8. Law Enforcement and Patient Privacy Among Survivors of Violence: A Nationwide Mixed-Methods Study

Abstract

During the emergent treatment of violently injured patients, law enforcement (LE) officers and health care providers frequently interact. Both have duties to protect patient health, rights, and public health, however, the balance of these duties may feel at odds. The purpose of this study is to assess hospital-based violence intervention program (HVIP) representatives' experiences with LE officers among survivors of violence and the impact of hospital policies on interactions with LE officers.A nationwide survey was distributed to the 35 HVIPs that form the Health Alliance for Violence Intervention. Data regarding respondent affiliation, programs, and perceptions of hospital policies outlining LE activity were collected. Follow-up video interviews were open coded and qualitatively analyzed using grounded theory.Respondents from 32 HVIPs completed the survey (91%), and 22 interviews (63%) were conducted. Common themes from interviews were: police-patient interactions; racism, bias, and victims' treatment as suspects; and training and education. Only 39% of respondents knew that policies existed and were familiar with them. Most representatives believed their hospitals' existing policies were inadequate, ineffective, or biased. Programs that reported good working relationships with LE officers offered insight on how their programs maintain these partnerships and work with LE officers towards a common goal.Unclear or inadequate policies relating to LE activity may jeopardize the health and privacy of violently injured patients. Primary areas identified for improvement include clarifying and revising hospital policies, education of staff and LE officers, and improved communication between health care providers and LE officers to better protect patient rights.

Abstract

BACKGROUND: The COVID-19 pandemic has affected sexual and reproductive health (SRH) service use and unmet need, but the impact is unknown. We aimed to determine the proportion of participants reporting sexual risk behaviours, SRH service use and unmet need, and to assess remote sexually transmitted infection (STI) testing service use after the first national lockdown in Britain. METHODS: We used data from the National Surveys of Sexual Attitudes and Lifestyles (Natsal)-COVID cross-sectional, quasi-representative web survey (Natsal-COVID Wave 1). Adults aged 18-59 years who resided in England, Scotland, or Wales completed the survey between July 29 and Aug 10, 2020, which included questions about the approximate 4-month period after announcement of the initial lockdown in Britain (March 23, 2020). Quota-based sampling and weighting were used to achieve a quasi-representative population sample. Participants aged 45-59 years were excluded from services analysis due to low rates of SRH service use. Among individuals aged 18-44 years, we estimated reported SRH service use and inability to access, and calculated age-adjusted odds ratios (aORs) among sexually experienced individuals (those reporting any sexual partner in their lifetime) and sexually active individuals (those reporting any sexual partner in the past year). Unweighted denominators and weighted estimates are presented hereafter. FINDINGS: 6654 individuals had complete interviews and were included in the analysis. Among 3758 participants aged 18-44 years, 82·0% reported being sexually experienced, and 73·7% reported being sexually active. 20·8% of sexually experienced participants aged 18-44 years reported using SRH services in the 4-month period. Overall, 9·7% of 3108 participants (9·5% of men; 9·9% of women) reported being unable to use a service they needed, although of the participants who reported trying but not being able to use a SRH service at least once, 76·4% of participants also reported an instance of successful use. 5·9% of 1221 sexually active men and 3·6% of 1560 sexually active women reported use of STI-related services and 14·8% of 1728 sexually experienced women reported use of contraceptive services, with SRH service use highest among individuals aged 18-24 years. Sexually active participants reporting condomless sex with new partners since lockdown were much more likely to report using STI-related services than those who did not report condomless sex (aOR 23·8 [95% CI 11·6-48·9]) for men, 10·5 [3·9-28·2] for women) and, among men, were also more likely to have an unsuccessful attempt at STI-service use (aOR 13·3 [5·3-32·9]). Among 106 individuals who reported using STI testing services, 64·4% accessed services remotely (telephone, video, or online). Among 2581 women aged 25-59 years, 2·4% reported cervical screening compared with an estimated 6% in a comparable 4-month period before the pandemic. INTERPRETATION: Many people accessed SRH care during the initial lockdown; however, young people and those reporting sexual risk behaviours reported difficulties in accessing services and thus such services might need to address a backlog of need. Abstract

