Sedation is necessary in the management of critically ill patients, both to alleviate suffering and to cure patients with diseases that require admission to the intensive care unit. Such sedation should be appropriate to the patient needs at each timepoint during clinical evolution, and neither too low (undersedation) nor too high (oversedation). Adequate sedation influences patient comfort, safety, survival, subsequent quality of life, bed rotation of critical care units and costs. Undersedation is detected and quickly corrected. In contrast, oversedation is silent and difficult to prevent in the absence of management guidelines, collective awareness and teamwork. The Zero Oversedation Project of the Sedation, Analgesia and Delirium Working Group of the Spanish Society of Intensive and Critical Care Medicine and Coronary Units aims to offer a practical teaching and collective awareness tool for ensuring patient comfort, safety and management with a view to optimizing the clinical outcomes and minimizing the deleterious effects of excessive sedation. The tool is based on a package of measures that include monitoring pain, analgesia, agitation, sedation, delirium and neuromuscular block, keeping patients pain-free, performing dynamic sedation according to clinical objectives, agreeing upon the multidisciplinary protocol to be followed, and avoiding deep sedation where not clinically indicated.

Background and purpose Definitive radiation therapy (RT) with or without chemotherapy has become the standard treatment for non-metastatic unresectable non-small cell lung cancer (NSCLC). However, treatment outcomes can differ substantially and patients’ genetic background could play a crucial role. Potential associations between single-nucleotide polymorphisms (SNP) in Heat shock protein beta-1 (HSPB1) and survival have been reported in prior single-institution retrospective reports. Materials and methods The current assay aims to validate such connection in a prospective multicenter study in a European cohort including 181 NSCLC patients. Median follow-up time for all patients was 13 months (range, 3–57 months). Results The results obtained show an association between the rs2868371 GG genotype and better overall survival (HR: 0.35; 95%CI: 0.13–0.96; p = 0.042) in multivariate analysis. Two-year overall survival rate was 72% for patients carrying the rs2868371 GG genotype versus 36% for those patients harboring the rs2868371 CC/CG genotypes (p = 0.013). Additionally, the rs2868371 GG genotype was found to be associated with better disease-free survival in the multivariate analysis (HR: 0.36; 95%CI: 0.13–0.99; p = 0.048). In silico analysis of the potential functional SNP suggested significant difference in the affinity of the Glucocorticoid Receptor binding site between alternative allelic variants, confirmed by chromatin immunoprecipitation analysis displaying stronger affinity for the risk allele (C). Furthermore, our findings indicate that the rs2868371 influences (mRNA) HSPB1 expression, offering insight into the regulation of HSPB1 transcription. Conclusion The functional HSPB1 rs2868371 promoter variant may affect lung cancer survival by regulation of HSPB1 expression levels through glucocorticoid receptor interaction.

Lampreys are jawless vertebrates, the most basal group of extant vertebrates. This phylogenetic position makes them invaluable models in comparative studies of the vertebrate central nervous system. Lampreys have been used as vertebrate models to study the neuronal circuits underlying locomotion control and axonal regeneration after spinal cord injury. Inhibitory inputs are key elements in the networks controlling locomotor behaviour, but very little is known about the descending inhibitory projections in lampreys. The aim of this study was to investigate the presence of brain-spinal descending inhibitory pathways in larval stages of the sea lamprey Petromyzon marinus by means of tract-tracing with neurobiotin, combined with immunofluorescence triple-labeling methods. Neurobiotin was applied in the rostral spinal cord at the level of the third gill, and inhibitory populations were identified by the use of cocktails of antibodies raised against glycine and GABA. Glycine-immunoreactive (-ir) neurons that project to the spinal cord were observed in three rhombencephalic reticular nuclei: anterior, middle and posterior. Spinal-projecting GABA-ir neurons were observed in the anterior and posterior reticular nuclei. Double glycine-ir/GABA-ir spinal cord-projecting neurons were only observed in the posterior reticular nucleus, and most glycine-ir neurons did not display GABA immunoreactivity. The present results reveal the existence of inhibitory descending projections from brainstem reticular neurons to the spinal cord, which were analyzed in comparative and functional contexts. Further studies should investigate which spinal cord circuits are affected by these descending inhibitory projections.

11. Análisis de los estilos de vida de la mujer climatérica: un componente esencial en el estudio de las necesidades educativas

Resumen Objetivo Conocer el comportamiento de los factores conductuales en las usuarias potenciales del programa de Atencion a la mujer climaterica de la Zona Basica de Salud Molino de la Vega, de Huelva. Metodo Estudio descriptivo sobre un total de 70 mujeres (de entre 40 y 60 anos) adscritas a dicha zona. Se aplico un cuestionario de 146 preguntas, agrupadas en 5 dimensiones, previa prueba de jueces y pilotaje. Analisis estadistico: frecuencias absolutas y relativas, aplicacion del test de independencia, analisis de varianza y test de Newman-Keuls. Resultados a) Comportamiento alimentario: el 65% de las encuestadas tomaba leche 2 o mas veces al dia; el 90% mantenia una ingesta moderada de fibra; tomaban carne de ave varias veces a la semana el 83%; cerdo, el 55%; huevos, el 78%; pescado azul, el 75%; hidratos de carbono de absorcion lenta, el 76%. El 86,7% utilizaba el aceite de oliva. b) Actividad fisica: el 40% dijo realizar ejercicio fisico raramente o nunca. c) Conducta respecto al sueno: el 78,3% se despertaba a menudo. d) Conductas psicosociales: el 66% de las mujeres solo salia con su pareja o amistades, de vez en cuando o nunca; en sus relaciones sexuales, el 64,5% no tomaba la iniciativa. e) Otras conductas: el 83,3% no habia fumado nunca, el 18,4% consumia hipnoticos a menudo, el 25% tranquilizantes, y el 48,3% analgesicos. Conclusiones Aunque en todas las dimensiones hay aspectos que deben trabajarse en la intervencion educativa, los comportamientos relacionados con la actividad fisica y los aspectos psicosociales son los puntos donde el repertorio conductual se muestra menos saludable.
