Resumen El cancer de mama asociado al embarazo representa una compleja situacion clinica por la dificultad de su diagnostico. La clinica principal del cancer de mama gestacional es el nodulo palpable, sin embargo, debido a los cambios fisiologicos que se producen en la glandula durante el embarazo o la lactancia, el diagnostico es especialmente dificil, lo que provoca un retraso en el diagnostico. El carcinoma escamoso es un subtipo histologico raro, que representa menos del 0,5% de los canceres de mama. Se trata de un tipo de tumor que suele presentarse en forma de masa quistica y cuyo diagnostico puede confundirse con una entidad benigna. Se presenta el caso de una mujer de 35 anos diagnosticada de un carcinoma escamoso de mama tras multiples ingresos medicos por sospecha de galactoceles y mastitis. El cancer de mama debe ser el primer diagnostico diferencial que tener en cuenta ante un nodulo persistente que no mejora tras tratamiento medico y es importante pensar en el carcinoma escamoso en caso de que se trate de una masa quistica o absceso mamario.

Resumen Objetivo Evaluar el rendimiento de la biopsia por puncion (BPP) en el diagnostico de tumor filodes (TF) de la mama. Material y metodos Se han revisado todos los diagnosticos anatomopatologicos de TF emitidos tanto en BPP como en biopsia quirurgica (BQ). Se analizan las caracteristicas clinicas, de imagen y patologicas de los casos y la concordancia diagnostica entre la BPP y la BQ. Resultado Se han estudiado un total de 87 casos con diagnostico de TF, 16 en BPP y 71 en BQ. En 18 casos se dispuso de BPP y BQ. Los TFB diagnosticados en BPP fueron: 7 FAD (uno de ellos de la variedad celular), 2 TFB, un TFL, 2 TFM y un hamartoma. Dos casos de TFB en BQ habian sido diagnosticados respectivamente en la BPP de fibrosis e hiperplasia. Un TFM en BPP resulto ser un TFL en la BQ. Hubo concordancia diagnostica en un TFL y un TFM. Se detectaron 5 (27%) falsos negativos y 8 falsos positivos (38%). Conclusiones El rendimiento de la BPP en el diagnostico de los TF es bajo con una alta tasa de falsos negativos y falsos positivos. Por ello se aconseja emitir un diagnostico en la BPP de lesion fibroepitelial con estroma celular dejando pendiente un diagnostico definitivo a disponer de la BQ.

Niemann Pick type C (NPC) disease is a progressive lysosomal storage disorder caused by mutations in genes encoding NPC1/NPC2 proteins, characterized by neurological defects, hepatosplenomegaly and premature death. While the primary biochemical feature of NPC disease is the intracellular accumulation of cholesterol and gangliosides, predominantly in endolysosomes, mitochondrial cholesterol accumulation has also been reported. As accumulation of cholesterol in mitochondria is known to impair the transport of GSH into mitochondria, resulting in mitochondrial GSH (mGSH) depletion, we investigated the impact of mGSH recovery in NPC disease. We show that GSH ethyl ester (GSH-EE), but not N-acetylcysteine (NAC), restored the mGSH pool in liver and brain of Npc1-/- mice and in fibroblasts from NPC patients, while both GSH-EE and NAC increased total GSH levels. GSH-EE but not NAC increased the median survival and maximal life span of Npc1-/- mice. Moreover, intraperitoneal therapy with GSH-EE protected against oxidative stress and oxidant-induced cell death, restored calbindin levels in cerebellar Purkinje cells and reversed locomotor impairment in Npc1-/- mice. High-resolution respirometry analyses revealed that GSH-EE improved oxidative phosphorylation, coupled respiration and maximal electron transfer in cerebellum of Npc1-/- mice. Lipidomic analyses showed that GSH-EE treatment had not effect in the profile of most sphingolipids in liver and brain, except for some particular species in brain of Npc1-/- mice. These findings indicate that the specific replenishment of mGSH may be a potential promising therapy for NPC disease, worth exploring alone or in combination with other options., Graphical abstract fx1, Highlights • GSH-EE but not NAC restores mitochondrial GSH stores and protects against oxidative stress in NPC disease. • GSH-EE treatment improves motor coordination and extends survival in Npc1-/- mice. • GSH-EE therapy rescues cerebellar mitochondrial dysfunction in Npc1-/- mice. • GSH-EE treatment attenuates the liver phenotype in Npc1-/- mice.

