In the present work, a novel algorithm for the continuous projection of point triangles belonging to a triangle mesh is presented. The algorithm uses the normals defined at the vertices of the triangle to perform the projection. The direction of projection is not chosen a priori but depends on the point to be projected: the proposed algorithm, named Fast Continuous Projection method (FCP) lets the projection direction vary continuously on the mesh. Moreover, the direction is coherent with the original surface that is approximated with the triangle mesh. An optimized version of the algorithm is also presented: this uses pre-evaluated matrices to reduce the calculation time. This algorithm can be effectively used when a large set of points has to be projected on a coarse mesh as, for example, to generate the scanning vectors for laser engraving/milling.

Classification of myeloid neoplasms with isolated isochromosome i(17q) [17p deletion with inherent monoallelic TP53 loss plus 17q duplication] is controversial. Most cases fall within the WHO unclassifiable myelodysplastic/myeloproliferative neoplasms (MDS/MPN-U) category. The uniformly dismal outcomes warrant better understanding of this entity. We undertook a multi-institutional retrospective study of 92 adult MDS/MPN-U cases from eight institutions. Twenty-nine (32%) patients had isolated i(17q) [MDS/MPN-i(17q)]. Compared to MDS/MPN without i(17q), MDS/MPN-i(17q) patients were significantly younger, had lower platelet and absolute neutrophil counts, and higher frequency of splenomegaly and circulating blasts. MDS/MPN-i(17q) cases showed frequent bilobed neutrophils (75% vs. 23%; P = 0.03), hypolobated megakaryocytes (62% vs. 20%; P = 0.06), and a higher frequency of SETBP1 (69% vs. 5%; P = 0.002) and SRSF2 (63% vs. 5%; P = 0.006) mutations that were frequently co-existent (44% vs. 0%; P = 0.01). TP53 mutations were rare. The mutation profile of MDS/MPN-U-i(17q) was similar to other myeloid neoplasms with i(17q) including atypical chronic myeloid leukemia, chronic myelomonocytic leukemia, myelodysplastic/myeloproliferative neoplasm with ring sideroblasts and thrombocytosis, myelodysplastic syndrome and acute myeloid leukemia, with frequent concomitant SETBP1/SRSF2 mutations observed across all the diagnostic entities. Over a median follow-up of 52 months, patients with MDS/MPN-i(17q) showed a shorter median overall survival (11 vs. 28 months; P < 0.001). The presence of i(17q) retained independent poor prognostic value in multivariable Cox-regression analysis [HR 3.686 (1.17-11.6); P = 0.026] along with splenomegaly. We suggest that MDS/MPN-i(17q) warrants recognition as a distinct subtype within the MDS/MPN-U category based on its unique clinico-biologic features and uniformly poor prognosis.

BACKGROUND: Risdiplam is an oral small molecule approved for the treatment of patients with spinal muscular atrophy, with approval for use in patients with type 2 and type 3 spinal muscular atrophy granted on the basis of unpublished data. The drug modifies pre-mRNA splicing of the SMN2 gene to increase production of functional SMN. We aimed to investigate the safety and efficacy of risdiplam in patients with type 2 or non-ambulant type 3 spinal muscular atrophy. METHODS: In this phase 3, randomised, double-blind, placebo-controlled study, patients aged 2-25 years with confirmed 5q autosomal recessive type 2 or type 3 spinal muscular atrophy were recruited from 42 hospitals in 14 countries across Europe, North America, South America, and Asia. Participants were eligible if they were non-ambulant, could sit independently, and had a score of at least 2 in entry item A of the Revised Upper Limb Module. Myeloid/lymphoid neoplasms (M/LN) with 13q12/FLT3 rearrangement have been suggested as candidates for possible inclusion in the World Health Organization classification group of M/LN with eosinophilia (M/LN-eo). We report 12 patients with confirmed FLT3 rearrangement, six with t(12;13)/ETV6-FLT3; one with ins(13;22)/BCR-FLT3; and five with an unconfirmed partner gene located on chromosome bands 2p16, 3q27, 5q15, 5q35, and 7q36. Disease presentations were heterogeneous, including lymphoblastic leukemia/lymphoma, myeloid sarcoma, chronic eosinophilic leukemia, chronic myelomonocytic leukemia, and myelodysplastic syndrome. However, some common features were observed, such as extramedullary involvement (n = 7, 58%), associated eosinophilia in blood, bone marrow, or tissue (n = 8, 67%), multilineage involvement, either as biphasic myeloid/lymphoid neoplasms (n = 2) or mixed phenotype acute leukemia (n = 2). Mutations were detected in 4/8 (50%) patients by next-generation sequencing. None (0/10) had FLT3 or KIT mutations. Eleven patients received disease-based chemotherapy or hypomethylating agents, three received FLT3 inhibitors, and five patients proceeded to hematopoietic stem cell transplant. Together with a review of 16 cases published in the literature, it is apparent that M/LNs with FLT3 rearrangement show disease features reminiscent of members in the category of M/LN-eo with PDGFRA, PDGFRB, FGFR1, and PCM1/JAK2 rearrangement, characterized by a specific gene rearrangement, frequent eosinophilia, multi-lineage involvement and therapeutic benefit from kinase inhibitors.

