Public decisions are typically related to large investments leaving long legacies. We should therefore strive for wide societal agreement regarding such decisions, which meet the diversity of preferences between stakeholders and over time. But if, how and why do stakeholder preferences change over time? In decision analysis, these questions received little attention. We explored them using three real-world public decision processes, based on Multi-Criteria Decision Analysis (MCDA). We used repeatedly elicited ranking of objectives over time. These were obtained during three to five moderated workshops we organised several months apart (total N = 200 questionnaires, and 100 stakeholders). We analysed individual and aggregated (group) preferences, their changes and potential drivers including demographic and experience variables. We also analysed the effect of preference evolution on the performance of decision-alternatives with MCDA over time. We found that stakeholder preferences often changed over time, both on an individual and group level. These changes did not systematically diminish over time, but some convergence of preferences was observed for stakeholders who repeatedly participated in workshops. High-ranking objectives were relatively stable and similar between stakeholders. While preference changes could not be explained by demographics and personal experiences, repeated interaction with the decision problem might play a role. Neither the observed disagreement between stakeholders, nor the preference changes over time affected the best and worst performing alternatives in our decision problems. Thus, despite changing stakeholder preferences over time, public decision-makers can contrive robust solutions to complex public decision problems in the present.

Background The heterologous expression of parasitic proteins is challenging because the sequence composition often differs significantly from host preferences. However, the production of such proteins is important because they are potential drug targets and can be screened for interactions with new lead compounds. Here we compared two expression systems for the production of an active recombinant aldehyde dehydrogenase (SmALDH_312) from Schistosoma mansoni, which causes the neglected tropical disease schistosomiasis. Results We produced SmALDH_312 successfully in the bacterium Escherichia coli and in the baculovirus expression vector system (BEVS). Both versions of the recombinant protein were found to be active in vitro, but the BEVS-derived enzyme showed 3.7-fold higher specific activity and was selected for further characterization. We investigated the influence of Mg2+, Ca2+ and Mn2+, and found out that the specific activity of the enzyme increased 1.5-fold in the presence of 0.5 mM Mg2+. Finally, we characterized the kinetic properties of the enzyme using a design-of-experiment approach, revealing optimal activity at pH 7.6 and 41°C. Conclusions Although, E. coli has many advantages, such as rapid expression, high yields and low costs, this system was outperformed by BEVS for the production of a schistosome ALDH. Xpert MTB/RIF testing has improved tuberculosis (TB) diagnostics and rifampicin (Rif) resistance testing worldwide. However, it has weaknesses, such as its restriction to Rif resistance testing and the inability to use extracted DNA for further testing. Herein, a holistic diagnostic workflow, including TB detection and resistance testing toward Rif, isoniazid, and important second-line drugs (SLDs), based on a novel microfluidic DNA extraction cartridge (TB-Disk), is presented. DNA from 73 precharacterized sputum samples was extracted with TB-Disk, including 45 clinical and bacteriologically confirmed TB samples, nine TB-negative samples, and 19 sputum samples spiked with twofold dilutions of TB bacteria. The extracted DNA was subjected to further testing with FluoroType MTB (FT-MTB), GenoType MTBDRplus (GT-plus), and GenoType MTBDRsl. A total of 100% (20/20) and 72% (18/25) of smear-positive and smear-negative TB samples were identified as Mycobacterium tuberculosis complex positive. A total of 79% (33/42) of subsequently GT-plus tested samples yielded a valid result. Eight samples were identified as multidrug-resistant TB by GT-plus and further tested for resistance toward SLDs using GenoType MTBDRsl, yielding 75% (6/8) valid results. FT-MTB with cartridge-based DNA extraction (Disk-DNA) and DNA extracted with FluoroLyse yielded similar analytical sensitivities. FT-MTB with Disk-DNA was 100% specific. TB-Disk in combination with FT-MTB enables sensitive TB detection. The Disk-DNA can be further used for screening resistance toward first-line drugs and SLDs.

