Abstract

Background & aims: An estimated 38% of adults worldwide have non-alcoholic fatty liver disease (NAFLD). From individual impacts to widespread public health and economic consequences, the implications of this disease are profound. This study aimed to develop an aligned, prioritised fatty liver disease research agenda for the global health community. Methods: Nine co-chairs drafted initial research priorities, subsequently reviewed by 40 core authors and debated during a three-day in-person meeting. Following a Delphi methodology, over two rounds, a large panel (R1 n = 344, R2 n = 288) reviewed the priorities, via Qualtrics XM, indicating agreement using a four-point Likert-scale and providing written feedback. The core group revised the draft priorities between rounds. In R2, panellists also ranked the priorities within six domains: epidemiology, models of care, treatment and care, education and awareness, patient and community perspectives, and leadership and public health policy. Results: The consensus-built fatty liver disease research agenda encompasses 28 priorities. The mean percentage of ‘agree’ responses increased from 78.3 in R1 to 81.1 in R2. Five priorities received unanimous combined agreement (‘agree’ + ‘somewhat agree’); the remaining 23 priorities had >90% combined agreement. While all but one of the priorities exhibited at least a super-majority of agreement (>66.7% ‘agree’), 13 priorities had [removed]90% combined agreement. Conclusions: Adopting this multidisciplinary consensus-built research priorities agenda can deliver a step-change in addressing fatty liver disease, mitigating against its individual and societal harms and proactively altering its natural history through prevention, identification, treatment, and care. This agenda should catalyse the global health community's efforts to advance and accelerate responses to this widespread and fast-growing public health threat. Abstract

The principal limitations of the terms NAFLD and NASH are the reliance on exclusionary confounder terms and the use of potentially stigmatising language. This study set out to determine if content experts and patient advocates were in favour of a change in nomenclature and/or definition. A modified Delphi process was led by three large pan-national liver associations. The consensus was defined a priori as a supermajority (67%) vote. An independent committee of experts external to the nomenclature process made the final recommendation on the acronym and its diagnostic criteria. A total of 236 panellists from 56 countries participated in 4 online surveys and 2 hybrid meetings. Response rates across the 4 survey rounds were 87%, 83%, 83%, and 78%, respectively. Seventy-four percent of respondents felt that the current nomenclature was sufficiently flawed to consider a name change. The terms “nonalcoholic” and “fatty” were felt to be stigmatising by 61% and 66% of respondents, respectively. Steatotic liver disease was chosen as an overarching term to encompass the various aetiologies of steatosis. The term steatohepatitis was felt to be an important pathophysiological concept that should be retained. The name chosen to replace NAFLD was metabolic dysfunction–associated steatotic liver disease (MASLD). There was consensus to change the definition to include the presence of at least 1 of 5 cardiometabolic risk factors. Those with no metabolic parameters and no known cause were deemed to have cryptogenic steatotic liver disease. A new category, outside pure metabolic dysfunction–associated steatotic liver disease, termed metabolic and alcohol related/associated liver disease (MetALD), was selected to describe those with metabolic dysfunction–associated steatotic liver disease, who consume greater amounts of alcohol per week (140–350 g/wk and 210–420 g/wk for females and males, respectively). Abstract

Unlabelled: The principal limitations of the terms nonalcoholic fatty liver disease (NAFLD) and nonalcoholic steatohepatitis (NASH) are the reliance on exclusionary confounder terms and the use of potentially stigmatising language. This study set out to determine if content experts and patient advocates were in favour of a change in nomenclature and/or definition. Methods: A modified Delphi process was led by three large pan-national liver associations. Consensus was defined a priori as a supermajority (67%) vote. An independent committee of experts external to the nomenclature process made the final recommendation on the acronym and its diagnostic criteria. Results: A total of 236 panellists from 56 countries participated in four online surveys and two hybrid meetings. Response rates across the 4 survey rounds were 87%, 83%, 83% and 78%, respectively. 74% of respondents felt that the current nomenclature was sufficiently flawed to consider a name change. The terms 'non-alcoholic' and 'fatty' were felt to be stigmatising by 61% and 66% of respondents, respectively. Steatotic liver disease (SLD) was chosen as an overarching term to encompass the various aetiologies of steatosis. The term steatohepatitis was felt to be an important pathophysiological concept that should be retained. The name chosen to replace NAFLD was metabolic dysfunction-associated steatotic liver disease (MASLD). There was consensus to change the definition to include the presence of at least one of five cardiometabolic risk factors. Those with no metabolic parameters and no known cause were deemed to have cryptogenic SLD. A new category, outside pure MASLD, termed MetALD was selected to describe those with MASLD who consume greater amounts of alcohol per week (140 to 350g/week and 210 to 420g/week for females and males respectively). Conclusions: The new nomenclature and diagnostic criteria are widely supported, non-stigmatising and can improve awareness and patient identification.

