Prefabricated dowels do not always provide intraradicular sealing in the root canal dentin, and the lack of sealing predisposes the dowel to adhesive failure and debonding. It is unclear if fiber-reinforced composite resin dowels provide better sealing.The purpose of this in vitro study was to evaluate the intraradicular sealing and morphological fit of prefabricated dowels and fiber-reinforced composite resin dowels in root canal dentin. The thickness of the resin cement layer and push-out bond strength were determined to assess their effects on the sealing of the dowels.A total of 50 permanent maxillary central incisors were endodontically treated and divided into 2 subgroups (n=25). In one group, prefabricated dowels were cemented; in the second group, fiber-reinforced composite resin dowels were placed. The thickness of the resin cement layer was assessed in 3 different locations: coronal, middle, and apical of the root canal dentin with fluorescence confocal laser microscopy. The push-out bond strength was then determined, and intraradicular sealing observed by using scanning electron microscopy.Fiber-reinforced composite resin dowels showed a closer intraradicular fit and seal in the root canal dentin, and the morphology of the apical portion of the fiber-reinforced composite resin dowels showed a sealing area with the gutta percha in the apical portion. The mean thickness of the resin cement layer was significantly reduced for the fiber-reinforced composite resin dowels compared with the prefabricated dowels (P.05) in the cervical area (197.0 μm versus 311.0 μm) and in the apical portion of the root canal (57.3 μm versus 131.6 μm). The mean push-out strength was higher for the fiber-reinforced composite resin dowels (22.98 N/mmThe morphological fit of fiber-reinforced composite resin dowels provides better intraradicular sealing in the cervical and apical portions, reducing the resin cement thickness. The increased push-out strength can therefore be assumed to result from increased frictional retention compared with prefabricated dowels.

Statement of problem Various strategies have been proposed to reduce the cement space of foundation restorations for endodontically treated teeth. However, they may add more operative steps, or the dentist must keep different sizes of drills and posts in stock. A 2-piece universal adjustable post system has been developed to overcome this problem, but whether the system has acceptable fatigue survival performance is unclear. Purpose The purpose of this in vitro study was to evaluate the fatigue survival and stress distribution of endodontically treated teeth without a ferrule and restored with different glass fiber post strategies versus a recently introduced universal 2-piece fiber post system. Material and methods Bovine incisor roots were randomly assigned to 3 groups as per the post used (n=13): adapted glass fiber post with post space preparation of the same size, composite resin-custom glass fiber post (CTM), and universal 2-piece glass fiber-reinforced composite resin post (UNI). The posts were adhesively luted, the composite resin core was added, and a composite resin crown was produced with computer-aided design and computer-aided manufacturing (CAD-CAM), and then adhesively luted to each core. A fatigue test was performed with the stepwise stress method (10 000 cycles/step; 20 Hz; load=100 N to 750 N; step=50 N) until fracture, and the failure mode analyzed. The stress distribution was evaluated by finite element analysis with the maximum principal stress criteria by following the parameters of the in vitro test. The solids were considered homogeneous, linear, and isotropic, except for the glass fiber post (orthotropic), and a load of 450 N at 30 degrees was applied. The fatigue failure load and the number of cycles for failure were analyzed with Kaplan-Meier and Mantel-Cox (log rank test) (α=.05). The finite element analysis results were analyzed with colorimetric graphs. Results The highest fatigue failure load and the number of cycles for failure were found in the UNI system, whereas the lowest results were found in the CTM group. All groups exhibited repairable failures. The finite element analysis showed the lowest stress in root dentin in the UNI system. The CTM system had the largest stress regions at the dentin and dentin-core interface. Conclusions The use of a 2-piece universal glass fiber post system resulted in more fatigue behavior compared with composite resin-custom glass fiber posts.

Objective: To investigate the effect of novel polymeric nanoparticles (NPs) doped with dexamethasone (Dex) on viscoelasticity, crystallinity and ultra-nanostructure of the formed hydroxyapatite after NPs dentin infiltration. Methods: Undoped-NPs, Dex-doped NPs (Dex-NPs) and zinc-doped-Dex-NPs (Zn-Dex-NPs) were tested at dentin, after 24 h and 21 d. A control group without NPs was included. Coronal dentin surfaces were studied by nano-dynamic mechanical analysis measurements, atomic force microscopy, X-ray diffraction and transmission electron microscopy. Mean and standard deviation were analyzed by ANOVA and Student-Newman-Keuls multiple comparisons (p < 0.05). Results: At 21 d of storage time, both groups doped with Dex exhibited the highest complex, storage and loss moduli among groups. Zn-Dex-NPs and Dex-NPs promoted the highest and lowest tan delta values, respectively. Dex-NPs contributed to increase the fibril diameters of dentin collagen over time. Dentin surfaces treated with Zn-Dex-NPs attained the lowest nano-roughness values, provoked the highest crystallinity, and produced the longest and shortest crystallite and grain size. These new crystals organized with randomly oriented lattices. Dex-NPs induced the highest microstrain. Crystalline and amorphous matter was present in the mineral precipitates of all groups, but Zn and Dex loaded NPs helped to increase crystallinity. Significance: Dentin treated with Zn-Dex-NPs improved crystallographic and atomic order, providing structural stability, high mechanical performance and tissue maturation. To investigate the effects of salivary esterases on biostability of collagen treated by galloylated polyphenols.Human dentin was microtomed into 6-μm-thick films, which were demineralized and treated for 60 s using solutions containing 0.6% and 2% of one of the crosslinkers: tannic acid (TAC), epigallocatechin gallate (EGCG), epigallocatechin (EGC), and N-[3-dimethylaminopropyl]-N'-ethylcarbodiimide hydrochloride (EDC)/N-hydroxysuccinimide (NHS), and for 1 h using EDC/NHS. Half of the treated and untreated (control) films were subjected to human saliva incubation. Collagen biostability was assessed via exogenous protease biodegradation by weight loss and hydroxyproline release, and endogenous MMPs by in situ zymography. The degradation products of galloylated polyphenols (TAC and EGCG) by saliva were monitored using proton nuclear magnetic resonance (Collagen films treated with TAC and EGCG exhibited significantly improved biostability (p 0.05); however, the enhanced biostability was severely reduced after saliva incubation (p 0.001). For EDC/NHS treated collagen, saliva incubation showed negligible effect on the biostability.The galloyl group plays important role in collagen crosslinking, inducing higher biostability. However, galloylated polyphenols crosslinked on collagen are highly susceptible to metabolism of human saliva by salivary esterase, dramatically compromising the enhanced biostability.

ObjectivesThe aim was to investigate shear bond strengths and failure modes of four self-etch bonding agents to bovine dentin and enamel and to compare evaluation of data sets with or without exclusion of cohesive failure specimens.MethodsComposite-cylinders were bonded perpendicularly to bovine dentin and enamel surfaces. Shear-strengths were measured 24 h post-bonding of: Scotchbond Universal® (SBU, 3 M), OptiBond™ XTR (OBXTR, Kerr), OptiBond™ universal (OBU, KaVo-Kerr) and Prime & Bond active® (PBA, Dentsply-Sirona). Analysis of overall data was made via a linear mixed-model. This was repeated after exclusion of specimens associated with cohesive failures.ResultsWhen both adhesive and cohesive failures were considered, OBU and OBXTR showed comparable dentin and enamel bond strengths, whereas lower strengths were found on enamel for SBU (p < 0.001) and PBA (p = 0.015). For OBXTR higher shear strengths were measured for specimens associated with cohesive failures. When cohesive failures were excluded, the majority of shear bond strengths of adhesive failure specimens were only slightly different from overall results. However, uniquely with OBXTR dramatically lower shear bond strengths were found for dentin substrate.SignificanceAfter exclusion of cases with cohesive failures OBXTR adhesive fell behind other materials in the sequence of average shear strengths. This did not reflect the actual performance of the material. Therefore, in statistical analysis we do not recommend exclusion of data based on a specific fracture mode.

Although the clinical use of self-adhesive flowable composite resins is convenient, the decision to use flowable or conventional composite resin remains controversial.The purpose of this systematic review and meta-analysis was to assess the in vitro bond strength of self-adhesive flowable composite resins to enamel or dentin compared with conventional composite resins using etch-and-rinse or self-etch adhesive systems.Two independent reviewers performed searches in the PubMed (MEDLINE), Cochrane Wiley, Web of Science, Scopus, and Google Scholar databases for studies reporting on the bond strength of self-adhesive flowable and conventional composite resins to enamel and dentin published from January 2010 up to September 2020. A meta-analysis software program was used for the meta-analysis. Comparisons were conducted using standardized mean differences considering the random-effects model (α=.05).Twenty-two studies were considered for the meta-analysis. Immediate and long-term bond strength to enamel was improved when a conventional composite resin was used in combination with an adhesive system (P.001). Bond strength to dentin was improved by using an adhesive system in combination with a conventional composite resin (P.001).The bond strength of self-adhesive flowable composite resins is lower than that of conventional composite resins, indicating limited ability to bond to enamel and dentin.