Resumen La incidencia del cancer de mama (CM) ha aumentado progresivamente y aproximadamente el 15% de las mujeres son diagnosticados antes de los 45 anos. Este subgrupo de pacientes suelen tener tumores mas agresivos y seran tratadas con terapia sistemica (quimioterapia, terapia hormonal o ambos). Por otra parte, la tendencia a retrasar la edad de maternidad implica que una gran proporcion de pacientes con CM jovenes no han completado su deseo reproductivo. El impacto del tratamiento oncologico en la reserva ovarica depende de la edad de los pacientes, el tipo de esquema y la dosis recibida. El senologo deberia ser sensible al deseo gestacional y realizar una derivacion inmediata a la Unidad de Preservacion de la Fertilidad. Esta maniobra no implica un retraso en el inicio terapeutico de la enfermedad como demuestran nuestros resultados. En nuestro centro, 40 pacientes fueron sometidas a crioconservacion de ovocitos entre 2010 y 2015. La media de dias entre el diagnostico de CM y el inicio del tratamiento oncologico fue de 37,6 dias. El tiempo de estimulacion (inicio del tratamiento de estimulacion hasta la recuperacion de los ovocitos) presento una media de 12 dias (7-21). Por lo tanto, consideramos que las pacientes jovenes deben ser remitidas a una unidad de asesoramiento reproductivo, tal y como aconseja EUSOMA. La preservacion de fertilidad requiere de la participacion coordinada tanto del equipo de Oncologia y el equipo de Reproduccion Humana.

Background & Aims Acetaminophen (APAP) overdose is a major cause of acute liver failure (ALF). Mitochondrial SH3BP5 (also called SAB) and phosphorylation of c-Jun N-terminal kinase (JNK) mediate the hepatotoxic effects of APAP. We investigated the involvement of steroidogenic acute regulatory protein (STARD1), a mitochondrial cholesterol transporter, in this process and sensitization by valproic acid (VPA), which depletes glutathione and stimulates steroidogenesis. Methods Nonfasted C57BL/6J mice (control) and mice with liver-specific deletion of STARD1 (Stard1ΔHep), SAB (SabΔHep), or JNK1 and JNK2 (Jnk1+2ΔHep) were given VPA with or without APAP. Liver tissues were collected and analyzed by histology and immunohistochemistry and for APAP metabolism, endoplasmic reticulum (ER) stress, and mitochondrial function. Adult human hepatocytes were transplanted into Fah−/−/Rag2−/−/Il2rg−/−/NOD (FRGN) mice to create mice with humanized livers. Results Administration of VPA before administration of APAP increased the severity of liver damage in control mice. The combination of VPA and APAP increased expression of CYP2E1, formation of NAPQI-protein adducts, and depletion of glutathione from liver tissues of control mice, resulting in ER stress and the upregulation of STARD1. Livers from control mice given VPA and APAP accumulated cholesterol in the mitochondria and had sustained mitochondrial depletion of glutathione and mitochondrial dysfunction. Inhibition of ER stress, by administration of tauroursodeoxycholic acid to control mice, prevented upregulation of STARD1 in liver and protected the mice from hepatoxicity following administration of VPA and APAP. Administration of N-acetylcysteine to control mice prevented VPA- and APAP-induced ER stress and liver injury. Stard1ΔHep mice were resistant to induction of ALF by VPA and APAP, despite increased mitochondrial levels of glutathione and phosphorylated JNK; we made similar observations in fasted Stard1ΔHep mice given APAP alone. SabΔHep mice or Jnk1+2ΔHep mice did not develop ALF following administration of VPA and APAP. The ability of VPA to increase the severity of APAP-induced liver damage was observed in FRGN mice with humanized liver. Resumen Objetivo Analizar las recidivas de las pacientes diagnosticadas, tratadas y seguidas en nuestro centro por carcinoma ductal in situ de mama, y establecer que variables se asocian a un mayor riesgo de desarrollarlas. Pacientes y metodos Se ha realizado un estudio descriptivo retrospectivo de los casos de carcinoma ductal in situ diagnosticados y tratados en nuestro centro desde enero de 1999 hasta enero de 2012. Se excluyeron los casos en que coexistia componente infiltrante y aquellos con antecedente de neoplasia y/o radioterapia previa en la mama afecta. Las variables que se analizaron fueron: la edad de la paciente, el tamano tumoral, el grado nuclear, el estado de los margenes quirurgicos, el tipo de cirugia y el tratamiento complementario (radioterapia y hormonoterapia). Resultados Se estudiaron 162 casos de carcinomas in situ en el periodo 1999-2012. De estos, 117 (72,2%) fueron tratados con cirugia conservadora y 45 (27,7%) mediante mastectomia. Se produjeron 16 recidivas (9,9%) en el periodo estudiado. No se encuentran diferencias estadisticamente significativas en la tasa de recidivas en funcion del tamano tumoral, la distancia quirurgica al margen, el grado histologico ni la edad de la paciente. En el subgrupo de pacientes tratadas con tumorectomia, la supervivencia libre de enfermedad fue mayor en las que recibieron de forma complementaria radioterapia y hormonoterapia que en aquellas que solo recibieron uno o ninguno de los tratamientos (p = 0,001). Conclusion En el subgrupo de pacientes con carcinoma in situ tratadas con tumorectomia el riesgo de recidiva es 19 veces superior en los casos que no recibieron ningun tratamiento complementario que en aquellos tratados con tumorectomia, radioterapia y hormonoterapia (p = 0,001).

