Abstract

Introduction The use of multiple substances heightens the risk of overdose. Multiple substances, including alcohol, are commonly found among people who experience overdose-related mortality. However, the associations between alcohol use and the use of a range of other substances are often not assessed. Therefore, this study examines the associations between drinking patterns (e.g., binge drinking) and other substance use in the U.S., the concurrent use of alcohol and prescription drug misuse, and how other substance use varies by binge-drinking frequency. Methods Past 30–day alcohol and other substance use data from the 2016–2018 National Survey on Drug Use and Health were analyzed in 2020 among 169,486 U.S. respondents aged ≥12 years. Results The prevalence of other substance use ranged from 6.0% (nondrinkers) to 24.1% (binge drinkers). Among people who used substances, 22.2% of binge drinkers reported using substances in 2 additional substance categories. Binge drinking was associated with 4.2 (95% CI=3.9, 4.4) greater adjusted odds of other substance use than nondrinking. Binge drinkers were twice as likely to report concurrent prescription drug misuse while drinking as nonbinge drinkers. The prevalence of substance use increased with binge-drinking frequency. Conclusions Binge drinking was associated with other substance use and concurrent prescription drug misuse while drinking. These findings can guide the implementation of a comprehensive approach to prevent binge drinking, substance misuse, and overdoses. This might include population-level strategies recommended by the Community Preventive Services Task Force to prevent binge drinking (e.g., increasing alcohol taxes and regulating alcohol outlet density).

Abstract

Summary Background Since August, 2019, US public health officials have been investigating a national outbreak of e-cigarette, or vaping, product use-associated lung injury (EVALI). A spectrum of histological patterns consistent with acute to subacute lung injury has been seen in biopsies; however, autopsy findings have not been systematically characterised. We describe the pathological findings in autopsy and biopsy tissues submitted to the US Centers for Disease Control and Prevention (CDC) for the evaluation of suspected EVALI. Methods Between Aug 1, 2019, and Nov 30, 2019, we examined lung biopsy (n=10 individuals) and autopsy (n=13 individuals) tissue samples received by the CDC, submitted by 16 US states, from individuals with: a history of e-cigarette, or vaping, product use; respiratory, gastrointestinal, or constitutional symptoms; and either pulmonary infiltrates or opacities on chest imaging, or sudden death from an undetermined cause. We also reviewed medical records, evaluated histopathology, and performed infectious disease testing when indicated by histopathology and clinical history. Findings 21 cases met surveillance case definitions for EVALI, with a further two cases of clinically suspected EVALI evaluated. All ten lung biopsies showed histological evidence of acute to subacute lung injury, including diffuse alveolar damage or organising pneumonia. These patterns were also seen in nine of 13 (69%) autopsy cases, most frequently diffuse alveolar damage (eight autopsies), but also acute and organising fibrinous pneumonia (one autopsy). Additional pulmonary pathology not necessarily consistent with EVALI was seen in the remaining autopsies, including bronchopneumonia, bronchoaspiration, and chronic interstitial lung disease. Three of the five autopsy cases with no evidence of, or a plausible alternative cause for acute lung injury, had been classified as confirmed or probable EVALI according to surveillance case definitions. Interpretation Acute to subacute lung injury patterns were seen in all ten biopsies and most autopsy lung tissues from individuals with suspected EVALI. Acute to subacute lung injury can have numerous causes; however, if it is identified in an individual with a history of e-cigarette, or vaping, product use, and no alternative cause is apparent, a diagnosis of EVALI should be strongly considered. A review of autopsy tissue pathology in suspected EVALI deaths can also identify alternative diagnoses, which can enhance the specificity of public health surveillance efforts. Abstract

