Abstract

Objectives The aim of this AFIRE (Atrial Fibrillation and Ischemic Events With Rivaroxaban in Patients With Stable Coronary Artery Disease) trial subgroup analysis was to examine rivaroxaban monotherapy benefits and their relation to the time between stenting and enrollment among patients after coronary stenting. Background Of 2,215 patients with atrial fibrillation and stable coronary artery disease in the AFIRE trial, rivaroxaban monotherapy was noninferior to rivaroxaban plus antiplatelet therapy (combination therapy) in terms of efficacy and superior for safety endpoints. However, thrombotic risk after antiplatelet therapy cessation remained a concern among 1,444 patients who had undergone coronary stenting >1 year before enrollment. Methods The benefits of rivaroxaban monotherapy in coronary stenting subgroups were assessed for efficacy (a composite of stroke, systemic embolism, myocardial infarction, unstable angina requiring revascularization, or death of any cause), safety (major bleeding defined according to International Society on Thrombosis and Haemostasis criteria), ischemic endpoints, net adverse clinical event, and time between stenting and enrollment. Results Efficacy and safety endpoints for monotherapy were superior to combination therapy, with HRs of 0.70 for efficacy (95% CI: 0.50-0.98; P = 0.036) and 0.55 for safety (95% CI: 0.33-0.92; P = 0.019). For ischemic endpoints, the HR was 0.82 (95% CI: 0.58-1.15; P = 0.240). The HR became smaller with longer time between stenting and enrollment (efficacy, P for interaction = 0.158; safety, P = 0.097). Conclusions In patients with atrial fibrillation after coronary stenting, the benefits of rivaroxaban monotherapy for efficacy and safety endpoints were consistent with those in the whole AFIRE trial population. The benefits became apparent with longer time between stenting and enrollment. Abstract

ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system, to reach the desired performances during its various phases of operation. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities.In order to enable such development, a Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, called SPIDER and MITICA. SPIDER is dedicated developing and char-acterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accel-erator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full perfor-mances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign, in particular events/issues occurred during operation, which led to the identification of improvement opportunities/necessities (e.g. RF discharges, local burns, water leaks, other damages, configuration/design upgrades to maximize chances/margin to quest target parameters).Parallelly, developments on MITICA, the full-scale prototype of the ITER Neutral Beam Injector (NBI) featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of the beam source, accel-erator and the beam line components, while power supplies and auxiliary plants, already installed, are under final testing and commissioning.Integration, commissioning and tests of the 1 MV power supplies are essential for this first-of-kind system, unparalleled both in research and industry field. 1.2 MV dc insulating tests of high voltage components were successfully completed. The integrated test to confirm 1 MV output by combining invertor systems, DC gener-ators and transmission lines extracted errors/accidents in some components. To realize a concrete system for ITER, said events have been addressed and solutions for the repair and the improvement of the system were developed.Hence, NBTF is emerging as a necessary facility, due to the large gap with existing injectors, effectively dedicated to identify issues and find solutions to enable successful ITER NBI operations in a time bound fashion. The lessons learned during the implementation on NBTF and future perspectives are here discussed.

Abstract

Objective Describe who is able to return to driving (RTD) after moderate-to-severe traumatic brain injury (TBI), when this occurs, who maintains that activity, and the association with outcome. Design Cross-sectional descriptive study. Setting Eight follow-up sites of the TBI Model Systems (TBIMS) program. Participants 618 participants enrolled in the TBIMS and 88 caregivers (N=706). Interventions Not applicable. Main Outcome Measures A survey was completed from 1-30 years postinjury focusing on RTD. Descriptors included demographic information, injury severity, and current employment status. Outcome was assessed at the time of the interview, including depression, quality of life, functional status, and community participation. Results Of 706 respondents, 78% (N = 552) RTD, but 14% (N = 77) of these did not maintain that activity. Of those who RTD, 43% (N = 192) did so within 6 months of the injury and 92% did so within 24 months postinjury. The percentage of people driving after TBI did not differ significantly based on age at time of injury or follow-up. There were significant differences between drivers and nondrivers with respect to severity of injury, seizures, race, education, employment, rural vs urban setting, marital status, and family income. We performed a multivariate logistic regression to examine the association between driving status and demographic variables, adjusting for other variables in the model. The strongest associations were with current employment, family income, race, seizures, and severity of injury. Driving was associated with greater community participation, better functional outcomes, fewer symptoms of depression, and greater life satisfaction. Conclusions Over a span of 30 years, three-quarters of people experiencing moderate-to-severe TBI return to driving a personal vehicle, although not everyone maintains this activity. Employment, race, family income, and seizures are strongly associated with RTD.

