15. Risks of injury in distal metatarsal minimally invasive osteotomy when comparing standard and modified techniques: A cadaveric study

Abstract

The objectives of the study were to evaluate the structures at risk in distal metatarsal mini-invasive osteotomy (DMMO) and to compare standard and intraosseous approaches.DMMO was performed on the second and fourth metatarsals of 11 fresh-frozen cadaveric specimens. The standard technique was performed in 11 metatarsals. It was then compared to a modified intraosseous technique that entails starting inside the bone in 11 other metatarsals. The cadavers were dissected to identify unintentional injury to soft tissue structures.In the standard group the most injured structures were the metatarsal joint capsules (MJC) (27%), extensor digitorum longus (EDL) (18%), and extensor digitorum brevis (EDB) (9%). The modified intraosseous group injured the EDL (27%), not the MJC (0%) and the EDB (0%). Distances between osteotomies and the dorsal metatarsal head articular surface (DMHAS) were 6.08 ± 3.99 mm in the standard and 9.92 ± 3.42 mm in the modified (p = 0.02).The DMMO techniques most frequently injured the EDL. Intra-articular positioning of the osteotomy was more observed in the standard. Overall, it appears the modified method could be an alternative to the standard DMMO.The modified minimally invasive DMMO has a comparable rate of potential iatrogenic injuries. This intraosseous procedure may present as an option when planning surgery to the lesser metatarsals.Level III. Comparative Cadaveric Study.

Abstract

Introduction In surgically treated rotational malleolar fractures, residual syndesmotic instability is typically assessed following fixation with the widely used intraoperative Cotton test. However, due to its dynamic nature, there are inconsistencies of the magnitude and direction of the distraction force when attempting to pull the fibula away from the tibia using a bone hook. The novel Tap test advances a cortical tap through a drilled hole in the fibula with a stable, unidirectional distraction force applied to the tibia. The objective of this cadaveric study was to compare the Cotton and Tap tests as diagnostic tools for coronal plane syndesmotic instability. Methods Tibiofibular Clear Space (TFCS) of 10 cadaveric specimens was measured for: intact, non-stressed; intact, stressed; injured, non-stressed; and injured, stressed (Tap and Cotton tests). In injured conditions, the syndesmotic ligamentous complex was sectioned using an anterolateral longitudinal approach. Perfect fluoroscopic Mortise images were acquired for all conditions. Two independent and blinded Orthopaedic Foot and Ankle Surgeons measured TFCS 1 cm proximal to the ankle joint line. Intra and interobserver reliabilities were assessed by Intraclass Correlation Coefficient. Syndesmotic TFCS values for all conditions were compared by paired Wilcoxon. Diagnostic performance of the Cotton and Tap tests was assessed using a relative increase of TFCS > 2 mm when comparing intact stressed and injured stressed conditions. P-values Results The intraclass correlation coefficient for intraobserver and interobserver reliability was respectively 0.96 and 0.79. TFCS measurements were similar in intact non-stressed, intact stressed (both Cotton and Tap tests) and injured non-stressed conditions, with mean values and 95% Confidence Intervals of: intact non-stressed, 3.5 mm; intact stressed, 3.6 mm (Cotton test) and 4.0 mm (Tap test); injured non-stressed, 3.8 mm. The Cotton test and Tap test had, respectively, 73.3% and 70% sensitivity, 100% and 90% specificity, 86.7% and 80% diagnostic accuracy. Conclusions Our cadaveric study compared the Cotton and Tap tests for detection of coronal plane syndesmotic instability. Both tests demonstrated similar increases in the TFCS measurements in stressed injured conditions when compared to intact non-stressed and stressed conditions, as well as injured non-stressed conditions.

