Purpose Research on parental incarceration and the health of offspring is relatively scarce despite studies linking childhood adverse experiences to a range of physical and mental health conditions. This study aimed to estimate the associations between parental incarceration and sexual risk outcomes (early sexual onset, inconsistent condom use, and sexually transmitted infections [STIs]) in young adulthood. Methods We used logistic regression to estimate associations of sexual risk taking behaviors with parental incarceration during childhood in a sample of 3,972 participants in The National Longitudinal Study of Adolescent to Adult Health (Add Health) between 2001 and 2009. Results Parental incarceration was associated with early sexual onset (adjusted odds ratio [AOR] = 1.4, 95% confidence interval [CI] = 1.03–2.03) and STIs (AOR =2.0, 95% CI = 1.3–3.2). Maternal incarceration was associated with increased odds of early sexual onset (AOR = 3.6, 95% CI = 1.9–6.7), inconsistent condom use (AOR = 3.4, 95% CI = 1.3–8.9), and STIs (AOR = 5.5, 95% CI = 1.7–17.6). Additionally, paternal incarceration and parental incarceration occurring before age 10 were associated with STIs (AOR = 1.7, 95% CI = 1.1–2.8; AOR = 2.0, 95% CI = 1.1–3.7). Conclusions Parental incarceration, especially maternal imprisonment, is associated with risky sexual behavior and sexually transmitted infections in young adults in the United States. Intervening during or prior to the adolescent developmental period may ameliorate risky sexual behaviors and related health outcomes among children of incarceration parents.

© 2016 Jacobs Institute of Women's Health. Objective: To describe the reproductive and mental health of American Indian and Alaska Native (AI/AN) women, an understudied population. Methods: Data from the 2004 Behavioral Risk Factor Surveillance System survey were analyzed to determine the 1) prevalence of female sterilization among a nationally representative sample of reproductive age AI/AN women and 2) the association of female sterilization and poor mental health among AI/AN women compared with non-Hispanic White, non-Hispanic Black, and Hispanic women. Results: Nearly 25% of AI/AN women reported female sterilization, a prevalence higher than the comparison racial/ethnic groups (< .005). Adjusting for sociodemographic characteristics, AI/AN women reporting female sterilization had nearly 2.5 times the odds of poor mental health compared with AI/AN women not reporting female sterilization (= .001). The same magnitude of relationship between female sterilization and poor mental health was not found for non-Hispanic White, non-Hispanic Black, and Hispanic women. Conclusions: The prevalence of female sterilization is greater among AI/AN women compared with non-Hispanic White, non-Hispanic Black, and Hispanic women, and AI/AN women reporting female sterilization have higher odds of reporting poor mental health. Common cultural experiences, such as a shared ancestral history of forced sterilizations, may be relevant, and could be considered when providing reproductive and mental health services to AI/AN women.

Background Important outcome predictor variables for emergency medical admissions are the Manchester Triage Category, Acute Illness Severity, Chronic Disabling Disease and Sepsis Status. We have examined whether these are also predictors of hospital episode costs. Methods All patients admitted as medical emergencies between January 2008 and December 2012 were studied. Costs per case were adjusted by reference to the relative cost weight of each diagnosis related group (DRG) but included all pay costs, non-pay costs and infra-structural costs. We used a multi-variate logistic regression with generalized estimating equations (GEE), adjusted for correlated observations, to model the prediction of outcome (30-day in-hospital mortality) and hospital costs above or below the median. We used quantile regression to model total episode cost prediction over the predictor distribution (quantiles 0.25, 0.5 and 0.75). Results The multivariate model, using the above predictor variables, was highly predictive of an in-hospital death-AUROC of 0.91 (95% CI: 0.90, 0.92). Variables predicting outcome similarly predicted hospital episode cost; however predicting costs above or below the median yielded a lower AUROC of 0.73 (95% CI: 0.73, 0.74). Quantile regression analysis showed that hospital episode costs increased disproportionately over the predictor distribution; ordinary regression estimates of hospital episode costs over estimated the costs for low risk and under estimated those for high-risk patients. Conclusion Predictors of outcome also predict costs for emergency medical admissions; however, due to costing data heteroskedasticity and the non-linear relationship between dependant and predictor variables, the hospital episode costs are not as easy to predict based on presentation status.