To identify the characteristics of uterine sarcomas and assess the impact of morcellation on prognosis.Case-control study. (Canadian Task Force classification II-2).Hospital Quiron-Dexeus, an academic hospital.Patients with uterine sarcoma histologically diagnosed and treated in our center between 1987 and 2013.All descriptive data, including type of surgery and clinical and pathological data, were reviewed. Survival analysis was performed comparing patients with hysterectomy/myomectomy without any type of morcellation and patients with morcellation during surgery.A total of 37 sarcomas were diagnosed during the study period. The most common symptom was metrorrhagia (50%). The indication for surgery was related to myoma growth in 40% of cases and to metrorrhagia in 37.1% of cases. Open surgery was performed in 23 patients (62.2%), and laparoscopy was performed in 9 (24.3%). Myomectomy was performed in 14 patients (37.8%), and 23 patients (62.1%) underwent hysterectomy as initial surgery. Morcellation for tumor extraction was done in 8 cases (21.6%). Survival analysis by surgical approach showed increased disease-free survival (DFS) in the laparotomy group compared with the laparoscopy group (median, 70.3 months vs 10.4 months; p = .018). Median DFS according to type of surgery was 6.3 months in morcellation cases, 11.9 months in vaginal fragmentation cases, and 149.9 months in nonmorcellated cases (p.002). The median time to progression was shorter in morcellated cases (laparocopic and vaginal) compared with nonmorcellated cases (11.9 vs 14.9 months; p.001). No statistically significant differences in prognosis were related to myomectomy versus hysterectomy; however, there were significants difference between morcellation and nonmorcellation cases.Taking into account the negative impact of morcellation in sarcomas, the use of this technique should be reconsidered in cases of myoma with atypical clinical presentation or symptomatology. Patients must be informed about the possibility of a nonidentified sarcoma and the possible impact on prognosis resulting from its morcellation.

