Information on optimal techniques and outcomes following secondary reduction mammaplasty remains sparse, with only 280 patient cases reported in the literature to date. A total of 7179 medical charts of patients who underwent nononcological breast reduction at the Cleveland Clinic Health System from January 2001 to October 2023 were screened to identify whether the procedure was a secondary reduction. Patient charts were then reviewed to extract demographic, surgical, and outcome data. Patients were also contacted through the electronic medical record to fill out the BREAST-Q V2 questionnaire about postoperative satisfaction. Surgical techniques and satisfaction were compared for patients with complications versus those without. One hundred and twenty-two (1.7%) patients underwent secondary breast reduction. An inferior pedicle was the most used for both primary (n = 59, 48.4%) and secondary (n = 64, 52.5%) surgeries. The primary pedicle was recreated in 62 of the 84 (73.8%) rereductions where a primary pedicle was known. A total of 15 patients (12.3%) completed the BREAST-Q questionnaire. Body mass index was significantly and negatively associated with satisfaction with outcome (R=−0.66, p = 0.01), sexual well-being (R=−0.58, p = 0.04), and physical well-being (R=−0.69, p = 0.006). No patients experienced nipple-areola complex (NAC) necrosis following secondary reduction. However, 21 (17.2%) of patients experienced complications or breast fat necrosis following rereduction. Unmatched secondary pedicles showed a trend toward higher complication rates than matched secondary pedicles (31.3% vs. 12.9%, p = 0.051). The choice of pedicle in reduction mammaplasty is highly variable with prior studies demonstrating high patient satisfaction in most cases. This study aimed to examine the impact of pedicle type on clinical and patient-reported outcomes in patients undergoing reduction mammaplasty. A total of 588 patients underwent bilateral reduction mammaplasty with Wise pattern or modified Robertson incision by 13 surgeons at a single institution. Clinical outcomes were compared according to the pedicle type in all patients and BREAST-Q responders (32% response rate). Survey respondents were sub-grouped by resection volume, and the BREAST-Q satisfaction scores were compared. Among all included reduction mammoplasties, 439 (75%) were performed using inferior pedicles, and 149 (25%) using superior or superomedial pedicles. Responders and non-responders were similar in preoperative characteristics including age, body measurements, and comorbidities. Although a higher incidence of infection occurred among the responders, clinical outcomes were comparable across all pedicle types. A total of 187 patients completed the BREAST-Q. Compared to the superior pedicle group, respondents in the inferior pedicle group reported higher nipple satisfaction, even when adjusted for resection weight over 500 g. In contrast, the superior pedicle group had better sexual well-being scores, which persisted in resection weight less than 500 g (all p values <0.05). Inferior pedicles were associated with greater nipple satisfaction and superior pedicles were associated with greater sexual satisfaction. Our findings suggest that those with resections less than 500 g were more satisfied with superior pedicles whereas those with greater resections were more satisfied with inferior pedicles. Anterior transpedicular screw (ATPS) internal fixation of the lower cervical spine is an alternative for patients who cannot tolerate combined anterior and posterior surgery. The cervical vertebral anatomy varies with many factors, including age, gender, height, weight, and race. Three-dimensional (3D) CT reconstructions were performed on 122 patients. We selected the best level and measured the relevant parameters on both sides of the cervical vertebrae. We identified the entry point and orientation parameters of ATPS fixation for the C3–C7 vertebrae, and analyzed cervical pedicle parameters. Outer pedicle width (OPW), outer pedicle height (OPH), and pedicle axis length (PAL) were not correlated with body weight and age, but were positively correlated with body height (P < 0.05). After multiple linear regression analysis to exclude the effects of body height, no significant differences in OPW, OPH, and PAL were found between male and female subjects at most cervical levels. Pedicle cortical thickness was negatively correlated with age (P < 0.05). The percentage of pedicles with OPW <4.5 mm was: C3, 38.10%; C4, 34.92%; C5, 12.70%; C6, 9.52%; and C7, 0%. The percentage of pedicles with OPWs ≤4.5 mm, ≤4.0 mm, and ≤3.5 mm was higher among subjects with body height <160 cm. This study presents the internal anatomy of the cervical spine and provides accurate preoperative evaluation data for ATPS fixation. Previous studies mainly reported perpendicular and medial inclination insertion methods for C1 transpedicular screw insertion (TSI). Our recent study showed the ideal C1 transpedicular screw trajectory (TST) can be achieved by medial inclination, perpendicular or even lateral inclination insertion, and Axis C can be a reliable trajectory. The purpose of this study is to confirm Axis C is an ideal C1 TST by comparing the cortical perforation differences between actual C1 TSI and virtual C1 transpedicular screw insertion along Axis C (Virtual C1 Axis C TSI). Firstly, the cortical perforations of the transverse foramen and vertebral canal caused by C1 TSIs in twelve randomly selected patients were evaluated based on their postoperative CT data. Secondly, Virtual C1 Axis C TSIs were performed based on same patients' preoperative CT data. Thirdly, the cortical perforation differences between actual and virtual screws were compared. In actual C1 TSI group, there were thirteen locations of cortical perforation in the axial plane, with five sides in transverse foramen and eight sides in vertebral canal, the cortical perforation rate was 54.2%; the degree of perforation was mild in twelve locations and medium in one location. In contrast, there was no cortical perforation in Virtual C1 Axis C TSI group. Axis C is an ideal trajectory for C1 TSI, it can be utilized as a navigation route for computer assisted surgery system. Pedicle screw fixation provides one of the most stable spinal constructs. Their designs together with osseous characteristics have been known to influence the screw-bone interplay during surgical maneuvers and thereafter the fusion process. Various technical modifications to enhance screw performance have been suggested. This study evaluated the pull-out strength and axial stiffness of a novel pedicle screw design with variable thread geometry and pitch. The newly designed triple threaded pedicle screw is tapered, and has unique out-turned flanges to hold the cancellous bone and a finer pitch at its distal and proximal end to engage the cortical bone. Five lumbar and 4 lower thoracic cadaveric vertebrae were divided into hemivertebrae. A standard cancellous pedicle screw and the newly designed pedicle screw were inserted into each hemivertebra. Axial stiffness and peak pull-out force between the screw types were compared; a finite element analysis was also performed to additionally compare the pull out under toggle forces. In cadaveric study, the axial stiffness of the new screw was significantly better than that of the standard screw. However, the peak load between the screws was not statistically different. Finite element analyses suggested lesser stress at bone-implant interface for the new screw along with better axial stiffness under both co-axial and toggle forces. Our novel pedicle screw design with variable thread geometry demonstrates greater axial stiffness compared with the standard screws, and therefore is likely to withstand a greater surgical manipulation. Background Context: To our knowledge, there is no comparison study of two different Axis-based C1 transpedicular screw trajectory (TST) designs.Purpose: To compare two different Axis-based C1 TST designs.Study Design: The computed tomography (CT) morphologic analysis of the two different C1 Axis-based TST designs.Methods: Firstly, the design of Axis C/M and related measurements were made on a work station by using neck computed tomography angiography (CTA) data of 62 patients. The central axes (Axis M and Axis C) of C1 TST were designed by multiplanar reconstruction (MPR) technique. Based on Axis M and Axis C, the following parameters were measured: (A, A'), distance between the insertion point (IP) and the midline. (B, B'), distance between IP and the inferior aspect of C1 posterior arch. (C, C'), distance between IP and the C1 anterior cortex of the lateral mass along Axis M/C. (D, D'), insertion angle (IA) based on Axis M/C. (E, E'), the narrowest width of the inner medullary cavity (IMC) along Axis M/C.Result: The C1 TST can be designed by MPR technique based on CT volume scan. The design of Axis C trajectory was easier to Axis M trajectory. A, A' were 20.7±1.6mm and 20.6±1.9mm. Both B, B' were 2.0±0.6mm. C, C' were 27.2±2.1mm and 27.5±2.1mm. D, D' were 9.9±5.0°and 10.4±7.7°. E, E' were 5.0±1.3mm and 5.4±1.4mm. Among 62 patients (124 sides), Axis C can be achieved by medial inclination, perpendicular, lateral