Background: Perioperative atrial fibrillation (AF) and myocardial injury after noncardiac surgery (MINS) are common complications after noncardiac surgery. Inflammation has been implicated in the pathogenesis of both disorders. The COP-AF trial tests the hypothesis that colchicine reduces the incidence of perioperative AF and MINS in patients undergoing major noncardiac thoracic surgery. Design: The 'COlchicine for the Prevention of Perioperative Atrial Fibrillation' (COP-AF) trial is an international, blinded, randomized trial that compares colchicine to placebo in patients aged at least 55 years and undergoing major noncardiac thoracic surgery with general anesthesia. Exclusion criteria include a history of AF and a contraindication to colchicine (e.g., severe renal dysfunction). Oral colchicine at a dose of 0.5 mg or matching placebo is given within 4 hours before surgery. Thereafter, patients receive colchicine 0.5 mg or placebo twice daily for a total of 10 days. The two independent co-primary outcomes are clinically important perioperative AF (including atrial flutter) and MINS during 14 days of follow-up. The main safety outcomes are sepsis or infection and non-infectious diarrhea. We aim to enroll 3,200 patients from approximately 40 sites across 11 countries to have at least 80% power for the independent evaluation of the two co-primary outcomes. Summary: COP-AF is a large randomized and blinded trial designed to determine whether colchicine reduces the risk of perioperative AF or MINS in patients who have major noncardiac thoracic surgery. For an arbitrary valued field $(K,v)$ and a given extension $v(K^*)\hookrightarrow\Lambda$ of ordered groups, we analyze the structure of the tree formed by all $\Lambda$-valued extensions of $v$ to the polynomial ring $K[x]$. As an application, we find a model for the tree of all equivalence classes of valuations on $K[x]$ (without fixing their value group), whose restriction to $K$ is equivalent to $v$. In the henselian case, we apply these results to show that there is a complete parallelism between the arithmetic properties of irreducible polynomials $F\in K[x]$, encoded by their Okutsu frames, and the valuation-theoretic properties of their induced valuations $v_F$ on $K[x]$, encoded by their MacLane-Vaqui\'e chains. This parallelism was only known for defectless irreducible polynomials., Comment: V2, April 2022

- Abstract
Robotic manipulation of cloth is a complex task because of the infinite-dimensional shape-state space of textiles, which makes their state estimation very difficult. In this paper we introduce the dGLI Cloth Coordinates, a finite low-dimensional representation of cloth states that allows us to efficiently distinguish a large variety of different folded states, opening the door to efficient learning methods for cloth manipulation planning and control. Our representation is based on a directional derivative of the Gauss Linking Integral and allows us to represent spatial as well as planar folded configurations in a consistent and unified way. The proposed dGLI Cloth Coordinates are shown to be more accurate in the representation of cloth states and significantly more sensitive to changes in grasping affordances than other classic shape distance methods. Finally, we apply our representation to real images of a cloth, showing that with it we can identify the different states using a distance-based classifier.

- Abstract
We introduce a new isometric strain model for the study of the dynamics of cloth garments in a moderate stress environment, such as robotic manipulation in the neighborhood of humans. This model treats textiles as surfaces that are inextensible, admitting only isometric motions. Inextensibility is derived in a continuous setting, prior to any discretization, which gives consistency with respect to remeshing and prevents the problem of locking even with coarse meshes. The simulations of robotic manipulation using the model are compared to the actual manipulation in the real world, finding that the difference between the simulated and the real position of each point in the garment is lower than 1cm in average even when a coarse mesh is used. Aerodynamic contributions to motion are incorporated to the model through the virtual uncoupling of the inertial and gravitational mass of the garment. This approach results in an accurate, when compared to the recorded dynamics of real textiles, description of cloth motion incorporating aerodynamic effects by using only two parameters.

Milk is a source of bioactive proteins with defensive properties of great value for protecting the newborn. The activity of bovine milk lactoferrin (LF) was investigated as an antibacterial agent in the internalisation of the emergent pathogen Cronobacter sakazakii into Caco-2/TC7 cells, a model of human intestinal epithelium. The effect of LF on oxidative stress and expression of Toll-like receptors (TLR) was also investigated. LF exerted a clear antibacterial activity against C. sakazakii, as well as an inhibitory effect on C. sakazakii adhesion and internalisation into Caco-2/TC7 cells. Incubation with C. sakazakii induced an increase in oxidative stress on the lipid fraction of Caco-2/TC7 cells, which was reversed by LF. Additionally, LF altered the expression of TLR, with a clear decrease in the expression levels of TLR4 at 24 h of incubation. These results suggest that LF has very interesting properties as a potential ingredient for functional foods.