The future use of coal as a fuel for power generation in the US depends on the availability of financially viable technologies for capture and storage of CO2 emissions from power plants. Key second-generation candidates for CO2 capture include high temperature and pressurized oxy-firing of coal, which has the potential to increase efficiency, lower capital costs, avoid air ingress and reduce oxygen requirements. However, unquantified challenges, such as flame behavior, heat transfer, ash transformation, ash deposition and char oxidation, still exist for those technologies. This study specifically focuses on the formation of submicron particles and initial layer ash deposition during high temperature oxy-coal combustion. Previous work has shown that the initial layer deposits are mainly formed of submicron size ash aerosols transported by thermophoresis. Unfortunately, the importance of submicron particle deposition has not received much attention, probably due to the insignificant deposit mass and difficulty in prediction of the submicron particles formation. In this work, models including mineral matter vaporization model, scavenging model and deposition model are developed and applied into a three-dimensional CFD framework to predict the submicron particles formation and subsequent initial layer deposits formation. The model results are comparable to experimental data. The merits of this work are that it has led to the development of a novel approach to predict both submicron particle formation and initial layer ash deposition during oxy-coal combustion.

Polar motion (PM) series from various observation methods and sources are analyzed using one and the same simple analysis tool. The maximum time resolution of the Earth Rotation Parameter (ERP) series considered here is one day, which leaves out ERP series with sub-daily time resolution. A longer spacing between subsequent epochs is permissible, a variable spacing between epochs like in the VLBI (Very Long Baseline Interferometry) series as well. The shortest length of the PM time series analyzed, based on GNSS (Global Navigation Satellite Systems), is 26+ years, the longest one, the IERS C01 series, 173+ years. Following the tradition, PM is decomposed into a mean motion, i.e., the motion of the center of the PM curve on the surface of the Earth, a prograde Chandler motion with a nominal period of 432.25 days, and a prograde annual motion with a nominal period of 365.25 days. In our analysis, Secular PM is not assumed as linear, but as piecewise linear and continuous. The defining parameters of the periodic parts of polar motion are not assumed as constant either, but as piecewise linear and continuous functions of time as well. All parameters of the PM model based on one particular ERP series are determined in one and the same linear least squares estimation. Parameters of the same type may be purely deterministic or filtered (smoothed) by constraining the second time differences of the parameters (the second time derivative of the parameter functions) of the same kind. The key results achieved by applying our method to a wide variety of ERP series are visualized and discussed. They include: (a) the description of a sharp bend of mean PM around 1996, (b) strong evidence for a multi-component model for Chandler motion with periods separated by few days, similar to that proposed by Chandler (1901), (c) a reconstruction of mean PM from 1846 to present taking into account the composite nature of the series, and (d) a prediction of Chandler motion for the next decades. A sharp bend of mean PM was found around 1995/96, along the meridian at about 70 ° West to the meridian at about 12–15° West. All PM curves analyzed show a mean motion in this new direction after 1995/95. Smaller excursions w.r.t.d this mean exist, with periods 10 years. A periodic variation with a period of about 180 years was observed in the Chandler signal of PM with an amplitude of more than 200 mas. The variation may be explained approximately by a composite nature of Chandler motion, similar to the model already proposed by Chandler (1901). This simple model was generalized using a Fourier series separated in period by few days. This alternative model of Chandler motion is used to predict the Chandler part of PM, with an amplitude close to zero currently, to reach values of the order of 100 mas around 2028–2030.

National data on patient characteristics, treatment, and outcomes of critically ill coronavirus disease 2019 (COVID-19) solid organ transplant (SOT) patients are limited. We analyzed data from a multicenter cohort study of adults with laboratory-confirmed COVID-19 admitted to intensive care units (ICUs) at 68 hospitals across the United States from March 4 to May 8, 2020. From 4153 patients, we created a propensity score matched cohort of 386 patients, including 98 SOT patients and 288 non-SOT patients. We used a binomial generalized linear model (log-binomial model) to examine the association of SOT status with death and other clinical outcomes. Among the 386 patients, the median age was 60 years, 72% were male, and 41% were black. Death within 28 days of ICU admission was similar in SOT and non-SOT patients (40% and 43%, respectively; relative risk [RR] 0.92; 95% confidence interval [CI]: 0.70-1.22). Other outcomes and requirement for organ support including receipt of mechanical ventilation, development of acute respiratory distress syndrome, and receipt of vasopressors were also similar between groups. There was a trend toward higher risk of acute kidney injury requiring renal replacement therapy in SOT vs. non-SOT patients (37% vs. 27%; RR [95% CI]: 1.34 [0.97-1.85]). Death and organ support requirement were similar between SOT and non-SOT critically ill patients with COVID-19.