inclination method in 81.5%, 13.7%, 4.8% sides, respectively; Axis M can be achieved by medial inclination, perpendicular, lateral inclination method in 90.3%, 8.9%, 0.8% sides, respectively.Conclusion: Axis C can be a reliable trajectory for C1 transpedicular screw insertion as long as there is no displaced fracture in the C1 pedicle and lateral mass. Because of the individual differences, the ideal C1 TST can be achieved by medial inclination, perpendicular or even lateral inclination method, although its inclination direction is medial in the majority of patients. Background Context: There are several options for the stabilization of high-grade lumbosacral spondylolisthesis including transdiscal screws, the Bohlman technique (transdiscal fibular strut) and the modified Bohlman technique (transdiscal titanium mesh cage). The choice of an optimum construct remains controversial; therefore, we endeavoured to study and compare the biomechanical performance of these 3 techniques.Purpose: The aim of this study was to compare 3 types of transdiscal fixation biomechanically in an in vitro porcine lumbar-sacral spine model.Study Design/setting: Porcine cadaveric biomechanical study.Methods: 18 complete lumbar-sacral porcine spines were split into 3 repair groups, transdiscal screws (TS), Bohlman technique, and a modified Bohlman technique (MBT). Range of motion (L3 - S1) was measured in an intact and repaired state for flexion, extension, left/right lateral bending, and left/right torsion. To recreate a high-grade lumbosacral spondylolisthesis a bilateral L5/S1 facetectomy, removing the intervertebral disc completely, and the L5 body was displaced 50%-60% over the sacral promontory. Results were analyzed and compared to intact baseline measurements. Standard quasi-static moments (5 Nm) were applied in all modes.Results: All range of motion (ROM) were in reference to intact baseline values. TS had the lowest ROM in all modes (p=.006-.495). Statistical difference was found only in extension for TS vs. BT (p=.011) and TS vs. MBT (p=.014). No bone or implant failures occurred.Conclusion: TS provided the lowest ROM in all modes of loading compared to Bohlman technique and MBT. Our study indicates that TS results in the most biomechanically stable construct.Clinical Significance: Knowledge of the biomechanical attributes of various constructs could aid physicians in choosing a surgical construct for their patients. To directly compare robotic-versus fluoroscopy-guided percutaneous pedicle screw (PPS) placement in thoracolumbar spine trauma with a focus on clinically acceptable pedicle screw accuracy and facet joint violation (FJV). A retrospective chart review assessed 37 trauma patients undergoing percutaneous thoracic and/or lumbar fixation. Postoperative computed tomography images were reviewed by authors blinded to surgical technique who assessed pedicle screw trajectory accuracy and FJV frequency. Seventeen patients underwent placement of 143 PPS with robotic assistance (robot group), compared with 20 patients receiving 149 PPS using fluoroscopy assistance (control group). Overall, the robot cohort demonstrated decreased FJV frequency of 2.8% versus 14.8% in controls (P = 0.0003). When further stratified by level of surgery (i.e., upper thoracic, lower thoracic, lumbar spine), the robot group had FJV frequencies of 0%, 3.2%, and 3.7%, respectively, compared with 17.7% (P = 0.0209), 14.3% (P = 0.0455), and 11.9% (P = 0.2340) in controls. The robot group had 84.6% clinically acceptable screw trajectories compared with 81.9% in controls (P = 0.6388). Within the upper thoracic, lower thoracic, and lumbar regions, the robot group had acceptable screw trajectories of 66.7%, 87.1%, and 90.7%, respectively, compared with 58.8% (P = 0.6261), 91.1% (P = 0.5655), and 97.6% (P = 0.2263) in controls. There was no significant difference in clinically acceptable screw trajectory accuracy between robotic versus fluoroscopy-guided PPS placement. However, the robot cohort demonstrated a statistically significantly decreased FJV overall and specifically within the thoracic spine region. We sought to compare clinical outcomes in skeletonized versus pedicled left internal mammary artery (LIMA) grafts in elective coronary artery bypass grafting through a systematic review and meta-analysis. A comprehensive electronic literature search of PubMed, Ovid, Embase, and Scopus was conducted from inception to January 2020. Only short-term (30 days) studies which compared both techniques have been included in our analysis. Primary outcomes were post anastomosis flow rate and sternal wound infection rate (SWI); secondary outcomes were conduit length, acute myocardial infarction and 30-day mortality. Thirteen articles with a total of 6222 patients met the inclusion criteria. Except for the prevalence of diabetes mellitus being significantly lower in the skeletonized cohort (odds ratio [OR] 0.77 95% confidence interval [CI] [0.61, 0.97], P = 0.03), there were no differences in the preoperative demographics between the 2 groups. The skeletonized LIMA conduit was significantly longer when compared to the pedicled conduit (weighted mean difference -2.64 cm 95% CI [-3.71, -1.56], P < 0.0001). SWI rates were not significantly different in the skeletonized versus pedicled LIMA group (OR 0.71 95% CI [0.47, 1.06], P = 0.10). New onset of acute myocardial infarction and 30-day mortality rate was similar in the 2 groups (OR 1.04 and 0.97, respectively, P > 0.05 in both). The postanastomoses flow rate was higher in skeletonized LIMA (Weighted Mean Difference -11.51 mL/min 95% CI [-20.54, -2.49], P < 0.01). Harvesting the LIMA using the skeletonized technique is associated with higher postanastomosis flow rates and longer conduit lengths; with no difference in SWI and mortality rates when compared to the pedicled technique. Pedicle screw placement is the most common method of fixation in the thoracic spine. Use of the "superior facet rule" allows the operator to locate the borders of the pedicle reliably using posterior landmarks alone. This study investigated the ability of 3-dimensionally (3D)-printed thoracic vertebrae, made from combined thermoplastic polymers, to demonstrate pedicle screw cannulation accurately using the superior facet as a reliable landmark. An anonymized computed tomography scan of the thoracic spine was obtained. The T1–T12 thoracic vertebrae were anatomically segmented and 3D-printed. The pedicle diameters and distance from the midpoint of the superior facet to the ventral lamina were recorded. A total of 120 thoracic pedicles in 60 thoracic vertebral models were instrumented using a freehand technique based only on posterior landmarks. The vertebral models were then coronally cut and examined for medial or lateral violations of the pedicle after screw placement. A total of 120 pedicle screws were placed successfully within the 3D-printed thoracic vertebral models. Average measurements fell within 1 standard deviation of previous population studies. There were no pedicle wall violations using standard posterior element landmarks for instrumentation. There were 3 lateral violations of the vertebral body wall during screw placement, all attributable to the insertion technique. 3D-printed thoracic vertebral models using combined thermoplastic polymers can accurately demonstrate the anatomical ultrastructure and posterior element relationships of the superior facet rule for safe thoracic pedicle screw placement. Reconstruction of finger-pulp defects remains a challenge, although the treatment varies widely. The homodigital island flap based on dorsal branches of the proper digital artery (PDA) is one of the most popular methods for repair of finger-pulp defects. The aim of this study was to introduce our experiences of using modified dorsolateral proximal phalangeal island flaps for reconstruction of finger-pulp defects. From July 2016 to November 2018, 16 modified dorsolateral proximal phalangeal (MDPP) island flaps were performed for finger-pulp reconstruction. The comparison group included 11 patients treated with homodigital dorsal perforator (HDP) flaps. Flap survivals were assessed and active range of motion (ROM) of the joints, time to return to work, static 2-point discrimination and hand performance were analysed at final follow-up. Both the Group MDPP and the Group HDP had similar satisfactory flap survival. There were no differences in static 2-point discrimination, time to return to work and the ROM of the injured fingers. However, the Michigan Hand Outcomes Questionnaire (MHQ) summary scores of the satisfaction and aesthetics were much better in Group MDPP. The modified homodigital dorsolateral proximal phalangeal island flap is a reliable alternative for finger-pulp reconstruction with good functional and cosmetic outcome. [ABSTRACT FROM AUTHOR]

