Many available software packages do this, but we show that very different results can be obtained from different packages even when using the same data and model. Seven different fitting packages that run on four different platforms are compared using various data functions and data sets that reveal there are stark differences between the packages. In addition to comparing the prediction accuracy, the predictive variance-which is important for evaluating precision of predictions and is often used in stopping criteria-is also evaluated.

Introduction Unlike previous technologies that lack the diagnostic performance to reliably detect mosaicism for preimplantation genetic testing for aneuploidy (PGT-A), next generation sequencing (NGS) based platforms now allow for the detection of lower-level mosaicism. Due to this increased sensitivity, thresholds need to be put in place to classify embryos as euploid, aneuploid, or mosaic. The lower cutoff for mosaic samples ranges from 20 % to 30 %, and the upper cutoff for mosaic samples ranges from 70%-80% in most commercial PGT laboratories in the field today. These varying cutoffs will lead to varying percentages of mosaic embryos reported, suggested to be somewhere between 20 %. A more stringent threshold could result in greater pregnancy rates. Here we compare clinical outcomes and embryo reporting between two different PGT-A reference laboratories that use different testing platforms and different mosaic cutoffs. Materials and Methods Retrospective analysis of PGT-A results (n=623) and frozen embryo transfer (FET) cycles (n=99) using embryos diagnosed by either Cooper Genomics or Igenomix between January 2018 - January 2019. Cooper Genomics uses Illumina's MiSeq platform with Bluefuse software and thresholds of 20% to 80 % (lower to upper cutoff) while Igenomix uses ThermoFisher's Ion Torrent platform with Ion Reporter software and thresholds of 30% to 70 %. Patients with a history of recurrent miscarriage were excluded from this study. Results Fifty six (56) cycles included embryos analyzed by Cooper Genomics and 43 cycles included embryos analyzed by Igenomix as a PGT-A reference laboratory. There was no differences in maternal age or number of embryos replaced per transfer between the two cohorts. IVF outcome was similar between the two groups: The pregnancy (+FHB) observed in cycles that transferred embryos diagnosed as euploid by Cooper Genomics was 67.9% (38/56) compared to 69.8% (30/43) with Igenomix. Miscarriage rate was also similar, 8.9% (5/56) and 6.9% (3/43) respectively. Cooper Genomics reported a much higher rate of mosaicism (17.0%) compared to Igenomix (4.9%) (p Conclusion Retrospective analysis of PGT-A+FET cycles revealed there was no differences in pregnancy or miscarriage rates indicating a similar diagnostic efficacy achieved by the two PGT-A platforms. Although the embryos diagnosed as euploid by the two platforms appeared to have the same potential to generate a pregnancy, the methodology and higher threshold utilized by Igenomix resulted in significantly less mosaic embryos and a slightly greater percentage of euploid embryos available for transfer. It is assumed that cumulative pregnancy rates between the two reference laboratories would vary based on the finding that there were more euploid embryos available from Igenomix. Results from subsequent FET cycles will be collected and further examined. NGS has now created three categories of embryos that exist after PGT-A, the interpretation of the millions of data points generated by this technology still remains a challenge and it is important that the diagnostic methodology used does not overestimate the degree of mosaicism.

Transient receptor potential vanilloid 1 (TRPV1) was shown to modulate hippocampal CA1 pyramidal cell synaptic plasticity, including long-term potentiation (LTP) and long-term depression (LTD). Synaptic plasticity is the cellular mechanism thought to mediate declarative learning and memory in the hippocampus. Although TRPV1 is involved in modulating hippocampal plasticity, it has yet to be determined how TRPV1 mediates its effects. Using field electrophysiology in hippocampal CA1 stratum radiatum we investigated how TRPV1 agonists modulate LTP, low frequency stimulation-induced LTD, and (RS)-3,5-dihydroxyphenylglycine (DHPG)-induced LTD. First we confirmed that TRPV1 agonists induce enhancement of CA1 pyramidal cell LTP in the absence the GABAA receptor antagonist picrotoxin. Because it was recently determined that TRPV1 mediates a novel form of LTD in CA1 inhibitory GABAergic interneurons, which can disinhibit CA1 pyramidal cells, we used picrotoxin to block the effect of the GABAergic circuitry on CA1 LTP. When using picrotoxin, the TRPV1 agonist-induced enhancement of CA1 LTP was eliminated suggesting that the GABAergic circuitry is required for TRPV1 agonist mediated increases. Regarding LTD, in contrast to previously reported data, we did not see TRPV1 agonist-mediated effect on low frequency-induced stimulus LTD. However, during DHPG-induced LTD, TRPV1 was involved in the acute, but not the long-term depression phase of this plasticity. In summary, our findings support TRPV1 agonist involvement in hippocampal synaptic plasticity, including its enhancement of CA1 LTP. We demonstrate that the enhancement mediated by TRPV1 agonists requires GABA input to pyramidal cells thus providing a mechanism for how TRPV1 agonists modulate hippocampal synaptic plasticity.

Alcohol, tobacco, and to a lesser degree, cannabis all have been a part of the history and culture of the United States since before the founding of our nation, yet these substances are regulated in very different ways today. Some advocate that cannabis should be regulated in the same way that alcohol and tobacco are regulated, while others are strongly opposed. With alcohol and tobacco both causing far more fatalities each year than marijuana and with addiction rates higher for alcohol and tobacco than marijuana, one cannot help but wonder why alcohol and tobacco are merely regulated, while marijuana is illegal on the federal level under the most restrictive category, as a Schedule I substance under the Controlled Substances Act. This curiosity is deeply tied into the history of these drugs and several aspects of human psychology. In the meantime we are left to wonder, what’s up with that?

OBJECTIVE: Corticotropin-releasing hormone and gonadotropin-releasing hormone are produced by the human placenta and have been measured in the maternal circulation during pregnancy. Our objective was to determine concentrations of these substances in maternal plasma throughout normal pregnancies and in early pregnancy loss. STUDY DESIGN: Fifty-one pregnant women were followed up prospectively and plasma samples were drawn at 8, 10, 12, 14, 16, 28, and 36 weeks' gestation and during labor. Specific and sensitive radioimmunoassays were used to determine corticotropin-releasing hormone and gonadotropin-releasing hormone concentrations in these samples. RESULTS: Blood samples were drawn at all time points and outcome data were available from 33 women who completed their pregnancies at term without complications. In this normal group circulating corticotropin-releasing hormone concentrations increased from low or undetectable concentrations at 8 weeks (≤23.2 ± 1.3 pg/ml, mean ± SEM) to measurable values at 16 weeks (34.3 ± 2.2 pg/ml). Thereafter there was a significant increase to 1294 ± 113 pg/ml in labor. Gonadotropin-releasing hormone demonstrated a trimodal distribution, increasing significantly from 8 to 14 weeks, decreasing at 16 weeks, and increasing again by term. The ratio of corticotropin-releasing hormone to gonadotropin-releasing hormone in the normal group demonstrated a 30-fold increase from 8 weeks to term. In eight cases of early pregnancy loss corticotropin-releasing hormone and gonadotropin-releasing hormone concentrations were not significantly different from those of the normal group in early pregnancy. In two cases of premature delivery gonadotropin-releasing hormone concentrations and ratios were within the normal range; corticotropin-releasing hormone levels were normal in both cases of premature delivery. CONCLUSION: In this study we determined maternal concentrations of corticotropin-releasing hormone and gonadotropin-releasing hormone in normal pregnancies and in labor at term. Neither maternal concentrations of corticotropin-releasing hormone nor gonadotropin-releasing hormone were useful in identifying pregnant women at risk for early pregnancy loss.