To investigate the potential mineralising effects of calcium silicate-based dentine replacement material (Biodentine™) in comparison with glass-ionomer cement (GIC) (Fuji IX™) on different human dentine substrates using a multimodal non-invasive optical assessment.Cements were applied on artificially demineralised or naturally carious dentine and stored for 4 weeks in phosphate-rich media +/- tetracycline used for mineralisation labelling. Interfacial dentine was examined from the same sample and location before and after aging using two-photon fluorescence microscopy, fluorescence lifetime imaging (FLIM) and second harmonic generation (SHG) imaging. Additionally, Raman spectroscopy was used to detect changes in the mineral content of dentine.Significant changes in the fluorescence intensity and lifetime were detected in partially demineralised dentine and caries-affected dentine underneath both tested cements, after storage (p 0.001). This was associated with a significant increase in the mineral content as indicated by the increased intensity of the phosphate Raman peak located at 959 cm

The latest advancements in dentin bonding have focused on strategies to impair degradation mechanisms in order to extend the longevity of bonded interfaces. Protease inhibitors can reduce collagen degradation within the hybrid layer (HL). Collagen cross-linkers allow better adhesive infiltration and also inhibit proteases activity. Particles added to adhesive can promote mineral precipitation within the HL, reducing nanoleakage and micropermeability, besides possible antimicrobial and enzymatic inhibition effects. Most of these approaches are still experimental, and aspects of the adhesive under the clinician's control are still determinant for the long-term stability of adhesive restorations.

To characterize the behavior of three different polymeric agents before and after an erosive challenge on dentin permeability, to analyze their degradation in both conditions, and to analyze their degree of conversion (DC).The permeability of human dentin disks (1.0 ± 0.2 mm) was measured with smear layer, after its removal, after treatment (LpTreat) with Gluma Desensitizer, PRG Barrier Coat (PBC) or Icon infiltrant (n = 11/group) and after exposure to citric acid (LpEro) (6%, pH 2.1, 1 min). The specimens were analyzed under a Laser Scanning Confocal Microscope (n = 2/group) and the products' DC were calculated. Data were subjected to 2-way repeated measures ANOVA and post-hoc Bonferroni (permeability analysis), to paired t-test (for specimens treated with Icon) and to t-test (DC analysis) (α 0.05).Icon showed the lowest LpTreat and LpEro values, while PBC and Gluma did not differ from each other under these conditions. Icon and PBC showed LpEro similar to a dentin with smear layer. Under the Laser Scanning Confocal Microscope, more deposits were noticeable on dentin after treating with PBC. Gluma presented the deepest penetration in dentin. The DC of PBC was the highest.Icon caused the highest reduction on permeability values, followed by PBC and Gluma. PBC generated more deposits covering dentin and seemed to be more efficient after an erosive challenge. The association of a polymeric resin with inorganic ion-releasing fillers seem to be a great strategy to manage dentin hypersensitivity under erosive conditions.

This study aimed to evaluate the clinical efficacy and histological outcomes of autogenous demineralised dentin matrix (ADDM) as bone graft material compared with Bio-Oss® in bone augmentation for the treatment of patients with oral bone deficits. Eight databases (PubMed, EMBASE, Cochrane Library, Science Direct, Scopus, Web of Science, CNKI, and WFPD) were searched for randomised controlled trials (RCT) performed from the date of inception of each database to July 2021. The Cochrane Collaboration's risk assessment tool was used to conduct the methodological quality assessment. Stata 15.0 software was used to perform data analysis. Seven RCTs including 220 patients were considered eligible for this study. No significant difference was found in the percentage of new bone formation (NBF) and implant stability quotient (ISQ). Patients who received ADDM grafting showed a significantly lower sinus height (SH) and percentage of residual graft material (RGM) compared with Bio-Oss® grafting. ADDM is as effective as Bio-Oss® in bone augmentation for oral bone defects.

The aim of this study was to evaluate the effect of previous root canal treatment on dentinal tubule penetration of calcium silicate-based sealers during endodontic retreatment and to compare 2 different (commonly used and modified) tubule penetration depth measurement techniques.The crowns of mandibular premolar teeth were removed, and root canals were prepared (ProTaper Universal; Dentsply Sirona, York, PA) and obturated using AH Plus (AH; Dentsply DeTrey, Konstanz, Germany), MTA Fillapex (MTAF; Angelus, Londrina, Brazil), and WellRoot ST (WRST; Vericom, Gyeonggi-do, South Korea); 0.1% red Rhod-2 dye (Chemodex, St Gallen, Switzerland) was added to the sealers. After storage for 3 weeks (37°C, 100% humid), the following sealer combination subgroups were created (the sealer used during the primary root canal treatment/the sealer used during the retreatment, n = 8): AH/AH, AH/MTAF, and AH/WRST; MTAF/AH, MTAF/MTAF, and MTAF/WRST; and WRST/AH, WRST/MTAF, and WRST/WRST. Green Fluo-3 dye (0.1%) was added to the sealers during retreatment. Sealer penetration depths were measured at 8 points and averaged, and the ratio of the retreatment sealer to the initial sealer was recorded as the mean tubule penetration depth ratio in the first technique, whereas the percentage of the circumferential penetration area ratios of the retreatment and initial sealers was calculated in the second technique. Statistical analyses were performed using Kolmogorov-Smirnov, Kruskal-Wallis, and Mann-Whitney U tests.The mean penetration depth of AH and MTFA was found to be higher at the coronal third region in the control groups (P .05). Both methods showed a significant difference among the groups at the coronal and median regions (P .05). The AH/WRST, MTAH/AH, MTAF/MTAF, MTAF/WRSTS, and WRST/WRST groups showed deeper tubule penetration (first technique, P = .01). Both techniques showed that the penetration depth of the sealers was the same at the apical region (P .05), whereas a difference was found among the techniques at the coronal and median regions. The sealers used in the previous root canal treatment limited the penetration depth of the sealers used during the retreatment process (P .05). When MTAF was used as the initial sealer, the penetration depth of the second sealer did not change, whereas if WRST was the initial sealer, the penetration depth of the sealers at the median and apical regions is negatively affected during the retreatment process (P .05).Previous root canal treatment affects the penetration depth of calcium silicate-based sealers. The use of AH Plus and MTA Fillapex during the retreatment process showed an advantage. Both measurement techniques showed similar performances at the apical region, whereas the results varied at the coronal and midcoronal regions; therefore, a more stable sealer penetration measurement technique is still necessary.

To determine whether smear layer management, via conservative etching protocols, and the hydrophobic-rich content of hybrid layers would affect the fatigue strength of resin-dentin interfaces.Bar-shaped dentin beams obtained from sound third molars were wet-polished for 30 s. Dentin was etched with 32 % ortho-phosphoric acid for 3 or 15 s, 10 % meta-phosphoric acid for 15 s or by a prime-and-rinse application using a mild universal adhesive (Scotchbond Universal, 3M ESPE). Self-etch application served as control. Coating was performed with a solvent-free bisGMA-based resin. Composite buildups were made with a nanofilled composite. Resin-dentin beams with twin-bonded interfaces were sectioned and stored in deionized water for 24 h at 37 ℃ before 4-point flexural quasi-static monotonic testing (n = 16). Stress-life fatigue behavior was evaluated under cyclic loading (n = 35) by the staircase method at 4 Hz. The tension side of cyclic-loaded unfractured beams were evaluated under SEM, along with the micro-morphology of etched dentin surfaces. Monotonic data was analyzed by two-way ANOVA followed by the Tukey Test and cyclic-loaded data by Kruskal-Wallis on Ranks (α = 0.05).Etching protocols and higher hydrophobic-rich content produced significantly higher fatigue life distributions (p 0.05). Dentin demineralization was ranked as OPA 15 s MPA 15 s OPA 3 s P + R SE. Less aggressive etching and coating reduced crack formation at hybrid layers.Current oversimplification trends in resin-dentin bonding constitute a trade-off between hybridization quality and easier adhesive handling. Controlled dentin etching and increasing the hydrophobic-rich content of hybrid layers may be necessary to extend the longevity of mild universal adhesives.

This study aims to characterize the cytotoxicity potential of silver diamine fluoride (SDF) on dental pulp stem cells (DPSC) and gingival equivalents.DPSC cultured on 96-well plates was exposed directly to SDF (0.0001-0.01%) and cell viability (ICDPSC exposed directly to SDF showed a dose-dependent reduction in cell viability (ICSDF penetrated the dentin (≤1 mm thick) inducing significant death of the pulp cells. SDF also disrupted gingival epithelial integrity resulting in mucosal corrosion.