Cervical radiculopathy is a common cervical spine condition. There is a paucity of literature discussing the effect of partial pediculotomy and partial vertebrotomy for posterior endoscopic cervical foraminotomy (PPPV PECF) on cervical radiculopathy. We investigated the radiologic and clinical outcomes of this approach. This was a retrospective evaluation of 30 cases with cervical radiculopathy who underwent PPPV PECF. Preoperative, postoperative roentgenogram for evaluation of stability, computed tomography (CT) evaluation of foraminal dimensions, and area in sagittal view was performed. Three-dimensional reconstruction area of decompression evaluation was performed. Clinical outcomes of the visual analog scale, Oswestry Disability Index, and Macnab score were evaluated. There was no complication and recurrence in our PPPV PECF cohort during the study period. At preoperative, 1 week postoperative, and 3 months postoperative and final follow-up, the mean visual analog scale score had significant improvement, with scores of 7.6, 3.0, 2.1, and 1.7, respectively, P < 0.05, and also the mean Oswestry Disability Index, with scores of 73.9, 28.1, 23.3, and 21.5 respectively, P < 0.05. Macnab criteria showed all patients scoring good and excellent. Radiologic results showed PPPV PECF had a significant increase in decompression in the foramen area in all CT-measured parameters, as compared with the mean preoperative values; 1) sagittal area increased 60.1 ± 23.1 mm2, 2) CT craniocaudal length increased 4.0 ± 1.54 mm, 3) CT ventrodorsal length increased 4.0 ± 1.97 mm, and 4) 3-dimensional CT scan reconstruction decompression area increased 996 ± 266 mm2, P < 0.05. The pedicle-rib unit is regarded as an expanded pedicle and is a new approach to thoracic pedicle fixation. Previous studies were mostly focused on anatomic, radiographic, and biomechanical assessment. However, there is no study on anatomic relationship of bony structures in the pedicle-rib unit. This article investigates the anatomic relationships between transverse process, pedicle, rib, and corresponding vertebrae body in the pedicle-rib unit, so as to improve the clinical safety of pedicle-rib unit screw placement. Forty-five normal dry adult cadaver specimens were included. Anatomic parameters were measured. Anatomic parameters were 1) the anatomic relationship between the transverse process and the pedicle (the distance from the horizontal center line of the transverse process to the center and the superior and inferior border of the pedicle); 2) the anatomic relationship between the transverse process and the rib (the overlap portion between the costal neck and the transverse process); 3) the anatomic relationship between the transverse process and the vertebral body (the relative position of the horizontal midline of the transverse process and the vertebral body); and 4) the anatomic relationship between the pedicle and the rib (the overlap portion between the costal neck and the pedicle). The midline of the transverse process was located within the range of the superior and inferior borders of the pedicle in the thoracic spine. The distance between the midline of the transverse process and the center of the pedicle was the closest at T6 and T7, but the farthest at T11 and T12. The anterolateral part of the transverse process was mostly covered by rib from T1 to T8, but it was less covered from T9 to T12. The midline of the transverse process corresponded to the upper third or middle third of the vertebral body. The pedicle is completely or mostly overlapped by the rib from T1 to T9 on the anterolateral side, but from T10 to T12, the rib overlapped the pedicle partially. The pedicle-rib unit is a three-dimensional anatomic structure. The pedicle, transverse process, and rib are not completely in the same plane, and their positions vary in different segments. Pedicle-rib unit screw fixation is anatomically feasible. Setting the screw in the upper-middle thoracic spine is safer than setting it in the lower thoracic spine. [ABSTRACT FROM AUTHOR]

Latissimus dorsi flap is the gold standard for pedicled breast reconstruction but is limited by donor site scarring, which is difficult to conceal, and a high rate of post-operative seroma. We investigated the anatomic characteristics and feasibility of using a subcostal artery perforator (SCAP) flap, which was taken from the flank area and vascularised by a perforator artery through a subcostal pedicle, for a breast reconstruction. A literature review was undertaken to determine the anatomy of the SCAP flap pedicle and the vascular supply to this dermal-fat flap. Pedicled SCAP flaps were dissected on cadavers to identify the anatomy of the perforator arteries and the pedicle length. Pedicled SCAP flaps were then tunnelled through the chest area to evaluate their feasibility for breast reconstruction. Twelve SCAP flaps were prepared on six cadavers. Mean pedicle length was 17 cm (14.5–20 cm). Mean flap size was 13 × 18 cm (11.5 × 15.5–15 × 21 cm). For each flap, a pedicle of sufficient length was dissected to allow the tunnelling of the flap and to perform breast reconstruction with a dermal-fat flap of adequate size. In all cases, an SCAP flap was prepared, without taking muscle, to reduce the risk of parietal complications at the donor site in future clinical applications. Pedicled SCAP flap is a feasible option in breast reconstruction and could represent a less arduous surgical alternative to free flaps for some patients. Donor site scarring was acceptable. Abstract Background Anterior cervical trans-pedicle screw fixation was introduced to overcome some of the disadvantages associated with anterior cervical corpectomy and fusion. In vitro biomechanical studies on the trans-pedicle screw fixation have shown excellent pull-out strength and favorable stability. Comprehensive biomechanical performance studies on the trans-pedicle screw fixation, however, are lacking. Methods The control computed tomography images (C2-T2) were obtained from a 22-year-old male volunteer. A three dimensional computational model of lower cervical spine (C3-T1) was developed using computed tomography scans from a 22 year old human subject. The models of intact C3-T1 (intact group), anterior cervical trans-pedicle screw fixation (trans-pedicle group), and anterior cervical corpectomy and fusion (traditional group) were analyzed with using a finite element software. A moment of 1 N·m and a compressive load of 73.6 N were loaded on the upper surface and upper facet joint surfaces of C3. Under six conditions, four parameters such as the range of motion, titanium mesh plant stress, end-plate stress, and bone-screw stress were measured and compared on two treatment groups. Findings Compared with the intact model, the range of motions for treatment groups were decreased. Compared with cervical corpectomy and fusion, the titanium plant, C4 upper end-plate and C7 lower end-plate stresses in trans-pedicle group were reduced. No significant difference was discovered on bone-screw stress between the two groups for lateral flexion and rotation, but bone-screw stress is smaller in trans-pedicle group when compared with traditional group. With exception of individual difference, trans-pedicle group had better biomechanical results than traditional group in range of motions, titanium mesh plant stress, end-plate stress and bone-screw stress. Interpretation The trans-pedicle method has better biomechanical properties than the anterior cervical corpectomy and fusion making it a viable alternative for cervical fixations. The vertebral column, synonymous with spinal column or spine, and known colloquially as the backbone, forms the central axis of the body's skeleton. Superiorly, it articulates with the skull, and inferiorly, it articulates with the two hip bones which in turn give attachment to the lower limbs. The vertebral column comprises the following five regions in cephalocaudal sequence: cervical, thoracic, lumbar, sacral and coccygeal. The vertebral column contains the spinal cord within the vertebral canal and thereby protects the spinal cord from external trauma. Degenerative diseases of the vertebral column account for the vast majority of spinal disorders in clinical practice. Trauma, neoplastic disease and developmental anomalies account for most of the remainder of spinal problems. Optimal medical and surgical management of spinal disease are crucially dependent on accurate clinical and radiological diagnosis and these, in turn, are reliant on a sound understanding of the structural and functional anatomy of the vertebral column. Objective Spinal transpedicular screw fixation is widely performed. Broken pedicle screw rates range from 3%–7.1%. Several techniques have been described for extraction of broken pedicle screws. However, most of these techniques require special instruments. We describe a simple, modified technique for management of broken pedicle screws without extraction. No special instruments or drilling in an adjacent pedicle are required. Methods We used a high-speed air drill with a round burr. With C-arm fluoroscopy guidance, the distal fragment of a broken pedicle screw was palpated using free-hand technique through the screw entry hole. A high-speed air drill with a round burr (not a diamond burr) was inserted through the hole. Drilling began slowly and continued until enough space was obtained for new screw insertion. Using this space, we performed new pedicle screw fixation medially alongside the distal fragment of the broken pedicle screw. We performed the insertion with a previously used entry hole and pathway in the pedicle. The same size pedicle