Infected fracture nonunions often require prolonged treatment and recovery courses. It is unclear whether the bacterial microbiome influences the time to healing as well as the eradication of infection. The goals of this study are (1) to assess the bacterial microbiome affecting infected nonunions and (2) to evaluate the effects of bacterial speciation on associated outcomes. Between 2006 and 2022, data from 551 adult patients from a single academic institution who presented with a fracture nonunion were analyzed retrospectively for infection. All patients underwent revision surgery with three sets of cultures obtained intra-operatively. Patients with significant intra-operative cultures were grouped into gram-positive and gram-negative culture cohorts. These patients were managed with a standardized protocol involving surgical debridement, nonunion site fixation, and culture-directed antibiotic treatment. Primary outcome was time to fracture union. Secondary outcomes included number of re-operations and eventual amputation or reconstructive surgery. 56 nonunion patients (10 %) were diagnosed with an infected nonunion (44 g-positive, 12 g-negative). Of these, 3 g-positive patients received an amputation or arthroplasty procedure prior to fracture union, and seven were lost to follow-up. There were no significant differences in age, gender, or nonunion site between cohorts. Most nonunions occurred in the lower extremity. The most common bacteria were staph species (54.3 %). 36 g-positive and 10 g-negative patients achieved fracture union. Time to union was on average 158.4 days longer in the gram-negative cohort—but did not reach statistical significance (446.8 days gram-positive, 662.3 days gram-negative, p = 0.69). There was no difference in re-operation rates (1.9 % gram-positive, 2.2 % gram-negative, p = 0.84). Patients with infected nonunions had wide-ranging bacterial contamination that were treated successfully using a standardized protocol. Optimizing the orthopaedic trauma patient for surgical intervention presents unique challenges. Due to the acute nature of musculoskeletal injuries and time constraints for treatment, there is a limited window for overall patient comorbidity optimization prior to surgical fracture repair. Certain social behaviors including tobacco use, excessive alcohol consumption, and inappropriate opioid medication usage can have a negative impact on wound and fracture healing. Curbing such negative social behaviors can have a positive outcome on patients who are affected by musculoskeletal trauma. Addressing these challenging issues, via available resources, can have a significant impact on recovery after sustaining orthopaedic trauma. A tactical approach focused on relationship building with patients and their families as well as highlighting strategies to curb negative social behaviors is presented involving a patient with an infected tibial nonunion. In this retrospective case series, we aimed to study arthroscopic sesamoidectomy, including surgical methods, clinical outcomes, and complications. We retrospectively reviewed the medical records of patients with hallux sesamoid disorders who underwent arthroscopic sesamoidectomy from July 2015 to July 2017. The visual analog scale for rating pain, Japanese Society for Surgery of the Foot scale scores, number of days taken to return to normal daily living, number of months taken to return to playing sports, and complications were analyzed. The mean follow-up duration was 3.2 years. Among 14 patients, there were 8 females and 6 males (median age: 39 years). The mean visual analog scale score improved from 75.4 to 14.3 mm; the mean Japanese Society for Surgery of the Foot score improved from 55.2 to 88.0 points. The mean time to return to activities of daily living was 5.3 days. Among the 5 patients who played sports, 3 and 2 patients resumed playing sports at 3 and 12 postoperative months, respectively. Complications included wound pain (n = 4), discomfort in severe weather (n = 4), numbness (n = 3), pain in the sole other than in the ball of the foot (n = 2), pain during hallux dorsiflexion (n = 2), residual sesamoid discomfort (n = 1), swelling (n = 1), toe-in gait (n = 1), and metatarsal head bone marrow edema (n = 1). Aim: Although nonunions are among the most common complications after long-bone fracture fixation, the definition of fracture nonunion remains controversial and varies widely. The aim of this study was to identify the definitions and diagnostic criteria used in the scientific literature to describe nonunions after long-bone fractures.Methods: A comprehensive literature search was performed in PubMed, Cochrane Library, Web of Science, and Embase. Prospective clinical studies, in which adult long-bone fracture nonunions were investigated as main subject, were included in this analysis. Data on nonunion definitions described in each study were extracted and collected in a database.Results: Although 148 studies met the inclusion criteria, only 50% (74/148) provided a definition for their main study subject. Nonunion was defined in these studies based on time-related criteria in 85% (63/74), on radiographic criteria in 62% (46/74), and on clinical criteria in 45% (33/74). A combination of clinical, radiographic and time-related criteria for definition was found in 38% (28/74). The time interval between fracture and the time point when authors defined an unhealed fracture as a nonunion showed considerable heterogeneity, ranging from three to twelve months.Conclusion: In the current orthopaedic literature, we found a lack of consensus with regard to the definition of long-bone nonunions. Without