This study investigated the significance of pericervical dentin after coronal canal flaring on the biomechanical behavior and life span of a maxillary molar using finite element analysis (FEA).In addition to the intact tooth (IT) model, 4 experimental FE models were designed: conservative access cavity model (CON), and 3 models with different radicular preparations for the coronal 4 mL considering 3 instruments: ProTaper SX model (SX), OneFlare model (OF), and Gates-Glidden model (GG). Cyclic loading of 50 N was applied on the occlusal surface and number of cycles until failure (NCF) was compared with the IT model. Mathematical analysis was done to evaluate the stress distribution patterns and calculated maximum von Mises (VM) and maximum principal stresses.Access cavity preparation (CON) decreased NCF significantly when compared with the IT model (93.99%). The coronal preparation of the root canal did not have a significant effect even when the preparation was taken to the extreme (GG: 92.02%). VM analysis confirmed apical dispersion of stresses, with maximum value registered on the occlusal surface in the GG model (7.88 MPa), and minimum on the IT model (7.01 MPa). The furcation area showed higher maximum principal stresses, yet stress values remained minimal and distributed over larger surfaces with the progressive enlargement among models.Within the limitations of this study, coronal canal flaring affects tooth integrity minimally, and when loading conditions lie within normal functional ranges, tooth structure has the capacity to disperse increasing stresses over a wider surface area.

The conventional radiotherapy protocol to treat head-and-neck cancer is usually followed by tooth-decay onset. Radiation impact on mineralized tooth structures is not well-understood. This systematic review aimed to collect the recorded effects of therapeutic radiation on tooth chemical, structural and mechanical properties, in relation with their means of investigation.Systematic search (January 01 2012 - September 30 2021) terms were "Radiotherapy", "Radiation effects", "Dental enamel", "Dentin", "Human" and "Radiotherapy" NOT "Laser".PubMed, DOSS and Embase databases were searched.Selected studies compared dental enamel, coronal and root dentin properties before and after in vitro or in vivo irradiation up to 80 Gy.The systematic search identified 353 different articles, with 28 satisfying inclusion criteria. Their reference lists provided two more. Twenty-two studies evaluated dental enamel evolution, nine assessed coronal dentin and eight concerned root dentin. Coronal and root dentin results indicate a major impact of the radiation on their organic matrix. Dental enamel's chemical properties are less modified. Enamel and root dentin's hardness are decreased by therapeutic radiation, but no consensus arises for coronal dentin.Our findings revealed some interesting information about enzymatic degradation mechanisms of dentin organic matrix and highlighted that dental hard-tissue characterization requires highly specific expertise in materials science. That scientific knowledge is necessary to design suitable protocols, adequately analyze the obtained data, and, thus, provide relevant conclusions.Better knowledge and understanding of the mechanisms involved in the degradation of enamel and dentin would enable development of new preventive and therapeutic methods for improved medical care of patients undergoing radiotherapy.

The aim of this study was to evaluate the efficacy of autogenous dentin grafts with guided bone regeneration (GBR) for horizontal ridge augmentation. Nineteen patients with dentition and bone defects in whom tooth/teeth extraction was indicated were recruited. Autogenous teeth were prepared, fixed on the buccal sides of the defects, and covered with bone powder and resorbable membranes before implantation. The horizontal bone mass at 0 mm (W1), 3 mm (W2), and 6 mm (W3) from the alveolar crest was recorded using cone beam computed tomography, before, immediately after, and 6 months after dentin grafting. All adverse effects were recorded. The implant stability quotient (ISQ) was measured 6 months after implantation. Twenty-eight implants were placed 6 months after dentin grafting. At this time point, the bone mass was 4.72 ± 0.72 mm (W1), 7.35 ± 1.57 mm (W2), and 8.96 ± 2.38 mm (W3), which was significantly different from that before the surgery (P 0.05). The bone gain was 2.50 ± 0.72 mm (W1), 4.10 ± 1.42 mm (W2), and 4.56 ± 2.09 mm (W3). No soft tissue dehiscence or infection was observed. Overall, 26.3% of the patients experienced severe pain after dentin grafting. The ISQ was 78.31 ± 6.64 at 6 months after implantation. Autogenous tooth roots with GBR might be effective for horizontal ridge augmentation. This technique could be an alternative to augmentation using autogenous bone grafts.

Methods: PCL-based solutions (10% w/v) containing nHA (0 – control; 0.5; 1.0; or 2.0% w/v) were electrospun into nanofibrous scaffolds. The scaffolds were characterized for morphology and composition (MEV/EDS), solubility, the release of calcium/phosphate (C/P), and modulation of medium pH. Then, HDPCs were seeded on the scaffolds and evaluated for cell viability (alamarBlue and live/dead), adhesion and spreading (F-actin), total protein (TP; Lowry), alkaline phosphatase activity (ALP; thymolphthalein assay), expression of odontogenic genes (RT-qPCR), and formation of a mineralized matrix (Alizarin Red). Data were analyzed with ANOVA and post-hocs (α = 5%). Results: Higher nHA concentrations roughened fiber surfaces, whereas PCL+ 2%nHA increased the interfibrillar spaces. PCL+ 1%nHA or PCL+ 2%nHA significantly released more C/P but the medium pH was maintained below 8.0. HDPCs viability was not affected by nHA, while cell adhesion/spreading was favored, especially for PCL+ 2%nHA. Higher protein content and ALP activity were seen for scaffolds incorporated with nHA, after 21 days. PCL+ 1%nHA and PCL+ 2%nHA upregulated the expression of DSPP and DMP1 in 14 days, and COL1A1, ALPL, and DMP1 in 21 days. The formation of a mineralized matrix was nHA concentration-dependent, and it was about 9 × higher for PCL+ 2%nHA. Significance: nHA-incorporated PCL nanofibrous scaffolds are cytocompatible and can stimulate the adhesion and odontogenic potential of HDPCs. PCL+ 2%nHA formulation is a bioactive tissue engineering-based cell-homing strategy for VPT. The tooth-resin composite interface is frequently associated with failure because of microbial contamination, hydrolytic and collagenolytic degradation. Thus, designing a dentine bonding system (DBS) with an intrinsically antimicrobial polymerisable monomer is of significance especially if it can be used with self-etching primers enabling resistance to degradation of the interface.Experimental adhesives were developed incorporating eugenyl methacrylate (EgMA) at concentrations of 0,10 or 20 wt%, designated as EgMA0, EgMA10 and EgMA20, respectively, for use as a two-step self-etch DBS with the functional monomer bis[2-(methacryloyloxy) ethyl] phosphate (BMEP) in the primer. The curing, thermal and wettability properties of the adhesives were determined, and hybrid layer formation was characterised by confocal laser scanning microscopy, microtensile bond strengths (µTBS) and nanoleakage by back-scattered SEM. In situ zymography was used to assess MMP inhibitory activity of the BMEP-EgMA DBS.EgMA in the adhesives lowered the polymerisation exotherm and resulted in higher Tg, without negatively affecting degree of conversion. Water sorption and solubility were significantly lower with higher concentrations of EgMA in the adhesive. The formation of a distinct hybrid layer was evident from confocal images with the different adhesives, whilst EgMA20 yielded the highest µTBS post water storage challenges and lowest nanoleakage after 6 months. The experimental DBS exhibited minimal to no MMP activity at 3 months.The hydrophobic nature of EgMA and high cross-link density exerts considerable benefits in lowering water uptake and polymerisation exotherm. The application of EgMA, adhesives in conjunction with BMEP in a multi-functional self-etching DBS can resist MMP activity, hence, enhance longevity of the dentine-resin composite interface.

Chemically released growth factors play a vital role in regenerative endodontics. Transforming growth factor beta 1 (TGF-β1) is 1 of the most extensively studied bioactive molecules that promotes cell proliferation, differentiation, and chemotaxis. The goal of the current research was to analyze the effect of 7% maleic acid (MA) root conditioning of an infected root canal on the release of TGF-β1.Single-rooted human teeth were decoronated, and the canals were enlarged with a Peeso reamer. The samples were divided into biofilm and nonbiofilm groups. Subsequently, all the samples of both groups were flushed with 10 mL of each irrigant, namely, 1.5% sodium hypochlorite (NaOCl), 7% MA, 17% EDTA, and a combination of 1.5% NaOCl with 17% EDTA or 7% MA, for 10 minutes each. TGF-β1 was estimated quantitatively using an enzyme-linked immunosorbent assay kit.TGF-β1 release was lowest among the biofilm samples compared with nonbiofilm among all the groups. MA 7% with 1.5% NaOCl rendered higher amounts of growth factor release in contrast to the combination of 17% EDTA and 1.5% NaOCl in both the biofilm and nonbiofilm groups (P.048). The nonbiofilm samples treated with 7% MA alone illustrated higher growth factor release compared with 17% EDTA only (P.006), but there was no significant difference in growth factor release among the biofilm samples treated with 7% MA and 17% EDTA.Bacterial biofilms modified the release of TGF-β1. MA 7% was observed to be significantly more efficacious than 17% EDTA in TGF-β1 growth factor release from radicular dentin.

