Purpose: The technical success and safety of percutaneous inferior vena cava filters (IVCF) retrieval when filter elements are penetrating into adjacent bony structures is unknown. Therefore, our purpose was to evaluate the technical success and safety of IVCF retrieval when filter elements are penetrating into adjacent bone.Materials and Methods: Using percutaneous IVCF retrievals from 2008 to 2018 in adult patients, we conducted a multi-institutional, retrospective review of filters found to penetrate lumbar vertebrae by computed tomography scans. Technical success following the retrieval procedure was recorded. Patient records were assessed for complications from retrieval.Results: 13 patients (2 males; 11 females; mean age: 53.2 years (range: 22-71) were included. Hundred percent of the filters were optional (retrievable) filters. Venous thromboembolism (VTE) with a contraindication to anticoagulation (n = 7; 53.8%) and prophylaxis without venous thromboembolism (n = 3; 23.1%) were the 2 most common indications for IVCF placement. Two filters (15.4%) had >15° of tilt relative to the IVC prior to retrieval while no IVCFs had migrated or caused caval thrombosis. One filter (7.7%) was found to have a fractured strut prior to retrieval. Ten patients (76.9%) were either on anticoagulants or antiplatelet medications at the time of retrieval. No patients were on antibiotics and no patients received periprocedural antibiotics. Median dwell time was 116.5 days (range: 49-5395). All 13 IVCFs were successfully retrieved in a single session (technical success: 100%). Standard snare technique was used in 8 cases (61.5%), endobronchial forces were used in 3 cases (23.1%), and wire loop snare technique was used in 2 cases (15.4%). Abstract: Earlier exploratory work on a scoring algorithm for the Reasons for Drinking Questionnaire [Zywiak, W. H., Westerberg, V. S., Connors, G. J., & Maisto, S. A. (2003). Exploratory findings from the Reasons for Drinking Questionnaire. Journal of Substance Abuse Treatment, 25, 287–292.] presented a number of interesting findings, but was limited by a fairly low consistency in type of relapse between the first relapse and the second relapse (63%). This scoring algorithm objectively classifies alcohol relapses into one of three types (negative affect, social pressure, or craving/cued). While examining gender differences in the type of first relapse, evidence indicating that relapses were more consistent for men (81%) than for women (44%) was uncovered. For initial posttreatment relapses, women were more likely to have negative affect relapses, and men were more likely to have social pressure relapses. For men, negative affect relapses were predicted by the Beck Depression Inventory score. For women, negative affect relapses were predicted by the Alcohol Dependence Scale score, and craving/cued relapses were predicted by situational craving. [Copyright &y& Elsevier]

Thoracic organ transplantation from Hepatitis C (HCV) viremic donors is a promising strategy due to curative therapies for HCV. Currently, there is no consensus on the best time to initiate HCV therapy relative to time of transplantation. We assessed the difference in magnitude of viremia and time to clearance in recipients of heart (HT) and lung (LT) transplant, based on timing of starting antiviral HCV therapy. From January 2018 to October 2019, 42 patients received thoracic organs from viremic donors. All recipients were treated with Mavyret (glecaprevir/pibrentasvir) for 8 weeks. HT recipients received therapy at the time of detectable viremia, while LT recipients were preemptively treated within 3 days post-transplant. HCV viral load was monitored by RT-PCR. 23 patients received HT (mean age 59 ± 9 years) and 19 patients received LT (mean age 60 ± 9 years). HCV serologic testing was performed in HT recipients at a mean of 7 ± 1 days and in LT recipients at a mean of 4 ± 3 days post-transplant. At the time of testing, all HT and 14 LT patients had detectable viremia. Five LT patients never developed viremia. The mean viral load f HT was 4.5 logIU/mL and for LT was 1.6 logIU/ml. Viremia clearance was obtained at a mean of 28 ± 13 days in HT and 21±11 days in LT recipients (p=0.13) (Fig). The mean time to HCV antibody (AB) clearance was 130 ± 145 days in HT and 225 ± 103 days in LT recipients (p=0.058). There was no correlation between the 2 groups in either the duration of viremia or HCV AB clearance. Our study suggests that the magnitude and conversion to detectable viremia depends on the time of initiation of HCV therapy relative to time of transplant with complete conversion to HCV viremia in the HT group. Interestingly, there was no significance in time to viremia or HCV AB clearance between the two groups.