valid and reliable definition criteria for nonunion, standardization of diagnostic and treatment algorithms as well as the comparison of clinical studies remains problematic. The purpose of this study is to evaluate the ability of preoperative and postoperative radiographic union scores for tibia fractures (RUST) to predict treatment success of tibia fracture nonunion. Patients presenting for operative treatment of tibia fracture nonunion were enrolled in a prospective data registry. Enrolled patients were followed at regular intervals for 12 months. Preoperative and 12 week postoperative radiographs were reviewed and scored using the RUST criteria. Postoperative time to union was determined by clinical and radiographic measures. Multivariate regressions were conducted to predict time to union using preoperative and postoperative RUST while controlling for treatment method. Receiver operating characteristic (ROC) curve was conducted to determine the accuracy of preoperative RUST in predicting failure of treatment. Sixty-eight patients with aseptic tibia fracture nonunion treated operatively were identified. Sixty-one patients achieved union. Mean preoperative RUST was 7.5 (SD 1.4). Mean postoperative RUST was 9.2 (SD 1.4). Multivariate linear regressions demonstrated that preoperative (p = 0.043) and postoperative (p = 0.007) RUST are significant predictors of time to union after tibia fracture nonunion surgery. ROC curve demonstrated preoperative RUST below 7 was a good predictor of developing persistent tibia fracture nonunion (AUC = 0.83, Sensitivity = 1.000, Specificity = 0.745). RUST preoperatively and postoperatively predicts outcome after nonunion surgery. RUST can be used as part of the complete clinical picture to shape patient expectations and guide treatment. Purpose: The purpose of this study was to determine the effect of regional anaesthesia as compared to general anaesthesia on clinical, functional, and radiographic outcomes following long bone fracture nonunion repair.Methods: 262 patients who underwent operative repair of a long bone fracture nonunion and had at least 12 months of post-operative follow up were included in this study. Functional outcomes were assessed prospectively using the Short Musculoskeletal Function Assessment (SMFA) and Visual Analog Scale (VAS) pain scores prior to nonunion repair and at routine intervals post-operatively. Patients were divided into two matched groups based upon the type of anaesthetic method used in surgery. The regional anaesthesia cohort was composed of all patients who received regional anaesthesia (spinal anaesthesia or peripheral nerve block) alone or in addition to general anaesthesia, while patients who received general anaesthesia alone made up the general anaesthesia cohort. Univariate and multivariate analyses were performed to examine the effect of anaesthesia type on functional outcome scores, post-operative pain, bony healing, and complication rate.Results: The regional anaesthesia and general anaesthesia cohorts each consisted of 131 patients. Multiple linear regression demonstrated there to be no significant association between anaesthetic method and total SMFA scores at all post-operative time points. Additionally, anaesthetic method was not associated with post-operative VAS pain scores, time to union, or the rate of post-operative complications.Conclusion: In this cohort, the use of regional anaesthesia during operative repair of long bone fracture nonunion was associated with no significant difference in functional outcome scores or pain levels at all post-operative time points. Furthermore, the use of regional anaesthesia had no effect on the rate of post-operative complications. Either type of anaesthetic appears to be safe and effective in performing these surgeries. Introduction Much has been studied with reference to methicillin resistant Staphylococcus aureus (MRSA) and methicillin sensitive S. aureus (MSSA) colonization and associated outcomes and comorbidities. In the area of Orthopedic surgery, literature predominantly comes from the field of arthroplasty. Little is known about outcomes of fracture and Orthopedic trauma patients in the setting of S. aureus colonization. We believe that MRSA/MSSA colonization in and of itself may be a weak marker for generally poor protoplasm, potentially with complex medical history including previous hospitalization or rehab placement. This milieu of risk factors may or may not contribute to poorer outcomes after fracture and fracture nonunion surgery. The purpose of this study is to determine if nasal swabbing for S. aureus (MRSA or MSSA) carriage can predict operative culture, complications, or outcomes following fracture nonunion surgery. Methods Sixty-two consecutive patients undergoing surgery for fracture nonunion were prospectively followed. Data analyses were performed using grouped MRSA and MSSA carriers ( Staphylococcus carriers: SC). Outcomes analyzed included time to healing, need for additional surgery, and persistent nonunion. Results Twenty-six percent of patients (16/62) were identified as MSSA carriers, an additional 6.5% (4/62) carried MRSA. Follow-up of at least 12-months was obtained on 90% (56/62) of patients. White blood cell counts, erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) values did not differ between SCs and non-carriers pre-operatively. Carriers were just as likely as non-carriers to culture positively for any pathogen at the time of surgery. Although SC’s were three times as likely as non-carriers to grow S. aureus (15% vs. 5%), this difference did not reach statistical significance (p = 0.3). Post-operative wound complications, antibiotic use, pain at follow-up and progression to healing did not differ between groups. Conclusions Ultimately, pre-operative nasal swabbing for S. aureus is a simple and non-invasive diagnostic tool with prognostic implications in patients undergoing fracture nonunion surgery. This study found that MRSA and MSSA colonized patients with fracture nonunion of long bones do not have an increased association with positive cultures or a predisposition towards greater post-operative infectious complications. Data collected included pain scores measured by the Visual Analog Scale (VAS), clinical outcomes assessed by the Short Musculoskeletal Functional Assessment (SMFA), and range of motion. 