A novel method of combining chlorhexidine (CHX) loaded poly (amido amine) (PAMAM) dendrimers with a dental adhesive containing amorphous calcium phosphate (ACP) nanofillers are proposed for etch-and-rinse bonding system to enhance resin-dentin bonding durability.The CHX-loaded PAMAM and ACP nanofillers were synthesized and characterized. Their effects on the cytotoxicity were tested by MTT assay. Micro-tensile bond strength (μTBS) before and after thermomechanical challenges were used to evaluate the bonding durability. Anti-matrix metalloproteinase (MMPs) property was examined using in-situ zymography. A double-fluorescence technique was used to examine interfacial permeability after bonding. Dentin remineralization in Ca/P lacking solution was observed under scanning electron microscopy.Compared with a 0.2 wt% CHX solution, the PAMAM loaded CHX had less cytotoxicity, while the in situ zymography showed it could still inhibit MMPs activity within the hybrid layer after released from PAMAM. The application of the novel method maintained the μTBS better than the control group after thermomechanical challenges, and it did not negatively affect water permeability of the bonding interfaces. CHX-loaded PAMAM regulated the calcium (Ca) and phosphate (P) ions provided by the ACP-containing adhesives to remineralize the demineralized dentin surfaces without initial Ca/P in the environment.The novel method can reduce the cytotoxicity of CHX, inhibit MMPs activities, maintain μTBS, and induce dentin remineralization, which are crucial factors for enhancing bonding durability.

The purpose of this study was to evaluate the influence of a contracted endodontic cavity (CEC) on dentin preservation, biomechanical property, and instrumentation efficacy of first permanent molars in comparison with a traditional endodontic cavity (TEC).Forty-eight extracted intact maxillary and mandibular first molars were selected and scanned by micro-computed tomographic (micro-CT) imaging. Each tooth type was anatomically matched and assigned to the CEC group or the TEC group (n = 12). After root canal instrumentation with the ProGlider (Dentsply Maillefer, Ballaigues, Switzerland) and WaveOne Gold (Dentsply Maillefer), the specimens were scanned by micro-CT again. Pre- and postpreparation micro-CT imaging was used to evaluate the dentin preservation and instrumentation efficacy. The results on volume and thickness reduction in the coronal dentin and pericervical dentin (PCD), root canal volume and surface area, percentage of unprepared surface area, canal transportation and centering ratio, and canal wall thickness in the "danger zone" were measured and analyzed with the Shapiro-Wilk and independent sample t tests. Based on micro-CT imaging of maxillary and mandibular first molars, CEC and TEC models were constructed on the intact tooth, and 2 different static loads were applied to the occlusal load points. The stress distribution patterns and von Mises stress on the occlusal surface and cervical region were assessed by 3-dimensional finite element analysis.The CEC had a significantly lower volume and thickness reduction of coronal dentin and PCD above the alveolar crest compared with the TEC group (P .05), whereas no difference was observed in PCD below the alveolar crest between the 2 groups (P.05). There was no difference regarding all instrumentation efficacy outcomes (root canal volume and surface area, percentage of unprepared surface area, canal transportation and centering ratio, and canal wall thickness) in the danger zone between the 2 groups (P.05). The CEC effectively reduced the maximum von Mises stress and stress concentration area on the occlusal surface and cervical region compared with the TEC.The CEC preserved more coronal dentin and PCD above the alveolar crest and thus reduced stress concentration on the occlusal surface and cervical region. The CEC had no significant adverse effects on the instrumentation efficacy compared with the TEC given that the instrumentation procedures were performed with ProGlider and WaveOne Gold files.

OBJECTIVES: To assess dentin collagen denaturation from phosphoric acid and enzyme treatments using collagen hybridizing peptide (CHP) and to investigate the effect of collagen denaturation on bio-stabilization promoted by proanthocyanidins (PA). METHODS: Human molars were sectioned into 7-μm-thick dentin films, demineralized, and assigned to six groups: control with/without PA modification, H(3)PO(4)-treated collagen with/without PA modification, enzyme-treated collagen with/without PA modification. PA modification involved immersing collagen films in 0.65% PA for 30 seconds. H(3)PO(4) and enzyme treatments were used to experimentally induce collagen denaturation, which was quantitated by fluorescence intensity (FI) from the fluorescently-conjugated-CHP (F-CHP) staining (n=4). FTIR was used to characterize collagen structures. All groups were subject to collagenase digestion to test the bio-stabilization effect of PA on denatured collagen using weight loss analysis and hydroxyproline assay (n=6). Data were analyzed using two-factor ANOVA and Games-Howell post hoc tests (α=0.05). RESULTS: FTIR showed collagen secondary structural changes after denaturation treatments and confirmed the incorporation and cross-linking of PA in control and treated collagen. F-CHP staining indicated high-degree, medium-degree, and low-degree collagen denaturation from H(3)PO(4)-treatment (FI=83.22), enzyme-treatment (FI=36.54), and control (FI=6.01) respectively. The aim was to investigate the fatigue performance of endodontically treated (ET) molars restored by various dentin-replacing materials and material configurations. Moreover, the impact of additional adhesive treatment with glass-ionomer cement (GIC) was evaluated.250 intact molars were collected and randomly distributed into ten groups (n = 25). After endodontic procedure standard Class I cavities were prepared and restored with different direct restorative techniques and dentin-replacing materials. Two-group were restored with either packable or flowable short fiber-reinforced composites (SFRCs). Two-group were restored by experimental fiber-reinforced GIC with and without adhesive treatment. Four-group were restored by conventional and resin-modified GICs with or without adhesive treatment. One-group was restored with a dual-cure composite resin and last group was restored with only conventional composite resin (control). Fatigue-survival was measured for all specimens using a cyclic-loading machine until fracture occurred or a number of 40.000 cycles were achieved. Kaplan-Meyer survival analysis was conducted, followed by pairwise log-rank post hoc comparisons. Fracture mode was then examined by means of optical microscopy and SEM.Group restored with flowable SFRC showed significantly higher survival (p 0.05) compared to all of the groups, except for group restored with packable SFRC (p 0.05). Group restored with fiber-reinforced GIC had significantly (p 0.05) higher survival rates compared to other commercial GICs. SEM demonstrated change of the fracture line when fracture reached the SFRC layer.Direct restoration of Class I in ET molars with the use of SFRCs as dentin-replacing materials demonstrated its ability to reinforce the dental structures and to increase the fatigue resistance in this specific clinical situation.

Canal instrumentation with rotary nickel-titanium (NiTi) instruments might weaken the dentinal wall. This study aims to investigate the apical root defects during canal instrumentation with two NiTi rotary systems by using optical coherence tomography (OCT) scans.Twenty mandibular incisors were selected and divided into two groups instrumented using HyFlex CM (HCM) and ProTaper Universal (PTU) systems. OCT scans were taken immediately after canal instrumentation with file #25, #30 and #40. Each cross-sectional reconstructed image of 1, 2 and 3 mm from the apex was examined, root cracks were scored, and dentin thickness was measured at 12 sites. The risk sites with dentin thickness less than 0.30 mm were recorded.In HCM group, no cracks were observed after instrumentation with file #30/.06, while in PTU group, root cracks were found in 6 teeth with a total score of 9 after using F3 (#30/.09) (P 0.05). After file #40, 8 teeth with cracks had a score of 14 in PTU group, while one tooth was observed crack with a score of 1 in HCM group (P 0.05). The number of risk sites in PTU group was 49 (13.16%), which was significantly more than the 28 (7.78%) in HCM group (P 0.05) when canals were instrumented with file #30.Within the limitations of this study, when the canals of mandibular incisors were instrumented with size #30 and #40 files, OCT scans showed less root defects in HCM group.

Regenerative endodontic treatment was introduced with the aim of reinforcing the root and enabling further root development. In the process of dentin mineralization, bioactive molecules are entrapped in the dentin matrix, which can be released later by rinsing the dentin with chelating agents. This study aimed to compare the effects of citric acid and EDTA on radiographic root development in regenerative endodontic treatment in an animal model.Premolars of 3 beagle dogs were used in this study. The teeth at 1 side of the jaw were irrigated with EDTA according to the regeneration protocol of the American Association of Endodontists. The teeth in the other quadrant were irrigated with citric acid in the second treatment session. Follow-up radiographs were obtained at 6 months, and the primary and final data were analyzed in SPSS Version 23 (IBM Corp, Armonk, NY) using the analysis of covariance.There was no significant difference between the 2 irrigating solutions regarding the increase in root length (P = .668); however, EDTA showed significantly superior efficacy in increasing the root dentin thickness compared with citric acid (P = .022).Within the limitations of this study, the use of citric acid versus EDTA was not superior on radiographic root development in regenerative endodontic treatment.

testing if hypothetical transverse centripetal strains due to polymerization contraction of luting materials produce differential alterations in its bonding to luted structures, depending on distances to the center of the luting mass, and if this effect is C-factor related. Two hypotheses were tested: (1) there is a statistically significant decreasing relationship between the bonding strength and the transverse distances to the center of the luting material, and (2) there is a statistically significant difference between bonding strengths among luting spaces with different configuration factors.10 PMMA (15 mm Ø) pairs of cylinders were cemented (Scotchbond Universal adhesiveRelyx Universal, both chemically cured) in a compliant setup under two (20 and 70 N) luting forces forming 2 groups (5 samples each), resulting in different C-factors. Whole samples were sectioned in x and y directions obtaining non-trimmed beams from all along the luting surfaces. Their relative positions in each sample were assessed before separating and categorized (10 categories) according to their distances to the center of the sample. All beams were tested in tension and, because of their uneven bonding areas and to balance its influence, UTS results were transformed into UTSANOVA's p of the regression UTSbonded attachment of cemented materials decreases from centers to outbounds in plane, extensive surfaces, and this decrease is C-factor related.