screw was used. Results Three patients were treated with this modified technique. New screw insertion was successful in all cases after partial drilling of the distal broken pedicle screw fragment. There were no complications, such as screw loosening, dural tears, or root injury. Conclusion We describe a simple, modified technique for management of broken pedicle screws without extraction. This technique is recommended in patients who require insertion of a new screw. Summary Background The conventional procedure of the sural neuro-fasciocutaneous flap enables the supply of blood and venous drainage by increasing the width of the adipofascial tissue and preserving tiny venous return routes. Moreover, skin graft is a common method for donor site closure, which may lead to some complications and influence the aesthetic appearance. We report modifications for a distally based sural neuro-fasciocutaneous perforator flap and a relaying flap for donor site closure without skin graft. Methods Twelve patients undergoing the modified flap for foot and ankle reconstruction were included in this study between 2014 and 2016. A peroneal-based perforator, a superficial vein, and the vascular axis of the sural nerve were included in the pedicle. A Z-shape skin incision was performed to explore the perforator vessels and a relaying island perforator flap was used to close the donor site. Results All flaps survived completely without necrosis. The area of the flaps ranged from 16 × 8 cm to 30 × 15 cm. The diameter width of the pedicle ranged from 1.0 to 2.0 cm. A relaying perforator island flap was used in 10 cases for donor site closure and no skin graft was performed. There were no serious donor site complications. All patients were satisfied with the aesthetic outcome postoperatively at the final follow-up. Conclusions The distally based sural neuro-fasciocutaneous perforator flap is considered a reliable method for foot and ankle reconstruction. Background Lumbar cortical bone trajectory (CBT) screw constructs provide an alternative method of pedicle screw fixation in minimally invasive spine surgery. In this study, we explored the CBT technique in further anatomic detail. The primary aims were to evaluate variations in anatomy relevant to CBT screw placement and to determine optimal screw location, trajectory, and length using measures obtained from computed tomography (CT) scans. Methods One hundred CT scans of the lumbar spine were reviewed, and 14 total measurements of entry points, trajectories, and lengths for placement of CBT screws were evaluated. Results Across all lumbar levels, the mean right pedicle–pars interarticularis junction length ranged from 7.58 ± 1.18 mm to 8.37 ± 1.42 mm, and the mean left pedicle–pars interarticularis junction length ranged from 7.95 ± 1.42 mm to 8.6 ± 1.74 mm. The pedicle–pars interarticularis junction from L1 to L5 was deemed too small for a 5-mm-diameter CBT screw in 35%, 24%, 17%, 17%, and 19%, respectively, on the right, and in 30%, 17%, 17%, 17%, and 20%, respectively, on the left. The average length of a screw placed along the cranial cortical bone of the pedicle ranged from 27 ± 2.5 mm to 30.5 ± 3.4 mm, and the angle of the screw with respect to the vertebral body endplate ranged from 44 ± 4.1° to 48 ± 6.2°. Conclusions Improved anatomic knowledge relevant to CBT screw placement for lumbar fixation offers the potential for improving outcomes and reducing complications. Background Context: To our knowledge, there is no study that has systematically analyzed the relationship between C1 transpedicular screw trajectory and V3 segment of vertebral artery (VA V3 segment).Purpose: To study the relationship between C1 transpedicular screw trajectory and VA V3 segment.Study Design: A morphologic computed tomography angiography (CTA) analysis of the spatial relationship between C1 transpedicular screw trajectory and VA V3 segment.Methods: Measurements were made on a workstation by using CTA data of 62 patients. Firstly, parameters related to the relationship between C1 vertebral artery groove (VAG) and vertebral artery (VA) were measured: (A) the shortest distance between the posterosuperior aspect of C1 posterior arch and VA; (B) distance between the outer aspect of VAG and VA; (C) distance between midpoint of VAG and VA; and (D) distance between the inner aspect of the VAG and VA. Then, the central axis of trajectory perpendicular to the coronal plane (axis P) and the central axis of trajectory with a medial inclination (axis M) were designed for the basis of measurements. Parameters related to the relationship between axis P/M and VA V3 segment were measured respectively: (E, E'), distance between insertion point and anterior aspect of VA along axis P/M; (F, F'), the shortest distance between axis P/M and the outer cortex of C1 transverse foramen; and (G, G'), the narrowest width of C1 internal medullary canal along axis P/M.Result: A, B, C, and D were 1.7±1.0 mm, 1.6±0.9 mm, 1.5±0.7 mm, 2.3±1.1 mm, respectively. Background To collect a baseline computer software aided normative morphometric data of thoracic spine in the Indian population and analyze it to give pre-procedural guidelines to clinicians for safe surgical and anaesthetic procedures in the thoracic spine. Methods CT scans of thoracic spine of patients free from spinal disorders were reviewed in a total of 600 vertebrae in 50 patients. Parameters recorded with the help of computer software were pedicle width, length and height, transverse pedicle angles, chord length, canal dimensions, body width and height, spinous process angle and transverse process length. Results Pedicle width decreased from T1 (9.27 ± 1.01) to T4 (4.5 ± 0.93) and increased to T12 (8.31 ± 1.83). At T4 76% and at T5 62% of the pedicles were smaller than 5 mm and would not accept 4 mm screw with 1.0-mm clearance. However, at T1 2%, at T11 7% and at T12 8% would not accept a 4 mm screw. Chord length gradually increased in upper thoracic vertebrae and was relatively constant in middle and decreased in lower thoracic vertebrae. Shortest estimated chord length was at T1 (30.30 ± 2.11). On an average, from T1 to T6 and at T11 and T12, a screw length of 25–30 mm could be accommodated and from T7 to T10, 30–35 mm screw length could be accommodated. Transverse pedicle angle decreased from T1 (35.4 ± 2.21) to T12 (−9.8 ± 2.39). Canal dimensions were narrowest at T4/T5 (20.02 ± 1.23) in anteroposterior and 21.12 ± 1.23 in interpedicular diameters. Spinous process angle increased from T1 (30.11 ± 6.74) to T6 (57.89 ± 9.31) and decreased to 16.21 ± 7.38 at T12. Transverse process length increased from T1 to T7 (23.54 + 2.12 to 31.21 + 1.91) and then decreased to 12.11 + 2.3 at T12. Vertebral body dimensions showed increasing trends from T1 to T12. Conclusions A thorough knowledge of anatomical and radiological characteristics of the spine and their variations is essential for the clinicians. Data collected in the present study provides baseline normative values in Indian population and will help in guiding safe and effective completion of both surgical and anaesthetic procedures in the thoracic spine. Computer software aided morphometric data can help in selecting appropriate size and optimal placement of the implant with minimal procedural difficulties and complications during spine surgery. Treatment of a large articular cartilage defect in the distal radius poses a significant challenge to hand surgeons. To reduce the development of secondary degenerative arthritis, restoration of the articular surface is preferable. Summary Purpose The aim of this study was to elucidate the extent to which pedicled anterolateral thigh (ALT) flaps can reach in reconstruction of abdominal wall defects. Methods A total of 60 pedicled ALT flaps were raised from cadavers and were experimentally transferred to the abdominal region. The distance between the umbilicus and the most cranial point of the flap after transfer was defined as cranially reachable distance (CRD). Three issues were evaluated: (1) the difference in the CRD when the flap pedicle was positioned superficial or deep into the rectus femoris (RF) and sartorius (SA) muscles; (2) the difference in the CRD in those cases where the main artery of RF arises from the descending branch of the lateral femoral circumflex artery, and is preserved or severed; and (3) maximum values of CRD. Results (1) CRD was significantly greater when the pedicle was passed deep into the muscles (−2.5 ± 3.8 SD cm) compared with superficial (−5.8 ± 3.3 SD cm), indicating placement of pedicles beneath the two muscles enables additional extension. (2) CRD was significantly greater for the severed condition (−0.3 ± 4.0 SD cm) than for the preserved condition (−3.3 ± 4.1 SD cm), indicating severing the main artery of RF allows additional extension. (3) Out of the 60 specimens, the CRD was cranial to the umbilicus in 17 flaps, indicating pedicled ALT flaps can reach the umbilicus in less than one-third (17/60) of cases. Conclusion Pedicled ALT flaps can reliably reach regions inferior to the umbilicus. However, for defects superior to the umbilicus, other reconstructive options should be considered. Background context Pedicle subtraction osteotomy (PSO) is the most commonly