Background Multisystem inflammatory syndrome in children (MIS-C), also known as pediatric inflammatory multisystem syndrome, is a new dangerous childhood disease that is temporally associated with coronavirus disease 2019 (COVID-19). We aimed to describe the typical presentation and outcomes of children diagnosed with this hyperinflammatory condition. Methods We conducted a systematic review to communicate the clinical signs and symptoms, laboratory findings, imaging results, and outcomes of individuals with MIS-C. We searched four medical databases to encompass studies characterizing MIS-C from January 1st, 2020 to July 25th, 2020. Two independent authors screened articles, extracted data, and assessed risk of bias. This review was registered with PROSPERO CRD42020191515. Findings Our search yielded 39 observational studies (n = 662 patients). While 71·0% of children (n = 470) were admitted to the intensive care unit, only 11 deaths (1·7%) were reported. Average length of hospital stay was 7·9 ± 0·6 days. Fever (100%, n = 662), abdominal pain or diarrhea (73·7%, n = 488), and vomiting (68·3%, n = 452) were the most common clinical presentation. Serum inflammatory, coagulative, and cardiac markers were considerably abnormal. Mechanical ventilation and extracorporeal membrane oxygenation were necessary in 22·2% (n = 147) and 4·4% (n = 29) of patients, respectively. An abnormal echocardiograph was observed in 314 of 581 individuals (54·0%) with depressed ejection fraction (45·1%, n = 262 of 581) comprising the most common aberrancy. Interpretation Multisystem inflammatory syndrome is a new pediatric disease associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that is dangerous and potentially lethal. With prompt recognition and medical attention, most children will survive but the long-term outcomes from this condition are presently unknown. Funding Parker B. Abstract

Background Studies summarizing the clinical picture of COVID-19 in children are lacking. This review characterizes clinical symptoms, laboratory, and imaging findings, as well as therapies provided to confirmed pediatric cases of COVID-19. Methods Adhering to PRISMA guidelines, we searched four medical databases (PubMed, LitCovid, Scopus, WHO COVID-19 database) between December 1, 2019 to May 14, 2020 using the keywords “novel coronavirus”, “COVID-19” or “SARS-CoV-2”. We included published or in press peer-reviewed cross-sectional, case series, and case reports providing clinical signs, imaging findings, and/or laboratory results of pediatric patients who were positive for COVID-19. Risk of bias was appraised through the quality assessment tool published by the National Institutes of Health. PROSPERO registration # CRD42020182261. Findings We identified 131 studies across 26 countries comprising 7780 pediatric patients. Although fever (59·1%) and cough (55·9%) were the most frequent symptoms 19·3% of children were asymptomatic. Patchy lesions (21·0%) and ground-glass opacities (32·9%) depicted lung radiograph and computed tomography findings, respectively. Immunocompromised children or those with respiratory/cardiac disease comprised the largest subset of COVID-19 children with underlying medical conditions (152 of 233 individuals). Coinfections were observed in 5.6% of children and abnormal laboratory markers included serum D-dimer, procalcitonin, creatine kinase, and interleukin-6. Seven deaths were reported (0·09%) and 11 children (0·14%) met inclusion for multisystem inflammatory syndrome in children. Interpretation This review provides evidence that children diagnosed with COVID-19 have an overall excellent prognosis. Future longitudinal studies are needed to confirm our findings and better understand which patients are at increased risk for developing severe inflammation and multiorgan failure. Funding Parker B. Abstract

Floods on sediment-rich semiarid rivers often lead to significant geomorphic change, but luminescence dating of sediments deposited by past flood events has hitherto been unable to meaningfully inform on past flood dynamics. This is important because luminescence dating has commonly been applied to flood sediments, but the calculated sediment ages are in almost all cases older than the ages of known floods responsible for sediment deposition. Correctly interpreting these older ages in the context of flood dynamics is therefore a key problem. This study addresses this problem by discussing relationships between luminescence dating, sediments, and reach-scale geomorphology along the semiarid Sabie River, northeastern South Africa. Abstract