Salt sensitivity (SS) of blood pressure (BP) affects 25% of adults, shares comorbidity with hypertension, and has no convenient diagnostic test. We tested the hypothesis that urine-derived exfoliated renal proximal tubule cells (RPTCs) could diagnose the degree of an individual's SS of BP.Subjects were selected who had their SS of BP determined 5 y prior to this study (salt-sensitive: ≥7 mm Hg increase in mean arterial pressure (MAP) following transition from a random weekly diet of low (10 mmol/day) to high (300 mmol/day) sodium (Na(+)) intake, N=4; inverse salt-sensitive (ISS): ≥7 mm Hg increase in MAP transitioning from a high to low Na(+) diet, N=3, and salt-resistant (SR):7 mm Hg change in MAP transitioned on either diet, N=5). RPTC responses to 2 independent Na(+) transport pathways were measured.There was a negative correlation between the degree of SS and dopamine-1 receptor (D1R) plasma membrane recruitment (y=-0.0107x+0.68 relative fluorescent units (RFU), R(2)=0.88, N=12, P0.0001) and angiotensin II-stimulated intracellular Ca(++) (y=-0.0016x+0.0336, R(2)=0.7112, P0.001, N=10) concentration over baseline.Isolating RPTCs from urine provides a personalized cell-based diagnostic test of SS index that offers advantages over a 2-week controlled diet with respect to cost and patient compliance. Furthermore, the linear relationship between the change in MAP and response to 2 Na(+) regulatory pathways suggests that an individual's RPTC response to intracellular Na(+) is personalized and predictive.

For decades, Africa has been plagued by political and ethnic conflict, the health ramifications of which are often not investigated. A crisis occurred recently in Kenya following the 2007 presidential election. Ethnic violence ensued, targeting the incumbent President Kibaki's Kikuyu people. The violence occurred primarily in Nairobi and the Rift Valley of Kenya. We sought to examine the association between exposure to the 2007/2008 Kenyan Crisis and birthweight. Using the 2008/2009 Kenyan Demographic and Health Survey (KDHS), we compared birthweights of infants in utero or not yet conceived during the 15 months after the political turmoil following the 2007 presidential election (exposed) to those who were born before the crisis (unexposed). There were 663 "exposed" and 687 "unexposed" infants. Multivariate regression was used. We examined the possibility of two-way and three-way interactions between exposure status, ethnicity (Kikuyu versus non-Kikuyu), and region (violent region versus not). Overall, exposure to the Kenyan Crisis was associated with lower birthweight. Kikuyu women living in a violent region who were exposed during their 2nd trimester had the greatest difference in birthweight in comparison to all unexposed infants: 564.4g lower (95% CI 285.1, 843.6). Infants of Kikuyu exposed during the 2nd trimester and living in a violent region weighed 603.6g less (95% CI 333.6, 873.6) than Kikuyu infants born during the unexposed period. Political unrest may have implications for the birthweight of infants, particularly among targeted populations. Given the adverse sequelae associated with lowered birthweight, these results suggest that particular attention should be paid to pregnant women and targeted ethnic groups following such events.

Standard methods to monitor tuberculosis (TB) treatment response rely on sputum microscopy and culture conversion. Alternatives to these methods are needed for those patients whose sputum tests are smear or culture negative. Here, we examine anti-phospholipid IgM antibody level changes as a biomarker for treatment response in smear positive TB patients. Serum samples were obtained from 40 pulmonary TB patients at the start and end of the intensive phase treatment (IPT) from the CDC-TB Trials Consortium randomized clinical trial in Kampala, Uganda. Samples were screened by ELISA for IgM levels against five phospholipids found in Mycobacterium tuberculosis and host cells. Lipid antigens included cardiolipin (CL), phosphatidyl inositol (PI), phosphatidyl ethanolamine (PE), phosphatidyl choline (PTC), and sphingolipid (SL). Levels of IgM against all phospholipids significantly decreased (p = 0.034, 0.001, 0.008 0.008, 0.040, respectively) following anti-TB drug treatment in patients without lung cavitary disease at baseline. The mean sensitivity of this test in these patients was 83% when the IgM response to a single lipid antigen was used; it was >90% when responses to 2 or more lipids were assessed. In contrast, cavitary TB patients showed an overall IgM increase, with a significant rise against PE (p = 0.025). There was no significant difference in the change in antibody levels between patients who remained culture-positive and those who culture-converted after 40 doses of drug therapy. The measurement of IgM anti-phospholipid antibodies may be a useful biomarker to monitor treatment response in non-cavitary TB patients.