recommended technique for the correction of local post-traumatic thoracolumbar deformity; however, the surgical results are not always satisfactory because the possibly damaged upper disc is preserved, and all the posterior elements are resected. Purpose The aim was to compare the results of standard PSO and modified PSO in the treatment of post-traumatic thoracolumbar kyphosis. Study design This was a retrospective multicenter comparative clinical study. Patient sample A total of 86 patients were included in the final analysis. Outcome measures The outcome measures included local Cobb angle of the kyphosis, visual analog scale (VAS) score, and Oswestry disability index (ODI) score. Methods The upper disc was resected, and the inferior wall of the index pedicle and the lower facet joint were preserved in the modified PSO. Patients with focal kyphosis greater than 30° who were treated with one-level osteotomy, without the presence of spine neoplasm, infection, or previous surgery, were included. The measurements included the VAS score, ODI score, and preoperative and postoperative Cobb angles. Results Forty-two patients in the modified PSO group and 44 in the standard PSO group were included in the final analysis. The mean surgical time and blood loss were similar between the two groups. Both the VAS and ODI scores had improved significantly at the final follow-up in the two groups. The mean Cobb angle significantly improved from 39.6° to 5.6° in the modified PSO group and from 39.1° to 4.8° in the standard PSO group, with no significant difference between the two groups preoperatively or at the final follow-up. Conclusions The modified PSO provides an alternative method with which to correct kyphotic deformity in patients with post-traumatic thoracolumbar kyphosis. [ABSTRACT FROM AUTHOR]

Objective: The objective of this study is to evaluate the incidence and prognostic factors of breach rates following the placement of C2 pedicle screws using the anatomic, freehand technique. Methods: We retrospectively reviewed the medical records of all patients who underwent C2 transpedicular instrumentation over six years at a single institution. All intraoperative, image-guided techniques were excluded. Breaches were ascertained from immediate postoperative CT images. All images were analyzed by three independent reviewers. The screw length was correlated with (1) the breach rate and (2) the breach severity. Severity of the breached screws reflects the screw circumference (0-360°) perforating the pedicle wall (Grade 1-Grade 4). Results: Of the 341 C2 pedicle screws inserted in 181 patients, the average screw length was 22.93 ± 3.7 mm. The average distance from the foramen transversarium to the screw insertion point was 13.17 ± 2.63 mm. The distance from the medial rim of the pedicle to the dura of spinal cord was 3.53 ± 1.57 mm. Of the 341 screws, the overall breach rate was 17.3% (n = 59). Of the 59 breaches, 89.83% of screws (n = 53) breaching the spinal canal was statistically significantly higher than the 10.17% of screws (n = 6) breaching the foramen transversarium (p < 0.001). Moreover, 27 (45.8%) were Grade 1, 16 (27.1%) Grade 2, 6 (10.2%) Grade 3, and 10 (16.9%) Grade 4. None of the C2 breaches resulted in neurological sequela. No association was found between breach rate and gender, race or age. While the average screw length was 22.93 ± 3.7 mm [12-34 mm], screw length did not predict a cortical violation (p = 0.4) or severity of the breach (p = 0.42) in a multiple regression model. Conclusions: In this cohort study on the anatomic freehand placement of C2 pedicle screws, the breach rate was 17.3%. Lateral breaches were more common than medial breaches. Screw length was not statistically correlated with cortical violation or severity of breach. Therefore, screw length is not a prognostic factor for C2 pedicle screw misplacement. The cone synapse is a complex signaling hub composed of the cone photoreceptor terminal and the dendrites of bipolar and horizontal cells converging around multiple ribbon synapses. Factors that promote organization of this structure are largely unexplored. In this study we characterize the localization of adhesion and scaffolding proteins that are localized to the cone synapse, including alpha-n-catenin, beta-catenin, gamma-protocadherin, cadherin-8, MAGI2 and CASK. We describe the localization of these proteins during development of the mouse retina and in the adult macaque retina and find that these proteins are concentrated at the cone synapse. The localization of these proteins was then characterized at the cellular and subcellular levels. Alpha-n-catenin, gamma-protocadherin and cadherin-8 were concentrated in the dendrites of bipolar cells that project to the cone synapse but were not detected or stained very dimly in the dendrites of cells projecting to rod synapses. This study adds to our knowledge of cone synapse development by characterizing the developmental localization of these factors and identifies these factors as candidates for functional analysis of cone synapse formation. Background: Anterolateral thigh perforator flaps (ALTPFs) have long been considered workhorse flaps for head and neck reconstructions. However, in some instances, anteromedial thigh septocutaneous perforator flaps (AMTPFs) are easier to harvest than ALTPFs. This study aimed to determine when and how the AMTPF should be considered the first choice as a reconstructive tool.Methods: A retrospective cohort study was performed in the Department of Oral and Maxillofacial Surgery-Head & Neck Oncology, Shanghai Ninth People Hospital, from January 2014 to December 2018. Both the perforator-pedicle relationships and postoperative functional outcomes were compared.Results: A total of 168 patients were included in this study, among whom 49 underwent AMTPF reconstructions. The AMTPF perforators in this study were mostly septocutaneous (n = 38, 77.5%). Notably, the AMTPF perforators mainly originated from the medial branch of the descending branches of the lateral circumflex femoral arteries (n = 44, 89.8%), while for the other 5 patients, the perforators were found to originate from the superficial femoral arteries. Additionally, the average pedicle length of the AMTPFs was 8.7 cm (range: 6.0 to 13.0 cm), which was comparable to that of the ALTPFs (8.9 cm) (p = 0.442). The postoperative complication rate and functional outcomes were comparable between the two groups. The algorithm and harvesting approaches are presented.Conclusions: The role of AMTPF should not be downplayed. Abstract: Background: The comparative safety of breast reconstruction in obese patients remains to be clearly defined. This study utilized multi-institutional data to characterize the effect of body mass index (BMI) on breast reconstruction outcomes. Methods: Utilizing Current Procedural Terminology (CPT) codes, patients undergoing tissue expander, pedicled transverse rectus abdominis myocutaneous (TRAM) flap, latissimus dorsi flap, and free flap breast reconstruction were identified in the National Surgical Quality Improvement Program (NSQIP) database. Patients were stratified as obese (BMI ≥ 30) and non-obese (BMI < 30). Overall postoperative morbidity, flap complications, non-flap complications, and reoperation rates were compared among the groups. Results: Of 12,986 patients who underwent breast reconstruction, 3636 (28.0%) were obese. Overall morbidity was significantly elevated in obese patients across all forms of reconstruction (p < 0.05). BMI was correlated with increased surgical complications for tissue expander, pedicled TRAM, and free flap reconstructions (OR = 1.09, OR = 1.05, OR = 1.10, respectively; p < 0.05). Medical complications were higher in obese patients undergoing tissue expander and pedicled TRAM reconstructions (p = 0.001 and p < 0.001), but no significant difference was observed in latissimus and free flap reconstruction patients. Compared with obese tissue expander recipients, obese patients reconstructed using autologous tissue had higher rates of reoperations (12.8% versus 9.1%), overall morbidity (18.0% versus 9.5%), surgical (12.7% versus 8.3%), and medical complications (9.0% versus 2.2%). Conclusions: The NSQIP database allows for evaluation and comparison of reconstructive outcomes in the obese population. Increased BMI was associated with higher morbidity in autologous reconstruction than tissue expander reconstruction. Abstract: In this study we investigated the impact of the time at which vascular endothelial growth factor (VEGF) was given on the overall survival of flaps and time of the achievement of full perfusion in the rat epigastric flap model. Epigastric flaps were raised in 144 rats. VEGF was given to 108 rats at different time points. Perioperatively blood flow, velocity, haemoglobin (Hb) concentration, and oxygen saturation were measured non-invasively using laser-Doppler spectrophotometry. On postoperative days 3, 5, 7, or 10, the pedicle of the flap was ligated. One week later, the flap was harvested for immunohistochemical assessment of the density of microvessels. Differences significant from those in the control group were found in all specimens 3 and 5 days after the first operation, whereas no differences in the area