Bokoni is a unique archaeological region in South Africa, with settlements ranging from scattered homesteads to large towns. Research into the development of this region has been constrained by the limited number of directly dated sites. Radiocarbon dating has not yet been successful in the region because no charcoal-hosting middens were found, and organic preservation is generally poor due to soil type and related moisture retention. As an interim measure, oral histories were used in combination with architectural and spatial changes to define four occupation phases in Bokoni. However, this approach is not applicable to Phase I sites because these predate oral traditions, which narrates Bokoni history from the 17th century onwards. Consequently, until this research, the age of the start of terrace construction was unknown. Here, we present optically stimulated luminescence (OSL) ages of pottery and sediment samples from two homesteads (KG80 and KG81) in the site Komati Gorge Village 1, which based on settlement configuration, is believed to form part of Phase I. The dates constrain the timing of the initial construction and use of homestead KG80 from 1489CE ± 54 to 1577CE ± 30, and reuse of KG80 by the builders of KG81 from 1682CE ± 20 to 1765CE ± 20. KG81 itself was occupied and reused from 1738CE ± 19 to 1912CE ± 7.

Abstract

The first observation of the decay ηc(2S)→pp¯ is reported using proton-proton collision data corresponding to an integrated luminosity of 3.0fb−1 recorded by the LHCb experiment at centre-of-mass energies of 7 and 8 TeV. The ηc(2S) resonance is produced in the decay B+→[cc¯]K+. The product of branching fractions normalised to that for the J/ψ intermediate state, Rηc(2S), is measured to be Rηc(2S)≡B(B+→ηc(2S)K+)×B(ηc(2S)→pp¯)B(B+→J/ψK+)×B(J/ψ→pp¯)= (1.58±0.33±0.09)×10−2, where the first uncertainty is statistical and the second systematic. No signals for the decays B+→X(3872)(→pp¯)K+ and B+→ψ(3770)(→pp¯)K+ are seen, and the 95\% confidence level upper limits on their relative branching ratios are % found to be RX(3872)

Abstract

Scholars in Criminal Justice have argued that students should have a stand-alone course that focuses on fostering library literacy skills for Criminal Justice students in order to improve their writing, research, and presentation skills. In 2011, a required information literacy course was implemented in one Criminal Justice program to meet this need. Building on the existing literature, the current study is the first to explore student perceptions of a required information literacy course. The results offer valuable information on how to strengthen the course moving forward and can provide useful suggestions to other Criminal Justice programs thinking about instituting a library literacy course in their curriculum.

Abstract

The geomorphic effects of river floods in various climatic settings worldwide have been extensively described, but there is little information on sedimentary processes during and immediately following flood peaks. Here we reconstruct sedimentary processes and patterns of the 2012 flood on the Sabie River in Kruger National Park, South Africa, from the preserved geomorphic and sedimentary signature of this event. Based on a combination of geomorphic and sedimentary evidence, the maximal and waning flow stages of the flood event are identified, shown by cut and fill and fining up sequences within aggradational bars, and organic horizons, which may be buried, that correspond to the period of flood peak recession. The presence of organic horizons suggests a strongly seasonal flow regime typical of semi-arid South African rivers. It also provides an exemplar of a flood event stratigraphy that is usually poorly preserved in the geologic record.

