Abstract

Objectives: The aims of this study were to assess (i) the prevalence of antibiotic use, (ii) factors associated with their use and (iii) the association with in-hospital mortality in a large sample of hospitalised older people in Italy.Methods: Data were obtained from the 2010-2017 REPOSI register held in more than 100 internal medicine and geriatric wards in Italy. Patients aged >= 65 years with at least one antibiotic prescription during their hospitalisation were selected. Multivariable logistic regression models were used to determine factors associated with antibiotic use.Results: A total of 5442 older patients were included in the analysis, of whom 2786 (51.2%) were prescribed antibiotics during their hospitalisation. The most frequently prescribed antibiotic class was beta-lactams, accounting for 50% of the total prescriptions. Poor physical independence, corticosteroid use and being hospitalised in Northern Italy were factors associated with a higher likelihood of being prescribed antibiotics. Antibiotic use was associated with an increased risk of in-hospital mortality (odds ratio = 2.52, 95% confidence interval 1.82-3.48) also when accounting for factors associated with their use.Conclusion: Hospitalised older people are often prescribed antibiotics. Factors related to poor physical independence and corticosteroid use are associated with increased antibiotic use. Being prescribed antibiotics is also associated with an increased risk of in-hospital death. These results demand the implementation of specific stewardship programmes to improve the correct use of antibiotics in hospital settings and to reduce the risk of antimicrobial resistance. Abstract

The semiconductor industry is exploring technology scaling to pursuit the Moore's Law. The actual processors operation frequency grows the need for fast memories. Nowadays, SRAM cells occupy a considerable area in VLSI designs. Several challenges follow this performance improvement achieved at each new technology node. The Process, Voltage, and Temperature (PVT) variability, aging effects due to BTI influence and radiation-induced Single-Event Upset (SEU) are three relevant issues on the SRAM nanometer design. The main contribution of this work is to present a panorama of these effects on SRAM as technology scaling. The most frequently used SRAM cell, the 6 T, is evaluated from 45 nm to 7 nm bulk CMOS and FinFET technologies. Results observed the effects on delay, power, and noise margins, showing that process variability can introduce up to 100% of power deviation. Read Static Noise Margin (RSNM) presents about 20% of deviation under process variability and the cell noise robustness is reduced dramatically in worst cases. FinFET technology and high-performance models show more robustness against radiation. SRAM cells with low-power devices demonstrated more sensitive to delay degradation due to aging effects.

Abstract

The study of traditional built environment and the correct use of natural resources, nowadays, represent some of the multiple opportunities to product compatible structures with the environment, aimed to cope with high-energy demanding buildings and comfortless. The ethnic Mediterranean culture, purpose of the study, output of the traditional architectural knowledge, recognizes in the vernacular dwelling a symbiotic relationship between built and natural environment, respectful towards the territory thanks to the use of available raw materials that reduce human footprint. The correct use of the local raw materials and the definition of rules, archetypes and shapes are the perfect evidence of this symbiotic relationship, that ages improved achieving the perfect balance with the territory. The architectural elements, created to meet solely functional needs, changed over time, causing a rich heritage of shapes. To enhance the properties of the modern building envelopes it is necessary, preliminarily, to analyse the logic and architectural invariant of the traditional artefacts that are brought to mind, to a greater or to a lesser degree, for inspiration and to take us back on the way of harmonic planning with the place, actualizing the proposal.

Abstract

Many works in the literature are in favour of the universality of power-laws. However, more recently, a significant amount of research has exposed more subtle details about this subject. In this paper we present two generalisations that aim to solve these possible antagonistic conceptions in a mathematical framework. The first generalisation allows us to show that a vast range of power-laws can be produced through multiplicative process (for example through a single positive feedback mechanism). Abstract

The continuous technological evolution of protection and control systems has allowed improvements in the accuracy, operational flexibility and reduction of the size of equipment. The electricity sector is exploring commercially available solutions for the implementation of a process bus based on IEC61850. Functionally, IEC61850 is an equivalent solution to conventional wired logic systems; however, it has generated great expectations for reductions in the cost of design, construction, operation and maintenance. Several works on specific technical aspects of IEC 61850 are available in the literature, such as latency analysis, network topology, data models, services and types of message. However, when making decisions, a more comprehensive analysis of implementation costs, reliability, regulatory framework, operational context and technical trade-offs is required. This article proposes a mathematical model for the quantitative benefit–cost analysis of a process bus solution based on IEC 61850. In this work, four architectures of a substation automation system are compared in terms of their initial investment, scheduled maintenance, unscheduled maintenance, reliability, and penalties for energy not supplied considering the regulatory framework. The method used to carry out these evaluations is an analysis of the life cycle cost, which allows more objective decisions to be taken when considering the reliability requirements for electric power transmission and distribution systems. The goal of the model is to identify opportunities for companies in the electricity sector to involve themselves in the design and construction of the next generation of substations in such a way that their operation will be safe, reliable and economically efficient.