of necrosis were detected after 7 and 10 days. Preoperative values of measured Hb (p <0.0001, 95% confidence interval (CI) 0.57 to 1.02), flow (p <0.0001, 95% CI −0.32 to −0.13), and velocity (p <0.0001, 95% CI 0.48 to 1.89) predicted necrosis. After ligation of the pedicle, SO2 (p <0.0001, 95% CI −1.12 to −0.83) and flow (p <0.0001, 95% CI −0.38 to −0.26) were prognostic factors. The topical use of VEGF increases overall survival of flaps and decreases the time needed for a free flap to achieve full perfusion. The time of application of VEGF has no part in the postoperative course. The perfusion of vascularised free flaps in their wound beds was achieved between postoperative days 5 and 7 in the control group and postoperative days 3 and 5 after VEGF had been given. Abstract: The aim of the present study was to describe the clinical application of combined fixation in the cervical spine using posterior transfacet and pedicle screws. Ten patients with cervical disorders requiring stabilization were treated from May 2006 to December 2008. The operative details varied depending on indication, the need for decompression, and the number of levels to be included in the spinal construct. Radiographic analysis of the fusion was performed after surgery. A total of 23 transfacet screws were inserted at or caudal to the C4/5 facet. A total of 21 pedicle screws were placed. All patients underwent operative treatment without neurovascular complications. Fusion was achieved in all patients. When performed appropriately, the method of using posterior transfacet screws in the caudal cervical joints combined with pedicle screw fixation in the cephalic cervical spine is reliable and deserves more widespread use. Abstract: Coverage of the weightbearing heel poses a unique technical challenge to the reconstructive surgeon. In the present study, we share our clinical experience with the use of the medial plantar artery-based flap for coverage of tissue defects around the heel. Eighteen medial plantar artery flaps performed from January 1996 to December 2009 were included. All the procedures were performed by 2 surgeons at Aga Khan University and Hospital (Karachi, Pakistan) and Bahawal Victoria Hospital (Bahawalpur, Pakistan). Of the 18 patients, 16 were male and 2 were female. The indications were traumatic loss of the heel pad in 13, pressure sores in 2, and unstable plantar scars in 3. All the flaps were raised as sensate fasciocutaneous pedicled flaps based on the medial plantar artery. All the flaps healed uneventfully without major complications. The donor site was covered with a split-thickness skin graft, and we had partial graft loss in 1 case. The sensate flaps had slightly inferior protective sensation compared with the normal side. From our results, we suggest that the medial plantar artery flap is a good addition to the existing armamentarium. It provides tissue to the plantar skin with a similar texture and an intact protective sensation. The technique is easier to master compared with free microvascular flaps and has less risk of any functional donor site morbidity. [Copyright &y& Elsevier]

Summary: Perforator flaps are well established as a versatile option in reconstructive surgery that provide thin, pliable cutaneous or fasciocutaneous tissue. They are particularly useful in the reconstruction of large shallow defects, such as after the release of a burn contracture, however there are situations where the additional time spent islanding these flaps may be unnecessary, and the flap is then essentially a fasciocutaneous flap based, but not islanded, on a perforator artery. This paper documents a series of 22 severe burn contractures in 17 patients reconstructed with fasciocutaneous flaps in this way. The arteries were all located pre-operatively with a hand held Doppler probe, around half at the site of a known perforator and half by systematic scanning of the surrounding skin for an ad hoc perforator. All patients achieved a good functional range of motion. There were three cases of partial superficial flap necrosis treated with split skin grafting. The functional and aesthetic outcomes were far better then those expected with split skin grafting, and published series show that contractures treated with perforator flaps are unlikely to ever need further surgery. Without islanding the flap it becomes a feasible option for reconstructing these patients even in the extremely resource poor environment in which they often present, and is an option for all plastic surgeons to consider in the reconstruction of large superficial defects. Summary: The use of technology in the treatment of degenerative spinal diseases has undergone rapid clinical and scientific development. It has been extensively studied in combination with various techniques for spinal stabilization from both the anterior and posterior approach. Minimally invasive and instrumental approach via posterior fixation is increasingly being used for the treatment of adult degenerative disc disease, stenosis, and deformity of the lumbar vertebrae. Posterior access to the lumbar disc spaces for posterolateral fusion scan has been technically challenging, frequently requiring the use of an approach surgery for adequate exposure. For successful surgery and suitable instrumental design, adequate anatomical knowledge of the lumbar vertebra is also needed. Anatomic features of lumbar vertebrae are of importance for posterior screw fixation technique. The morphometry of L1–L5 has been studied to facilitate the safe application of pedicle screws. Thus, we aimed to evaluate the morphometric landmarks of lumbar vertebrae such as pedicle, vertebral body, vertebral foramen, intervertebral space height and volume for safe surgical intervention using a posterior fixation approach to offer anatomical supports for lumbar discectomy, stenosis and cases of deformity. The features of the L1–L5 vertebral body, the detailed morphometric parameters of lumbar vertebrae and the intervertebral space were analyzed using computerized tomography scan, magnetic resonance imaging and also dry lumbar vertebrae. Summary: Robbins described the inferior pedicle breast reduction in 1977. Since this time it has proved to be a safe and reliable technique. Favourable results have been reported with regard to breast feeding and nipple sensation. A common criticism of the technique is its tendency to inferior descent (pseudoptosis or bottoming out). This can leave a significantly empty upper pole. By a series of evolutionary modifications over the last 20 years, a technique has developed that overcomes the shortfalls of the inferior pedicle technique. Abstract: Background context: Of the injuries involving the lumbar spine, pedicle fractures are among the least common; those involving bilateral pedicles are rare. Purpose: The aims of the study were to provide the first documentation of bilateral pedicle fractures at two consecutive levels after a gunshot, to review the mechanism of injury, and to evaluate a nonfusion treatment option for pedicle fractures. Study design: This is a technical note and case report. Outcome measure: The outcome measures were lumbar range of motion, return of motor and sensory functions, and return to normal activities. Methods: A 20-year-old male sustained bilateral pedicle fractures at L4 and L5 with a massive dural tear, progressive neurologic deficits, and urinary incontinence. He underwent repair of the dural tear and lag screw fixation of the pedicle fractures without fusion. Results: The patient had full range of motion of his lumbar spine, full strength in his lower extremities, and bladder control. Conclusions: This is the first report of bilateral multilevel lumbar pedicle fractures after a single penetrating gunshot wound. The case documents this injury pattern after a gunshot, reviews the mechanism of injury, and presents the successful application of a nonfusion treatment option. [Copyright &y& Elsevier]