Abstract

Since the beginning of the COVID-19 pandemic and its economic impacts, scientists and policymakers alike have been advocating for green recovery packages focusing on clean energy technologies, to align economic recovery with climate change mitigation. In this study, we use three energy-economy models combined with portfolio analysis to estimate the emissions and employment benefits of a potential allocation of announced recovery packages towards clean energy projects, in six major emitting regions: Canada, China, EU, India, Japan, and USA. Despite trade-offs and regional differences, we find potential technology portfolios that both reduce emissions and increase employment across all regions and models. Solar PV is the dominant technology for achieving both emissions cuts and employment gains in most countries, while optimal packages also include funds for other low-carbon technologies. The selected packages significantly contribute to achieving NDC objectives and reducing pandemic-driven unemployment in the EU, China, and Canada. In USA and India, the proposed packages are too small to make a significant contribution. We find strong model structure-driven differences in outcomes, confirming the relevance of applying diverse models to avoid false precision in outcomes.

Abstract

This paper focuses on strategic storage as a solidarity measure in response to short-term “high-impact, low-probability” (HILP) disruptions in the European Union (EU) gas supply from major suppliers (i.e., Ukraine, Russia, Norway, and North Africa), assuming its implementation in selected Member States. A two-stage stochastic cost minimization gas transport model is used to evaluate the impact of HILP events on the level of demand curtailment, survival time, and the natural gas supply mix of EU regional risk group. Results show that geographic proximity alone, without solidarity measures, is inadequate in providing system resilience. In contrast, solidarity measures lead to a longer survival time for regional risk groups (14 days) and to a reduction in system (15%) and LNG (70%) costs relative to a base scenario with no strategic storage. The analysis stresses the value of the coordinated use of strategic storage in balancing the natural gas network during emergencies, and provides further evidence supporting the EU legislative path towards an Energy Union.

Abstract

Background Despite advances in malleolar fractures treatment, the overall risk to develop posttraumatic ankle osteoarthritis after 20 years is almost 40%, especially bimalleolar Weber type B and C fractures and fractures involving the posterior tibial rim. Methods We performed a systematic literature review of clinical studies targeting the changes in cartilage, synovial cells and synovial fluid after malleolar fractures. Results The acute ankle injury initiates a sequence of biological events potentially leading to progressive articular surface damage resulting from inflammatory changes in cartilage, synovial tissue and synovial fluid. Conclusion A better understanding of the molecular and histological changes induced by acute trauma may potentially lead to novel, targeted treatment of malleolar fractures besides anatomical reduction and adequate stabilization.

Abstract

As of 2020, North American natural gas extraction and use in the electricity sector have both reached all-time highs. The combination of North America's increased reliance on natural gas with a potential disruption to the natural gas market has several energy security implications. Additionally, policymakers interested in economic resiliency will find this study's results useful for informing the implications of the energy sectors' long-term planning and investment decisions. This paper evaluates how both the electricity and natural gas sectors could respond to hypothetical gas price shocks under different system configurations. We impose unforeseen natural gas price shocks under reference and alternative configurations resulting from a renewable generation mandate or variations to renewable capacity costs. Results from several different models are presented for the electricity and natural gas sectors separately for Canada, Mexico, and the United States. Generally, the US becomes more (less) reliant on electricity imports from Canada given a high (low) gas price shock but increases (decreases) exports to Mexico. The renewable mandate is demonstrated to buffer electricity price increases under high price shocks but price reductions under the low price shocks are dampened given less flexibility to take advantage of the low-priced natural gas. The United States is demonstrated to reduce natural gas production and net exports with high natural gas price shocks given a reduction in demand.

Abstract

Similarly to other transition metal sulfides, nickel sulfide nanocrystals can be potentially used for functional device applications. However, controlling morphology and stoichiometry to target specific applications is a synthesis challenge. In this work we developed a rapid, one-step, chemical vapor deposition synthesis of nickel sulfide dendritic nanostructures with fractal geometry. Microtome-EDS compositional analysis of the mature crystal indicates a trend of decreasing sulfur and increasing nickel concentration towards the tip of the mature crystals. Following thorough investigation of these nanocrystals at different stages of their nucleation and growth by means of XRD, HR-SEM, HR-TEM, and Raman spectroscopy, we suggest possible kinetic mechanisms for the crystal formation and development. This work contributes to the understanding of growth mechanisms of dendritic structures with complex morphology.
