Abstract

The COVID-19 pandemic has disrupted access to, adherence to, and perceptions of routine vaccinations. We developed the Shift in Vaccine Confidence (SVC) survey tool to assess the impact of the pandemic on routine vaccinations, with a focus on the HBV vaccine, in Kinshasa, Democratic Republic of Congo (DRC). This study describes the content validation steps we conducted to ensure the survey tool is meaningful to measure changes in vaccine confidence to regular immunization (HBV vaccine) due to the pandemic. Three rounds of stakeholder feedback from a DRC-based study team, content and measurement experts, and study participants allowed us to produce a measure with improved readability and clarity.

Abstract

Summary Background The aim of this study was to compare, in terms of cost and serious complications, the use of biosynthetic resorbable parietal mesh with biologic mesh in patients undergoing contaminated ventral hernia repair (modified Ventral Hernia Working Group grade 3). Poly-4-hydroxy-butyrate (P4HB) biosynthetic mesh has rarely been the subject of comparative studies in the context of contamination. Data are required to confirm the effects of a transition from biological mesh to biosynthetic resorbable mesh. Patients and methods A cost-effectiveness analysis was conducted. It was based on a decision analysis model built with clinical and economic data issued from a before-after study that included 94 patients hospitalized for ventral hernia repair at the University Hospital of Strasbourg (France) from June 2011 to February 2018. The effectiveness endpoint was the number of patients presenting with a serious specific complication or a general complication at 6 months. Data for surgical hospitalization stays, home hospitalizations and ambulatory care costs were included. Results We found fewer serious complications with biosynthetic mesh: 21% versus 33% with biologic mesh. A cost savings of US $5146 was determined. Deterministic sensitivity analyses and a probabilistic analysis confirmed our findings and the robustness of the model. Conclusion P4HB biosynthetic resorbable mesh appeared to be the most effective and the least costly option. Additional data will be needed to confirm the superiority of biosynthetic mesh in terms of the recurrence risk reduction over a longer period.

Resume But de l’etude Le but de l’etude etait de comparer, en termes de couts et de complications postoperatoires graves, l’utilisation d’une prothese biosynthetique resorbable avec des protheses biologiques, lors d’une cure d’eventration contaminee (modified Ventral Hernia Working Group grade 3). La prothese Phasix® a peu fait l’objet d’etudes comparative en milieu contamine, et des donnees supplementaires sont requises pour confirmer un changement d’utilisation de protheses en milieu contamine. Patients et methodes Une analyse cout–efficacite a ete conduite. Elle etait basee sur un modele d’analyse decisionnelle, construit avec des donnees cliniques et economiques issues d’une etude avant–apres, qui incluait 94 patients operes d’une cure d’eventration a l’hopital universitaire de Strasbourg (France) entre juin 2011 et fevrier 2018. Le critere d’efficacite etait le nombre de patient presentant une complication grave generale ou specifique a 6 mois postoperatoire. La duree d’hospitalisation a l’hopital, a domicile et les soins ont ete pris en compte dans les couts. Resultats Nos resultats montrent moins de complications avec la prothese biosynthetique comparativement aux protheses biologiques : 21 % versus 33 %. Nous avons determine une epargne de US $5146. L’analyse de sensibilite deterministe et l’analyse probabiliste ont confirme nos resultats et la robustesse de notre modele. Conclusion La prothese biosynthetique P4HB apparait comme etant la solution la plus efficiente et la moins chere. Des donnees supplementaires doivent par la suite confirmer la superiorite de prothese biosynthetique en termes de recidive a long terme. ClinicalTrials.gov ID NCT03590184 .