Abstract: Patients with sickle cell disease (SCD) who have deformities and vertebral fractures due to osteoporosis may require surgery. Spinal surgeons must become familiar with the vertebral morphometry of patients with SCD and to that aim we have examined the morphometry of the thoracolumbar spine in these patients. A cohort of 100 patients with SCD was examined using plain thoraco lumbar anteroposterior/lateral radiographs and dual energy X-ray absorptiometry (DEXA). Vertebral morphometry (vertebral body diameters, pedicle, spinal canal and deformity) was assessed for different age groups. Results were compared to published studies of healthy subjects. The vertebral dimensions for the 16–20-year and the 21+-year-old groups were significantly smaller for females than males at most spinal levels, while measurements in the 6–10years and 11–15years age groups were similar across both sexes at most levels. No significant statistical difference was found between the diameters of the right and left pedicles. With the exception of the sagittal diameter, most of the dimensions of the vertebral bodies measured in SCD patients were less than those of healthy individuals; multiple deformities were also observed. Low bone density was noted in 32 patients. Our data highlight the differences in vertebral bone mineral density, anatomy and deformities in patients with SCD compared to healthy individuals. When considering surgical intervention for patients with SCD, it is important that pre-operative radiography and planning is undertaken, and that the surgeon is familiar with the geometry of the pedicles of the thoracolumbar vertebrae necessary for the safe insertion of pedicle screws. Lateral mass screws are the most commonly used fixation technique in the Subaxial Cervical Spine (SCS), their main advantages being that they are easy to insert and safe in their application. Pedicle screws are significantly longer, are quite challenging to insert in most settings and are accompanied by the risk of serious complications such as vascular and neural injuries. We have therefore developed a new technique, which permits safe insertion of long screws in the SCS. A radiological evaluation was carried out to determine the maximum possible insertable screw length in the SCS when using the following techniques: pedicle, lateral mass (Magerl's) and "Attallah" screws. Scans of 66 cervical spines were analyzed to determine the maximum possible screw lengths of all three screw insertion techniques, based on the standard description through the vertebrae from C3 to C7. The maximum possible length of the Attallah screw of 20.7 ± 2.5 mm (mean value ± SD) is only 2.4 mm shorter than the pedicle screw (23.1 ± 1.8 mm) along the SCS. The lateral mass screw is with 10.2 ± 1.3 mm full 12.9 mm shorter than the pedicle screw. The maximum possible length of the Attallah screw is close to that of the pedicle screw and significantly greater than that of the lateral mass screw. Summary: Background: Based on a previous clinical case report in which the pedicled subcostal artery perforator flap allowed for the closure of a large defect of the lumbar region, the present study was designed to investigate the anatomy of the subcostal artery perforator flap and to evaluate its potential for wider clinical use. Methods: A series of 14 human cadavers was studied and 28 subcostal artery perforator flaps were dissected. The location of the perforator vessel was charted against anatomical landmarks. Measurements included the perforator calibre, pedicle length, and flap size following methylene blue injection. The findings were compared by Doppler sonography in 15 volunteers. Results: The subcostal artery perforator was present in all dissected specimens and in all volunteers. Its calibre measured in mean 2mm. The location was constant at the lateral border of the latissimus dorsi muscle and between 1 and 3cm below the lower rib end. The pedicle length reached a mean of 10.5cm when dissected up to the border of the erector spinae musculature. The vascular supply covered a mean flap size of 10×14cm. The in vivo investigations confirmed the constant perforator location from the anatomical landmarks. Conclusion: This anatomical study reveals a considerable potential for the clinical use of the subcostal artery perforator flap for defect coverage in the lumbar area, due to its constant and reliable anatomy. Knowledge of the anatomical dimensions of distinct areas of the vertebral bodies and vertebral arches of the subaxial cervical spine are indispensable in the planning of osteosynthesis. The minute dimensions and complex anatomical orientation of the posterior vertebral arch structures in the subaxial spine render the insertion of screws - where needed - a challenging procedure. The pedicle option carries the risk of serious complications while the alternative of lateral mass only permits the insertion of short screws. The transverse process of the subaxial vertebrae offers unique possibilities on all counts and seems quite comparable in its dimensions with the pedicle. To our knowledge it has not been used previously for the insertion of screws in the subaxial spine. Therefore, the scans of 66 cervical spines were analysed for distinct lines of both structures. The widths and lengths of the dorsal part of the transverse processes and of the pedicles are similar between both sides. Clear differences between females and males could be observed. The widths of both structures were closer to each other in C3 than in C4 to C7, while the lengths derived the most in C7. The dorsal part of the transverse process might be suitable for the insertion of screws to stabilize the vertebral arch of the subaxial cervical spine. Gender adaptation might be required. Background Context: A linear relation exists between the insertional torque when placing a screw and the ultimate screw pullout strength; therefore, undertapping of the pedicle followed by insertion of a larger screw may enhance the pullout strength of the screw. Purpose: To report the risk of pedicle wall breech with placement of 6.2-mm pedicle screws after undertapping with a 5.2-mm tap and to evaluate the effectiveness of direct visualization to identify these breeches. STUDY DESIGN/SETTINGS: Prospective evaluation of risks of pedicle wall breech with undertapping for a larger screw and the utility of directly visualizing pedicle walls during access and placement of pedicle screws for evidence of breech. Patient Sample: Sixty-five consecutive patients. Outcome Measures: Intraoperative direct visualization of pedicle wall breech. Postoperative radicular leg symptoms. Methods: We prospectively analyzed placement of pedicle screws in sixty-five consecutive patients during lumbar-instrumented fusions after laminectomies performed by a single spine surgeon. Forty-four patients were primary fusions, and 21 were revisions. Twenty-seven were male, and 38 were female, with a mean age of 54.7 years (range, 15-85 years). Four hundred twenty-eight total pedicle screws were placed for an average of 6.6 screws per patient (range, 2-12 screws). Results: The incidence of pedicle wall breech during access was 0.7% (3/428) and 1.6% (7/428) after screw placement. 71% (5/7) of the screw breeches were noted with direct inspection of the pedicles intraoperatively, and 29% (2/7) were identified on postoperative X-ray films as lateral breakthroughs that were missed on intraoperative fluoroscopy and direct visualization. Therefore, 100% of misplaced screws potentially harmful to nerve roots were identified on direct visualization. Eighty percent occurred from L2-4. Conclusions: This study provides evidence that undertapping of pedicles followed by placement of a larger diameter pedicle screw may breech the pedicle cortices. Although the risk is low, the authors recommend direct visualization of the pedicle walls during screw placement to identify caudad, medial, and cephalad breeches. Fingertip amputations through the distal 50% of the nail matrix usually create a finger that is best served by maintaining length. When the amputation is more proximal than this, revision with ablation of the small residual nail unit may provide greater overall patient satisfaction. When amputations in the distal 50% of the nail matrix are transverse or short oblique in orientation, a flap of some kind is needed to restore the contact surface of the pulp. Many alternative reconstructive strategies are possible but have inherent limitations that detract from the quality of the final result. Inadequate padding, lack of sensibility, excessive tension, hook nail deformity, poor durability, and donor site morbidity are but some of the considerations. Although not a panacea for all distal fingertip amputations, the antegrade-flow homodigital neurovascular pedicle flap offers a good ratio of advantages to disadvantages. Abstract: Background context: Degenerative lumbar spinal stenosis manifests primarily after the sixth decade of life as a result of facet hypertrophy and degenerative disc disease. Congenital stenosis, on the other hand, presents earlier in age with similar clinical findings but with multilevel involvement and fewer degenerative changes. These patients may have subtle anatomic variations of the lumbar spine that may increase the likelihood of thecal sac compression. However, to the authors'' knowledge, no quantitative studies have addressed various radiographic parameters of symptomatic, congenitally stenotic individuals to normal subjects. Purpose: To radiographically quantify and compare the anatomy of the lumbar spine in symptomatic, congenitally stenotic individuals to age- and sex-matched, asymptomatic, nonstenotic controlled individuals. Study design/setting: A prospective, control-matched, cohort radiographic analysis. Patient sample: Axial and sagittal magnetic resonance imaging (MRI) and lateral, lumbar, plain radiographs of 20 surgically treated patients who were given a clinical diagnosis of congenital lumbar stenosis by the senior author were randomized with images of 20, asymptomatic age- and sex-matched subjects. Outcome measures: MRIs and lateral, lumbar, plain radiographs were independently quantitatively assessed by two individuals. Measurements obtained from the axial MRIs included: midline anterior-posterior (AP) vertebral body diameter, vertebral body width, midline AP canal diameter, canal width, spinal canal cross-sectional area, pedicle length, and pedicle width. From the sagittal MRIs, the following measurements were calculated: AP vertebral