1. Survival outcomes of alternate dosing schedule of pemetrexed as maintenance therapy in NSCLC: Single institution experience

Abstract

Maintenance therapy with pemetrexed has shown survival benefit in patients with advanced stage non-small cell lung cancer (NSCLC). The recommended dose schedule is 500 mg/m 2 in 21-day cycles. Prolonged treatment with maintenance pemetrexed can result in cumulative toxicities. We sought to compare treatment outcomes in patients receiving an alternate maintenance schedule in realworld practice.This single-center, retrospective study investigated patients with advanced stage (IIIB and IV) NSCLC receiving at least two doses of maintenance pemetrexed from May 1, 2011 to June 30, 2016. The objective was to compare time on treatment with maintenance pemetrexed therapy initiated at a standard schedule (q3 weeks) versus an alternate schedule (q4 weeks or longer). Also evaluated were progressionfree survival (PFS) and overall survival (OS) differences between the two groups.129 patients were included, of whom 40 started the alternate schedule no later than cycle 3 of treatment (29 of 40 patients initiated maintenance treatment on the alternate schedule). Average time on maintenance treatment for patients appeared to be longer in the patients who received the alternate schedule regimen (195 vs 263 days, p =0.008). OS trended towards better survival among patients receiving the alternate schedule regimen (11.9 vs 18.1 months, p =0.3). Limiting the analysis to ALK wildtype, the patients showed a similar trend, with median PFS (7.6 vs 11.5 months, p =0.46) and OS (11.9 vs 17.6 months, p =0.38), still favoring the alternate schedule.The alternate dosing schedule of maintenance pemetrexed (q4 weeks or longer) is feasible and not detrimental to OS. Future investigations evaluating the optimal administration schedule of maintenance pemetrexed is warranted.

Abstract

The optimal approach to combine gemtuzumab-ozogamicin (GO) with various chemotherapy backbones and other newer agents safely remains to be determined. We performed a retrospective analysis of the safety and outcomes of newly diagnosed adult AML patients treated with GO in combination with intensified versus standard anthracycline doses (i.e. daunorubicin dose 90 mg/m(2) vs 60 mg/m(2)) ± FLT3 inhibitors. Chi-square test and Mann Whitney U test were used to compare categorical and continuous data. Survival estimates were calculated by Kaplan–Meier method and survival comparisons made using log-rank test. We report an ORR of 97% in newly diagnosed AML patients (n=37) with a median age of 54 (range, 19–75) years treated with GO in combination with standard induction. All eleven patients (100%) receiving GO plus daunorubicin dose 90 mg/m(2) as part of 7+3 induction achieved CR vs 91% (20/22) CR in the standard daunorubicin 60 mg/m(2) dose group (p=NS), No increased toxicity was noted with the higher daunorubicin dose or when GO and 7+3 were combined with FLT3 inhibitors in younger patients (age

Abstract

Peripheral neuropathies are a group of acquired and hereditary disorders presenting with different distribution and nerve fiber class involvement. The overall prevalence is 2.4%, increasing to 8% in the elderly population. However, the frequency may vary depending on the underlying pathogenesis and association with systemic diseases. Distal symmetric polyneuropathy is the most common form, though multiple mononeuropathies, non-length dependent neuropathy and small fiber neuropathy can occur and may require specific diagnostic tools. The use of uniform outcome measures in peripheral neuropathies is important to improve the quality of randomized controlled trials, enabling comparison between studies. Recent developments in defining the optimal set of outcome measures in inflammatory neuropathies may serve as an example for other conditions. Diagnostic and outcome measure advances in peripheral neuropathies will be discussed.

