13. Analgesic efficacy of theta-burst stimulation for postoperative pain.

Abstract

OBJECTIVE: Repetitive transcranial magnetic stimulation (rTMS) may be a relevant method to assist postoperative pain. However, studies to date have only used conventional 10 Hz rTMS and targeted the DLPFC for postoperative pain. A more recent form of rTMS, termed intermittent Theta Burst Stimulation (iTBS), enables to increase cortical excitability in a short period of time. This preliminary double-blind, randomised, sham controlled study was designed to evaluate the efficacy of iTBS in postoperative care across two distinct stimulation targets. METHODS: A group of 45 patients post laparoscopic surgery were randomised to receive a single session of iTBS over either the dorsolateral prefrontal cortex (DLPFC), primary motor cortex (M1), or Sham stimulation (1:1:1 ratio). Outcome measurements were number of pump attempts, total anaesthetic volume used, and self-rated pain experience, assessed at 1 hour, 6 hours, 24 hours, and 48 hours post stimulation. All randomised patients were analysed (n = 15 in each group). RESULTS: Compared to Sham stimulation, DLPFC-iTBS reduced pump attempts at 6 (DLPFC = 0.73 ± 0.88, Sham = 2.36 ± 1.65, P = 0.031), 24 (DLPFC = 1.40 ± 1.24, Sham = 5.03 ± 3.87, P = 0.008), and 48 (DLPFC = 1.47 ± 1.41, Sham = 5.87 ± 4.34, P = 0.014) hours post-surgery, whereby M1 stimulation had no effect. No group effect was observed on total anaesthetics, which was mainly provided through the continuous administration of opioids at a set speed for each group. There was also no group or interaction effect on pain ratings. Pump attempts were positively associated with pain ratings in the DLPFC (r = 0.59, P = 0.02) and M1 (r = 0.56, P = 0.03) stimulation. CONCLUSIONS: Our findings show that iTBS to the DLPFC reduces pump attempts for additional anaesthetics following a laparoscopic surgery. However, reduced pump attempts by DLPFC stimulation did not translate into a significantly smaller volume of total anaesthetic, due to the continuous administration of opioids

Abstract

Fold-and-thrust belts (FTBs) develop widely in and around active orogenic belts on Earth. With the accumulation of structural shortening, thrust-related folds can grow by increasing their amplitudes and lengths, providing insights into the structural evolution of FTBs. Investigations of fold growth patterns and landscape responses through time and space have important implications for hydrocarbon exploration and geo-hazard assessment. Evidence for lateral fold propagation (i.e., increase in length) usually comes from geochronological constraints based on differences in onset timings along strike. However, these traditional methods are time consuming and economically costly. Alternatively, previous studies have proposed the use of geomorphic features to investigate the lateral fold propagation, including spatial variations in the patterns and densities of river channels, the configuration of the topographic profile along the fold crest, and the occurrence of wind gaps. However, such analyses are based on morphological features of present-day landscapes, which still need process-based validations. Here, we conduct a series of landscape evolution models to reproduce fold growth and their associated geomorphic adjustments. Our results show that deflected rivers and inflected fold-crest elevation profile with decreasing gradient are reliable evidence of lateral fold propagation. The location of propagation is in accordance with the distribution of crest-profile slope breaks and the position where river deflection began. Then, we apply these findings to two natural examples located at the deformation fronts of the Tian Shan (NW China). Results suggest that 1) the Kashi anticline has experienced lateral propagation both westwards and eastwards; and 2) that the Yaken anticline has experienced accelerated tectonic uplift as well as eastwards propagation. These findings are supported by the constraints from geochronological dating and quantitative structural analysis. Future work that integrates folding mechanisms may provide novel insights into landscape response to complex deformation in active FTBs.