Abstract

BACKGROUND: Estimates of the effectiveness of water, sanitation, and hygiene (WASH) interventions that provide high levels of service on childhood diarrhoea are scarce. We aimed to provide up-to-date estimates on the burden of disease attributable to WASH and on the effects of different types of WASH interventions on childhood diarrhoea in low-income and middle-income countries (LMICs). METHODS: In this systematic review and meta-analysis, we updated previous reviews following their search strategy by searching MEDLINE, Embase, Scopus, Cochrane Library, and BIOSIS Citation Index for studies of basic WASH interventions and of WASH interventions providing a high level of service, published between Jan 1, 2016, and May 25, 2021. We included randomised and non-randomised controlled trials conducted at household or community level that matched exposure categories of the so-called service ladder approach of the Sustainable Development Goal (SDG) for WASH. Two reviewers independently extracted study-level data and assessed risk of bias using a modified Newcastle-Ottawa Scale and certainty of evidence using a modified Grading of Recommendations, Assessment, Development, and Evaluation approach. We analysed extracted relative risks (RRs) and 95% CIs using random-effects meta-analyses and meta-regression models. This study is registered with PROSPERO, CRD42016043164. FINDINGS: 19 837 records were identified from the search, of which 124 studies were included, providing 83 water (62 616 children), 20 sanitation (40 799 children), and 41 hygiene (98 416 children) comparisons. Compared with untreated water from an unimproved source, risk of diarrhoea was reduced by up to 50% with water treated at point of use (POU): filtration (n=23 studies; RR 0·50 [95% CI 0·41-0·60]), solar treatment (n=13; 0·63 [0·50-0·80]), and chlorination (n=25; 0·66 [0·56-0·77]). Compared with an unimproved source, provision of an improved drinking water supply on premises with higher water quality reduced diarrhoea risk by 52% (n=2; 0·48 [0·26-0·87]). Overall, sanitation interventions reduced diarrhoea risk by 24% (0·76 [0·61-0·94]). Compared with unimproved sanitation, providing sewer connection reduced diarrhoea risk by 47% (n=5; 0·53 [0·30-0·93]). Promotion of handwashing with soap reduced diarrhoea risk by 30% (0·70 [0·64-0·76]). INTERPRETATION: WASH interventions reduced risk of diarrhoea in children in LMICs. Interventions supplying either water filtered at POU, higher water quality from an improved source on premises, or basic sanitation services with sewer connection were associated with increased reductions. Our results support higher service levels called for under SDG 6. Notably, no studies evaluated interventions that delivered access to safely managed WASH services, the level of service to which universal coverage by 2030 is committed under the SDG. Abstract

Summary Identifying whether a given genetic mutation results in a gene product with increased (gain-of-function; GOF) or diminished (loss-of-function; LOF) activity is an important step toward understanding disease mechanisms because they may result in markedly different clinical phenotypes. Here, we generated an extensive database of documented germline GOF and LOF pathogenic variants by employing natural language processing (NLP) on the available abstracts in the Human Gene Mutation Database. We then investigated various gene- and protein-level features of GOF and LOF variants and applied machine learning and statistical analyses to identify discriminative features. We found that GOF variants were enriched in essential genes, for autosomal-dominant inheritance, and in protein binding and interaction domains, whereas LOF variants were enriched in singleton genes, for protein-truncating variants, and in protein core regions. We developed a user-friendly web-based interface that enables the extraction of selected subsets from the GOF/LOF database by a broad set of annotated features and downloading of up-to-date versions. These results improve our understanding of how variants affect gene/protein function and may ultimately guide future treatment options.

Abstract

Resume Nous rapportons le cas d’une patiente de 51 ans, aux antecedents de myelome multiple en remission apres plusieurs cures de chimiotherapie et une autogreffe en avril 2019, adressee par le centre anti-douleur de l’hopital en decembre 2020 pour la realisation d’une scintigraphie osseuse dans le cadre d’une suspicion d’algodystrophie du membre superieur gauche, apres chirurgie d’une hernie discale cervicale en septembre 2020. L’acquisition precoce corps entier faisant suite a l’injection de 630 MBq de 99mTc- oxidronate de sodium mettait en evidence une masse hyperhemique intense de l’epaule gauche, associee a une hyperhemie diffuse du reste du membre superieur gauche. L’imagerie hybride TEMP-TDM a confirme la presence d’une volumineuse masse de densite tissulaire associee a une lyse majeure de la tete humerale gauche. On retrouvait egalement une hyperfixation diffuse reactionnelle de l’ensemble du membre superieur gauche ainsi que deux lesions lytiques lacunaires frontales hyperfixantes inconnues auparavant. Devant ces resultats, la patiente a egalement beneficie d’une TEP-TDM au 18F-FDG retrouvant la volumineuse lesion humerale gauche ainsi que plusieurs lesions osteolytiques hypermetaboliques. La patiente a finalement ete hospitalisee en medecine interne et presentee en RCP regionale, qui a conclu a une recidive myelomateuse avec plasmocytome humeral gauche. Bien que la scintigraphie osseuse ne fasse pas partie des examens recommandes dans le bilan du myelome, elle s’est illustree en permettant la correction du diagnostic et la detection de la recidive. La TEP-TDM au 18-FDG s’est revelee dans notre cas etre un examen plus sensible dans la detection des lesions myelomateuses, ayant permis la realisation d’un bilan d’extension plus complet de la maladie et la detection de lesions passees inapercues a la scintigraphie osseuse.