11. Effects of woodland restoration and management on the community of surface-active arthropods in the metropolitan Chicago region

Abstract

Restoration of woodlands that have been invaded by exotic plants has primarily focused on restoring vegetation structure by removing invaders and planting native species that have declined in abundance. Management practices to date continue to focus on plant communities, but if restoring ecosystem integrity is the goal of restoration, knowledge of how the fauna has recovered is essential. We examined the impact of vegetation restoration and management on the surface-active arthropod community across a spectrum of 22 woodland sites in the greater metropolitan Chicago region. Sites were grouped into three categories based on existing condition. Invaded sites had never been restored or managed (“Control”, n = 5); had been undergoing restoration for 3–21 years (“Managed-int”, n = 12) but were not yet near the management goal; or were restored plots (11–21 years of management) that land managers identified as representative of their restoration target based upon the vegetation present (“Managed-REF”, n = 5). Each site was a one-ha plot containing four pitfall traps used to assess activity-densities of 35 taxa of epigeic arthropods. Permutational analysis of variance (PERMANOVA) and subsequent canonical analysis of principal coordinates (CAP) revealed that arthropod community structure varied between Control and Managed-REF sites, with the Managed-int sites demonstrating convergence toward the Managed-REF. The activity-densities of non-native isopods (detritivores) were nearly twice as high in Control sites compared to Managed-REF sites, whereas traps in Managed-REF sites had four times the number of Collembola (fungivores). Distance-based redundancy analysis (dbRDA) revealed that invasive woody plant cover and rates of uptake of soil P and NO3− by root simulators explained over 40% of the variation in arthropod community structure. Our findings suggest that restoration management targeted at the vegetation also restores the arthropod community in woodlands to a composition that has fewer non-native arthropods.

Abstract

First- and second-generation immunometric PTH assays during treatment of hyperparathyroidism with cinacalcet HCl.BackgroundFirst-generation immunometric assays for “intact” parathyroid hormone (iPTH) also measure large N-terminally truncated PTH fragments, whereas second-generation assays, such as the “bio-intact” PTH (biPTH) assay, measure only full-length biologically active PTH(1-84). This study compared iPTH and biPTH assays during cinacalcet treatment in subjects with secondary HPT receiving dialysis.MethodsFour hundred and ten subjects were enrolled in a 26-week randomized, double-blind, placebo-controlled trial of oral cinacalcet (or placebo), 30 to 180 mg once daily, and efficacy was assessed using biPTH and iPTH assays.ResultsCompared with control treatment, cinacalcet improved the management of secondary HPT. Both biPTH and iPTH decreased by 38% ± 3% during weeks 13 to 26 in the cinacalcet group; biPTH increased by 23% ± 4% and iPTH increased by 9.5% ± 3% in the control group (P < 0.001). Fifty-six percent of cinacalcet subjects and 10% of control subjects had a ≥30% reduction in biPTH, and 61% and 11%, respectively, had a ≥30% reduction in iPTH. Significant correlations between biPTH and iPTH levels were observed throughout the study. Both assays correlated similarly with bone-specific alkaline phosphatase levels. The ratio of biPTH to iPTH was maintained at 56% ± 1% after treatment in both treatment groups. Increasing serum calcium levels were associated with a decreasing ratio of biPTH to (iPTH-biPTH).ConclusionThese data show that PTH can be monitored with either iPTH or biPTH assays during therapy with cinacalcet, and that cinacalcet therapy does not exert a major influence on the ratio between PTH(1-84) and large, N-terminally truncated PTH fragments.