Abstract

Background Human epidermal growth factor 2 (HER2) is a potential driver oncogene. Although HER2-targeted precision therapy has been tested in non-small cell lung cancer (NSCLC), the demographic characteristics of HER2-positive NSCLC have not been systematically defined. Methods Patients with pathologically confirmed stage IIIB/IV or recurrent NSCLC, Eastern Cooperative Oncology Group performance status 0 to 2, were prospectively registered. HER2 immunohistochemistry (IHC) and fluorescence in situ hybridization (FISH) assays were performed to screen patients. HER2 mutations were identified by using direct gene sequencing. The aim of this study was to clarify the frequency, characteristics, and outcome of HER2-positive NSCLC. HER2 was defined as positive if the tumor harbored IHC3+, IHC2+/FISH+, or exon 20 insertion mutations. Results Of the 1,126 tumors screened, 34 (3.0%) were IHC3+, and 34 (3.0%) were IHC2+/FISH+. Among the 724 epidermal growth factor receptor wild-type tumors, 21 (2.9%) were HER2-mutant tumors, including A775-G776insYVMA (n = 15). Interestingly, the IHC3+ tumors and mutant tumors were entirely exclusive. Female patients had HER2-mutant tumors more frequently, whereas both IHC3+ and IHC2+/FISH+ tumors were detected more often in male subjects and smokers. Patients with an HER2-aberrant tumor had a significantly worse prognosis than those with epidermal growth factor receptor-positive and anaplastic lymphoma kinase-positive tumors, possibly due to the low proportion that received HER2-targeted therapies (n = 15 [26%]) and low response rates of 0% and 14% in patients with HER2-overexpressing and HER2-mutant tumors, respectively. Conclusions This prospective large-scale cohort study is the first to show comprehensively the frequency and clinical demographic characteristics of those with HER2-positive advanced lung tumors in detail, providing critical historical data for future drug development against HER2-positive NSCLC. Future treatment strategies would be developed stratified according to the types of HER2 aberrations. Abstract

Indigestible polysaccharides, such as dietary fibers, benefit the host by improving the intestinal environment. Short-chain fatty acids (SCFAs) produced by gut microbial fermentation from dietary fibers exert various physiological effects. The bacterial polysaccharide curdlan benefits the host intestinal environment, although its effect on energy metabolism and SCFA production remains unclear. Hence, this study aimed to elucidate the effect of curdlan intake on gut microbial profiles, SCFA production, and energy metabolism in a high-fat diet (HFD)-induced obese mouse model. Gut microbial composition of fecal samples from curdlan-supplemented HFD-fed mice indicated an elevated abundance of Bacteroidetes, whereas a reduced abundance of Firmicutes was noted at the phylum level compared with that in cellulose-supplemented HFD-fed mice. Moreover, curdlan supplementation resulted in an abundance of the family Bacteroidales S24-7 and Erysipelotrichaceae, and a reduction in Deferribacteres in the feces. Furthermore, curdlan supplementation elevated fecal SCFA levels, particularly butyrate. Although body weight and fat mass were not affected by curdlan supplementation in HFD-induced obese mice, HFD-induced hyperglycemia was significantly suppressed with an increase in plasma insulin and incretin GLP-1 levels. Curdlan supplementation elevated fecal bile acid and SCFA production, improved host metabolic functions by altering the gut microbial composition in mice.

