relatively constant along with increasing altitude.

Abstract

Background Ghŗta mūrcchana is a process of pre-treatment recommended in Ayurveda to purify ghee before it can be used for siddha ghŗta which is claimed to improve the properties of the ghee in general and that of the prepared siddha ghŗta. Objective This work is aimed at studying the physiochemical properties of ghee and mūrcchita ghŗta in order to understand the impact of ghŗta mūrcchana process. Materials and methods Ghee and mūrcchita ghŗta were prepared from the milk of local Pahadi, Jersey and Holstein cows. The samples were characterized by FTIR spectroscopy, differential scanning calorimetry and free fatty acid measurements. Results Among the samples studied, the Holstein cow ghee was found to contain the least amount of free acid (1.34%) whereas ghŗta mūrcchana process led to further decrease in the free acid content polymorphism was observed in the samples as evidenced by multiple melting points. In most cases, mūrcchita ghŗta was found to contain less solid fat than the corresponding ghee implying that the high melting compound was converted to low melting one during the process. Conclusion The observed lowering of free fatty acid and solid fat contents in the ghee samples may provide a possible validation to the performance enhancement of the ghŗta mūrcchana process.