Abstract

Sodium channel Na(v)1.7 is preferentially expressed in dorsal root ganglion (DRG) and sympathetic ganglion neurons. Gain-of-function Na(v)1.7 mutations/variants have been identified in the painful disorders inherited erythromelalgia and small-fiber neuropathy (SFN). DRG neurons transfected with these channel variants display depolarized resting potential, reduced current-threshold, increased firing-frequency and spontaneous firing. Whether the depolarizing shift in resting potential and enhanced spontaneous firing are due to persistent activity of variant channels, or to compensatory changes in other conductance(s) in response to expression of the variant channel, as shown in model systems, has not been studied. We examined the effect of wild-type Na(v)1.7 and a Na(v)1.7 mutant channel, D623N, associated with SFN, on resting potential and membrane potential during interspike intervals in DRG neurons. Resting potential in DRG neurons expressing D623N was depolarized compared to neurons expressing WT-Na(v)1.7. Exposure to TTX hyperpolarized resting potential by 7 mV, increased current-threshold, decreased firing-frequency, and reduced NMDG-induced-hyperpolarization in DRG neurons expressing D623N. To assess the contribution of depolarized resting potential to DRG neuron excitability, we mimicked the mutant channel's depolarizing effect by current injection to produce equivalent depolarization; the depolarization decreased current threshold and increased firing-frequency. Voltage-clamp using ramp or repetitive action potentials as commands showed that D623N channels enhance the TTX-sensitive inward current, persistent at subthreshold membrane voltages, as predicted by a Hodgkin-Huxley model. Our results demonstrate that a variant of Na(v)1.7 associated with painful neuropathy depolarizes resting membrane potential and produces an enhanced inward current during interspike intervals, thereby contributing to DRG neuron hyperexcitability.

Abstract

Small fiber neuropathy (SFN) is a neuropathy selectively involving small diameter myelinated and unmyelinated nerve fibers. Interest in this disorder has considerably increased during the past few years. It is often idiopathic and typically presents with peripheral pain and/or symptoms of autonomic dysfunction. Diagnosis is made on the basis of the clinical features, normal nerve conduction studies (NCS) and abnormal specialized tests of small nerve fibers. Among others, these tests include assessment of epidermal nerve fiber density, temperature sensation tests for sensory fibers and sudomotor and cardiovagal testing (QSART) for autonomic fibers. Unless an underlying disease is identified, treatment is usually symptomatic and directed towards alleviation of neuropathic pain.

Abstract

Although a number of studies have shown that the incidence of late potentials is lower after thrombolytic therapy, it is not known whether this is paralleled by fewer arrhythmic events during long-term follow-up. In patients with first acute myocardial infarction, filtered QRS duration was significantly shorter when treated with streptokinase (95 +/- 11 ms, n = 53) than when treated with conventional therapy (99 +/- 12 ms, n = 77, p0.05). The low-amplitude signal (D40) was shorter after thrombolysis (28 +/- 11 vs 33 +/- 12 ms, p0.02). Terminal root-mean-square voltage did not differ significantly (41 +/- 24 vs 35 +/- 23 microV). Irrespective of treatment, late potentials were predictive in the complete group (n = 171) for arrhythmic events during follow-up (13 +/- 6 months, range 6 to 24) (hazard ratio 7.7, p0.02, Cox proportional-hazards survival analysis), but treatment (streptokinase vs conventional) did not significantly affect outcome when added to the model. It is concluded that thrombolysis prevents the development of late potentials. However, this study does not confirm the hypothesis that prevention of late potentials leads to a decrease in arrhythmic events.

Abstract

The honeycomb strip chamber (HSC) is a new position sensitive detector. It consists of a stack of folded foils, forming a rigid honeycomb structure. In the centre of each hexagonal cell a wire is strung. Conducting strips on the foils, perpendicular to the wires, pick up the induced avalanche charge. Test results of a prototype show that processing the signals from three adjacent strips nearest to the track gives a spatial resolution better than 64 μm for perpendicular incident tracks. The chamber performance is only slightly affected by a magnetic field.

Abstract

We have observed B0-B0 mixing in Z0→bb decays using hadronic events containing dileptons. The data sample corresponds to 118 200 hadron events at √s≈Mz. From a fit to the dilepton p and p⊥ spectra, we determine the mixing parameter to be χB=0.178-0.040 +0.049. © 1990.

Abstract

Whatever the field, major change implies as much as deep transformation of thinking i.e to put in network new skills in particular, those made available by the availability of new technologies. In this context, only one individual cannot join together all necessary skills. It becomes clear that the goal of space valorization cannot be considered without relational investment between the partners and their accumulated background. The local empowerment concerns a collective phenomenon of acquisition and development of skills rests on an learning way appropriate to the potential of the territory to form or promote. The local mediation sets for objective the creation of relations between pre-existent skills and their transfer between the actors, individuals or groups, private or public. But the capacity of the territory to solve these problems will depend closely on the richness of the network and co-operations which it shelters or it will suggest.