Abstract

The suspension feeding benthic bivalve surf clam, Pseudocardium sachalinense , which feeds on phytoplankton , benthic microalgae, and detritus in coastal waters, is one of the most important marine resources in Hamanaka, Hokkaido Prefecture, Japan. We measured stable carbon and nitrogen isotope ratios (δ 13C and δ15 N) and carbon‑nitrogen (C/N) ratios of surf clam adductor muscle and stomach contents, oceanic particulate organic matter (POM), oceanic sediments, riverine POM, and soil, as well as physical and chemical parameters such as temperature, salinity, chlorophyll-a, and nutrients in Hamanaka from April through September 2015. We also examined the food sources that support the surf clam, the possible connection between wetlands and coastal waters, and the spatial and seasonal variation. Our results showed that the isotope and C/N values of surf clams and oceanic organic matter were significantly different from those of riverine organic matter, and that there were no significant differences among different sites of coastal areas. This means that land-derived organic matter is not a direct food source for the surf clam and does not affect the coastal environment. In addition, benthic microalgae and epiphytes are considered to be the major food sources for the surf clam in all sampling seasons, since they have isotope and C/N values similar to those of the surf clam. The overall results, also with reference to previous relevant studies, imply the importance of conservation of wetlands which buffer excessive inflow of land-derived organic matter that may harm surf clams and the coastal environment.

Abstract

There are several key components to the meaningful and comprehensive assessment of patients with acquired brain injuries with respect to management of the upper motor neuron syndrome. Type of brain injury, trajectory of recovery, relevant concomitant complications, development of appropriate goals, and an understanding of resources available for patients are all factors to assess when developing a treatment plan. Using appropriate outcome measures will help monitor the efficacy of interventions and guide ongoing management of spasticity.

Abstract

OBJECTIVE To explore the feasibility and safety of a single-lead, fully implantable peripheral nerve stimulation system for the treatment of chronic shoulder pain in stroke survivors. PARTICIPANTS Participants with moderate to severe shoulder pain not responsive to conservative therapies for six months. METHODS During the trial phase, which included a blinded sham introductory period, a percutaneous single-lead peripheral nerve stimulation system was implanted to stimulate the axillary nerve of the affected shoulder. After a three-week successful trial, participants received an implantable pulse generator with an electrode placed to stimulate the axillary nerve of the affected shoulder. Outcomes included pain, pain interference, pain-free external rotation range of motion, quality of life, and safety. Participants were followed for 24 months. RESULTS Twenty-eight participants underwent trial stimulation and five participants received an implantable pulse generator. The participants who received the implantable generator experienced an improvement in pain severity (p = 0.0002). All five participants experienced a 50% or greater pain reduction at 6 and 12 months, and four experienced at least a 50% reduction at 24 months. There was an improvement in pain interference (p

Abstract

Purpose There are limited data reporting outcomes of children with trisomy 21 undergoing heart transplantation (HTx). This project reports the waitlist and post-HTx outcomes of children with trisomy 21. Methods This is a retrospective cohort study of children with trisomy 21 listed for or who underwent HTx at 11 centers within North America and internationally. Demographic, medical, surgical, and post-HTx outcome data were collected and described. Results Twenty-one patients with trisomy 21 were listed for HTx from 1992 through 2019 (median age 8.5y, 43% male, 52% white). High-risk or failed repair of congenital heart disease was the indication for transplant in 16 (76%) patients with atrioventricular septal defect being the most common (N=12). Other diagnoses included dilated cardiomyopathy in 4 patients (secondary to anthracyclines in 2) and myocarditis in 1 patient. A total of 18 (86%) patients survived to transplant. Of this group, support at the time of transplant included inotropes (N=10, 56%), ventricular assist device (N=7, 39%), and mechanical ventilation (N=2, 11%). No patients required post-operative extracorporeal membrane oxygenation or dialysis. All centers reported the use of standard immunosuppression without alterations secondary to trisomy 21. All transplanted patients survived to hospital discharge with a median post-HTx length of stay of 19d. With a median follow-up of 2.9y, 16 (89%) patients were alive, 5 (28%) had rejection within the 1st year post-HTx, 2 (11%) developed post-transplant lymphoproliferative disorder (PTLD), and 7 (39%) were hospitalized for infection within the 1st year post-HTx. For the two post-HTx mortalities, the causes of death were 1) coronary allograft vasculopathy at 22y post-HTx and 2) multiple episodes of rejection and PTLD at 1.5y post-HTx (Figure). Conclusion Waitlist and intermediate post-HTx outcomes are acceptable in children with trisomy 21. The presence of trisomy 21 should not be an absolute contraindication to heart transplantation in the appropriate clinical setting.