The aim was to compare the compressive strength of dentin after irrigation with different concentrations of sodium hypochlorite (NaOCl).A total of 259 extracted human tooth roots with no root caries or previous endodontic treatment were collected. They were sorted by size and length, and assigned to 7 groups (n = 37): negative control (no instrumentation, no irrigant), positive control (saline irrigant), 1%, 2.5%, 4%, 5.25%, and 10% NaOCl. A total of 13 mL NaOCl or saline (in 1-mL increments) was used per canal while being instrumented to #35 files (approximately 13 minutes). EDTAC (15%) was then used to remove the smear layer (2 minutes), followed by NaOCl as a final rinse. The roots were sectioned into 2-mm-thick disks and subjected to compression testing.There was a significant difference between the negative control group and all instrumented groups (P .05). The positive control group was significantly different from the 5.25% group (P .05) and had a low P value when compared with the 2.5% group and the other higher concentrations (P ≅ .1). When the NaOCl groups were compared with each other, there was a significant difference between 1% NaOCl and all higher concentrations (P .05), but no significant difference when 2.5% NaOCl was compared with higher concentrations.A concentration of 1% NaOCl had the least effect on the compressive strength of dentin and this was significantly different from all higher concentrations tested. Therefore, the use of concentrations of NaOCl above 1% should be reconsidered.

Mineralized Peyronie's plaque (MPP) impairs penile function. The association, colocalization, and dynamic interplay between organic and inorganic constituents can provide insights into biomineralization of Peyronie's plaque. Human MPPs (n = 11) were surgically excised, and the organic and inorganic constituents were spatially mapped using multiple high-resolution imaging techniques. Multiscale image analyses resulted in spatial colocalization of elements within a highly porous material with heterogenous composition, lamellae, and osteocytic lacuna-like features with a morphological resemblance to bone. The lower (520 ±179 mg/cc) and higher (1024 ± 155 mg/cc) mineral density regions were associated with higher (11%) and lower (7%) porosities in MPP. Energy dispersive X-ray and micro-X-ray fluorescent spectroscopic maps in the higher mineral density regions of MPP revealed higher counts of calcium (Ca) and phosphorus (P), and a Ca/P ratio of 1.48 ± 0.06 similar to bone. More importantly, higher counts of zinc (Zn) were localized at the interface between softer (more organic to inorganic ratio) and harder (less organic to inorganic ratio) tissue regions of MPP and adjacent softer matrix, indicating the involvement of Zn-related proteins and/or pathways in the formation of MPP. In particular, dentin matrix protein-1 (DMP-1) was colocalized in a matrix rich in proteoglycans and collagen that contained osteocytic lacuna-like features. This combined materials science and biochemical with correlative microspectroscopic approach provided insights into the plausible cellular and biochemical pathways that incite mineralization of an existing fibrous Peyronie's plaque. Statement of Significance Aberrant human penile mineralization is known as mineralized Peyronie's plaque (MPP) and often results in a loss of form and function. This study focuses on investigating the spatial association of matrix proteins and elemental composition of MPP by colocalizing calcium, phosphorus, and trace metal zinc with dentin matrix protein 1 (DMP-1), acidic proteoglycans, and fibrillar collagen along with the cellular components using high resolution correlative microspectroscopic techniques. Spatial maps provided insights into cellular and biochemical pathways that incite mineralization of fibrous Peyronie's plaque in humans.

OBJECTIVES: Universal adhesives (UAs) can optionally be applied in either an etch-and-rinse (E&R) or self-etch (SE) bonding mode. As the preferred bonding mode differs for enamel versus dentin, a universal conditioner for both enamel and dentin in replacement of the relatively aggressive phosphoric-acid etchant remains desirable. This study aimed to test if a metal salt-based etchant (ZrO(NO3)2) provides as durable bonding to dentin as a classic E&R or SE bonding mode METHODS: Before applying the UA Adhese Universal ('AdU'; Ivoclar Vivadent) to bur-cut dentin of 24 teeth (n = 8), dentin was conditioned with either (1) an experimental metal salt-based conditioner ('ZON'; Ivoclar Vivadent) or (2) 37% phosphoric acid (Total Etch gel, Ivoclar Vivadent), representing a classic 'E&R' mode; (3) a third experimental group involved AdU applied in SE mode. Bonding effectiveness was determined in terms of immediate ('1w') and aged ('50k' TC) micro-tensile bond strength (μTBS) to bur-cut dentin. Adhesive-conditioned dentin interfacial interactions were characterized by S/TEM. RESULTS: Linear mixed-effects modeling revealed significantly higher immediate μTBS to dentin of ZON_AdU than E&R_AdU, while ZON_AdU performed not significantly different from SE_AdU. No significant differences were found between the three experimental groups after 50k TC (aged μTBS). S/TEM disclosed less exposure of dentinal collagen fibrils when AdU was bonded upon ZON etching than when applied in E&R mode. Moreover, ZON resulted in more hydroxyapatite (HAp) crystals remaining at the bottom of the hybrid layer, while dentinal tubule orifices remained nearly fully closed, by which hardly any resin tags were formed. SIGNIFICANCE: The alternative metal salt-based conditioner revealed at dentin a more HAp-protected hybrid layer with less exposure of collagen fibrils, while a comparable bond strength was obtained to that with a phosphoric-acid E&R as well as with an SE (no conditioner) bonding mode. Whether ultrasonic activation of the adhesive system improves dentin tubule penetration and the bond strength of fiber posts to root dentin is unclear.The purpose of this in vitro study was to evaluate the effect of ultrasonic activation of 2 adhesive systems (etch-and-rinse and self-etch) and 1 glass ionomer cement on the dentin tubule penetration and pushout bond strength of fiber posts to root dentin.Sixty maxillary central incisors were endodontically treated and divided into 6 groups (n=10) as per the post cementation strategy: etch-and-rinse, etch-and-rinse and ultrasonic, self-etch, self-etch and ultrasonic, glass ionomer cement, and glass ionomer cement and ultrasonic. The primers, the adhesives, and the glass ionomer cement were activated for 20 seconds each, and the fiber posts were cemented with a resin cement. Dentin tubule penetration was evaluated by confocal laser scanning microscopy and the pushout bond strength measured at 3 post locations: cervical, middle, and apical. The failure patterns were also described after pushout testing.Self-etch and ultrasonic showed higher dentin tubule penetration than the other cementation strategies (P.05) and improved the bond strength values (P.05), which were higher than etch-and-rinse and ultrasonic and glass ionomer cement and ultrasonic (P.05). Adhesive failures at the cement and dentin interface were predominant in the etch-and-rinse, self-etch, and self-etch and ultrasonic groups.Ultrasonic activation improved the dentin tubule penetration of a self-etch adhesive system. The bond strength of fiber posts cemented with a self-etch adhesive system and a resin cement was improved after ultrasonic activation.

Background/purpose Sealer residues on dentin may affect bonding to restorative materials. This study aimed to evaluate the bond strength to sealer-contaminated dentin after using different cleaning protocols. Materials and methods Freshly extracted bovine incisors were prepared and exposed the buccal pulp chamber dentin, obtaining segments measuring 5 mm × 5 mm with a height of 3 mm. The segments were randomly distributed into 4 groups (n = 7) according to different protocols. Control group: no contamination was performed. In the three experimental groups, the segments were contaminated with epoxy resin-based sealer for 5 min, and different cleaning protocols were performed. Acetone group: acetone-saturated cotton pellets were used to wipe the sealer. Ultrasound group: ultrasonic ET-20D tip cleaning. Acetone combined with ultrasound group: cleaning with acetone-saturated cotton pellets and ultrasonic tip. All segments were bonded using a self-etch adhesive. Two samples in each group were scanned by swept-source optical coherence tomography (SS-OCT) to evaluate sealer residues. A microtensile test was performed on the remaining 5 samples, which were built up with composite resin. Results Sealer residues were observed in 3 of 14 (21.4%) sections of acetone group by SS-OCT. Compared to the control, ultrasound alone or in combination with acetone preserved the bond strength (P > .05). The ultrasound group exhibited the highest bond strength (39.38 MPa), which differed from that of the acetone group, which provided the lowest bond strength (32.88 MPa) (P Conclusion Cleaning epoxy resin-based sealer-contaminated dentin surfaces using ultrasound or combined with acetone could preserve the bond strength.