Abstract

Summary Background Two billion peripheral venous catheters are sold globally each year, but the optimal skin disinfection and types of devices are not well established. We aimed to show the superiority of disinfection with 2% chlorhexidine plus alcohol over 5% povidone iodine plus alcohol in preventing infectious complications, and of closed integrated catheters, positive displacement needleless-connectors, disinfecting caps, and single-use prefilled flush syringes used in combination (innovation group) over open catheters and three-way stopcocks for treatment administration (standard group) in preventing catheter failure. Methods We did an open-label, randomised-controlled trial with a two-by-two factorial design, for which we enrolled adults (age ≥18 years) visiting the emergency department at the Poitiers University Hospital, France, and requiring one peripheral venous catheter before admission to the medical wards. Before catheter insertion, patients were randomly assigned (1:1:1:1) using a secure web-based random-number generator to one of four treatment groups based on skin preparation and type of devices (innovative devices or standard devices; 2% chlorhexidine plus alcohol or 5% povidone iodine plus alcohol). Primary outcomes were the incidence of infectious complications (local infection, catheter colonisation, or bloodstream infections) and time between catheter insertion and catheter failure (occlusion, dislodgment, infiltration, phlebitis, or infection). This study is registered with ClinicalTrials.gov , NCT03757143 . Findings 1000 patients were recruited between Jan 7, and Sept 6, 2019, of whom 500 were assigned to the chlorhexidine plus alcohol group and 500 to the povidone iodine plus alcohol group (250 with innovative solutions and 250 with standard devices in each antiseptic group). No significant interaction was found between the two study interventions. Local infections occurred less frequently with chlorhexidine plus alcohol than with povidone iodine plus alcohol (0 [0%] of 496 patients vs six [1%] of 493 patients) and the same was observed for catheter colonisation (4/431 [1%] vs 70/415 [17%] catheters among the catheters cultured; adjusted subdistribution hazard ratio 0·08 [95% CI 0·02–0·18]). Median time between catheter insertion and catheter failure was longer in the innovation group compared with the standard group (50·4 [IQR 29·6–69·4] h vs 30·0 [16·6–52·6] h; p=0·0017). Minor skin reactions occurred in nine (2%) patients in the chlorhexidine plus alcohol group and seven (1%) patients in the povidone iodine plus alcohol group. Interpretation For skin antisepsis, chlorhexidine plus alcohol provides greater protection of peripheral venous catheter-related infectious complications than does povidone iodine plus alcohol. Resume Pour une revue medicale, la diffusion a grande echelle de chacun de ses articles est aujourd’hui indispensable pour toucher un large public scientifique. Dans ce contexte, l’utilisation du reseau social Twitter permet a chaque nouvelle publication d’etre diffusee tres rapidement, a faible cout, pour des lecteurs precis et partout dans le monde. C’est de loin le reseau social le plus utilise pour partager les publications en 2021. Les reflexions autour de ce sujet ont ainsi abouti a la creation en novembre 2020 de la Social Media Team (SoMe Team) d’Anaesthesia Critical Care and Pain Medicine, le journal anglophone international de la Societe francaise d’anesthesie et de reanimation (SFAR). Cette equipe est coordonnee par un editeur en charge des reseaux sociaux, en lien direct avec le redacteur en chef du journal. Il elabore le planning des tweets en fonction du programme de publication du journal, il fleche le type de contenu qui y sera associe et sollicite les auteurs pour l’obtention d’images ou de courtes videos de presentation de leur publication. Le contenu graphique et la diffusion sur Twitter sont assures par les membres de l’equipe dediee aux reseaux sociaux.

Abstract

Resume Les infections du site operatoire (ISO) sont la deuxieme cause d’infections associees aux soins dans les etablissements de sante apres les infections urinaires et sont responsables d’une morbi-mortalite et d’un surcout eleves. Malgre l’amelioration des techniques anesthesiques et chirurgicales, leur incidence reste stable ces dernieres annees. Majoritairement d’origine endogene (contamination par la flore bacterienne du patient), Staphylococcus aureus et Escherichia coli sont les deux germes les plus souvent retrouves. L’application stricte de mesures de prevention, pendant les periodes preoperatoire, peroperatoire et postoperatoire, permet de reduire leur incidence. L’hygiene des mains, la preparation cutanee du patient, l’antibioprophylaxie et le maintien de la normothermie du patient sont les mesures les plus efficaces dans la reduction des ISO. La generalisation de la surveillance active de la survenue d’ISO permet d’ameliorer les pratiques professionnelles au sein de chaque etablissement.