body diameter, vertebral body height, and AP canal diameter at the mid-vertebral level. On the lateral, lumbar, plain radiograph (L3 level), the AP diameters of the vertebral body spinal canal were measured. Methods: The images of these 40 individuals were then randomized and distributed in a blinded fashion to five separate spine surgeons who graded the presence and severity of congenital stenosis utilizing a five-tier scale. Images consisting of 15 symptomatic individuals, graded definitely congenitally stenotic (mean age, 51.7 years; range, 43–65 years), and 15 asymptomatic individuals, graded definitely not stenotic (mean age, 50.7 years; range, 41–55 years), were age- and sex-matched and included for further review. From these 30 patients, a lateral, lumbar, plain radiograph and axial and sagittal MRIs (T1/T2 weighted) from L2-L5 were quantitatively analyzed. Rater reliability was assessed by Kappa coefficient testing. Results: The cross-sectional area of the canal was significantly smaller in the congenitally stenotic patients at all lumbar levels measured (L2: 176 mm2 vs. 259 mm2, L3: 177 mm2 vs. 275 mm2, L4: 183 mm2 vs. 283 mm2, L5: 213 mm2 vs. 323 mm2, p<.05). Pedicle length was markedly shorter in the stenosis group at each lumbar level (L2: 5.9 mm vs. 8.9 mm, L3: 6.0 mm vs. 8.8 mm, L4: 6.5 mm vs. 9.2 mm, L5: 5.8 mm vs. 9.1 mm, p<.05). Furthermore, midline, axial AP canal diameter, vertebral body width, and sagittal AP canal diameter were all significantly smaller than the control patients (p<.05). A ratio of the AP diameter of the pedicle length to the vertebral body was also noted to be statistically significant on both the lateral plain radiographs (L3: 0.426 vs. 0.704) and sagittal MRI (L2: 0.343 vs. 0.461, L3: 0.361 vs. 0.461, L4: 0.362 vs. 0.481, L5: 0.354 vs. 0.452, p<.05). No difference was noted comparing the AP diameter of the vertebral body (axial and sagittal images), vertebral body height, canal width, and pedicle width. Kappa testing coefficient indicated a strong rater reliability (k=0.81, 95% confidence interval: 0.62–0.94). Conclusions: Congenital lumbar stenosis has not been clearly defined radiographically. Clinically, congenitally stenotic patients present at a younger age with fewer degenerative changes and multiple levels of involvement. Radiographically, these patients have a shorter pedicular length and as a result a smaller cross-sectional spinal canal area (mean critical values of 6.5 mm and 213 mm2 were observed, respectively). The mean critical ratios were 0.43 (2:1 AP vertebral body: pedicle length) on the lateral lumbar radiograph and 0.36 on the sagittal MRI. The altered canal anatomy resulting from a decreased pedicle length may anatomically predispose these patients to earlier complaints of symptomatic neurogenic claudication. Background: One use of the anteroposterior lumber radiograph is to determine axial (y-axis) rotation of the lumbar vertebrae. Rotation might be an element of interest to clinicians seeking to evaluate vertebral positioning. Objectives: Correlate and quantify movements of the projected pedicles relative to the projected vertebral body during axial rotation and determine if vertebral asymmetry and changes in object film distance affect these movements. Design: A three-dimensional computer model of the fourth and fifth lumbar vertebrae, a modeled radiograph source, and a modeled film were produced. The vertebral model was placed in various degrees of axial rotation at a number of different object film distances. Lines from the source were passed through the pedicles of the fourth lumbar vertebral model and additional lines erected tangent to the lateral body margins. These lines were extended to points of contact with the modeled film. Results: The projected pedicles move relative to the projected vertebral body during y-axis rotation. Vertebral asymmetry and object film distances can also affect the distance of the projected pedicle relative to the projected lateral body margin. Conclusion: Axial rotation produces movement of the projected pedicles relative to the projected vertebral body. However, vertebral asymmetry and change sin object film distance also affect the position of the projected pedicles relative to the projected lateral body margin and might serve as confounders to the clinician seeking to analyze vertebral rotation through the use of the projected pedicles. : Background contextConventional transpedicular decompression of the neural canal requires a considerable amount of lamina, facet joint and pedicle resection. The authors assumed that it would be possible to remove the retropulsed bone fragment by carving the pedicle with a high-speed drill without destroying the vertebral elements contributing to spinal stabilization. In this way, surgical treatment of unstable burst fractures can be performed less invasively.: PurposeThe purpose of this study is to demonstrate both the possibility of neural canal decompression through a transpedicular approach without removing the posterior vertebral elements, which contribute to spinal stabilization, and the adequacy of posterior stabilization of severe vertebral deformities after burst fractures.: Study designTwenty-eight consecutive patients with complete or incomplete neurological deficits as a result of the thoracolumbar burst fractures were included in this study. All patients had severe spinal canal compromise (mean, 59.53%±14.92) and loss of vertebral body height (mean, 45.14%±7.19). Each patient was investigated for neural canal compromise, degree of kyphosis at fracture level and fusion after operation by computed tomography and direct roentgenograms taken preoperatively, early postoperatively and late postoperatively. The neurological condition of the patients was recorded in the early and late postoperative period according to Benzel-Larson grading systems. The outcome of the study was evaluated with regard to the adequate neural canal decompression, fusion and reoperation percents and neurological improvement.: MethodsModified transpedicular approach includes drilling the pedicle for removal of retropulsed bone fragment under surgical microscope without damaging the anatomic continuity of posterior column. Stabilization with pedicle screw fixation and posterior fusion with otogenous bone chips were done after this decompression procedure at all 28 patients included in this study.: ResultsTwenty-three of 28 patients showed neurological improvement. The percent of ambulatory patients was 71.4% 6 months after the operation. The major complications included pseudarthrosis in five patients (17.8%), epidural hematoma in one (3.5%) and inadequate decompression in one (3.5%). These patients were reoperated on by means of an anterior approach. Of the five pseudarthrosis cases, two were the result of infection.: ConclusionAlthough anterior vertebrectomy and fusion is generally recommended for burst fractures causing canal compromise, in these patients adequate neural canal decompression can also be achieved by a modified transpedicular approach less invasively. Abstract: We report a 58 year old man who presented with severe C7 radiculopathy which failed to respond to conservative measures. Subsequent CT and MR imaging of his cervical spine demonstrated C6/7 foraminal stenosis as well as unusual low take-off of the C7 nerve root in relation to a congenital low-set C7 pedicle, findings which were subsequently confirmed intra-operatively. The relevance of the bony and neural anatomy is described and its implications for surgical management are discussed. Summary: Background: The angiogenic induction property of the omentum makes it a promising pedicle for prefabricating flaps. Therefore, the objective of this paper is to establish the abdominal area to be prefabricated by the omental pedicle and to analyse the prefabricated potential (PP) according to the time delay between the pedicle introduction and the flap release. Methods: Forty-four rabbits were divided into four groups (A, B, C and D). In group A, a piece of skin, subcutaneous tissue and abdominal cutaneous muscle was fully released and sutured again in place. In the other groups, a 9-cm2 omental pedicle containing the gastromental vessels distally tied was transposed and sutured to abdominal cutaneous muscle. A second procedure, consisting of incision and release of the flap that contained skin, subcutaneous and cutaneous abdominal muscle pediculated only by the omentum, was carried out. The only variation was the time delay between the two procedures: 7, 21 and 56 days for groups B, C and D, respectively. The flaps were inspected 15 days after the last procedure. The pieces of viable area were immunostained using anti-CD31 for estimation of the microvascular density. Results: The mean and maximum viable areas in group D were 45.29 and 99.37cm2, respectively (average PP=5.03 and maximum PP=11.04). There was no significant difference between the viable areas in groups C and D. The mean microvascular densities of groups B, C and D were 24.54, 33.20 and 27.03 vessels/mm2, respectively. Conclusion: The omental tissue has great potential for prefabrication of flaps, and the delay time for the second procedure should be at least 21 days. [Copyright &y& Elsevier]