Highlights • Breast sarcoma is a rare but aggressive entity. • Core biopsy will be the procedure of choice for the diagnosis. • Lymph node metastases are uncommon and surgery represents the only potentially curative therapy. • Tumor size and, specially, an adequate resection margin are the most important prognostic factors and determinant of long-term survival. • There is no consensus in the use of adjuvant therapy, it will depend mainly on the risk of tumor recurrence., Introduction Breast sarcomas are rare with an annual incidence of 4.6 cases/1,000,000 women. They can appear as primary forms or secondary to radiation therapy or chronic lymphedema. Presentation of case A 41 year old woman attended our hospital after having noticed an increase in the size of her fibroadenoma. The examination revealed a 7 cm retroareolar nodule. Breast sonography described a hypoechoic bilobulated lesion and MRI showed a large size polinodular image, suggesting a Phyllodes tumor. A core needle biopsy was performed with a histological result of low-grade fusiform cells sarcoma on Phyllodes tumor so we proceeded to surgical treatment with a mastectomy. After two years and a half she noticed a tough nodule over the mastectomy scar, which was resected with a histological result of fusiform cells sarcoma. Considering the diagnosis of recurrence of the disease, surgery was undertaken. Discussion Breast sarcoma is a rare but aggressive entity. Core biopsy is the procedure of choice for the diagnosis. Lymphatic spread is uncommon so nodal status in breast sarcoma is less informative. Staging study differs from other breast tumors and chest computed tomography is helpful since lungs are the predominant metastatic sites. The use of radiotherapy or chemotherapy is controversial and will depend on the risk of tumor recurrence. Conclusion Surgery represents the only potentially curative therapy for breast sarcoma. Tumor size and adequate resection margin are the most important prognostic factors. Approximately 80% of recurrences appear in the first two years.

IntroductionEndometriosis is defined as the emergence and growth of endome-trial tissue (glands and stroma) outside of the uterus. It affects 8–15%ofthepopulationandismostfrequentlyfoundintheovary.Extrapelvicimplantationmayoccurinanyorganortissue,butisrareintheabdom-inal wall. When it does occur there, a history of surgery, whetherlaparotomy or laparoscopy, is usually present.Malignancy of this type of endometriosis in an abdominal wall scarmay occur from just a few months until up to 18 years after surgery(Hensen et al., 2006). The case we discuss below deals with themalignant transformation of cesarean scar endometriosis with nodalmetastasis at preperitoneal level. Resumen Fundamento y objetivo El cancer de mama asociado al embarazo se define como aquel que aparece durante la gestacion o durante el primer ano posparto. Pacientes y metodo Estudio retrospectivo analitico observacional en el que se comparan 56 canceres de mama y embarazo (CME) diagnosticados entre 1976-2008 con 73 pacientes con cancer de mama no asociado al embarazo (CMNE). Se analizan los diversos datos demograficos, los factores pronosticos, el tratamiento y la supervivencia en ambos grupos. Resultados La prevalencia de CME en nuestro centro es 8,13/10.000 embarazos. La mayor frecuencia (62%) aparecio durante el puerperio. Los estadios son mayores en el CME respecto al CMNE, siendo el 31,3% avanzados en el CME frente al 13,3% en el CMNE (p iii frente al 15,8% del CMNE. En el grupo CME el 33,3% tenian receptores para estrogeno negativos, el 48,7% receptores para progesterona negativos y el 34,5% eran Her2Neu positivo frente al 22,2, 24,1 y 31%, respectivamente, en CMNE. En el 52,8% de CME aparecieron ganglios afectados frente al 33,8% del CMNE (p Conclusiones El peor pronostico que se observa en el grupo CME es debido posiblemente a la presencia de factores de pronostico adversos: metastasis ganglionares, receptores hormonales negativos y grado tumoral tipo iii , asi como al diagnostico tardio, con un porcentaje mayor de canceres avanzados.