Aging is a physiological process with profound impact on the biology and function of biosystems, including the human dentition. While resilient, human teeth undergo wear and disease, affecting overall physical, psychological, and social human health. However, the underlying mechanisms of tooth aging remain largely unknown. Root dentin is integral to tooth function in that it anchors and dissipates mechanical load stresses of the tooth-bone system. Here, we assess the viscoelastic behavior, composition, and ultrastructure of young and old root dentin using nano-dynamic mechanical analysis, micro-Raman spectroscopy, small angle X-ray scattering, atomic force and transmission electron microscopies. We find that the root dentin overall stiffness increases with age. Unlike other mineralized tissues and even coronal dentin, however, the ability of root dentin to dissipate energy during deformation does not decay with age. Using a deconstruction method to dissect the contribution of mineral and organic matrix, we find that the damping factor of the organic matrix does deteriorate. Compositional and ultrastructural analyses revealed higher mineral-to-matrix ratio, altered enzymatic and non-enzymatic collagen cross-linking, increased collagen d-spacing and fibril diameter, and decreased abundance of proteoglycans and sulfation pattern of glycosaminoglycans . Therefore, even in the absence of remodeling, the extracellular matrix of root dentin shares traits of aging with other tissues. To explain this discrepancy, we propose that altered matrix-mineral interactions, possibly mediated by carbonate ions sequestered at the mineral interface and/or altered glycosaminoglycans counteract the deleterious effects of aging on the structural components of the extracellular matrix. STATEMENT OF SIGNIFICANCE: Globally, a quarter of the population will be over 65 years old by 2050. Because many will retain their dentition, it will become increasingly important to understand and manage how aging affects teeth. Dentin is integral to the protective, biomechanical, and regenerative features of teeth. Here, we demonstrate that older root dentin not only has altered mechanical properties, but shows characteristic shifts in mineralization, composition, and post-translational modifications of the matrix. This strongly suggests that there is a mechanistic link between mineral and matrix components to the biomechanical performance of aging dentin with implications for efforts to slow or even reverse the aging process.

Objective This study investigated the chemical and structural changes in the mineral phase and collagen of dentin during application of a mild universal adhesive. Particular attention was paid to the role of isopropanol and changes in water molecules. Methods In vitro application of the mild universal adhesive on dentin with two established etching modes (self-etch and etch-and-rinse) was studied using solid state nuclear magnetic resonance spectroscopy. Results It was evidenced that the etch-and-rinse mode leads to a decrease of the inorganic apatite and a reorganization of the residual mineral phase with a low amount of adhesive phosphate monoesters calcium salt formed, compared to the self-etch mode. In contrast, the adhesive interacts very similarly to the level of dentin collagen in both protocols, with a strong decrease in the amount of the free water molecules induced by the presence of isopropanol as the adhesive solvent, but without significant changes in the initial collagen structure. For both modes, the adhesive acrylates monomers remain mobile and can infiltrate the collagen. Significance Understanding the molecular interactions between dentin and adhesive solutions is a major challenge for designing products that lead to the formation of ideal dentin resin hybrid layer. Notably, one point considered essential is the presence of unbound water which, over time, is associated with a hydrolytic degradation of the organic matrix. Isopropanol, as an adhesive solvent, leads to a decrease in the amount of the less stable water molecules while the water molecules strongly attached to the collagen are retained, thus preserving the collagen structure.

OBJECTIVES: To evaluate the effects of different polyphenols and solvents on dentin collagen’s crosslinking interactions and biostabilization against MMPs and collagenase degradation. METHODS: Two polyphenols [proanthocyanidin (PA) and quercetin (QC)] with different water solubility were prepared as treatment solutions using ethanol (EtOH) or dimethyl sulfoxide (DMSO) as solvents. 6-um-thick dentin films were microtomed from dentin slabs of third molars. Following demineralization, films or slabs were subject to 60-s treatment (PA or QC) or no treatment (control) with subsequent extended-rinse with original solvent (EtOH or DMSO) or distilled water (DW). Collagen crosslinking interactions were assessed by FTIR. Biostability was assessed through endogenous MMPs activity via confocal laser scanning microscopy, and exogenous collagenase degradation via weight loss, hydroxyproline release and SEM. Finally, direct collagenase inactivation was also evaluated. Data were analyzed by three-way ANOVA and post-hoc tests (α=0.05%). RESULTS: Distinct effects of two polyphenols and solvents on collagen crosslinking and biostabilization were observed. Higher crosslinking and biostability efficacy occurred with PA than QC (p < 0.001) that demonstrated negligible collagen interactions. With DMSO solvent, efficacy results were significantly reduced with both polyphenols (p < 0.05). DMSO-rinse further weakened interactions of PA with collagen, diminishing biostability (p < 0.05). Low biostability was detected with QC and DW-rinse, suggesting direct enzymatic inhibition due to physical presence in collagen. SIGNIFICANCE: Collagen crosslinking interactions and biostability depend on polyphenol chemical characteristics. Treatment-solution solvents may affect interactions between polyphenols and collagen, specifically, DMSO showed detrimental effects on collagen crosslinking and biostability and should be used with caution.

Aims The aim of this study was to develop and characterize a temporary restorative material based on a zinc oxide matrix containing niobophosphate bioactive glass (NbG) for the caries-affected dentin treatment. Material and methods NbG was added to a ZnO2 matrix in different concentrations (wt%). EDS-SEM, ATR-FTIR and XRD analyses were performed to characterize the cement. Calcium release was evaluated in TRIS solution after 1, 7 and 14 days by colorimetric method (A650). Compressive strengths and setting times were performed to analyze mechanical properties. Results EDS spectra confirmed the presence of Ca, P and Nb in the groups containing NbG. EDS mapping exhibit the ZnO2 homogeneous distribution, and NbG immersed in this matrix. Peaks suggesting interaction between matrix and NbG were not detected in Ftir spectra. Calcium releasing showed to be time-dependent for experimental groups containing 10, 20, 30 and 40%. The NbG incorporation progressively increased the compressive strength values in the experimental groups. NbG incorporation seemed to influence the ZnO2 matrix early setting reaction. No statistical difference was observed in the final setting time. Conclusion The addition of NbG particles into zinc oxide matrix could work as a mechanical reinforcement. It is suggested that the calcium released by the cement containing at least 10% NbG could induce apatite formation.

Purpose: To verify the color change and contrast ratio of resin composites after curing and after 30 days of storage in water. Methods: Dentin A2 shades of different light-cured dental resin composites (Vittra APS, FGM, Brazil; Z350 XT, 3M ESPE, EUA; Tetric N-Ceram, Ivoclar Vivadent, Liechtenstein, and Charisma Diamond, Heraeus Kulzer, Germany) were tested. Ten rounded specimens (8 mm × 2 mm) were prepared for each material. Reflectance for all samples was obtained using a spectrophotometer (Minolta CM 3700d, Konica Minolta, Japan) before curing, immediately after curing, and after 30 days of storage in water. The color change (ΔE*lab) and contrast ratio (CR) data were analyzed using a one-way analysis of variance with Tukey's and paired t-tests (α = 1%). Results: For all materials tested, significant color changes were noticeable after curing and after 30 days in water (p 0.01). Conclusion: The CR values and color changes after curing and 30 days of storage in water varied depending on the material tested. This study corroborates the clinical practice of curing a small amount of unpolymerized resin composite on the tooth surface to select the desired shade before undertaking esthetic restorative procedures.

Dentin hypersensitivity (DH) is characterized by short or transient sharp pain arising from exposed dentin in response to thermal, physical, osmotic, or chemical elements (Holland et al., 1997, West et al., 2011). According to the hydrodynamic theory, hypersensitivity results from the rapid movement of the fluid within the dentinal tubules (Brannstrom et al., 1968). It is imperative to the efficacy of the DH treatment to address the etiological factors of the condition (Femiano et al., 2017). Desensitizing therapies consist of the use of chemical substances, laser irradiation, restorative treatments, periodontal surgeries which aim basically to control the hydrodynamic mechanisms of pain. However, these treatments do not achieve predictable results in all patients and may lose their effect over time (Cartwright, 2014, Zhu et al., 2015, Sole-Magdalena et al., 2017). Bioactive glass materials have been proposed for the treatment of DH (David, 2010, Curtis et al., 2010, Barry et al., 2011, Sauro et al., 2011, Cartwright, 2014, Hall et al., 2017, Bansal and Mahajan, 2017). The mechanism of action of bioactive glass starts when the material is exposed to an aqueous environment. The sodium ions (Na+) in the particles immediately are switched with hydrogen cations (H+ or H3O+). This rapid release of ions allows the calcium ions (Ca+2) in the particle structure, as well as the phosphate ions (PO4-3), to be released from the glass particles. The initial reactions occur in seconds of exposure and the release of the Ca+2 and PO4-3 ions continues as long as the particles are exposed to an aqueous environment. A localized and transient increase in pH occurs during the initial material exposure due to the release of Na+. This increase in pH helps to precipitate the Ca+2 and PO4-3 ions of the particle to form a layer of calcium phosphate Ca(PO4)2. As particle reactions and the deposition of calcium and phosphorus complexes are maintened, the layer crystallizes into hydroxycarbonate apatite which is chemically and structurally equal to biological apatite. The combination of residual particles and the newly formed apatite layer lead to physically occluding the dentin tubules, resulting in remission or pain reduction (Zhong et al., 2002, Zanotto et al., 2004, Hench, 2006). The maintenance of this layer on the dentin surface is a major challenge for the materials existing in the market, besides knowing the effectiveness of them for the treatment of DH. Studies have shown the absence of parameters such as: patient follow-up time, control groups, assessment scale for degree of pain, DH diagnostic methods and form of application of the product (Matranga et al., 2017, Hannigan and Lynch, 2013, Kim et al., 2011, Pandis et al., 2011, Fleming et al., 2013). The use of bioactive materials is a promising proposal for DH due to the the neoformation of a bioactive barrier on dentin surface (Bansal and Mahajan, 2017). They showed the ability to occlude the dentinal tubules and form a mechanically strong layer of hydroxyapatite on the dentin surface, which can resist degradation by repeated acid challenges (Du et al., 2008). Thus, the aim of this systematic review was to assess randomized controlled trials (RCT) carried out in the permanent teeth of adult patients in the DH treatment with bioactive dentrifices, either at-home or in-office.