Abstract

The L3 experiment is one of the six large detectors designed for the new generation of electron-positron accelerators. It is the only detector that concentrates its efforts on limited goals of measuring electrons, muons and photons. By not attempting to identify hadrons, L3 has been able to provide an order of magnitude better resolution for electrons, muons and photons. Vertices and hadron jets are also studied. The construction of L3 has involved much state of the art technology in new principles of vertex detection and in new crystals for large scale electromagnetic shower detection and ultraprecise muon detection. This paper presents a summary of the construction of L3.

Abstract

Dystrophinopathy patient registries are very useful for improvement of standard care and planning of clinical trials. The Dutch Dystrophinopathy Database contains information of Duchenne muscular dystrophy (DMD) and Becker muscular dystrophy (BMD) patients in the Netherlands. The historical incidence of DMD is estimated at 20 boys per year and unknown for BMD. DMD and BMD patients received information through patient organisations, treating physicians, the genetic test centre or a website ( www.lumc.nl/duchenne ). Included patients gave consent for registration in the national patient registry and the international TREAT-NMD database, permission to contact their physician and filled out a questionnaire about their disease course. On January 2013 the database contained disease milestones of 420 DMD and 140 BMD patients, including data from 78 deceased DMD and 16 deceased BMD males. 324 DMD and 104 BMD patients have a DNA confirmed diagnosis. The mean yearly incidence for DMD was 18 between 1980 and 2000. Given a mean life expectancy of 27 years this would suggest a prevalence of 486 DMD patients, and thus an inclusion rate in the database of 70%. The mean yearly incidence for BMD was 4 between 1960 and 2000. The mean life expectancy was 55 years suggesting a prevalence of 220 BMD patients, and thus an inclusion rate in the database of 56%. In the DNA confirmed DMD cohort mean age at diagnosis decreased from 5.6 years before 1970 to 4.0 years more recently. The mean age at loss of ambulation increased to 9.8 years and for scoliosis surgery to 14 years. Age at start of mechanical ventilation ranged from 14 to 28 years. Close collaboration during five years between patient organisations, physicians and researchers resulted in a patient registry covering three quarters of all Dutch DMD patients. The yearly update is a constant challenge, but the advantage is one national registry representative for the whole population, limiting selection bias to a minimum.

Abstract

Aim: Chemotherapy-induced peripheral neuropathy (CIPN) is a dose-limiting side effect of oxaliplatin which can negatively influence quality of life. We aimed to study the influence of cumulative dose, dose schedule and dose reductions of adjuvant oxaliplatin on long-term severity and prevalence of CIPN among colorectal cancer (CRC) survivors. Methods: A total of 188 patients, diagnosed with CRC between 2000 and 2009 who underwent adjuvant treatment with oxaliplatin, were included. Patients were identified by the Eindhoven Cancer Registry. All patients completed the EORTC QLQ-CIPN20 2-11 years after diagnosis. Data on oxaliplatin administration and acute neuropathy during treatment were extracted from the medical files. Results: Oxaliplatin was given for a median 6 cycles (range 1-12) at a mean cumulative dose of 680 mg/m2 (SD 265). Patients who received cumulative oxaliplatin dose of ≥842 mg/m2 had a significantly worse EORTC QLQ-CIPN20 sensory score compared to those who received a low cumulative dose of Conclusions: Cumulative dose of oxaliplatin is associated with long-term CIPN in CRC survivors. The risk of developing long-term CIPN could only be reduced by decreasing the cumulative dose of oxaliplatin early in the treatment period. Future studies should focus on identifying patients who are at risk of developing CIPN. Disclosure: All authors have declared no conflicts of interest.

Abstract

The performance of transducers used for medical diagnosis depends to a great extent on matching layer, backing impedance and geometry of the active surface. In this paper special attention is given to the element vibrational modes, the optimum matching, the grating lobe and change of the real acoustic impedance into an imaginary impedance as a function of the product of the width and wavelength. Grating lobes on the echo image and the effect of a mismatch of the matching layer are illustrated. Suggestions are made for the design of linear and phased array transducers considering the above-mentioned aspects.