Natural killer (NK) cells are lymphocytes of the native immune system that play a pivotal role in host defense and immune surveillance. While the conceptual view of NK-neoplasms is evolving, little is known about the rare NK lymphoblastic leukemia (NK-LL), which remains as a provisional entity in the 2016 WHO Classification. The goal of this study is to characterize NK-LL cases and compare with other CD56 co-expressing acute leukemias. We identified 105 cases, diagnosed as NK-LL (6), CD56+ acute undifferentiated leukemia (AUL) (6), CD56+ T-lymphoblastic leukemia (T-LL) (51), and CD56+ acute myeloid leukemia (AML) (42). Compared to AUL patients, NK-LL patients were significantly younger (p = 0.021) and presented with higher white blood cell (WBC) (p = 0.037) and platelet counts (p = 0.041). Flow cytometry showed more frequent expression of cytoplasmic CD3 (cCD3, p = 0.064) and CD33, (p = 0.065), while HLA-DR was significantly absent from NK-LL (p = 0.035) compared to AUL. Compared to T-ALL, NK-LL cases showed less frequent cCD3 (p = 0.002), CD4 (p = 0.051), and CD10 expression (p = 0.06). The frequency of abnormal karyotypes was similar between NK-LL, AUL, and T-ALL. The mutational profile differed in four leukemia groups, with a significance enrichment of NOTCH1 (p = 0.002), ETV6 (p = 0.002) and JAK3 (p = 0.02) mutations in NK-LL as compared to AML. As compared to T-ALL, NK-LL cases showed a higher number of total mutations (p = 0.04) and significantly more frequent ETV6 mutations (p = 0.004). Clinical outcome data showed differences in overall survival between all four groups (p = 0.0175), but no difference in event free survival (p = 0.246). In this largest study to date, we find that that NK-LL shows clinical presentation, immunophenotypic and molecular characteristics distinct from AUL, T-ALL, and AML. Our findings suggest NK-LL is a distinct acute leukemia entity and should be considered in the clinical diagnosis of acute leukemias of ambiguous lineage.

Cautionary stories in which misbehavior results in negative outcomes are often used in public service announcements (PSAs) to promote behavioral change. These cautionary stories can either disclose or withhold their endings and the associated negative outcomes for the characters involved. In four experiments, we show that disclosing (vs. withholding) a story’s ending increases persuasion due to greater counterfactual thinking about alternative actions that could have prevented the negative outcomes. Integrating these findings within the Transportation-Imagery Model of narrative persuasion, we also show how dispositional levels of need for cognitive closure can amplify the effect of ending disclosure in a PSA. Our findings have important implications for both marketing communicators and policy makers who seek to improve the effectiveness of PSAs.

Library material, and thus parchment, is frequently subjected to bio-deterioration processes caused by microorganisms. Fungi and bacteria cause alterations in the parchment inducing, in some cases, even the partial detachments of the surface layer and the loss of any text present on it. An important contribution to disinfection of the cultural heritage artefacts is given by the use of ionizing radiation. In this work, a preliminary study on the applicability of X-ray radiation as treatment for bio-deterioration removal is proposed. The results on the microbial growth after different irradiation treatments are shown in order to detect the dose protocol for the bio-degradation removal. Furthermore, the evaluation of the irradiation effects on the parchment microstructure is presented in order to define the applicability of the method on parchment artefacts.

A laboratory investigation was performed to better understand the performance of Cement-Treated Base (CTB) mixtures containing Recycled Asphalt Pavement (RAP) during a long-lasting service period. Two CTB mixtures, one containing only virgin aggregates and the other containing RAP and virgin aggregates, were tested by triaxial tests (static and cyclic) and resonant column tests to evaluate stiffness and dampening at very small deformations. Similar mechanical properties and response to cyclic loadings were obtained for the two mixtures. The results led to the conclusion that the use of RAP as a replacement for virgin aggregates for a CTB mixture does not cause any significant alteration in terms of mechanical performance.