Purpose This study compared microleakage of different resin based composite (RBC) materials bonded to dentin, after chlorhexidine (CHX) application, by different adhesion protocols of a universal adhesive system. Methods Class V cavities were prepared on the buccal and lingual surfaces of 40 premolar teeth. The “etch-and-rinse” technique of a universal bond adhesive system (Single Bond Universal Adhesive) was used on buccal preparations, while the “self-etch” protocol was used on the lingual surfaces. Two RBCs, one bulk fill (Filtek Bulk Fill [FBF]) and one conventional (Filtek Z350 XT [Z350XT]), were used. Teeth were divided into two groups of 20 teeth each, 10 per each RBC (n = 10): (1) control; and (2) pretreatment with 2% CHX. For FBF groups, teeth were restored with a single increment; however, for Z350XT, a layering technique was used. Teeth were aged by thermo-cycling and prepared for microleakage testing. Dye penetration was evaluated and scored from 0 to 4. Data were analyzed at a significance level of P Results The highest microleakage mean scores were found in the control group of the etched margins for both RBCs (2.80 ± 1.033 FBF and 2.10 ± 1.370 Z350XT). The CHX-pretreated group showed significantly lower microleakage than the control for FBF only (P = 0.008). No significant difference was found between groups for the “self-etch” protocol (χ2 = 0.884, P = 0.08). No significant differences were found between FBF and Z350XT in all study groups (P > 0.2). Conclusions When the “self-etch” protocol of the universal adhesive system was used, dentin microleakage was not affected by CHX-pretreatment when teeth were restored with bulk fill or conventional RBCs. In the “etch-and-rinse” protocol, CHX application improved the marginal seal before restoration with bulk fill material. However, in the absence of CHX, the “etch-and-rinse” protocol negatively affected marginal integrity.

Purpose: To evaluate the effect of phytic acid (IP6) on the surface roughness and microhardness of human root canal dentin and compare it to other smear layer removal agents. Materials and methods: Fifty extracted human maxillary incisors were sectioned longitudinally into a total of 100 specimens followed by embedding in auto-polymerizing acrylic resin. The specimens were polished and then randomly divided into five groups (n = 20) according to the test solution used to condition root canal dentin: 17% ethylenediaminetetraacetic acid (EDTA); 10% citric acid (CA); 1% IP6; 37% phosphoric acid (PA); or distilled water (control group). Each specimen was treated with a total volume of 1 ml of each solution for 1 min with agitation. Each group was then divided into two subgroups of 10 specimens each. The specimens of the first subgroup were used to determine microhardness, using Vickers hardness tester, and the specimens of the second subgroup were used to measure surface roughness, using a confocal laser scanning microscope. The results were analyzed statistically using one-way ANOVA and Tukey tests, α = 0.05. Results: All the tested groups exhibited microhardness and surface roughness values that were statistically significantly different when compared with the control group (P 0.05). Conclusions: IP6 and PA showed the lowest microhardness and the highest surface roughness values.

The purpose of this study was to assess the antimicrobial activity of root canal sealers modified with novel highly loaded antimicrobial drug-silica coassembled particles (DSPs) on Enterococcus faecalis-infected root canal dentin.DSPs were synthesized through coassembly of silica and octenidine dihydrochloride (OCT) surfactant drug (35% w/w OCT). DSPs (1% wt of the total mass of the sealer) were mixed homogenously with either epoxy resin sealer (AH Plus [AH]; Dentsply Sirona, Tulsa, OK) or calcium silicate-based sealer (EndoSequence BC Sealer [BC]; Brasseler, Savannah, GA). To assess the antimicrobial activity of DSP-loaded sealers, the apical third of single-rooted teeth was obtained and infected with E. faecalis for 3 weeks followed by the application of experimental (DSP-loaded) sealers or corresponding controls for up to 28 days. Microbiological analysis and laser scanning confocal and scanning electron microscopy were used to determine the colony-forming unit (CFU)/mL, the percentage of live bacteria, and the intratubular bacterial and sealer penetrations. Factorial analysis of variance and Tukey post hoc tests were used to assess the antimicrobial effect of DSPs on different sealers.All experimental groups showed significant reductions in CFUs at all-time points compared with positive controls (P .05). The addition of DSPs to BC significantly reduced the CFUs (2.11 ± 0.13, 2.22 ± 0.19, and 2.25 ± 0.17 at 1, 7, and 28 days, respectively) compared with the unmodified sealer (3.21 ± 0.11, 4.3 ± 0.15, and 4.2 ± 0.2 at 0, 7, and 28 days). DSPs enhanced the antimicrobial performance of AH only at 1 day (4.21 ± 0.17 vs 5.19 ± 0.12, P .05). AH and AH + DSPs showed higher bacterial viability compared with BC and BC + DSPs at all incubation periods (P .05).Loading endodontic sealers with DSPs had a material-dependent effect on the antimicrobial properties and could reduce the incidence of secondary infections.

OBJECTIVE This study tested the effects of small leucine-rich proteoglycan (SLRP) proteins on phosphoric acid (PA)-treated dentin bonding overtime and the role of such SLRPs in the remineralization potential of demineralized dentin collagen. METHODS Coronal dentin sections of human molars were used. SLRPs were either decorin (DCN) or biglycan (BGN) in core or proteoglycan form (with glycosaminoglycans, GAGs). Groups were: No treatment (control), DCN core, DCN + GAGs, BGN core, BGN + GAGs. Samples were etched with PA for 15 s and prior to application of Adper Single Bond Plus and composite buildup an aliquot of the specific SLRPs was applied over dentin. Twenty-four hours or 6 months after the bonding procedure, samples were tested for microtensile bond strength (MTBS). Debonded beams were analyzed by scanning electron microscopy (SEM). For remineralization studies, dentin blocks were fully demineralized, infused with the SLRPs, placed in artificial saliva for 2 weeks, and evaluated by transmission electron microscopy (TEM). RESULTS MTBS test presented a mean of 51.4 ± 9.1 MPa in control with no statistically significant difference to DCN core (47.6 ± 8.3) and BGN core (48.3 ± 6.5). The full proteoglycan groups DCN + GAGs (27.4 ± 4.5) and BGN + GAGs (36.4 ± 13.6) showed decreased MTBS compared to control (p < 0.001). At 6 months, control or core-treated samples did not have a statistically significant difference in MTBS. However, SLRPs with GAGs showed statistically significant improvement of bonding (62.5 ± 6.0 for DCN and 52.8 ± 8.1 for BGN, p < 0.001) compared to their baseline values. SEM showed that GAGs seem to favor water retention but overtime help remineralization. TEM of demineralized dentin indicated a larger collagen fibril diameter pattern of samples treated with core proteins compared to control and a smaller diameter with DCN + GAGs in water with evidence of mineralization with DCN + GAGS, BGN core and BGN + GAGs. SIGNIFICANCE In conclusion, core proteins seem not to affect dentin adhesion significantly but the presence of GAGs can be detrimental to immediate bonding. However, after ageing of samples, full proteoglycans, particularly DCN, can significantly improve bonding overtime while promoting remineralization which can prove to be clinically beneficial.

Objectives The objective of this study was to determine the effects of antimicrobial photodynamic therapy (aPDT) with indocyanine green (ICG) and toluidine blue (TB) on protease activity (matrix-bound cathepsin K and matrix metalloproteinase (MMP) and dentin bond strength. Methods Caries-free human third molars were assigned to five groups: 1—control group, 2—application of ICG with activation using an 810 nm diode (aPDT), 3—application of ICG, 4—application of TB with activation using a 660 nm diode (aPDT), and 5—application of TB. For the enzymatic investigation, dentin beams were incubated for either 3 days or 3 weeks. Aliquots of the incubation media were analyzed by ELISA for CTX (C-terminal cross-linked telopeptide of type I Collagen) and ICTP (cross-linked carboxy-terminal telopeptide of type I collagen). For microtensile bond strength testing (μTBS), composite resins were layered onto the tooth surface; the samples were then subjected to μTBS. Kruskall–Wallis and Mann–Whitney U tests were applied for statistical analysis of CTX and ICTP, one way-ANOVA and Tukey’s test were applied for statistical analysis of μTBS. Results Pretreating the dentin matrices with aPDT decreased the endogenous protease activity. ICG with laser activation resulted in the highest μTBS. Therefore, aPDT should be considered as a treatment method because it can reduce MMP-mediated dentin degradation and increase the μTBS. Significance Inhibiting endogenous protease activity improves the stability of the dentin–adhesive bond and the durability of the bond strength.