BD MAX MDR-TB assay is a new molecular platform for the detection of Mycobacterium tuberculosis complex (MTBC) in clinical specimens and simultaneous detection of resistance towards isoniazid and rifampicin. In this study, we assessed its diagnostic accuracy using pre-characterized MTBC-culture negative (n=257), smear-negative / MTBC-culture positive (n=93), and smear-positive / MTBC-culture positive (n=153) respiratory specimens. Compared to culture, overall sensitivity and specificity of BD MAX MDR-TB were 86.6% and 100%, respectively; sensitivities for smear-positive and smear-negative samples were 100% and 64.5%, respectively. Sensitivity and specificity for isoniazid and rifampicin resistance were 58.3% (biased low due to sample collection strategy in low prevalence setting), 99.3%, 100%, and 98.2%, respectively, compared to phenotypic drug resistance testing and 100%, 99.4%, 100% and 99.4%, respectively, compared to Genotype MTBDRplus. In conclusion, BD MAX MDR-TB is an accurate assay for the diagnostic detection of MTBC in respiratory samples and its resistance toward the most important anti-TB drugs isoniazid and rifampicin. Due to its medium to high throughput, good validity and ease of use, the assay will be of great benefit for medium-size to large TB diagnostic centers.

Philadelphia-negative myeloproliferative neoplasms (MPNs) - polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF) - often present with thrombosis. We aimed to determine the inpatient mortality, cost of care, and length-of-stay among individuals with Philadelphia-negative MPNs who had arterial or venous thrombosis associated with admission versus those who were admitted for non-thrombosis-related events.Using ICD-10M coding, we identified 7,128,770 patients from the National Inpatient Sample (NIS) database who were hospitalized in 2016. 31,302 patients had a diagnosis of a Philadelphia-negative MPN. Mortality, length-of-stay, and cost of care were compared between patients who had thrombosis included among the top three diagnoses and those who were admitted for other reasons. Chi-squared test for categorical variables and t-test for continuous variables were used to compare baseline characteristics. Final multivariable models were constructed to determine predictors of outcomes.Inpatient mortality was significantly higher among individuals with Philadelphia-negative MPN who had thrombosis associated with admission as compared to those who were hospitalized for other reasons (5.7% versus 3.1%, P 0.001). Unadjusted cost of care was also significantly higher for patients with thrombosis as compared to those without thrombosis ($25,539.06 versus $19,002.72 USD, respectively, P 0.001). Length-of-stay was longer among the former group as compared to the latter (8.26 versus 7.95 days, P = 0.0963). However, this finding did not reach statistical significance.Hospitalization for MPN-related thrombotic events is associated with excess inpatient mortality and higher cost of care. However, thrombosis has no statistically significant effect on length-of-stay among this population. The underlying causes of mortality and cost disparities among patients with MPN-associated thrombosis warrant further investigation.

Using random germline mutagenesis in mice, we identified a viable hypomorphic allele (boh) of the transcription-factor-encoding gene Ovol2 that resulted in obesity, which initially developed with normal food intake and physical activity but decreased energy expenditure. The stilbene moiety is commonly found in natural products and these compounds display an extraordinary range of biological activity. Efforts to derive useful drugs from stilbenes must address the potential liabilities of this structure, including a propensity for cis/trans isomerization. To identify olefin replacements that address this limitation while preserving biological activity we have prepared analogues of two bioactive stilbenes, a pawhuskin and a schweinfurthin, where a 1,2,3-triazole ring formally replaces the stilbene double bond. The new schweinfurthin analogue (23) has been tested for anti-proliferative activity against 60 cell lines, and shows a strong correlation of bioactivity when compared to the compound that inspired its synthesis (22).

The use of vegetation as a source of energy and its clearing for agricultural production has contributed to the degradation of the drylands in world. On soil, the effects of deforestation are still controversial and need to be better understood. In this study, the effect of deforestation and soil topography on soil morphological properties, nutrient availability, and carbon (C) contribution in a topossequence that was deforested 12 years before in the Brazilian semi-arid region was evaluated. For this purpose, two areas (preserved and deforested) were selected and separated into three positions along the slope: lower, middle, and upper third. For each position, the characterization of the soil fertility and the physical and morphological attributes of the selected profiles were carried out. In general, deforestation had no effect on the total organic carbon (TOC) of the soil, but, on the other hand, was responsible for increasing the structural fragility of the soil. The major effect of deforestation on soil fertility was reduced availability of cationic nutrients (Mg2+ and K+). We conclude that topography had no effect on the restoration of soil properties after deforestation in this semiarid ecosystem. For these conditions, similar management strategies (e.g. reforestation), can be adopted for the different topographical positions of the slope. Our results can be used in forest restoration projects of degraded ecosystems in drylands, to select the best practices of management, aiming at restoring the natural ecological processes of the ecosystem.