Hydrogen adsorption stability, geometry, electronic structure and mechanism has been investigated on Ni4 cluster embedded in graphene with three, four and six vacancies by density functional theory (DFT) calculations. An energetic analysis of hydrogen adsorption by addition of one to four H2 molecules was performed for each system in order to determine their hydrogen storage capacity. Dispersion force contribution to the adsorption energy is quantitative evaluated to know whether H2 molecules adsorption behavior is dominated by chemical or van der Waals interactions. A further analysis of this type of interactions is also addressed by total and partial density of states. Bonding and charge transfer characteristics for the different steps involved in the adsorption mechanism are also included. Special attention is given to the effects caused by this new Ni/graphene interface to the hydrogen adsorption behavior.

The study of bronze statues obtained by the lost-wax casting method results of particular relevance from the artistic and historical points of view. In particular, the characterization of structural and subsurface elements of bronzes is very useful to gather insight concerning their preservation state and manufacturing process. For these purposes, infrared thermography has been successfully employed to investigate different kind of items of artistic bronzes, such as repairs, defects and inserts, providing also the possibility to perform the depth-resolved study of the observed subsurface features thanks to the possibility it also provides to evaluate the thermal transport properties of the investigated materials. In this work, the experimental configurations and the capabilities of such a technique have been reviewed. The state of the art of the results obtained by infrared thermography for the analysis of artistic bronzes is presented. It is thus shown that infrared thermography can be nowadays considered as a reliable and consolidated technique to map in a non-destructive way a wide spectrum of features of bronzes.

TP53 alteration in chronic lymphocytic leukemia indicates a high-risk disease that is usually refractory to chemotherapy. It may be caused by deletion of 17p involving the loss of TP53 gene, which occurs in low percentage of patients at diagnosis but can be acquired as the disease progresses. Since patients may harbor TP53 mutation without chromosome 17p deletion, consensus recommendations call for both cytogenetic and PCR mutation analysis of TP53 in chronic lymphocytic leukemia. We conducted a single-institution retrospective study to investigate the clinicopathologic features of chronic lymphocytic leukemia with TP53 alterations as well as the utility of different diagnostic modalities to identify p53 alterations. Forty percent of chronic lymphocytic leukemia patients with TP53 alterations demonstrated atypical lymphocytes with cleaved/irregularly shaped nuclei and/or large atypical lymphoid cells with abundant cytoplasm in the peripheral blood. Progression was also observed in lymph node and bone marrow samples (21% with Richter transformation; 33% with findings suggestive of “accelerated phase” of chronic lymphocytic leukemia including prominent proliferation centers and/or increased numbers of prolymphocytes). However, the presence of the morphologic features suggestive of “accelerated phase” had no effect on overall survival within the chronic lymphocytic leukemia group with TP53 abnormalities (p > 0.05). As previously reported by others, a subset of patients with TP53 alterations were only identified by either PCR mutation analysis (12%) or cytogenetic studies (14%). p53 immunostain positivity was only identified in approximately half of the patients with TP53 alterations identified by either method, and it failed to identify any additional patients with p53 abnormalities. In summary, chronic lymphocytic leukemia patients with TP53 alterations frequently show atypical morphologic features. Use of multiple modalities to identify p53 abnormalities is recommended to ensure optimal sensitivity and specificity.

Surface structuring of medical implants has been shown to reduce the rejection rate of medical devices inserted inside living tissue. Several studies have shown that by texturing the surface of medical implants at micro/nanoscale it is possible to foster cell adhesion. The surface texturing of medical devices poses many challenges in terms of cost and time when the nano texturing process is performed directly on the part surface. A viable solution for mass production of polymer based surface enhanced medical devices is achieved via injection molding. However, nano structuring of micro injection molding molds, requires techniques enabling processing of complex surfaces, small curvature radii and with high aspect ratios. Furthermore the accessibility of all surfaces to be structured and the processing time are the main concerns and limiting factors on the selection of the structures dimensions and geometries. In this work, two different process chains are analyzed for the nano structuring of a bone micro plug. The first method relies on a chemical deposition process in order to produce a semi random structure on the entire mold surface of the medical component. The characteristic size of the structures is controlled by acting on the deposition parameters. The second process, relies on laser structuring of the mold components by producing Laser-Induced Periodic Surface Structures (LIPSS).The two process chains are compared and an evaluation of the applicability in different production scenarios presented, with specific focus on surface accessibility, nano structures uniformity and applicability to surfaces with complex geometry

In the present work, the generation of Laser Induced Periodic Surface Structures (LIPSS) by using femtosecond laser source is investigated in the surface texturing of Ti grade 5 dental implants. The proposed procedure permits a fast treatment of dental implants characterized by complex shape through the combined and synchronous use of a galvo scanner and of workpiece movements. The obtained surface micro- and nanostructures are characterized from a morphological point of view while in-vitro essays are performed to evaluate cellular proliferation. The results indicate the effectiveness of the LIPSS as method to increase cell viability and the efficiency of the proposed procedure to treat complex geometries as dental implants.