Abstract

Rising Star Cave in the Cradle of Humankind, South Africa, contains one of the richest hominin-bearing deposits in the world, and is the type locality for the Homo naledi fossils. This paper provides a stratigraphic and geochronological framework, within which published and future fossil finds from Rising Star Cave can be placed. Detailed mapping of flowstone horizons combined with new age constraints based on both U-Th disequilibrium and 234U/238U dates and one new OSL date help define seven periods of flowstone formation that punctuate episodes of clastic sedimentation and erosion. Clastic sediments entered the cave through an opening in the roof of the Postbox Chamber from about 600 ka onward, until the opening was choked by coarse breccia blocks, probably sometime after 180 ka. Depositional and erosional events changed the internal morphology of the cave chambers over time, and thereby changed the access route into the Dinaledi Chamber where the bulk of the H. naledi fossils were found. Periods of pervasive flowstone formation at all levels of the cave occurred at >600 ka, ~500–400 ka, ~225–190 ka and ~110-90 ka. Additional periods of localised flowstone growth restricted to individual chambers (or parts thereof) occurred at ~300 ka, ~160 ka, ~70 ka, ~50 ka, ~30 ka, and ~10 ka. Flowstone horizons bracket sedimentary units that include a variety of sediment types that changed with time. The oldest flowstones overlie lithified mud clast breccias (LMCB), which were partly eroded before they were covered by externally derived laminated orange sands (LOS) and 500–400 ka flowstones. These flowstones and sands were removed between 290 ka and 225 ka, with sediment transported to deeper parts of the cave via erosion channels characterised by massive orange sands (MOS). In this period of time, the access route into the Dinaledi Chamber may have further changed due to the collapse of the Dragon's Back block. Deposition of laminated orange-red mud (LORM) from suspension occurred between 225 and 190 ka and temporally overlaps with widespread flowstone formation around ~225 ka and ~200 ka. The LORM deposits were largely removed from the upper chambers by ~110 ka, before the youngest group of flowstones formed in the cave; some of which are still growing today. The U-Th ages from Rising Star Cave, combined with other dating constraints reveal age clusters of flowstone formation, which coincided with warmer interglacial or interstadial periods. The patterns recognised in Rising Star Cave overlap with periods of flowstone formation recognised in nearby caves in the Cradle of Humankind, thereby confirming a regional climatic control on flowstone growth in caves during the past 500 ky. The new ages further constrain the minimum age of H. naledi to ~241 ka. Thus, H. naledi entered the cave between 241 ka and 335 ka, during a glacial period, at which time clastic sediments inside the cave were undergoing erosion. H. naledi would probably have entered the cave through an access point in the roof of the Postbox Chamber and made its way along a SW trending fracture towards the Dragon's Back and Dinaledi Chambers.

Abstract

Aims: To examine the effect of lifestyle (diet and physical activity) interventions on the prevalence of GDM, considering the method of GDM ascertainment and its association with early pregnancy characteristics and maternal and neonatal outcomes in the LIFE-Moms consortium. Methods: LIFE-Moms evaluated the effects of lifestyle interventions to optimize gestational weight gain in 1148 pregnant women with BMI ≥ 25 kg/m2 and without known diabetes at enrollment, compared with standard care. GDM was assessed between 24 and 31-weeks gestation by a 2-hour, 75-gram OGTT or by local clinical practice standards. Abstract

Three pedons on the alluvial terraces of the Sabie-Sand River Basin within Kruger National Park, South Africa, were studied to improve our understanding of recent environmental changes, and assess degree of chemical weathering and pedogenesis in the area using geochemical and micromorphology proxies. Particle-size distributions were obtained using Malvern Mastersizer; soil geochemistry was determined by XRF and thin sections by routine laboratory procedures. The soils are predominantly sandy (> 94% sand in all samples). The mean phi-values of the soils had little variation suggesting that reworking of sediments upwards in individual profiles produced a more uniform pedogenesis rather than coming from different physical sources. Calcification is the dominant pedogenic process in these alluvial soils. The Chemical Index of Alteration (CIA) proved a more suitable index than Chemical Index of Weathering (CIW) for evaluating weathering in the terraces. The micromass and b-fabrics are mostly granostriated and partly brown mosaic speckled. MISECA values for the degree of soil development range from 4 to 9, which mean weakly to moderately-developed soils. Coarse secondary calcite nodules and coatings are responsible for cementation as observed in pedon 2, which suggests calcium carbonate precipitation from periodical flooding and evaporating groundwater events. The features and diagnostic properties of the soils on the alluvial terraces along the Sabie-Sand River provide evidence for land surface impacts of recent environmental changes in this internationally important conservation area. Precise dating of calcium carbonate precipitates is, however, needed to put the observed evidence into a wider geochronological perspective.