75% of patients in this study had lower extremity fracture nonunions and 25% had upper extremity fracture nonunions. Femur fracture nonunions were the most common. Data at latest follow-up was compared to one-year follow-up using the independent t -test. : Sixty-two patients were available for follow-up data at an average of eight years. There were no differences in patient reported outcomes between one and eight years according to the standardized total SMFA (p = 0.982), functional index SMFA (p = 0.186), bothersome index SMFA (p = 0.396), activity index SMFA (p = 0.788), emotional index SMFA (p = 0.923), or mobility index SMFA (p = 0.649). There was also no difference in reported pain (p = 0.534). Range of motion data was collected for patients who followed up in clinic for an average of eight years after their surgical treatment. 58% of these patients reported a slight increase in range of motion at an average of eight years. : Patient functional outcomes, range of motion, and reported pain all normalize after one year following surgical treatment for fracture nonunion and do not change significantly at an average of eight years. We have used the principles of extracorporeal shock wave therapy (ESWT) in the treatment of nonunion of fractures in 44 patients (49 bones).There were 35 males and 9 females with a mean age of 34 years(range14-70). Clinical and radiological assessment was performed at regular time intervals with a minimum follow up of 18 months. Most common sites involved were the femur and tibia. The average time from initial fracture treatment to intervention with ESWT was 11.9 months (6 months to 5 years). Thirty eight non-union sites had one session of treatment and the rest (11) had more than one session. Union was successful in 75.5% of cases at a mean time of 10.2 months (range 3-19). Failure in the remaining cases was due to more than 5mm gap, instability, compromised vascularity (type of bone) and deep low grade infection; which was discovered at the time of surgical intervention when no signs of radiological healing occurred after 6 months from treatment. Abstract: Background: Transverse fractures through the body of the axis, rather than at the base of the odontoid are uncommon and management with an external orthosis is usually recommended. Oblique fractures through the body of the axis accompanying a hangman''s fracture have not been reported and are not described as part of any classification system. Such fractures may be at high risk for treatment failure in an external orthosis. Case description: We report on a case of an oblique axis fracture that failed treatment with external orthosis. Posterior instrumented fusion was employed successfully using a C1–C3 and C4 poly axial screw rod construct. Frameless stereotaxy and a biomodel were useful surgical adjuncts. Twelve month follow up revealed bony union in an asymptomatic patient. Conclusions: Oblique fractures of the body of the axis can displace in a halo-thoracic orthosis. Serial radiological review is required to detect displacement prior to fracture union. Oblique fractures of the body of the axis can be managed surgically with preservation of atlanto-occipital motion, resulting in satisfactory clinical and radiological outcomes. Insufficiency type stress fractures are common in older patients with osteoporosis. Persistent pain after nonunion of these fractures can be disabling, with the management options often limited. We aimed to assess the suitability of fluoroscopic-guided injection of bone cement into a persistently symptomatic nonuniting calcaneal insufficiency fracture. To the best of our knowledge, this technique has not previously been described in the published data. After local subcutaneous anesthesia, the midpoint of the fracture site was accessed by trocar insertion under radiographic guidance, and bone cement was injected directly into the site. A preprocedure visual analog scale pain score of 90 of 100 was recorded. This had improved to 0 of 100 at the 12-month follow-up point after the procedure. The aim of the present case report was to raise awareness of percutaneous calcaneoplasty, which we believe to be a safe and well-tolerated technique for the management of osteoporotic insufficiency fracture of the calcaneus. Purpose: Scaphoid fractures have the highest prevalence of nonunion in the human body, but despite this, little is known about the microstructure of the bone. Given that microstructural changes in the bone at the site of nonunion define the nonunion, it was our purpose to characterize the trabecular microarchitecture on both sides of the nonunion and compare these to the normal structure. Methods: Fifteen human scaphoid nonunions (14 men, 1 woman; average age, 29 years; interval between injury and examination ranged from 7 to 156 months) were excised during surgical reconstruction and assessed by high-resolution micro–computed tomography. Results: The bone from the nonunions was