Objective Resin-based dental adhesion is mostly utilized in minimally invasive operative dentistry. However, improving the durability and stability of resin–dentin bond interfaces remain a challenge. Graphene quantum dots (GQDs) reinforced by 1-ethyl-3-(3-dimethylaminopropyl) carbodiimide (EDC) were introduced to modify the resin–dentin bond interfaces, thereby promoting their durability and stability. Methods GQDs, EDC, and EDC+GQDs groups were designed to evaluate the effects of GQDs and EDC on collagenase activity, the interaction of GQDs with collagen, and the resin–dentin interface. First, the effects of GQDs and EDC on collagenase activity was evaluated by Collagenase (EC 3.4.24.3) reacting with its substrate. The interaction of GQDs and EDC with collagen were evaluated by cross-linking degree analysis, sodium dodecyl sulfate–polyacrylamide gel electrophoresis, attenuated total reflection Fourier transform infrared spectroscopy and enzymatic hydrolysis. Second, the acid-etched and rinse adhesive system was used to evaluate the resin–dentin bond on the basis of microtensile bond strength, in situ zymography and fluorescence confocal laser scanning microscopy. Results GQDs could inhibit collagenase activity. GQDs with the aid of EDC could cross-link collagen via covalent bonds and improve the anti-enzymatic hydrolysis of collagen. In the resin–dentin adhesion model, the μTBS of the EDC+GQDs group was significantly higher than the other control groups after thermocycling. The addition of EDC to GQDs could inhibit matrix metalloproteinase activity and promote the integrity of the bonding interfaces after thermocycling. Significance This study presents a novel strategy to modify the resin–dentin interface and provides a new application for GQDs. This strategy has the potential to improve the durability of resin-based restoration in dentistry.

The limited durability of dentin bonding harshly shortens the lifespan of resin composites restorations. The controlled, dynamic movement of materials through non-contacting forces provides exciting opportunities in adhesive dentistry. We, herein, describe comprehensive investigations of a new dental adhesive with superparamagnetic iron oxide nanoparticles (SPIONs) sensitive to magnetic fields for bonding optimization. This contribution outlines a roadmap of (1) designing and tuning of an adhesive formulation containing SPIONs to enhance penetrability into etched dentin guided by magnetic-field; (2) employing a clinically relevant model of simulated hydrostatic pulpal pressure on the microtensile bond to dentin; and (3) investigating a potential antibacterial effect of the formulated adhesives, and their biocompatibility. SPION-concentration-dependency chemical and mechanical behavior was shown via the degree of conversion, ultimate tensile strength, and micro shear bond strength to dentin. The effects of SPIONs carried on a dental adhesive on the bonding strength to dentin are studied in depth by combining experiments with in vitro simulated model. The results show that under the guided magnetic field, 0.07 wt.% of SPIONs-doped adhesive increased the bond strength that surpasses the reduction caused by hydrostatic pulpal pressure. Using a magnetic guide workflow during the bonding procedures, SPIONs-doped adhesives improved dentin's adhesion without changing adhesives’ physicochemical properties. This outcome addresses the key challenge of poor resin infiltration of dentin's conventional total etching during the bonding procedure. The real-time magnetic motion of dental adhesives may open new paths to enhance resin-based restorations’ longevity. Statement of significance In this study, dental adhesives containing superparamagnetic iron oxide nanoparticles (SPIONs) were developed to enhance penetrability into dentin guided by a magnetic field. The adhesives were screened for physical, chemical, antibacterial properties, and cytotoxicity. For the first time, simulated pulpal pressure was used concurrently with the magnetic field to simulate a clinical setting. This approach showed that it is feasible to overcome pulpal pressure jeopardization on bond strength when SPIONs and a magnetic field are applied. The magnetic-responsive adhesives had great potential to improve bond strength, opening new paths to enhance resin-based restorations' longevity without affecting adhesives' biological properties. The use of magnetic-responsive particles and magnetically assisted motion is a promising strategy to improve the sealing ability of dental adhesives.

Objective Existing agents to induce enamel self-repair and inhibit the progression of dental caries in the early stage have been proven to be inadequate and far from satisfactory. In this study, a honokiol-loaded poly(amido amine) (PAMAM) dendrimer (PAMH) was constructed to combat early caries lesions in enamel. Methods PAMH was prepared via a codissolution method. Computational simulation analysis was used to explore the mechanism of honokiol release. The cytotoxicity of PAMH was tested. The antibacterial effects of PAMH were tested by planktonic growth assays and biofilm formation inhibition assays. The remineralization effect of PAMH was examined via transverse microradiography and scanning electron microscopy after a pH cycling model. The in vivo anti-caries effect of PAMH was carried out in a rat model. Results Honokiol released from PAMH was slower but more durable in a cariogenic pH environment than in a neutral pH environment, which could be explained through the computational simulation analysis results. Under electrostatic action, P3 beads with the same charge repelled each other and extended outwards, resulting in the rapid expansion of the PAMAM dendrimer and accelerating the release of the drug. At a low pH of 5.5, the protonated P3 beads were not charged and the protonated P1 beads were positively charged. However, the electrostatic repulsive interaction between protonated P1 beads was restricted by the P3 beads in the outermost layer of the PAMAM dendrimer, so the swelling rate was relatively slow, resulting in the slow release of drug molecules in the acidic environment. The cytotoxicity demonstration and the biocompatibility experiment in animal study showed that PAMH is biologically safe. PAMH showed excellent enamel remineralizing ability after pH cycling and showed a long-term antibacterial effect in vitro. Meanwhile, PAMH showed long-term anticaries efficacy in vivo. Significance Our findings indicated that PAMH had great potential to combat early caries lesions in enamel for future clinical application.

A straightforward technique is presented for an interim or short-term definitive esthetic replacement of missing anterior teeth requiring no tooth preparation. Composite resins are injected into transparent silicone indices fabricated from 3-dimensional–printed casts of a digital waxing. The dentin core is formed of a durable short fiber–reinforced injectable composite resin and veneered with an enamel-shade composite resin for enhanced esthetics. Besides being noninvasive, this technique is more straightforward than traditional options, reducing chair time while providing an accurate outcome.

The aim of this study was to summarize and systemically review the different types of bioceramics that have been proposed to be used as direct dental pulp capping materials for dentin regeneration in permanent teeth. This article reviewed the studies carried out on the bioceramic materials as dental pulp capping agents. PubMed, Embase, Cochrane Central Register of Controlled Trials, and Scopus electronic databases were searched with the selected keywords. The retrieved articles were evaluated by 2 reviewers according to the inclusion and exclusion criteria and the extracted data were arranged in Tables. After removing duplicate articles and further screening, 3126 articles were retrieved in this review. Following discussing the composition and characteristics of different types of bioceramics reported in the literature, 47 articles that met the inclusion criteria were included in this systematic review. The results of this review showed that certain types of calcium silicate-based materials, mineral trioxide aggregate (MTA), and Biodentine seem to be the most effective bioceramics currently used for direct pulp capping in permanent teeth compared with that of other types of bioceramics. In addition, different types of calcium phosphate materials like tricalcium phosphates and bioactive glasses have been recently introduced as alternative bioceramics for dental pulp capping. Calcium phosphates might be considered as more appropriate pulp capping materials in future because of their unique characteristics. However, because of lack of promising information, only further well-designed in vitro and long term clinical studies are required to confirm the most appropriate type of bioceramics for direct dental pulp capping procedures.

Objectives The purpose of the study was to evaluate the integrity of dentine type I collagen after self-etching (SE) treatments with strong and mild universal adhesives. Methods Coronal dentine specimens (n = 10/product) were imaged by optical microscopy and analyzed by ATR-FTIR spectroscopy before and after treatment with 32% phosphoric acid gel (PA-negative control), 17% neutral EDTA (ED-positive control) conditioners and Adhese Universal (AD), Clearfil Universal Bond Quick (CQ), G-Premio Bond (GP), Prelude One (PR) and Scotchbond Universal (SB) adhesives. From the spectroscopic analysis the following parameters were determined: a) Extent of dentine demineralization (DM%) and b) percentage area of the Amide I curve-fitted components of β-turns, 310-helix/β-turns, α-helix, random coils, β-sheets and collagen maturation (R) index. Statistical analysis was performed by one-way ANOVA (DM%), paired t-test/Wilcoxon test (Amide I components) and Spearman correlation coefficient (DM% vs Amide I components) at an a = 0.05 level. Results PA, ED and GP removed the smear-layer and opened tubule orifices, whereas all other treatments removed only the intratubular smear-layer fraction. The ranking of the statistically significant differences in DM% was PA > GP > ED > AD, SB, CQ, PR, with AD being significantly different from PR. Regarding the Amide I components, PA demonstrated a significant reduction in β-turns, α-helices and an increase in β-sheets, GP a reduction in β-turns, AD an increase in β-turns and random coils, and CQ an increase in β-turns. PR, SB and ED showed insignificant differences in all the Amide I components. Significant correlations were found between DM%-random coils and DM%-R. Significance The universal adhesives used in the SE mode induced none to minimal changes in dentine collagen structure, without evidence of the destabilization pattern observed after conventional phosphoric acid treatments.