Traditionally, exploration and exploitation of resources have been viewed as mutually exclusive behaviours in which animals can either allocate time to gathering information or to using known resources. But these behaviours can also be viewed as opposite ends of a continuum, with intermediate behaviours that balance exploration and exploitation, such as information maintenance. Updating previously acquired information through information maintenance can allow animals in unpredictable environments to track changing environmental conditions. Theoretical studies predict that the degree of involvement in information maintenance should depend on environmental predictability – when the overall environment is less predictable, animals should update previously acquired information more frequently because such information is less certain and could change quickly. We tested this hypothesis by allowing wild food-caching mountain chickadees, Poecile gambeli, to visit and sample multiple feeders with temporarily stable, unlimited food for five consecutive winters. We used an index of feeder use breadth to explore how feeder visits across multiple feeders varied with environmental conditions. Each feeder visit is associated with information updating, and more information maintenance should be associated with distributing more visits across more feeders. While controlling for the total number of visits by each individual, we found that (1) chickadees redistributed feeder use among more feeders when environmental conditions were harsh and unpredictable, (2) juveniles had a higher feeder use breadth than adults, and individuals reduced their feeder use breadth as they aged, (3) better spatial learning and memory ability but not spatial cognitive flexibility was associated with smaller feeder use breadth and (4) learning associated with decreased food availability reduced subsequent feeder use breadth. Our data supported our predictions that factors affecting the predictability of resource information (environmental conditions and individual characteristics such as cognition and age) affect how individuals engage in information maintenance.

Abstract

Background the rare occurrence of collagenous gastritis (CG) makes its epidemiology difficult to investigate. We designed a study to determine the demographic and clinical characteristics as well as the associations of CG with other upper gastrointestinal diseases in a large national clinicopathological database. Methods from the IDEA database we extracted all patients with histopathologically documented CG and, in a case-control study, we compared 168 subjects with and 1,286,165 subjects without CG using odds ratios (OR) with their 95% confidence intervals (CI). Results the prevalence of CG was 13 per 100,000 EGDs. CG was significantly more common among female than male patients (OR: 1.69, 95% CI: 1.20–2.39) and was characterized by a bi-modal age distribution (first peak in patients aged 10–19, second peak primarily in females aged >60 years). CG patients presented with diarrhea (18%), anemia (12%), weight loss (11%), and vomiting (10%). CG was significantly associated with other lymphocytic disorders of the upper gastrointestinal tract, including celiac sprue (2.12, 1.55–2.88), duodenal intraepithelial lymphocytosis (3.71, 2.30–5.98), and lymphocytic gastritis (23.2, 10.9–49.5). CG persisted in 69% of patients who underwent multiple consecutive endoscopies. Conclusions the epidemiologic features of collagenous gastritis reflect on different etiologies contributing to its occurrence in children and adults.

Abstract

Collective cell behaviour during embryogenesis and tissue repair requires the coordination of intercellular junctions, cytoskeleton-dependent shape changes controlled by Rho GTPases, and integrin-dependent cell-matrix adhesion. Many different integrins are simultaneously expressed during wound healing, embryonic development, and sprouting angiogenesis, suggesting that there is extensive integrin/integrin cross-talk to regulate cell behaviour. Here, we show that fibronectin-binding β1 and β3 integrins do not act synergistically, but rather antagonize each other during collective cell processes in neuro-epithelial cells, placental trophoblasts, and endothelial cells. Reciprocal β1/β3 antagonism controls RhoA activity in a kindlin-2-dependent manner, balancing cell spreading, contractility, and intercellular adhesion. In this way, reciprocal β1/β3 antagonism controls cell cohesion and cellular plasticity to switch between extreme and opposing states, including epithelial versus mesenchymal-like phenotypes and collective versus individual cell migration. We propose that integrin/integrin antagonism is a universal mechanism to effectuate social cellular interactions, important for tissue morphogenesis, endothelial barrier function, trophoblast invasion, and sprouting angiogenesis.

Abstract

Nest construction is a widespread behaviour that is a critical fitness investment in many taxa. Nests provide stable environments for offspring production, development and at times adult survival. Avian nest structure and composition has been linked to insulative properties and the reduction of energetic requirements for incubating females. However, it is unclear how and why nest structure varies across landscapes within and between species, especially in cavity-nesting species. Here we examined nest size in a secondary cavity nester, the mountain chickadee, Poecile gambeli. Nests were measured across two years at two elevations varying in ambient temperature to test whether variation in nest size was associated with local ambient temperatures and internal nest cavity microclimate. Despite both elevations having high variation in ambient temperature during nest building in both years, there were no significant relationships between multiple nest measurements and temperature. Nest size was highly consistent between years within females, suggesting that individual females are the best predictor of nest size. Large variation in nest size, internal nest cavity microclimate temperature and ambient temperature during development in one year was not associated with significant differences in reproduction; but fluctuating asymmetry, a measure of developmental stress, was significantly lower in fledglings at the higher-elevation site independent of temperature. However, there were no significant differences between elevations in the second year for any reproductive parameter. Overall, our results suggest that chickadees do not adjust nest size in response to large differences in ambient or microclimate temperature and that large variation in nest size, both across and within sites, is a result of consistent individual preferences, which do not seem to have clear fitness consequences.