Abstract

The substoichiometric tungsten oxide (WO3-x) nanostructures are important for the catalytic, energy and optoelectronic applications, yet it remains challenging to control their morphologies and oxygen vacancies. Here we address the challenge of synthesizing the WO3-x nanoflakes and nanorods in ethanol and ethanoic acid by solvothermal method, where tungsten hexachloride (WCl6) is used as the precursor of tungsten. The results characterized by different analytical methods indicate that the WO3-x nanoflakes and nanorods are crystalline structures composed of different phases, respectively. The formation of different WO3-x nanostructures is related to the difference in the formation mechanisms of WO3-x monomers in ethanol and ethanoic acid. Due to the difference in the mobility of WO3-x monomers caused by the properties of ethanol and ethanoic acid, the WO3-x nanoflakes are formed in ethanol depending on the interface between gas and liquid ethanol. In contrast, the WO3-x nanorods are formed in ethanoic acid due to the anisotropic intermediate product formed by the reaction of WCl6 and ethanoic acid. Furthermore, the photoluminescence (PL) of WO3-x nanoflakes and nanorods is studied at room temperature and low temperature. Both the WO3-x nanostructures generate the ultraviolet, green, red and near-infrared light at room temperature, which are related to the transition from the defect resonant state level in the conduction band to the valence band, the transition between defect state levels and relaxation of polarons, respectively. The PL emission at low temperature was enhanced due to the weak lattice vibration and the increase of oxygen vacancy concentration. Abstract

Properties of substoichiometric tungsten oxide (WO3-x) semiconducting nanomaterials in diverse applications are morphology-dependent. However, it remains challenging to control morphology and structure of WO3-x nanomaterials. Here, we report the solvothermal synthesis of WO3-x nanostructures with different morphologies in tungsten hexachloride-ethanol solution through adjusting the filling fraction of ethanol in autoclave. The structural and morphological conversion mechanisms are related to the changes in the viscosity and Brownian motion caused by the concentration of tungsten hexachloride in ethanol. The studies on the photoluminescence (PL) properties reveal that the WO3-x nanostructures emit ultraviolet, blue, green and red emissions. The PL emissions are attributed to the recombination between the electron occupying the resonant defect state in the conduction band and the hole in the valence band and the transitions between the conduction and valence bands as well as the transition between the oxygen vacancy states. The enhancement of PL intensity is related to the increase of oxygen vacancies and the phonon suppression during the PL emission as well as the nanocavity structure. Furthermore, the potential applications in the catalysis fields are discussed. These results provide a new technique to control the structure of WO3-x nanomaterials and contribute to the development of next-generation PL emission nanodevices based on vacancy- and structure-engineered WO3 materials and the rational design of catalytic materials.

Abstract

OBJECTIVES: Depression symptom questionnaires are not for diagnostic classification. Patient Health Questionnaire-9 (PHQ-9) scores ≥10 are nonetheless often used to estimate depression prevalence. We compared PHQ-9 ≥10 prevalence to Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders (SCID) major depression prevalence and assessed whether an alternative PHQ-9 cutoff could more accurately estimate prevalence. STUDY DESIGN AND SETTING: Individual participant data meta-analysis of datasets comparing PHQ-9 scores to SCID major depression status. RESULTS: A total of 9,242 participants (1,389 SCID major depression cases) from 44 primary studies were included. Pooled PHQ-9 ≥10 prevalence was 24.6% (95% confidence interval [CI]: 20.8%, 28.9%); pooled SCID major depression prevalence was 12.1% (95% CI: 9.6%, 15.2%); and pooled difference was 11.9% (95% CI: 9.3%, 14.6%). The mean study-level PHQ-9 ≥10 to SCID-based prevalence ratio was 2.5 times. PHQ-9 ≥14 and the PHQ-9 diagnostic algorithm provided prevalence closest to SCID major depression prevalence, but study-level prevalence differed from SCID-based prevalence by an average absolute difference of 4.8% for PHQ-9 ≥14 (95% prediction interval: -13.6%, 14.5%) and 5.6% for the PHQ-9 diagnostic algorithm (95% prediction interval: -16.4%, 15.0%). CONCLUSION: PHQ-9 ≥10 substantially overestimates depression prevalence. There is too much heterogeneity to correct statistically in individual studies.