We have prepared high surface area, conductive, mechanically robust, responsive polyaniline–carbon nanotube composite films. These were produced by filtration from dilute dispersions of polyaniline nanofibers and single-walled carbon nanotubes. Unlike polyaniline alone, these composites are mechanically stable, maintain large intractable surfaces and exhibit greatly enhanced response/recovery behavior to changes in their local environment. This is illustrated by exposing the films to ammonia.

Purpose This clinic- and community-based study of young women investigated the relationship between previous sexual abuse and early pregnancy, examining the effect of the developmental period in which sexual abuse occurred and type of sexual abuse, while also providing methodological advances in the assessment of distinctive sexual abuse and its sequelae. Methods Secondary data analysis using Cox proportional hazards models was conducted to determine the association between sexual abuse in childhood, in adolescence, or both, and risk of early pregnancy among 1,790 young women. In addition, this study examined the type of sexual abuse that occurred during each period. Results As compared with women with no history of sexual abuse, women who experienced sexual abuse only in childhood had a 20% greater hazard of pregnancy; women who experienced sexual abuse only in adolescence had a 30% greater hazard of pregnancy; and women who experienced sexual abuse in both childhood and adolescence had an 80% greater hazard of pregnancy. Across these periods, attempted rape and rape were associated with an increased hazard of pregnancy. The association between sexual abuse and pregnancy was mediated by age at first intercourse and moderated by a woman's education level. Conclusion This study provides evidence that both the developmental timing and the type of sexual abuse contributes to an increased risk for early pregnancy. The study findings indicate that sexual abuse leads to an earlier age of first sexual intercourse, which in turn increases the likelihood of an early pregnancy. Women with higher educational attainment are less likely to experience early pregnancy as a result of abuse.

The use of polycrystalline cubic boron nitride (PCBN) cutting tools in hard turning applications is continuously growing with the number of commercially available grades increasing, allowing new application areas to be explored. In order to take full advantage of the benefits offered by PCBN it is necessary to understand the behaviour of the material in application. Tool behaviour is influenced by many factors which include the composition of the PCBN material, the steel workpiece, the nature of the cutting operation, the cutting conditions and the tool geometry. The focus of this paper is the continuous turning of hardened steels. A significant amount of research has been carried out in this area and a literature review of the relevant work is presented. This identifies the primary wear modes and discusses the many theories proposed to explain the mechanisms contributing to PCBN tool wear and failure. The final section of the paper considers the critical factors that influence the behaviour of PCBN tools in continuous hard turning and how this knowledge can be applied to optimise tool performance.

Metadata only record This article examines female attitudes towards and experiences with agricultural production and land rights in Namaqualand, South Africa. Namaqualand was chosen as the study site because it is home to six different "coloured" reserves or rural areas reserved exclusively for peoples racially defined as "coloured," which have traditionally been held under communal management. Today, a large part of the land is still organized and managed by local officials. Another reason Namaqualand was selected for this research is due to the discrepancy between the lack of independent land rights for women and their important economic contributions. The empirical work in this article was based on 45 in-depth interviews given to women from the reserves and fieldwork conducted in Namaqualand common areas. The participants were recruited using an informal, word-of-mouth technique which lessens the statistical representation of the study group. However, strong efforts were made by the authors to include a diverse group of women taking into account many different characteristics like age and marital status. The research suggests that Namaqualand women typically access land through relationships with male relatives and because of this fact single women find it almost impossible to gain access to land and widowed or divorced women are susceptible to the loss of their land rights. The study shows Namaqualand women are involved in a wide range of agricultural production activities. Most women carry out those activities on land controlled by male relatives; however, a small proportion of women independently administer land. In this study women displayed conservative views on the patriarchal land use system and were unwilling to challenge traditionally gendered land rights. Even so, Namaqualand women expressed a strong desire for secure access to land.