1,25-dihydroxyvitamin D(3) (1,25(OH)(2)D(3)), the biologically active form of vitamin D, is widely recognized as a modulator of the immune system as well as a regulator of mineral metabolism. The objective of this study was to determine the effects of vitamin D status and treatment with 1,25(OH)(2)D(3) on diabetes onset in non-obese diabetic (NOD) mice, a murine model of human type I diabetes. We have found that vitamin D-deficiency increases the incidence of diabetes in female mice from 46% (n=13) to 88% (n=8) and from 0% (n=10) to 44% (n=9) in male mice as of 200 days of age when compared to vitamin D-sufficient animals. Addition of 50 ng of 1,25(OH)(2)D(3)/day to the diet prevented disease onset as of 200 days and caused a significant rise in serum calcium levels, regardless of gender or vitamin D status. Our results indicate that vitamin D status is a determining factor of disease susceptibility and oral administration of 1,25(OH)(2)D(3) prevents diabetes onset in NOD mice through 200 days of age.

Abstract

Reactions of chloro(diethylenetriamine)palladium(II) with 4-thio-2′-deoxyuridine, 4-thiouridine-5′-monophosphate, d(T8s4UT8), 6-thio-2′-deoxyinosine, 6-mercapto-purine riboside-5′-monophosphate and d(T8s6IT8) have been investigated using diode-array and conventional stopped-flow spectroscopy in acidic aqueous solution as a function of excess concentration of Pd(II) and temperature. The observed rate constants and activation parameters indicated a reaction mechanism where the labile chloro ligand is directly replaced by the thione unit in the coordination sphere of the Pd(II) complex. Incorporation of both thionucleosides into single-stranded oligonucleotides was found to increase the rate of adduct formation by a factor of ca. 2–3, compared to reactions of the corresponding mononucleotides. Compared to the monomer reactions, adduct formation in DNA-environment results in an increase of both the ΔH≠- and ΔS≠-values. The observed DNA-promoted reactivity is thus a consequence of the reduced contribution to the activation energy from the ΔS≠-term.

A direct base-to-base disulfide bond is formed by oxidation of two appropriately positioned thionucleoside bases. Cross-link formation between 4-thio-2′-deoxyuridine and 6-thio-2′-deoxyinosine is demonstrated in three sequence contexts (3′ to 3′, 5′ to 5′, and opposed). Disulfide formation occurs in high yield in both duplex and hairpin DNA. Molecular modeling and circular dichroism were used to demonstrate minimal distortion of the constrained B-DNA double helix.

The synthesis of 6-thio-2′-deoxyinosine (d S6 I) in a form suitably protected for solid-phase oligonucleotide synthesis is reported. This thionucleic acid was incorporated in high yield into short oligodeoxynucleotides, and the thiocarbonyl group could be modified by S-alkylation with complete chemoselectivity. The quantitation of incorporation and facile post-synthetic modification was demonstrated by enzymatic digestion and HPLC analysis, and the effect of covalent alkylation was determined by ΔT m measurements of the corresponding duplex oligonucleotides with dC as the complementary base.

Abstract

As the surgical resection of sinonasal malignancies becomes more sophisticated, so must the handling of adjacent organs, most importantly, the eye. The traditional approach to superior nasal vault tumors that have penetrated through the bony orbit is to perform an orbital exenteration. This article provides the authors’ experience in handling the eye in these cases, and their strong bias toward preserving it. This experience, in conjunction with the associated review of functional results and complications, should assist the head and neck surgeon in being able to spare this vital organ when resecting malignancies of this region.

Abstract

Objective: To continue the retrospective analysis reported in 1988 that supported preservation of the eye in the treatment of sinonasal cancers when bony erosion of the orbit was noted on pretreatment radiographic analysis. Design: Using the eye-sparing protocol previously reported, which included preoperative radiotherapy (with or without chemotherapy, depending on tumor size, involvement, or other characteristic) for malignant neoplasms of the superior nasal vault that had eroded the bony orbit, retrospective analysis of the results of therapy for the additional 33 patients treated between 1986 and 1993 was performed. These data were added to those from the original series to provide a total of 74 patients. Forty-one patients (55%) showed bony erosion at initial evaluation and 14 (19%) had periorbital involvement. During surgery, the periorbita was evaluated by frozen section control. If tumor was found, that region of periorbita was resected and, if necessary, replaced with fascia or split-thickness graft. Results: Five patients from the original series had orbital sacrifice. Of the remaining 36 patients, 4 (11%) had recurrent disease involving the orbit, but not at the primary site. Twenty (55%) of the 36 patients had no orbital complications, and only 1 (3%) had a permanent motility disturbance and 4 (13%) developed cataracts after radiotherapy. Conclusion: With the use of preoperative radiotherapy in resection of involved periorbita with frozen section control in tumors of the sinonasal vault that involve the bony orbit, the eye can be spared in most instances without compromising oncologic safety.