PMCID: PMC3779525.-- et al., [Background & Aims]: The pathogenesis of alcohol-induced liver disease (ALD) is poorly understood. Here, we examined the role of acid sphingomyelinase (ASMase) in alcohol induced hepatic endoplasmic reticulum (ER) stress, a key mechanism of ALD. [Methods]: We examined ER stress, lipogenesis, hyperhomocysteinemia, mitochondrial cholesterol (mChol) trafficking and susceptibility to LPS and concanavalin-A in ASMase-/- mice fed alcohol. [Results]: Alcohol feeding increased SREBP-1c, DGAT-2, and FAS mRNA in ASMase+/+ but not in ASMase-/- mice. Compared to ASMase+/+ mice, ASMase-/- mice exhibited decreased expression of ER stress markers induced by alcohol, but the level of tunicamycin-mediated upregulation of ER stress markers and steatosis was similar in both types of mice. The increase in homocysteine levels induced by alcohol feeding was comparable in both ASMase+/+ and ASMase-/- mice. Exogenous ASMase, but not neutral SMase, induced ER stress by perturbing ER Ca2+ homeostasis. Moreover, alcohol-induced mChol loading and StARD1 overexpression were blunted in ASMase-/- mice. Tunicamycin upregulated StARD1 expression and this outcome was abrogated by tauroursodeoxycholic acid. Alcohol-induced liver injury and sensitization to LPS and concanavalin-A were prevented in ASMase-/- mice. These effects were reproduced in alcohol-fed TNFR1/R2-/- mice. Moreover, ASMase does not impair hepatic regeneration following partial hepatectomy. Of relevance, liver samples from patients with alcoholic hepatitis exhibited increased expression of ASMase, StARD1, and ER stress markers. Resumen Objetivo Determinar la variabilidad entre 2 sistemas de gradacion a la hora de catalogar la respuesta patologica a la quimioterapia neoadyuvante en una misma lesion. Material y metodos En 8 anos se han estudiado 51 pacientes con cancer infiltrante de la mama sometidas a quimioterapia neoadyuvante de forma consecutiva. Se ha establecido la respuesta patologica de cada caso siguiendo el sistema de Miller y Payne (MyP) y el de la carga tumoral residual (RCB) correlacionando los resultados de ambos sistemas en la misma lesion. Resultados Segun el sistema de gradacion de MyP las lesiones se clasificaron en grado 1 (6%), 2 (25%), 3 (27%), 4 (27%) y 5 (14%). La clase RCB fue 0 (13%), I (13%), II (54%) y III (20%). La correlacion entre MyP 5 y RCB 0, entre MyP 4 y RCB I, y entre MyP 2 y RCB III fue buena. Se detectaron mas discrepancias entre MyP 1, 2, 3 y RCB II. Conclusion La correlacion en la respuesta completa es buena entre los 2 sistemas de gradacion. Hay discrepancias en la clasificacion en la ausencia de respuesta entre ambos sistemas. Estas discrepancias pueden ser debidas a la inclusion del estado ganglionar en el sistema RCB.

Resumen Objetivos La quimioterapia neoadyuvante (QNA) es el tratamiento de eleccion en las pacientes con cancer de mama localmente avanzado. El objetivo de este estudio es evaluar la utilidad de QNA en tumores pequenos pero con factores pronosticos desfavorables. Material y metodos Se compara la respuesta a la QNA en pacientes con tamano tumoral T3-T4 frente a T1c-T2. Desde enero de 2000 a enero de 2011, 110 pacientes han recibido QNA. Se han revisado los datos demograficos, el tamano tumoral, los factores pronosticos, la respuesta radiologica y anatomopatologica a la QNA, segun la clasificacion de Miller y Payne (MP), y la supervivencia en ambos grupos. Resultados Se revisaron 76 casos en el grupo T1c-T2 y 34 en el T3-T4. En el grupo T3-T4 se observo en mayor porcentaje tumores con factores pronosticos desfavorables (afectacion ganglionar, receptores hormonales negativos y Her2Neu positivo). En el grupo T1c-T2, 23 casos (30,3%) recibieron antraciclinas y 53 (69,7%) antraciclinas y taxanos, mientras que en el grupo T3-T4, 10 (29,4%) y 24 (70,6%) casos recibieron dichas pautas, respectivamente. En el grupo T1c-T2 se observo en mayor porcentaje el subtipo tumoral receptores hormonales positivos frente a tumores Her2Neu positivos en los T3-T4. En relacion con la respuesta a la QNA no se observaron diferencias estadisticamente significativas en la valoracion radiologica o anatomopatologica, 38 pacientes (50%) T1c-T2 presentaron MP 4/5 frente a 16 (47,1%) en T3-T4. Conclusiones En las pacientes con cancer de mama precoz existe la misma respuesta anatomopatologica a la QNA que en el grupo T3-T4. En nuestro estudio, el unico factor independiente asociado a la respuesta patologica tumoral es el subtipo tumoral, presentando los tumores Her2Neu positivos mejor respuesta.