clearly different from that of normal scaphoids (obtained from nonreplantable hand amputations or postmortem). Bone on both sides of the nonunions was characterized by higher bone mineral density and an increased number of trabeculae, which were thinner and more tightly packed together compared with the normal scaphoid bones. Further evaluation showed that the bone on the proximal side of the nonunion had higher bone mineral density, higher bone volume fraction, decreased bone surface area, increased trabecular thickness, and increased trabecular number when compared with the distal side. Conclusions: These data demonstrate the differing microstructure of the bone at the scaphoid nonunion, particularly on the proximal side, and indicate that the biology also differs from normal. The data also show that scaphoid nonunions produce new bone at the site, which may have positive implications toward hardware fixation and healing stimulation. [Copyright &y& Elsevier]

Purpose: To describe the intraosseous arterial anatomy of the proximal ulna. Methods: We used 9 fresh-frozen, above-elbow amputations to map the intraosseous arterial supply using the Spalteholtz technique. We treated 3 additional above-elbow amputations similarly, and then dissected them to map the extraosseous arterial supply. We recorded the extraosseous arterial patterns and compared them with the intraosseous arterial findings. Results: The intraosseous arterial supply to the proximal ulna is derived primarily from a large nutrient vessel, a branch of the ulna artery, entering the anterior cortex of the ulna distal to the coronoid base, and from 2 medium-sized branches of the posterior and medial arcade entering near the olecranon tip. A watershed area is seen between these 2 vessel groups, halfway between the tips of the olecranon and coronoid. Conclusions: The arterial supply of the proximal ulna is relatively consistent and flows in opposite directions from the 2 separate areas. Clinical relevance: Knowledge of the arterial anatomy of the proximal ulna may assist the surgeon in minimizing surgical trauma, which may lead to improved healing after fracture fixations, osteotomies, and other surgical procedures around the elbow. Introduction The aim of this study was to explore the predisposing factors for fracture nonunion after a lateral screw was combined with C2 pedicle/laminar screw for a type II odontoid fracture and hopefully provide references in decision making and surgical planning for spinal surgeons. Methods This is a retrospective study. By retrieving the medical records from January 2010 to July 2015 in our hospital, 117 type II odontoid fracture patients were reviewed. According to the occurrence of fracture union at the final follow-up, patients were divided into 2 groups: union and nonunion. To investigate the predisposing factors for fracture nonunion, 3 categorized factors were analyzed statistically: patient characteristics—age, sex, body mass index, preoperative Japanese Orthopaedic Association (JOA) scores, duration, comorbidity, and complicated injuries; surgical variables—surgery time, blood loss, C2 fixation manner, vertebral artery injury, bone source for fusion between the posterior arch of C1 and the laminae and spinous process of C2; radiographic parameters—preoperative and immediate postoperative data of C 0-2 curvature, C 2-7 curvature, C 2-7 sagittal vertical axis, C7 slope, fracture classification, congenital hypoplastic vertebral artery, and the separation and displacement of the odontoid fracture. Other variables including JOA and visual analog scale scores for neck pain, neck stiffness, and patient satisfaction at final follow-up were recorded and compared between the 2 groups. Results Postoperative fracture nonunion was detected in 76 of 117 patients (65%) at final follow-up. There was no statistically significant difference between the 2 groups in patient characteristics of sex, body mass index, JOA score, comorbidity, and complicated injuries. The mean age at operation was younger in the union group than in the nonunion group, and the mean duration was shorter in the union group than in the nonunion group. There was no difference in surgical variables of surgery time, blood loss, C2 fixation manner, vertebral artery injury, bone source for fusion between the posterior arch of C1 and the laminae and spinous process of C2. There was no difference in radiographic parameters of fracture classification, congenital hypoplastic vertebral artery, preoperative and immediate postoperative C 0-2 curvature, C 2-7 curvature, C 2-7 SVA, and C7 slope. No difference was found in preoperative and immediate postoperative displacement of the odontoid fracture or immediate postoperative separation of the odontoid fracture, while the preoperative separation of the odontoid fracture was shorter in the union group than in the nonunion group. The logistic regression analysis revealed that advanced age (>45 years), long duration (>2 months), and preoperative separation of the odontoid fracture (>4 mm) were independently associated with the postoperative fracture nonunion. There were no differences between the 2 groups in JOA, neck pain, neck stiffness, and patient satisfaction at final follow-up. Conclusions Advanced age, long duration, and preoperative separation of odontoid fracture >4 mm are predisposing factors for fracture nonunion after posterior C1 lateral screw combined with C2 pedicle/laminar screw fixation for type II odontoid fracture. Our findings did not demonstrate any evidence of lower functional outcome and patients satisfaction for those patients who had odontoid nonunion. [ABSTRACT FROM AUTHOR]