With the transition to democracy in 1994, South Africa was faced with an enormous challenge in redressing the highly unequal and racialized pattern of land rights inherited from the colonial and apartheid past. In Namaqualand, a history of land dispossession and racial segregation presented the new government with a complex set of problems, which led to a series of distinct policy responses within the context of the wider national land reform programme. Land reform in Namaqualand aims to impact positively on local people's access to land, improve livelihood opportunities and develop the local economy. Unique features of the land reform process in Namaqualand include the reform of tenure in the former Coloured Rural Areas, the prominent role played by local municipalities and the heavy reliance on municipal commonage as a form of landholding. This study provides and overview of the process of land reform in Namaqualand since 1994, considering the three elements of tenure reform, land redistribution and restitution of historical land rights. It concludes that, while considerable progress has been made in provision of additional land to historically disadvantaged communities, obstacles remain in the area of post-transfer support to new and emerging farmers.

Novel polymer–nanotube composites have been fabricated by the introduction of polymeric material into the free volume contained in carbon nanotube (CNT) mats grown by chemical vapour deposition. The resulting composite material can then be peeled from the substrate to give a free-standing film, the properties of which are controlled by the nanotubes. The nanotube mat acts as a percolative network resulting in increases in surface and bulk conductivities by factors of 107 and 108, respectively. The presence of CNTs also causes the Young’s Modulus of the polymer film to double. It is shown that the presence of CNTs has no effect on the morphology of the polymer itself.

The fungal pathogen Candida albicans has the ability to grow as a biofilm on synthetic materials. This presents a significant problem in the clinical situation when the organism grows as a biofilm on medical devices resulting in infections which are resistant to antifungal agents. Determining the extent to which certain genes are involved in biofilm formation is an important aspect for the development of strategies to control pathogenic biofilms. ALS1 is a member of the ALS (agglutinin-like sequence) family, the protein products of which are implicated in attachment to endothelial cells and biofilm formation. The expression of ALS1 in biofilms grown on silicone elastomer, a material used in the manufacture of medical devices, and planktonically grown cells was investigated using a novel real-time quantitative reverse transcriptase PCR (q-RT PCR) on the LightCycler. This study demonstrates quantitatively that ALS1 is clearly up-regulated during biofilm growth. The real-time q-RT PCR assay described here has the potential to be used as an indicator of biofilm formation on medical devices.

Purpose: To test whether the degree of acculturation predicts subsequent alcohol use among Asian-American adolescents, and to test the moderating effect of parental attachment. Methods: This was a prospective study using a subsample of the National Longitudinal Adolescent Health data set. A nationally representative sample of 714 AsianAmerican boys (n 332) and girls (n 382) in grades 7–12 was analyzed. In-home self-report data were collected on two types of acculturation status, alcohol use, demographics, and parental attachment. After controlling for acculturation status and background variables at Wave I, logistic regression analysis was used to estimate the odds ratios to assess the association between acculturation and alcohol use at Wave II for adolescents. Results: Asian-American adolescents with the highest level of acculturation (English use at home, born in the United States) were identified as the highest risk group. For adolescents with low parental attachment, the odds of alcohol use were 11 times greater in the highly acculturated group than in the least acculturated group. However, the odds of alcohol use for adolescents with moderate or high levels of parental attachment did not vary across acculturation groups. Conclusions: Overall, a greater level of acculturation was associated with greater alcohol use. However, when parental attachment was taken into account, highly acculturated adolescents with moderate or high parental attachment had no greater risk than adolescents with same levels of parental attachment who were less acculturated. Thus, it appears that acculturation per se was not a risk factor unless it was accompanied by a low level of parental attachment. © Society for Adolescent Medicine, 2003

BSE, first identified in the UK in 1986 is thought to have arisen from feeding scrapie infected Meat and Bone Meal (MBM), produced under sub-optimal conditions, to cattle. For quality and safety reasons there is a requirement for a good analytical test for the surveillance of processed MBM. This study describes species-specific PCR assays for the identification of ovine, porcine and poultry species in MBM. A comparison between two distinct DNA extraction methods, i.e. the silica-guanidiumthiocyanate DNA isolation procedure and a commercial DNA extraction kit, is also presented. Application of this technology to species identification in industrial MBM was investigated as part of this study.

In studies aimed at understanding the impact performance of structures made from carbon-fibre composites, effects of structural geometry, material type and impact location have been investigated in skin-stringer panels representative of aircraft structure. Effects were investigated for low-velocity impacts to the skin in the bay between stringers, over a stringer foot, and over a stringer centreline. Detailed studies of the impact damage at these locations were investigated using ultrasonic techniques, and optical and electron microscopy. The damage characteristics were explained in terms of the proportion of energy absorbed through damage, such as delamination, and elastic effects, such as structural response of the panel.