Una proporción significativamente mayor de mujeres que de varones vivían solas (el 38,2 frente al 10,9%), tenían dificultades para mantener su casa acogedora (el 15,6 frente al 8,8%) y llegar a final de mes con suficientes recursos monetarios (el 18,0 frente al 13,6%). Globalmente, hubo una mayor proporción de mujeres que de varones que presentaban problemas médicos, problemas cognitivos, depresión y dificultades para llevar a cabo actividades de la vida diaria. Los sujetos que tenían dificultades para llegar a final de mes con suficientes recursos monetarios también presentaron globalmente mayores prevalencias de problemas en todas las dimensiones de esta evaluación. Los sujetos que vivían solos tenían una prevalencia menor de problemas en todas las dimensiones, pero una prevalencia mayor de depresión.ConclusiónLa evaluación multidimensional en ancianos por correo postal es un procedimiento eficaz que permite identificar múltiples problemas sociales y sanitarios. A significantly greater proportion of women than of men lived single (38.2 vs 10.9%); they had difficulties to maintain its house cosy (15.6 vs 8.8%) and they had difficulties to make ends meet (18.0 vs 13.6%). Globally, there were a greater proportion of women than of men whom they had: medical problems, cognitive problems, depression, and difficulties to carry out activities of the daily living. Overall, subjects that have had difficulties to make ends meet had greater prevalences of problems in all the dimensions of this evaluation. On the other hand, subjects that lived single had lower prevalences of problems in all the dimensions except they had a greater prevalence of depression.ConclusionThe multidimensional evaluation in elderly patients by the postal method is an efficient procedure that allows identifying many socioeconomic and health problems. In addition, it is feasible to identify to most fragile subjects and latter on to carry out preventive and curative interventions on them as well as to make their follow up.

The Multicenter Automatic Defibrillator Implantation Trial (MADIT)-II has broadened the indications for cardioverter defibrillator implantation. We present a retrospective study designed to estimate the number of patients in Spain eligible for an implantable defibrillator according to the MADIT-II criteria. From January 1999 to October 2002, 758 consecutive patients were admitted to our center with the diagnosis of acute myocardial infarction. Sixty-seven had a left ventricular ejection fractionor = 30% (mean, 23[5]) and were not eligible for revascularization. Excluding patients older than 80 years and patients with marked co-morbidity, 47 patients met the MADIT-II criteria for an implantable defibrillator. After a mean follow-up of 18 months, there were 20 deaths, 6 of which were considered sudden. In conclusion, application of the MADIT-II criteria for defibrillator implantation may benefit 6% of the patients with myocardial infarction in Spain. This proportion translates as 4110 defibrillator implantations.

El Multicenter Automatic Defibrillator Implantation Trial (MADIT) II amplia las indicaciones de los desfibriladores automaticos implantables. Presentamos un estudio retrospectivo que tiene como objetivo conocer el numero de pacientes con criterios MADIT-II en nuestro entorno. Entre enero de 1999 y octubre de 2002, 758 pacientes fueron ingresados en nuestro servicio por un infarto agudo de miocardio. En 67 pacientes, la fraccion de eyeccion fue. 30% y no eran revascularizables. La fraccion de eyeccion media de este grupo fue del 23 ± 5%. Si excluimos a los pacientes de mas de 80 anos y a los que presentaban una marcada morbilidad asociada, 47 pacientes hubieran cumplido los criterios MADIT-II (6%). En un seguimiento medio de 18 meses hubo 20 muertes, 6 de ellas de forma subita. Si extrapolamos estos datos a nuestro pais, el numero anual de implantes se incrementaria hasta unos 4.110.