Abstract

A search for the decays of the $B_c^+$ meson to $p\overline p\pi^+$ is performed for the first time using a data sample corresponding to an integrated luminosity of 3.0 $\mathrm{fb}^{-1}$ collected by the LHCb experiment in $pp$ collisions at centre-of-mass energies of $7$ and $8$ TeV. No signal is found and an upper limit, at 95\% confidence level, is set, $\frac{f_c}{f_u}\times\mathcal{B}(B_c^+\to p\overline p\pi^+)

Abstract

Luminescence dating of sediment requires the sand grains to have been exposed to sunlight prior to their most recent burial. Under fluvial transport, the amount of sunlight exposure may not always be sufficient to reset the luminescence signal, a phenomenon known as ‘partial bleaching'. The extent of bleaching is dependent on a combination of geomorphic, sedimentological and fluvial processes. If bleaching can be quantified, and the relationship with these processes understood, it could potentially be used as a new environmental proxy for changes in the dynamics of river systems. Here, we use a recently developed statistical model to evaluate the extent of bleaching, by inferring the proportion of well-bleached grains in the small-aliquot population. We sampled low-flow and flood deposits at a single site on the River Sabie, South Africa. We show that the low-flow sediment is almost perfectly bleached (>80% of grains well bleached), while sediment at flood elevations is partially bleached (20–70 % of grains well bleached). The degree of bleaching may show a relationship with flood magnitude as defined by elevation above normal river level, and we speculate on the causes of variability in bleaching between flood samples.

Abstract

A search for time-integrated CP violation in the Cabibbo-suppressed decay D-0 -> pi(+)pi(-)pi(+)pi(-) is performed using an unbinned, model-independent technique known as the energy test. This is the first application of the energy test in four-body decays. The search is performed for P-even CP asymmetries and, for the first time, is extended to probe the P-odd case. Using proton proton collision data corresponding to an integrated luminosity of 3.0 fb(-1) collected by the LHCb detector at centre-of-mass energies of root s = 7 TeV and 8 TeV, the world's best sensitivity to CP violation in this decay is obtained. The data are found to be consistent with the hypothesis of CP symmetry with a p-value of (4.6 +/- 0.5)% in the P-even case, and marginally consistent with a p-value of (0.6 +/- 0.2)% in the P-odd case, corresponding to a significance for CP non -conservation of 2.7 standard deviations. Abstract

Here we report on a Holocene inter-dune site close to Walvis Bay (Namibia) which contains exceptionally well-preserved children’s footprints. The footprint surface is dated using Optically Stimulated Luminescence (OSL) methods to approximately 1.5 ka. These dates are compared to those obtained at nearby footprint sites and used to verify a model of diachronous footprint surfaces and also add to the archaeological data available for the communities that occupied these near-coastal areas during the Holocene. This model of diachronous footprint surfaces has implications for other soft-sediment footprint sites such as the 1.5 Ma old footprints at Ileret (Kenya). The distribution of both human and animal tracks, is consistent with the passage of small flock of small ungulates (probably sheep/goats) followed by a group of approximately 9 ± 2 individuals (children or young adults). Age estimates from the tracks suggest that some of the individuals may have been as young as five years old. Variation in track topology across this sedimentologically uniform surface is explained in terms of variations in gait and weight/stature of the individual print makers and is used to corroborate a model of footprint morphology developed at a nearby site. The significance of the site within the literature on human footprints lies in the quality of the track preservation, their topological variability despite a potentially uniform substrate, and the small size of the tracks, and therefore the inferred young age of the track-makers. The site provides an emotive insight into the life of the track-makers.