Introduction Several studies warn that overweight and obesity have become an epidemic with severe consequences in the population's health. The objective of the present study is to estimate the prevalence of overweight and obesity in Castile and Leon in a sample of children at 6, 11 and 14 years of age, to describe its evolution and its association with life habits and antecedents. Material and methods Observational study, retrospective cohort, performed by the pediatricians of the Health Sentinel Network. The study is part of a joint Project with the National Research Center on Human Evolution (CENIEH) in Burgos to know the pattern of growth and development of the child population of Castile and Leon. In 2012, a health examination and a retrospective collection were carried out based on the clinical history of a simple of 326 children, from the cohort was born in 1998. Results Following the WHO references, at age 14 there was 25.3% of boys and 18.5% of girls with overweight. Obesity was estimated to affect the 8.2% and 4.8% of them respectively. At 11 years of age there was the maximum of overweight in girls and of obesity in boys. At the urban-non urban environment, quantity and quality of sleep and the parents’ BMI have been some of the associated factors to weight level of the children. Discussion In Castile and Leon, as in other populations of our environment, there is a severe problem of overweight and obesity. For their control it is necessary to deepen understanding of lifestyles and antecedents related to the birth as well as to calculate the growth rate in the pediatric age.

New drugs have been added to the treatment of type 2 diabetes mellitus. These drugs have represented an advance in the physiopathological approach to the disease, with the possibility of acting on the different mechanisms involved in raised blood glucose levels and allowing treatment to be individualised, as recommended in clinical practice guide-lines and consensus documents. The incretins were added to the therapeutic arsenal of diabetes in 2006. This group in-cludes glucagon-like peptide-1 receptor antagonists and dipeptidyl peptidase-4 inhibitors (marketed in Spain in 2007), which act on the incretin system in the gut, reducing glycae-mia, with a decreasing or neutral effect on weight and with a low risk of hypoglycaemic episodes. Their introduction coincided with the publication of the controversial results of studies on intensive glycemic control, leading the Food and Drug Administration (2008) to require demonstration of the cardiovascular safety of new glucose-lowering drugs. The publication of subsequent studies has demonstrated cardiovascular safety and some have shown cardiovascular and survival benefits. These results are modifying the recommenda-tions of the clinical practice guidelines and consensus documents on the treatment of type 2 diabetes. The present article describes the main pharmacological and cardiovascular characteristics of the safety and tolerability of dipeptidyl peptidase-4 inhibitors, which need to be known and updated in primary care, given their wide prescription and the need for correct use of drug therapy in type 2 diabetes.

The HAWC γ-ray observatory is adding a fresh view of the high-energy sky through the unbiased survey of two thirds of the celestial sphere. HAWC has found TeV emission from 39 sources, a third of which were not previously detected. Most of the HAWC sources are in the Milky Way, either towards the Galactic anti-center or along the First Galactic Quadrant which features a Δ l ≃ 80 ∘ band of TeV emission spanning from Cygnus down to the vicinity of the Galactic Center. These cosmic accelerators are presented in a comparative context with other ground-based and space-borne high energy observatories.

The High Altitude Water Cherenkov observatory (HAWC) is an air shower array devised for TeV gamma-ray astronomy. HAWC is located at an altitude of 4100 m a.s.l. in Sierra Negra, Mexico. HAWC consists of 300 Water Cherenkov Detectors, each instrumented with 4 photomultiplier tubes (PMTs). HAWC re-uses the Front-End Boards from the Milagro experiment to receive the PMT signals. These boards are used in combination with Time to Digital Converters (TDCs) to record the time and the amount of light in each PMT hit (light flash). A set of VME TDC modules (128 channels each) is operated in a continuous (dead time free) mode. The TDCs are read out via the VME bus by Single-Board Computers (SBCs), which in turn are connected to a gigabit Ethernet network. The complete system produces ~ 500 MB/s of raw data. A high-throughput data processing system has been designed and built to enable real-time data analysis. The system relies on off-the-shelf hardware components, an open-source software technology for data transfers (ZeroMQ) and a custom software framework for data analysis (AERIE). Multiple trigger and reconstruction algorithms can be combined and run on blocks of data in a parallel fashion, producing a set of output data streams which can be analyzed in real time with minimal latency (< 5 s). This paper provides an overview of the hardware set-up and an in-depth description of the software design, covering both the TDC data acquisition system and the real-time data processing system. The performance of these systems is also discussed., 14 pages, 3 figures, submitted to Nucl. Instrum. Meth. A

The Latin American Giant Observatory (LAGO) is an international network of water-Cherenkov detectors (WCD) set in different sites across Latin America. On top of the Sierra Negra volcano in Mexico at an altitude of 4530 m, LAGO has completed its first out of three instrumented detector. It consists of a cylindrical water tank with a diameter of 7.3 m and a height of 1 m and a total detection area of 40 m2 that is sectioned in four equal slices. In this work we present the full calibration procedure of this detector and the initial measurements of stability in rate. We also derive the effective area to gamma-ray bursts for the complete array using the LAGO simulation chain, based on CORSIKA and GEANT4.