Abstract

Aquatic pollution is known to reduce biodiversity and disrupt wildlife populations. Mercury (Hg) pollution is pervasive worldwide, contributing to the degradation of ecosystems, and causing deleterious effects to exposed organisms and populations. Batoids have a life history linked to the benthic substrate of coastal areas and occupy upper trophic levels. These combined with large bodies, long lifespan, and slow growth rates contributes to increased uptake and accumulation of Hg. However, mechanisms governing these associations are not well understood. Using multiple biogeochemical tracers (δ13C, δ15N and total Hg), we describe trophic interactions of three sympatric batoid species inhabiting an urbanized estuary and identify diet sources that contribute to Hg accumulation and trophic position among these mesopredators. We also use the Bat-ray (Myliobatis californica) as a model species, to compare diet composition, trophic position, and isotopic niche between two populations in two Californian bays. Trophic plasticity in M. californica was characterized by isotopic niche, diet proportions, and trophic position estimates using Bayesian statistics. Abstract

The temporal distribution of 236U in the Northwest Pacific Ocean was reconstructed through measurements of the annual changes in 236U/238U atom ratios in a coral core collected from Kume Island located within the Kuroshio Current region. In total, 74 years (1940–2014) of temporal trend of 236U/238U atom ratios were reconstructed using Accelerator Mass Spectrometry (AMS). The annually resolved 236U/238U ratios showed a strong increase from 1952, and two prominent peaks were found in the annual bands for 1954 (11.0 ± 1.2 × 10− 9) and 1958 (8.55 ± 1.17 × 10− 9); also an enhanced peak was observed for the band of 1963 (3.51 ± 0.20 × 10− 9). These peaks coincided with the occurrence of large nuclear tests in the Pacific Proving Grounds and/or the highest global fallout rate, suggesting a clear influence of these nuclear events on 236U/238U atom ratios. Furthermore, the coral results showed some disagreement with the annual changes for 236U in East Asian corals collected at the entrance to the Sea of Japan. Based on these results, the mixing ratios of the surface seawater currents in the Northwest Pacific Ocean have been estimated.

Abstract

Moderate to severe traumatic brain injury (TBI) is a common cause of long-term disability. Due to challenges that include inconsistent access to follow-up care, persons with TBI being discharged from inpatient rehabilitation facilities (IRFs) are at risk for rehospitalization, poor reintegration into the community, family stress, and other unfavorable outcomes resulting from unmet needs. In a six-center randomized pragmatic comparative effectiveness study, the BRITE trial (Brain Injury Rehabilitation: Improving the Transition Experience, ClinicalTrials.gov NCT03422276 ), we compare the effectiveness of two existing methods for transition from IRF to community living or long-term nursing care. The Rehabilitation Discharge Plan (RDP) includes patient/family education and referrals for continued care. The Rehabilitation Transition Plan (RTP) provides RDP plus individualized, manualized care management via phone or videoconference, for 6 months. Nine hundred patients will be randomized (1:1) to RDP or RTP, with caregivers also invited to participate and contribute caregiver-reported outcomes. Extensive stakeholder input, including active participation of persons with TBI and their families, has informed all aspects of trial design and implementation planning. We hypothesize that RTP will result in better patient- and caregiver-reported outcomes (societal participation, quality of life, caregiver well-being) and more efficient use of healthcare resources at 6-months (primary outcome) and 12-months post-discharge, compared to RDP alone. Planned analyses will explore which participants benefit most from each transition model. With few exclusion criteria and other pragmatic features, the findings of this trial are expected to have a broad impact on improving transitions from inpatient TBI rehabilitation. Abstract