Abstract

NADH cytochrome b5 reductase 3 (CYB5R3) is critical for reductive reactions such as fatty acid elongation, cholesterol biosynthesis, drug metabolism, and methemoglobin reduction. Although the physiological and metabolic importance of CYB5R3 has been established in hepatocytes and erythrocytes, emerging investigations suggest that CYB5R3 is critical for nitric oxide signaling and vascular function. However, advancement toward fully understanding CYB5R3 function has been limited due to a lack of potent small molecule inhibitors. Because of this restriction, we modeled the binding mode of propylthiouracil, a weak inhibitor of CYB5R3 (IC50 = ∼275 μM), and used it as a guide to predict thiouracil-biased inhibitors from the set of commercially available compounds in the ZINC database. Using this approach, we validated two new potent derivatives of propylthiouracil, ZINC05626394 (IC50 = 10.81 μM) and ZINC39395747 (IC50 = 9.14 μM), both of which inhibit CYB5R3 activity in cultured cells. Moreover, we found that ZINC39395747 significantly increased NO bioavailability in renal vascular cells, augmented renal blood flow, and decreased systemic blood pressure in response to vasoconstrictors in spontaneously hypertensive rats. These compounds will serve as a new tool to examine the biological functions of CYB5R3 in physiology and disease and also as a platform for new drug development.

Abstract

Background: Chronic pain is a frequent characteristic of elderly people and represents an actual and still poorly debated topic. Objective: We investigated pain prevalence and intensity, and its pharmacological therapy in elderly patients hospitalized in 101 internal medicine wards. Methods: Taking advantage of the “REgistro POliterapie Società Italiana Medicina Interna” (REPOSI), we collected 2535 patients of whom almost a quarter was older than 85 years old. Among them, 582 patients were affected by pain (either chronic or acute) and 296 were diagnosed with chronic pain. Results: Patients with pain showed worse cognitive status, higher depression and comorbidities, and a longer duration of hospital stay compared to those without pain (all p <.0366). Patients with chronic pain revealed lower level of independency in their daily life, worse cognitive status and higher level of depression compared to acute pain patients (all p <.0156). Moreover, most of them were not treated for pain at admission (73.4%) and half of them was not treated with any analgesic drug at discharge (50.5%). This difference affected also the reported levels of pain intensity. Patients who received analgesics at both admission and discharge remained stable (p =.172). Conversely, those not treated at admission who received an analgesic treatment during the hospital stay decreased their perceived pain (p <.0001). Conclusions: Our results show the need to focus more attention on the pharmacological treatment of chronic pain, especially in hospitalized elderly patients, in order to support them and facilitate their daily life after hospital discharge.

Abstract

The estimated glomerular filtration rate (eGFR) is a predictor of important outcomes and its reduction has been associated with the risk of all-cause mortality in both general population and elderly patients. However while reduced renal function is common in older people, the best method for estimating GFR remains unclear, especially in an acute care setting. Most studies analyzing the accuracy of eGFR in the elderly were carried out in different heterogeneous settings. In this study, we compare the prognostic value of different formulas estimating GFR in predicting the risk of in-hospital morbidity and mortality within 3 months from discharge in elderly hospitalized patients. Data were extracted from "Registro Politerapia Societh. Italian di Medicina Interna (REPOSI)". Patients with available creatinine values at hospital admission were selected and eGFR was calculated according to the different formulas: Cockcroft-Gault, Modification of Diet in Renal Disease equation, Chronic Kidney Disease Epidemiology Collaboration, Berlin Initiative Study and Full Age Spectrum. 4621 patients were included in the analysis. Among these, 4.2% and 14.2% died during hospitalization and within 3 months from discharge, respectively. eGFR > 60 ml/min/1.73 m(2) at admission was associated with a very low risk of mortality during the hospital stay and within 90 days from discharge, while an eGFR < 60 ml/min/1.73 m(2) was associated with unfavorable outcomes, although with a poor level of accuracy (AUC 0.60-0.66). No difference in predictive power between different equations was found. Physicians should be aware of the prognostic role of eGFR in a comprehensive assessment of elderly in-patients.

Abstract

Background: The aim of this study was to identify which factors were associated with a risk of hospital readmission within 3 months after discharge of a sample of elderly patients admitted to internal medicine and geriatric wards. Methods: Of the 1178 patients aged 65 years or more and discharged from one of the 66 wards of the 'Registry Politerapie SIMI (REPOSI)' during 2010, 766 were followed up by phone interview 3 months after discharge and were included in this analysis. Univariate and multivariate logistic regression models were used to evaluate the association of several variables with rehospitalization within 3 months from discharge. Results: Nineteen percent of patients were readmitted at least once within 3 months after discharge. By univariate analysis in-hospital clinical adverse events (AEs), a previous hospital admission, number of diagnoses and drugs, comorbidity and severity index (according to Cumulative Illness Rating Scale-CIRS), vascular and liver diseases with a level of impairment at discharge of 3 or more at CIRS were significantly associated with risk of readmission. Multivariate logistic regression analysis showed that only AEs during hospitalization, previous hospital admission, and vascular and liver diseases were significantly associated with the likelihood of readmission. Conclusions: The results demonstrate the need for increased medical attention towards elderly patients discharged from hospital with characteristics such as AEs during the hospitalization, previous admission, vascular and liver diseases. © 2012 European Federation of Internal Medicine.
