Abstract

Coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) in revascularization of left main coronary artery (LMCA) disease has been evaluated in previous studies. However, there has been minimal study of the relationship between co-existing non-coronary atherosclerosis (NCA) and LMCA disease revascularization. We aim to examine this relationship. The Gulf-LM study is a retrospective analysis of unprotected LMCA revascularization cases undergoing PCI with second generation drug-eluting stent vs CABG across 14 centers within 3 Gulf countries between January 2015 and December 2019. A total of 2138 patients were included, 381 with coexisting NCA and 1757 without. Outcomes examined included major adverse cardiovascular and cerebrovascular events (MACCE), cardiac and non-cardiac death, and all bleeding. In patients with NCA, preexisting myocardial infarction and congestive heart failure were more common, with PCI being the most common revascularization strategy. A statistically significant reduction in in-hospital MACCE and all bleeding was noted in patients with NCA undergoing PCI as compared to CABG. At a median follow-up of 15 months, MACCE and major bleeding outcomes continued to favor the PCI group, though no such difference was identified between revascularization strategies in patients without NCA.In this multicenter retrospective study of patients with and without NCA who require revascularization (PCI and CABG) for unprotected LMCA disease, PCI demonstrated a better clinical outcome in MACCE both in-hospital and during the short-term follow-up in patients with NCA. However, no such difference was observed in patients without NCA.

Abstract

We present the evolution of Norwood operation outcomes and practice pattern changes over 15 years from a single institution in Saudi Arabia. We intended to identify time trends in patient selection, procedural details, and outcome predictors over time.Patients who underwent a Norwood operation (n = 145) between 2003 and 2018 with the use of a Blalock-Taussig shunt (BT group; n = 72), right ventricle to pulmonary artery shunt (Sano group; n = 66), or a primary cavopulmonary shunt (CPS group; n = 7) were included. The study outcomes were operative mortality, long-term survival, and multistate transition to CPS, Fontan, and death.Median age was 29 days. Predictors of operative mortality were lower weight (P = .026), and longer bypass time (P = .014), whereas age, and type of shunt were not. Predictors of improved long-term survival were greater weight at operation (P = .0016), later era (P = .006), and shorter bypass time (P = .001). The multistate model revealed that patients with lower weight were more likely to undergo Sano versus BT (P .001), and if BT was chosen in such patients, they were more likely to die (P = .027). The likelihood of receiving Sano shunt was 3-fold greater in the recent era (P = .003).Improved outcomes of the Norwood operation are evident in the recent era and with Sano shunt, especially in patients of smaller weight. Late presentation or older age is not a contraindication to Norwood operation. The incorporation of a primary CPS at stage one operation is feasible in selected patients.

Abstract

Background Preoperative anemia is accused for increased complications, morbidity and mortality following cardiac surgery. Due to the nature of cardiac patients who usually have poor myocardium reserve and multiple concomitant comorbidities, it is difficult to identify the role of preoperative hemoglobin (HB) concentration in the postoperative outcome. Accordingly, correction of low HB level prior to cardiac surgery still is a matter of debate. The aim of this work was to determine the impact of preoperative HB concentration on postoperative outcome following coronary artery bypass grafting (CABG). Methods Data were collected on all patients who underwent CABG surgery in our hospital in the last three years. Preoperative Anemia was defined as HB level below 12 gm/dl. Accordingly, three groups were identified: low, normal and high preoperative HB. Postoperative bleeding, red blood cells transfused, myocardial injury, infection rate, lengths of ventilation, renal injury and length of stay in intensive care unit (ICU) were recorded. Results We recruited 804 patients in our study. Preoperative anemia were identified in 159 patients assigned in group I. Patients in this group had postoperative blood transfusion in 69% of patients and postoperative bleeding > One liter in 31.6% of patients with P-value of 0.006,0.096 respectively. It showed higher postoperative infection rate in 9.5% of patients (P-value 0.49) and higher length of stay in ICU in 43.3% of patients (P value 0.003). Group II was 481 patients who had HB (12.1–15 gm/dl) showed the shortest postoperative hospital stay in 56.1% of patients, P value 0.0001. Group III, 164 patients had HB more than 15.1 gm./dl. No cases of ICU readmission were recorded. Conclusion Preoperative HB level can be used as an indicator of outcome after cardiac surgery. Preoperative anemia should be optimized before CABG.

Abstract

Using the method of Bolles and Drago (1) the thermodynamic properties of drug - aromatic amino acid complexes were determined calorimetrically. The comparison of K c evaluated from vapour pressure osmometry. ORD, partition and calorimetry shows a considerable agreement, whereas the data calculated from solubility measurements differ widely from our results. Qualitatively the relationship between solubilisation through different salts and Δ G 0 for the complexation with the same salts could be demonstrated. Methylxanthines form complexes with anions and cations, the complex formation in H 2 O and D 2 O, determined calorimetrically, shows the greater stability of the complexes in D 2 O.