The identification of the relative position of two real coplanar ellipses can be reduced to the identification of the nature of the singular conics in the pencil they define and, in general, their location with respect to these singular conics in the pencil. This latter problem reduces to find the relative location of the roots of univariate polynomials. Since it is usually desired that all generated expressions are algebraic to simplify further analysis, including the case in which the ellipses undergone temporal variations, all recent methods available in the literature rely mathematical tools such as Sturm–Habicht sequences or subresultant sequences. This paper presents an alternative based on more elementary tools which results in a binary decision tree to classify the relative location of two ellipses in 12 different classes. The decision at each node is taken based on the sign of a set of algebraic/rational expressions on the ellipses coefficients, the most complex of them being third and second order polynomial discriminants.

An increase in temperature of approximately 4 °C is expected to occur in the Mediterranean by the end of the century. Concomitant to this warming, a foreseeable rise in salinity will affect aquatic species. We addressed the effects of warming and salinity, and their interaction on three coexisting characean species (Chara aspera, C. hispida and C. vulgaris) from a Spanish Mediterranean interdunal pond (spring water temperature 20–23 °C, when charophytes re-grew; salinity 1.3–1.8 PSU). A laboratory experiment was designed with two levels of water temperature treatment (23 and 27 °C), plus two levels of salinity treatment (0.4 and 4.0 PSU). The variables considered were total length, number of ramifications and nodes, dry weight, growth rate and stoichiometric composition. Three clear response patterns arose: (i) C. aspera growth was favoured by the 4 °C rise in temperature and the 10-fold increase in salinity when considered separately, but no synergy occurred when these factors interacted; (ii) C. hispida growth did not change with the temperature, but was negatively affected by the salinity increase, and (iii) C. vulgaris growth was enhanced by the temperature, a fact that compensated for the damaging effect of increased salinity. Variations of N and P concentrations with treatments were similar in C. aspera and C. vulgaris. We demonstrate how charophyte response to the interaction of increased temperature and salinity is species-specific and different under isolated stressors. Therefore, this study highlights how simultaneous drivers of global change might affect the composition of charophyte meadows in Mediterranean shallow wetlands.

This work describes a straightforward diastereoselective synthetic access to azirino[2,1-b]benzo[e][1,3]oxazines containing phosphorus substituents such as phosphonate or phosphine oxide, by means of nucleophilic addition of functionalized phenols to the C–N double bond of 2H-azirine derivatives. In addition, the cytotoxic effect on cell lines derived from human lung adenocarcinoma (A549) and human embryonic kidney (HEK293) was also screened. Some azirino[2,1-b]benzo[e][1,3]oxazines 4 and 6 exhibited very good activity against the A549 cell line in vitro. Furthermore, selectivity towards cancer cell (A549) over (HEK293), and non-malignant cells (MCR-5) has been detected.

Introduction Transoesophageal echocardiography (TOE) use has experienced an exponential growth in cardiac surgery (CS). The latest guidelines recommend TOE for all adult patients undergoing CS, but the management of unexpected-TOE findings (ATOEF) after cardiopulmonary bypass (CPB) might be controversial. We reviewed the outcomes of those patients with ATOEF after CBP managed without immediate surgical revision. Methods A systematic TOE use policy is applied in our institution for patients undergoing CS. Intraoperative TOE is performed by a senior anaesthesiologist. Data from intraoperative TOE cases were collected prospectively between January 2014 and December 2017. The presence of post-CPB ATOEF and its impact on the immediate surgical treatment decision-making was analysed. Results A total of 2421 TOE examinations were registered, with 197 post-CPB ATOEF. Among those with post-CPB ATOF, 108 (55%) led to immediate surgical treatment. The remaining 89 (45%) cases with post-CPB ATOEF did not received surgical treatment. These non-treated ATOEF were characterised into: 25 (28%) periprosthetic leak, 1 (1%) intraprosthetic regurgitation, 20 (22%) regurgitations after mitral valve (MV) repair, 11 (12%) regurgitations after aortic valve (AoV) repair, 7 (8%) and 2 (2%) regurgitations related to native MV and AoV respectively, 12 (13%) systolic anterior motion of the MV and 11 (12%) classified as other findings. The immediate postoperative follow-up showed the persistence of 36% of periprosthetic regurgitations, 100% of intraprosthetic regurgitations, 70% and 91% related to MV and AoV repair respectively, 86% and 100% related to native MV and AoV respectively. Two patients died in the immediate postoperative period and none required immediate re-do surgery due to the findings. The 6-12 month follow-up showed the persistence of 34% of periprosthetic leaks, 100% of intraprosthetic regurgitations, 70% and 91% of those related to MV and AoV repair, 71% and 100% of those related to native MV and AoV. Among all post-CPB ATOEF, 2 (2%) patients have required surgical treatment related to the finding in the long-term follow-up. Discussion Almost half of the post-CBP ATOEF did not lead to an immediate surgical treatment. Follow up echocardiography in the immediate postoperative period and at 6-12months, showed reduction of periprosthetic leaks. However, we observed stability of left-sided valve repair regurgitations. The incidence of reinterventions due to post-CBP ATOEF was low, reinforcing our current decision making.