Phosphoinositide 3-kinase (PI3K) is an enzyme that mediates endocrinological responses, such as intracellular signaling of insulin and growth factors, and plays important roles in muscle homeostasis and growth. In this study, the effect of antemortem PI3K activity on meat quality traits was investigated using broiler chickens whose PI3K was inhibited pharmacologically. Breast and thigh muscles were harvested from broilers treated with the PI3K inhibitor wortmannin, and meat quality traits were evaluated by determination of color, water-holding capacity, and breaking strength. The pH and concentrations of glycogen and free amino acids were also investigated as determinants of the chemical properties of meat. The results indicated that antemortem PI3K inhibition by wortmannin modified breast muscle color with lower L∗ values (P < 0.05) and b∗ values (P < 0.05) and higher a∗ values (P < 0.05). Antemortem PI3K inhibition also increased the water-holding capacity of breast muscles (P < 0.05), although breaking strength was not much affected. In addition, antemortem PI3K inhibition increased the concentrations of free amino acids in breast muscles, especially arginine (P < 0.05) and glutamic acid (P < 0.05). Similar effects were observed in thigh muscles. Lower glycogen levels at sacrifice (P < 0.05) and the resultant higher pH during the postmortem period (P < 0.05) were associated with PI3K inhibition-induced changes in meat quality traits. The wortmannin-treated muscles shared certain features with dark, firm, and dry meat which is a common abnormal meat. These findings suggest that antemortem PI3K activity contributes to meat quality traits and is involved in the molecular mechanism of the production of meat quality abnormalities.

Use of grafts from donors after cardiac death would greatly contribute to the expansion of the donor organ pool. The two major methods of preservation are cold storage and machine perfusion (MP) preservation, and each has its own advantages. Several studies have reported the relative merits of MP for the preservation for grafts from donors after cardiac death. In this study, we used scanning electron microscopy (SEM) to assess the damage to the liver between hypothermic and rewarming preservation conditions. METHODS: Porcine livers were perfused with a newly developed MP system. The livers were perfused for 4 hours with a modified University of Wisconsin solution-gluconate solution. In group 1, grafts were preserved with warm ischemic time for 60 minutes and hypothermic machine perfusion (HMP) for 4 hours. In group 2, grafts were preserved with warn ischemic time for 60 minutes and had rewarming up to 22°C by MP (RMP) for 4 hours. RESULTS: A significant enlargement of the mitochondria were observed in both the HMP and RMP groups under higher magnification, Additionally, vacuoles appeared occasionally in hepatocytes in the RMP for 4 hours group, but not in the HMP for 4 hours group. CONCLUSIONS: An analysis by scanning electron microscope appears to be useful to evaluate the levels of damage of hepatocytes compared with transmission electron microscopy, and further study is needed to analyze the significance of the appearance of swelling of mitochondria and vacuolization during preservation.

Abstract

A 50-layer stacked InGaAs/GaAs quantum dot solar cell (QDSC) grown on GaAs (100) with a self-organized texture formed on its backside has been fabricated in aim for enhancing the photoabsorption by the QD layers. Since doped GaAs substrate has a large free-carrier absorption, a semi-insulating substrate was used for fabricating the QDSC with backside texture. A micrometer-scale texture was formed by a simple wet-etching in a NH4OH solution. The light scattering at the textured backside surface caused a light trapping effect and resulted in an increased effective optical path length by multiple reflection inside QDSC, and this effect was confirmed by the external quantum efficiency (EQE) measurement. The photoabsorption attributed to InGaAs QD layers increased by about 2.4 times compared to QDSC without a textured backside. As a result, the short-circuit current density (Jsc) measured at an air-mass 1.5 global (AM1.5G) spectrum illumination increased from Jsc = 20.9 to 21.5 mA/cm2.

Purpose Registry data report that 15-26% of pediatric patients on the heart transplant (HT) waiting list are supported with biventricular assist devices (BiVAD), with high waitlist mortality. There are little data on post-HT outcomes for this cohort. Methods The United Network for Organ Sharing (UNOS) database was queried for patients Results Of 4904 Pediatric HT recipients, patients were grouped by no VAD support (3934, 80.2%), LVAD only (736, 15%) and BiVAD (234, 4.8%). Overall graft survival analysis indicates crossing hazard rates between groups over time (Fig. 1a) with a significant difference of graft survival at 1 year post-HT, lowest for BiVAD (p Conclusion Pre-HT BiVAD support leads to worse graft survival 1 year post-HT compared to no VAD and LVAD patients, but is not constant over time with acceptable long-term survival. Further research is needed to investigate causes of decreased graft survival in the pediatric BiVAD population early post-HT.