Abstract

In 2016, 19% of HIV diagnoses were in women. About 40% of HIV infections in women aged 18-34 years have been attributed to anal sex, suggesting that women who report high risk behaviors such as anal sex might benefit from HIV testing and prevention with preexposure prophylaxis (PrEP). In this analysis, we estimated HIV testing rates among women who reported anal sex.We analyzed data from the 2011-2015 National Survey of Family Growth to estimate the proportion of sexually active, nonpregnant US women aged 15-44 years who had an HIV test within the past year, stratified by those who reported anal sex and other risk factors, including ≥2 sexual partners, condomless sex with a new partner or multiple partners, gonorrhea in the past year, or any history of syphilis.Overall, 7.9 million of 42.4 million sexually active, nonpregnant US women (18.7%) reported an HIV test within the past year. Among 42.4 million sexually active women, 9.0 million (20.1%) reported they had anal sex in the past year. Among these 9.0 million women, 19.2% reported that their providers asked about their type of intercourse, and 20.1% reported an HIV test within the past year. Overall, HIV testing was higher among women who reported anal sex and reported that their providers asked about type of sex than those whose provider did not ask (37.8% vs 15.9%; P.001). HIV testing in the past year was higher for women with other risk behaviors compared with anal sex, ranging from 35.8% to 47.2%.Overall, HIV testing rates within the past year were low among women with sexual behaviors that increase their risk of acquiring HIV and especially low among those who reported anal sex. Early detection and treatment of HIV, and HIV prevention with PrEP, are effective health services that protect women's health and well-being but that can be offered only based on HIV testing results. Women's health care providers are uniquely poised to assess risk for acquiring HIV, including taking a sexual history that asks about anal sex, and performing HIV testing to identify women who need HIV treatment or might benefit from PrEP.

Abstract

In July 2000 a statewide infection control survey was mailed out to 483 long-term care facilities (LTCFs) in Queensland, Australia. The survey aimed to identify the presence and composition of infection control programmes in LTCFs and to investigate their relationship to the process elements of infection control programmes. A response rate of 43 per cent (n=206) was achieved. The survey found that almost all LTCFs (97 per cent) had an infection control programme in place. A majority (70 per cent) had an infection control committee or another committee with responsibility for infection control. The time allocated to infection control was only 1-2 hours per week in the majority (56 per cent) of facilities, and there was no correlation between this time allocation and bed numbers. Almost all (99 per cent) facilities provided infection control education to their staff. Most facilities conducted surveillance for urinary tract infection, respiratory tract infection or wound and skin infection. Surveillance data was used for staff feedback, education and/or accreditation in 95 per cent of facilities. Further investigation is warranted in order to identify the amount of time required to fulfil infection control responsibilities in LTCFs. This study also suggests that minimum standard definitions for surveillance are required for the LTCF in order to facilitate data collection and analysis, benchmarking ability and possible aggregation of information.

Abstract

Objective Prior analysis in the Open vs Endovascular Repair Veterans Affairs (VA) Cooperative Study (CSP #498) demonstrated that survival, quality of life, and total health care costs are not significantly different between the open and endovascular methods of repair of abdominal aortic aneurysm. The device is a major cost of this method of repair, and the objective of this study was to evaluate the costs of the device, abdominal aortic aneurysm repair, and total health care costs when different endograft systems are selected for the endovascular repair (EVR). Within each selected system, EVR costs are compared with open repair costs. Methods The study randomized 881 patients to open (n = 437) or EVR (n = 444). Device selection was recorded before randomization; therefore, open repair controls were matched to each device cohort. Data were excluded for two low-volume devices, implanted in only 13 individuals, leaving 423 control and 431 endovascular patients: 166 Zenith (Cook Medical, Bloomington, Ind), 177 Excluder (W. L. Gore & Associates, Flagstaff, Ariz), and 88 AneuRx (Medtronic, Minneapolis, Minn). Mean device, hospitalization, and total health care costs from randomization to 2 years were compared. Health care utilization data were obtained from patients and national VA and Medicare data sources. VA costs were determined using methods previously developed by the VA Health Economics Resource Center. Non-VA costs were obtained from Medicare claims data and billing data from the patient's health care providers. Results Implant costs were 38% of initial hospitalization costs. Mean device (range, $13,600-$14,400), initial hospitalization (range, $34,800-$38,900), and total health care costs at 2 years in the endovascular (range, $72,400-$78,200) and open repair groups (range, $75,600-$82,100) were not significantly different among device systems. Differences between endovascular and corresponding open repair cohorts showed lower mean costs for EVR (range, $3200-$8300), but these were not statistically different. Conclusions The implant costs of endovascular aneurysm repair are substantial. When evaluating total health care system expenditures, there is large individual variability in costs, and there is no significant difference at 2 years among systems or when an individual system is compared with open repair.