Abstract

Radial profile of externally applied resonant magnetic perturbation (RMP) field with mode numbers of m = 6 and n = 2 in a small tokamak device HYBTOK-II have been investigated using a magnetic probe array, which is able to measure the radial profile of magnetic field perturbation induced by applying RMP. Results of RMP penetration into the plasma show that the RMP decreased toward the plasma center, while they were amplified around the resonant surface with a safety factor q = 3 due to the formation of magnetic islands. This suggests that RMP fields for controlling edge plasmas may trigger some kind of MHD instabilities. In addition, simulation results, based on a linearized four-field model, which agrees with the experimental ones, indicates that the penetration and amplification process of RMP strongly depend on a Doppler-shifted frequency between the RMP and plasma rotation.

Abstract

We have investigated the synthesis conditions, and the magnetic properties of the Cu 2 U-type hexagonal ferrite, Ba 4 Cu 2 Fe 36 O 60 . The Cu 2 U-type hexaferrite was synthesized at the sintering temperature of 1050 °C with the initial composition of Ba 4.4 Cu 2 Fe 37.6 O 62.8 (Cu 2 U+0.2T-block). The saturation magnetizations at 300 K and 5 K are 46.8 A m 2 /kg and 65.0 A m 2 /kg, respectively. The Curie temperature is 420 °C which is lower than that of the M-type ferrite (450 °C) but higher than that of the Cu 2 Y-type ferrite (320 °C). The amount of the nonmagnetic impurity in this sample is estimated to be about 10 wt% by the electron probe micro analysis. We estimated the expected saturation magnetization to be 65.2 A m 2 /kg, by assuming the model of a Neel-type ferrimagnetic structure and the reduction of magnetization by the 10 wt% nonmagnetic impurity. This is consistent with the observed magnetization of 65.0 A m 2 /kg at 5 K.

Abstract

We performed neutron diffraction study of Sc–In alloy which indicates that the ordered magnetic moment in this material is intrinsically tiny. This tiny magnetic moment (0.04–0.05 μB/Sc) is not caused by the uncompensated antiferromagnetic ordering of large magnetic moments. The temperature dependence of the resistivity is in proportion to T 3 2 , which suggests that this material is naturally close to the quantum critical point where ferromagnetism vanishes. This is consistent with the fact that this material shows ferromagnetism in an extremely narrow range of compositions which is typical of materials exhibiting quantum critical phenomenon. In the vicinity of the Curie temperature the magnetization depends on magnetic fields as M 5 ∝ μ 0 H . This behavior for itinerant ferromagnets can be explained by assuming that the total spin fluctuation amplitude is conserved.

Abstract

In this Letter, comprehensive results on π±, K±, K0 S , p(p) and ¯ Λ(Λ)¯ production at mid-rapidity (0 < yCMS < 0.5) in p–Pb collisions at √sNN = 5.02 TeV, measured by the ALICE detector at the LHC, are reported. The transverse momentum distributions exhibit a hardening as a function of event multiplicity, which is stronger for heavier particles. This behavior is similar to what has been observed in pp and Pb–Pb collisions at the LHC. The measured pT distributions are compared to d–Au, Au–Au and Pb– Pb results at lower energy and with predictions based on QCD-inspired and hydrodynamic models.

Abstract

The consumption of plants cultivated in soils contaminated by radioactive cesium can lead to internal exposure and health problems in humans. It is therefore very important to clarify the uptake mechanism of radioactive cesium from contaminated soils. In this study, the variation of cesium concentrations in plants was examined using mediums that contained no potassium and different cesium concentrations of 50, 100, 250 and 500 ppm. Raphanus sativus was selected as a typical edible vegetable and hydroponically cultivated. Cesium concentrations in leaves were analyzed with a submilli-PIXE camera. The concentration of cesium in plants was observed to increase as concentrations in the medium increased. As the concentration of cesium in the medium increased, the transfer coefficient decreased. However, there was little difference between the 250 and 500 ppm treatments. In future work, PIXE analysis will be performed on different mediums and the relationship with other materials will be investigated.