It is suggested that teaching Women's Studies often differs from teaching more conventional disciplines, but that this offers a chance for academic innovation.

Abstract

Antibiotic sensitivities of 15302 organisms of common pathogenic species isolated in one hospital pathology department in 1971 and 1974 have been studied. Resistance to cephaloridine did not change materially. The proportion of strains resistant to ampicillin did, however, increase, and this is attributed to the widespread therapeutic use of the antibiotic. Cephaloridine, on the other hand, was largely used in the hospital as a single-dose, intra-incisional prophylactic against surgical wound sepsis.

Abstract

Simone de Beauvoir occupies, and deservedly so, a central place in the history of feminism. The Second Sex, published in 1949, is a classic study of the status of women and the causes of their subordination in all aspects of social tile. Her other works, which include novels, essays on existential philosophy, a four-volume autobiography and a lengthy study of old age, demonstrate a capacity for intellectual breadth (and, one must add a quite monumental talent for documentation) which is comparable to that of her life long companion, Jean-Paul Sartre. Yet whilst any essay on de Beauvoir must note her considerable intellectual power and range, it is also important to examine her work more critically than has generally been the case. Accolades, particularly from feminists, have been so generously heaped on her work that some of its shortcomings have been obscured. I would like to suggest here that whilst de Beauvoir claims that much of her work is concerned with the overall condition of women, she turns away from many of the issues which are central to women's lives and in particular accords very little place in her epistemology to areas of human experience which are not immediately amenable to rational understanding. Thus in this paper I shall argue that a major weakness in de Beauvoir's work is a rejection of many of the problems which women (and indeed men) face and a failure to acknowledge that the actions of both sexes are often motivated by needs and desires which, although frequently explained and rationalized from the conscious mind, do not always derive from it. • In a relatively short space it is impossible to do more than summarize and indicate the work of a prolific writer whose career has now spanned almost 40 years° De Beauvoir was born in 1908, the elder daughter of a lawyer, whose fortunes became progressively worse during his daughter's childhood and adolescence. A limited secondary education at a Catholic girls' school was followed by the study of philosophy at the Sorbonne, where at the age of 21 she received her degree and almost immediately afterwards the agrdgation, the coveted qualification allowing her to teach in lyc6es and universities. De Beauvoir's earliest published works did not, however, follow rapidly upon her brilliant academic career: she graduated from the Sorbonne in 1929, but her first novel (She Came to Stay) did not appear until 1943. It was rapidly followed by other works: two more novels (The Blood of Others, published in 1945 and All Men are Mortal, published in 1947) and two essays on existential philosophy (Pyrrhus et Cindas and Pour Une Morale de 'l'Ambiguitg)o But t~e two works which brought her lasting, and international, fame were The Second Sex and The Mandarins (the latter published in 1954 and for which she won the Prix Goncourt). Abstract

Jaundice often complicates recovery after surgery. We have studied 180 patients undergoing 218 major operations and found an incidence of 3.7% severe jaundice and 16.5% mild jaundice. The causes of the jaundice were diverse, but the most important was a bilirubin overload (primarily from blood transfusion) which could not be excreted effectively by a liver whose cellular metabolism was disturbed. We found no evidence of a relationship between halothane and liver damage.

Abstract

It is generally agreed that rheumatic fever is more prevalent in temperate than in tropical zones. It has also been stated that the salutary effect of the tropics on rheumatic patients may be due to the lower incidence of upper respiratory infections in regions enjoying a tropical climate. The present study confirms the rarity of rheumatic disease in the tropical zones of Mexico, but also shows that the rate of streptococcus carriers is as high in the tropics as in temperate regions. It also gives data showing that antistreptolysin titers are higher in the sera from persons inhabiting the tropics, thus pointing to the greater prevalence of streptococcal infections in tropical zones. The above considerations suggest that the tropical climate affects the response of the host before the rheumatogenic agent.
